Abstract:Background Respiratory therapists (RTs) play a vital role in healthcare, specializing in the treatment and management of heart and lung conditions, particularly during the COVID-19 pandemic. Despite the importance of RTs, little attention has been paid to RTs in the Chinese health system. This study aimed to understand and describe the work experiences and career development expectations of RTs in China. Methods This study utilized a qualitative research design and conducted semi-structured, in-depth, audio-recorded interviews with 16 RTs aged 28–40 years, purposively selected from six regions in mainland China from May to June 2024. Qualitative thematic content analysis was conducted to identify and group the themes that emerged from the discourse. Interviews were transcribed and analyzed using thematic analysis. Results Four key themes were identified: (1) “Complex Career Motivation” delves into the career trajectories and role adaptations of RTs; (2) “Positive Career Feedback” explores feelings of job satisfaction and career accomplishments; (3) “Uncertain Career Predicament” sheds light on the negative impact of multidimensional career challenges; (4) “Demand-Driven Future Outlook” reflects RTs’ urgent expectations for professional advancement. Conclusion The barriers and facilitators in the professional journey of Chinese RTs maintain a fragile balance, and the future development of the profession may determine whether they can persist in their careers. Healthcare managers and leaders should pay attention to the real needs of RTs, enhance their professional confidence, and adopt effective coping strategies to avoid the loss of human resources and promote the development of the professional team.

Practice Pattern Of Aerosol Therapy Among Patients Undergoing Mechanical Ventilation In Mainland China: A Web-Based Survey Involving 447 Hospitals

Abstract:Background and objectiveAerosol therapies are widely used for mechanically ventilated patients. However, the practice pattern of aerosol therapy in mainland China remains unknown. This study aimed to determine the current practice of aerosol therapy in mainland China.MethodsA web-based survey was conducted by the China Union of Respiratory Care (CURC) from August 2018 to January 2019. The survey was disseminated via Email or WeChat to members of CURC. A questionnaire comprising 16 questions related to hospital information and 12 questions related to the practice of aerosol therapy. Latent class analysis was employed to identify the distinct classes of aerosol therapy practice.Main resultsA total of 693 valid questionnaires were returned by respiratory care practitioners from 447 hospitals. Most of the practitioners used aerosol therapy for both invasive mechanical ventilation (90.8%) and non-invasive mechanical ventilation (91.3%). Practitioners from tertiary care centers were more likely to use aerosol therapy compared with those from non-tertiary care centers (91.9% vs. 85.4%, respectively; p = 0.035). The most commonly used drugs for aerosol therapy were bronchodilators (64.8%) followed by mucolytic agents (44.2%), topical corticosteroids (43.4%) and antibiotics (16.5%). The ultrasonic nebulizer (48.3%) was the most commonly used followed by the jet nebulizer (39.2%), the metered dose inhaler (15.4%) and the vibrating mesh nebulizer (14.6%). Six latent classes were identified via latent class analysis. Class 1 was characterized by the aggressive use of aerosol therapy without a standard protocol, while class 3 was characterized by the absence of aerosol therapy.ConclusionsSubstantial heterogeneity among institutions with regard to the use of aerosol therapy was noted. The implementation of aerosol therapy during mechanical ventilation was inconsistent in light of recent practice guidelines. Additional efforts by the CURC to improve the implementation of aerosol therapy in mainland China are warranted.

Abstract:Objective To clarifying the professional orientation of clinical radio-chemist (CRC) and radio-pharmacist (RP) in China,and to promote standardization of their education.Methods The legal status,responsibilities and competence requirements of CRC and RP are reviewed.Limitations of current educational system are also analyzed.Except that,suggestions for improvement are proposed.Results The number of these practitioners is insufficient now in China.Their academic background and professional skills are various and sometimes unsatisfying.The probable reasons are:1) professional orientation of CRC and RP recognized by academic societies,governmental administrations and medical institutions are actually not clear;2) the status quo of educational system for CRC and RP is far from perfect.The historical experience from European Union and United States is valuable as reference.Conclusions The survey of demand for CRC and RP practitioner,the improvement of legal system and educational system for CRC and RP,and more propaganda and communication in professionals should be in plan.Along with the progress of nuclear medicine,the variety of radio-pharmaceuticals involved in keeps growing and their formulations become more and more complex.The trend that clinical radio-chemical and radio-pharmaceutical service (including preparation,quality control and administration) are charged by professional CRC and RP is becoming more and more certain.CRC and RP will be increasingly important in nuclear medical team.

