Abstract:Objective To understand the perioperative blood transfusion status and trend of abdominal tumor surgical of 18 310 cases in West China Hospital, and to provide a data support for strengthening blood protection and a further blood transfusion guideline in patients undergoing abdominal tumor surgery. Methods The information of demographic, diagnosis, surgical styles, and blood transfusion in all patients who underwent abdominal tumor surgery from 1996 to 2007 were collected. The time of blood transfusion was defined as pre-(T_0) , intra-(T_1 ) , 2 d after surgery(T_2) , and more than 3 d after surgery(T_3). The blood transfusion rate, amount, and time of patients in different year were analyzed. Results The total blood transfusion amount was about 8 000 L in 12 years, which included 79% Red blood cells (RBCs) and 21% fresh frozen plasma ( FFP). There were 39. 84% patients underwent abdominal tumor surgery received RBCs transfusion, and 8.79% patients received FFP transfusion. The RBCs transfusion percentage was high up to 82. 59'% in 1996, and gradually decreased to 30. 11 % in 2007 ( P < 0. 05 ). The transfusion percentage of FFP was significant increasing from 2.41 % in 1996 to 22. 8% in 2007 (P < 0.05). After 2004, the percentage and amount of FFP transfusion increased rapidly. Conclusion Nearly 40% abdominal tumor surgical cases received allergenic transfusion perioperatively in 1996 - 2007 in our hospital. The transfusion of RBCs didn' t increase with the abdominal tumor surgical cases, which indicated that blood protection has taken effect in our hospital. The use of FFP has trend to increase rapidly after 2004, which suggested that FFP abuse still existed in our hospital, and it is necessary to prompt reasonable use of FFP.

Abstract:Objective:To analyze blood transfusion for patients with brain tumor in perioperative period and set the scientiifc and rational principle for clinical blood transfusion.Methods: One hundred and one patiens with brain tumor were enrolled from January to December 2014 in Huashan hospital. The patients were divided into the young (aged from 18 to 39 years,13 cases), middle (aged from 40 to 59 years, 50 cases), and elderly (aged from 60 to 80 years, 38 cases)groups. The blood transfusion data of three groups in perioperative period were retrospectively analyzed.Results:Among patients receiving blood transfusion in perioperative period,those with meningioma acounted for 47.52% (48/101), meningioma glioma 19.80% (20/101) and metastatic tumor 10.89% (11/101). The patients in middle-aged group acounted for 49.50% (50/101) in patients receiving blood transfusion in perioperative period. Forty eight patients with meningioma received 170 U of the red blood cells, 282 U of the plasma and 11 U of the platelet,which were higher than other patients with brain tumor. The average transfusion of the red blood cells was 2.43 U, 3.2 U and 3.67 U per person in the young,middle-aged and elderly groups, respectively. Conclusion: The transfusion of the red blood cells is increasing with aging of the patients.It is necessary to pay more attention to the blood management of the middle-aged and the elderly patients in perioperative period.

Abstract:Transfusion-transmitted infections have been a major threat to the safety of transfusion, although remarkable advances have been made in the prevention of the major transfusion-transmitted diseases due to improvement in donor screening, testing and viral inactivation of blood products, and rational use of blood in clinical works. At the same time, some established blood-borne pathogens can not yet be detected and cleared thoroughly, besides, novel agents have now appeared, and are still emerging. This presents continuous menaces to the safety of blood transfusion. The authors have highlighted the different transfusion-transmitted infections and the update on the screening technologies, and have focused on the precautionary strategies that can be implemented for improvements in blood safety.

Abstract:Traumatic brain injury is always associated with hemorrhage and coagulopathy, leading the occurrence of anemia, platelet function inactivation, platelet and coagulation factor consumptive reduction. Theoretically, transfusion therapy should be given to supplement the missing component in an appropriate range. However, whether the transfusion therapy can improve the prognosis of patients with traumatic brain injury, and the indications of transfusion, have been the focus of academic debate for a long time. This article reviews the latest progress of transfusion therapy in traumatic brain injury, and provides reference for better guidance of transfusion in clinical treatment.

Abstract:Blood transfusion is a common therapeutic intervention in hospitalized patients. There are numerous indications for transfusion, including anemia and coagulopathy with deficiency of single or multiple coagulation components such as platelets or coagulation factors. Nevertheless, the practice of transfusion in critically ill patients has been controversial mainly due to a lack of evidence and the need to consider the appropriate clinical context for transfusion. Further, transfusion carries many risk factors that must be balanced with benefits. Therefore, transfusion practice in ICU patients has constantly evolved, and we endeavor to present a contemporary review of transfusion practices in this population guided by clinical trials and expert guidelines.

