Abstract:Objective To introduce a new flap of cutaneous branches of posterior tibial artery on the part of superior medial malleolus to repair soft tissues defect on the feet and the ankles Methods From October 1997 to December 2001,a flap with the pedicle called cutaneous branches of posterior tibial artery on the part of superior medial malleolus were used to repair 11 cases which suffered skin and soft tissue defect on the feet and the ankles due to various trauma Results The flaps were survived completely on 10 cases and the flaps distal had about 2 cm necrosis on 1 case,the size of the flap ranged from 28 cn×11 cm to 8 cm×6 cm,all of the 11 cases were evaluated as satisfactory after 6 months to 2 years follow-up Conclusions The new procedure can repair mostly soft tissue defect on the foot and the ankle and does not sacrifice the major arteries. The flap is easy to be performed,and it is an safety and ideal method.
[Repair of Soft Tissue Defects of Lower Extremity by Using Cross-Bridge Contralateral Distally Based Posterior Tibial Artery Perforator Flaps or Peroneal Artery Perforator Flaps].

Abstract:OBJECTIVE:To discuss the feasibility of repairing soft tissue defects of lower extremity with a distally based posterior tibial artery perforator cross-bridge flap or a distally based peroneal artery perforator cross-bridge flap.METHODS:Between August 2007 and February 2010, 15 patients with soft tissue defect of the legs or feet were treated. There were 14 males and 1 female with a mean age of 33.9 years (range, 25-48 years). The injury causes included traffic accident in 8 cases, crush injury by machine in 4 cases, and crush injury by heavy weights in 3 cases. There was a scar (22 cm x 8 cm at size) left on the ankle after the skin graft in 1 patient (after 35 months of traffic accident). And in the other 14 patients, the defect locations were the ankle in 1 case, the upper part of the lower leg in 1 case, and the lower part of the lower leg in 12 cases; the defect sizes ranged from 8 cm x 6 cm to 26 cm x 15 cm; the mean interval from injury to admission was 14.8 days (range, 4-28 days). Defects were repaired with distally based posterior tibial artery perforator cross-bridge flaps in 9 cases and distally based peroneal artery perforator cross-bridge flaps in 6 cases, and the flap sizes ranged from 10 cm x 8 cm to 28 cm x 17 cm. The donor sites were sutured directly, but a split-thickness skin graft was used in the middle part. The pedicles of all flaps were cut at 5-6 weeks postoperatively.RESULTS:Distal mild congestion and partial necrosis at the edge of the skin flap occurred in 2 cases and were cured after dressing change, and the other flaps survived. After cutting the pedicles, all flaps survived, and wounds of recipient sites healed by first intention. Incisions of the donor sites healed by first intention, and skin graft survived. Fifteen patients were followed up 7-35 months with an average of 19.5 months. The color and texture of the flaps were similar to these of the recipient site. According to American Orthopaedic Foot and Ankle Society (AOFAS) ankle and hindfoot score system, the mean score was 87.3 (range, 81-92).CONCLUSION:A distally based posterior tibial artery perforator cross-bridge flap or a distally based peroneal artery perforator cross-bridge flap is an optimal alternative for the reconstruction of the serious tissue defect of contralateral leg or foot because of no microvascular anastomosis necessary, low vascular crisis risk, and high survival rate.
Pressure boost in repair of soft tissue defect in limbs by free transplantation of thinned anterolateral thigh perforator flap

