Abstract:Aim To analyze the clinicopathologic and immunohistochemical features, diagnosis, differential diagnosis of primary melanocytoma of central nervous system. Methods Two cases of primary melanocytoma of CNS in 2007 and 2014 were collected and the clinicopathologic and immunohistochemical data were analyzed. Results Two patients presented without melanocytic tumour of the skin. The tumours were solitary and located in sella area and intraspinal canal with slow-growing. The duration of disease onset was 3 years and 10 years respectively. Generally the tumours were brown and solid, histologically slightly spindled or oval tumour cells containing variable melanin arranged in whorls, nests. Cytologic mitoses were absent (on average less than 1/10 HPFs). Immunohistochemical study showed that the two cases were positive for s-100, HMB45, vimentin and negative for cytokeratin, epithelial membrane antigen, GFAP. The staining for NSE were partly positive in one case. Conclusion Primary melanocytoma of CNS located in sella and lumbar spinal cord is rarer. It presents some pathological characteristics. Although slow-growing, it carries a recurrence rate and even has manignant change, warranting long-term follow up needed.

Abstract:Abstract OBJECTIVE: Medulloblastoma (MB) is a malignant embryonal tumor that develops especially in childhood, with overall survival (OS) at 5 years of up to 70%. The aim of the study is to explore the clinical profile and outcome among the histo-molecular subgroups in Chinese children. METHODS: A total of 93 patients of MB who underwent surgical treatment at Xinhua Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from January 2006 to December 2015. The clinical profile, treatment, and survival patterns are discussed. The relevant statistical analysis was done using SPSS software, version 21.0. RESULTS: At the most recent follow-up date, 15 (16.1%) had been lost to follow-up，78 cases were tested for RNA-seq, of which 54 cases were successful, and 24 cases failed the quality control due to sample quality problems.The male-to-female ratio was 2.4:1, and the patients’ mean age at diagnosis was 5 years (ranged from 4 months to 11 years).The estimated 5-year overall survival and progression-free survival, based on Kaplan-Meier analysis, were 55.1% ± 5.6% and 40.9% ± 5.7%.Multivariate survival analysis showed that progression -free survival were significantly affected by extent of surgery (P=0.026) and postoperative radiotherapy(P＜0.001).Preoperative metastasis (P=0.032) and postoperative radiotherapy (P=0.001) had a significant influence on overall survival. While，the molecular subtypes showed no statistically significant differences in 5-year PFS and 5-year OS. CONCLUSION: Extent of resection of MB and postoperative radiotherapy were the important clinical prognostic factors for survival. In the future, we will continue to expand cases and study the clinical characteristics and prognostic risk factors of different molecular types of medulloblastoma in Chinese children,and to improve the survival rate of children with medulloblastoma.

Abstract:Background Cerebellar liponeurocytoma is a rare benign neoplasm of the central nervous system, which arises mainly in adult patients with only 3 cases reported in children. Due to its rarity, the diagnosis and treatment strategies for cerebellar liponeurocytoma remain unclear. The purpose of this study was to explore the epidemiology, clinical features, imaging findings, pathological characteristics, different diagnoses, treatment, and prognosis of cerebellar liponeurocytoma in juveniles. Case Description A 5-year-old boy was admitted to the department of neurosurgery due to a 5-month history of headaches, nausea, vomiting, dizziness, dysphoria, as well as visual blurring associated with the peak of the headache. Magnetic resonance imaging showed a 4.9×5.4×6.2 cm mass located in the fourth ventricle and cerebellar vermis combined with hydrocephalus and periventricular edema. The mass was completely removed, and pathological examination indicated a cerebellar liponeurocytoma of the World Health Organization Grade II classification. Conclusion The present study was the first to report a cerebellar liponeurocytoma with total tumor resection and adjuvant radiotherapy in a pediatric patient. Total tumor resection and postoperative radiotherapy together with close and long-term follow-up seem to be the optimal treatment strategy for juvenile patients. However, the side-effect of radiation needs to be considered.

