Abstract:Background: A novel protocol for accurate stellate ganglion block under ultrasound guidance was designed in rats. This technique raises the success rate of stellate ganglion block and reduces the incidence of brachial plexus and vagus nerve block. Methods: Fifty-six Sprague-Dawley were randomly divided into an ultrasound-guided group ( n = 28) and a blind technique group ( n = 28). The rats in the blind technique group were injected with 1.5% lidocaine mixed with methylene blue after signs of brachial plexus stimulation were elicited. The lateral side of the cephalic brachial vein was located under the first rib, where lidocaine was injected into the rats in the ultrasound-guided group. The up-and-down sequential method of Dixon was used to determine the minimum effective volume for stellate ganglion block in rats. Furthermore, we calculated the required operative duration of the two methods and observed the difference in the lidocaine diffusion range between the two groups. Results: The minimum effective volume for stellate ganglion block in the ultrasound-guided group was 0.040 ml, and the 95% CI was 0.026–0.052 ml. In the blind technique group, the minimum effective volume was 0.639 ml, and the 95% CI was 0.490–0.733 ml. Within the 95% CI of the lowest effective volume, the incidence of brachial plexus block as a complication of stellate ganglion block under ultrasound guidance was 10.00%. Conclusion: Stellate ganglion block under ultrasound guidance is more accurate than blind detection, which the incidence of complications of stellate ganglion block under ultrasound guidance was significantly lower than under blind detection; the rate of methylene blue staining in the vagus nerve was significantly lower under ultrasound guidance.

Abstract:The high quality embryos of 129S1/SvImJ strain mouse were chosen after superovulation, and directly seeded in the irradiated frozen-thaw mouse embryonic fibroblast cells. Compared with the low concentration long time digestion, the high concentration short time digestion was adopted in the study, and 4 lines of mouse embryonic stem cells were successfully established by trypsinization of 0.25%trypsin-0.04%EDTA, which is the high concentration short time digestion method. The ratio of establishment cell lines is 36.3%, and the cell strains exhibited the typical ES cell characteristic identified by different ways including the AKP and karyotype assay, the differentiation potential in vivo and in vitro, and the examination of specific surface markers expression of Oct-4 and Nanog. The ES cell lines can be made scalable culture and used as an ideal experimental material for the further research.

Abstract:To explore the feasibility of stellate ganglion block (SGB) for the treatment of brain injury and the inhibitory effect of neuronal apoptosis. SD rats were randomly divided into control group, model group (LPS 0.6 mg/kg), and SGB group (LPS 0.6 mg/kg + lidocaine 0.2 mL). The rats were sacrificed after SGB block for 2 h to separate the hippocampus. The pathological changes of hippocampal tissue were observed by HE staining, the apoptosis of hippocampal neurons was observed by TUNEL staining, the positive expression of B-cell lymphoma-2-Associated X (Bax) in hippocampal tissue was detected by immunohistochemistry, and the expression of Bax mRNA in hippocampal tissue was detected by qRT-PCR method. Results: Compared with the control group, the hippocampal tissue of the model group was severely damaged, and the apoptosis rate of hippocampal neurons, the positive rate of Bax protein, the expression of Bax mRNA, and the protein expressions of Baxwere significantly increased, the protein expression of Bcl-2 was significantly decreased ( P < 0.05).Compared with the model group, the hippocampal tissue damage of the SGB group was significantly reduced, and the hippocampal neuron apoptosis rate, Bax protein positive rate, Bax mRNA expression and Bax protein expressions were significantly decreased, and the expression of Bcl-2 protein was significantly increased ( P < 0.05). Conclusion: SGB can reduce the apoptosis of neurons in rats and improve brain injury by inhibiting the expression of Bax gene.

Abstract:Objective This study aimed to investigate the effects of stellate ganglion block (SGB) through different approaches under guidance of ultrasound. Methods A total of 130 patients undergoing SGB in our hospital between February 2019 and February 2020 were enrolled as the research subjects. According to the random number table method, these subjects were divided into two groups: a modified 6th cervical vertebra (C6) group ( n = 65) and a 7th cervical vertebra (C7) group ( n = 65). Under the guidance of ultrasound, the subjects in the modified C6 group were punctured at the level of the C6 transverse process, and the subjects in the C7 group were punctured at the level of the C7 transverse process. The operation duration, number of puncture angle adjustments, block effects, and adverse reactions for SGB were compared between the two groups. Results The modified C6 group showed shorter SGB operation duration and a lower number of puncture angle adjustments than the C7 group, and the differences were statistically significant ( P < 0.05). Horner Syndrome occurred in both groups after SGB. The incidence of adverse reactions in the modified C6 group was 4.62%, comprising 1 case of hoarseness and 2 cases of slowed pulse, while that in the C7 group was 6.15%, with 1 case of hoarseness and 3 cases of slowed pulse; the difference between the two groups was not statistically significant ( P > 0.05). Conclusion The operation duration for modified SGB guided by ultrasound puncturing at the C6 transverse process is shorter and requires fewer puncture angle adjustments than puncturing at the C7 transverse process; however, there is no significant difference between the incidence of adverse reactions or the blocking effects of the two methods.