Abstract:Objective:To explore the surgical technique and clinical effect of pressure boost in repairing soft tissue defects of limbs with thinned anterolateral thigh perforator flap (ALTP) .Methods:From January, 2015 to December, 2018, 18 cases with soft tissue defects of limbs with various damages of blood vessels and nerves with explosure of tendon and bone. There were 13 males and 5 females aged between 18 to 56 (averaged of 36.3) years, which were 6 defects in shank, 4 in foot and ankle, 5 in forearm, and 3 in hand. The soft tissue defect area was 7 cm ×12 cm to 13 cm ×30 cm. Thinned ALTP was used to repair the wound surface. The perforating vessels of the distal flap were anastomosed with one branch of the internal vessel pedicle flap to increase the pressure hence the blood supply of the distal region. The donor sites were sutured directly or covered by skin graft. Followed-up was conducted by 1-2 monthly clinic visits and telephone or on-line review to check the flap survival and recovery of functions.Results:All flaps survived without arterial or venous crisis. One flap had partial necrosis at the distal end, and healed after dressing change. One case had a swelling flap due to a congestion beneath the flap. The wound achieved primary healing after removal of sutures, ligation of subcutaneous vessels and drainage of hematoma. All patients were followed-up for 6 to 18 (average, 9.5) months. All flaps had good appearance and texture. After rehabilitation treatment, most of the joint activity had been recovered: extension and flexion of wrists joints ranged 60°-80°, 70°-80° for metacarpophalangeal joints and 40°-60° for ankle joints. One patient underwent ankle joint dorsiflexion function reconstruction and flap thinning at 6 months after operation due to the defects of most of the extensor tendon.Conclusion:During the use of free ALTP to repair soft tissue defect of limbs, application of the technique of pressure boost is able to increase blood supply to the distal region of flap. It helps to reduce the incidence of infection and necrosis at the edge of the flap.

Abstract:Purpose To study the availability and effects for application of 4 kinds of perforator flaps in repairing skin and soft tissue defects in different positions around the knee joints. Methods The present study included 26 cases (16 males and 10 females) with soft tissue defects with ages from 7 to 82 years who came to our hospital during September 2012 to August 2016. The wound size ranged from 6 x 3 cm to 15 x 12 cm, and the exposed area ranged from 4 x 2 cm to 10 x 8 cm. Patients were treated with different perforator flaps (descending genicular artery perforator flap, sural neurocutaneous flap, lateral superior genicular artery perforator flap, and lateral popliteal artery perforator flap) and were followed up for 6 months. Results All the transplanted flaps survived well without advent events. Exposed bone, ligament, and skin and soft tissue defects healed well. In the early period, flaps were locally bloated. In the later period, the appearance of flaps was good and close to normal, with abrasion-resistant surfaces. All patients were satisfied with the flap appearance and functional recovery. No secondary necrosis, ulcers, deep infection, or nonunion of bone occurred during the follow-up. Conclusions These 4 kinds of perforator flaps are available methods to repair the skin and soft tissue defects around the knee joints. Suitable perforator flaps should be chosen for different locations of wounds. Level of Evidence Therapeutic study, level IV.
[Clinical Effects of Free Thinned Deep Inferior Epigastric Artery Perforator Flap in Repairing Extensive Soft Tissue Defects in Extremities].

Abstract:Objective: To explore the feasibility and clinical effects of using free thinned deep inferior epigastric artery perforator flap to repair extensive soft tissue defects in extremities. Methods: From April 2010 to January 2014, 12 patients with extensive soft tissue defect in extremities after trauma, including 10 males and 2 females, aged 21 to 48 years, 6 patients with defect in the back of wrist and 6 patients with defect in ankle were admitted to the Department of Bone Microsurgery of Xi'an Honghui hospital. After debridement, the size of soft tissue defect ranged from 15.0 cm×4.5 cm to 28.0 cm×11.0 cm. The free thinned deep inferior epigastric artery perforator flap was designed, cut and transferred for reconstruction, with size of 15.0 cm×5.0 cm to 29.0 cm×12.0 cm. The flap thickness ranged from 4.0 to 6.5 cm before defatting, and was 0.6 to 0.9 cm after defatting. All the donor sites of flaps were closed directly by suturing. The flap survival and the appearance and function of flap and donor site were observed during follow-up. Results: All the flaps survived smoothly after surgery. During follow-up of 10 to 42 months, the flaps showed no bloat in appearance, no further flap revision or defatting procedures were required, the distance of static 2-point discrimination was 11 to 17 mm (14.5 mm on average). The abdominal function of patients was not affected, and no postoperative abdomen hernia or ulceration was noted. Conclusions: The free thinned deep inferior epigastric artery perforator flap is thin and suitable for repairing extensive soft tissue defects in extremities with very good outcomes.

Abstract:Objective Analysis of the anterior tibial skin defects using the clinical effect of the superficial peroneal artery antegrade island flap repair.Methods Anterior tibial admitted to our hospital from May 2010 to March 2011,18 patients with skin defects as the object of study.Superficial peroneal artery antegrade island flap repair method,patients were followed up for 8 weeks to 6 years,the flap survival evaluate and compare different postoperative period in patients with limb resting a sense of numbness.Results 18 patients with flaps all survived patients flap partial necrosis healed after treatment,patients with limb resting a sense of numbness gradually weakened over time,completely disappeared after 16 weeks,resting a sense of numbness The disappearance time comparison(P<0.05).Conclusion The anterior tibial skin defects using peroneal artery antegrade island flap repair not only surgery is safe and reliable,and the effects do not affect significantly the main vessel,the preferred method for the treatment of skin defects of the anterior tibial.