Congenital malignant melanoma- the unique entity of pediatric melanoma: two case reports and literature review

Abstract:Background: Pediatric malignant melanoma (MM) is categorized by the age as congenital, childhood and adolescent subtypes. Congenital melanoma is present at birth or in the first year of life. It is an exceedingly rare entity for the low incidence and difficult diagnosis. Here, we present two cases of congenital malignant melanoma. Besides, we describe the special clinicopathologic and molecular features of pediatric MM especially congenital sub-type by reviewing the literature. Case presentation: We report two cases of congenital malignant melanoma. One is a boy patient who had been born with a mass in the occipital scalp, the other newborn girl presented with a mass at birth on her right shoulder. Two patients all underwent the excision of the nodular lesion. Histological evaluation revealed the boy case is pigmented epithelioid melanocytoma (PEM), a unique low-grade variant of malignant melanoma of the skin. The other is congenital melanoma. After the operation, the boy patient presented with cervical lymph node metastasis on the third postoperative month, but is still alive and is developing well. The girl patient recovered, with no recurrence or metastasis since follow-up.Conclusions: Congenital melanoma is a rare sub-type of pediatric melanoma. Accurate diagnosis of congenital MM is challenging and requires the combination of clinical presentation, pathologic and molecular findings. The cases we offered will contribute to the understanding of epidemiological features and molecular biology of congenital MM and pediatric MM.
Familial medulloblastoma:case report of one family and review of literature

Abstract:Objective We report a case of familial medulloblastoma in one family and review the literature to discuss the possible pathogenesis.Method We describe the presentation of medulloblastom in two siblings and their two relatives showing male preponderance,3 male and 1 female.They had underwent resection of the posterior fossa masses due to intracranial hypertension.Resuits Histological sections of tumors revealed medulloblastoma.One patient died on the second day after operation,two died of the recurrence of the tumor,the rest was living well.Conclusions Familial medulloblastomas are rare.To our knowledge.only 11 case reports of familial medulloblastoma have been published.This article has maximum patients in one family of medulloblastoma,and the family is the second case involving members of different generations.The familial clustering of these tumors does suggest genetic susceptibility in these families,but the exact pathogenesis and genetics involved in familial medulloblastoma remain unclear.Further study is needed.

Abstract:Background:Medulloblastoma in adults is rare and treatment decisions are largely driven from pediatric literature. We sought to characterize recurrent medulloblastoma in adults.Methods:From a single-institution dataset of 200 adult patients diagnosed with medulloblastoma during 1978-2017, those with recurrence were analyzed for clinical features, treatment, and outcome.Results:Of the 200 patients, 82 (41%) with median age of 29 years (18-59) had recurrence after a median follow-up time of 8.4 years (95% CI = 7.1, 10.3). Of these, 30 (37%) were standard-risk, 31 (38%) were high-risk, and 21 (26%) had unknown-risk diseases at the time of initial diagnosis. Forty-eight (58%) presented with recurrence outside the posterior fossa, of whom 35 (43%) had distant recurrence only. Median Progression-free survival (PFS) and OS from initial surgery were 33.5 and 62.4 months, respectively. Neither PFS nor OS from initial diagnosis differed between the standard-risk and high-risk groups in those who experience recurrence (P = .505 and .463, respectively). Median OS from first recurrence was 20.3 months, also with no difference between the standard-risk and high-risk groups (P = .518). Recurrences were treated with combinations of re-resection (20 patients; 25%), systemic chemotherapy (61 patients; 76%), radiation (29 patients; 36%), stem cell transplant (6 patients; 8%), and intrathecal chemotherapy (4 patients; 5%). Patients who received radiation at recurrence had better OS (32.9 months) than those who did not (19.2 months) (P = .034).Conclusions:Recurrent medulloblastoma in adults has a poor prognosis irrespective of initial risk stratification. Recurrence commonly arises outside the posterior fossa years after initial diagnosis.