Abstract:BackgroundAllogeneic blood transfusion-induced immunomodulation (TRIM) and its adverse effect on the prognosis of patients treated surgically for cancer remain complex and controversial. However, the potential risk associated with allogeneic blood transfusion has heightened interest in the use of autologous blood transfusion. In the present study, the serum concentrations of neopterin, interferon-gamma (IFN-γ), T lymphocyte subsets (CD3+, CD4+, CD8+, CD4+/CD8+) and a possible association between these variables were investigated. The purpose was to further evaluate the effect of autologous versus allogeneic blood transfusion on immunological status in patients undergoing surgery for gastric cancer.MethodsSixty ASA I∼II (American Society of Anesthesiologists) patients undergoing elective radical resection for stomach cancer were randomly allocated to receive either allogeneic blood transfusion (n=30) or autologous blood transfusion (n=30). Serum concentrations of the neopterin, IFN-γ and T lymphocyte subsets in the recipients were measured before induction of anesthesia, after operation, and on the 5th postoperative day.ResultsBoth two groups, serum neopterin, IFN-γ, percentages of T-cell subsets (CD3+, CD4+), and CD4+/CD8+ ratio had significantly decreased after operation, but decreased more significantly in group H (receiving allogeneic blood transfusion) than those in group A (receiving autologous whole blood transfusion) (P<0.05). On the 5th postoperative day, serum neopterin, IFN-γ, CD3+, CD4+ T-cells, and CD4+/CD8+ ratio returned to the baseline values in group A. In contrast, the above remain decreasing in group H, where there were no significant relations between serum neopterin and IFN-γ.ConclusionPerioperative surgical trauma and stress have an immunosuppressive impact on gastric cancer patients. Allogeneic blood transfusion exacerbates the impaired immune response. Autologous blood transfusion might be significantly beneficial for immune-compromised patients in the perioperative period, clearly showing its surperiority over allogeneic blood transfusion.

Abstract:Objective To study the safety and feasibility of auto-transfusion of blood drained from patients own abdomen in patients with blunt trauma of spleen as conservative treatment.Methods From January 1,2008 to December 31,2010,94 patients were diagnosed to be blunt trauma of spleen.After careful assessment and series lab tests,44 patients were eligible to be non-operative treated.They were randomly (random number) into two groups in equal number (n =22).The patients of auto-transfusion group were transfused the blood drained from patients own abdomen,and the control group was transfused red blood cells from donors.The comparison of demographics,ISS (injury severity scores),AAST (American Association for the Surgery of Trauma) and lab findings between two groups before and after transfusion with t test and chi-square test.Then paired-samples t test was used to analyze the hemodynamics,blood components and blood clotting indexes of two groups before and after blood transfusion.Meanwhile t test was done as well to compare the differences in the above variables between two groups after transfusion.SPSS 10.0 version was used to analyze the collected data.Results There were no significant differences in demographics and physical condition between two groups.After blood transfusion,the hemodynamics and anemia of two groups were significantly improved,and there were no differences in the changes of the above variables between two groups.Conclusions Blood drained from patient own abdomen can be auto-transfused in patients with blunt trauma of spleen treated non-operatively as safe and efficient as transfusion of blood from donors.This study proves the autologous transfusionto to be an easy,feasible and economic measures in urgent situation as expected.

Abstract:Blood transfusion is an important part in themedical treatment, but there remains wide variation in practice, such as some risks and unnecessary blood transfusions. The patient-centered blood management(PBM), which describes an evidence-based, multi-disciplinary therapeutic approach, plays an important role in optimization of blood products and reduction of risks associated with blood transfusion. The PBM made by the multi-disciplinary team(MDT) can provide a management plan which aims at reducing patients’ blood loss and the rate of blood transfusion in the treatment and improving outcomes of the patients. This paper describes the importance of PBM in four aspects including anemia management, coagulation optimization, multi-technology combination and patient-centered decision making.

