Abstract:In less than 2 decades, electronic health records (EHRs) have moved from a niche tool used by only a minority of hospitals and health care professionals to an inevitable norm; by 2021, nearly all hospitals and 80% of office-based physicians in the US had adopted an EHR system. 1 A major driver for this trend has been the promise of improving health care quality and safety by reducing medical errors. Much evidence supports this promise, as well as refutes it. Furthermore, the lived experiences of EHR users have been decidedly mixed. 2 EHR systems are complex, and so are the outcomes associated with their use. To be sure, EHRs have had some unintended consequences on health care. 3 Paper charts offered a variety of visual and tactile cues that may have subtly helped clinicians avoid safety threats, such as a wrong-patient error, but in the EHR, each chart is just a row in a patient list or a tab in the EHR window. 4 EHRs have also overhauled how communication happens in health care. Telephone calls and pagers have given way to secure chat messages electronically delivered to the clinician's mobile device. These innovations are not only complex but also carry unintended consequences yet to be discovered and studied. In this issue of JAMA Network Open , Lou et al 5 present their findings of an association between secure messaging use and wrong-patient order entry errors among clinicians in the inpatient setting. They used a previously established measure for estimating wrong-patient order entry errors, called the wrong-patient retract-and-reorder (RAR) method. 6 Although the RAR method has been used in several studies focused on measuring or reducing wrong-patient errors, it has a known limitation: It only measures self-intercepted wrong-patient order entry errors. Therefore, all other forms of wrong-patient errors (eg, those intercepted by another clinician, those that are not intercepted and reach the patient, or those that do not involve order entry) are not captured by RAR. The causal pathway for those errors may be similar to self-intercepted wrong-patient order entry errors, but from the standpoint of scientific rigor, findings from RAR event data should be generalized with caution to other forms of wrong-patient errors. Lou et al 5 used data from the EHR about the use of secure messaging to define the exposure variable. They studied the association between this exposure variable with the outcome variable of RAR event rate, and they accounted for some observable potential confounders to adjust their analyses. All the data used in their study came from the EHR audit logs; ie, the authors neither collected data directly from the users nor used data from other sources (eg, the safety reporting system) to corroborate or augment their analyses. What they found was that clinicians who used secure messaging more than their peers also had a modestly higher rate of RAR events than their peers. The study by Lou et al 5 is observational in nature and cannot establish causality, but they report a notable association nonetheless. This association remained consistent when they adjusted it for a few possible confounders, such as the clinician's role and age, or proxy variables representing the clinician's patient load. Notably, the association was stronger among attending physicians (who send and receive fewer secure messages in general) than among resident physicians (who use secure messaging more frequently). This finding does not seem to be associated with age. While attending physicians tend to be older than resident physicians and while the younger generation may be more tech savvy, the authors did not find age to be a significant confounder. It is plausible that clinicians who use secure messaging sporadically may be more susceptible to its associated distractions. Previous research on wrong-patient errors has indicated that distraction may be the predominant reason users cite for placing an order for the wrong patient. 6 Findings from a randomized study indicated that wrong-patient errors happen much more frequently in the emergency and inpatient settings, where distractions are more abundant compared with the outpatient setting. 7 These findings corroborate with the observations Lou et al 5 report about the association between secure messaging use and wrong-patient errors. However, there are important alternative explanations for the associations observed in this study. For instance, it is possible that the higher volume of secure messaging among clinicians with higher wrong-patient RAR rates may be partly due to other clinicians messaging them about having made a wrong-patient error. It is also plausible that wrong-patient errors may occur more frequently in complex care settings or for complex patients, and secure messaging may be more frequent in -Abstract Truncated-

