Abstract:This article presents an intensive analysis of a good-outcome case of emotion-focused therapy—the case of Lisa—using the Innovative Moments Coding System (IMCS). IMCS, influenced by narrative therapy, conceptualizes narrative change as resulting from the elaboration and expansion of narrative exceptions or unique outcomes to a client's core problematic self-narrative. IMCS identifies and tracks the occurrence of five different types of narrative change: action, reflection, protest, reconceptualization, and performing change. This is the first attempt to use the IMCS with cases outside the narrative tradition. We discuss the results, emphasizing the commonalities and major differences between this case and other good-outcome cases.

Abstract:OBJECTIVE: Innovative moments (IMs) are moments in the therapeutic dialog that constitute exceptions toward the client's problems. These narrative markers of meaning transformation are associated with change in different models of therapy and diverse diagnoses. Our goal is to test if IMs precede symptoms change, or, on the contrary, are a mere consequence of symptomatic 15 change.METHOD: For this purpose, IMs and symptomatology (Outcome Questionnaire-10.2) were assessed at every session in a sample of 10 cases of narrative therapy for depression. Hierarchical linear modeling was conducted to explore whether (i) IMs in a given session predict patients' symptoms in the following session and/or (ii) symptoms in a given session predict IMs in the next session.RESULTS: Results suggested that IMs are better predictors of symptoms than the reverse.CONCLUSIONS: These results are discussed considering the contribution of meanings and narrative processes' changes to symptomatic improvement.

Abstract:This study aims to assess moments of meeting in psychotherapy -understood as moments of intense connectedness and intimacy, shared between patient and therapist during any therapeutic encounter that enable a spin in the therapy process- using a qualitative interview with patients who were undergoing or had finished psychological treatment. Micro-pehenomenological interviews were conducted with nine patients who were undergoing or had finished psychological treatment. Transcriptions of the interviews were subject to micro-phenomenological analysis. A general structure of episodes of meeting showing their temporal evolution was identified and divided into six consecutive phases. These findings suggest that episodes of meeting in psychotherapy are lived and remembered by patients in a significant way; they are emotionally charged and have a meaning for each participant. Also, emotional and nonverbal cues seem to be highly relevant features. Moments of meeting could be understood as implicit mechanisms that allow changes in the implicit relational domain.

Abstract:Starting from two accounts of psychological change in therapeutic settings, based respectively on the analysis of metaphors (Faccio et al. 2011, this issue) and on transformative moments in self narratives (Ribeiro and Gonçalves 2011, this issue), this paper examines core processes of psychological change. Drawing on sociocultural psychology, the paper first argues that core processes of change in such therapeutic settings take place at the level of the organization of a person's semiotic sets. Second, the paper suggests that such therapeutic frames are likely to provoke changes in other aspects of a person's life as these aim at transforming the persons thinking capacities, and as these have as objects situations external to them.

Abstract:This project entailed an intensive qualitative analysis of six-session psychotherapies conducted by three eminent humanistic psychotherapists working with bereaved clients. The Innovative Moments Coding System (IMCS), rooted in narrative therapy, is designed to measure change across therapy orientations. Research using the IMCS suggests that the psychotherapy change process occurs through the emergence, elaboration, and expansion of identifiable change moments for a client—innovative moments (IMs)—which present as exceptions to a client's presenting problematic narrative. There are five identified types of IMs: action, reflection, protest, reconceptualization (RC), and performing change (PC). The current study aimed to inform theory regarding the patterns of IMs across three humanistic approaches—constructivist, person-centered, and existential—when working with bereaved clients, while linking these patterns to observable change in each client's functioning. The alliance between each client and therapist was also assessed across the therapy process, showing consistently strong alliances across the three cases. Findings from the current study reinforce the salience of reflection, RC, and PC IMs in successful grief therapy cases, and also suggest the importance of meaning-making interventions in grief therapy. Clinical implications and suggestions for future research are also addressed.

