Abstract:Précis: Gonioscopy-assisted Transluminal Trabeculotomy may be an effective first line surgery for decreasing intraocular pressure and medication burden in patients with uveitis-related ocular hypertension or glaucoma. Purpose: The purpose of the study is to determine the efficacy of Gonioscopy-assisted transluminal trabeculotomy (GATT) to lower the intraocular pressure (IOP) in uveitis-related ocular hypertension (OHT) or glaucoma. Methods: Retrospective case series that included patients with uveitis-related OHT or glaucoma that underwent GATT with or without concomitant cataract extraction and intraocular lens implantation at two Canadian academic centres from July 2018 to May 2022. Primary outcomes were : complete (no medications) and qualified success (with medication), and failure defined as (1) IOP > 21mmHg with maximal medical therapy, (2) the need of additional glaucoma procedure, (3) loss of light perception secondary to glaucoma, (4) IOP < 6 mmHg for 3 months. Results: Twenty-one eyes from 18 patients were included with a mean preoperative IOP of 26.2±7.3 mmHg on 4.3±0.7 classes of glaucoma drops. Average follow-up was 29.2±17.6 months and 76% of eyes (n=16) had reached at least 12 months of follow-up. At the 12 month follow-up visit, there was a significant decrease in average IOP by 9.9±7.9 mmHg (38%, P =0.005) and a decrease of 1.9 in glaucoma medication classes ( P =0.002). Fourteen percent of eyes achieved complete success while 80% of eyes achieved qualified success. Six eyes failed (29%) and five patients (24%) required an additional glaucoma surgery. The most common postoperative complication was hyphema (n=9; 43%). Conclusion: This small case series suggests that GATT may be an effective first line surgery for decreasing IOP and medication burden in patients with uveitis-related OHT or glaucoma. Further studies with longer follow-up should be conducted to assess its long-term outcomes.

Abstract:Prcis: This study concludes that GATT can be an effective and safe surgical alternative for managing IOP in eyes with prior failed glaucoma surgeries, associated with minimal complications. Purpose: To report outcomes of gonioscopy-assisted transluminal trabeculotomy (GATT) in eyes with prior failed glaucoma surgery. Patients and methods: A retrospective study involving 30 eyes of 30 patients, all of whom had open angles on gonioscopy, experienced prior glaucoma surgery failures, and subsequently underwent GATT. The primary outcome measure was success defined as complete when the intraocular pressure (IOP) was >5 and ≤21/16 mm Hg without glaucoma medications and qualified with medications. Results: The mean age was 51.8±16.1 years. Twenty-one eyes underwent GATT and 9 eyes underwent phaco-GATT. Twenty-seven eyes had failed trabeculectomy and 3 eyes had failed glaucoma drainage device. Post-GATT, the IOP decreased from 27.1±7 to 16.9±6 mm Hg (P<0.001) at the end of 15 months, with a mean drop in AGM from 4.9±1.0 to 2±1.6. At postoperative 1 year, the probability of complete success was 20% (95% CI: 9-43) for an IOP criterion of both 21 and 16 mm Hg. The qualified success probability at 1 year was 82% (67-100) for an IOP criterion of 21 mm Hg and 57% (38-84) for an IOP criterion of 16 mm Hg. Risk factor for failure was older age [hazard ratio (HR): 1.03, 95% CI: 1.01-1.06]. The complications noted were hyphema in 14 eyes (46%), majority resolved within 1 week and all by 2 weeks. None needed any intervention. Conclusions: This study concludes that GATT can be an effective and safe surgical alternative for managing IOP in eyes with prior failed glaucoma surgeries, associated with minimal complications.

Abstract:Glaucoma is a disease that leads to optic nerve damage and irreversible loss of the visual field. Gonioscopy-assisted transluminal trabeculotomy (GATT) is a minimally invasive glaucoma surgery technique, where the surgeon opens the trabecular network and Schlemm's canal to improve the outflow of the aqueous humor. This method has shown efficacy in reducing intraocular pressure in patients with primary and secondary glaucoma. GATT has gained popularity due to its minimally invasive nature, compatibility with cataract surgery and relatively low cost. Despite its advantages, the procedure can be associated with many complications such as hyphema, intraocular pressure spikes and corneal edema. In rare cases, it can lead to Descemet's membrane detachment or cystoid macular edema. The success of GATT procedure mostly depends on the surgeon's skill and experience, but also on the patient's risk factors, including age and preoperative intraocular pressure levels. Further studies are required to understand long outcomes and identify the patients, who may be at higher risk of complications.

