Abstract:The cornea serves as the main refractive component of the eye with the corneal stroma constituting the thickest component in a stratified layered system of epithelia, stroma, and endothelium. Current treatment options for patients suffering from corneal diseases are limited to transplantation of a human donor cornea (keratoplasty) or to implantation of an artificial cornea (keratoprosthesis). Nevertheless, donor shortage and failure of artificial corneas to integrate with local tissue constitute important problems that have not been yet circumvented. Recent advances in biofabrication have made great progress toward the manufacture of tailored biomaterial templates with the potential of guiding partially or totally the regeneration process of the native cornea. However, the role of the corneal stroma on current tissue engineering strategies is often neglected. Here, we achieved a tissue-engineered corneal stroma substitute culturing primary keratocytes on scaffolds prepared via melt electrowriting (MEW). Scaffolds were designed to contain highly organized micrometric fibers to ensure transparency and encourage primary human keratocytes to self-orchestrate their own extracellular matrix deposition and remodeling. Results demonstrated reliable cell attachment and growth over a period of 5 weeks and confirmed the formation of a dense and highly organized de novo tissue containing collagen I, V, and VI as well as Keratocan, which resembled very closely the native corneal stoma. In summary, MEW brings us closer to the biofabrication of a viable corneal stroma substitute.

Abstract:Since the 21st century, the development of corneal tissue engineering technology has been developing rapidly. With the progress of biomaterials, cell culture and tissue engineering technology, tissue engineering cornea has gained great development in both basic scientific research and clinical application. In particular, tissue engineered corneal scaffolds are the core components of tissue engineered corneas. It is the focus of current research on tissue engineering cornea to search for scaffolds with good biocompatibility, high safety and good biomechanical properties. In this paper, the recent research progress of tissue engineering corneal materials is reviewed.

Abstract:BACKGROUND:The clinical application of corneal transplantation is restricted by the lacking of donor corneas and the poor prognosis after high-risk keratoplasty.Therefore,how to construct the bio-engineered cornea that has excellent compatibility provides a good vision.Corneal stroma is the key point of the construction of tissue-engineered cornea.OBJECTIVE:To review the advances on the seed cells and biomaterial scaffold of tissue-engineered corneal stroma.RETRIEVE STRATEGY:A computer-based online search of Pubmed was undertaken for the English articles published from January 1980 to June 2007 with the of "corneal stroma,tissue engineering".Meanwhile,we searched CNKI database and Wanfang database for the Chinese articles dated from January 1997 to June 2007 with the same keywords in Chinese.Totally 59 articles were collected and after the first trail,only articles ①related with the construction of tissue-engineered corneal stroma and ②published in recently or in authoritative journals were included.Repetitive researches were excluded.LITERATURE EVALUATION:The articles were mainly from the experiments of constructing tissue-engineered corneal stroma.In the 30 selected articles,5 were review articles,and the others were clinical or basic experimental studies. DATA SYNTHESIS:①There are many keratopathy-induced blind patients in domestic,and the clinical demand for donor corneas is urgent.Therefore,to construct the tissue-engineering cornea that has good bioactivity and could meet certain physiological function is significant for clinical application.②Autologous or allogenic corneal stromal cells are the earliest and most widely used seed cells.It seems that skin fibroblasts may be another ideal candidate for the construction of tissue-engineered corneal stroma.③It is reported that biomaterials including collagen,gelatin and chitosan are used to construct the tissue-engineered corneal stroma.Some researchers reveal that keratocytes grow well on the degradable polyglycolic acid scaffold.④The development of stem cell study,materials science and tissue engineering provides a good prospect for tissue-engineered corneal stroma.But there are still some problems unsolved like how to induce the stem cells to differentiate into keratocytes,how to modify the scaffold materials to maintain good transparence and the nature of biomaterials.CONCLUSION:Tissue-engineered corneal stroma is the key foundation of functional human corneal equivalents,indicating the era of tissue engineering technique to treat keratopathy-induced blindness.It would provide a new strategy for the management of contradiction between supply and demand of human cornea and the high-risk keratoplasty.
Recombinant Human Collagen Hydrogels with Hierarchically Ordered Microstructures for Corneal Stroma Regeneration

