Abstract:OBJECTIVETo evaluate the quality of life (QOL) of patients with chronic obstructive pulmonary disease (COPD) and correlated factors.METHODSData of 20 245 patients with COPD were collected from the cross-sectional survey of COPD, which was conducted between 2002 and 2004 in urban and rural areas of Beijing, Shanghai, Guangdong, Liaoning, Tianjin, Chongqing and Shanxi for residents aged over 40 years old. The recruited populations were interviewed with questionnaire and tested for spirometry. The quality of life was assessed with 12-item short-form health survey questionnaire (SF-12). Those with less than 70% of post-bronchodilator FEV(1)/FVC were identified as having COPD. The differences between groups in SF-12 scores converted by rank were compared using general linear model. Stepwise multiple linear regressions were conducted to study the main determinants of QOL.RESULTSCompared to subjects without COPD, those with COPD had impaired QOL (56 +/- 7 vs. 57 +/- 6 in mental component scores, F = 4.442, P < 0.05; 46 +/- 9 vs. 50 +/- 6 in physical component scores, F = 453.960, P < 0.05). Among COPD patients, the mental component score was associated with scores of dyspnea, BMI, comorbidities, sex and living areas, while the physical component score was associated with scores of dyspnea, severity of COPD, comorbidities, exposure to dusts/gases/fumes, sex, age, educational level and previous diagnosis of respiratory diseases (all P < 0.05).CONCLUSIONSThe QOL in patients with COPD was impaired and associated with scores of dyspnea, severity of COPD, comorbidities and BMI. Improvement of dyspnea, nutritional support, prevention of comorbidities and keeping away from risk factors may improve the QOL in COPD patients.

Abstract:Objective To provide direct interventions for patients with chronic obstructive pulmonary disease(COPD)and relevant bases of medical decision-making and resource allocation for the government by investigation of those biology-psychology-social factors which influence COPD patients medical behaviors. Methods COPD patients were followed in July in 2014 with general conditions, severity grading and questionnaire of quality of life(QOL). Telephoned patients every month, processed the health education every six months and recorded their attendance times and their compliance status. The study ended in December 2015.The final QOL questionnaire investigation was proceeded and those objects were divided into different groups according their outcomes: aggravating group, illness group without change, improved group. Multinomial Logistic regression were used to find those biologicalpsychological-social factors that influenced these COPD patients' diagnosis and outcome. Variance analysis were used in groups or between-group to find out the relationship between prognosis and QOL. Results Patients with reimbursement proportion 70％ higher, better compliance with health education, no complications, illness lighter(A/B)in the beginning would get better than the others(OR: 10.35 vs2.147 vs 5.791 vs 1.51, P＜0.05). Scores of aggravating group patients in the physiological, psychological and environmental area of QOL were significantly lower at the end of the study than that in the beginning of study(P＜0.05). Scores in psychological area of illness group without change were obviously higher at the end of the study than that in the beginning of study(P＜0.05). Scores in physiological area of improved group were obviously higher at the end of the study than that in the beginning of study(P＜0.05). In the beginning of the study the QOL scores in all area of improved group patients were obviously higher than that of aggravating group patients(P＜0.05).Scores in social relationship and environmental area of illness group without change patients were obviously higher than that of aggravating group patients(P＜0.05).At the end of the study the QOL scores in all area of both improved or illness group without change patients were obviously higher than that of aggravating group patients(P＜0.05). Conclusions Reimbursement proportion 70％ or higher, better compliance, no complications, illness lighter(A/B)are protective factors. Underweight or normal, frequent acute attack and lower compliance are risk factors of COPD. Patients with higher QOL scores got improved or stable conditions. The QOL scores in both illness group without change or improved group patients are higher than that in aggravating group patients.
Quality of life and its associated factors for mild chronic obstructive pulmonary disease patients of urban community settings.