Abstract:European Journal of Ophthalmology, Ahead of Print. BackgroundFuchs uveitis syndrome (FUS) is a chronic intraocular inflammatory disease. It can lead to glaucoma which can be intractable to both medical and surgical treatment. In the present report, we describe our experience with GATT as a primary surgical option in the treatment of glaucoma secondary to FUS.ResultsFour consecutive patients with FUS-associated glaucoma underwent GATT in a single institution. Case 1 was a 28-year-old pseudophakic male with a preoperative intraocular pressure (IOP) of 43 mmHg had unsuccessful circumferential cannulation; his IOP remained high after GATT and received trabeculectomy followed by Ahmed glaucoma valve (AGV) implantation to control IOP. Although IOP was stable following AGV, he required descemet membrane endothelial keratoplasty due to persistent corneal decompansation. He had a stable IOP of 12 mmHg over 21 months. Case 2 was a 42-year-old male with preoperative IOP of 35 mmHg and posterior subcapsular cataract had 360-degree GATT combined with phacoemulsification; his IOP reduced to 14 mmHg over 18 months. Case 3 was an 52-year-old pseudophakic male with a preoperative IOP of 28 mmHg had 360-degree GATT; his IOP remained stable at 10 mmHg over 18 months. Case 4 was an 47-year-old pseudophakic female with a preoperative IOP of 39 mmHg had 360-degree GATT; her IOP remained stable at 14 mmHg over 12 months.ConclusionGATT may be used as an initial approach in secondary glaucoma due to FUS. But, further evidence is still warranted to better ascertain the usefulness of GATT in this specific patient population.

Abstract:Purpose: To compare outcomes of gonioscopy-assisted transluminal trabeculotomy (GATT) over a 12-month period with trabeculectomy in patients with advanced pigmentary glaucoma (PG). Methods: This was a pilot randomised controlled trial of patients with advanced PG (mean deviation worse than -12 dB), undergoing either GATT or a fornix-based trabeculectomy. Absolute success (criterion A) was defined as a postoperative intraocular pressure (IOP) between 6 and 18 mm Hg, with a drop of at least 30% from the treated preoperative value without need of any IOP-lowering medication. Success (criterion B) was also defined as per the target IOP, with an upper limit of 15 mm Hg for eyes with mean deviation (MD) between -12 and -24 dB, and 12 mm Hg or lower for MD values worse than -24 dB. Qualified success was a similar IOP standard on the same or fewer antiglaucoma medications. Results: For GATT (n=10), mean preoperative IOP and number of glaucoma medications were 28.2±11.2 mm Hg and 4±0.8 that reduced to 11.8±2.5 mm Hg and 0.7 at 12 months postoperatively, while in the trabeculectomy (n=12) group, they were 27.3±5.5 mm Hg and 3.6±0.7 that reduced to 11.5±2.2 mm Hg and 0.5±0.9, respectively. All eyes (100%) achieved qualified success. Absolute success was 60% and 67.7% by criterion A and 50% and 58.3% by criterion B for GATT and trabeculectomy, respectively. Two eyes in the trabeculectomy group developed hypotony while none of the GATT group had any sight-threatening complications (p=0.4). Conclusions: GATT alone demonstrated a significant reduction in IOP and number of glaucoma medications in patients with advanced PG.

Abstract:This case report outlines the successful management of angle recession with gonioscopy-assisted transluminal trabeculotomy (GATT). A 53-year-old male with a history of blunt trauma to the left eye resulting in angle recession presented with an IOP of 38 mm Hg on four antiglaucoma agents. Following GATT, a notable reduction in IOP to 10 mm Hg was observed on the first postoperative day. Subsequent one year follow-up visit demonstrated sustained improvement, with IOP stabilizing at 18 mm Hg without the need for antiglaucoma medication. This is the first case report that demonstrates the role of GATT in the management of angle recession.

Abstract:Uveitic glaucoma (UG) is secondary glaucoma, present as a clinical challenge in both diagnosis and management.We report a case of complex UG, which initially presented as pupillary block and rupture of the anterior lens capsule. We performed cataract extraction with preservation of posterior capsule. Then, the case turned to aphakic malignant glaucoma. We performed anterior vitrectomy with posterior capsule resection in this case. After the second operation, the patient had a satisfactory recovery. Specifically, ultrasonographic biomicroscopy was useful during the diagnosis process and follow-up period in this case.UG presenting as pupillary block, rupture of the anterior lens capsule, and aqueous misdirection seldom presents in clinical practice. Earlier and more active surgical intervention may be necessary for effective preservation of visual function in complex cases of UG.