Abstract:Objective: To analyze the problems and give some advice to the clinical pharmacists' cultivation. Methods: The status of clinical pharmacy education in China was analyzed and discussed by using survey research and literature study. Results and Conclusions: We should construct clinical pharmacy education system, establish the concept of clinical pharmacy discipline, strengthen the teaching in practice, optimize teaching content, focus on the graduate education and post-graduate education, and actively seek the support of social policies and regulations.

Abstract:Abstract Background China launched its new round of health care reform to develop primary care in 2009, establishing 954,390 primary care institutions that employed over 10 million staff by 2019. However, some studies have shown that the prevention and management of respiratory diseases is inadequate in these institutions. Methods We conducted a cross-sectional survey of grassroots institutions throughout China between September and December 2020 based on the standardized Prevention and Treatment System and Capacity Building Project of Respiratory Diseases in primary care settings. The operation of the respiratory department in primary health care institutions was evaluated in terms of facilities, drugs, personnel and management of chronic diseases by means of questionnaires. Descriptive analyses were performed to calculate percentages and frequencies of key parameters. Results A total of 144 primary health care institutions were surveyed, including 51 in the east, 82 in the west, 9 in the central and 2 in the northeast. Approximately 60% of institutions had spirometers and pulse oximeters. The majority had short-acting bronchodilators, theophylline, systemic corticosteroids, antibiotics, and traditional Chinese medicine. More than half had at least one respiratory physician and operator for spirometry. Half of the institutions carried out screening of chronic obstructive pulmonary disease within the jurisdiction. The institutions in the east were superior to those in the west regarding the equipment, common drugs, medical staff, and management of respiratory diseases. Conclusions The study reveals that the overall operation of the respiratory department in primary care settings needs to be further strengthened. It is crucial to provide adequate essential equipment, medical professionals, and medicines for proper diagnosis and treatment of chronic respiratory diseases, as well as improving the management of diseases.

Abstract:The purpose of this article is to explore the training patterns of general/family physicians in China by comparing with the programs in other countries.Education of general/family medicine overseas is commonly involved in three parts: undergraduate education,residency training and continuing education.Education of general/family medicine has four ways in China .That is knowledge education of general/family medicine for undergraduates in medical college,postgraduate,retraining programs and continuous medical education.This article is to discuss some issues related to such training,that is the enrollment criteria,compensation mechanism,multiple levels and channels of the training,and the accreditation of general/family physicians.

Abstract:Horticultural therapy, originated at the end of 17th century，is an intercross subject including horticulture，medicine and psychology. The therapeutic effects of horticulture were recognized in the late 18th century. After the World War II, especially since the foundation of the American Horticultural Therapy Association, both research and application of horticultural therapy have developed greatly. Currently, famous organizations in this field includes the American Horticultural Therapy Association, Thrive and so on. Qualitative research methods were mainly adopted in the beginning, and then experimental statistical methods were added by modern researchers. Researches have confirmed the therapeutic effects of horticultural therapy, but its own theory has not formed. Presently, researchers mostly refer to theory of attention restoration and so on. The research of horticultural therapy in China started very late. Horticulture therapy is promising in China.

[The current status of the study on asthma in China]

Abstract:Purpose: To review the recent developments in asthma research in China, presenting the background of further asthma research and promoting academic exchanges between Chinese and foreign respirologists. Data sources: A literature search was conducted using 5 authoritative journals covering the years 1994-1998. Study selection: Sixty-seven of 238 asthma-remated original articles published between the years 1994-1998 were selected to reflect the recent developments in asthma in China. Data extraction: The papers reviewed pertain mainly to the epidemiology, genetics, pathogenesis, prevention and treatment of asthma. Results: Since 1993, the hot topics in asthma research in China have been pathogenesis, prevention and treatment. There has been relatively less investigation into the genetics of asthma in China. An epidemiological survey of asthma has been conducted in parts of China and was reviewed. Conclusions: More and more attention has been paid to the study of asthma during the last five years in China. The prevalence of asthma has increased in recent years. The study of the pathogenesis of asthmatic airway inflammation has made further progress, which will help the management and treatment of asthma. The treatments for asthma, including herbal medicine, have been explored and the clinical efficacy of inhaled corticosteroids has been established.

