Abstract:We report a case of ileo-colonic Histoplasmosis without apparent respiratory involvement in a patient who had previously undergone an orthotopic liver transplant (OLT) for primary biliary cholangitis 15 years earlier. The recipient lived in the United Kingdom, a non-endemic region for Histoplasmosis. However, she had previously lived in rural southern Africa prior to her OLT. The patient presented with iron deficiency anaemia, diarrhoea, abdominal pain and progressive weight loss. She reported no previous foreign travel, however, it later became known that following her OLT she had been on holiday to rural southern Africa. On investigation, a mild granulomatous colitis primarily affecting the right colon was identified, that initially improved with mesalazine. Her symptoms worsened after 18 mo with progressive ulceration of her distal small bowel and right colon. Mycobacterial, Yersinia, cytomegalovirus and human immunodeficiency virus infections were excluded and the patient was treated with prednisolone for a working diagnosis of Crohn’s disease. Despite some early symptom improvement following steroids, there was subsequent deterioration with the patient developing gram-negative sepsis and multi-organ failure, leading to her death. Post-mortem examination revealed that her ileo-colonic inflammation was caused by Histoplasmosis.
Uncertainty in neural perceptual systems

Abstract:Organisms face different kinds of uncertainty when perceiving their environment and they have to cope with this fact to survive. Such uncertainty might stem from unreliable sensors, ambiguities in the limited data about the physical world, or other imponderables. In order to enable optimal inferences from observed information, such uncertainty has to be taken into account and recent studies provide empirical evidence that this is realised in perceptual systems. In most cases recurrent processing structures form the basic machinery for dealing with this problem. This work examines how we can formally analyse example problems of perception in terms of information processing and how they can be solved by a consistent model of neural functioning. To provide a sound foundation for this goal, we describe basic facts about the underlying biological systems and the mathematical methods to analyse them. We start by reviewing existing approaches suggesting how recurrent processing architectures can deal with uncertainty and use the formal frameworks of Bayesian inference and information theory to enable a quantitative treatment. In more detail, we then describe different probabilistic interpretations of neural processing that represent uncertainty in the activation patterns of both single units and populations. We discuss how properties observable at the system level can be traced back to properties of its constituent elements. In the core part of this work, we describe and analyse a concrete model for the dynamics of a single processing unit that enables a direct interpretation of its variables in terms of an inference process. We derive implications for signal transmission in single units and how effects of attention can be accounted for. Single units of this type can be arranged in simple circuits to improve information transmission and results from computer simulations illustrate the properties of such circuits. The approach described in this work establishes a probabilistic interpretation for a simple dynamical system to enable a functional interpretation of its characteristics. Beyond precisely describing dynamical properties of perceptual systems this provides an answer to the question why it might " make sense " for a system to implement dynamics of a specific form. Interpreting perception as an inference task and showing that the specific dynamics solve this task in an optimal manner provides such an explanation.
Quantitative Analysis of Sagittal Curvatures of the Tibial Plafond by Computed Tomography