Abstract:Objective To explore the clinical effects of sequential flap of thigh perforator for closure of the anterolateral thigh free flap donor site.Methods From March,2014 to May,2015,8 consecutive patients underwent elective anterolateral thigh free flap reconstruction for limb soft tissue defect.To design a sequential flap of thigh perforator to achieve primary closure of the anterolateral thigh free flap donor site.The area of anterolateral thigh free flap ranged from 9.0 cm×6.0 cm to 20.0 cm×1 1.0 cm,respectively.The area of thigh perforator flap ranged from 7.0 cm×3.0 cm to 12.0 cm×7.0 cm.Results All flaps healed uneventfully.Distal skin flap necrosis appeared in 1 flap which was healed after local transposition flap.All the patients were followed up for 1 to 6 months,with mean time of 4 months.The color,texture and appearance of flaps were satisfactory.Conclusion Sequential flap of thigh perforator,with reliable blood supply and constant position,represents an ideal model to close the anterolateral thigh free flap donor site of small defect.
Clinical application of posterior tibial artery perforator flap within saphenous nerve branch for sensory reconstruction

Abstract:Objective To explore the operative technique and clinical results of posterior tibial artery perforator flap within saphenous nerve branch for sensory reconstruction.Methods From January,2016 to June,2018,9 patients suffered from soft tissue defect were treated by the posterior tibial artery perforator flap containing saphenous nerve branch.Seven patients were males and 2 were females,with age ranged from 31 to 62 years.Soft tissue defects located in hands in 5 patients,plantar in 2 patients,ankle in 1 patient and dorsal foot in 1 patient.The size of soft tissue defects ranged from 8.0 cm×2.5 cm to 21.0 cm×4.0 cm.The regular post-operative followed-up was performed.Results All flaps survived without complications.The size of flap ranged from 10.0 cm×3.5 cm-23.0 cm×5.0 cm.Donor sites were primarily closed in 5 patients and secondary closed in 4 patients.Followed-up ranged from 6 to 15 months with 10 months in average.The contour of flaps were satisfied and the sensory function of the donor sites were normal.At 6 months followed-up,SW test reached 5.07 in all flaps,and 2PD ranged from 14 to 35 mm.Conclusion The novel sensory flap can provide satisfied sensory outcome without sacrificing main artery and saphenous nerve,and is a good candidate for sensory reconstruction of soft tissue defects.
[Application of Augmented Reality Technique in Repairing Soft Tissue Defects of Lower Limbs with Posterior Tibial Artery Perforator Flap].

Abstract:Objective:To investigate the accuracy and reliability of augmented reality (AR) technique in locating the perforating vessels of the posterior tibial artery during the repair of soft tissue defects of the lower limbs with the posterior tibial artery perforator flap. Methods:Between June 2019 and June 2022, the posterior tibial artery perforator flap was used to repair the skin and soft tissue defects around the ankle in 10 cases. There were 7 males and 3 females with an average age of 53.7 years (mean, 33-69 years). The injury was caused by traffic accident in 5 cases, bruising by heavy weight in 4 cases, and machine injury in 1 case. The size of wound ranged from 5 cm×3 cm to 14 cm×7 cm. The interval between injury and operation was 7-24 days (mean, 12.8 days). The CT angiography of lower limbs before operation was performed and the data was used to reconstruct the three-dimensional images of perforating vessels and bones with Mimics software. The above images were projected and superimposed on the surface of the affected limb using AR technology, and the skin flap was designed and resected with precise positioning. The size of the flap ranged from 6 cm×4 cm to 15 cm×8 cm. The donor site was sutured directly or repaired with skin graft. Results:The 1-4 perforator branches of posterior tibial artery (mean, 3.4 perforator branches) in 10 patients were located by AR technique before operation. The location of perforator vessels during operation was basically consistent with that of AR before operation. The distance between the two locations ranged from 0 to 16 mm, with an average of 12.2 mm. The flap was successfully harvested and repaired according to the preoperative design. Nine flaps survived without vascular crisis. The local infection of skin graft occurred in 2 cases and the necrosis of the distal edge of the flap in 1 case, which healed after dressing change. The other skin grafts survived, and the incisions healed by first intention. All patients were followed up 6-12 months, with an average of 10.3 months. The flap was soft without obvious scar hyperplasia and contracture. At last follow-up, according to the American Orthopedic Foot and Ankle Association (AOFAS) score, the ankle function was excellent in 8 cases, good in 1 case, and poor in 1 case. Conclusion:AR technique can be used to determine the location of perforator vessels in the preoperative planning of the posterior tibial artery perforator flap, which can reduce the risk of flap necrosis, and the operation is simple.