Abstract:BACKGROUND:Chronic obstructive pulmonary disease (COPD) is a serious disease with impaired quality of life (QoL). Few studies have focused on the QoL for mild COPD patients in Chinese urban communities. This study aimed to evaluate the QoL and explore its associated factors among patients with mild COPD in Shanghai. METHODS:A cross-sectional survey of 275 mild COPD patients was conducted in 6 communities of Pudong New Area in Shanghai, China, in 2016. Data on socioeconomic and health conditions were acquired through a face-to-face interview and a physical examination. EuroQoL five-dimension questionnaire (EQ-5D)- Chinese version was applied to evaluate their QoL. Logistic regression was used to estimate odds ratios (ORs) and their 95% CI for risk factors associated with QoL. RESULTS:Among 275 subjects, the overall visual analog scale score and utility of QoL were 70.6 and 0.889, respectively. Logistic regression analysis indicated that exacerbation in recent year were negatively associated with the top 25% utility of QoL (OR =2.02, 95% CI: 1.03-3.97), and the worse exercise capacity (distance of 6-minute walking test <525.3 m) was negatively correlated the outcome (OR =2.92, 95% CI: 1.10-7.75) too. In multi-factor logistic regression, the associations were slightly weakened. CONCLUSIONS:The QoL was impaired in mild COPD patients living in urban communities. Exacerbation in the past year and exercise capacity were significantly correlated to QoL. More interventions should be developed to improve the QoL at the early stage of COPD.

This study aimed to examine authoritative research findings, research hotspots, and trends in the field of COPD nursing. Descriptive statistics and bibliometric and visual analyses of the literature were conducted. Methods: Bibliometric data were obtained from the Web of Science database. Citespace was used to explore publication trends, countries, institutions, journals, authors, keywords, and co-citation characteristics of the included literature in order to summarize the key research in the field of COPD nursing. Results: In total, 693 articles on COPD nursing were published. 1998– 2014 showed a rapid growth period in this research field, which stabilized in 2015– 2022. The research content could mostly be summarized into five categories: acute exacerbation, quality of life, risk, evidence-based nursing, and pulmonary rehabilitation. The research hotspots in 1998– 2014 included randomized controlled trials, education, elderly patients, nursing home residents, nursing homes, rehabilitation, and prevalence. Research in 2015– 2022 focused on impact, palliative care, needs, and predictors. In recent years, research mainly concentrated on symptom management models, cost-effectiveness, and cumulative meta-analysis. Conclusion: Bibliometric analysis of COPD nursing articles indicates that the focus of COPD nursing research is shifting from tertiary prevention to primary and secondary prevention. Helping patients achieve self-management of symptoms, reducing the financial burden of COPD on healthcare, and summarizing research evidence by meta-analyses will likely remain the focus of future research. Quality of life and its association with direct medical costs for COPD in urban China

Abstract:Background Chronic obstructive pulmonary disease (COPD) is one of the leading causes of death worldwide. Few studies have focused on the quality of life (QoL) associated medical costs for COPD in China. Methods A cross-sectional survey of 678 COPD patients was conducted in four major cities (Beijing, Shanghai, Guangzhou and Chengdu), China, in 2011. Data on socio-demographic information, health conditions and medical costs were collected through a face-to-face interview combined with medical record searching. The EuroQol (EQ-5D) health questionnaire was applied to assess the QoL of COPD patients. Results Among 678 patients with COPD, nearly 40% had difficulties in mobility, usual activities and pain/discomfort, one third had various degrees of anxiety/depression, and one fifth had difficulties in self-care. The COPD patients had a median utility score of 0.768 and a median visual analog scale score of 70. The degree of difficulties in any dimensions significantly increased, and utility and health scores decreased with severity of the disease. Age, gender and disease severity were significantly associated with the quality of life after taking other covariates into consideration. Poorer QoL was a significant indicator of higher direct medical costs for COPD patients. Conclusion Impaired quality of life was significantly linked to increased medical costs for COPD patients and could be an important measure for policy- and decision-making in COPD care.
Influencing factors of good quality of life among chronic obstructive pulmonary disease patients living in Zhejiang Province, China