Abstract:Background: Although the talar morphology has been well understood, studies on the corresponding tibial plafond are still lacking. Based on computed tomography (CT) data, this quantitative study divided the tibial plafond into anterior and posterior regions on five sagittal sections. The objectives of this study were (I) to determine whether the sagittal curvatures of the tibial plafond can be quantitatively and accurately described using the double-diameter method; (II) to compare the difference between the anterior and posterior diameters on five sagittal sections. Methods: In this study, CT data were collected from 100 adult ankles, and the three-dimensional (3D) ankle joint model was reconstructed using CT images. An anatomical coordinate system of the 3D ankle joint model was created to establish the standard coronal and sagittal planes. The measurement outcomes of sagittal curvatures included: the anterior and posterior diameters, the distal tibial arc length (TiAL) and the distal tibial mortise depth (TMD) on five sagittal sections (the most medial, medial 1/4, middle, lateral 1/4 and the most lateral section). Subgroup analysis was performed to compare the differences between males and females. Results: Analysis of the sagittal curvatures showed that the anterior diameter of tibial plafond was significantly smaller than the posterior diameter on five sagittal sections with a mean difference ranging from 3.9 to 6.8 mm (P<0.001). For the anterior diameters, the anteromedial curve had the smallest diameter (35.3 +/- 5.3 mm), and the anterolateral curve had the largest diameter (38.0 +/- 5.8 mm). For the posterior diameter, the posteromedial curve had the smallest diameter (39.2 +/- 6.4 mm), and the posterolateral 1/4 curve had the largest diameter (43.5 +/- 6.9 mm). One-way analysis of variance (ANOVA) revealed significant differences in the anterior and posterior diameters among five groups (P<0.012). Subgroup analysis showed that gender partly affected the results of sagittal curvature measurements. Conclusions: The sagittal curvatures of the tibial plafond can be described quantitatively and accurately using anterior and posterior diameters. Our study showed that there were significant differences between the anterior and posterior diameters, and gender was an important factor influencing the sagittal curvatures of the tibial plafond.
The patellofemoral morphology and the normal predicted value of tibial tuberosity-trochlear groove distance in the Chinese population

Abstract:Abstract Background Our objective was to obtain normal patellofemoral measurements to analyse sex and individual differences. In addition, the absolute values and indices of tibial tuberosity-trochlear groove (TT-TG) distances are still controversial in clinical application. A better method to enable precise prediction is still needed. Methods Seventy-eight knees of 78 participants without knee pathologies were included in this cross-sectional study. A CT scan was conducted for all participants and three-dimensional knee models were constructed using Mimics and SolidWorks software. We measured and analysed 19 parameters including the TT-TG distance and dimensions and shapes of the patella, femur, tibia, and trochlea. LASSO regression was used to predict the normal TT-TG distances. Results The dimensional parameters, TT-TG distance, and femoral aspect ratio of the men were significantly larger than those of women (all p values 0.05). The Pearson correlation coefficients between the anterior femoral offset and other indexes were consistently below 0.3, indicating no relationship or a weak relationship. Similar results were observed for the sulcus angle and the Wiberg index. Using LASSO regression, we obtained four parameters to predict the TT-TG distance (R 2 = 0.5612, p < 0.01) to achieve the optimal accuracy and convenience. Conclusions Normative data of patellofemoral morphology were provided for the Chinese population. The anterior-posterior dimensions of the women were thicker than those of men for the same medial-lateral dimensions. More attention should be paid to not only sex differences but also individual differences, especially the anterior condyle and trochlea. In addition, this study provided a new method to predict TT-TG distances accurately.

CT Measurement of Proximal Femoral Medullary Cavity Morphology in Healthy Subjects from Northern China

Abstract:Abstract Purpose: In total hip arthroplasty (THA), optimal stability and fixation is achieved via tight adaptation of the prosthesis to the bone. This study aimed to analyze the proximal femoral medullary cavity morphology of Chinese subjects on three-dimensional reconstructed imagery to explore the factors influencing these parameters.Methods: CT was performed on 63 healthy subjects from northern China (94 hips; 31 men, 63 women). Three-dimensional CT reconstruction of the proximal femoral medullary cavity was performed using Mimics 22.0 software. The anatomical parameters related to THA were measured to examine the relationships between sex, age, and femoral length. These parameters were compared with previously published data.Results: The internal and external coronal diameters of the femoral medullary cavity, and the anterior and posterior sagittal diameters had different contracting trends. There were correlations between the femoral length, femoral head diameter, and coronal diameter, and between the sagittal diameter and cross-sectional area of the medullary cavity. Age was negatively correlated with the CFI(canal flare index). Sex was correlated with the cross-sectional coronal/sagittal diameters, but not with the canal flare index. The proximal end of the medullary cavity was similar to that reported in Caucasians, but the Asian medullary cavity was narrower distal to the level of the lesser trochanteric line.Conclusions: THA-related anatomy was affected by sex, age, and subpopulation. Compared with sex, age had a greater effect on the fit of the stem to the femoral canal. Each patient must be treated individually, as the anatomical structure varied between individuals.
Shape Approximation and Size Difference of the Upper Part of the Talus: Implication for Implant Design of the Talar Component for Total Ankle Replacement