Abstract:With the development of human society,the needs of breathing apparatus for production safety and emergency rescue are stronger,chemical oxygen respirator has become an important research direction.The working principle,development history and technology situation of chemical oxygen respirator were introduced,then the advantages and disadvantages of the chemical oxygen respirator were analyzed and compared.On this basis,it was pointed out that optimizing the oxygen production device,developing the high efficiency cooling technology,reducing the overall volume and weight,making the technical standard and developing measurement technology are the important and urgent tasks of chemical oxygen respirator research.

Abstract:BACKGROUND:Pulmonary rehabilitation (PR) has demonstrated physiological, symptom reducing, psychosocial, and health care savings benefits in multiple outcome areas for patients with chronic respiratory diseases. Physicians' PR awareness and PR referral practices are key in PR promotion. However, PR awareness and referral among respiratory physicians in China have rarely been studied. This study aims to explore respiratory physicians' perceptions towards PR and assess the referral of PR in China.METHODS:A self-administered questionnaire was distributed via WeChat and emails to respiratory physicians in hospitals to assess their attitudes toward and knowledge of PR and identify treatment barriers. The study was conducted from June through October 2019.RESULTS:As reported in the 520 questionnaires collected through October 2019 most respondents had heard about PR, and many had knowledge of PR practice, but relatively few had referred patients to PR before having responded to the survey. Education, region of practice, and duration of practice are significant factors that influenced the participating respiratory physicians' awareness of PR. The percentage of referral was influenced by physicians' education, region, and duration of practice. The absence of PR facilities was the main barrier to respiratory physicians' referral of patients to PR.CONCLUSIONS:Chinese respiratory physicians' awareness of PR and referral to PR remain insufficient to support the delivery of PR to patients with chronic respiratory diseases. PR training for respiratory physicians and building PR centers are necessary to remedy these conditions.

Abstract:By analyzing the standard movements of the revised Standard of Technical Levels of Aerobic Athletes and based on a primary investigation into the current situation of the young aerobic athletes of China, we have learned that the Standard has been fully popularized within a short time. Yet, from the angle of cultivating young athletes, the lack of training grounds and coaches has become the most prominent issue to be solved. There is no aerobic class in primary and high schools. Analyzing the current situation of young aerobic athletes from several aspects, the paper puts forward the solutions.

Status quo of nursing for chronic respiratory disease in community health centers of Beijing Miyun District

Abstract:Objective:To investigate the status quo of nursing capacity for chronic respiratory disease in community health service centers.Methods:A questionnaire survey on nursing capacity for chronic respiratory disease was conducted in November 2018 among nurses working in internal medicine, general practice and chronic disease management departments of 4 community health service centers in Beijing Miyun District. The questionnaire included the knowledge and work content of chronic respiratory diseases. At the same time semi-structured interviews was conducted on the role of nurses in the management of chronic respiratory diseases among nurses selected by purposive sampling method.Results:In the study 63 valid questionnaires were collected and 14 nurses were interviewed. The results showed that the total score of nurses′ knowledge about chronic respiratory diseases was (62.0±14.4), and the scores of knowledge of chronic obstructive pulmonary disease (COPD), bronchial asthma and obstructive sleep apnea were (55.5±19.3), (61.0±21.6), (69.5±25.1), respectively. The total knowledge score of nurses with middle-rank professional title or above was higher than that of those with primary title [(67.0±12.9) vs. (54.0±13.6), t=-3.07, P<0.01]. The work contents mainly involved were nursing procedures(22/63,34.9%), health education(18/63,28.6%) and nursing evaluation(8/63,12.7%).Three themes were extracted from the deep interviews: the main work contents for nurses should be nursing procedures, nurses should participate in the chronic respiratory disease management as a member of the integrated medical team; nurses expected practice-oriented trainings of chronic respiratory disease management. Conclusions:The knowledge of chronic respiratory diseases among nurses in community health center needs to be improved, and the nursing skills special for respiratory diseases are needed. It is suggested to develop training courses and nursing guidelines suitable for community health centers and for nurses to participate in chronic respiratory disease management as team members, so that nurses in primary care institutions can play a better role in the management of chronic respiratory diseases.

Abstract:Standardized resident training is necessary for residents.The article focused on the establishment of standardized resident training model in the department of respiratory in Zhongshan Hospital of Fudan University.Zhongshan Hospital developed reasonable training plan,formulated strict evaluation and examination system and emphasized on cultivating practical,innovative and independent talents.Detailed measures included strengthening the basic knowledge and practical skills,developing independent learning ability,communication ability and innovative thinking and promoting medical ethics.All these experiences and explorations of standardized resident training in the department of respiratory will be beneficial.
