Clinical significance of urodynamic studies in neurogenic bladder dysfunction caused by intervertebral disk hernia

Abstract:Aims: To approach the manifestation and clinical significance of urodynamic study in neurogenic bladder dysfunction caused by intervertebral disk hernia. Methods: Thirty patients of neurogenic bladder dysfunction caused by inter-vertebral disk hernia were divided into three groups: the group of detrusor hyperreflexia without dyssynergia, the group of detrusor hyperreflexia with dyssynergia and the group of detrusor areflexia, three groups: the group of urinary retention, the group of urinary incontinence and the group of the others, and three groups: cervical hernia, thoracic hernia, and lumbar hernia. All patients had undertaken computer tomography (CT), magnetic resonance imaging (MRI), and urodynamic study, and the results were analyzed statistically. Results: The category of detrusor muscle reflex had a significant effect on bladder compliance (BC, P < 0.05) and pressure at maximum flow (P (det,Q(max)), P < 0.01), and there were significant differences in post-voiding residual urine (PVR), maximum cystometric capacity (MMC) and maximum flow rate (Q(max)) between the three groups of urinary retention, urinary incontinence, and the other (P < 0.05). There were statistical correlation between the location of the intervertebral disk hernia and the urodynamic results. And there were significant correlation between the bladder compliance (BC, the threshold is 15 ml/cmH(2)O), detrusor pressure (Pdet, the threshold is 40 cmH(2)O), detrusor sphincter dyssnergia (DSD), the location of intervertebral disk hernia and the upper urinary tract lesion (P < 0.01). Conclusions: The urodynamic study is indispensable in the classification, diagnosis, treatment and prognosis of neurogenic bladder dysfunction caused by intervertebral disk hernia.