Abstract:Purpose: To investigate the prognostic factors affecting gonioscopy-assisted transluminal trabeculotomy (GATT) surgical success. Methods: Fifty-three eyes were retrospectively enrolled. Open-angle glaucoma patients with at least 6-month follow-up were included. At baseline, demographic characteristics, intraocular pressure (IOP), number of anti-glaucomatous medications (AG), and glaucoma type were recorded. Postoperatively, IOP, complications, and number of AG were noted. The effects of these parameters on surgical success were investigated. Results: The median follow-up time was 13.7 months. The mean IOP decreased from 25.6 ± 6.2 mm Hg at baseline to 14.6 ± 3.5 mm Hg at final, and the number of AG decreased from 3.2 ± 0.78 to 1.2 ± 1.3. When target IOP was considered as 18 mm Hg and 15 mm Hg, surgical success rates were 81.1% and 60.4%, respectively. When target IOP was considered as 18 mm Hg, a positive effect of the combination with cataract surgery and a negative effect of postoperative macrohyphema on success rates were observed. Other factors did not show any association with the success rates for both target IOP values when analyzed by the Cox proportional hazards regression analysis. Conclusion: Postoperative macrohyphema may affect surgical success rates negatively. The contribution of the combination with cataract surgery is controversial. Glaucoma stage does not seem to affect surgical success.
Combined Trabeculectomy and Vitrectomy for Intractable Glaucoma with Severe Ocular Contusion Involving the Posterior Segment.

Abstract:Background Intractable secondary glaucoma with severe ocular contusion involving the posterior segment is difficult to treat. In this study, we evaluated the safety and efficacy of combined trabeculectomy and vitrectomy for intractable glaucoma with severe ocular contusion involving the posterior segment.Methods This retrospective interventional study enrolled 18 eyes from 18 consecutive cases with uncontrolled intraocular pressure, angle recession exceeding 1800, dense vitreous hemorrhage, and/or limited retinal detachment after severe ocular contusion. Combined trabeculectomy and vitrectomy was performed after giving the patient sufficient anti-inflammatory and intraocular pressure (IOP)-lowering medication. Follow-up averaged 26.2 (range, 6.0-48.0) months.Results The mean IOP decreased from (36.4 +/- 10.0) mmHg on maximum IOP-lowering medications to (14.6 +/- 4.4) mmHg on (0.7 +/- 1.2) topical medications at the final follow-up (P <0.0001). During the first postoperative month, four eyes (22%) had a short hypertensive phase and six eyes (33%) had early hypotony, including one case (5.6%) of bleb leakage. Seven eyes (39%) developed recurrent hypertension 3-6 months postoperatively, five of which were controlled by topical medications. Ten (55.6%) eyes were classified as a complete success, five (27.8%) as a qualified success, and three eyes (16.7%) as failures. Kaplan-Meier survival analysis for complete and cumulative success showed 53.5% and 80.0% survival at 48 months, respectively. Visual acuity was improved in 15 eyes (83.3%). Three eyes (16.7%) had unchanged visual acuity, one (5.6%) of which developed atrophia bulbi despite a normal intraocular pressure. No recurrent retinal detachment or vitreous hemorrhage developed.Conclusion Combined trabeculectomy and vitrectomy is a viable surgical procedure for the management of intractable glaucoma with severe ocular contusion involving the posterior segment in selected cases. Abstract:<h3 class="Heading">Objectives</h3><p class="Para">To compare successes and complications of XEN GEL Stent implantation and gonioscopy-assisted transluminal trabeculotomy (GATT) surgery in patients with open-angle glaucoma (OAG).</p><h3 class="Heading">Methods</h3><p class="Para">The multicentre, non-randomized, comparative, interventional, retrospective chart review study included 114 eyes undergoing XEN implantation and 107 eyes undergoing GATT for treatment of OAG. A comprehensive ophthalmic examination consisting of best-corrected LogMAR visual acuity (BCVA), Goldmann Applanation Tonometry, biomicroscopy, fundoscopy and gonioscopy was performed, and then, clinical findings, complications and number of antiglaucomatous medications were noted at the 3-, 6-, 12-, 18- and 24-month postoperative visits. Surgical success was defined as IOP ≤ 21 mmHg and 20% ≥ IOP reduction from baseline with (qualified success) or without (complete success) further medication, and without any further IOP-lowering surgery.</p><h3 class="Heading">Results</h3><p class="Para">The mean ages were 65.8 ± 10.6 and 59.1 ± 14.3 in XEN and GATT groups, respectively (<em class="EmphasisTypeItalic">p</em> = 0.001). In XEN group, the percentage of IOP reduction and need of medication were significantly more improved than those in GATT group. The complete surgical success rates were 34.2% and 50.5% in XEN and GATT groups, respectively (<em class="EmphasisTypeItalic">p</em> = 0.039), with a success rate of 41.1% in total. Qualified surgical success rates were 97.4% and 89.7% in XEN and GATT groups, respectively (<em class="EmphasisTypeItalic">p</em> = 0.025), with a success rate of 93.7% in total. Transient hyphema, the most observed postoperative complication in both groups, cleared in a few days.</p><h3 class="Heading">Conclusions</h3><p class="Para">Both MIGS have good efficacy and safety outcomes in lowering IOP and need of medication in patients with OAG. XEN Gel Stent implantation may be preferred in patients with lower IOP values targeted.</p>

