Abstract:In global health, a discipline with a racist and colonial history, white supremacy, white privilege and racism are still present today.1 Although many believe we are witnessing a resurgence of racism in global health, because of a recent rise of extreme right comments and racism in the community and online2, 3. In reality racism has always been an inherent aspect of global health and its predecessors: tropical health and international health by prioritizing the health issues of the colonizers over those of the native populations.4 As such, we are rather bearing witness to long standing issues that have been persistently overlooked. There is a need for a paradigm shift to enable true authentic leadership that promotes the values of our shared humanity.

Abstract:ObjectivesRecently, public health has acknowledged racism as a social determinant of health. Much evidence exists on the impact of individual-level racism and discrimination, with little to no examination of racism from the standpoint of systems and structures. The purpose of this systematic literature review is to analyze the extent to which public health currently addresses systemic racism in the published literature.MethodsUtilizing the PRISMA guidelines, this review examines three widely used databases to examine published literature covering the topic as well as implications for future research and practice.ResultsA total of 85 articles were included in the review analysis after meeting study criteria.ConclusionsAcross numerous articles, the terms racism and systemic racism are largely absent. A critical need exists for an examination of the historical impact of systemic racism on the social determinants of health and health of marginalized populations.

Abstract:Racism, xenophobia, and discrimination are key determinants of health and equity and must be addressed for improved health outcomes. We conclude that far broader, deeper, transformative action is needed compared with current measures to tackle adverse effects of racism on health. To challenge the structural drivers of racism and xenophobia, anti-racist action and other wider measures that target determinants should implement an intersectional approach to effectively address the causes and consequences of racism within a population. Structurally, legal instruments and human rights law provide a robust framework to challenge the pervasive drivers of disadvantage linked to caste, ethnicity, Indigeneity, migratory status, race, religion, and skin colour. Actions need to consider the historical, economic, and political contexts in which the effects of racism, xenophobia, and discrimination affect health. We propose several specific actions: a commission that explores how we action the approaches laid out in this paper; building a conversation and a series of events with international multilateral agency stakeholders to raise the issue and profile of racism, xenophobia, and discrimination within health; and using our multiple platforms to build coalitions, expand knowledge, highlight inequities, and advocate for change across the world.

Abstract:This article reviews the scientific research that indicates that despite marked declines in public support for negative racial attitudes in the United States, racism, in its multiple forms, remains embedded in American society. The focus of the article is on the review of empirical research that suggests that racism adversely affects the health of nondominant racial populations in multiple ways. First, institutional racism developed policies and procedures that have reduced access to housing, neighborhood and educational quality, employment opportunities, and other desirable resources in society. Second, cultural racism, at the societal and individual level, negatively affects economic status and health by creating a policy environment hostile to egalitarian policies, triggering negative stereotypes and discrimination that are pathogenic and fostering health-damaging psychological responses, such as stereotype threat and internalized racism. Finally, a large and growing body of evidence indicates that experiences of racial discrimination are an important type of psychosocial stressor that can lead to adverse changes in health status and altered behavioral patterns that increase health risks.

Abstract:Environmental health research in the US has shown that racial and ethnic minorities and members of low-socioeconomic groups, are disproportionately burdened by harmful environmental exposures, in their homes, workplace, and neighborhood environments that impact their overall health and well-being. Systemic racism is a fundamental cause of these disproportionate exposures and associated health effects. To invigorate and inform current efforts on environmental justice and to raise awareness of environmental racism, the National Institute of Environmental Health Sciences (NIEHS) hosted a workshop where community leaders, academic researchers, and NIEHS staff shared perspectives and discussed ways to inform future work to address health disparities.