Abstract:Tissue engineered scaffolds have demonstrated values in fabricating corneal equivalence. Attempts in this area tend to develop functional scaffold with physiochemical properties and structure resembling the native corneal tissue. Herein, a novel recombinant human collagen (RHC) based scaffold with ordered microstructures is presented to induce the regeneration of corneal stroma. As the RHC is modified with methacrylate anhydride (MA) under optimized concentration and is tailored with hierarchically ordered microstructures of aligned microgrooves and inverse opal nano-scale pores, the derived collagen hydrogels (RHCMA) can possess physiochemical properties resembling native cornea. Diverse topological structures (culture dish, RHCMA hydrogel without patterns, MI-RHCMA hydrogel patch with different width of microgrooves) are determined to study their influence on the growth and differentiation of corneal limbal stromal stem cells (LSSCs), and results demonstrate that the novel MI-RHCMA hydrogel patch could induce LSSCs to grow orderly and differentiate into keratocyte in vitro. Rat intrastromal keratoplasty are performed to examine the tissue repair efficacy of the derived collagen hydrogels, and the results indicate that the novel MI-RHCMA hydrogel patch could induce the regeneration of damaged corneal stroma in vivo. These results indicate that our presented scaffolds can be an effective transplantation graft and will find important values for the damaged cornea in the clinic.
Reconstruction of Rabbit Corneal Stroma Using Tissue Engineering Technique

Abstract:OBJECTIVE:Reconstruct corneal stroma by tissue engineering.METHODS:Primary corneal stromal cells were isolated from newborn rabbit cornea. When the cultured cells reaching confluence, the stromal cells were mixed with polyglycolic acid (PGA) to form a cell-scaffold construct. After 1 week in vitro culture, the constructs were implanted into mother rabbit corneal stroma. Parts of corneal stromal cells were transfected with GFP gene as mark of transplanted cells. Tissues were harvested at 8 weeks for transmission electron microscopy (TEM), histology and Western blot evaluation. In control, PGA alone was implanted into the other cornea.RESULTS:The engineered corneal stroma became transparent gradually over a period of 8 weeks. The histology of engineered stromal lamellar was relatively similar to that of natural one, no significant differences were found in the diameter of cornea collagen fiber [(29.4 +/- 4.7) nm] in experimental rabbits compared with control [(28.5 +/- 3.5) nm], Student's t-test: P = 0.1316 > 0.05. TEM demonstrated that collagen fibrils deposited in engineered stroma had a similar diameter compared to that of normal counterpart. In addition, Western blot showed the positive expression of type I collagen in the collagen fibrils. In contrast, no new stroma tissue was formed when PGA alone implanted. A green colored stroma was observed when engineered with GFP-labeled cells under fluorescence light microscope.CONCLUSION:The results demonstrate that nearly transparent corneal stroma can be obtained by the technique of cornea engineering.

Abstract:PURPOSE: To construct a scaffold using silk fibroin (SF) and chitosan (CS) that could replace the corneal stroma with the biological characteristics of the scaffold materials still intact.METHODS: To develop an organotypic corneal stroma, SF and CS were chosen to synthesise the tissue-engineered bioscaffold. We cultured primary rabbit corneal epithelial cells and corneal stromal cells in vitro. Keratocytes were used to assess cytotoxicity on SF-CS (SFCS) blends, which was determined by a Cell Counting Kit-8 assay. The corneal lamellar scaffolds were developed with sequential culture techniques to form cell-scaffold constructs. Implantation was tested in 15 New Zealand White rabbits. The corneal substitutes were analysed by light and electron microscopy.RESULTS: The reconstructed lamellar cornea was comparable to native tissue, with high levels of K3/12 expression in the corneal epithelial cells and vimentin in the stromal cells; moreover, the morphology and the position of the cells could be distinguished by histological methods. There was no sign of any immune reaction in or around the transplanted discs 12 weeks after implantation.CONCLUSION: A SFCS scaffold might be a suitable blend for corneal tissue engineering.

