Abstract:AIM To report the safety, efficacy, and accuracy of small-incision lenticule extraction (SMILE) or femtosecond-assisted laser in situ keratomileusis (FS-LASIK) for the correction of myopia or myopic astigmatism in patients with deep corneal opacity denoted by anterior segment optical coherence tomography (AS-OCT). METHODS Four patients with monocular corneal opacity (3 due to mechanical injury, 1 due to a firecracker wound) were recruited and treated with refractive surgery (3 for SMILE, 1 for FS-LASIK combined with limbal relaxing incision (LRI). Preoperative ocular manifestations, surgical details, postoperative visual outcomes, corneal opacity parameters, and corneal topography were analyzed. RESULTS Preoperatively, spherical diopter ranged from -3.0 D to -4.75 D with cylinder ranging from -0.75 to -5.0 D, and corrected distance visual acuity (CDVA) ranging from 20/25 to 20/20. One eye's corneal opacity was located in the central zone and three were in the mid-peripheral optical zone. Three patients underwent uneventful SMILE in both eyes, whilst one patient underwent FS-LASIK for high astigmatism in both eyes and LRI in the right eye. CDVA of the eye with corneal opacity ranged from 20/22 to 20/20 one to six weeks postoperatively. Two patients achieved better CDVA and no patients lost Snellen lines. The postoperative diopter was within ±0.75 D for all eyes. Significant edema existed above the corneal opacity in one eye and dissipated soon. No eccentric corneal topography or morphological anomaly of the corneal cap or flap was observed. CONCLUSION The cases demonstrate that SMILE or FS-LASIK is safe and effective to treat myopic astigmatism combined with deep corneal opacity lesions after comprehensive preoperative evaluation and appropriate candidate selection. FS-LASIK combined with LRI is also sufficient for correcting high astigmatism due to corneal scarring.
Tensile biomechanical properties and constitutive parameters of human corneal stroma extracted by SMILE procedure

Abstract:The biomechanical behavior of human corneal stroma under uniaxial tension was investigated by the experimental analysis of cornea stromal lenticules taken out by corneal refractive surgery. Uniaxial tests were conducted to determine their stress-strain relationship and tensile strength. The Gasser-Ogden-Holzapfel (GOH) model was used to describe biomechanical behavior of the corneal stroma. The theoretical stress-strain relationship of the GOH model in the uniaxial tensile test was deduced. The corneal specimens were collected from ten patients (4 male and 6 female), aged from 17 to 36. The differences between corneal stress-strain relationship in the horizontal and vertical direction were compared. The constitutive parameters C10, k1 and k2 were evaluated through least squares curve-fitting of experimental data.
Primary culture and identification of fibroblasts derived from human corneal stroma using the femtosecond laser small incision lenticule extraction

Abstract:Objective To evaluate the feasibility of culturing and identifying fibroblasts derived from human corneal stroma of myopic patients that had underwent the femtosecond laser small incision lenticule extraction(SMILE) in vitro.Methods Corneal stromal tissues from 8 patients(16 eyes) with myopia that had underwent femtosecond laser SMILE were primarily cultured using explant tissue culture technique,respectively,and were then identified by morphological observation and immunocytochemical staining.Results All the primary culture of fibroblasts derived from human corneal stroma were successfully carried out and the cell passages were stable.The cells resembled long spindle or triangle.Immunocytochemical staining results showed that vimentin staining was positive,while keratin staining,desmin staining and S-100 staining were all negative.Conclusion Corneal stroma of myopic patients that had underwent femtosecond laser SMILE is a convenient origin of tissue for the culture of fibroblasts derived from human corneal stroma.

Abstract:This report describes the application of a recently developed spectroscopic technique, near- infrared-excited Fourier transform Raman scattering (abbreviated as near-IR FT-Raman) in the molecular-level characterization of normal and pathological human ocular tissues. The near-IR FT-Raman technique was shown to be particularly well-suited for the noninvasive analysis of intact ophthalmic samples because it exhibits such attractive features as complete fluorescence elimination, great sampling flexibility, high data acquisition speed, and measurement accuracy. For both intact human lenses and corneas, systematic FT-Raman studies were carried out in order to probe detailed molecular changes involved in cataract formation and cornea diseases. FT-Raman measurement was also made for synthetic biomaterials that can be attached to the corneal surface for laser refractive surgery. The technique of near-IR FT- Raman spectroscopy is potentially a nondestructive, non-invasive fingerprinting modality for monitoring lens aging, cataract formation, and corneal disease development.
Exploration of the Pathophysiology of High Myopia Via Proteomic Profiling of Human Corneal Stromal Lenticules.