Abstract:Stellate ganglion block (SGB) has been widely used in clinical treatment for various pain and non-pain, including the pain of head and face, ear, nose and throat, oral, neck and shoulder, and upper limb. Additionally, SGB has shown a curative effect on refractory angina pectoris, arrhythmia, and depression. Previously, the SGB procedure had been primarily dependent on the anatomical landmark. However, with the popularization and adopted application of ultrasound in pain, the ultrasound-guided SGB has gradually become the mainstream. This consensus summary reflects the clinical experience of several experts. The topics range from introducing the stellate ganglion anatomy to the indications and contraindications of SGB, blocking methods, drug selection, treatment course, efficacy evaluation, complications and precautions. Overall, the consensus hopes to provide clinical guidance for SGB.
Development and Validation of a New Technique for Ultrasound-Guided Stellate Ganglion Block

Abstract:BACKGROUND AND OBJECTIVES: Although the stellate ganglion is located anteriorly to the first rib, anesthetic block is routinely performed at the C6 level. Ultrasonography allegedly improves accuracy of needle placement and spread of injectate. The technique is relatively new, and the optimal approach has not been determined. Moreover, the location of the cervical sympathetic trunk relative to the prevertebral fascia is debatable.METHODS: Three-dimensional sonography was performed on 10 healthy volunteers, and image reconstruction was completed. On the basis of analysis of pertinent anatomy, a lateral trajectory for needle placement was simulated. Accuracy was tested by injection of methylene blue in cadavers. A clinical validation study was then conducted. A block needle was inserted according to the predetermined lateral path, and 5 mL of a mixture of bupivacaine and iohexol was injected. Spread of the contrast agent was verified fluoroscopically.RESULTS: Image reconstruction revealed that the cervical sympathetic trunk is located posterolaterally to the prevertebral fascia on the surface of the longus colli muscle. The mean anteroposterior width of the muscle at the C6 level was 11 mm. The lateral approach does not interfere with any visceral or nerve structures. Anatomic dissection in cadavers confirmed entirely subfascial spread of the dye and staining of the sympathetic trunk. The contrast agent spread was seen in all patients between the C4 and T1 levels in a typical prevertebral pattern.CONCLUSIONS: This study revealed that, at the C6 level, the cervical sympathetic trunk lies entirely subfascially. Subfascial injection via the lateral approach ensures reliable spread of a solution to the stellate ganglion.

Establishment of Heterotopic Small Bowel Transplantation Model in Mice

Abstract:Objective To investigate the surgical procedures for establishing a reliable murine model of heterotopic small bowel transplantation(SBTx),with the purpose of providing a good experimental tool for the rejective reaction study in SBTx.Methods C57BL/6 mice were used as donor and the BALB/c mice used as recipient animals.The procedure of segmental heterotopic SBTx in allogenic mice was performed as following: the donor′s portal vein was anastomosed end-to-side to the recipient′s inferior vena cave(IVC),and then the donor′s superior mesenteric artery(SMA) with an aortic patch was anastomosed end-to-side to the recipient abdominal aorta.Subsequently,proximal end of the graft intestine was ligated and the distant end was anastomosed end-to-side to the recipient′s jejunum.The mice were fasted for 3 days,but free access to water after the operation.In addition,the mice were given 5% glucose-saline solution(2 mL) twice daily by subcutaneous injection,but without use of antibiotic and immunosuppressive therapy.The models survived beyond 5 days were considered to be successful.Results Segmental SBTx were perfomed in 30 mice,with a postoperative day(POD) 5 survival rate of 70 %(21/30).The operating time in donor was(41±5.5) min,the graft warm ischemic time was about 0.5 min,the time for trimming the aortic patch in SMA was 3 min and cold preservation of donor′s small bowel was(30±7.5) min.The time of recipient preparation and transplantation was(90±7.5) min,in which the abdominal aorta and IVC blockage time was(40±3.0) min,the average time for the vein anastomosis and the arterial anastomosis was(10±2.0) min and(15±2.5) min,respectively.The average bleeding volume was 0.2 mL.The operation failed in 9 cases,due to arterial anastomotic thrombosis and anastomosis stenosis(6 cases),massive hemorrhage(2 cases) and intraperitoneal infection(1 case).Conclusions The high quality of the donor′s small bowel segment harvesting,excellent vascular and intestinal anastomosis as well as adequate supplement of liquid to both donor and recipient are all crucial factors to ensure the efficiency and success of SBTx.