Abstract:Primary melanocytic tumors of central nerve system (CNS) are rare, primary diffuse leptomeningeal melanomatosis (PDLM), a subtype of malignant melanomas of CNS, is extremely rare,especially in pediatrics. As the clinical manifestation of PDLM is not characteristic, It is often misdiagnosed as tubercular meningitis and hemorrhage.A 13-year-old boy was admitted to our department with symptoms of recurrent headache and vomiting twice. As the brain imaging revealed a lesion located in the left temporal lobe mimicked hemorrhage, so there was a misdiagnosis of hemorrhage in first hospitalization. He was admitted again for the recurrence of the headache and vomiting. Detailed physical examination showed multiple melanin changes in the skin of the whole body which were ignored in last hospitalization. Brain imaging showed the significantly enlarged lesion in the left temporal lobe and several smaller lesions in the left parietal lobe and cerebellum which indicated metastasis.According to the history,physical examination and the radiological finding, the patient was diagnosed with malignant melanoma of central never system possibly.The patient underwent left temporal and parietal lesions total resection with a craniotomy.The diagnosis of PDLM was established according to pathological characteristics and the negative finding of positron emission tomography (PET)-computed tomography (CT) outside CNS. The patient got no further treatment for economic reasons and experienced the progression and died 5 months after operation.PDLM is extremely rare in CNS, as the clinical manifestation, radiological changes are not special, early diagnosis is difficult. The confirmed diagnosis is established by leptomeningeal biospy or surgical tissue. PET-CT can help differential diagnosis with metastastic leptomeningeal melanomas. The prognosis is dismal due to the inefficiency of chemotherapy or radiotherapy.

Abstract:Medulloblastoma (MB) is the most common childhood malignant brain tumor, accounting for approximately 20% of all pediatric central nervous systemtumors. Current standard treatments involving surgical interventions followed by craniospinal irradiation and adjuvant chemotherapy have severe motor and cognitive defects. Therefore, individualized treatment regimens with reduced toxicity designed according to the presence of specific oncogenic `driver' genes are urgently demanded. To this end, recent genetic and epigenetic findings have advanced the classification of MB into the international consensus of four distinct MB molecular subgroups (WNT, SHH, Group 3, and Group 4) based on their respective molecular and histopathological characteristics. More recent studies have indicated that up to seven molecular subgroups exist in childhood MB. Moreover, studies on the inter- and intra-tumoral features of the four subgroups revealed that each subgroup contains variant subtypes. These results have greatly helped risk stratification ofMB patients at diagnosis and significantly improved clinical treatment options. Herein, we highlight the recent advances and challenges associated with MB classification, and the development of therapeutic treatments targeting novel subgroup-specific molecular and epigenetic factors, especially those in the SHH-driven MB tumors. Abstract:Introduction: Medulloblastoma is the most common malignant brain tumor in children, often requiring intensive multimodal therapy, including chemotherapy with alkylating agents. However, therapy-related complications, such as therapy-related myeloid neoplasms (t-MNs), can arise, particularly in patients with genetic predisposition syndromes. This case report presents three pediatric cases of medulloblastoma with subsequent development of t-MNs, highlighting the potential role of genetic predisposition and the importance of surveillance for hematological abnormalities in long-term survivors. Case presentation: We describe three cases of pediatric medulloblastoma who developed t-MNs after receiving chemotherapy, including alkylating agents. Two of the patients had underlying genetic predisposition syndromes ( TP53 pathologic variants). The latency period between initial diagnosis of medulloblastoma and the development of secondary cancer varied among the cases, ranging from 17 to 65 months. The three cases eventually succumbed from secondary malignancy, therapy-related complications and progression of primary disease, respectively. Conclusions: This report highlights the potential association between genetic predisposition syndromes and the development of therapy-related myeloid neoplasms in pediatric medulloblastoma survivors. It underscores the importance of surveillance for hematological abnormalities among such patients.

Analysis of Clinical Prognosis and Risk Factors for Children with Medulloblastoma