Abstract:This study aimed to investigate the underlying pathophysiology of high myopia by analyzing the proteome of human corneal stromal lenticule samples obtained through small incision lenticule extraction (SMILE). A total of thirty-two patients who underwent SMILE were included in the study. Label-free quantitative proteomic analysis was performed on corneal stromal lenticule samples, equally representing high myopia (n = 10) and low myopia (n = 10) groups. The identified and profiled lenticule proteomes were analyzed using in silico tools to explore biological characteristics of differentially expressed proteins (DEPs). Additionally, LASSO regression and random forest model were employed to identify key proteins associated with the pathophysiology of high myopia. The DEPs were found to be closely linked to immune activation, extracellular matrix, and cell adhesion-related pathways according to gene ontology analysis. Specifically, decreased expression of COL1A1 and increased expression of CDH11 were associated with the pathogenesis of high myopia and validated by western blotting (n = 6) and quantitative real time polymerase chain reaction (n = 6). Overall, this study provides evidence that COL1A1 and CDH11 may contribute to the pathophysiology of high myopia based on comparative proteomic profiling of human corneal stromal lenticules obtained through SMILE.
In Vivo Evaluation of the Effects of SMILE with Different Amounts of Stromal Ablation on Corneal Biomechanics by Optical Coherence Elastography

Abstract:This work aims to depth-resolved quantitatively analyze the effect of different stromal ablation amounts on the corneal biomechanical properties during small incision lenticule extraction (SMILE) using optical coherence elastography (OCE). A 4.5-MHz ultrasonic transducer was used to excite elastic waves in the corneal tissue. The OCE system combined with the antisymmetric Lamb wave model was employed to achieve a high-resolution, high-sensitivity, and depth-resolved quantitative detection of the corneal Young’s modulus. Eighteen rabbits were randomly divided into three groups; each group had six rabbits. The first and second groups underwent -3D and -6D SMILE surgeries, and the third group was the control group, respectively. Young’s modulus of the corneal cap and residual stromal bed (RSB) were both increased after SMILE, which shared the stress under intraocular pressure (IOP). Furthermore, the Young’s modulus of both the corneal cap and RSB after 3D SMILE group were significantly lower than that in the -6D group, which indicated that the increases in the post-operative corneal Young’s modulus were positively correlated with the amount of stromal ablation. The OCE system for quantitative spatial characterization of corneal biomechanical properties can provide useful information on the extent of safe ablation for SMILE procedures.

Abstract:Background: To investigate the correlation between corneal biomechanical characteristics ( in vitro and in vivo ) and corneal densitometry in myopia. Methods: The Pentacam (Oculus, Wetzlar, Germany) corneal densitometry (CD) and Corvis ST (Oculus, Wetzlar, Germany) exams were conducted prior to surgery for myopic patients who were intended to undergo small-incision lenticule extraction (SMILE). CD values (grayscale units, GSUs), and in vivo biomechanical parameters were obtained. The stromal lenticule was subjected to a uniaxial tensile test to obtain the elastic modulus E in vitro . We exam the correlations among in vivo , in vitro biomechanical characteristics and CD values. Results: In this study, 37 myopic patients (63 eyes) were included. The mean age of participants was 25.14 ± 6.74 years (range:16–39 years). The mean CD values of the total cornea, anterior layer, intermediate layer, posterior layer, 0–2 mm region and 2–6 mm region were 15.03 ± 1.23 GSU, 20.35 ± 1.98 GSU, 11.76 ± 1.01 GSU, 10.95 ± 0.83 GSU, 15.57 ± 1.12 GSU and 11.94 ± 1.77 GSU, respectively. Elastic modulus E ( in vitro biomechanical indicator) was negatively correlated with intermediate layer CD (r = −0.35, p = 0.01) and 2–6 mm region CD (r = −0.39, p = 0.00). A negative correlation was also found between 0-2 mm central region CD and in vivo biomechanical indicator SP-HC (r = −0.29, p = 0.02). Conclusion: In myopic patients, densitometry is negatively correlated with biomechanical properties both in vivo and in vitro . With an increase in CD, the cornea deformed more easily.