Abstract:Goniotomy assisted transluminal trabeculotomy (GATT) is a kind of ab interno angle procedure. GATT spares conjunctiva and sclera, so it is a real mini invasive glaucoma surgery (MIGS). MIGS techniques are typically used in cases of early and moderate open angle glaucoma. The aim is to present the possibility of GATT application in the selected types of refractory glaucoma or secondary open angle glaucoma which are believed to be difficult in management. The results of GATT application in refractory glaucoma, inflammatory glaucoma and silicon oil glaucoma will be presented with the evaluation of IOP lowering potential in short term observation. Additionally, selected clinical cases will be showed.

Abstract:Purpose: To evaluate the effect of Gonioscopy-assisted transluminal trabeculotomy (GATT) on macular thickness and to compare the combined GATT and cataract extraction (CE) with GATT surgery alone regarding macular thickness. Methods: A retrospective, comparative study was designed. The patients who underwent GATT or combined GATT and CE with preoperative and regular postoperative spectral domain optic coherence tomography analysis (SD-OCT) were included. The macula was visualized in a 5 × 5 mm2 area by SD-OCT using MM5 mode. Two different groups were defined as follows: group 1 enrolled patients who underwent GATT alone, and group 2 enrolled patients who underwent combined GATT and CE. Pre- and postoperative macular thickness measurements by performing intragroup analysis and comparing group 1 with group 2 with regard to change in macular thickness were assessed. Results: Fifty-four eyes fulfilled the inclusion criteria and were enrolled in this study, (Group 1 = 29 eyes, Group 2 = 25 eyes). The mean thicknesses of central macula (CM), superior inner macula (SIM), and nasal inner macula (NIM) significantly increased 1 month after GATT alone (p < 0.05). This difference became statistically insignificant at 3rd month after the surgery, (p > 0.05). The mean thicknesses of SIM, temporal inner macula (TIM), NIM, superior outer macula (SOM), inferior outer macula (IOM), and temporal outer macula (TOM) showed a significant increase 1 month after combined GATT and CE, (p < 0.05), which return its preoperative levels 3 months after the surgery (p > 0.05). There was no significant difference between group 1 and group 2 with regard to change in macular thickness at each time points, (p > 0.05). Postoperative clinically significant CME was observed in one patient who underwent GATT alone (p = 0.351). Conclusion: GATT can be a safe procedure with minimal and transient adverse effect on macular thickness and structure. Performing GATT together with CE caused no additional increase in the risk of macular thickening.

Abstract:Objectives: To evaluate the long-term structural changes of the anterior chamber (AC) angle following gonioscopy-assisted transluminal trabeculotomy (GATT). Methods: The AC angle of 10 eyes that underwent GATT at least 6 years previously was assessed for structural changes. A detailed gonioscopy was performed to determine the state of the cleft and the position of the trabecular flap. An anterior segment optical coherence tomography (AS-OCT) examination was performed on the corresponding areas on gonioscopy. Results: The typical finding of the angle following GATT was an open cleft with a visible trabecular flap. However, the gonioscopy of our patients revealed three different cleft appearances: open, closed, and segmentally open cleft. In the long-term, the trabecular flap re-approximated the incision site in some areas resulting in the appearance of a closed cleft on gonioscopy. On AS-OCT the cleft was identified when the lumen of Schlemm's canal was connected to the AC, while the position of the flap differed. The cleft was observed as open in median 4.0 (IQR: 2.8-6.0) clock hours. The cleft was found open mostly in the superior quadrants of the angle (nine eyes). No correlation was found between the extent of open cleft and the percentage of IOP reduction. Conclusion: AS-OCT, when used in conjunction with gonioscopy, was found helpful to evaluate the structural changes following GATT. As observed in the study, the cleft tended to close in some areas. It was found preserved mostly in the superior half of the angle in the long term.

Abstract:Précis: We report a case of a 62-year-old male patient who developed a wipe-out phenomenon after uncomplicated gonioscopy-assisted transluminal trabeculotomy combined with cataract surgery. A 62-year-old male patient with a diagnosis of pseudo-exfoliation and open-angle glaucoma underwent uncomplicated gonioscopy-assisted transluminal trabeculotomy (GATT) combined with phacoemulsification surgery due to advanced glaucomatous damage and high intraocular pressure. Postoperative decrease in visual acuity and central visual field deterioration suggested a wipe-out phenomenon. Although the wipe-out phenomenon has been reported after other surgeries in patients with advanced glaucoma, as far as we know, this is the first report of a wipe-out phenomenon secondary to GATT combined with phacoemulsification surgery. This complication, which can lead to severe vision loss, should be considered before the surgical decision.