Abstract:BACKGROUND: A stellate ganglion block (SGB) causes increased blood flow in the maxillofacial region, exhibiting the potential for regenerative effects in damaged tissue. The focus of this study was to understand the efficacy of SGB for regenerative effects against nerve damage. A rat model of the superior cervical ganglion block (SCGB) was created instead of SGB, and facial blood flow, as well as sympathetic nervous system function, were measured.METHODS: A vertical incision was made on the left side of the neck of a Wistar rat, and a 5-mm resection of the superior cervical ganglion was performed at the back of the bifurcation of the internal and external branches of the left common carotid artery. Blood flow in the skin at the mandibular angle and mean facial temperature were measured using a laser-Doppler blood flow meter and a thermographic camera, respectively, over a 5-week period after the block. In addition, the degree of ptosis and miosis were assessed over a period of 6 months.RESULTS: The SCGB rat showed significantly higher blood flow at the mandibular angle on the block side (P < 0.05) for 3 weeks, and significantly higher skin temperature (P < 0.05) for 1 week after the block. In the SCGB rat, ptosis and miosis occurred immediately after the block, and persisted even 6 months later.CONCLUSIONS: SCGB in rats can cause an increase in the blood flow that persists over 3 weeks.

Abstract:Cervical sympathetic and stellate ganglion blocks (SGB) provide a valuable diagnostic and therapeutic benefit to sympathetically maintained pain syndromes in the head, neck, and upper extremity. With the ongoing efforts to improve the safety of the procedure, the techniques for SGB have evolved over time, from the use of the standard blind technique, to fluoroscopy, and recently to the ultrasound (US)-guided approach. Over the past few years, there has been a growing interest in the ultrasound-guided technique and the many advantages that it might offer. Fluoroscopy is a reliable method for identifying bony surfaces, which facilitates identifying the C6 and C7 transverse processes. However, this is only a surrogate marker for the cervical sympathetic trunk. The ideal placement of the needle tip should be anterolateral to the longus colli muscle, deep to the prevertebral fascia (to avoid spread along the carotid sheath) but superficial to the fascia investing the longus colli muscle (to avoid injecting into the muscle substance). Identifying the correct fascial plane can be achieved with ultrasound guidance, thus facilitating the caudal spread of the injectate to reach the stellate ganglion at C7-T1 level, even if the needle is placed at C6 level. This allows for a more effective and precise sympathetic block with the use of a small injectate volume. Ultrasound-guided SGB may also improve the safety of the procedure by direct visualization of vascular structures (inferior thyroidal, cervical, vertebral, and carotid arteries) and soft tissue structures (thyroid, esophagus, and nerve roots). Accordingly, the risk of vascular and soft tissue injury may be minimized.

Effect of Stellate Ganglion Block on Brain-Derived Neurotrophic Factor and Serum Interleukin-8 of Dorsal Root Ganglion

Abstract:Objective To investigate the effect of stellate ganglion block ~SGB) on formalin-induced nociceptive behavior of rabbits, the expression of brain-derived neurotrophic factor ~BDNF) of dorsal root ganglia ~DRG) and the concentration of serum interleukin-8 ~IL-8) in rabbits. Methods Twenty-four rabbits were used in this study. Catheters were inserted closely to right stellate ganglia of the animals, which were randomly divided into control group ~A), SGB group ~B) and pain model group ~C). Group B and C were administered with an injection of 0.5ml of 3% formalin into the right front paw subcutaneously, group A 0.5ml saline by the same way. 0.5ml of (0.25)% bupivacaine was administered via the catheter in group B at 10 minutes before injection, while group A and C were given 0.5ml of saline at the same time. Nociceptive behavior was observed for 60 minutes after injection, and assessed by weighted pain score. Serum IL-8 was measured radioimmunologically at 10 minutes ~T_0) before injection, 10 ~T_1), 60 ~T_2) and 120 minutes ~T_3) after injection. The expression of BDNF in the right DRG of cervical 7, 8 and thoracic 1 was investigated immunohistochemically at 120 minutes after injection. Results Formalin-induced nociceptive behavior in phase Ⅱ was relieved following SGB. The concentration of serum IL-8 in groups B and C at T_2 and T_3 was significantly increased than that at T_0 and T_1~P(0.01)), and significantly lower than that in group C at the same time ~P0.01). The expression of BDNF in DRG in group B was significantly lower than that in group C, but higher than that in group A ~P0.01). Conclusion SGB can inhibit the BDNF expression of DRG and the upregulation of serum IL-8 in rabbits with formalin-induced nociception. It might be related to the mechanisms of SGB for the treatment of inflammatory pain.