Abstract:Background: To compare the corneal biomechanical changes after small incision lenticule extraction (SMILE) and femtosecond laser-assisted in situ keratomileusis (FS-LASIK) with the same programmed optical zone (POZ) and similar refractive correction in patients with high myopia.Methods: In this prospective comparative study of the contralateral eye, 50 patients with high myopia with the same POZ and similar refractive correction who underwent SMILE in one eye and FS-LASIK in the other eye. Corneal biomechanical parameters and central corneal thickness (CCT) were measured using a Corvis ST II. All the patients were evaluated during follow-up visits beyond one year. Additionally, the corneal volume (CV) of the 10-mm diameter region was measured using a Pentacam.Results: Ambrosio relational thickness to the horizontal profile (ARTh) and stiffness parameter A1 (SP-A1) decreased significantly after SMILE and FS-LASIK, whereas deformation amplitude ratio 2.0 mm (DA ratio 2.0 mm) and integrated radius (IR) increased significantly in both groups. The ARTh and SP-A1 were greater after SMILE than those after FS-LASIK at all the follow-up visits. In addition, there were greater amounts of CCT and CV after SMILE compared with that after FS-LASIK. Moreover, a positive correlation was found between ARTh and SP-A1 and postoperative CCT, while a negative correlation was found between IR and DA ratio 2.0 mm and postoperative CCT. A moderate correlation was observed between SP-A1 and CV after both SMILE and FS-LASIK, whereas there were no relationships between CV and ARTh, IR, or DA ratio 2.0 mm.Conclusions: SMILE had greater CCT, CV, ARTh, and SP-A1 than FS-LASIK in high myopia with the same POZ and similar refractive correction. Three-year results of central corneal stromal thickness reduction in small-incision lenticule extraction for high myopia correction

Abstract:Background: To investigate the long-term corneal stromal remodeling and central stromal thickness (CST) reduction accuracy after small-incision lenticule extraction (SMILE) for high myopia correction. Methods: This prospective study included 30 patients (50 eyes) who had undergone SMILE. Measurements of CST reduction using optical coherence tomography were performed at 1 month, 6 months, 1 year, and 3 years after surgery. Correlations were performed between planned and achieved CST reductions. Results: The study enrolled 50 eyes of 30 patients. The mean spherical equivalent was -9.25±1.52 D(diopters). The postoperative CST increased in the first month after surgery and remained stable for a year. Thereafter, it remained stable during follow-up from 1 to 3 years postoperatively. The predicted CST reduction was 146.4 ± 10.3 μm. The achieved CST reductions at 1 month, 6 months, 1 year, and 3 years after surgery were 135.3 ± 12.1 μm, 130.8 ± 10.6 μm, 125.9 ± 9.4 μm, and 122.2 ± 10.6 μm, respectively. An overestimation of CST reduction was observed three years after surgery. Correlation analysis revealed a strong correlation between planned and achieved CST reductions; however, no correlation was found between CST reductions predicted error and the planned CST reductions. Conclusion: During long-term follow-up, our findings revealed a significant stromal remodeling following SMILE in patients with high myopia. Therefore, clinicians should consider it when screening patients with high myopia for SMILE.
Short-term Evaluation of Visual Quality, Amplitude of Accommodation, and Stereoacuity Between Patients with Moderate-to-High Myopia Who Underwent ICLV4c Implantation and SMILE.

