Cerebral sparganosis: CT characteristics and pathological correlation

Abstract:Fifty CT studies of 34 patients with cerebral sparganosis were retrospectively analysed and correlated with pathological findings. On initial CT, the following characteristics were noted: (1) low density in white matter and cerebral hemisphere associated with ipsilateral ventricular dilatation (30/34, 88%); (2) irregular focal contrast enhancement in cortical or subcortical areas (30/34, 88%); (3) a few small, punctate calcifications (26/34, 76%). On follow-up studies the focal enhancing nodule appeared to change in location (5/16, 31%). The CT findings correlated well with pathological findings showing a chronic inflammatory process with active granulomatous lesions and co-existing encephalomalacia, probably caused by migration of the long-surviving spar-ganum.