Abstract:Objective: We evaluated nursing-related free-text communication orders to identify potential safety hazards and describe patterns and scope of care domains addressed that may reveal preventable workarounds and potential gaps in electronic health record (EHR) functionality. Materials and methods: A retrospective analysis of free-text EHR-based communication orders sent to or by nurses providing inpatient care at a major academic health system. Using built-in EHR tools and selection criteria, 13 193 orders were extracted, including 1373 unique orders. Using the Clinical Care Classification system standardized framework, we classified content by care domain and identified unique requests within each order. We reviewed each order for error-prone textual features based on standard patient safety guidance. We describe the distribution of domains, co-occurrence when 2 domains were present, and common patterns. Results: The 1373 unique orders included a single request (65.3%), 2 requests related to 1 or 2 domains (19%), or 3 or more requests (15.7%). No orders included terms on the Joint Commission's "Do Not Use" list. However, 13.6% of unique orders, and 16.7% of those related to medications, included error-prone symbols or abbreviations according to Institute for Safe Medication Practices guidance. Order content spanned 20 different care components but physical regulation, fluid volume, nutritional, safety, and medication were most frequently identified as single or co-occurring topics. Patterns were heterogenous. Discussion: Free-text communication orders reveal workarounds, responses to upstream workarounds, and design constraints that should be further investigated. Remediation strategies are needed to reduce safety hazards and workflow impediments. Conclusions: Analysis of free-text communication orders revealed opportunities for improvement.
Risk factors for wrong-patient medication orders in the emergency department

Abstract:Objectives: This paper investigates the risk factors for wrong-patient medication orders in an emergency department (ED) by studying intercepted ordering errors identified by the "retract-and-reorder" (RaR) metric (orders that were retracted and reordered for a different patient by the same provider within 10 min). Materials and methods: Medication ordering data of an academic ED were analyzed to identify RaR events. The association of RaR events with similarity of patient names and birthdates, matching sex, age difference, the month, weekday, and hour of the RaR event, the elapsed hours since ED shift start, and the proximity of exam rooms in the electronic medical record (EMR) dashboard's layout was evaluated. Results: Over 5 years (2017-2021), 1031 RaR events were identified among a total of 561 099 medication orders leading to a proportional incidence of 184 per 100 000 ED orders (95% CI: 172; 195). RaR orders were less likely to be performed by nurses compared to physicians (OR 0.54 [0.47; 0.61], P < .001). Furthermore, RaR pairs were more likely to have the same sex (OR 1.26 [95% CI 1.10; 1.43], P = .001) and the proximity of the exam rooms was closer (-0.62 [95% CI -0.77; -0.47], P = .001) compared to control pairs. Patients' names, birthdates, age, and the other factors showed no significant association (P > .005). Discussion and conclusion: This study found no significant influence from factors such as similarity of patient names, age, or birthdates. However, the proximity of exam rooms in the user interface of the EMR as well as patients' same sex emerged as risk factors.
Academic hospitalist perspectives on the benefits and challenges of secure messaging: A mixed methods analysis

Abstract:Background: Hospitals and patients rely on effective clinician communication. Asynchronous electronic secure messaging (SM) systems are a common way for hospitalists to communicate, but few studies have evaluated how hospitalists are navigating the adoption of SM and the benefits and challenges they are encountering. Objectives: The objective of this study is to assess academic hospitalist perspectives on SM to guide future research and quality improvement initiatives. Methods: This was a mixed methods study utilizing an embedded REDCap survey and six virtual semistructured focus groups. It took place during a Hospital Medicine ReEngineering Network Zoom meeting on October 13, 2023. Rapid qualitative methods were used to define major themes. Results: There were 28 hospitalists and one patient representative across 24 separate academic institutions. There was a 71% survey completion rate (N = 20). SM was felt to be an effective and efficient communication modality but was associated with a large amount of multitasking and interruptions. Perspectives around SM clustered around three main themes: SM has been widely but variably adopted; there is a lack of institutional guidance about how to best engage with SM; and SM is changing the landscape of hospitalist work by increasing ease but decreasing depth of communication, increasing cognitive load, and changing interpersonal relationships. Recommendations for SM improvements included the need for institutions to work with frontline workers to develop and implement clear usage guidelines. Conclusion: SM is likely contributing to both positive and negative effects for clinicians and patients. Understanding hospitalist perspectives on SM will help guide future research and quality improvement initiatives.

