Abstract:Objective To summarize the epidemiological features,diagnosis and treatment of the penile fractures in single clinical center in recent 25 years so as to improve the treatment level.Methods A retrospective case series study was conducted on 23 patients with penile fractures from January 1993 to June 2017.The average age of patients was 37.78 years (range,21-66 years).Nineteen patients were married and four were not.Fourteen patients had penile fractures in summer,five in winter,three in spring and one in autumn.Three patients had Bachelor's degree or above,while 20 had less than university education.There were nine urban and 14 rural residents.Nineteen patients were caused by violent intercourse,two by violent masturbation,one by oral sex and one without willing explanation.All patients were unilateral rupture of albuginea penis,including five with urethral rupture.No misdiagnosis or missed diagnosis occurred.The time from injury to surgery was average 32 h (1993-2002),10.5 h (2003-2012),and 3.8 h (2013-2017),respectively.Emergency operations were performed using the degloved distal foreskin to repair the albuginea penis.The penile erectile function,urination and complications were recorded.Results All patients were followed up for 6-62 months (mean,26 months).In all patients,morning erection was restored at days 1 to 3 after operation,and normal urination was attained after the removal of urethral catheter.Normal sexual life was restored within 3 months after the operation.Penis elephantiasis combined with poor postoperative skin healing was seen in one patient,and mild penile erection pain was reported by one patient.No obvious erectile dysfunction,abnormal urination or other severe complications were reported.Conclusions The epidemiological features of penile fractures include low incidence,relatively concentrated region and age of onset,diversity of causes,and patients with insufficient education.Penile fractures can be diagnosed based on medical history and body examination.Emergency surgery is the first choice for penile fracture,with satisfactory outcomes and few postoperative complications.

Abstract:Objective To evaluate the etiology, diagnosis and treatment of penile fracture. Methods 21 patients with penile fracture were retrospectively reviewed. Results The main causes of penile fracture were excessive force at coitus and masturbation. 12 cases were diagnosed by ultrasonography and 1 case by cavernosography. Disrupt of tunica albuginea was easily and accurately found with alongitudinal incision. 13 cases were operated in 12 hrs after injury and 3 cases were operated after 12 hrs. 5 cases were conservatively treated. 2 cases who were operated in 12 hrs after injury were complicated with tunica albugineous scleroma, but their sexual functions were normal. 1 case who was operated after 12 hrs and 2 cases who were conservatively treated were complicated with tunica albugineous scleroma, penile angulation deformity and astyphia.Conclusion The main causes of penile fracture were excessive force at coitus and masturbation. Ultrasonography was the first option for diagnosing penile fracture. Operation on early stage could reduce complications. Disrupt of tunica albuginea was easily and accurately found with alongitudinal incision which had less trauma and was suited for clinic practice.
Surgical Outcomes in Penile Fractures: A Single Center Experience in China

Abstract:Introduction Penile fracture is an uncommon urological emergency resulting from tunica albuginea rupture during penile erection. It is a rare condition requiring urgent surgery. Despite immediate surgical repair, the patients' erectile functions may still be impacted by penile fracture. This study aims to investigate the efficacy of surgical repair in penile fractures and its impact on erectile function. Methods Our cohort was composed of patients diagnosed with penile fractures and received surgical repair from September 2014 to August 2022 in Peking University First Hospital. Penile color Doppler ultrasound confirmed the diagnosis. Surgical exploration was conducted, and postoperative complications were evaluated during follow-up. Erectile function was assessed using the International Index of Erectile Function-5 (IIEF-5) score. Univariate analysis was conducted employing the chi-square test, t test, and Mann-Whitney U test to identify factors that may impact postoperative erectile function. Furthermore, multivariate analysis was conducted using logistic regression and linear regression to determine the independent risk factors influencing postoperative erectile function. Results A total of 58 patients were enrolled in our study. The majority of injuries (69.0%, 40/58) resulted from vigorous sexual intercourse. Most of the patients (69.0%, 40/58) presented within 24 hours. Sixteen patients (27.6%) presented with concomitant urethral injury. The median size of the tunical tear was 1.5 (IQR, 1.0–2.0) cm. Presentation delay correlated significantly with the difference in IIEF-5 score before and after surgery, with corresponding p values of 0.028. Urethral injury correlated significantly with postoperative erectile dysfunction (ED), postoperative IIEF-5 score, and the difference in IIEF-5 score before and after surgery, with corresponding p values of 0.002, 0.004, and 0.002, respectively. Conclusions To conclude, surgical repair of penile fracture provides good functional results with few morbidities and urethral injury may adversely affect postoperative erectile function after penile fracture repair.
A new plastic surgical technique for adult congenital webbed penis