Abstract:Objective To explore the method and effect of the conformably and moderately modified anterolateral thigh perforating branch free flap in treatment soft tissue defectsMethods 11 patients with the soft tissue defects in the head,face and the four limbs were repaired by the conformably and moderately modified anterolateral thigh perforating branch free flap.the defects were treated by the flap with 1 or 2 perforating branch designed according to the size,shap and the amount of tissue defect,the giant donor site was closed with the intermediate split thickness skin graft.Results The flaps survived in all 11 patients and the wound had a primary healing,approximately 1.5cm epidermis necrosis was found in the distal end of one flap.After 1～24months follow-up,it showed good appearance in the flap and the donor site.Conclusion The application of the conformably and moderately modified anterolateral thigh perforating branch free flap is an ideal choice for repairing soft tissue defects with less donor site morbidity,flexible tissue cut and better result at the recipient site.
Free peroneal artery perforator flap for reconstruction of traumatic limb soft tissue defects: A retrospective case series study

Abstract:BackgroundThe free peroneal artery perforator (FPAP) flap is used for soft tissue defects after burns and trauma. However, the use of FPAP flaps to repair limb soft tissue defects for immediate reconstruction was rarely reported previously. Therefore, the purpose of this report is to evaluate free peroneal artery perforator flap to reconstruct traumatic limb soft tissue defects for immediate reconstruction. Patients and MethodsA total of 25 cases of limb soft tissue defects undergoing immediate reconstruction of FPAP flap transfer were retrospectively evaluated from January 2019 to June 2019 in our institute. The locations of defects included the palm (10 cases), finger (5 cases), foot (7 cases), ankle (2 cases) and wrist (1 case). The sizes of defect varied from 3 x 2 cm to 15 x 7 cm (54.1 cm(2) in average). Flaps were harvested based on the peroneal perforator vessels, initially marked using hand-held Doppler. ResultsAverage size of harvested flap was 9.7 x 6.2 cm (ranging from 3.5 x 2 cm to 16 x 8 cm). All perforators were harvested from the peroneal artery and the arterial diameter ranged from 0.8 to 1.7 mm. The average pedicle length was 3.04 cm (range, 1.85-4.75 cm). Five vascular thrombosis were found including three cases of arterial thrombosis and two cases of venous thrombosis which were successfully salvaged by re-operation and vein graft. Satisfying functional outcome and acceptable appearance were achieved at 6 months or longer after surgery (range, 6-15 months, 12 months in average). All flaps survived at the end-point. ConclusionsThe FPAP flap is a reliable and thin fasciocutaneous flap, which can be used for repairing limb soft tissue defects. The FPAP flap can be used for covering defects with various appearances, locations, and sizes.
Anatomical Study and Clinical Application of the Vascularized Composite Fibula Perforating Osteoseptocutaneous Flap

Abstract:Objective To report the anatomical study and clinical application of the vascularized composite fibula perforating osteoseptocutaneous flap. Methods Latex injection studies were performed on 24 adult cadaveric lower limbs. The presence, prevalence, and location of the peronel artery and its perforators in the distal leg were documented. 16 vascularized fibul osteoseptocutaneous flaps were performed for reconstruction of composite soft-tissue defect of limbs. The flap areas ranged from 6 cm×4 cm to 16 cm× 8 cm. Results A mean of 5.3 perforating vessels of the peroneal artery was noted in the specimen. 40.6% of them were muscularcutanous perforators, and the other were septacutanous perforators. The perforating vessels were all located within 3.1 cm from the postlateral fibula. The average diameter was 1.1 mm at the deep fascial level, and the interval of them was 4-8 cm. There were two constant perforators which located about 15-25 cm and 4-7 cm from the lateral maleolar respectively. All of the 16 clinical cases survived uneventfully and the time to union of the graft-host junction site were 3 to 5 months. The appearance of flaps and the function of the limbs were satisfied during 10 to 36 months following up. Conclusion It is the anatomic basis that there are pedorating vessels of the peroneal constantly. The vnscularized compound fibula perforating osteoseptocutaneous flap has the advantage of flexible design. It is the safe and suitable choice in the cases when compound bone and soft tissue defects of the limbs are required for reconstruction.
Direct popliteal artery perforator flap: anatomical study and clinical application