Abstract:This study explores the uptake of secure messaging technology and the influence it has on the work of three interconnected occupational roles. It was executed in an outpatient clinic, where the administrative staff adopted the new secure messaging tool, expecting it to improve efficiency. The study found that the efficiency-enhancing tool over time introduced new inefficiencies into the work of administrative assistants, nurses, and physicians, and this phenomenon is noted as the efficiency paradox , which is examined and explained throughout the study. This research contributes to health information technology and role reconfiguration literature by highlighting the crucial role of emotions in this emergent and relational process and discussing the implications of that in the context of patient-provider communication.

Frequency and Types of Patient-Reported Errors in Electronic Health Record Ambulatory Care Notes

Abstract:Importance: As health information transparency increases, patients more often seek their health data. More than 44 million patients in the US can now readily access their ambulatory visit notes online, and the practice is increasing abroad. Few studies have assessed documentation errors that patients identify in their notes and how these may inform patient engagement and safety strategies. Objective: To assess the frequency and types of errors identified by patients who read open ambulatory visit notes. Design, setting, and participants: In this survey study, a total of 136 815 patients at 3 US health care organizations with open notes, including 79 academic and community ambulatory care practices, received invitations to an online survey from June 5 to October 20, 2017. Patients who had at least 1 ambulatory note and had logged onto the portal at least once in the past 12 months were included. Data analysis was performed from July 3, 2018, to April 27, 2020. Exposures: Access to ambulatory care open notes through patient portals for up to 7 years (2010-2017). Main outcomes and measures: Proportion of patients reporting a mistake and how serious they perceived the mistake to be, factors associated with finding errors characterized by patients as serious, and categories of patient-reported errors. Results: Of 136 815 patients who received survey invitations, 29 656 (21.7%) responded and 22 889 patients (mean [SD] age, 55.16 [15.96] years; 14 447 [63.1%] female; 18 301 [80.0%] white) read 1 or more notes in the past 12 months and completed error questions. Of these patients, 4830 (21.1%) reported a perceived mistake and 2043 (42.3%) reported that the mistake was serious (somewhat serious: 1563 [32.4%]; very serious: 480 [9.9%]). In multivariable analysis, female patients (relative risk [RR], 1.79; 95% CI, 1.72-1.85), more educated patients (RR, 1.38; 95% CI, 1.29-1.48), sicker patients (RR, 1.89; 95% CI, 1.84-1.94), those aged 45 to 64 years (RR, 2.23; 95% CI, 2.06-2.42), those 65 years or older (RR, 2.00; 95% CI, 1.73-2.32), and those who read more than 1 note (2-3 notes: RR, 1.82; 95% CI, 1.34-2.47; ≥4 notes: RR, 3.09; 95% CI, 2.02-4.73) were more likely to report a mistake that they found to be serious compared with their reference groups. After categorization of patient-reported very serious mistakes, those specifically mentioning the word diagnosis or describing a specific error in current or past diagnoses were most common (98 of 356 [27.5%]), followed by inaccurate medical history (85 of 356 [23.9%]), medications or allergies (50 of 356 [14.0%]), and tests, procedures, or results (30 of 356 [8.4%]). A total of 23 (6.5%) reflected notes reportedly written on the wrong patient. Of 433 very serious errors, 255 (58.9%) included at least 1 perceived error potentially associated with the diagnostic process (eg, history, physical examination, tests, referrals, and communication). Conclusions and relevance: In this study, patients who read ambulatory notes online perceived mistakes, a substantial proportion of which they found to be serious. Older and sicker patients were twice as likely to report a serious error compared with younger and healthier patients, indicating important safety and quality implications. Sharing notes with patients may help engage them to improve record accuracy and health care safety together with practitioners.