Abstract:Objective: To reconstruct corneal stroma with cultured cells and trace the cultured cells by Green Fluorescence Protein (GFP) gene transfection. Methods: Corneal stromal cells were isolated from newborn rabbit cornea and were cultured and expanded in vitro. At the time of reaching confluency, corneal stromal cells were mixed with Polyglicolic Acid (PGA) to form a cell-scaffold construct. At 1 week after culture, the constructs were implanted into mother rabbit corneal stroma and portions of corneal stromal cells were transfected with GFP gene to mark the seed cells by retroviral vector. Tissues were harvested at 8 weeks for histologic evaluation. Results: The engineered tissue proceeded to become transparent over an 8-week period. Stroma engineered in cornea revealed the histology of lamellar relatively similar to that of natural stroma. Additionally, a green colored stroma was observed when engineered with GFP-labeled cells under fluorescence light microscope, suggesting that the stroma was constituted by implanted cells. Conclusions: The results demonstrate that stroma could be engineered in recipient cornea and the composed cells originate from cultured cells.
Construction of tissue engineered porcine corneal stroma with skin fibroblasts

Abstract:Objective To explore the feasibility of constructing tissue engineered porcine corneal stroma with skin fibroblasts in vivo.Methods Skin fibroblasts were isolated from embryonic porcine,cultured and expanded in vitro.Cells were labeled with green fluorescence protein(GFP) gene by retro-viral infection.Cells at passage 3 were seeded on polyglycolic acid(PGA) non-woven fibers to form a cell-scaffold complex.The complexes were then implanted into porcines' corneal stroma after culturing in vitro for 1 week.Engineered stroma was observed continuously and harvested after 8 weeks for gross and histological evaluation.PGA with corneal stromal cells was served as control. Results The engineered tissue in the stroma gradually became transparent over a period of 8 weeks,showing no difference with the control group.Histologically,the engineered stromal lamellar was relatively regular and similar to the control.The implanted cells were confirmed by GFP expression under fluorescent microscope.By transmission electron microscopy examination, no significant difference in the diameter of collagen fiber was observed between the engineered stroma and normal stroma. Conclusion Tissue engineered corneal stroma may be formed with skin fibroblasts in porcine corneal microenvironment.

Abstract:Corneal disease is one of the most important causes of blindness worldwide. Currently, the dominating treatment of corneal blindness is corneal transplantation. However, the main source of cornea for transplantation is based on donations which is far from enough to meet the requirement (less than 1:70 of cases). The severe shortage of donor cornea promotes the studies of effective corneal alternatives. However, many problems remain and can't be solved in current researches, such as original geometry reconstruction and ocular optical function restoring. 3D bioprinting can be a promising approach for corneal substitution. The advantages of this technology in corneal regeneration enable personalized corneal implant and single or multi-layer corneal equivalents with controllable structure and designed refractive ability. In this review, the progress, applications and limitations of most influential works among current keratoprosthesis and tissue-engineering cornea researches are discussed. Then the applications of 3D bioprinting in manufacturing multi-layered structures and surface are mentioned. Further, the potential, advantages in current research of 3D bioprinting single or multi-layer corneal equivalents and alternatives are discussed. Finally, an insight into the technical challenges and prospective facing the future research of 3D bioprinting corneal alternatives in vivo and in vitro is provided.

Abstract:BACKGROUND: Tissue engineering has a promising prospect in corneal transplantation. Scaffolds are always restricting the development of tissue-engineered cornea. OBJECTIVE: To analyze application of different seed cells and scaffolds, and to summarize the progress of tissue-engineered cornea in recent years.METHODS: First author searched literature from CNKI (2000/2010-10) and PubMed database (2000/2010-10). The key words are "tissue engineering, corneal transplantation" in Chinese or English. A total of 223 literatures were seized by computers, according to the inclusion criteria, papers concerning the advance, application and reconstruction of tissue-engineered cornea were analyzed. Finally, 33 papers were included for further analysis. The present study was to analyze the seed cells, scaffolds, organ building and clinical applications of tissue-engineered cornea and investigate the development direction in the future. RESULTS AND CONCLUSION: The results show that seed cells and scaffolds are the focus of the studies now. Corneal transplantation has a high success rate in organ transplantation since the immune privilege of eye and avascular cornea, and it will be able to be a tissue engineering organ that can be largely built, easy to transplant. Reconstruction of cornea has reached first base, but every kind of scaffold has certain drawbacks, so the next goal is to find an ideal scaffold material.