Abstract:Objective To investigate and evaluate the clinical effect of the direct popliteal artery perforator flap.Methods Thirty embalmed lower limbs of adult cadavers which perfused with red latex were used to dissection,major observations were the origin,perforators and anastomoses regulations of the direct popliteal artery.Based on the anatomical study,direct popliteal artery perforator flaps were designed and used clinically to 7 patients who had soft tissue defects in popliteal fossa.Results The direct popliteal artery perforator was direct started from lateral wall of the superior segment of popliteal artery,and about 7-11 cm above knee joint.Then it goes up along the middle line of posterior region of thigh,and pierced from the carvity between semitendinosus and biceps femoris and gave off several branches in superficial fascia.Finally,these branches anastomoses with many perforating branches which were gave off form deep femoral artery (the 1st to 3rd perforator artery),obturator artery and lateral circumflex femoral artery.Clinically,all flaps were survived eventually,and 6 of them were healed quickly,only 1 case needed to change dressings to healed its partial necrosis for the pedicle had too much soft tissues and too swelling to block its blood supply.After 2-12 months of following-up,the colors and appearances of these flaps were excellent,and the function of knees were nearly normal.Conclusion Direct popliteal artery perforator flap has relatively constant location and sufficient blood supply to use in clinical application,it is safe and easy to use for recovering soft tissue defects in popliteal fossa.
[Modified anterolateral thigh perforator flap pedicled by cross-bridge microvascular anastomosis for repairing soft tissue defects in middle and lower segments of leg]

Abstract:Objective: To explore the effectiveness of modified anterolateral thigh perforator flap pedicled by cross-bridge microvascular anastomosis in treatment of soft tissue defects in the middle and lower segments of the leg. Methods: Between March 2011 and June 2015, 15 cases with skin and soft tissue defects in the middle and lower segments of the legs were treated. There were 9 males and 6 females, aged 22-48 years (mean, 32.6 years). Of whom, 8 patients caused by traffic accidents, 5 by machine twist, and 2 by crash injury of heavy object. The mean interval from injury to admission was 82.6 hours (range, 2 hours to 1 week). The area of defect ranged from 13 cm×9 cm to 23 cm×16 cm. After primary debridement and vaccum sealing drainage treatment, the defects were repaired with modified anterolateral thigh perforator flap pedicled by cross-bridge microvascular anastomosis. The size of flap ranged from 15 cm×10 cm to 25 cm×15 cm. The donor sites were sutured directly or repaired with the skin grafts. The pedicle division was done at 4 weeks after operation. Results: After operation, venous crisis occurred in 1 case and distal skin necrosis in 2 cases which was healed by dressing change. The other tissue flaps survived successfully and wounds healed by first intention. All skin grafts at donor site survived after operation, and primary healing of wound was obtained. All patients were followed up 6-24 months (mean, 13 months). All flaps were characterized by soft texture, satisfactory appearance, and restoring the protective sensation. Moreover, the two-point discrimination ranged from 15 to 28 mm (mean, 19.5 mm) at 6 months after operation. The function of both lower extremities were normal without obvious contracture of scar at donor site. Conclusion: Modified free anterolateral thigh perforator flap, with little damage in donor site, a reliable blood supply by making a cross-bridge microvascular anastomosis with pretibial or posterior tibial blood vessel on normal leg, is a reliable alternative method for repairing soft tissue defects with the main vessels of serious injury in the middle and lower segments of the leg.
[Antegrade Extended Peroneal Artery Perforator Flap for Knee Reconstruction].