Abstract:Previous reviews have examined evidence of the impact of CPOE on medication errors, but have used highly variable definitions of "error". We attempted to answer a very focused question, namely, what evidence exists that CPOE systems reduce prescribing errors among hospital inpatients? We identified 13 papers (reporting 12 studies) published between 1998 and 2007. Nine demonstrated a significant reduction in prescribing error rates for all or some drug types. Few studies examined changes in error severity, but minor errors were most often reported as decreasing. Several studies reported increases in the rate of duplicate orders and failures to discontinue drugs, often attributed to inappropriate selection from a dropdown menu or to an inability to view all active medication orders concurrently. The evidence-base reporting the effectiveness of CPOE to reduce prescribing errors is not compelling and is limited by modest study sample sizes and designs. Future studies should include larger samples including multiple sites, controlled study designs, and standardized error and severity reporting. The role of decision support in minimizing severe prescribing error rates also requires investigation.

Evaluation of Secure Messaging Applications for a Health Care System: A Case Study

Abstract:Objective: The use of text messaging in clinical care has become ubiquitous. Due to security and privacy concerns, many hospital systems are evaluating secure text messaging applications. This paper highlights our evaluation process, and offers an overview of secure messaging functionalities, as well as a framework for how to evaluate such applications. Methods: Application functionalities were gathered through literature review, Web sites, speaking with representatives, demonstrations, and use cases. Based on similar levels of functionalities, vendors were grouped into three tiers. Essential and secondary functionalities for our health system were defined to help narrow our vendor choices. Results: We stratified 19 secure messaging vendors into three tiers: basic secure communication, secure communication within an existing clinical application, and dedicated communication and collaboration systems. Our essential requirements revolved around functionalities to enhance security and communication, while advanced functionalities were mostly considered secondary. We then narrowed our list of 19 vendors to four, then created clinical use cases to rank the final vendors. Discussion: When evaluating a secure messaging application, numerous factors must be considered in parallel. These include: what clinical processes to improve, archiving text messages, mobile device management, bring your own device policy, and Wi-Fi architecture. Conclusion: Secure messaging applications provide a Health Insurance Portability and Accountability Act (HIPAA) compliant communication platform, and also include functionality to improve clinical collaboration and workflow. We hope that our evaluation framework can be used by other health systems to find a secure messaging application that meets their needs.

Abstract:Patient portal secure messaging (PPSM) systems, enable direct communication between patients and providers. Studies on patients' experiences with PPSM have reported positive outcomes. However, mixed results have been published on physician user experiences with concerns about increased workload and negative impact on physician wellness. With the expanding scope and increasing demand for gastroenterology (GI) procedures, GI providers face unique barriers to managing PPSM messages when compared to other less procedure-intensive specialties. This could negatively impact wellness and contribute to GI provider burnout. While previous studies have evaluated PPSM use among general physicians, none have specifically investigated its use among GI providers. This study aims to assess PPSM workload, management and impact on wellness among GI providers.

Abstract:OBJECTIVEErrors of omission are a common cause of systems failures. Physicians often fail to order tests or treatments needed to monitor/ameliorate the effects of other tests or treatments. The authors hypothesized that automated, guideline-based reminders to physicians, provided as they wrote orders, could reduce these omissions.DESIGNThe study was performed on the inpatient general medicine ward of a public teaching hospital. Faculty and housestaff from the Indiana University School of Medicine, who used computer workstations to write orders, were randomized to intervention and control groups. As intervention physicians wrote orders for 1 of 87 selected tests or treatments, the computer suggested corollary orders needed to detect or ameliorate adverse reactions to the trigger orders. The physicians could accept or reject these suggestions.RESULTSDuring the 6-month trial, reminders about corollary orders were presented to 48 intervention physicians and withheld from 41 control physicians. Intervention physicians ordered the suggested corollary orders in 46.3% of instances when they received a reminder, compared with 21.9% compliance by control physicians (p < 0.0001). Physicians discriminated in their acceptance of suggested orders, readily accepting some while rejecting others. There were one third fewer interventions initiated by pharmacists with physicians in the intervention than control groups.CONCLUSIONThis study demonstrates that physician workstations, linked to a comprehensive electronic medical record, can be an efficient means for decreasing errors of omissions and improving adherence to practice guidelines.