Abstract:Objective To investigate prognosis and risk factors for children with medulloblastoma.Methods The clinical data of 92 children with medulloblastoma admitted to the Department of Neurosurgery,Beijing Tiantan Hospital,Capital Medical University from January 2003 to January 2007 were analyzed retrospectively.The tumor resection degree,imaging data,pathological type,radiotherapy,chemotherapy or not,and clinical prognosis of the children were investigated and summarized.Kaplan-Meier single factor analysis and Cox multivariate regression analysis were used to conduct statistical analysis for various influencing factors.Results Of the 92 children with medulloblastoma,27 were females and 65 were males.Their age ranged from 3 to 17.The follow-up time ranged from 6 to127 months (median followup time 84.0 months).Sixty children had classic medulloblastoma,24 were desmoplastic type,1 was large cell type,and 7 were anaplastic type.Nineteen children had tumor spread before procedure,12 had postoperative residual tumor ＞ 1.5 cm2,and 6 had both of them.The tumors of 81 children were located in the posterior fossa midline,53 of them infiltrated the base of the fourth ventricle,and 11 were located in one side of the cerebellar hemisphere or cerebellopontine angle.Regression analysis showed that the large cell/ anaplastic histological type and tumor spread were the independent risk factors for affecting the prognosis of the children (P ＜ 0.001).Conclusions The large cell/anaplastic histological type and tumor spread are the independent risk factors for affecting the prognosis of the children.The prognosis of the children is poor and needs to be treated with different treatment options.
Clinical characteristics of children with medulloblastoma

Abstract:Objective To investigate the clinical characteristics of children with medulloblastoma (MB). Methods The correlations amongst MB histopathological subtype,age at diagnosis,gender,primary tumor locations, relapsed tumor and relapsed tumor locations were analyzed retrospectively in 83 children who were diagnosed as MB by histopathology subtypes from February 2012 to April 2015 in Beijing Shijitan Hospital Affiliated to Capital Medical Uni-versity.The data was conducted by using SPSS 22.0 statistical software.Results Among the 83 cases (53 boys and 30 girls),there were 14 patients younger than 3 years old (9 boys and 5 girls)and 69 patients (44 boys and 25 girls)ol-der than 3 years old,including 28 relapsed (19 boys and 9 girls)and 55 non -relapsed cases (34 boys and 21 girls). The median age was 80.2 (13.1 -184.7)months at diagnosis.Of these 83 cases,48.2% (40 /83 cases)was classic medulloblastoma (CMB)(2 cases less than 3 years old),24.1 % (20 /83 cases)was desmoplastic /nodular medullo-blastoma (DMB)(6 cases less than 3 years old),12.1 % (10 /83 cases)was large cell/anaplastic medulloblastoma (LC /AMB)(1 case less than 3 years old),3.6% (3 /83 cases)was extensive nodular medulloblastoma (MBEN)(1 case less than 3 years old),and 12.1 % (10 /83 cases)(3 cases less than 3 years old)was mixed subtype.The rela-tionships between age at diagnosis and histopathological subtype,gender and primary tumor location were all statistically significant (χ2 =0.014,0.013,all P <0.05).Conclusions The incidence of boys with MB is higher than girls.CMB is the main histopathologic subtype in children over 3 years old.The primary tumor location involving the cerebellar vermis or cerebellar vermis and the fourth ventricle is higher in girls with MB.The primary tumor location involving the fourth ventricle,the fourth ventricle and other parts of the central nervous system,Cerebellar vermis and other parts of the central nervous system or other parts of the central nervous system is higher in boys with MB.

Abstract:Introduction and Patient Characteristics Medulloblastoma (MB) is the most common malignant brain tumor in children and the leading cause of cancerrelated mortality in children, and it comprises up to 1% of tumors in adults. Peak incidence occurs between the ages of 3 and 5 years, with a second spike in incidence between the ages of 20 and 40 years. This primary neuroectodermal tumor arises in the cerebellum (most commonly at the vermis), and 10% to 40% of cases disseminate through the cerebrospinal fluid (CSF) to the cerebral hemispheres or more commonly the spinal cord. In children, most cases present before age 10 years, and 75% are diagnosed before age 15 years. MB occurs twice as often in men and African-Americans; it is also associated with several neoplastic syndromes. Adult tumors are more often hemispheric than midline, and are more often cystic and necrotic with less-defined margins. In this review, the reader will learn about the epidemiology, presenting clinical characteristics, and neuroimaging characteristics of MB. Additionally, inheritance patterns, genetic syndromes, sporadic genomic alterations, and histological patterns for medulloblastoma will be reviewed.

