Abstract:Objective To Evaluate the clinical efficacy of non-invasive ventilation with average volume assured pressure support(AVAPS) in chronic obstructive pulmonary disease(COPD) with acute respiratory failure. Methods One hundred and eighty five cases in intensive care unit(ICU) who had used non-invasive positive pressure ventilation(NPPV) for treating acute exacerbation of COPD(AECOPD) with acute respiratory failure were analyzed retrospectively. According to the mode of NPPV, they were divided into the AVAPS group(80 cases)and the control group(105 cases). The changes in ventilation parameters: tidal volume(VT), minute ventilation(MV), peak airway pressure(PIP), expiratory pressure(EPAP)and respiratory frequency(RR) after 1 h and 24 h ventilation were recorded. Data of arterial blood gas analysis, heart rate(HR), mean arterial pressure(MAP) and Glasgow coma scale(GCS) were compared before and after 1 h, 24 h ventilation;The NPPV related complications(bloating, gas leakage, aspiration pneumonia, hypotension), incidence of endotracheal in-tubation, noninvasive mechanical ventilation time, length of stay in ICU, total time of hospitalization and prognosis between the two groups were compared. Results ① After 24 h ventilation when compared with after 1 h ventilation, VT increased and RR reduced in both of the two groups, while EPAP had no significant change. PIP decreased significantly(P=0.026)and MV reduced significantly(P=0.041) after 24 h ventilation when compared with that after 1 h ventilation in AVAPS group. Compared with control group, VT(P=0.032), MV(P=0.047)and PIP(P=0.015)in AVAPS group were significantly higher after 1 h ventilation, while VT was still higher(P=0.043)and PIP was slightly lower but had not reached significant difference after 24 h ventilation. ② After 24 h ventilation, the pH, partial pressure of carbon dioxide in artery(PaCO2), partial pressure of oxygen in artery(PaO2), PaO2 /FiO2 improved significantly and HR and MAP decreased in both of the two groups, but in the AVAPS group pH(P=0.046) and PaCO2(P=0.027) both improved significantly after 1 h ventilation and GCS was elevated significantly(P=0.041)after 24 h ventilation. Compared with the control group, level of PaCO2 was significantly lower in AVAPS group after 1 h and 24 h ventilation, and GCS was significantly higher after 24 h ventilation. ③ The incidence of gaseous distention was significantly lower in AVAPS group than in control group(P=0.045). The rate of endotracheal intubation(P=0.037), non-invasive ventilation time(P=0.041)and length of stay in ICU(P=0.035)were significantly lower in AVAPS group than in control group, while the incidence of other complications(aspiration pneumonia, hypotension)and the total time of hospitalization and mortality had no statistically significant differences. Conclusions In the treatment of AECOPD with acute respiratory failure,the AVAPS mode of non-invasive ventilation therapy could be adjusted for the disease progress and needs of patients, improving the ventilation efficacy and worthy to be promoted.