Abstract:This paper provides an overview of United States-based research on the ways in which racism can affect mental health. It describes changes in racial attitudes over time, the persistence of negative racial stereotypes and the ways in which negative beliefs were incorporated into societal policies and institutions. It then reviews the available scientific evidence that suggests that racism can adversely affect mental health status in at least three ways. First, racism in societal institutions can lead to truncated socioeconomic mobility, differential access to desirable resources, and poor living conditions that can adversely affect mental health. Second, experiences of discrimination can induce physiological and psychological reactions that can lead to adverse changes in mental health status. Third, in race-conscious societies, the acceptance of negative cultural stereotypes can lead to unfavorable self-evaluations that have deleterious effects on psychological well-being. Research directions are outlined.

Abstract:Black individuals in the USA experience disparities in mental health that lead to unfavorable health outcomes and increased morbidity from mental illness due to centuries of racism. We emphasize the need to understand the roots of racial injustice to achieve racial equity. Historical factors such as European imperialism, enslavement, the myth of Black inferiority, and scientific racial classification have all perpetuated disparities, leading to the current underestimation, misdiagnosis, and inadequate treatment of mental illness in Black populations. Many of the issues discussed herein apply to Black people globally; however, our focus is on Black Americans and the inequities that result from the current US mental health system. We discuss the limitations of using the DSM-5 classification system and common epidemiological surveys, which do not capture or call for a comprehensive analysis of the systems producing mental health issues, to understand mental illness among Black Americans.

Abstract:Review of General Psychology, Ahead of Print. Systemic racism is a public health crisis that needs to be urgently and effectively addressed. Historical racism, contemporary biases, and prejudicial attitudes have persistently affected and damaged the mental health of people of color (POC) throughout the world. Although the 2021 apology from American Psychological Association (APA) acknowledged the profession's contribution to racism, much more work needs to be done to rectify and address past and current harm. A comprehensive, actionable, and concrete plan is needed to catalyze change. Adopting a socioecological approach can help tackle the multiple levels in which discrimination and unequal treatment take place. This necessitates more than just addressing the social determinants of illness, but also transforming the cultural competence of providers, organizations, and the mental health delivery system. Key to improving outcomes for POC domestically and abroad is increasing the affordability and access to care, diversifying and improving the cultural competence of the mental health workforce, and developing culturally adapted and effective care tailored to meet the culture-specific needs of diverse communities. Progress can only happen through proactive action and utilization of an anti-racist stance that commits significant financial and scientific resources to promote institutional, structural, systemic, and policy changes.

Abstract:Abstract Systemic racism is a scientifically tractable phenomenon, urgent for cognitive scientists to address. This tutorial reviews the built-in systems that undermine life opportunities and outcomes by racial category, with a focus on challenges to Black Americans. From American colonial history, explicit practices and policies reinforced disadvantage across all domains of life, beginning with slavery, and continuing with vastly subordinated status. Racially segregated housing creates racial isolation, with disproportionate costs to Black Americans’ opportunities, networks, education, wealth, health, and legal treatment. These institutional and societal systems build-in individual bias and racialized interactions, resulting in systemic racism. Unconscious inferences, empirically established from perceptions onward, demonstrate non-Black Americans’ inbuilt associations: pairing Black Americans with negative valences, criminal stereotypes, and low status, including animal rather than human . Implicit racial biases (improving only slightly over time) imbed within non-Black individuals’ systems of racialized beliefs, judgments, and affect that predict racialized behavior. Interracial interactions likewise convey disrespect and distrust. These systematic individual and interpersonal patterns continue partly due to non-Black people’s inexperience with Black Americans and reliance on societal caricatures. Despite systemic challenges, Black Americans are more diverse now than ever, due to resilience (many succeeding against the odds), immigration (producing varied backgrounds), and intermarriage (increasing the multiracial proportion of the population). Intergroup contact can foreground Black diversity, resisting systemic racism, but White advantages persist in all economic, political, and social domains. Cognitive science has an opportunity: to include in its study of the mind the distortions of reality about individual humans and their social groups.