Abstract:Chronic obstructive pulmonary disease (COPD) is a chronic, progressive and debilitating disease that affects quality of life (QOL), especially among patients living in poor environments. This study aimed to determine the influencing factors of good QOL among COPD patients living in Zhejiang, China. A cross-sectional study was conducted to collect data from participants in six tertiary hospitals in Zhejiang Province by a simple random sampling method. A validated questionnaire was used to collect general information, environmental factors, and COPD stage. The standardized St. George's Respiratory Questionnaire (SGRQ) was used to assess QOL. Logistic regression was used to determine influencing factors of good QOL among COPD patients at a significance level of α = 0.05. A total of 420 participants were recruited for analysis. The overall prevalence of patients with good QOL was 25.7%. Six variables were found to be associated with good QOL in the multivariable analysis. Patients who were employed had 2.35 times (95% CI 1.03–5.34) greater odds of having good QOL than those who were unemployed. Those whose family income was higher than 100,000 CNY had 2.49 times (95% CI 1.15–5.39) greater odds of having good QOL than those whose family income was lower than 100,000 CNY. Those who had treatment expenses less than 5,000 CNY had 4.57 (95% CI 1.57–13.30) times greater odds of having good QOL than those who had treatment expenses of 5,000 CNY or higher. Those who had mild or moderate airflow limitation were 5.27 times (95% CI 1.61–17.26) more likely to have good QOL than those who were in a severe or very severe stage of COPD. Those who had a duration of illness less than 60 months had 5.57 times (95% CI 1.40–22.12) greater odds of having good QOL than those who had a duration of illness of 120 months or more. Those who were not hospitalized within the past 3 months had 9.39 times (95% CI 1.62–54.43) greater odds of having good QOL than those who were hospitalized more than twice over the past 3 months. Socioeconomic status, disease stage and accessibility were associated with good QOL among COPD patients in Zhejiang Province, China. Increasing family income and implementing measures to improve the accessibility of medical care, including developing a proper system to decrease the cost of treatment for COPD patients, can improve patients' QOL.

Abstract:Abstract Background Chronic obstructive pulmonary disease (COPD) is one of the diseases with the highest morbidity and mortality globally. The Quality of Life Instrument for Chronic Diseases(QLICD)-COPD(V2.0) was designed to assess the health condition of patients with COPD. The objective of this study was to evaluate the quality of life (QOL) of patients, the influential clinical factors, and the relationships between QOL and clinical objective indicators. Methods Two hundred and sixty-one in-patients with COPD in the acute exacerbation stage were evaluated using the QLICD-COPD(V2.0) and data on clinical objective indicators were collected. The relationships between QOL and the clinical objective indicators were determined using canonical correlation analysis. Results The standardized scores for the patients in four domains, namely physical function, psychological function, social function, and a disease-specific module, were 49.00 ± 12.91, 59.89 ± 13.51, 68.59 ± 11.94, and 51.84 ± 13.58, respectively. The total score for the QOL of patients was 57.17 ± 10.26. Two pairs of canonical correlation variables were statistically significant (P < 0.05), with coefficients of 0.35 and 0.26. These variables respectively accounted for 45.8% and 33.8% of the variance. The levels of total protein, albumin, serum sodium, and alkaline phosphatase and the percentages of neutrophils and lymphocytes were correlated with the QOL. Conclusion During COPD treatment, clinicians should pay close attention to the levels of total protein, albumin, serum sodium, and alkaline phosphatase and the percentages of neutrophils and lymphocytes to improve the QOL of patients.
The Relationship Between Benefit Finding and Quality of Life in Patients with Chronic Obstructive Pulmonary Disease: The Mediating Effects of Self-Management