Abstract:Experiences of change from the perspective of both chronically conflictual couples and their therapist has not been explored, thus inhibiting current understanding of what is important for in-session change. The present study examines couple and therapist experiences of therapy for chronically conflictual couples. Sixteen interviews from four couples ( n = 8) and their therapists ( n = 4) were analyzed using Colaizzi's method of phenomenological inquiry. Results highlight environmental, couple, and therapist factors that are integral to experiencing change in session. Results show the importance of therapists facilitating intentional, non-conflictual, and connecting couple interactions. We discuss clinical implications of the findings.

Abstract:This overview addresses for whom and under what conditions psychotherapy produces change, the course of change over sessions, and why change happens. Specifically, the most confirmed moderators are pretreatment symptoms level, readiness for change, assertiveness, agency, defense maturity, quality of object relationships, mentalization, and interpersonal functioning. Regarding the course of change, processual models better describe change than stage ones, capturing its nonlinear and time-embedded nature. Moreover, trajectories of change describe different classes of change by clients (i.e., from three to five) and might be helpful feedback for clinicians. Furthermore, the most confirmed change mechanisms in literature for individual psychotherapy are insight, affective awareness, reflective functioning, externalizing difficulties, self-talk, coping, emotion regulation, the reappraisal of threat, fear extinction, change of interpersonal cognitions/avoidance, compensatory skills, mood regulation expectancies, diet habits, and self-efficacy. Conversely, emotional closeness, couple satisfaction, blamer softening, family functioning, and parental competence are confirmed mechanisms for couple/family psychotherapy. Finally, transversal mechanisms common to many approaches should not be considered equal to common factors, such as alliance, emotional regulation, and insight.

Abstract:The current study is an extension of a complementary investigation examining links between process and outcome within six-session psychotherapies in which three eminent psychotherapists each worked with a bereaved client, using person-centered, existential, and constructivist approaches, respectively. The Innovative Moments Coding System (IMCS) was used in both studies to examine processes of change in the moment-by-moment unfolding of grief therapy. The initial study's findings help inform theory regarding the relevance of humanistic interventions when working with bereaved clients, particularly those that help promote meaning making in the wake of loss. The current investigation extends the original by offering a fine-grained analysis of the progression of innovative moments (IMs) in the context of good-outcome grief therapy, liking specific therapy procedures with positive indicators of client functioning, while providing case illustrations of each therapy dyad. Findings highlight the promise of humanistic approaches in grief therapy, and suggest directions for further research in this area. Implications for future use of the IMCS in psychotherapy process and outcome research are also discussed.

Abstract:AIM: We understand ambivalence as a cycle of opposing expressions by two internal voices. The emergence of a suppressed voice produces an innovative moment (IM), challenging the dominant voice, which represents the client's problematic self-narrative. The emergence of the IM is opposed by the dominant voice, leading to a return to the problematic self-narrative. This study analyzed therapist and client responses to each other in episodes of ambivalence.METHOD: The therapeutic collaboration coding system (TCCS) assesses whether and how the therapeutic dyad is working within the therapeutic zone of proximal development (TZPD) by examining client responses to therapist interventions. We applied the TCCS to episodes in which a good- and a poor-outcome client in narrative therapy expressed ambivalence.RESULTS: In both the good- and poor-outcome cases, the therapist responded to the emergence of ambivalence similarly, balancing challenging and supporting. The good-outcome case responded at the developmental level proposed by the therapist when challenged, while the poor-outcome case lagged behind the level proposed.DISCUSSION: This supports the theoretical explanation that the therapist did not match client's developmental level in the poor-outcome case, working beyond the client's current TZPD and contributing to the maintenance of ambivalence.

