Abstract:Objective To investigate the distribution and antibiotic resistance of bacterial pathogens in patients with acute exacerbation of chronic obstructive pulmonary diseases(AECOPD).Methods Sputum and tracheal secretions were collected by bronchoscopy in 625 patients with AECOPD who were hospitalized during the period from March 2007 to March 2009 in our hospital. The results of pathogen identification and susceptibility testing were analyzed. Results A total of 478 strains of microorganisms were isolated from 440 of the 625 patients.Gram-negative bacteria accounted for 65.27%,including P.aeruginosa,K.pneumoniae,Acinetobacter spp.,E.coli,and S.maltophilia.Gram-positive bacteria accounted for 20.71%,including S.aureus,S.pneumoniae,and Enterococcus spp.Fungi accounted for 14.02%,including C.albicans(6.69%),C.tropicalis and other yeasts.Susceptibility testing results showed that gram-negative bacteria were highly resistant to third generation cephalosporins, but relatively susceptible to piperacillin-tazobactam and carbapenems.Gram-positive cocci were highly resistant to penicillin, clindamycin, and erythromycin. No vancomycin-resistant strain was found in gram-positive isolates. Conclusions The main pathogens in lower respiratory tract infection in AECOPD patients are gram-negative bacteria. The bacterial isolates are highly resistant to antibiotics.Clinicians should pay attention to isolation and culture of pathogens and antimicrobial susceptibility test for more rational antibiotic therapy.