Abstract:Objective: To explore the relationships among benefit finding (BF), self-management, and quality of life (QOL) among patients with COPD. Methods: A total of 205 patients with COPD were selected via a convenient sampling method. BF refers to the ability to find meaning or benefit from difficult situations. The Benefit Finding Scale (BFS), self-management scale, and 36-item Short-Form Health Survey (MOS SF-36) were used to investigate BF, self-management and QOL (including a physical component summary (PCS) and a psychological component summary (MCS)). Structural equation modeling was used to examine the relationships among BF, self-management and QOL in patients with COPD and to analyze the effects of BF and self-management on QOL. Results: The total QOL score of patients with COPD was 61.38±21.15, and the PCS and MCS scores were 57.67±23.60 and 65.09±21.24, respectively. BF and self-management had positive predictive effects on both the PCS (βBF = 0.519, PBF = 0.012; βself-management = 0.473, Pself-management = 0.012) and MCS (βBF = 0.425, PBF = 0.013; βself-management = 0.535, Pself-management = 0.016) of patients with COPD, and self-management mediated the relationships of BF with the PCS (β = 0.144, P = 0.008) and MCS (β = 0.162, P = 0.007). Conclusion: The QOL of patients with COPD needs to be improved, especially in terms of physical aspects. Helping COPD patients obtain better BF not only helps them improve their PCS and MCS directly but also indirectly through enhancing self-management to improve their PCS and MCS.

Abstract:Objective To investigate the basic condition of those who carry on prevention and treatment of chronic obstructive pulmonary disease(COPD) in community of Zhejiang province,and their awareness level of each index on performance management of COPD,and thus to establish the basic indexes on management and assessment of performance of COPD control in community.Methods A self-designed questionnaire survey was conducted among 258 subjects enrolled by using stratified cluster sampling method,The results were statistically analyzed.Results The first five indexes on the COPD control management in community were in order of healthy education rate(88.8%),treating rate(86.8%),follow-up rate(85.7%),record building rate(85.3%),and detection rate(82.6%).And the first five indexes on performance assessment were in order of healthy education rate(98.4%),periodical physical examination rate(89.1%),record building rate(88.4%),treat rate(86.4%),and follow-up rate(86.0%).Conclusion Totally 9 indexes on COPD control management in community and 7 indexes on performance assessment are established through investigation and study.And the performance assessing method of COPD control in community is summarized,which is of reference significance for control of other noninfectious chronic diseases.
The relationship between self-efficacy, health literacy, and quality of life in patients with chronic diseases: a cross-sectional study in China

Abstract:Background: Self-efficacy and health literacy are closely related to the quality of life in patients with chronic diseases; however, it remains unclear whether their combined effects on the quality of life (QoL) in these patients operate through mediation, interaction, or a combination of both. Methods: The research occurred in China between July 10 and September 15, 2021. A multi-stage random sampling technique was utilized to gather information on self-efficacy, health literacy, and QoL among individuals with chronic diseases. Linear regression models investigated the relationships between these patients' self-efficacy, health literacy, and QoL. Additionally, the four-way decomposition method was used to decompose the overall effects of self-efficacy and health literacy on the QoL in patients with chronic diseases. Results: Significant correlations were found between self-efficacy, health literacy, and QoL among individuals with chronic diseases (all p < 0.05). In the four-way decomposition results, the results of the European Quality of Life Five Dimension Five Level (EQ-5D-5L) displayed the interaction effects mediated by self-efficacy, and the reference interaction effects were not significant, with small effect sizes observed. The influence of health literacy levels on the QoL in these patients was primarily attributed to the controlled direct effect (CDE), accounting for approximately 86.12% [excess relative risk = 0.00415; 95% CI: 0.00326, 0.00504; p < 0.0001]. The proportion solely attributable to the pure indirect effect (PIE) of self-efficacy was 14.5% [excess relative risk = 0.0007; 95% CI: 0.00031, 0.00109; p < 0.0001]. In the EQ visual analog scale (EQ-VAS) results, the proportion of the controlled direct effect was 84.9% [excess relative risk = 0.62443; 95% CI: 0.52269, 0.72618; p < 0.0001], while the proportion solely attributable to the pure indirect effect of mediation was 14.8% [excess relative risk = 0.10876; 95% CI: 0.06409, 0.15344; p < 0.0001]. Conclusion: Self-efficacy and health literacy primarily influence QoL in patients with chronic diseases through controlled and pure indirect effects. Enhancing patients' health literacy and self-efficacy can contribute to improving their QoL.
Investigation of Health-Related Quality of Life and Impact Factors in Elderly Patients with COPD in Beijing