Abstract:Objective To investigate the effects of stellate ganglion block (SGB) on myocardial electro physiology in the infarct border zone. Methods Sixty-four New Zealand white rabbits were randomly assigned into two groups: MI group (n=32), ligation of the anterior descending coronary; SO, sham operation group (n=32), without coronary ligation. Both of MI group and SO group were divided into four subgroups according to the left or right SGB and the corresponding control (re = 8, each). For SGB, 0. 5 ml of 0. 25 % bupi vacaine was used. After eight weeks of surgery,90% of monophasic action potential duration (MAPD_(90)),effective re fractory period (ERP),transmural dispersion of repolarization (TDR).ventricular fibrillation threshold (VFT) were measured at the infarct border zone (MI group) and corresponding zone (SO group) following SGB. Result Compared with the corresponding control group,left SGB (LSGB) prolonged the MAPD_(90), reduced the TDR,and increased the ERP and VFT in both the MI group and SO group (P <0. 05). However,Right SGB (RSGB) shortened the MAPD^,increased the TDR,and reduced ERP and VFT in both the MI group and SO group (P<0.05). Conclusion LSGB can increase the electrophysiological stability of ventricular myocardium.

Abstract:Stellate ganglion block (SGB) is highly beneficial when used for treating pain syndromes, including acute herpes zoster of the face, postherpetic neuralgia, and sympathetically mediated pain of the head. Horner's syndrome is not reliable as a traditional indicator of successful SGB because of ambiguous and subjective judgment.<a class="ejp-citation-link js-ejp-citation-link">[1]</a> Cutaneous temperature change was reported to be a simple and objective method for assessing successful SGB.<a class="ejp-citation-link js-ejp-citation-link">[2]</a> However, temperature increase was only observed at least 10 min after successful SGB, demonstrating that the assessment of skin temperature cannot predict the efficacy of SGB in the early stage.<a class="ejp-citation-link js-ejp-citation-link">[3]</a> Laser speckle contrast imaging (LSCI) provides a powerful tool for quantitative assessment of flow dynamics for microcirculation over large areas with high spatiotemporal resolution.<a class="ejp-citation-link js-ejp-citation-link">[4,5]</a> According to our knowledge, LSCI has not been studied as an indicator of successful SGB.

A Rat Model of Middle Cervical Sympathetic Ganglion Block

Abstract:Objective To establish an effective rat model of middle cervical sympathetic ganglion block.Methods A total of 18 male SD rats were used in this study,6 rats were randomly selected for the anatomy of middle sympathetic ganglia;another 6 were used for simulated blockade of the middle cervical sympathetic ganglion by dorsally injecting 0.4 mL ink via the interval of the first and second costotransverse joints,where the spinal process of the second thoracic vertebra was employed as a marker to show the range of infiltration;and the remaining 6 were used for blocking middle cervical sympathetic ganglion with 0.4 mL of 0.25% bupicaine.Results There was a middle sympathetic ganglion with a length of 3.5-5 mm on both left and right sides,lying in front of the first and second costotransverse joints and deeply below subclavicular artery and vein.The middle cervical ganglia with simulated blockade were completely infiltrated by ink.Horner sign appeared on the blocked sides within 1-2 min in all rats injected with bupicaine,which lasted for 2-2.5 h.Conclusions The method of middle cervical ganglion blockage we developed is effective,convenient,accurate and safe,which takes advantage of the spinous process of the second thoracic vertebra as a bony marker and administers bupicaine directly into the middle cervical ganlion via the interval of the first and second costotransverse joints.

