Abstract:Objective To investigate the risk factors,etiology,clinical and imaging characteristics of anterior cerebral artery (ACA) territory infarction in Chinese patients.Methods The clinical and neuroradiological data of the patients with acute ACA territory infarction were reviewed.The risk factors, etiology,distributions of infarct lesions,and clinical manifestations of the patients were summarized.The distribution of infarct lesions and clinical manifestations of an embolization group (cardiogenic or carotid artery embolism) and an ACA atherosclerosis group were compared.Results A total of 44 patients were included and investigated,accounting for 4.9% of patients with acute ischemic stroke admitted during the same hospitalization period.The top three risk factors were hypertension (81.8%),hyperlipidemia (38.6%),and smoking (38.6%).The TOAST classification;cardioembolism (n=6;13.6%),largeartery atherosclerosis (n=33;75.0%),stroke of other determined etiology (n=2;4.5%),and cryptogenic stroke (n=3;6.8%).The single-infarcts were 10 (62.5%) and 3 (12.5%) respectively in the embolization group (16/44,36.4%) and the ACA atherosclerosis group (24/44,54.5%) (x~2=10.940, P0.001);the infarction involving the cortex was 14 (87.5%) and 11 (45.8%) respectively (x~2= 7.112,P0.01);the infarction involving the subcortical white matter was 2 (12.5%) and 16 (66.7%) respectively (x~2=11.381,P0.001);the infarction involving the corpus callosum was 6 (37.5%) and 19 (79.2%) respectively (x~2=7.112,P0.01);and the infarction involving the areas beyond ACA was 5 (31.3%) and 0(0) respectively (P=0.007).There were 11 patients (45.8%) with emotional disorder in the ACA atherosclerosis group,and 2 (12.5%) in the embolization group (x~2=4.862;P=0. 040).There was no significant differences in other clinical manifestations between the 2 groups (P0. 05).Conclusion ACA infarction is less common,hypertension is the most important risk factor,and atherosclerotic occlusive lesions of ACA is the chief cause.The distributions of atherosclerotic occlusive lesions of ACA caused infarct focus are significantly different from those of cardiogenic or carotid artery embolism, and it suggests that there are different mechanisms in ACA infarction.