Abstract:The implant design of the talar component for total ankle replacement (TAR) should match the surface morphology of the talus so that the replaced ankle can restore the natural motion of the tibiotalar joint and may reduce postoperative complications. The purpose of this study was to introduce a new 3D fitting method (the two-sphere fitting method of the talar trochlea with three fitting resection planes) to approximate the shape of the upper part of the talus for the Chinese population. 90 models of the tali from CT images of healthy volunteers were used in this study. Geometrical fitting and morphological measurements were performed for the surface morphology of the upper part of the talus. The accuracy of the two-sphere fitting method of the talar trochlea was assessed by a comparison of previously reported data. Parameters of the fitting geometries with different sizes were recorded and compared. Results showed that compared with previously reported one-sphere, cylinder, and bitruncated cone fitting methods, the two-sphere fitting method presented the smallest maximum distance difference, indicating that talar trochlea can be approximated well as two spheres. The radius of the medial fitting sphere RM was 20.69±2.19 mm which was significantly smaller than the radius of the lateral fitting sphere RL of 21.32±1.88 mm. After grouping all data by the average radius of fitting spheres, the result showed that different sizes of the upper part of the talus presented significantly different parameters except the orientation of the lateral cutting plane, indicating that the orientation of the lateral cutting plane may keep consistent for all upper part of the talus and have no relationship with the size. The linear regression analyses demonstrated a weak correlation (R2<0.5) between the majority of parameters and the average radius of the fitting spheres. Therefore, different sizes of the upper part of the talus presented unique morphological features, and the design of different sizes of talar components for TAR should consider the size-specific characteristics of the talus. The parameters measured in this study provided a further understanding of the talus and can guide the design of different sizes of the talar components of the TAR implant.
Development of a simplified, reproducible, parametric 3D model of the talus

Abstract:Computational foot models have significant application in surgical decision making, injury and disease diagnosis and prevention, sports performance analysis and footwear engineering. However, due to the substantial time in model building and the heavy computational costs from the complexity of the models, daily clinical application of these foot models has yet to be achieved. Much of the previous research adopted a detailed-geometry approach in modeling bones that potentially contributed to the heavy computational costs. In this research, we developed a computational talus model based on CT section image data, image reconstruction and segmentation, contact surface identification, standard shape fitting, and finite element auto meshing algorithms. Modeling the bones as rigid is common, and modeling the contact surfaces only for the rigid body saves additional computational resources. Priority, therefore, in the shape fitting with optimization is given to the contact surfaces of the talus. Thirteen sets (9 males and 4 females) of CT section data were obtained. Image reconstruction, segmentation and bone labeling were conducted on each set of CT data to identify talus and its adjacent bones. Contact surfaces of the talus were then identified based on bone spatial relationships. Apart from the talar dome surface which was fitted by a 3rd-order polynomial, standard shapes such as ellipsoids and planes were used to fit the selected contact surfaces so that the geometrical parameters maintain physical significance. Based on these parameters, we automatically recreated and meshed the least-squares fitted shapes rapidly with limited elements. Last, mean major contact surfaces of the talus were obtained and fitted by standard shapes. Although the number of samples in this study was relatively small, our method provides sufficient and accurate geometric parameters of these contact surfaces to completely describe and reproduce the talus, on both a subject specific and average basis. The method for describing the talus here helps to parametrize computational models using planes and ellipsoids, improves surgical decision making and implants with a more precise and physically significant measures, and the description provides bone geometric parameters which can later be used to relate risk analysis for bone shape specific injury rates.
Measurement of Posterior Slope of the Medial Tibial Plateau in Chinese