Abstract:OBJECTIVE:To investigate the operative technique and clinical results of repairing the soft tissue defects of knee with antegrade extended peroneal artery perforator flap.METHODS:From October 2007 to January 2008, 3 patients (2 men and 1 woman) with the soft tissue defects of knee were treated, with the ages of 18, 31 and 42 years, respectively. The first case sustained femur and pelvis fractures and soft tissue defect over his right popliteal fossa, which were treated with open reduction and internal fixation (ORIF) and debridement of knee joint 2 weeks ago. The second case was necrosis of skin 3 weeks after ORIF for fracture of tibial plateau. The third case suffered from open fracture of tibial plateau and soft tissue defect, which were treated with external fixation and debridement 3 weeks ago. The defect sizes were 16 cm x 9 cm, 11 cm x 6 cm and 14 cm x 7 cm. The flap was raised by dividing the peroneal artery and veins distally and elevating them proximally, which covered for the defects of knee. The flaps were designed with the size of 18 cm x 10 cm, 12 cm 7 cm and 15 cm x 8 cm. The pure vascular pedicle of the flap was 10 cm to 17 cm in length, including the peroneal vessels and one or two perforator branches. The donor site is covered by a split thickness skin graft.RESULTS:All flaps survived after surgery. The donor sites healed by first intention and the skin grafts survived. After following up for 6, 8 and 11 months, the appearance and function of the flaps were all satisfactory. Based on the modified HSS knee performance system, post-operative knee functional outcomes of three patients were excellent.CONCLUSION:The antegrade extended peroneal artery perforator flap supplied by a pure vascular pedicle can be a good alternative for reconstruction of knee. The flap, with a long and thin pure vascular pedicle, could provide good texture and contour matching the recipient area.

Abstract:Objective Significant anatomical variability of perforator vessel can affect the perforator flap application in accuracy,randomness,and the final result of the repair.Multidetector-row computed tomography angiography(MDCTA) is an useful preoperative identification of perforator vessel,and which is discussed as a guidance to the clinical utilities of "free-style" perforator flap in lower extremity.Methods From February 2011 to December 2012,forty patients with soft tissue defects in lower extremity were treated.Preoperatively MDCTA was performed on all patients to assess features of the perforating branches,including their location,caliber,course,structure and relationship of neighboring tissues.Various perforator flaps for soft tissue reconstruction using dominant perforator vessels were designed.Moreover the coincidence rate between preoperative and intraoperative location of the perforator vessel was evaluated.Results All the flaps which consisted of 19pedicled propeller flaps and 21 free flaps survived postoperatively.Only 1 case which involved the descending genicular artery perforator flap presented with a 2 cm × 2 cm distal necrosis which healed after regular dressing.For 37 cases,the donor site were primarily closed while partial skin grafting was performed in 3 cases.The rate of coincidence between the preoperative and intraoperative location of perforating branches was 97.5％.Conclusion Preoperative MDCTA can provide precise anatomical information on the perforator vessel.For sofi tissue defects reconstruction in the lower extremity,MDCTA makes the flap design easier and more accurate,and the surgery simpler and safer,thereby reducing damage to donor site and achieving the best repair.
Free Extended Posterior Tibial Artery Perforator Flap with the Neurovascular Plexus of a Saphenous Nerve Branch for Large Soft Tissue and Sensory Reconstruction: Anatomic Study and Clinical Application.

Abstract:Background The posterior tibial artery perforator (PTAP) flap is a useful tool for reconstruction of soft tissue defects in the leg. However, the size and reliability of the flap largely depends on the quality of the perforator by which the flap is supplied, and the sensory recovery of the flap is limited. In this study, the anatomy of the saphenous nerve branches and their accompanying vessels was investigated, and a free extended PTAP flap with the neurovascular plexus of a saphenous nerve branch was designed for large soft tissue and sensory reconstruction in a series of clinical cases. Methods Sixteen adult cadaveric legs perfused with red latex in the femoral artery were dissected. The number and location of the saphenous nerve branches and the features of their accompanying vessels were dissected and studied. From January 2016 to December 2017, six patients with soft tissue defects ranged from 8 x 2.5 cm to 21 x 4 cm were repaired by the free extended PTAP flap. The patients' average age was 48 years. The causes of the defects included machine injuries in three patients and traffic injuries in the other three. The defects located at the hand in three cases, foot in two cases, and ankle in one case. The flap was designed based on the perforators of the posterior tibial artery and included a branch of saphenous nerve. The perforator pedicle and the nerve branch were connected to the vessels and nerve in the recipient site, respectively. Results The saphenous nerve gave off 5.8 +/- 1.1 branches, with a relatively constant one issuing 8.1 +/- 0.7 cm distal to the medial femoral condyle. Every nerve branch had an accompanying vessel, which connected with the PTAPs and supplied the skin. The size of the flap ranged from 10 x 3.5 cm to 23 x 5 cm. All of the flaps survived completely without complications. Follow-up varied from 6 to 12 months. All the patients obtained cold/hot sensation and pain sensation. The results of Semmes-Weinstein monofilament test ranged from 4.31(2 g) to 5.46 (26 g), and the 2-point discrimination test varied from 20 to 35 mm. Conclusion The free extended PTAP flap, containing the saphenous nerve branch and its accompanying vessels, may be an alternative for large soft tissue reconstruction with improved sensation recovery.

