Abstract:Hospice service is a totally new medical model in the beginning in China. Donated by Mr. Li Ka Shing, the well-known patriotic personage from Hong Kong, the first terminal care institute-hospice canter, was set up in our hosp ital in 1998. Free of any charge, the center provides a series home-cared service to advanced cancer patients, including pain-killing treatment, psychological assistance, life ethics services and so on. After four years running of the center, we deeply realized that the hospice service can not only improve the life qualities of terminal patients, but also benefits a lot to the patients and the society at the same time, thus achieving great social rewards.

Abstract:Palliative and Hospice Care initially developed in Great Britain. It was founded by Cicely Saunders for the humane care of terminally ill cancer patients and her institution was established as the first modern hospice in England. The concept developed gradually into a systematic medical program for terminal ill patients and was subsequently been generalised worldwide. The palliative and hospice care model currently in Taiwan of China is characterised by the joint participation of the government and society. This experience and knowledge is expected to contribute to the system of palliative and hospice care in the process of being established in the mainland of China.

Abstract:Hospice and palliative care have been demonstrated to improve quality of life, shorten hospital stays, and save costs. As a developing country, faced with a rapidly aging population and an increasing number of patients with life-threatening illnesses such as cancer, China has made great progress in promoting hospice and palliative care during the past few decades. A trained nurse can play a significant role in promoting quality hospice and palliative care by providing care, coordinating a multidisciplinary team, calling for policy support, and raising public awareness. It is critical for nurses to clearly understand the development and challenges that currently exist in the country. With this awareness, nurses will improve their ability to recognize and address patients' and families' needs, resulting in quality, dignified, and individualized end-of-life care.

Abstract:Background: The aim of this study is to synthesize the existing evidence on various palliative care (PC) models for cancer patients. This effort seeks to discern which facets of PC models are suitable for various patient cohorts, elucidate their mechanisms, and clarify the circumstances in which these models operate. Methods: A comprehensive search was performed using MeSH terms related to PC and cancer across various databases. The Preferred Reporting Items for Systematic Reviews and a comprehensive evidence map were also applied. Results: Thirty-three reviews were published between 2009 and 2023. The conceptual PC models can be classified broadly into time-based, provider-based, disease-based, nurse-based, issue-based, system-based, team-based, non-hospice-based, hospital-based, community-based, telehealth-based, and setting-based models. The study argues that the outcomes of PC encompass timely symptom management, longitudinal psychosocial support, enhanced communication, and decision-making. Referral methods to specialized PC services include oncologist-initiated referral based on clinical judgment alone, via referral criteria, automatic referral at the diagnosis of advanced cancer, or referral based on symptoms or other triggers. Conclusion: The gold standard for selecting a PC model in the context of oncology is a model that ensures broad availability of early PC for all patients and provides well-timed, scheduled, and specialized care for patients with the greatest requirement.
Characteristics of palliative nursing and difficulties experienced by the hospice nurses:a qualitative research

Abstract:Objective? To explore the working characteristics and difficulties of hospice nurses and provide reference for improvement of palliative care practice. Methods? By purposive sampling, 10 nurses from a cancer specialist hospital and a community hospital in Beijing were selected and interviewed by semi-structured in-depth interviewing method and the interview data was analyzed by Colaizzi method. Results? The palliative nurses need patience and detailed mind, sympathy, compassion and empathy; the working characteristics of nursing included establishing trust, all the way accompany; palliative nursing and nurses have mutual influences: while getting the self-improved and sense of accomplishment, nurses' psychological burden was heavy and needed psychological adjustment; the palliative nurses faced difficulties such as unsuccessful communication with patients and their family members, lacking in relevant knowledge, misunderstanding from the colleagues and short of medical resources. Conclusions? Hospitals should strengthen the emotional adjustment support for the palliative nurses and to relieve the nurses from stress and negative affect; the government and hospitals should strengthen the propaganda of palliative care so that the citizens can better understand and accept palliative care.