Abstract:Objective: To measure the posterior slope angle of the medial tibial plateau for providing parameter of the total knee arthroplasty and the knee prosthesis design that suits for Chinese. Methods: The angles were measured between the tangent of medial tibial plateau and the perpendicular in lateral X-rays of 160 osteoarthritic knees, 80 rheumatoid knees and 120 meniscus injury knees: anterior tibial cortex (Method one); tibial proximal anatomical axis(Method two); posterior tibial cortex (Method three). Results: The posterior slope angle were 11.74°±3.87°(range -1.5°～22.5°)with the method one; 8.91°±3.57°(range -2.5°～19.5°)with the method two; 6.21°±3.51°(range -6°～17°)with the method three. The values obtained with 3 methods were different but strongly correlated ( r0.85; P0.01 ), and were not different in diagnoses and in gender. The values weakly correlated with age and were smaller than that of foreign reported with the same methods. Conclusion: The posterior slope angle of the medial tibial plateau in Chinese is smaller. These results suggest that the posterior slope angle varies among individuals and the cutting angle should be determined in each case.
Statistical Shape Modeling of Mean Shape and Principal Variability of the Human Talar Bone in the Chinese Population

Abstract:Purpose The talar bone plays a crucial role in ankle biomechanics and stability. Understanding the shape variability of the talar bone within specific populations is essential for various clinical applications. In this study, we aimed to investigate the mean shape and principal variability of the human talar bone in the Chinese population using statistical shape modeling (SSM). Methods CT scans of 214 tali were included to create SSM models. Principal component analysis was used to describe shape variation among the male, female, and overall groups. Results The largest amount of variation among three groups ranges from 17.2%–18.8% of each variation. The first seven principal components (modes) captured 62.4%–67.5% of the cumulative variance. No dominant shape of the talus was found. Male tali generally have a larger size than the female tali, with the exception of the articular surface of the anterior subtalar joint. Conclusions SSM is an effective method of finding mean shape and principal variability. Considerable variabilities were noticed among these three groups and all principal modes of variation. No dominant talar model was found to represent the majority of tali, regardless the gender. Such information is crucial to improve the current understanding of talar pathologies and their treatment strategies.
Gender Variation in the Shape of Superior Talar Dome: A Cadaver Measurement Based on Chinese Population

Abstract:Understanding the shape of superior talar dome is essential for a better size compatibility between talar component of ankle implant and bone. The purpose of this study was to determine whether there were gender variations in (1) width (TW) and length (TL) of talus, as well as anterior width (DAW), middle width (DMW), posterior width (DPW), and length (DL) of superior talar dome; (2) differences between the DAW, DMW, and DPW; (3) the ratios between these parameters. Fifty-one cadaveric ankle specimens were included. Two observers measured all the specimens using vernier caliper. Intraclass correlation coefficients (ICCs) were used for intraobserver and interobserver reliability analysis and the reliability was thought to be good if the ICC>0.75. A two-tailed unpaired t-test or the rank-sum test was used to investigate gender variations. A single-factor ANOVA was utilized to identify the differences between the width of the superior talar dome surface and p value of <0.05 was considered significant. Intraobserver and interobserver reliability were good. Significant gender variations were found, in which TW, TL, DAW, DMW, DPW, and DL of female specimens were much smaller than those of male. The width of talar dome linearly decreased from DAW to DPW; however, the linearly decreased rate from anterior to posterior width was bigger in female. Moreover, significant differences were found in DAW/DPW, DMW/DPW, DL/DAW, DL/DMW, and DL/DPW between male and female. Based on our result, there was no difference in the 2D shape of the whole talus instead gender variation existed in the 2D shape of superior talar dome between male and female. The current 2D data could contribute to figure out more suitable size of talar component for Chinese population and might indicate a gender-specific shape of bone-implant interface, which could reduce the potential bone-component incompatibility when performing ankle replacement using standard component.
Measurement of femoral posterior condylar offset and posterior tibial slope in normal knees based on 3D reconstruction