Abstract:Objective To introduce a novel surgical technique for correction of adult congenital webbed penis. Methods From March 2010 to December 2011, 12 patients (age range: 14–23 years old) were diagnosed as having a webbed penis and underwent a new surgical procedure designed by us. Results All cases were treated successfully without severe complication. The operation time ranged from 20 min to 1 h. The average bleeding volume was less than 50 ml. All patients achieved satisfactory cosmetic results after surgery. The penile curvature disappeared in all cases and all patients remained well after 1 to 3 months of follow-up. Conclusions: Adult webbed penis with complaints of discomfort or psychological pressure due to a poor profile should be indicators for surgery. Good corrective surgery should expose the glans and coronal sulcus, match the penile skin length to the penile shaft length dorsally and ventrally, and provide a normal penoscrotal junction. Our new technique is a safe and effective method for the correction of adult webbed penis, which produces satisfactory results.

Abstract:How to understand the erectile function? How to define erectile dysfunction? In general, erectile function is evaluated by the quality of penile erection. If penis shows good erection, we think erectile function is normal. Otherwise, erectile dysfunction occurs. This concept is reflected in the definition of erectile dysfunction (ED). ED is defined as the persistent inability to attain and maintain an erection sufficient to permit satisfactory sexual performance. From the definition of ED, it is comprehensible that the penile erection is only related to the hardness of penile erection. Therefore, if the hardness of penile erection is sufficient to complete a satisfactory sexual intercourse, we could consider the erectile function is normal. Conversely, if the hardness of penile erection is not enough, erectile dysfunction is indicated. However, sometimes we meet the following kinds of patients in clinics. Firstly, the patients who are diagnosed as phallocampsis. The morphology of penis is flexural after erection: upward bending, down bending, left bending, right bending, S-shaped curve or rotating, et al. Secondly, in some patients, proximal part of penis is much more slender than glans, like a spindle. Full erection could be achieved in these patients without any abnormalities indicated by nocturnal penile tumescence (NPT) test, intracavernous pressure (ICP) measurement and color duplex ultrasonography. But the penis is very easy to swing during the sexual intercourse and the stability of penile erection is abnormal.

Abstract:The clinical data of 16 patients with rupture of corpus cavernosum penis were retrospectively reviewed.The patients were referred to the Department of Urology from January 2005 to November 2009.The causes of penile injury were trauma due to sexual intercourse ( 12 cases), trauma due to masturbatory (2 cases), traffic accidents ( 1 case) and others ( l case).All patients received immediate surgical treatment; 14 out of 16 patients were followed up for a mean period of 3 months.Sexual function was recovered with satisfactory erection in 11 patients, 3 patients complained pain or uncomfortab]eness during erection.Sexual trauma was the main cause of penile fracture.Immediate intervention for penile fracture can achieve satisfactory early and late results.