Abstract:Hospitals have played a leading role in providing palliative care in Taiwan as its care model has developed over the past few decades. However, earlier local studies in Taiwan showed that terminal patients prefer to die at home, highlighting the need to promote community-based palliative care instead of hospital-based care. Along with this shift, how community nurses provide palliative home care merits further exploration. This qualitative descriptive study aims to understand (1) how community nurses implement community-based palliative care, (2) what preparations are needed, and (3) what challenges they may face. Purposive sampling was used for recruiting nurses. We conducted one-on-one, in-depth, semi-structured interviews. Interview recordings were transcribed verbatim and analyzed using thematic analysis. Eight community nurses with a range of experience in palliative home care were interviewed. Four major themes emerged: (1) Opportunities, (2) Qualifications, (3) Support, and (4) Commitments. Psychological preparedness, well-developed professional capabilities, external assistance, and peer support motivate community nurses to offer community-based palliative care. As the requests for palliative home care services increase, community nurses play a critical role in palliative home care. Although the sample size is small and the findings retrieved from a small number of experiences might not be generalized to every region, the study results could inform future experience-sharing and workshop sessions to train more nurses for community-based care, expanding service coverage, and providing optimal palliative care.

Abstract:Hospice care emerged in mainland China in 1988, 30 years ago, but its development since that time has been slow. In 1994, the Ministry of Health listed hospice care into the Catalogue of Diagnostic and Therapeutic Subjects of Medical Institutions,<sup><a class="ref showRefEvent2" href="#">1</a></sup> but the public's awareness of hospice and palliative care and the supply of it are still limited. Large numbers of people still die in acute hospitals. In recent years, hospice and palliative care has witnessed rapid development: in February 2017, the State Health Planning Commission issued a guideline<sup><a class="ref showRefEvent2" href="#">2</a></sup> for hospice care, which began to promote the work of hospice care nationwide. In May of 2019, the second batch of national pilot work on hospice care were chosen, 71 regions including whole area of Shanghai are being involved in developing local hospice care service.

Abstract:Background Intensive care units (ICUs) in China primarily focus on active rescue efforts, and it is not common to provide palliative care services within the ICU. As nurses play a primary role as caregivers for end‐of‐life patients in the ICU, it is necessary to explore the factors that impede or facilitate palliative care from their perspective. Aim To explore the barriers and facilitators associated with implementing palliative care in Chinese adult ICUs from nurses' perspectives. Study Design This study utilized a descriptive phenomenological research approach and purposive sampling to conduct face‐to‐face semi‐structured interviews with nurses working in adult ICUs from three comprehensive hospitals in China during the period between February and May 2023. A total of 17 nurses were interviewed, and the collected data were transcribed, coded, and synthesized thematically. Results Two themes of barriers and facilitators of palliative care in the Chinese adult ICU were extracted. The three sub‐themes of hindering factors are as follows: (1) The influence of Chinese traditional culture. (2) The specificity of the ICU context. (3) Lacking sufficient attention in the ICU. The three sub‐themes of the promoting factors are as follows: (1) Government and society value palliative care. (2) Patients and their families have palliative care needs. (3) Nurses view palliative care positively. Conclusion Currently, integrating palliative care into the ICU may face challenges such as cultural factors, the specificity of the ICU context, and insufficient attention. However, it is worth noting that as the government and society place more emphasis on palliative care, more and more people are gradually paying attention to the palliative care needs of critically ill patients and their families. Relevance to Clinical Practice This study serves as a reference for exploring an ICU palliative care service model that is suitable for China's national conditions, such as education and training, resource allocation, service processes, and the palliative care environment, among others.

Abstract:Objective To develop a new home hospice service mode to satisfy the current demands of the advanced cancer patients and evaluate its efficacy. Methods 70 advanced cancer patients at home in Desheng community were selected from January,2010 to January,2013 as the research subjects. We provided them with home hospice care service,and then ana-lyzed their benefits,cost saving and satisfaction. Results We had set up a home hospice service mode,a remote video conferen-cing platform and developed two electronic management systems. Among the 70 patients with terminal cancer,18(25. 7% )were lung cancers,60(85. 7% )were patients with severe or moderate pain and the mean survival time(MST) from diagnosis to death was 30 months. The daily average hospitalization expense in Community Health Center in the end - period was 1 / 13 of that in 3 A grade hospitals(243: 3 126). 69 patients(98. 6% )were satisfied with the new service model. Conclusion This home hospice care service model in Desheng community has better effects in terms of benefits and cost saving and can meet the basic de-mands of home terminal patients so it accords with the present condition of community hospice care service in our country and has popularizing values.