Abstract:Abstract Background Femoral posterior condylar offset (PCO) and posterior tibial slope (PTS) are important for postoperative range of motion after total knee arthroplasty (TKA). However, normative data of PCO and PTS and the correlation between them among healthy populations remain to be elucidated. The purpose of this study was to determine PCO and PTS in normal knees, and to identify the correlation between them. Methods Eighty healthy volunteers were recruited. CT scans were performed followed by three-dimensional reconstruction. PCO and PTS were measured and analyzed, as well as the correlation between them. Results PTS averaged 6.78° and 6.11°, on the medial and lateral side respectively ( P = 0.002). Medial PCO was greater than lateral (29.2 vs. 23.8 mm, P < 0.001). Both medial and lateral PCO of male were larger than female. On the contrary, male medial PTS was smaller than female, while there was no significant difference of lateral PTS between genders. There was an inverse correlation between medial PCO and PTS, but not lateral. Conclusions Significant differences exhibited between medial and lateral compartments, genders, and among individuals. An inverse correlation exists between PCO and PTS in the medial compartment. These results improve our understanding of the morphology and biomechanics of normal knees, and subsequently for optimising prosthetic design and surgical techniques.

CT Measurements of the Proximal Femoral Medullary Cavity in Healthy Adults: a Cross-Sectional Study

Abstract:Objective: To analyse the proximal femoral morphology on three-dimensional reconstructed imagery to explore the factors influencing the relevant parameters. Method: The cross-sectional study was conducted at Peking University Third Hospital in northern China from January 2019 to August 2020, and comprised healthy adults who underwent computed tomography scanning. Three-dimensional computed tomography reconstruction of the proximal femoral medullary cavity was performed using Mimics 22. The anatomical parameters related to total hip arthroplasty were measured to examine the relationship among gender, age and femoral length. Data was analysed using SPSS 20. Results: Of the 63 adults, meaning 126 hips, 21(33.3%) were males, meaning 42(33.3%) hips, and 42(66.6%) were females, meaning 84(66.6%) hips. The overall mean age was 51.5±23.1 years (range: 23-68 years). The inflection point of the medullary cavity curved at 5-10mm distal to the lesser trochanteric line. Most horizontal plane parameters significantly differed between men and women (p<0.05), with the mean medullary cavity being wider in men than women. There was a significant difference between the genders in the sagittal anterior-posterior diameter of the canal flare index (p<0.05). Age was negatively correlated with the coronal medial-lateral diameter and coronal lateral diameter of canal flare index. In the coronal and sagittal planes, there was a positive correlation between the metaphysis and diaphysis, and the coronal and sagittal planes were positively correlated with the orthogonal plane. Conclusions: Femoral morphology could be influenced by gender and age. Morphological changes of the proximal femoral medullary cavity were not present in a single plane, but were affected by multiple planes. When the diameter of one plane became larger, its orthogonal plane concomitantly increased. Key Words: Proximal femoral, Morphology, CT measurement, Correlation.
Comparative Study of Sex Differences in Distal Femur Morphology in Osteoarthritic Knees in a Chinese Population.

Abstract:Objectives: The purpose of this study was to investigate sex differences in resected distal femoral morphology in Chinese osteoarthritic knees.Methods: The study included 130 osteoarthritic knees in 65 men and 65 women in China. None had anterior femoral osteophyte or serious patellar femoral joint degeneration. The following were measured using computed tomography and analyzed to identify morphological differences according to sex in the resected distal femurs: anterior lateral condylar height (ALCH), anterior medial condylar height (AMCH), and mediolateral (ML) and anteroposterior (AP) dimensions. The ML/AP aspect ratio was calculated.Results: The average ALCH and AMCH were 8.2 +/- 1.8 mm, 3.1 +/- 1.5 mm for men and 7.4 +/- 1.7 mm, 3.6 +/- 1.5 mm for women. There were significant differences between men and women in ALCH values (P = 0.014) but not in AMCH values (P = 0.09). Women had smaller ML/AP aspect ratios than men for a given AP dimension. This indicated that the femoral ML dimension of a prosthesis with a given AP dimension may have overhang in women.Conclusions: This study suggested that sex differences should be taken into account in the design of femoral prosthesis for Chinese men and women.
In-vivo measurementof the geometric rotation axis of the talocrucial joint during normal gait