Abstract:BACKGROUND: Ultrasound imaging is an ideal tool for stellate ganglion block (SGB) due to clarity, portability, lack of radiation, and low cost. Ultrasound guided anterior approach requires the application of pressure to the anterior neck and is associated with more risk of injury to inferior thyroid artery, vertebral artery, and esophagus. The lateral approach does not interfere with nerve or vascular structures. Blockade at the C6 vertebral level results in more successful sympathetic blockade of the head and neck with less sympathetic blockade of the upper extremity compared to sympathetic blockade at C7 vertebral level, which produces successful sympathetic blockade of upper extremity. This is helpful in patients of complex regional pain syndrome of the upper limb. Hence, we conducted a study using the lateral approach at C7 level.MATERIALS AND METHODS: Ultrasound guided SGBs using lateral in-plane technique at C7 level were given in 20 patients suffering from chronic pain patients of upper extremity, head, and neck using 4 ml of 0.25% bupivacaine and 1 ml of 40 mg triamcinolone. The patients were assessed for a numeric pain intensity score (NPIS), the rise in axillary temperature, the range of motion of joints of upper extremity, and resolution of edema at various time intervals up to 3 months.RESULTS: NPIS showed a statistically significant decrease from baseline at 30 min, which was sustained till 3(rd) month. The rise in axillary temperature after the block was statistically significant, which was sustained till 2(nd) week. The edema score decreased significantly at all-time intervals (P ≤ 0.001). The restriction of motion in all joints of upper limb decreased from 13 to 3 patients.CONCLUSION: There is a significant variation in the anatomy of stellate ganglion at the level of C6 and C7. Ultrasound guided lateral approach increases the efficacy of SGB by deposition of drug subfascially with real-time imaging.
Stellate ganglion block beyond chronic pain: A literature review on its application in painful and non-painful conditions

Abstract:Cervical sympathetic or stellate ganglion blocks (SGBs) have been commonly used in the treatment of painful conditions like complex regional pain syndrome (CRPS). However, there is literature to suggest its utility in managing non-painful conditions as well. The focus of this literature review is to provide an overview of indications for SGB for painful and non-painful conditions. We identified published journal articles in the past 25 years from Embase and PubMed databases with the keywords "cervical sympathetic block, stellate ganglion blocks, cervical sympathetic chain, and cervical sympathetic trunk". A total of 1556 articles were obtained from a literature search among which 311 articles were reviewed. Among painful conditions, there is a lack of evidence in favor of or against the use of SGB for CRPS despite its common use. SGB can provide postoperative analgesia in selective surgeries and can be effective in temporary pain control of refractory angina and the acute phase of herpes zoster infection. Among non-painful conditions, SGB may have beneficial effects on the management of post-traumatic stress disorder (PTSD), refractory ventricular arrhythmias, hot flashes in postmenopausal women, and breast cancer-related lymphedema. Additionally, there have been various case reports illustrating the benefits of SGB in the management of cerebral vasospasm, upper limb erythromelalgia, thalamic and central post-stroke pain, palmar hyperhidrosis, orofacial pain, etc. In our review of literature, we found that SGB can be useful in the management of various non-painful conditions beyond the well-known treatment for CRPS, although further studies are required to prove its efficacy.
Ultrasound-guided Stellate Ganglion Block Improves Gastrointestinal Function After Thoracolumbar Spinal Surgery

Abstract:PURPOSE: The purpose of this study was to investigate the effects of stellate ganglion block (SGB) on gastrointestinal function after thoracolumbar spinal surgery.METHODS: Forty patients with thoracolumbar fracture scheduled for posterior spinal surgery were randomly assigned to 2 groups: ultrasound-guided SGB or sham (control group). After induction, ultrasound-guided SGB (n = 20) or a sham procedure (n = 20) was conducted with a 6-mL injection of 1% lidocaine or 0.9% saline, respectively. Primary outcomes were postoperative gastrointestinal function, including auscultation of bowel sounds, incidence and degree of abdominal bloating, flatus time, and paralytic ileus.FINDINGS: Patients in the SGB group had better gastrointestinal function compared with those in the sham (control) group, as indicated by more regular bowel sounds at 24 and 36 hours postoperatively (80% vs 40%, P = 0.024, and 95% vs 60%, P = 0.023), lesser abdominal bloating (10% vs 45%, P = 0.034), and slightly shorter flatus time (mean [SD], 12.0 [4.4] vs 14.7 [4.6] hours, P = 0.068). Survival analysis based on regular bowel sounds found a significant difference between the 2 groups (P = 0.004). In addition, more patients had higher satisfaction after SGB (85% vs 45%, P = 0.020). No postoperative ileus or mortality occurred.IMPLICATIONS: In this preliminary study, ultrasound-guided SGB accelerated the return of gastrointestinal transit, alleviated bowel symptoms, and improved patient satisfaction after thoracolumbar spinal surgery. chictr.org.cn identifier: ChiCTR-TRC-14004289.