Abstract:Transplantation of corneal epithelial grafts constructed ex vivo is now a standard procedure in the treatment of severe ocular surface disorders. With the development of tissue engineering, tissue engineered epithelial grafts becomes an important donor for corneal transplantation. This review will introduce some of the major contributions made during the past years in the advancement of seed cells, biomaterial carriers and forming methods of corneal epithelium reconstruction.

Abstract:Previously, we successfully engineered a corneal stromal layer using corneal stromal cells. However, the limited source and proliferation potential of corneal stromal cells has driven us to search for alternative cell sources for corneal stroma engineering. Based on the idea that the tissue-specific environment may alter cell fate, we proposed that dermal fibroblasts could switch their phenotype to that of corneal stromal cells in the corneal environment. Thus, dermal fibroblasts were harvested from newborn rabbits, seeded on biodegradable polyglycolic acid (PGA) scaffolds, cultured in vitro for 1 week, and then implanted into adult rabbit corneas. After 8 weeks of implantation, nearly transparent corneal stroma was formed, with a histological structure similar to that of its native counterpart. The existence of cells that had been retrovirally labeled with green fluorescence protein (GFP) demonstrated the survival of implanted cells. In addition, all GFP-positive cells that survived expressed keratocan, a specific marker for corneal stromal cells, and formed fine collagen fibrils with a highly organized pattern similar to that of native stroma. However, neither dermal fibroblast-PGA construct pre-incubated in vitro for 3 weeks nor chondrocyte-PGA construct could form transparent stroma. The results demonstrated that neonatal dermal fibroblasts could switch their phenotype in the new tissue environment under restricted conditions. The functional restoration of corneal transparency using dermal fibroblasts suggests that they could be an alternative cell source for corneal stroma engineering.

Abstract:OBJECTIVE:To explore whether skin fibroblasts could be used as a cell source for reconstruction of the corneal stroma.METHODS:It was an experimental study. Skin fibroblast cells were isolated from newborn rabbits, cultured and expanded in vitro. Cells were labeled with green fluorescence protein (GFP) gene by retro-viral infection. Fibroblasts at passage 3 were seeded on polyglycolic acid (PGA) non-woven fibers to form a cell-scaffold construct. Constructs were then implanted into the adult rabbit corneal stroma layer after being cultured in vitro for 1 week. Engineered stroma were observed continuously and harvested after 8 weeks of transplantation for gross, histological evaluation and Keratocan examination. PGA alone was used as control.RESULTS:The engineered tissue in the cornea became transparent gradually over a period of 8 weeks. Histological analysis showed that engineered stromal lamellar was relatively regular and the orientation of fibers was parallel to the surface of cornea, which is similar to normal cornea. The implanted cells were confirmed by GFP expression under fluorescent microscope, which also express Keratocan. By transmission electron microscopy examination, no significant difference in the diameter of collagen fiber was observed between engineered stroma (33.08 + or - 2.47) nm and normal stroma (t = 1.80, P = 0.0771).CONCLUSION:Skin fibroblast cells could be used as seed cells for reconstruction of the corneal stroma.
3D-Printed Nano-Structural Poly(Epsilon-Caprolactone) (pcl) Scaffolds for Corneal Stromal Regeneration