Abstract:PURPOSE: To compare the visual quality, accommodation, and stereoacuity between patients with a myopia magnitude of-3.00 to-8.50 diopters (D) who underwent ICLV4c (STAAR Surgical) implantation and small incision lenticule extraction (SMILE). METHODS: The visual quality parameters, amplitude of accommodation (AMP), and stereoacuity were compared between and within the ICLV4c and SMILE groups (40 eyes/ group) with a mean spherical equivalent (SE) of-6.14 D. Differences in these parameters before and after surgery were analyzed using SPSS 22.0 software (SPSS, Inc).RESULTS: Modulation transfer function (MTF) cut-off increased significantly by 3.24 cycles/degree (t =-2.111, P =.041) and AMP decreased significantly by 1.66 D (t = 7.519, P < .001) from baseline to 3 months after surgery in the ICLV4c group. However, the MTF cut-off was significantly lower (t = 2.123, P = .037) and the AMP was significantly better (t =-3.605, P = .001) in the SMILE group than in the ICLV4c group at 3 months postoperatively. Other parameters such as Objective Scatter Index, Strehl ratio, and Optical Quality Analysis System (Visiometrics) under different contrast values did not show significant changes (P > .05). Stereoacuity did not improve significantly, except in a patient with anisometropia (2.50 D) in the ICLV4c group.CONCLUSIONS: The ICLV4c implantation yielded a better visual quality but worse AMP than did SMILE in patients with a myopia magnitude of-3.00 to-8.50 D in the short term.
Metabolomic Analysis in Corneal Lenticules from Contact Lens Wearers.

Abstract:PURPOSE:To investigate the mechanisms of pathological changes in corneal stroma and the wearing time of soft contact lenses using the metabolomic method.METHODS:Laser scanning confocal microscopy was used to evaluate the pathological changes of corneal stroma between wearing time groups before small incision lenticule extraction. After small incision lenticule extraction, 190 corneal stroma samples were obtained, and a metabolomic method using high performance liquid chromatography coupled with time of flight mass spectrometry was established to analyze the changes in metabolites between wearing time groups.RESULTS:Laser scanning confocal microscope results demonstrated that the corneal nerve fiber length, the number of corneal anterior stromal cells, and the number of corneal posterior stromal cells were reduced gradually with increasing wearing time. The metabolomic study demonstrated that 11 biomarkers were identified between patients who did and did not wear soft contact lenses and 6 biomarkers were identified between less than 5 years and more than 5 years of wearing time. These biomarkers participate in energy metabolism, lipid metabolism, inflammatory reactions, and neuroprotecton processes, and partially lead to the pathology of dry eyes, eye inflammation, and corneal nerve fiber length decrease. Five biomarkers in the citrate cycle metabolism pathway were found demonstrating that energy metabolism was seriously disturbed.CONCLUSIONS:This study systematically revealed the metabolite mechanism for eye discomfort and related disease after wearing soft contact lenses. The identified biomarkers and related physiology pathways supply a new direction for avoiding the side effects of wearing soft contact lenses. [J Refract Surg. 2020;36(5):317-325.].
Evaluation of Changes in Human Corneas After Femtosecond Laser-Assisted LASIK and Small-Incision Lenticule Extraction (SMILE) Using Non-Contact Tonometry and Ultra-High-Speed Camera (Corvis ST)

Abstract:PURPOSE: To evaluate theoretical biomechanical advantages of human corneas treated with small incision lenticule extraction (SMILE) compared with femtosecond laser-assisted LASIK (FS-LASIK) Patients and methods: In a prospective, comparative, non-randomized, consecutive case series patients with moderate to high myopia and/or astigmatism underwent corneal refractive surgery. Patients either received standard FS-LASIK or SMILE. Preoperatively and up to 3 months postoperatively data were analyzed including tomography with Pentacam HR, Goldmann tonometry, non-contact tonometry, and deformability of the cornea using an ultra-high-speed camera (Corvis ST). Data were analyzed and p < 0.05 was considered as statistically significant (t-test).RESULTS: Seventy-three patients (128 eyes) were treated. Forty-eight eyes of 29 patients underwent FS-LASIK and 80 eyes of 44 patients underwent SMILE. Preoperative spherical equivalent (SE) was -3.23 ± 1.64 D (FS-LASIK group) and -4.83 ± 1.63 D (SMILE group) (p < 0.0001). Almost all parameters obtained by Corvis ST between preoperative and postoperative measurements showed significant changes after refractive surgery. Significant changes were seen between both groups postoperatively. However, matched pair subgroup analysis (n = 69; 26 eyes FS-LASIK; 43 eyes SMILE) of eyes with initially equal pachymetry, intraocular pressure, SE, and difference of pre- to postoperative pachymetry (p>0.05), showed no significant changes in parameters measured with Corvis ST (p>0.05).CONCLUSIONS: Corneal biomechanical parameters measured preoperatively with Corvis ST showed significant differences postoperatively in total and in both groups. In subgroup analysis with homogenous groups, FS-LASIK showed no significant changes in biomechanical data measured with Corvis ST compared with SMILE.