Abstract:Objective To discuss clinical application of reversed tibialis posterior artery perforator-based saphenous neurocutaneous flap for treatment of skin and soft tissue defect around the foot and ankle joint.Methods 48 patients had been treated with perforator-based saphenous neurocutaneous flap for repairing skin and soft tissue defect around the foot and ankle joint.Simple reversed tibialis posterior artery perforator-based saphenous neurocutaneous flap was applied in 43 cases;musculocutaneous flap was applied in 5 cases.There are 29 fascial-perforator-based saphenous neurocutaneous flaps and 19 perforator-based saphenous neurocutaneous flaps.The area of soft tissue defects ranged from 10 cm×9 cm to 4 cm×3 cm.The area of flaps ranged from 11 cm×9.5 cm to 6 cm×5 cm.Results 43 flaps survived completely;5 flaps encountered partial necrosis of distal portion,were treated by dressing change or secondary skin graft.All flaps were slightly swelling postoperatively without congestion.All the patients have been followed up for 6 to 18 months.The cosmetic appearance and texture of flaps were satisfied without corpulenced pedicle,the protective sensation was renewed in donor site after transplanting cellulocutaneous flap.Conclusion Reversed tibialis posterior artery perforator-based saphenous neurocutaneous flap with reliable in blood-supply and modest texture is a good option for repair of skin and soft tissue defect around the foot and ankle joint.

Abstract:Objective To explore the clinical efficacy of minitype vascularized superficial peroneal artery's singleness perforator flap,and then to accurately repair skin and soft tissue defects of hands and foots. Methods From November 2009 to January 2011,eight cases(one case of left foot,one case of right foot,three cases of left hand and 3 cases of right band,1.3 cm × 5.0 cm - 2.5 cm× 6.0 cm of skin and soft tissue defects)were treated by minitype vascularized superficial peroneal artery's singleness perforator flap.Based on preoperative applied anatomy papers, it was the superficial peroneal artery's perforator position in the middle of the lateral lower leg to the fibula head; We designed the flap based on the size and shape of skin defect,and then to analysize the flap design,lap cut,he vascular anastomosis of flap and recipient,effect and characteristics of survival. Results All the flaps(1.5 cm × 5.2 cm-2.8 cm × 6.2 cm) survived and satified in shape and texture, they acquired good functional recovery postoperation; There was blister,dark purple but survival after the scab off on 1 patient; There was phalanx ostomyelitis and healed after treatment on 1 patient. Conclusion The clinical effects were satisfactory for repairing small skin and soft tissue defect of hands and foots by minitype vascularized superficial artery's singleness perforator flap.

Abstract:Anatomy of cutaneous perforators of the posterior tibial artery were studied in 20 limbs of 10 cadavers. The majority of the perforators (n = 74, 61%) were located in the middle two quarters of the leg, at an average of 18.6 cm (s.d. 4.5 cm; range 10.5–26 cm) from the medial malleolus, or around 54% (s.d 16%) of the length of the leg. There were usually 3 or 4 perforators in this region, with an average caliber of 1.5 mm (s.d. 0.2 mm; range 1–2 m.m.) and an average length from the posterior tibial artery to the skin of 4.0 cm (s.d. 1.3 cm; range 2.5–6 c.m.). A free fasciocutaneous skin flap based on one of these perforators (the posterior tibial perforator flap, PTP flap) was successfully transplanted in 6 cases.