The politicizing clinic: insights on 'the social' for mental health policy and practice

Abstract:Purpose: In this paper, we explore how Brazilian socially sensitive therapy can respond to care-users' desire to change the social and political forces shaping their lives. We use this case to demonstrate the limits of the "social determinants of health" agenda which, when operationalized, tends to leave questions of lasting structural change aside. Methods: We report on mixed methods ethnographic and epidemiological results from the 1982 Pelotas (Brazil) birth cohort study, a prospective study of 5914 children. Ethnographic analysis explored the cyclical relationship between schooling, mental health care, conceptualizations of mental distress, social and political engagement, and experiences with diverse forms of discrimination. Epidemiological bivariate and multivariate analyses examined differences in socio-political participation and the reporting of discrimination at different time-points for participants who used therapy with those who did not. Effect modification analysis tested the hypothesis that the socially empowering effects of therapy were greater for marginalized and minoritized youth. Results: Most young people living in situations of precarity experienced therapy, particularly when based in schools, to be a blame-inducing process. A more fulfilling and impactful therapeutic experience took shape when young people were able to shift the focus away from symptom reduction and behavioral management toward narrative life analyses, social debate, and political agency. Use of socially sensitive therapy was statistically associated with increased political participation and reporting of discrimination after controlling for confounders. The empowering effects of therapy were greater for those with less formal education and family income, but not for young people who identified as black, brown, or non-white. Conclusion: The findings underscore the importance of considering agency, sociality, and politics when theorizing "the social" in clinical practice, and health and social policy.
Eliciting reconceptualization innovative moments to foster change in Career Construction Counseling

Abstract:This study tests an add‐on procedure to foster change in Career Construction Counseling by eliciting reconceptualization innovative moments. After the career counseling intervention with 42 participants, 21 participants were allocated to the add‐on procedure, consisting of two additional sessions focused on eliciting reconceptualization innovative moments In turn, participants in the comparison group attended two extra intervention sessions in which the lifetime task was used. Outcome measures assessing vocational behavior and psychological distress were applied at the onset, at the end of counseling and after the two additional sessions. The findings show that Career Construction Counseling was effective, allowing a significant increase in vocational certainty, vocational identity, career decision‐making, and a decrease in psychological distress. After counseling, participants in the add‐on intervention fostering reconceptualization innovative moments revealed greater gains in vocational identity and psychological distress than those in the comparison group. The practical implications of these findings are discussed.

Single session therapy in pediatric healthcare: the value of adopting a strengths-based approach for families living with neurological disorders

Abstract:Background: Pediatric patients with neurological disorders often require lifelong management of symptoms and behaviours that can result in enduring emotional burden, stress and impacted health-related quality of life. Single session therapy (SST) draws upon patients' existing skills and knowledge and has emerged as a therapeutic approach to address pediatric patient and family needs in a timely manner. This study aimed to assess the clinical effectiveness of SST for pediatric patients with neurological disorders and their families, considering self-efficacy, distress, anxiety, therapeutic alliance and client satisfaction, as well as perceptions of whether SST met their pressing needs. Methods: A convergent parallel mixed-methods design included quantitative data collection via five standardized questionnaires across three time points and qualitative data collection through semi-structured interviews. Quantitative and qualitative data were analyzed independently and then integrated. Results: The study comprised of 135 participants, including patients, parents and siblings across diverse neurological conditions. Scores of self-efficacy and anxiety in children, and distress and anxiety in adults, improved significantly after the SST. Notably, changes in anxiety in adults remained significant five to seven weeks after the SST. Seventeen participants participated in 12 semi-structured interviews. Participants described that SST (1) was a missing piece in ongoing clinical care, (2) illuminated existing strengths and resilience, and (3) effected a lasting impact beyond the single session. Conclusions: SST may be a promising initial, strengths-based treatment to support the short-term and potentially long-term needs of pediatric patients with neurological disorders and their families by emphasizing existing strengths, supporting therapeutic alliance and cultivating hope.