Abstract:Purpose : Regeneration of corneal stroma is believed to be among the greatest challenges in corneal tissue engineering. The aim of this study is to investigate the effect of a 3D-printed nano-structural poly(e-caprolactone) (PCL) scaffold on the material-cellular interaction, and to evaluate the efficiency for the corneal stroma regeneration in vitro. Methods : Two nano-structural scaffolds of different design and fiber distribution were fabricated by melt electrospinning 3D printing method using PCL. Human keratocytes were isolated from collagenase-digested limbal stromal tissue and seeded on the 3D-printed PCL scaffold. Cell morphology on scaffolds was studied by confocal microscopy, and cell proliferation was analyzed by PrestoBlue assay. Extracellular matrix(ECM) produced by keratocytes on the scaffolds was characterized by scanning electron microscope(SEM) and two-photon fluorescent microscopy. Results : The 3D-printed PCL scaffolds exhibited different micro-structure under SEM. As revealed in Figure.1, scaffold on the left presented offset fiber distribution, while the other one showed a fiber distribution at regular intervals. SEM and two-photon fluorescent microscopy demonstrated that keratocytes tended to grow along the fibrous network, and spread well on both scaffolds. Collagenous ECM produced by cells on the regular-fibrous scaffold was more abundant than on the offset-fibrous scaffold, forming a relatively thicker cross-section (Figure 2). Compared to a collagen fibrillar network secreted on regular-fibrous scaffolds, a lamellar ultrastructural ECM composed of aligned collagen was remodeled on offset-fibrous scaffolds, which was similar to native cornea stroma. Immunofluorescence staining showed that regular-fibrous scaffolds induced less secretion of type I collagen (COLI) and type V collagen (COLV) than offset-fibrous scaffolds. Conclusions : Our study showed that the fiber composition and topography of 3D-printed nanofibrous PCL scaffolds have a significant effect on the cell behavior through the material-cellular interaction. The refined design of nano-structure by melt electrospinning method stimulates self-organization and self-adaption of ECM, which helps mimicking native tissue microenvironments and offering the potential as cornea stroma regeneration substitutes.

Abstract:Interest in developing tissue-engineered cornea has increased with the decrease in the supply of donor tissue. The aim of the present study was to investigate the feasibility and method of reconstructing corneal equivalents with porcine corneal acellular matrix as the scaffold in a dynamic culturing system. Applying the detergent Triton X-100 (1%) and a freeze-drying process, porcine corneas were decellularized and prepared as a scaffold, and hematoxylin-eosin staining and scanning electron microscopy showed no cells in the decellularized stroma. In order to measure the in vivo biocompatibility, part of the scaffold was transplanted into a pocket of rabbit corneal stroma and observed for 3 months. No sign of rejection were observed, and the acellular matrix gradually integrated in the rabbit cornea, indicating that the scaffold had good biocompatibility. To reconstruct a tissue-engineered cornea, cultured rabbit keratocytes were seeded into the scaffold. After 1 week of culture in a culturing vessel, rabbit epithelial and endothelial cells were seeded on both sides of the stroma, respectively. The reconstructed cornea consisted of three layers in histological structure: the epithelium, stoma and endothelium. Stratified epithelial cells formed on the surface, which were cytokeratin 3 positive in the cytoplasm; endothelial cell monolayers were located on the inner side, and pump-related aquaporin 1 was found in the cells. These results confirmed that the corneal acellular matrix can be used as a scaffold for tissue-engineered cornea, and a biological corneal equivalent can be reconstructed in a dynamic culturing system. Biomimetic corneal stroma using electro-compacted collagen

Abstract:<p>Engineering substantia propria (or stroma of cornea) that mimics the function and anatomy of natural tissue is vital for in vitro modelling and in vivo regeneration. There are, however, few examples of bioengineered biomimetic corneal stroma. Here we describe the construction of an orthogonally oriented 3D corneal stroma model (3D-CSM) using pure electro-compacted collagen (EC). EC films comprise aligned collagen fibrils and support primary human corneal stromal cells (hCSCs). Cell-laden constructs are analogous to the anatomical structure of native human cornea. The hCSCs are guided by the topographical cues provided by the aligned collagen fibrils of the EC films. Importantly, the 3D-CSM are biodegradable, highly transparent, glucose-permeable and comprise quiescent hCSCs. Gene expression analysis indicated the presence of aligned collagen fibrils is strongly coupled to downregulation of active fibroblast/myofibroblast markers α-SMA and Thy-1, with a concomitant upregulation of the dormant keratocyte marker ALDH3. The 3D-CSM represents the first example of an optimally robust biomimetic engineered corneal stroma that is constructed from pure electro-compacted collagen for cell and tissue support. The 3D-CSM is a significant advance for synthetic corneal stroma engineering, with the potential to be used for full-thickness and functional cornea replacement, as well as informing <em>in vivo</em> tissue regeneration.</p>

Abstract:Corneal epithelial defects and excessive wound healing might lead to severe complications. As stem cells can self-renew infinitely, they are a promising solution for regenerating the corneal epithelium and treating severe corneal epithelial injury. The chemical and biophysical properties of biological scaffolds, such as the amniotic membrane, fibrin, and hydrogels, can provide the necessary signals for stem cell proliferation and differentiation. Multiple researchers have conducted investigations on these scaffolds and evaluated them as potential therapeutic interventions for corneal disorders. These studies have identified various inherent benefits and drawbacks associated with these scaffolds. In this study, we provided a comprehensive overview of the history and use of various stem cells in corneal repair. We mainly discussed biological scaffolds that are used in stem cell transplantation and innovative materials that are under investigation.