Abstract:Introduction Electronic messages are growing as an important form of patient-provider communication, particularly in the primary care setting. However, adoption of healthcare technology has been under-utilized by underserved patient populations. The purpose of this study was to describe how adoption and utilization of electronic messaging occurred within a large primary care urban-based patient population. Methods In this retrospective study, the frequency of electronic messages initiated by adult outpatient primary care patients was observed. Patients were classified as either non-portal adopters, non-message utilizers, low message utilizers, and high message utilizers. Logistic regression modeling was used to compare factors associated with message utilization rates to determine disparities in access. Results Among a sample of 27,453 ethnically diverse adult patients from the Houston, Texas Metropolitan area, 33,497 unique messages were sent (1.22 messages/patient). Message burden was predominantly derived by a small number of high utilizers (individuals who sent 3 or more messages), who comprised 15.7 % of the study population (n = 4302) but accounted for 77 % of the message volume (n = 25,776). These high utilizers were typically older, White, English speaking, from middle to upper income zip codes, had higher number of comorbidities, and a higher number of clinical visits. Conclusions Most inbox messages were generated by a small number of patients. While it was reassuring to see older and sicker individuals utilizing electronic messaging, patients from minority and/or lower income background utilized electronic messaging much less. This may propagate systematic bias and decrease the level of care for traditionally underserved patients.

Abstract:Background: Medication errors, adverse drug events and potential adverse drug events are common and serious in terms of the harms and costs that they impose on the health system and those who use it. Errors resulting in preventable adverse drug events have been shown to occur most often at the stages of ordering and administration. This paper describes the protocol for a pragmatic trial of electronic prescribing to reduce prescription error. The trial was designed to overcome the limitations associated with traditional study design. Design: This study was designed as a 65-week, cluster randomized, parallel study. Methods: The trial was conducted within ambulatory outpatient clinics in an academic tertiary care centre in Ontario, Canada. The electronic prescribing software for the study is a Canadian electronic prescribing software package which provides physician prescription entry with decision support at the point of care. Using a handheld computer (PDA) the physician selects medications using an error minimising menu-based pick list from a comprehensive drug database, create specific prescription instructions and then transmit the prescription directly and electronically to a participating pharmacy via facsimile or to the physician's printer using local area wireless technology. The unit of allocation and randomization is by 'week', i.e. the system is "on" or "off" according to the randomization scheme and the unit of analysis is the prescription, with adjustment for clustering of patients within practitioners. Discussion: This paper describes the protocol for a pragmatic cluster randomized trial of point-of-care electronic prescribing, which was specifically designed to overcome the limitations associated with traditional study design. Physician-Nurse Communication Surrounding Computerized Physician Order Entry Systems From Social and Technical Perspective: An Ethnographic Study

Abstract:Although computerized physician order entry systems improve order transmission and patient safety, overdependence on these systems can impede users' communication. This ethnographic study explored physician-nurse communication surrounding computerized physician order entry systems using a sociotechnical framework. Fieldwork conducted in a tertiary teaching hospital comprised 89 hours of participant observation, and individual semistructured interviews were held with seven nurses and five physicians. In addition, documents and artifacts were collected. Three core themes emerged. First, computerized physician order entry quality-related issues undermined the work efficiency of physicians and nurses. Specifically, usability was error prone because of cognitive overload, and the system was unable to perform relevant traces and raise alerts, demonstrating poor interoperability. Second, social factors, including insufficient training, unclear responsibilities, and a lack of awareness concerning interdisciplinary communication, compounded communication problems. Last, environmental factors, including noncoterminous spaces and times and insufficient technical support, impeded the resolution of communication problems. Technical and social contextual factors relating to computerized physician order entry systems jointly affected physician-nurse communication. Cognitive issues and insufficient alerts impacted work efficiency the most and were compounded by contextual individual- and team-related factors and environmental factors. Therefore, improved functions of computerized physician order entry systems and interprofessional communication training are required to optimize technical and social aspects of physician-nurse communication.
Distractions During Patient Handoff: The Application-Based Messaging Volume on General Surgery Interns