Abstract:The conventional view on interventions as mechanistically causing interchangeable clients to get better has come under attack. Group-based and linear approaches fall short in adequately describing the idiosyncratic and dynamic nature of treatment processes. Non-linear dynamic system theories in contrast hold great potential to better conceptualize and understand the generalities and idiosyncrasies of psychotherapeutic change processes. The aim of this study was to examine whether we can detect markers of complex dynamical systems behavior in two single-case therapies. All sessions from both therapies were coded with sequential plan analysis using a 10s sampling frequency. The coding system incorporates verbal and non-verbal behaviors and allows for the representation of contextualized interactive behaviors. The high sampling frequency results in long time series, which allowed us to apply non-linear analysis techniques. We found strong support for complex behavior and the existence of a butterfly effect, i.e., a relatively short prediction horizon in which reliable predictions about the system's future behavior could be made. Further, critical fluctuations as a marker for phase-transitions were detected that were accompanied with different interactional patterns in both therapies. Finally, there was strong support for self-organized pattern formation, with a few interactional patterns dominating the interaction. Considering that we are intervening on complex dynamical systems means that we have to (1) acknowledge the principal individuality of change processes, (2) accept the fundamental limitations of the mechanistic input-output model of treatment effects and (3) appreciate the impossibility of long-term predictions of treatment responses.
The interplay between therapeutic relationship and therapeutic technique: "It takes two to tango"

Abstract:The idea that the therapeutic relationship is at the core of psychotherapy is shared by most therapeutic approaches. Also, an increasing variety of therapeutic approaches, consider experiential techniques as central tools to promote effective therapeutic change. Commonly, it is argued that the creation of a positive, empathic, safe, and solid therapeutic alliance should be a prerequisite for administering these techniques. However, recent evidence is beginning to challenge this cautious stance showing that the introduction of experiential work early in therapy can be safe and effective. So, the quality of the therapeutic relationship may either facilitate or hinder the use of experiential techniques, but the use of these techniques may also strengthen or cause ruptures in the therapeutic alliance. So how do we balance the attention to the therapeutic relationship and the use of active, emotionally arousing techniques? In this paper, we present an issue of the Journal of Clinical Psychology: In Session exploring the complex interplay between therapeutic relationship and experiential practices in light of different psychotherapeutic approaches. We do so by first providing an initial overview of the topic. Next, we introduce the five papers of the issue along with some thoughts on how authors from diverse psychotherapeutic orientations contributed to illustrate and reflect on this multifaceted "dance" between therapeutic relationship and techniques.
“TAKE THE MUSIC WHERE YOU ARE READY TO GO”: SINGLE SESSION ADAPTED FOCUSED MUSIC IMAGERY IN ADULT ACUTE PSYCHIATRY

Abstract:Due to COVID-19, therapeutic services in some psychiatric hospitals had to be changed significantly. This paper details how music therapy services were modified from the usual 50- to 60-minute group session, to brief individual sessions lasting 7–8 minutes. Because the sessions could not include anything that would require disinfection (instruments, song books, art supplies, etc.), tailored versions of Focused Music Imagery and Vocal Psychotherapy were the main interventions used. This paper will describe how these approaches became combined and will be illustrated through seven clinical vignettes. A call for continued exploration and research is included within the historical context of Helen Bonny’s initial calling to exploring the therapeutic and transformative applications of music.
Having Impact and Doing It Quickly: The Place for Brief and Single-Session Cognitive-Behavioral Therapies in Sport Psychology Practice

Abstract:Cognitive-behavioral therapies and related approaches are highly prevalent within sport psychology practice. Traditionally, these approaches are delivered across interventions comprising multiple sessions. However, in the fast-paced environments in which many applied sport psychologists operate, practitioners are sometimes required to provide fast, effective, and impactful interventions to athletes at their point of need within a single session. Single-session integrated cognitive-behavioral therapy presents a potentially effective approach for practice wherein time is often at a premium, and there is frequently pressure to make an impact quickly to improve performance. In this article, we put forward a stimulus piece that contextualizes single-session integrated cognitive-behavioral therapy and overviews how sport psychology practitioners may use such techniques with athletes. We also put forth a call for more practitioners to report more idiographic case studies that feature the use of brief or single-session interventions to further build the evidence base for such approaches.