Abstract:Introduction: In the management of penile fractures, immediate surgical repair has resulted in better outcomes when compared with a conservative approach; however, there is currently no consensus on the treatment of patients presenting beyond the immediate period (>24 hours) following injury. Objectives: To examine the latest literature on management strategies in penile fracture and propose an optimal algorithm for the treatment of patients with delayed presentation. Methods: A comprehensive search was conducted following the PRISMA-P 2020 guidelines. A search was performed in several databases with the following strategy: ("Penile fracture" OR "fracture of penis" OR "rupture of corpora cavernosa" OR "rupture of tunica albuginea") AND (management OR treatment OR surgery OR "surgical reconstruction" OR "surgical repair"). This resulted in 108 relevant articles. Two independent reviewers screened these articles according to the inclusion criteria. Full-text review of 56 articles was performed, and ultimately 20 studies were selected. Measures included the use of diagnostic imaging, timing of surgical repair (immediate, <24 hours after injury; delayed, >24 hours), surgical approach, and long-term complications (ie, erectile dysfunction and penile curvature). Results: The review highlighted the benefits of immediate surgical repair in penile fractures, demonstrating improved patient outcomes. Furthermore, it found that surgical repair should be considered even in cases with delayed presentation (>24 hours after injury). To better evaluate the long-term impact of delayed surgical intervention on patient outcomes, we recommend standardized postoperative follow-up, with routine assessments of erectile function and penile curvature. Conclusion: Contemporary literature suggests that immediate and delayed surgical repair of penile fractures leads to adequate postoperative outcomes, and patients presenting >24 hours after injury should still be considered for surgery.

Abstract:Objectives: The primary objective is to compare the imaging and surgical findings in a cohort of patients with suspected penile fracture (PF). Methods: Retrospective cohort study of all patients with suspected PF over an 11-year period at a tertiary referral andrology centre. All dedicated presurgical imaging with ultrasound (US) and MRI was analysed and correlated with intraoperative findings; alternative diagnoses were recorded. Results: 193 patients were included. 104 (54%) had alternative diagnoses to PF including dorsal vein rupture and haematoma. 99 (51%) underwent surgical exploration of which 89 (46%) had PF.US correctly confirmed the presence and marked site of fracture in 92% of cases. MRI was primarily used as a problem-solving tool (13 cases) and demonstrated a more extensive injury than US (12 cases). The reported size of tunical defect on imaging was a median of 7 mm (IQR 4-10) significantly smaller than on exploration, (median 20 mm, IQR 10-30) p < 0.0001. Conclusion: US has a high positive predictive value in the confirmation of penile fracture. MRI improves the detection and characterising the extent of injury. Imaging marking informs surgical incision but defect size is under appreciated on all imaging modalities. Advances in knowledge: Penile imaging has a high positive predictive value to not only confirm the diagnosis of PF but to stage the extent of injury and mark the skin, which impacts the surgical technique. Alternative diagnoses to fracture are common and imaging could prevent unnecessary surgical exploration.

Abstract:Key clinical message: Penile fracture is a urological emergency that requires prompt surgical intervention for optimal outcomes. Early diagnosis based on clinical presentation and imaging if needed is crucial. Timely repair ensures the preservation of penile function and minimizes complications. Abstract: Penile fracture (PF) is a rare but significant urological emergency characterized by the rupture of the tunica albuginea, often resulting from blunt trauma during sexual intercourse or other activities. The typical presentation includes sudden and severe penile pain, an audible "cracking" sound followed by swelling and deformity. While PF generally does not involve the urethra, bleeding from the urethral meatus suggests an associated urethral lesion. Imaging such as ultrasonography or magnetic resonance imaging (MRI) can assess the extent of the injury. We report a case of a 28-year-old male who presented with sudden-onset penile pain, swelling, and blood dripping from the urethral meatus after a traumatic sexual encounter. The injury occurred during vigorous intercourse when his erect penis struck his partner's buttocks. He heard a cracking sound followed by immediate detumescence, penile curvature, and urethral bleeding. At the hospital, the patient exhibited tachycardia and elevated blood pressure. Examination revealed deviation of the penis to the right, urethrorrhagia, and a tender, swollen penis. A clinical diagnosis of bilateral corpora cavernosa injury and partial rupture of the penile urethra was made. A urethral catheter was placed with difficulty, draining initially reddish urine followed by clear urine. Laboratory tests showed normal hemoglobin and coagulation profiles with mild bacteriuria and hematuria. Due to clear clinical signs, no radiological imaging was performed before surgery. Surgical exploration performed 48 h postinjury revealed a PF involving the right corpus cavernosum and a complete rupture of the spongy urethra. The corpus cavernosum was repaired using Vicryl 2-0 sutures followed by urethroplasty with Vicryl 5-0. Postoperative care included antibiotics and analgesics. The patient had an uneventful recovery with preserved penile sensitivity and function. PF is a rare urological emergency that necessitates prompt diagnosis and management to prevent long-term complications. Although clinical presentation often suffices for diagnosis, imaging can be valuable in ambiguous cases. Surgical intervention is the preferred treatment, offering the best functional and cosmetic outcomes. This case underscores the importance of timely surgical repair to ensure favorable recovery and preservation of penile function.

