Abstract:To the editor: Tuberculosis of the central nervous system (CNS) is not commonly encountered compared to the involvement of other systems. Radiological diagnosis of a brain tuberculoma is difficult because the imaging presentation is varied and can be non-specific. We present a case of tuberculoma mimicking a high-grade glial tumor on magnetic resonance imaging (MRI) and clinical presentation. A 43-year-old man presented with three-year history of intermittent headache and epileptic seizure for once. Neurological examination revealed no abnormality. Hematological examination revealed a raised erythrocyte sedimentation rate (ESR). There was no past history of tuberculosis. A brain computed tomography (CT) scan revealed a left frontal round mass just beneath the left frontal bone and adjacent to the anterior falx with obvious parenchymal edema and mass effect. The mass appeared peripheral ring-like iso-density with a central low-density area. Axial non-contrast T1 weighted image (T1WI) showed the lesion with a peripheral hyper-intense ring and a central hypo-intense area. Axial T2 weighted image (T2WI) showed a peripheral low signal intensity ring and a central heterogeneous hyper-intense area of lesion with marked surrounding edema (Figure 1A). The lesion showed heterogeneous iso-signal intensity on diffusion weighted imaging (DWI). Contrast-enhanced T1WI showed peripheral ring-like enhancement of the lesion with a central hypo-intense area, it measured 37 mm × 42 mm × 39 mm (Figure 1B). Surgery was performed using left frontal craniotomy. The totally resected mass was firm and avascular. Contrast-enhanced T1WI showed peripheral ring-like enhancement of the lesion with a central hypo-intense area (B).Typical imaging findings of tuberculomas include solitary or multiple, round “nodules,” often located in the frontal or parietal lobes, with irregular walls and showing ring enhancement after contrast. The “target sign” has been described as a characteristic of tuberculomas, consisting of a ring-enhancing lesion with an additional central area of enhancement or calcification.1,2 MRI findings vary according to the stage of the lesion. The initial tuberculomas consisting of non-caseating granulomas are usually hypo-intense on T1WI, hyper-intense on T2WI and appear nodular or ring enhancement on postcontrast studies. With the further development of such lesions, there exists caseation within the center of lesion. Caseating granulomatous lesions with a solid center appear relatively hypo- or iso-intense both on T1 and T2 images and show ring-like enhancement on postcontrast T1WI with nonenhancement within solid caseation. The caseation in the center then usually liquefies. Tuberculomas with liquid caseation show hypo-intense on T1WI, hyper-intense on T2WI with peripheral hypo-intense rim and demonstrate ring enhancement on postcontrast studies.1 The lesion in this case, which contained elements of both liquefactive and coagulative necrosis showing mixed intensity on T1WI and T2WI with a rim of variable thickness, presented an unusual presentation of an isolated giant tuberculoma just like a malignant tumor. The imaging findings of intracranial tuberculomas are nonspecific, and they have to be differentiated from other causes of space-occupying lesions such as high grade gliomas, pyogenic abscess, metastases, toxoplasmosis, cysticercosis, and lymphoma.2 MR spectroscopy may be helpful in differentiating tuberculomas from other masses. Tuberculomas have a high peak of lipids, more choline, and less N-acetylaspartate and creatine. MR spectroscopy is characterized by a prominent decrease in N-acetylaspartate to creatine ratio (NAA/Cr) and slight decrease in NAA to choline ratio (NAA/Cho). The Cho/Cr ratio is greater than 1 in tuberculomas.3,4 Diffusion-weighted MR is a useful adjunct in abscesses as the central portion of an abscess will demonstrate restricted diffusion however this is not the case in a true tuberculoma.5