Corneal Biomechanical Properties Demonstrate Anisotropy and Correlate with Axial Length in Myopic Eyes

Abstract:Purpose The purpose of this study was to investigate the ex vivo and in vivo biomechanical characteristic of cornea in myopic eyes. Methods Fifty-one corneal stromal lenticules were obtained from myopic eyes during the SMILE procedure and were tested by a biaxial tensile system within 24 hours postoperatively. The material properties of the lenticules were described using stress-strain curves and were compared among axial length (AL) <26 mm and AL ≥ 26 mm group. Pre-operative stress-strain index (SSI) parameters were used to evaluate the biomechanical properties of the cornea in vivo. Results Compared with AL < 26 mm, the tangent modulus significantly decreased in horizontal and vertical directions when AL ≥ 26 mm (P < 0.05); SSI also significantly decreased when AL ≥ 26 mm (P < 0.05). Anisotropic parameter is positively correlated with AL (r = 0.307, P < 0.05). Compared with AL < 26 mm, anisotropic parameter significantly increased when AL ≥ 26 mm (P < 0.05). SSI was negatively correlated with AL (r = −0.380, P < 0.05) in the AL < 26 mm group but not in the AL ≥ 26 mm group (P > 0.05). Compared with 26 mm ≤ AL < 27 mm group, the tangent modulus significantly decreased in the horizontal direction (P < 0.05) but not in the vertical direction when 27 mm ≤ AL < 28 mm (P > 0.05). Conclusions The biomechanical properties of cornea decreased with the increase of AL. Tangent modulus significantly decreased in the horizontal direction compared with vertical direction. AL should be taken into account during calculation of corneal biomechanical parameters in order to improve validity.
Evaluation of Corneal Biomechanics Using Brillouin Microscopy in Chinese Adults With Myopia

Abstract:Purpose: To evaluate the corneal biomechanical metrics of Chinese adults with myopia and identify relevant factors of Brillouin microscopy. Methods: In this cross-sectional study, corneal biomechanics in Chinese adults with myopia were quantified and analyzed using Brillouin microscopy and the Corvis ST (CST) (Oculus Optikgeräte GmbH) and analyzed. Univariate linear regression was used with potential factors including age, sex, spherical equivalent (SE), intraocular pressure (IOP), central corneal thickness (CCT), and mean keratometry (Kmean). Results: The study included 87 eyes of 87 participants (mean age: 24.47 ± 6.27 years). Central, Mean, maximum (Max), minimum (Min), standard deviation, and Max-Min Brillouin modulus (BM) values obtained from Brillouin microscopy exhibited values of 2.826 ± 0.039, 2.827 ± 0.027, 2.864 ± 0.034, 2.790 ± 0.038, 0.108 ± 0.042, and 0.074 ± 0.041 GPa, respectively. No significant correlations were found between BM parameters and age, sex, SE, IOP, or CCT. However, the Mean (β = -0.251, P = .019), Min (β = -0.315, P = .003), and Max-Min (β = 0.229, P = .033) BM values were significantly associated with Kmean. The Central, Mean, Min, and Max BM values negatively correlated with the Tomographic Biomechanical Index measured by CST (Spearman's r = -0.24, -0.35, -0.29, and -0.23, respectively, all P < .05). Conclusions: Brillouin microscopy accurately reflects corneal biomechanical parameters in Chinese adults with myopia, independent of IOP and CCT, with a good correlation with CST. Concurrent evaluation of the corneal curvature is imperative when employing Brillouin microscopy in clinical practice. [J Refract Surg. 2024;40(10):e768-e776.].
Corneal biomechanical characteristics in myopes and emmetropes measured by Corvis ST: a meta-analysis