Abstract:Introduction Penile fracture refers to the rupture of tunica albuginea of the penile corpora cavernosum which most often occur due to trauma during coitus or manual manipulation. This can be associated with either a partial or a complete urethral injury up to 39% of cases, usually those fractures crossing the septum. It is important to address both conditions to ensure sexual and voiding function is maintained. Objective The patient presented in this video is a 35-year-old gentleman who presented to the emergency department post-trauma related to sexual intercourse causing immediate detumescence and swelling. Methods This video describes the penile fracture repair performed using a midline ventral incision repair technique. Prior to surgery the patient is imaged, usually ultrasound, to allow location and marking of the suspected penile fracture. Due to the suspected urethral injury requiring a suprapubic catheter insertion at the presenting hospital, a flexible cystoscopy was performed initially. This showed the urethra was inaccessible at the level of the penile scrotal junction but within the penile urethra. A ventral midline incision along the penile raphe over the suspected fracture site is made. Dartos and Bucks Fascia were dissected to expose the tunica albuginea and allow identification of the fracture site. Further dissection was required to determine the lateral extents of the injury. Stay sutures can then be placed to allow easier closure of the injury. The tunica albuginea was then sutured with PDS sutures, care was taken to ensure these are buried sutures to ensure the patient is unable to palpate these. Subsequently, urethral injury was addressed by performing primary urethral anastomosis. A catheter was introduced through the urethra, a complete transection was seen and the ends were then dissected out to allow an end to end anastomosis to be performed. The anastomosis was checked for a leak with a table urethrogram using dilute betadine. Upon adequate closure of the urethra the ventral midline incision was closed. Results The procedure was successful and uneventful. He has a urethrogram 3 weeks post-op on which no evidence of a leak was shown. This patient went on to have good erectile function at 2 months post-operatively. However, he has a 30-degree ventral curvature which he finds bothersome. This is currently being treated conservatively with a vacuum device. Conclusions The BAUS consensus documents reports erectile dysfunction and penile curvature rates following surgery for penile fracture repair are reported to be 5% and 5% respectively. Conservative management results have a much higher rate of complication with 62% chance of erectile dysfunction and 35% chance of curvature. Disclosure No.

Abstract:Introduction Penile fracture is traditionally considered a surgical emergency warranting immediate repair with the goal to maximize long-term erectile function and minimize penile curvature. Nonetheless, consensus on the optimal timing for penile fracture repair remains to be elucidated and is the subject of continued research efforts. Objectives This review aims to summarize the contemporary literature pertaining to optimal timing of penile fracture repair and associated outcomes. Methods We queried PubMed/MEDLINE and Google Scholar for relevant articles published between 2012 and 2022 to evaluate the most recent literature on the queried topic of early vs delayed intervention for penile fracture. All examined review articles were published within the last decade but may have included analyses of studies published prior to 2012. Reference lists of articles and reviews were manually reviewed to identify additional relevant articles. Results We identified 16 articles that met inclusion criteria: 12 primary articles and 4 systematic reviews or meta-analyses. Importantly, definitions of early and delayed intervention varied greatly among studies, making quantitative comparison challenging. In summary, 6 primary studies and 2 systematic review articles favored early intervention. There were also 6 primary studies and 2 systematic review articles suggesting equivocal outcomes between early and delayed repair. No articles demonstrated improved outcomes with delayed repair relative to early intervention. Conclusion Surgical intervention for penile fracture remains the gold standard, with superior long-term sexual and functional outcomes when compared with conservative management. Optimal timing of penile fracture repair remains to be elucidated with data limited by low incidence, resulting in small case series and a lack of randomized controlled trials. Nonetheless, recent data suggest that a brief delay in surgical intervention for patients presenting with penile fractures does not affect long-term sexual and functional outcomes.