Abstract:Objective:Many studies have shown that introperative blood transfusion may predispose patients to an increased risk of tumor recurrence after surgery.The study was to explore the significance of introperative blood transfusion for patients with large hepatocarcinoma treated with hepatectomy.Methods:The clinicopathologic and survival data of 166 consecutive patients with large hepatocarcinoma who underwent hepatectomy between 2002 and 2005 were retrospectively analyzed.Eighty-seven patients who received blood transfusion were enrolled into group A and 79 patients who didn't receive blood transfusion were enrolled into group B.The overall survival and tumor-free survival rates of the two groups were evaluated and compared with univariate log-rank test.The multivariate analysis was performed by Cox proportional hazards model.Results:The 1-,3-,and 5- year overall survival rates and tumor-free survival rates of all patients were 73.2%,31.5%,and 26.2%,and 27.0%,20.1%,and 18.0%,respectively.The 1-,3-,and 5-year overall survival rates and tumor-free survival rates of patients in group A were 71.6%,20.2%,and 12.7%,and 22.6%,13.6%,and 11.3%,respectively.The 1-,3-,and 5-year overall survival rates and tumor-free survival rates of patients in group B were 75.0%,45.2%,and 45.2%,and 32.1%,27.6%,and 27.6%,respectively,with significant differences between the two groups (P0.05).Multivariate analysis revealed that portal vein tumor thrombus,pathological grade and transfusion were independent risk factors for long-term survival of patients with large hepatocarcinoma.Conclusion:Introperative blood transfusion is one of the predictive factors for the prognosis of patients with large hapetocarcinoma who underwent hepatectomy and may promote tumor recurrence and reduce over all survival rate.

Abstract:Objective: To study the effects of perioperative transfusion on the immunity of tumor patients by establishing the experimental tumor-borne rat models with Intraoperative transfusion. Methods: Experimental female Wistar rats inoculated with Wolker 256 sarcoma cells were divided into six groups randomly who receiving different transfusions during laparotomy. Tumor mass was measured and the changes of T subsets was analyzed by flow cytometry postoperatively. Results: The rat transfused with whole blood and leucocyte fraction of SD rat had an obviously rapid tumor growth and depressed cell-mediated immunity (P<0.01); while the other groups (transfused with whole blood of Wistar rat,serum and erythrocyte fraction of SD rat) had no significant difference from the control group (P>0.05). Conclusion: The components of allogeneic leucocyte promotethe tumor cell growth; while the serum and erythrocyte have no effect.

Abstract:OBJECTIVE To explore the application of autologous blood transfusion in bone surgery.METHODS 47 patients with autologous blood transfusion in bone surgery were collected in our study,and other 40 patients were selected as control group.We compared the blood routine test,serum electrolyte and adverse actions of blood transfusion were compared.RESULTS The average blood loss was(1 147 ± 364) ml,and average recycled blood was(589 ± 264) ml,and the average blood loss was 52.40% ± 0.81%.There was no significant difference in RBC,Hb and HCT of the two groups before and after surgery(P﹥0.05).We compared the K +,Na + and Cl + of the two groups before and after surgery,and no significant difference was found(P﹥0.05).For 47 patients,no blood transfusion reaction and infection was found.CONCLUSION The blood loss is serious in bone surgery,and we should decrease the autoblood loss and allogenic blood transfusion,so as to decrease the reactions of blood transfusion and infection.

Abstract:Objective: To explore the influence of perioperative blood transfusion on the postoperative survival of patients with colon cancer. Methods: Univariate and multivariate retrospective analyses were performed on the survival in a total of 723 colon cancer patients which were treated surgically during a period of 10 years. Results: Kaplan-Meier estimates showed that more than 800 mL perioperative blood transfusion was the survival predictor. Blood transfusion influenced significantly the prognosis of patients 40 years old and younger, those undergoing helicoloectomy left side, those with papillary adenocarcinoma, those with big tumors (diameter ≥ 8 cm), those with stage I tumors, those with lymphatic node metastases and those without liver metastases. In multivariate analysis only the tumor location, radicality of operation, lymphatic invasion, liver metastasis, depth of tumor invasion and TNM stage retained their significance. Conclusion: Perioperative blood transfusion is the prognostic factor for patients with colon cancer to some extent. The indication of blood transfusion must be restricted strictly, specially in patients younger than 40 years old, with right side lesion, papillary adenocarcinoma, big tumors (diameter ≥ 8 cm), stage I tumors and lymphatic node metastases or without liver metastases. But perioperative blood transfusion may not be deleterious for patients with staging IV disease and with distant metastases.

Abstract:Objective:To compare the incidence of TA GVHD in patients with impaired immunity after transfusion with irradiated or unirradiated blood components.Method:Analyse retrospectively clinical features and outcome of the control group and the irradiation group,in the irradiation group,all blood components are irradiated by x ray,total dose is 2000 cGy,the dose rate is 200 cGy/min.Result:In the control group,four patients (2.0%) had TA GVHD,while in the irradiated group,no patient had TA GVHD.Conclusion:Lymphocyte in blood components should be inactivated or reduced when it is transfused to patients with impaired immunity,irradiation could reduce the incidence of TA GVHD.