Medulloblastoma in China: Clinicopathologic Analyses of SHH, WNT, and Non-SHH/WNT Molecular Subgroups Reveal Different Therapeutic Responses to Adjuvant Chemotherapy

Abstract:Medulloblastoma (MB) is one of the most common primary central nervous system tumors in children. Data is lacking of a large cohort of medulloblastoma patients in China. Also, our knowledge on the sensitivity of different molecular subgroups of MB to adjuvant radiation therapy (RT) or chemotherapy (CHT) is still limited. The authors performed a retrospective study of 173 medulloblastoma patients treated at two institutions from 2002 to 2011. Formalin-fixed paraffin embedded (FFPE) tissues were available in all the cases and sections were stained to classify histological and molecular subgroups. Univariate and multivariate analyses were used to investigate prognostic factors. Of 173 patients, there were 118 children and 55 adults, 112 males and 61 females. Estimated 5-year overall survival (OS) rates for all patients, children and adults were 52%, 48% and 63%, respectively. After multivariate analysis, postoperative primary radiation therapy (RT) and chemotherapy (CHT) were revealed as favorable prognostic factors influencing OS and EFS. Postoperative primary chemotherapy (CHT) was found significantly improving the survival of children (p<0.001) while it was not a significant prognostic factor for adult patients. Moreover, patients in WNT subtype had better OS (p = 0.028) than others (SHH and Non-SHH/WNT subtypes) given postoperative adjuvant therapies. Postoperative primary RT was found to be a strong prognostic factor influencing the survival in all histological and molecular subgroups (p<0.001). Postoperative primary CHT was found significantly to influence the survival of classic medulloblastoma (CMB) (OS p<0.001, EFS p<0.001), SHH subgroup (OS p = 0.020, EFS p = 0.049) and WNT subgroup (OS p = 0.003, EFS p = 0.016) but not in desmoplastic/nodular medulloblastoma (DMB) (OS p = 0.361, EFS p = 0.834) and Non-SHH/WNT subgroup (OS p = 0.127, EFS p = 0.055). Our study showed postoperative primary CHT significantly influence the survival of CMB, SHH subgroup and WNT subgroup but not in DMB and Non-SHH/WNT subgroup of MB.
Clinical characteristics and abandonment and outcome of treatment in 67 Chinese children with medulloblastoma

Abstract:OBJECTIVE The purpose of this study was to explore the clinical features and outcome of medulloblastoma in Chinese children. The authors analyze the reasons that treatment is abandoned and attempt to provide evidence-based recommendations for improving the prognosis of medulloblastoma in this population. METHODS A total of 67 pediatric cases of newly diagnosed medulloblastoma were included in this study. All of the children were treated at Xinhua Hospital between January 2007 and June 2013. The authors retrospectively analyzed the clinical data, treatment modalities, and outcome. The male-to-female ratio was 2:1, and the patients' median age at diagnosis was 51.96 months (range 3.96-168.24 months). The median duration of follow-up was 32 months (range 3-70 months). RESULTS At the most recent follow-up date, 31 patients (46%) were alive, 30 (45%) had died, and 6 (9%) had been lost to follow-up. The estimated 3-year overall survival and progression-free survival, based on Kaplan-Meier analysis, were 55.1% +/- 6.4% and 45.6% +/- 6.7%, respectively. Univariate analysis showed that standard-risk group (p = 0.009), postoperative radiotherapy (RI) combined with chemotherapy (p < 0.001), older age (>= 3 years) at diagnosis (p = 0.010), gross-total resection (p = 0.012), annual family income higher than $3000 (p = 0.033), and living in urban areas (p = 0.008) were favorable prognostic factors. Multivariate analysis revealed that postoperative RI combined with chemotherapy was an independent prognostic factor (p < 0.001). The treatment abandonment rate in this cohort was 31% (21 of 67 cases). CONCLUSIONS There was a large gap between the outcome of medulloblastoma in Chinese children and the outcome in Western children. Based on our data, treatment abandonment was the major cause of therapeutic failure. Parents' misunderstanding of medulloblastoma played a major role in abandonment, followed by financial and transportation difficulties. Establishment of multidisciplinary treatment teams could improve the prognosis of medulloblastoma in Chinese children.