Abstract:Background: Insomnia has become increasingly prevalent in modern society and is notoriously difficult to treat. Many patients exhibit a poor response to pharmacological interventions. Stellate ganglion block (SGB) has emerged as an effective method for managing insomnia; however, its efficacy may be compromised in some patients, primarily due to a variant vertebral artery anatomy. Case presentation: This case report describes a patient with severe insomnia accompanied by anxiety. Through cervical ultrasound scanning, we identified richly branched cervical arteries at the C6−C7 segment of the vertebral artery, along with anatomical variations, which could pose a heightened risk for the traditional SGB procedure. Therefore, after carefully adjusting the patient's positioning, we proceeded with ultrasound-guided SGB using a lateral paravein out-of-plane approach. Clinical signs of successful insomnia symptoms alleviation were consistently observed after each block utilizing this alternative technique multiple times in a single patient. Conclusion: Our report reveals a new lateral paravein out-of-plane approach for ultrasound-guided SGB to treat insomnia, which might be considered an alternative method. More studies should be carried out to confirm the efficacy of this new approach.

Abstract:Ischemic stroke is a disabling and fatal disease caused by the insufficient blood supply to the brain. Stellate ganglion block (SGB) is a type of anesthesia commonly used to relieve pain. Here, we sought to identify the effects of SGB on cerebral ischemia-reperfusion (I/R) injury. The middle cerebral artery occlusion (MCAO) model was established in rats. The brain injury was assessed using the 2,3,5-triphenyl-tetrazolium-chloride (TTC) staining assay and neurological score. Ferroptosis was analyzed by detecting cell death, Fe 2+ content, glutathione (GSH), malonic dialdehyde (MDA), superoxide dismutase (SOD), and ferroptosis-related factors. The mechanisms of SGB were assessed using the western blot. The results showed that I/R increased brain infarction and damaged neurological function. SGB decreased I/R-induced infarction and improved neurological function. Meantime, SGB inhibited ferroptosis of the hippocampus induced by I/R via the Hippo pathway. and the Yes1 associated transcriptional regulator (YAP) of this pathway was positively correlated with the ferroptosis-related solute carrier family 7 member 11 (SLC7A11). Inhibition of the Hippo pathway reversed the effects of SGB on brain injury and ferroptosis. In conclusion, SGB inhibited ferroptosis of hippocampal neurons via activating the Hippo pathway and thereby alleviated I/R injury. The data provide a novel insight into the treatment of ischemic stroke and even other ischemic encephalopathies.

Abstract:Diabetes mellitus is an independent risk factor for ischemic stroke. Both diabetes mellitus and stroke are linked to systemic inflammation that aggravates patient outcomes. Stellate ganglion block can effectively regulate the inflammatory response. Therefore, it is hypothesized that stellate ganglion block could be a potential therapy for ischemic stroke in diabetic subjects. In this study, we induced diabetes mellitus in rats by feeding them a high-fat diet for 4 successive weeks. The left middle cerebral artery was occluded to establish models of ischemic stroke in diabetic rats. Subsequently, we performed left stellate ganglion block with 1% lidocaine using the percutaneous posterior approach 15 minutes before reperfusion and again 20 and 44 hours after reperfusion. Our results showed that stellate ganglion block did not decrease the blood glucose level in diabetic rats with diabetes mellitus but did reduce the cerebral infarct volume and the cerebral water content. It also improved the recovery of neurological function, increased 28-day survival rate, inhibited Toll like receptor 4/nuclear factor kappa B signaling pathway and reduced inflammatory response in the plasma of rats. However, injection of Toll like receptor 4 agonist lipopolysaccharide 5 minutes before stellate ganglion block inhibited the effect of stellate ganglion block, whereas injection of Toll like receptor 4 inhibitor TAK242 had no such effect. We also found that stellate ganglion block performed at night had no positive effect on diabetic ischemic stroke. These findings suggest that stellate ganglion block is a potential therapy for diabetic ischemic stroke and that it may be mediated through the Toll like receptor 4/nuclear factor kappa B signaling pathway. We also found that the therapeutic effect of stellate ganglion block is affected by circadian rhythm.