Abstract:Although blood stations and medical institutions provided and used blood in a very strict way which was regulated by some related medical laws and regulations,a small part of patients was likely to be still infected by some infectious diseases due to the existence of undetected rate and window phase in the process of blood detection.The present ways to solve the social problems in our country were not perfect to protect the patients,blood stations as well as medical institutions very well.The article tried to provide some possible solutions to this social problem,including how to promote social security system and develop blood transfusion insurance system in order to minimize the conflicts between patients and medical institutions caused by no-fault blood transfusion infection.
Anemia Tolerance Versus Blood Transfusion on Long-Term Outcomes after Colorectal Cancer Surgery: A Retrospective Propensity-Score-matched Analysis

Abstract:BackgroundPerioperative anemia and transfusion are intertwined with each other, and both have adverse impacts on the survival of colorectal cancer (CRC) patients. But the treatment of anemia still relies on transfusion in several countries, which leads us to question the effects of anemia tolerance and transfusion on the long-term outcomes of CRC patients. We investigated the combined effect of preoperative anemia and postoperative anemia and of preoperative anemia and blood transfusion, which imposes a greater risk to survival, to compare the effects of anemia tolerance and transfusion on overall survival (OS) and disease-free survival (DFS) in patients undergoing CRC surgery.MethodsA retrospective propensity-score-matched analysis included patients with CRC undergoing elective surgery between January 1, 2008, and December 31, 2014. After propensity-score matching, Kaplan–Meier survival analysis and univariable and multivariable Cox proportional hazards models were used to study the prognostic factors for survivals. In univariate and multivariate Cox regression analysis, two novel models were built.ResultsOf the 8,121 patients with CRC, 1,975 (24.3%) and 6,146 (75.7%) patients presented with and without preoperative anemia, respectively. After matching, 1,690 patients remained in each group. In the preoperative anemia and postoperative anemia model, preoperative anemia and postoperative anemia was independent risk factor for OS (HR, 1.202; 95% CI, 1.043–1.385; P=0.011) and DFS (HR, 1.210; 95% CI, 1.050–1.395; P=0.008). In the preoperative anemia and transfusion model, preoperative anemia and transfused was the most dangerous independent prognostic factor for OS (HR, 1.791; 95% CI, 1.339–2.397; P<0.001) and DFS (HR, 1.857; 95% CI, 1.389–2.483; P<0.001). In patients with preoperative anemia, the OS and DFS of patients with transfusion were worse than those of patients without transfusion (P=0.026 in OS; P=0.037 in DFS).ConclusionsPreoperative anemia and blood transfusion imposed a greater risk to OS and DFS in patients undergoing CRC surgery, indicating that the harm associated with blood transfusion was greater than that associated with postoperative anemia. These findings should encourage clinicians to be vigilant for the timely prevention and treatment of anemia, by appropriately promoting toleration of anemia and restricting the use of blood transfusion in patients with CRC.

Abstract:There has been an accumulating body of studies provided to reveal that blood transfusion is more frequently and liberally administered to older than younger patients.Older patients have a higher transfusion rate than the younger;even they are just suspected but not diagnosed of co-morbidities.It is still under controversial whether older patients should adopt liberal or restrictive transfusion strategy.This review seeks to find a proper blood transfusion strategy for older patients by summarizing the current situation and progress of relevant studies in recent years,and analyzing the risks and complication of blood transfusion for older patients in different clinical settings.

Abstract:To analyze the status of clinical blood transfusion in tertiary hospitals in Shanghai and provide policy suggestions for reasonable blood transfusion.Methods: Blood transfusion information of 24 tertiary hospitals in 2010 was collected.The person-time,amount,age distribution and department distribution of blood transfusion were analyzed.Results: The blood transfusion proportions of type A,type B,type AB and type O were 30.77%,28.52%,9.79% and 30.92%,respectively.The proportions of blood consumption under the age of 18,19~40,41~59 and over the age of 60 were 3.69%,15.69%,36.61% and 44.01% respectively.In ingredient blood utilization,the proportions of the red blood cells consumption,platelets consumption and plasma consumption were 45.44%,35.13% and 15.80% respectively.Conclusion: The amount of blood transfusion in tertiary hospitals was big.The blood utilization of type O,type A and type B were higher,and elderly patients had the higher proportion of blood transfusion.

Abstract:Debate on appropriate triggers for transfusion of allogeneic blood products and their effects on short- and long-term survival in surgical and critically ill patients continue with no definitive evidence or decisive resolution. Although transfusion-related immune modulation (TRIM) is well established, its influence on immune competence in the recipient and its effects on cancer recurrence after a curative resection remains controversial. An association between perioperative transfusion of allogeneic blood products and risk for recurrence has been shown in colorectal cancer in randomized trials; whether the same is true for other types of cancer remains to be determined. This article focuses on the laboratory, animal, and clinical evidence to date on the mechanistic understanding of inflammatory and immune-modulatory effects of blood products and their significance for recurrence in the cancer surgical patient.