Abstract:Increasing empirical evidence implicates structural factors as the fundamental cause of health inequities and resultant poor health outcomes among historically excluded racial and ethnic populations. 1 The legacy of racist sociopolitical structures and their impacts among racial and ethnic minoritized populations with substance use disorder (SUD) can no longer be ignored. Racial and ethnic minoritized individuals with SUD have endured othering, discrimination, and stigmatization over several decades and, recently, alarming death rates during the COVID-19 pandemic. 2 The pandemic and the murder of George Floyd provided the social disruption that resulted in a paradigm shift in public discourse, uncovering what has been known for decades—the inequitable state of the US health care system, characterized by unequal distribution of resources and differential treatment of people based on their socioeconomic status as they interface with society's normative hierarchies. 3 ,4

Abstract:The part played by racism in COVID-19 deaths is receiving considerable attention.1 The UK Government Race Disparity Unit report released on 22 October 20202 with Dr Raghib Ali, senior advisor, stating that they '… ruled out structural racism as a driving factor behind BAME people's poorer health outcomes after catching Covid, saying there was "no evidence" to support the theory'. Five days later, the UK Labour Party released a new report on the disproportionate impact of the coronavirus crisis on minority ethnic communities3 stating structural racism had played a part in the disproportionate impact of COVID-19 on Black, Asian and Minority Ethnic communities. It is clear that racism has become an ideological battleground in which the present UK government seeks to deny its existence, while using 'race' as a classificatory category. Further evidence for this contradictory stance is the equalities minister questioning the validity of unconscious bias training and the application of critical race theory (which highlights structural racism and inequality). Another concern is over the selection of David Goodhart as a commissioner of the Equality and Human Rights Commission. Goodhart is a supporter of the Home Office's hostile environment policy and someone who has advocated for 'white self-interest'.4 This appointment is yet another example of a government relying on race to maintain and promote systems of oppression while simultaneously denying the full ramifications of structural racism.5

Mental health care among blacks in America: Confronting racism and constructing solutions

Abstract:Objectives: To describe reasons for unmet need for mental health care among blacks, identify factors associated with causes of unmet need, examine racism as a context of unmet need, and construct ways to improve service use. Data sources: Data from the 2011-2015 National Survey on Drug Use and Health were pooled to create an analytic sample of black adults with unmet mental health need (N = 1237). Qualitative data came from focus groups (N = 30) recruited through purposive sampling. Study design: Using sequential mixed methods, reasons for unmet need were regressed on sociodemographic, economic, and health characteristics of respondents. Findings were further explored in focus groups. Principal findings: Higher education was associated with greater odds of reporting stigma and minimization of symptoms as reasons for unmet need. The fear of discrimination based on race and on mental illness was exacerbated among college-educated blacks. Racism causes mistrust in mental health service systems. Participants expressed the importance of anti-racism education and community-driven practice in reducing unmet need. Conclusion: Mental health systems should confront racism and engage the historical and contemporary racial contexts within which black people experience mental health problems. Critical self-reflection at the individual level and racial equity analysis at the organizational level are critical.
White supremacy and the racial logic of the global preventing and countering violent extremism agenda

Abstract:This article analyses what the disavowal of abject forms of white supremacy reveals about the racial logic of the global preventing and countering violent extremism (P/CVE) agenda. We argue that the global P/CVE agenda is built on racialised concepts such as prevention, radicalisation and community - concepts that render it incommensurate with the newly identified problem of white supremacist violent extremism or domestic terrorism. Through analysis of interviews with experts and practitioners working within the broad field of P/CVE, we discursively analyse how the enmeshment of the agenda within the development and peacebuilding space exposes the agenda's primary intent to manage presumably ungovernable populations in or from the so-called Global South. Taking the theoretical insights culled from textual analysis of practitioner interviews, we then consider the inclusion of right-wing extremism, and specifically white supremacy, within Western states' domestic P/CVE agendas, primarily in the US. Our argument - that the move to consider far-right extremism within domestic CVE policy reveals rather than disrupts the P/CVE agenda's racist foundations and intentions - contributes to a growing body of research that insists on attending to race, racialisation and racism within security studies and international relations, and which includes an emphasis on whiteness as an organising principle.