Abstract:Objective:To measure and compare the geometric rotation axis of the talocrucial joint at different poses during the normal gait.Methods:The kinematic data of 15 healthy volunteers, 8 females, 7 males; age, 26.2±5.4 y (range 19-39 y); height, 170.4±6.9 cm (range 160-183 cm); mass, 65.6±14.0 kg (range 52-100 kg). They were collected using the dual-fluoroscopy technique, and the spatial position of the tibia and talus was determined using the 3D-2D registration method. The medial and lateral side of the trochlear of each talus was fitted by two spheres and the geometric rotation axis of the tibiotalar joint was defined as the line connecting the origins of two spheres. Compared the position and orientation of the axis for different poses during the gait and also compared the difference of the radius of the medial and lateral spheres fitting the trochlear of each talus.Results:The radii of the medial spheres fitting the trochlear of each talus 19.52±1.47 mm,were significantly less than that of the lateral spheres 20.62±1.49 ( t=7.081, P<0.05). The averaged anterior-posterior distance between the geometric rotation point of the tibiotalar joint were 1.54±1.84, 1.71±1.69, 1.70±1.57, 1.72±1.62, 1.80±1.75, 1.96±1.86 mm, respectively, while the averaged medial-lateral distance were 0.06±1.84, -0.03±1.83, 0.08±1.83, 0.10±1.73, 0.10±1.47, 0.09±1.46 mm, respectively, and the averaged superior-inferiordistance were -21.92±1.46, -22.10±1.32, -22.10±1.50, -22.06±1.64, -21.93±1.62, -21.98±1.50 mm, respectively. The averaged angle between the geometric rotation axis of the tibiotalar joint and coronal plane were 3.31°±2.48°, 3.10°±2.67°, 3.64°±2.71°, 3.96°±3.19°, 4.28°±2.82°, 4.16°±3.11°, respectively, while the averaged angle between that and sagittal plane were 84.11°± 2.42°, 83.77°±3.19°, 83.77°±3.45°, 83.81°±3.69°, 83.99°±2.97°, 84.23°±3.01°, respectively, and the averaged angle between that and transverse plane were 4.40°±2.93°, 4.54°±3.74°, 3.97°±3.34°, 3.73°±2.49°, 3.78°±2.76°, 4.48°±2.49°, respectively. The position and orientation of the geometrical rotation axis of the tibiotalar joint at different poses during the gait showed no significant difference ( P>0.05). The rotation axis orientated from laterally and inferiorly to medially and superiorly with an averaged inclination angle from the horizontal plane of 3.74° and an averaged deviation angle from the coronal plane of 4.15°. Conclusion:The geometric rotation axis of the tibiotalar joint was fixed during the gait, therefore fitting the talar trochlea with the two-sphere model with a small radius of the medial sphere and a large radius of the lateral sphere may better mimic the kinematics of tibiotalar joint.
Analysis of Variation in Sagittal Curvature of the Femoral Condyles