Corneal Stroma Regeneration with Acellular Corneal Stroma Sheets and Keratocytes in a Rabbit Model.

Abstract:Acellular corneal stroma matrix has been used for corneal stroma engineering. However, because of its compact tissue structure, regrowth of keratocytes into the scaffold is difficult. Previously, we developed a sandwich model for cartilage engineering using acellular cartilage sheets. In the present study, we tested this model for corneal stroma regeneration using acellular porcine corneal stroma (APCS) sheets and keratocytes. Porcine corneas were decellularized by NaCl treatment, and the APCS was cut into 20-μm-thick sheets. A rabbit corneal stroma defect model was created by lamellar keratoplasty and repaired by transplantation of five pieces of APCS sheets with keratocytes. Six months after transplantation, transparent corneas were present in the experimental group, which were confirmed by anterior segment optical coherence tomography examination and transmittance examination. The biomechanical properties in the experimental group were similar to those of normal cornea. Histological analyses showed an even distribution of keratocytes and well-oriented matrix in the stroma layer in the experimental group. Together, these results demonstrated that the sandwich model using acellular corneal stroma sheets and keratocytes could be potentially useful for corneal stroma regeneration.

Abstract:Millions of individuals across the world suffer from corneal stromal diseases that impair vision. Fortunately, three-dimensional (3D) bioprinting technology which has revolutionized the field of regenerative tissue engineering makes it feasible to create personalized corneas. In this study, an artificial cornea with a high degree of precision, smoothness, and programmable curvature was prepared by using digital light processing (DLP) 3D bioprinting in one piece with no support structure, and the construct was then confirmed by optical coherence tomography (OCT). On the basis of this approach, we developed a novel corneal decellularized extracellular matrix/gelatin methacryloyl (CECM-GelMA) bioink that can produce complex microenvironments with highly tunable mechanical properties while retaining high optical transmittance. Furthermore, the composite hydrogel was loaded with human corneal fibroblasts (hCFs), and in vitro experiments showed that the hydrogel maintained high cell viability and expressed core proteins. In vivo tests revealed that the hydrogel might promote epithelial regeneration, keep the matrix aligned, and restore clarity. This demonstrates how crucial a role CECM plays in establishing a favorable environment that encourages the transformation of cell function. Therefore, artificial corneas that can be rapidly customized have a huge potential in the development of in vitro corneal matrix analogs.

Biomimetic Corneal Stroma for Scarless Corneal Wound Healing via Structural Restoration and Microenvironment Modulation

Abstract:Corneal injury-induced stromal scarring causes the most common subtype of corneal blindness, and there is an unmet need to promote scarless corneal wound healing. Herein, a biomimetic corneal stroma with immunomodulatory properties is bioengineered for scarless corneal defect repair. First, a fully defined serum-free system is established to derive stromal keratocytes (hAESC-SKs) from a current Good Manufacturing Practice (cGMP)-grade human amniotic epithelial stem cells (hAESCs), and RNA-seq is used to validate the phenotypic transition. Moreover, hAESC-SKs are shown to possess robust immunomodulatory properties in addition to the keratocyte phenotype. Inspired by the corneal stromal extracellular matrix (ECM), a photocurable gelatin-based hydrogel is fabricated to serve as a scaffold for hAESC-SKs for bioengineering of a biomimetic corneal stroma. The rabbit corneal defect model is used to confirm that this biomimetic corneal stroma rapidly restores the corneal structure, and effectively reshapes the tissue microenvironment via proteoglycan secretion to promote transparency and inhibition of the inflammatory cascade to alleviate fibrosis, which synergistically reduces scar formation by ≈75% in addition to promoting wound healing. Overall, the strategy proposed here provides a promising solution for scarless corneal defect repair.