Abstract:Objective To assess the health-related quality of life ( HRQoL) of elderly patients with COPD in Beijing and to explore the impact factors. Methods 100 outpatients diagnosed with irreversible COPD in Beijing completed the SF-36 questionnaire. The scores of 8 dimensions of questionnaire in elderly patients with COPD were calculated and compared with those of the rural general elderly people in Tianjin. Multiple regression analysis was applied to determine the impact factors of different dimensions of SF-36. Results The scores of all the dimensions in the patients were significantly lower than those in general old rural people of Tianjin. The correlations between the different dimensions of the SF-36 and variables were: Physical functioning was well correlated with ADL, ability to carrying activities (6MWD) and anxiety. Role-physical was well correlated with anxiety, admission times in a year and nutrition ( BMI). Body pain was well correlated with anxiety. General health was well correlated with depression, social support, the number of combination chronic disease, ability to carrying activities (6MWD) and the severity of disease (FEV1. 0%). Vitality was well correlated with depression. Social functioning was well correlated with ADL, carrying activities (6MWD), social support and depression. Mental heath was well correlated with depression ,anxiety and nutrition (BMI).No variable showed significant correlation with the Role-emotional dimension. Conclusions The HRQoL of elderly patients with COPD is declined compared with general elderly people. There are a range of factors that impact different dimensions of HRQoL, chiefly emotional status and carrying activities. Stress Coping Framework As an Effort To Improve Quality of Life Patients with Chronic Obstructive Pulmonary Disease: A Literature Review

Abstract:ABSTRACT COPD patients usually experience a decrease in quality of life due to COPD symptoms such as coughing, shortness of breath,exacerbationAcute disease and comorbidities that occur in COPD patients cause limitations in daily activities. Stress coping is one of the factors that causes good or bad self-care behavior in COPD patients. To describe the influence, stress coping and level of selfcare in COPD patients on their quality of life. The method of this study is a literature review. Literature searches using electronic databases such as PubMed, Google Scholar and Science Direct. A literature review was carried out on international journals published in the last 5 years, available in full text. The keywords used were "self-care", stress coping" and "quality of life" for COPD patients. The selection of articles followed the PRISMA approach. This review summarizes eighteen articles. Five articles discussed self-care, the SelfEfficacy scale, and discriminative validity with higher self-care scale scores in individuals with greater COPD severity and poorer health status. Five articles discuss coping stress in COPD sufferers. COPD patients who have good knowledge will have more coping mechanism strategies so that their quality of life becomes more prosperous. Seven articles discussed the quality of life of COPD sufferers. If a COPD patient has good stress coping and strong confidence and confidence in his or her abilities, this will influence self-care actions and improve the COPD patient's quality of life. Future researchers are encouraged to explore similar research themes using other journal search engines to enrich the study. Keywords: Self Care, Stress Coping, Quality Of Life, COPD

Abstract:Pulmonary hypertension is a disease that impairs cardiopulmonary function and is characterised by the limitation of progressive physical activity and decline in health-related quality of life. Patients usually present with a range of severe symptoms, including dyspnoea, fatigue, chest pain and oedema, which affect their quality of life. Compared to the general population, patients usually have a lower health-related quality of life, with differences in several aspects such as physical functioning, mental health, social interactions and career. The aim of this review is to examine recent research advances in health-related quality of life in patients with pulmonary hypertension, including its influencing factors and improvement measures, in order to improve the understanding and management of this condition.
Canonical Correlation Analysis of Factors That Influence Quality of Life among Patients with Chronic Obstructive Pulmonary Disease Based on QLICD-COPD (V2.0)