Abstract:Objective: Mobile phone-based paging systems have become increasingly common for communication within hospitals. Surgical interns receive the most pages, and our aim is to objectively quantify and evaluate this burden to allow for targeted improvement. Design: We performed a retrospective review of our institutions mobile phone-based paging system data (Halo Health, Cincinnati, OH) from July 2019 to September 2019. Setting: Carolinas Medical Center, Charlotte, NC, USA. Participants: Seven general surgery postgraduate year (PGY) 1 residents. Results: Forty-five thousand eight hundred and one messages met inclusion criteria, with 27,397 messages received and 18,404 sent. PGY 1 residents each received an average of 48 ± 41 messages per shift, with 8 ± 17 messages per day while off-duty. Night shifts averaged more messages than day shifts (80 ± 39 vs 38 ± 32, p < 0.0001), and had more shifts with high message volume (30% vs 11%, p = 0.0005). Evaluating the total number of messages received per minute of the day, the largest number of high-volume message intervals (21) occurred during patient handoff (1700-1900 hours). Most messages were sent by nursing staff (55.8%), followed by medical providers (38.2%). Conclusions: PGY 1 residents receive a large number of pages using a messaging application, with many occurring at critical times. Residents received a higher volume of pages on night shifts, during patient handoff, and while off-duty. Since most pages are from nursing staff, targeted education and preventative actions may help decrease the volume of pages during these critical times.
Secure Messaging: Demonstration and Enrollment Patient Portal Program

Abstract:Vulnerable populations face challenges gaining access to quality healthcare, which places them at a high risk for poor health outcomes. Using patient portals and secure messaging can improve patient activation, access to care, patient follow-up adherence, and health outcomes. Developing and testing quality improvement strategies to help reduce disparities is vital to ensure patient portals benefit all, especially vulnerable populations. This quality improvement initiative aimed to increase enrollment in a patient portal, use secure messages, and adhere to follow-up appointments. Before the project, no patients were enrolled in the portal at this practice site. Over 8 weeks, 61% of invited patients were enrolled in the patient portal. Eighty-five percent were Medicaid recipients, and the others were underinsured. Eight patients utilized the portal for secure messaging. The follow-up appointment attendance rate was better in the enrolled patients than in those who did not enroll. The majority of survey respondents reported satisfaction in using the patient portal. Patient portal utilization and adoption in vulnerable groups can improve when a one-on-one, hands-on demonstration and technical assistance are provided.