Abstract:Purpose To comprehensively identify the corneal biomechanical differences measured by Corvis ST between different degrees of myopia and emmetropia. Design Systematic review and meta-analysis. Methods Electronic databases, including PubMed, Embase, and Web of Science, were systematically searched for studies comparing the corneal biomechanics among various degrees of myopes and emmetropes using Corvis ST. The weighted mean differences and 95% confidence intervals were calculated. Meta-analysis was performed in high and non-high myopes and in myopes and emmetropes, respectively. Results Eleven studies were included in this study. The meta-analysis among myopes and emmetropes included 1947 myopes and 621 emmetropes, and 443 high myopes and 449 non-high myopes were included in the meta-analysis among high and non-high myopia. Myopes showed the cornea with significantly longer time at the first applanation (A1t) and lower length at the second applanation (A2L) than emmetropes. High myopes showed significantly greater A1t, velocity at the second applanation (A2v), deformation amplitude at the highest concavity (HC-DA), and peak distance at the highest concavity (HC-PD) and decreased time at the second applanation (A2t) and radius of the highest concavity (HC-R). Conclusions Corneal biomechanics is different in myopia, especially in high myopia. Compared with non-high myopes, the corneas of high myopes deformed slower during the first applanation, faster during the second applanation, and showed greater deformation amplitude, indicating greater elasticity and viscidity.

Abstract:Keratoconus (KC) is a degenerative condition affecting the cornea, characterized by progressive thinning and bulging, which can ultimately result in serious visual impairment. The onset and progression of KC are closely tied to the gradual weakening of the cornea's biomechanical properties. KC progression can be prevented with corneal cross-linking (CXL), but this treatment has shortcomings, and evaluating its tissue stiffening effect is important for determining its efficacy. In this field, the shortage of human corneas has made it necessary for most previous studies to rely on animal corneas, which have different microstructure and may be affected differently from human corneas. In this research, we have used the lenticules obtained through small incision lenticule extraction (SMILE) surgeries as a source of human tissue to assess CXL. And to further improve the results' reliability, we used inflation testing, personalized finite element modeling, numerical optimization and histology microstructure analysis. These methods enabled determining the biomechanical and histological effects of CXL protocols involving different irradiation intensities of 3, 9, 18, and 30 mW/cm2, all delivering the same total energy dose of 5.4 J/cm2. The results showed that the CXL effect did not vary significantly with protocols using 3-18 mW/cm2 irradiance, but there was a significant efficacy drop with 30 mW/cm2 irradiance. This study validated the updated algorithm and provided guidance for corneal lenticule reuse and the effects of different CXL protocols on the biomechanical properties of the human corneal stroma.
Effects of Laser In Situ Keratomileusis and Small-Incision Lenticule Extraction on Corneal Biomechanical Behavior: A Finite Element Analysis

Abstract:Myopia, which is the result of the uncoordinated development of the eyeball, has become a major public health focus worldwide. Laser in situ keratomileusis (LASIK) and small-incision lenticule extraction (SMILE) have been successfully used in modern corneal refractive surgery. However, there are still controversies about postoperative results of LASIK and SMILE. In this study, a three-dimensional finite element model of the cornea was constructed based on the elevation and pachymetry data of a female volunteer. Surgical parameters, magnitudes of myopic correction, and intraocular pressure (IOP) were varied. Furthermore, an iterative algorithm was applied to retrieve the free-stress state of the intact corneal model, LASIK model, and SMILE model. To better evaluate the differences between LASIK and SMILE procedures, the displacement and Von Mises stress on the anterior and posterior corneal surface along the x- and y -axes were analyzed. Results for the zero-pressure model showed larger displacement compared to the image-based corneal model, suggesting that the initial corneal pre-stress stiffens the response of the cornea, both in the intact cornea and under refractive surgery. In addition, the displacement on the corneal surface in LASIK (both zero-pressure and image-based model) was obviously higher than that of the SMILE model. In contrast, SMILE increased Von Mises stress in the corneal cap and reduced Von Mises stress in the residual stromal bed compared with the LASIK model. However, the maximum Von Mises stress in the SMILE model was still smaller than that of the LASIK model. Moreover, the displacement and Von Mises stress on the residual stromal bed increased linearly with IOP. Overall, LASIK and SMILE refractive surgery could change biomechanical behaviors of the cornea. Compared to LASIK refractive surgery, SMILE may present a lower risk of ectasia. Creating a corneal cap rather than a corneal flap may have an advantage in improving corneal biomechanical stability.