Abstract:Introduction Penile fracture is not an uncommon occurrence. It is a serious injury involving the rupture of the tunica albuginea in the erect state of the penis. While most cases are transverse fractures occurring during coitus, we present a rare case of a "vertical" penile fracture, the second reported in English literature. Objective To document a rare case of penile fracture i.e. a vertical penile fracture and its management. Methods A 33-year-old man presented to the emergency room of the hospital past midnight. He experienced sharp excruciating pain while attempting to penetrate vagina in a rear entry position. He gave a history of popping sound followed by detumescence, and increasing swelling of penis with discoloration and curvature. The pain continued though reduced in severity over time. The patient was evaluated under the guidance of consultant urologist (Author 2). Magnetic resonance imaging reported suspected vertical tear in tunica albugenia and also suspected urethral injury. Results Emergency surgery revealed a vertical long tunica albugenia tear along with corpora spongiosal tear and intact urethral mucosa. The tunica albugenia tear was primarily repaired. Good erectile function was reported by the patient, and normal sexual activity resumed after 6 weeks. Conclusions This case report emphasizes the need for clinicians to be aware of and consider this uncommon entity, "Vertical Tunica Albugenia tear" while managing penile fractures. The utilization of MRI imaging of the penis can provide valuable insights into the pattern and extent of the injury, aiding in appropriate management decision. Surgical intervention within 6–8 hours of the injury offers excellent result and preserves sexual function without complications. Disclosure No.

Abstract:Penile crural fractures caused by traumatic injuries are rare urological emergencies similar to urethral bulb injuries. This case report discusses the findings of a 59-year-old patient who presented to our emergency department 12 h after an electric bicycle accident. Clinical examination revealed an elevated body temperature, bruised perineal skin, tender penis, and swollen scrotum. Imaging confirmed a penile fracture at the bilateral crus of the penis without considerable urethral trauma. The patient underwent conservative treatment and was followed up on an outpatient basis for approximately three months. Consequently, the patient is relieved of penile pain, has regained erectile function, and reports a satisfactory sexual life. Moreover, this study discusses the efficacy of conservative treatment combined with outpatient follow-up in managing bilateral penile crural fractures.

Abstract:A penile fracture is a rare but urgent urologic emergency that requires immediate surgical correction to preserve erectile function. It results from a rupture of the tunica albuginea of one or both corpora cavernosa. Patients classically will present with a history of recent sexual activity and a sudden "snap," followed by detumescence, pain, and swelling of the phallus. The rupture of the tunica albuginea typically happens ventrolaterally along the penile shaft, as the tunica albuginea thins at this location, especially during erections. In this report, we present a case of a penile fracture with a rupture of the tunica albuginea at an unusual location in the distal phallus between the corpora cavernosa and corpora spongiosum.
(150) Erectile Dysfunction and Peyronie’s Disease Diagnosis Rates After Penile Fracture – A Retrospective Claims Database Cohort Analysis