Abstract:Electronic health record adoption has produced tremendous growth in patient-physician inbox messages, 1 which accelerated during the COVID-19 pandemic, 2 ,3 with some health systems instituting charges for this mounting clinical burden. 4 ,5 However, message content across populations is poorly understood, limiting systematic approaches that can improve message-related workloads. The Kaiser Permanente Northern California (KPNC) Research Determination Office deemed this quality improvement study to be exempt from review and did not require informed consent because it was not human participants research. We followed the STROBE reporting guideline. In 2019, KPNC initiated the Desktop Medicine Program to operationalize a solution to address messaging volume and expeditiously direct patients to the optimal person, information, or service (eAppendix in Supplement 1). Program development integrated regional clinician teams and real-time natural language processing (NLP) algorithms to label message content and sort messages to appropriate roles (eg, teleservice representatives, medical assistants, pharmacists, physicians) before reaching individual physician inboxes. Category labels were added to the NLP output when specific Desktop Medicine response workflows were in place. The NLP pipeline was trained on a corpus of more than 20 000 messages annotated by nurses with triage experience (eAppendix in Supplement 1) and integrated within the electronic health record in May 2022 with bimonthly programmatic updates. The program initially focused on adult and family medicine messages and subsequently expanded to pediatric messages in October 2022. We examined the content and variability in message labels for adult and family medicine and pediatric physicians between April 1 and August 31, 2023, with descriptive statistics and coefficient of variation and quantified the association with physician inbox volume. Statistical analysis was performed using Python version 3.10.8 (Python Software Foundation), Pandas package version 1.5.3, and Microsoft Excel. There were 4 709 261 patient messages during the study period, of which 3 655 065 (77.6%]) received at least 1 label, most commonly for medications (1 546 411 [32.8%]), skin conditions (438 631 [9.3%]), messages with attachments (389 372 [8.3%]), and emergent content for expedited review (359 144 [7.6%]) (Table). Overall, 1 374 309 messages (29.2%) included at least 2 labels, most commonly including skin conditions with medications (179 707 [4.1%]), skin conditions with attachments (171 955 [4.0%]), and emergent content with medications (148 357 [3.4%]). Weekly label variability (Figure) was lowest for controlled substances (coefficient of variation: 4.2 [95% CI, 3.2-6.1]), medication (4.5 [95% CI, 3.4-6.5]), neck and back pain (4.7 [95% CI, 3.6-6.8]), and erectile dysfunction (5.3 [95% CI, 4.1-7.6]) and highest for influenza vaccine (96.4 [95% CI, 73.8-139.3]), Mpox (43.1 [95% CI, 33.0-62.2]), and COVID-19 vaccine (35.2 [95% CI, 26.9-50.8]). Of 4 835 200 messages evaluated by regional Desktop Medicine staff, 1 541 657 (31.9%) were resolved by regional medical assistants or teleservice representatives, physicians, and pharmacists before reaching individual physician inboxes. This study including more than 4 million patient messages found novel insights that inform health system approaches for addressing the high volume of patient-physician electronic communications in a timely and effective fashion. First, identifying common and recurring categories (eg, medication-related or skin condition–related labels) across many physicians informs the development of population-level approaches for reducing individual physician inbox message volume. Second, quantifying the variability in message categories highlights acute changes in specific categories that can be used to identify emerging trends and facilitate rapid operational responses (eg, COVID-19, influenza, or Mpox). Third, real-time message analysis, labeling content before reaching a physician inbox for expedited regional review, can shorten the time to clinical assessment for potentially emergent conditions. Additionally, although most messages were felt to be most appropriately addressed by the intended physician recipient, the program was effective at resolving more than 1.5 million patient messages, accounting for nearly one-third of overall volume. Study limitations included that the findings were from a single health care system and that the program did not include newer large language models (eg, GPT-4), 6 which could improve content labeling and responses. Future enhancements are also needed to effectively address messages with multiple topics and those with no category labels. Accuracy of medication histories derived from an Australian cloud-based repository of prescribed and dispensed medication records

Abstract:Background: Obtaining accurate medication histories at transitions of care is challenging, but important for patient safety. Prescription exchange services (PES) securely transfer electronic prescription and dispensing records between prescribers and pharmacies, which is potentially useful data for determining medication histories. Aim: To evaluate the accuracy of PES-derived medication histories. Methods: Prospective observational study, at two Australian tertiary-referral health services. A convenience sample of adult inpatients was recruited. The main outcome measure was: proportion of patients with ≥1 errors in their PES-derived pre-admission medication histories, compared with gold-standard best-possible medication histories, including prescribed and non-prescribed medications, obtained by pharmacists using multiple sources including patient/carer interview. Results: Of 154 patients (median age 76 years; interquartile range (IQR) 64-84 years; median 10.0 pre-admission medications; IQR 6.0-14.0), 153 (99.4%) had ≥1 errors in their PES-derived medication history (median 6.0 errors per patient; IQR 4.0-9.0). Excluding when-required medications, 146 (94.8%) patients had >1 errors (median 4.0 errors per patient; IQR 2.0-6.0). Omission was the most common error, affecting 549 (33.3%) of 1648 current medications (median 3.0; IQR 1.0-5.0 per patient); 396 (72.1%) omissions were over-the-counter medicines. Dose-regimen errors affected 276 (25.1%) of 1099 current medications captured in PES-derived medication histories (median 1.0 error per patient; IQR 0.0-3.0). Commission errors (medications in PES-derived histories that were not current) affected 224 (16.9%) of 1323 medications (median 1.0 error per patient; IQR 1.0-2.0). Conclusions: Medication histories derived solely from a cloud-based medication record repository had a high error rate compared with patients' actual medication use. Like all medication history sources, data from cloud-based repositories need to be verified with additional sources including the patient and/or their carer.