Abstract:PURPOSE: In vivo and in situ visualization of biomolecules without pretreatment will be important for diagnosis and treatment of ocular disorders in the future. Recently, multiphoton microscopy, based on the nonlinear interactions between molecules and photons, has been applied to reveal the localizations of various molecules in tissues. We aimed to use multimodal multiphoton microscopy to visualize the localizations of specific biomolecules in rat corneas.METHODS: Multiphoton images of the corneas were obtained from nonlinear signals of coherent anti-Stokes Raman scattering, third-order sum frequency generation, and second-harmonic generation.RESULTS: The localizations of the adhesion complex-containing basement membrane and Bowman layer were clearly visible in the third-order sum frequency generation images. The fine structure of type I collagen was observed in the corneal stroma in the second-harmonic generation images. The localizations of lipids, proteins, and nucleic acids (DNA/RNA) was obtained in the coherent anti-Stokes Raman scattering images.CONCLUSIONS: Imaging technologies have progressed significantly and been applied in medical fields. Optical coherence tomography and confocal microscopy are widely used but do not provide information on the molecular structure of the cornea. By contrast, multiphoton microscopy provides information on the molecular structure of living tissues. Using this technique, we successfully visualized the localizations of various biomolecules including lipids, proteins, and nucleic acids in the cornea. We speculate that multiphoton microscopy will provide essential information on the physiological and pathological conditions of the cornea, as well as molecular localizations in tissues without pretreatment.

Corneal biomechanical properties and potential influencing factors in varying degrees of myopia

Abstract:To compare the corneal biomechanical parameters measured by Corvis ST in subjects with varying degrees of myopia. And the factors that may affect corneal biomechanical properties were also investigated. Participants in this prospective cross-sectional study were classified into three groups according to spherical equivalent (SE) and axial length (AL): Non-myopia (NM, SE > - 0.50 D and AL < 26 mm), Mild-to-moderate myopia (MM, - 6.00 D < SE ≤ - 0.50 D and AL < 26 mm), high myopia (HM, SE ≤ - 6.00 D or AL ≥ 26 mm). Ten corneal biomechanical parameters were finally included. Linear mixed-effects model accounting for using both eyes in the same participant was carried out to evaluate how the corneal biomechanical parameter was influenced by varying degrees of myopia after adjusting for biomechanically corrected intraocular pressure (bIOP) and central corneal thickness (CCT). Further, multiple linear regression was performed to explore the correlation between corneal biomechanical parameter and SE, AL, bIOP or CCT. A total of 304 eyes from 224 healthy myopic subjects were recorded. There were 95 eyes with NM, 122 eyes with MM, and 87 eyes with HM. After adjusting for bIOP and CCT, eyes with high myopia showed shorter highest concavity time (HC-time, p = 0.025), greater peak distance (PD, p = 0.001), greater deflection amplitude (DA-Max, p = 0.002), smaller whole eye movement (WEM-Max, p < 0.001) and reduced stiffness parameter (SP-A1, p < 0.001). Multiple regression analysis showed that five parameters (HC-time, p < 0.001; PD, p < 0.001; DA-Max, p = 0.001; WEM-Max, p < 0.001; and SP-A1, p < 0.001) of Corvis ST were significantly correlated with AL, and one parameter (Corvis biomechanical index, p = 0.016) has significant relationship with SE. With the increase of myopia, significant changes in several corneal biomechanical parameters indicated a progressive decrease in corneal stiffness, independent of bIOP and CCT. Corneal biomechanical parameters may be predictors of scleral mechanical strength in high myopia, which has certain application value in clinical management of myopia.