Abstract:Abstract Introduction Penile fracture (PF) is a rare urologic emergency, with an estimated incidence of 500-600 PFs per year in the United States. Delays in surgical repair increase the risk of developing erectile dysfunction (ED) and Peyronie’s disease (PD). Objective Our objective was to analyze the rates of ED and PD following a PF using a large, multi-institutional claims database. Methods We conducted a cohort analysis utilizing the TriNetX Research database. The inclusion criteria included men ages 15 or older with a diagnosis of PF and any office visit within 5 years of the PF. Our exclusion criteria included any prior ED diagnosis, prescription of erectile aids, or penile prosthesis placement. Our primary outcome was the diagnosis of ED or prescription of erectile-aid dosing of phosphodiesterase-5 inhibitors (PDE5i) within five years. A secondary analysis was used to assess rates of PD following a PF. Subgroup analyses were performed for age and the impact of initial repair. Propensity-matching was performed to compare men with and without PF by calculating risk ratios (RR). Results 1,242 men were identified with a diagnosis of PF and subsequently matched to men without a PF, resulting in equal cohorts of 1,227 men. Men with a history of PF were more likely to receive a diagnosis of ED or require PDE5i (RR 3.18, 95% CI 2.30-4.40). Men over the age of 45 years who had a PF were more likely to develop ED or require ED treatment compared to men under 45 years (RR: 1.65, 95% CI 1.14-2.39). Men who did not undergo immediate repair had higher rates of ED diagnoses or treatment (RR: 1.84, 95% CI 1.22-2.78). Rates of PD diagnoses were significantly higher in men with a history of PF (5.8% vs 0%, p-value<0.0001). Rates of PD were significantly lower if immediate repair of the fracture was performed (RR: 0.20, 95% CI 0.10-0.41). Men over the age of 45 years with a history of a penile fracture were more likely to develop Peyronie’s Disease within 5 years compared to men under the age of 45 years with a history of penile fracture (RR: 3.73, 95% CI 1.94-7.16). Conclusions We found that a PF increases the risk of both ED and PD, especially for those treated with conservative measures or over the age of 45 years. Consequently, it is essential to follow up with these patients in the long term and survey potential adverse side effects relating to sexual function that may arise. Disclosure No.

Tri-tubular penile fracture: A case of complete rupture of urethra and bilateral corpus cavernosa

Abstract:Introduction: Tri-tubular penile fracture is a very rare subset of penile fractures, typically due to high-energy trauma to the erect penis. Case presentation: A 26-year-old healthy man presents with a triad of audible crack, immediate detumescence, and hematoma formation, following blunt trauma sustained during vaginal sexual intercourse. A diagnosis of penile fracture is made, with intraoperative finding of a complete traumatic transection of the urethra with total bilateral corpus cavernosa disruption. Both corpora cavernosa were repaired using 2/0 polydioxanone sutures, with the dartos fascia placed in between. The urethra was repaired with interrupted 5/0 polyglactin braided absorbable suture. At 5 weeks, patient reported normal erections and sexual activity with no physical or psychological issues. The literature on management was reviewed. Conclusion: Current evidence of surgical management of tri-tubular penile fractures remains predominantly expert opinion, due to difficulties of conducting prospective trials for a rare condition.

Abstract:Objective To evaluate the long-term outcome of albuginea penis repair for penile fracture and its influence on the erectile function. Methods Clinical data of 15 patients with penile fracture between July 1993 and January 2011 in our hospital were retrospectively analyzed. The long-term outcome of albuginea penis repair for penile fracture and its influence on the erectile function were evaluated using the International Index of Erectile Function (IIEF 5) questionnaire. Results The mean age of patients was 30.5±3.2 (ranging from 17~46 years). Of 15 patients, 13 patients (86.7%) had penile injury from sexual intercourse, one was from masturbation and one from penile trauma. Diagnosis of penile fracture was based on symptoms and ultrasonography findings. Thirteen patients (86.7%) underwent immediate surgical repair of the albuginea penis, whereas two (13.3%) patients with mild haematoma received conservative treatment. Thirteen patients(86.7%) showed unilateralcorporal injury without urethral injury. The mean time of follow-ups after repair was 3±1.9 years.(ranging from 1~7years). Only one patient had mild penile curvature(<15 degrees), the others had no severe complications. None of the 13 patients had erectile dysfunction. The mean IIEF 5 score for the 13 patients was 22.1±1.6, and there was no significant difference between before and after injury (23.1 ±2.6, P>0.05). Conclusion Immediate surgical repair of penile fracture showed a satisfied long-term outcome and it does not affect erectile function.
