Assessment of mildly coronary stenosis by intravascular ultrasound imaging

Abstract:[Objective] To investigate the lesion characteristics of angiographic mildly coronary stenosis lesion by intravascular ultrosound(IVUS) imaging. [Methods] 68 of 96 patients were angiographic mildly coronary stenosis(diameter stenosis 20%~70%). The other lesions which the percent diameter stenosis was more than 70% by the coronary angiography(CAG). All patients underwent the coronary angiography(CAG) and intravascular ultrasound(IVUS) imaging. The IVUS analysis included qualitative assessment of lesion and quantitative measurements of the external elastic membrane cross-sectional area(EEM CSA-MLA), minimum lumen diameter(MLA), plaque area(EEM CSA-LCSA), plaque burden, the thickness of fibrous cap, plaque volume, percent lipid area, remodeling index(RI), dissection and percent of thrombus. [Results] All patients had successively undergone the CAG and IVUS imaging. The mildly coronary stenosis had more soft plaque, eccentric plaque, thin of fibrous cap, positive remodeling, rupture plaque and less calcified plaque than that of severe stenosis(P <0.05). The lesion of hard and calcified plaques, non- or negative remodeling was mainly in the severe coronary stenosis(P <0.05). [Conclusion] IVUS was necessary in selected patients with the mildly coronary stenosis. It had the more sensitively and specially in the diagnosis of vulnerable plaques than CAG. IVUS was the important basis to decide whether the mildly coronary stenosis lesion had been accepted the intervention or not.