Introducing mindfulness and compassion-based interventions to improve verbal creativity in students of clinical and health psychology

Abstract:Objectives: In the field of psychotherapy, verbal creativity has been suggested as an important aspect in psychotherapists' training. In the present study, the effects of a mindfulness and compassion-based intervention (MCBI) on verbal creativity are analysed in students of clinical and health psychology (N = 90). Design: Students were randomly assigned to an experimental group (n = 37), in which an MCBI was applied, and a waiting list group (n = 26) with no intervention. We also assessed a non-randomized active control group (n = 27), in which students received training in basic psychotherapy skills. Methods: Verbal creativity (fluency, flexibility, and originality) was evaluated in a pre-, post-, and follow-up assessment. Results: Results indicated a significant increase in fluency (p = .001, d = .64), flexibility (p = .017, d = .67), and originality (p = .004, d = .72) in the experimental group, relative to the waiting list group, in the post-assessment. Fluency (p = .010, d = .64) and flexibility (p = .033, d = .62) were also found to be higher in the follow-up assessment. In addition, results indicated a significant increase in flexibility (p = .034, d = .74) in the experimental group, relative to the active control group, in the follow-up assessment. Conclusions: Introducing MCBI in the university education of psychotherapists seems to be a useful strategy to improve their verbal creativity, which could positively influence their ability to explore and appropriately respond to their patients' needs. Practitioner points: Mindfulness and compassion-based interventions (MCBIs) could be a useful strategy to improve verbal creativity in the university education of psychotherapists. After the MCBI, students of clinical and health psychology increased the number of ideas they produced when facing a specific situation, as well as their variety and originality.

Abstract:Objective: This observational study aims to analyze the reliability of the innovative moments coding system for groups (IMCS-G), developed to respond to the need for reliable measures to detect markers of change in group interventions. The IMCS-G represents an adaptation of the innovative moments coding system, which is used in individual psychotherapy to assess innovative moments (IMs), that is, change in meanings that contrast with the clients’ problematic patterns. The second aim is to examine the evolution of IMs throughout group counseling sessions in members who improved versus those who did not improve. Method: The IMCS-G consists of seven IM categories, grouped into two macrocategories: individual and group IMs. The IMCS-G was applied to the transcripts of 36 sessions from four counseling groups for underachieving university students (N = 25). Two trained raters coded the transcripts according to IMCS-G procedures. The agreement between the raters and the reliability of the IM categories were calculated. The efficacy of the counseling groups was measured in terms of the improvement in the students’ academic performance, reflective functioning, and psychological well-being. Multilevel models were performed to study the associations between the emergence of IMs throughout the sessions and the improvement in outcomes. Results: Results showed that IMCS-G was highly reliable (agreement = 84.10%–96.85%; Cohen’s kappa = .85–.99) and that the students who improved in academic performance produced more IMs (β = .42, t = 1.99, p = .048). Conclusions: Findings suggest that the IMCS-G is a reliable system able to detect markers of change associated with academic outcomes.

Abstract:This paper explores transformative strategies in hand therapy aimed at improving the patient experience - a critical factor linked to improved clinical outcomes and increased satisfaction for patients and therapists. The focus is on three key areas: personalized interactions, empathic communication, and sensory engagement. Personalized interactions ensure therapy is uniquely tailored to each patient's needs, fostering a sense of individual attention and care. Empathic communication involves understanding and responding effectively to patients' verbal and nonverbal cues, creating a supportive environment where patients feel genuinely heard. Sensory engagement utilizes the therapeutic environment's visual, olfactory, and auditory elements to create a calming, welcoming space conducive to healing and relaxation.