Abstract:In designing femoral components, which restore native (i.e., healthy) knee kinematics, the flexion-extension (F-E) axis of the tibiofemoral joint should match that of the native knee. Because the F-E axis is governed by the curvature of the femoral condyles in the sagittal plane, the primary objective was to determine the variation in radii of curvature. Eleven high accuracy three-dimensional (3D) femur models were generated from ultrahigh resolution CT scans. The sagittal profile of each condyle was created. The radii of curvature at 15 deg increments of arc length were determined based on segment circles best-fit to ±15 deg of arc at each increment. Results were standardized to the radius of the best-fit overall circle to 15 deg-105 deg for the femoral condyle having a radius closest to the mean radius. Medial and lateral femoral condyles exhibited multiradius of curvature sagittal profiles where the radius decreased at 30 deg flexion by 10 mm and at 15 deg flexion by 8 mm, respectively. On either side of the decrease, radii of segment circles were relatively constant. Beyond the transition angles where the radii decreased, the anterior-posterior (A-P) positions of the centers of curvature varied 4.8 mm and 2.3 mm for the medial and lateral condyles, respectively. A two-radius of curvature profile approximates the radii of curvature of both native femoral condyles, but the transition angles differ with the transition angle of the medial femoral condyle occurring about 15 deg later in flexion. Owing to variation in A-P positions of centers of curvature, the F-E axis is not strictly fixed in the femur.
Measurement of three-dimensional morphological characteristics of the calcaneus using CT image post-processing

Abstract:BackgroundAlthough computed tomography (CT) with three-dimensional (3D) rendering of the calcaneus is used for diagnostic evaluation of disorders, morphological measurements for the calcaneus are mostly based on a two-dimensional plane. The purposes of this study were to design a method for 3D morphological measurements of the normal calcaneus based on CT post-processing techniques, to measure morphological parameters in the male and female groups and describe gender differences of the parameters, and to investigate the reliability of such measurements.MethodsOne hundred and seventy-nine patients (83 men and 96 women) with a mean age of 40.6 (range, 21 to 59) years who underwent CT scans for their feet were reviewed retrospectively. The 3D structure of a normal calcaneus after shaded surface display reconstruction was extracted by interactive and automatic segmentation. Morphological measurements were achieved by means of a 3D measurement method based on CT image post-processing. Lengths and heights of the main parts of the calcaneus, Gissane’s angle, Böhler’s angle and the area of articular facet were worked out in 3D space. Gender-related size differences of parameters were compared using analysis of covariance (ANCOVA), adjusting for body height. Intra-observer and inter-observer reliabilities were assessed using intraclass correlation coefficients (ICCs) and the root mean square standard deviation (RMS-SD) for precision study.ResultsA large range of measurement values was found. Only the length of the anterior process was without gender difference (p > 0.05). The other parameters in the male group were greater than those in the female group (p < 0.01 for each, ANCOVA). All parameters had excellent reliability and reproducibility (ICC > 0.8). Precision was acceptable for intra-observer RMS-SD (linear, angular and areal measurements no more than 0.6 mm, 1.2° and 0.25 mm2, respectively). Inter-observer RMS-SD ranged from 0.4 to 1.6 mm for linear measurements, 1.2 to 2.5° for angles and 0.24 to 0.40 mm2 for areas.ConclusionsThree-dimensional morphological measurement based on a CT post-processing technique was highly reliable and repeatable for calcaneal anatomic morphological measurement. The current data will be helpful for anatomic reduction of calcaneal fractures and calcaneal malunion.

South China people patellar measurement and its clinical significance

Abstract:Objective To evaluate the characteristic parameters of the patellar of the south China people and its clinical significance.Methods DICOM 3.0 format image data collected from the total 103 cases of south China adult knee(male: 75,female: 28) were obtained by CT scan,and then were transformed by the Mimics10.01 software to achieve three-dimensional data of STL format.The STL file were further imported into the software Imageware to measure the maximal thickness(T),residual thickness(T1),resectable thickness(T2),maximal width(W),medial articular surface width(W1),lateral articular surface width(W2),maximal height(H),articular surface height(H1) of patella.Results The patella of the male were significantly bigger than the female as it is shown by all measured parameters(P<0.05).In the same gender groups,there was direct linear relationship between T and T1(male: r=0.72,female: r=0.79),and the linear correlations were statistically significant,respectively(P<0.05).Moreover,the results of regression equation demonstrated that T1,W1 and H1 were correspondingly increasing as T,W and H increased.Accordingly,the regression equations were statistically significant(P<0.05).Conclusions The characteristic parameters of patella analyzed in the present study facilitate the consideration of the resectable patellar thickness and residual patellar thickness for the patellar resurfacing and for the design of appropriate patellar prosthesis for the south China people.