Abstract:Gliosarcoma is a rare and highly malignant central nervous system tumor that accounts for 1%–8% of glioblastomas; it usually occurs in middle-aged and older adults between 40 and 60 years of age and is rare in children. We report an 11-year-old boy with right frontal lobe gliosarcoma who underwent aggressive gross total resection and postoperative radiotherapy, experienced recurrence and subsequently underwent a second operation. To better understand the disease and explore treatment options, we briefly report this case and review the relevant literature.

Abstract:BACKGROUND: Anaplastic lymphoma kinase-positive (ALK+) large B-cell lymphoma (LBCL) is a rare type of lymphoma with high invasiveness and rapid progression. It occurs in all age groups, but is extremely rare in children. The lesions mainly involve the lymph nodes and may present with extra-nodal involvement. Response to conventional chemotherapies and local radiotherapy is poor, with a 5-year overall survival of less than 40%. Recently, the use of ALK inhibitors for the treatment of this disease has been reported.CASE SUMMARY: We present a case of a 12-year-old boy diagnosed with ALK+LBCL. The patient had a 2-mo medical history of a calvarial mass, extensive systemic involvement, and positive bone marrow clathrin heavy chain (CLTC)-ALK fusion gene. Complete remission 1 (CR1) was achieved using the modified LMB89 Group C regimen followed by autologous stem cell transplantation. The patient relapsed 3 mo later. He then achieved CR2 with three short courses of chemotherapy (COP, reduced-dose ICE, low-dose Ara-c+VP16) and continuous alectinib targeted therapy. Afterward, allogeneic hematopoietic stem cell transplantation (allo-HSCT) was performed. At 16 mo after the allo-HSCT, the patient was still in CR2.CONCLUSION: The modified LMB89 Group C regimen and ALK inhibitors are effective. Allo-HSCT should be performed after remission.

Abstract:Medulloblastoma is an infratentorial primitive neuro-ectodermal tumour, which is commonly occurring in childhood. It often spreads through the cerebrospinal fluid pathways with a very low rate of systemic metastases. Extra-central nervous system metastases have a poor prognosis in medulloblastoma. Here, we reported a case of Group3 medulloblastoma with bone marrow and abdominal cavity metastasis. A 10-year-old girl complained of headache, dizziness, vomiting and nausea. Magnetic resonance imaging(MRI) of the brain showed a large mass in the fourth ventricle and obstructive hydrocephalus. The surgery was performed to remove the mass and ventriculoabdominnal shunt was performed for obstructive hydrocephalus. Histological verification was proven classic medulloblastoma and next-generation sequencing confirmed molecular group of Gruop3 with MYC amplication. Radiotherapy was subsequently performed one month after the surgery. During the radiotherapy, brain MRI showed metastasis on the surface of the brain, which confer a poor prognosis. After the radiotherapy According to the therapeutic schedule, She would accept chemotherapy one month after the radiotherapy. But She suffered severe bone marrow suppression manifested as anaemia and thrombocytopenia, which can not be improved by medication and blood transfusion. A bone marrow aspiration was subsequently performed, and bone marrow smear showed large numbers of tumor cells at high magnification and was lack of normal hematopoietic cells. The girl was deteriorating rapidly with jaundice, ascites, hepatosplenomegaly and pancreatitis after the bone marrow aspiration. We found a lot of tumor cells in the bleeding tendency ascites and finally confirmed bone marrow and abdominal cavity metastasis concurrently. Eventually, She died less than a month after the metastasis and the overall survival was only 6 months. This typical case demonstrated severe bone marrow suppression of unknown origin should caused alarm and be performed bone marrow aspiration as soon as possible. The prognosis of medulloblastoma with extra-central nervous system metastases is poor and people may die in months.

Abstract:[Objective] To olserve the treatment and prognosis of medulloblastoma in children.[Methods] 59 cases of medulloblastoma were examined by CT and MRI,the tumors were resected and certified postoperatively by pathological examination.[Results] 52 cases were totally resected,6 cases subtotally resected and 1 case partially resected.As a result,56 cases got good recovery,and 3 cases died after operation.[Conclusion] Getting total removal of tumor,relieving obstruction of cerebrospinal fluid circulation,combming with radiotherapy and other supplementary therapy can decrease the incidence of relapse,enhance patients' life quality and prolong survival period.