Abstract:Introduction: Two years since the murder of George Floyd, there has been unprecedented attention to racial justice by global public health organisations. Still, there is scepticism that attention alone will lead to real change. Methods: We identified the highest-ranked 15 public health universities, academic journals and funding agencies, and used a standardised data extraction template to analyse the organisation's governance structures, leadership dynamics and public statements on antiracism since 1 May 2020. Results: We found that the majority of organisations (26/45) have not made any public statements in response to calls for antiracism actions, and that decision-making bodies are still lacking diversity and representation from the majority of the world's population. Of those organisations that have made public statements (19/45), we identified seven types of commitments including policy change, financial resources, education and training. Most commitments were not accompanied by accountability measures, such as setting goals or developing metrics of progress, which raises concerns about how antiracism commitments are being tracked, as well as how they can be translated into tangible action. Conclusion: The absence of any kind of public statement paired with the greater lack of commitments and accountability measures calls into question whether leading public health organisations are concretely committed to racial justice and antiracism reform.

Abstract:The term "decolonization" has been increasingly used to refer to the elimination of the colonial experience and its legacy. However, the use of this overarching term masks the real root of the problem. European countries, whose populations are majority white, used their assumed supremacy as justification for the colonization of current low- and middle-income countries (LMICs) where the majority of non-white people live. This clear overlap between geographic and skin color differences explains how the white supremacy ideology triggered European colonization. Therefore, calls to decolonize global health education must focus on the roots of colonization and fight for the elimination of white supremacy ideology that is one of the pillars of the current ills of our global health architecture. A step in this process acknowledging the expertise that emerges from LMICs, alongside challenging the traditional high-income country (HIC) hegemony over knowledge and strengthening universities in LMICs to provide quality medical and global health education. Additionally, we also need to reevaluate curricula, research selection, and design as well as partnerships. Students need to be equipped with the skills to question norms and contribute to the creation of equitable, mutually beneficial partnerships. This needs to accompanied by the adoption of transdisciplinary education to address critical societal challenges. By challenging the white supremacy ideology, we can shift the center of gravity in global health to respect the right to equal say in education and research according to the disease burden and the distribution of the world population.

Abstract:Utilizing the work of Wilfred Bion, Harry Stack Sullivan, and other theorists and practitioners, as well as philosophers and students of race in America, this article argues that racism/white supremacy sabotages our work, and yet is fundamental to our way of doing, being, and thinking in our society. As a result of the centrality of racism/white supremacy, the author lays out four challenges which, if met, will aid in overcoming racism/white supremacy: (a) the first challenge of denial, (b) the second challenge of impaired empathy (and the failure to think), (c) the experience of shame, and (d) the denial of death. The article discusses each of these and concludes with a word on the possibility of passion—if we are able to overcome racism/white supremacy.

Abstract:Global Health Promotion, Ahead of Print. The Covid-19 pandemic reified pre-existing inequalities predicated on anti-Black racism, imperial geographical cartography, and the violent language of biomilitarism. In this reflective essay I deploy tools of historical sociology to underscore the importance of race, racism, racialization, and global responses to pandemics. I considerer the following questions. First, how can world society develop ideas and concepts for the imagination of a post-imperial global health regime? Second, can alternative futures be imagined if the monopolistic control of power, global scientific processes and knowledge regime is framed around a problematic lexicography of a Eurocentric totalizing project of being human? Lastly, if there is a scientific consensus that we need alternative futures, what kinds of knowledge is needed to bring about a post-imperial liberated order? The future of global health regime is a decolonial one predicated on a new biopolitics. I provide four paradigmatic approaches to subvert imperial global health: (i) pivoting ecocide in the imperial global health regime; (ii) abandonment of a Eurocentric conceptualization of racial hierarchy and modernity; (iii) disbanding the commodification of public health; and (iv) organizing a new world order through health reparations.