Abstract:One hundred and thirty osteoarthritic knees(65 males, 65 females) from a Chinese population were measured by computed tomography for tibial mediolateral (ML), middle anteroposterior (AP), medial anteroposterior (MAP), lateral anteroposterior (LAP) dimensions and ML/AP aspect ratio. The ML/AP aspect ratio were classified into 3 groups based on AP dimensions(<48 mm, 48–52 mm, >52 mm) to compare the morphologic differences of proximal tibia between males and females. The mean ML, AP, MAP and LAP dimensions of proximal tibia showed significant differences for sex (P < .01). We found a progressively decreased in the ML/AP aspect ratio with an increasing AP dimension, and males have larger ML/AP aspect ratio than that of females under a given AP dimension (P < .01). This indicates that under a given AP dimension prosthesis, the tibial ML dimension have the potential to be undersized in males and to overhang in females. This study may provide important reference in designing proper gender-specific tibia prosthesis with different ML/AP aspect ratio for Chinese males and females.
Age estimation from median palatine suture using computed tomography reconstructed 3D images: a comparison of Northern and Southwestern Chinese populations

Abstract:To investigate the potential of computed tomography (CT) images of median palatine suture (MP) for adult age estimation in the Northern and Southwestern Chinese populations. A total of 1110 cranial CT scans from individuals aged 10–79 years, including 557 northern Chinese and 553 southwestern Chinese, were collected for analysis. After volume reformation and multiplanar reconstruction, a total of 20 slices of median palatine suture were selected from each individual. The closure of sutures was analyzed into four stages, and the cumulative scores of 20 slices were recorded as the suture closure score (SCS). The correlations between SCS and age were compared among the two Chinese populations residing in diverse geographic regions. Regression models were established for age estimation. The estimation accuracy was evaluated based on the test set. The mean absolute error (MAE) and the correlation between predicted age and chronological age were calculated to evaluate estimation accuracy. The SCS of MP exhibited a significant correlation with age (0.613, northern male; 0.678, southwestern male; 0.730, northern female; 0.704, Southwestern female; 0.662, total). Furthermore, there were statistically significant differences in SCS among different regions and sex groups ( p < 0.001). The cubic regression model had the highest R2 value in all subjects, especially among Northern females and Southwestern males, while the power and quadratic regression models showed the highest R2 value in Northern males and Southwestern females, respectively. In the test set, the Northern cohort demonstrated a lower MAE (9.06 ± 7.32 years, males; 9.17 ± 5.28 years, females) compared to the Southwestern cohort (9.19 ± 7.49 years, male; 10.61 ± 6.83 years, female). Additionally, it was observed that males exhibited a lower MAE than females in both regional groups. This study demonstrated the potential utility of CT images of the MP for age estimation in Chinese populations, emphasizing the significance of incorporating regional and sex factors within this context.

Abstract:Surgeons do not give enough weight to the effects of bowing of the sagittal femoral shaft in total knee arthroplasty (TKA), which can result in damage to the cortex, fractures, or malalignment of the femoral component. To determine gender differences in bowing, we used spiral computed tomography to scan the femurs of 26 men and 47 women older than 50 years who required TKA. Skeletal extraction of the total sagittal femoral shaft from computed tomographic images was done by a matrix laboratory. The extracted curves were evenly divided into 3 sections. Comparison of the curvature on different sections of the same side of the femur showed that the distal third was significantly bowed. In addition, the curvature of the distal third was significantly larger in women than in men. Such morphological characteristics put forward new requirements in how intramedullary guide rods are used in TKA.