Abstract:Background The Quality of Life Instrument for Chronic Diseases (QLICD)-COPD (V2.0) was designed to assess the health condition of patients with chronic obstructive pulmonary disease (COPD). The objective of this study was to evaluate the quality of life (QOL) of patients, the influential clinical factors and the relationships between QOL and clinical objective indicators. Methods Two hundred and sixty-one inpatients with COPD in the acute exacerbation stage were evaluated using the QLICD-COPD (V2.0) and data on clinical objective indicators were collected. The relationships between QOL and the clinical objective indicators were determined using canonical correlation analysis. Results The standardised scores for the patients in four domains, namely, physical function, psychological function, social function and a disease-specific module, were 49.00 +/- 12.91, 59.89 +/- 13.51, 68.59 +/- 11.94 and 51.84 +/- 13.58, respectively. The total score for the QOL of patients was 57.17 +/- 10.26. Two pairs of canonical variables were statistically significant (r(1)=0.35, p<0.0001; r(2)=0.26, p<0.05). These variables accounted for 45.8% and 33.8% of the variance, respectively. The levels of total protein, albumin, serum sodium and alkaline phosphatase and the percentages of neutrophils and lymphocytes were correlated with the QOL, with correlation coefficients ranging from -0.435 to 0.675. Conclusion Clinicians should pay close attention to the levels of total protein, albumin, serum sodium and alkaline phosphatase and the percentages of neutrophils and lymphocytes to improve the QOL of patients.

Abstract:Objective:To probe into the value of Seattle Obstructive Lung Disease Questionnaire(SOLQ)for measuring the quality of life of patients with COPD in community.Methods:A total of 102 with community COPD patients were investigated by using SOLQ and COPD assessment test(CAT)to verify the application value of SOLQ.Results:The test retest reliability coefficient of SOLQ was 0.854(P <0.01),the Cronbach’s αcoefficient was 0.961,the structural validity between the total surface and its 4 dimensions was 0.361 ~0.891 (all P <0.01),the criterion related validity between SOLQ and CAT was 0.648;there was significant differ-ence between the high and the low score group(P <0.01).Conclusion:The SOLQ had higher reliability and va-lidity,and it could well reflect the condition of disease and the quality of life of patients with community COPD.

Abstract:Background: The pathogenesis of chronic obstructive pulmonary disease (COPD) has been studied in relation to the microbiome, providing space for more targeted interventions and new treatments. Numerous papers on the COPD microbiome have been reported in the last 10 years, yet few publications have used bibliometric methods to evaluate this area. Methods: We searched the Web of Science Core Collection for all original research articles in the field of COPD microbiome from January 2011 to August 2022 and used CiteSpace for visual analysis. Results: A total of 505 relevant publications were obtained, and the number of global publications in this field is steadily increasing every year, with China and the USA occupying the first two spots in international publications. Imperial College London and the University of Leicester produced the most publications. Brightling C from the UK was the most prolific writer, while Huang Y and Sze M from the USA were first and second among the authors cited. The American Journal of Respiratory and Critical Care Medicine had the highest frequency of citations. The top 10 institutions, cited authors and journals are mostly from the UK and the US. In the ranking of citations, the first article was a paper published by Sze M on changes in the lung tissue's microbiota in COPD patients. The keywords "exacerbation", "gut microbiota", "lung microbiome", "airway microbiome", "bacterial colonization", and "inflammation" were identified as cutting-edge research projects for 2011-2022. Conclusion: Based on the visualization results, in the future, we can use the gut-lung axis as the starting point to explore the immunoinflammatory mechanism of COPD, and study how to predict the effects of different treatments of COPD by identifying the microbiota, and how to achieve the optimal enrichment of beneficial bacteria and the optimal consumption of harmful bacteria to improve COPD.

Abstract:OBJECTIVE The study aimed to explore the effects of family nursing intervention on the quality of life of patients with stable chronic obstructive pulmonary disease(COPD). METHODS 36 cases of stable COPD were educated and urged to implement long-term home oxygen therapy, exercise training, smoking cessation, nutrition support, and mental intervention, so to ensure that the patients and their families possess sufficient COPD-related knowledge and nursing skills and that the training measures are effectively reinforced. RESULTS Significant differences in quality of life( including physical health, mental health, social adaptation, and environmental adaptation) were observed between 2 years after the intervention and before the intervention, 1 year after the intervention, and 2 years without the intervention(P﹤ 0.05 or P﹤ 0.01).CONCLUSION Family nursing intervention can effectively improve the quality of life of patients with stable chronic obstructive pulmonary disease.
