Abstract:Purpose: To investigate the prognostic factors affecting gonioscopy-assisted transluminal trabeculotomy (GATT) surgical success. Methods: Fifty-three eyes were retrospectively enrolled. Open-angle glaucoma patients with at least 6-month follow-up were included. At baseline, demographic characteristics, intraocular pressure (IOP), number of anti-glaucomatous medications (AG), and glaucoma type were recorded. Postoperatively, IOP, complications, and number of AG were noted. The effects of these parameters on surgical success were investigated. Results: The median follow-up time was 13.7 months. The mean IOP decreased from 25.6 ± 6.2 mm Hg at baseline to 14.6 ± 3.5 mm Hg at final, and the number of AG decreased from 3.2 ± 0.78 to 1.2 ± 1.3. When target IOP was considered as 18 mm Hg and 15 mm Hg, surgical success rates were 81.1% and 60.4%, respectively. When target IOP was considered as 18 mm Hg, a positive effect of the combination with cataract surgery and a negative effect of postoperative macrohyphema on success rates were observed. Other factors did not show any association with the success rates for both target IOP values when analyzed by the Cox proportional hazards regression analysis. Conclusion: Postoperative macrohyphema may affect surgical success rates negatively. The contribution of the combination with cataract surgery is controversial. Glaucoma stage does not seem to affect surgical success.

Abstract:Prcis: This study concludes that GATT can be an effective and safe surgical alternative for managing IOP in eyes with prior failed glaucoma surgeries, associated with minimal complications. Purpose: To report outcomes of gonioscopy-assisted transluminal trabeculotomy (GATT) in eyes with prior failed glaucoma surgery. Patients and methods: A retrospective study involving 30 eyes of 30 patients, all of whom had open angles on gonioscopy, experienced prior glaucoma surgery failures, and subsequently underwent GATT. The primary outcome measure was success defined as complete when the intraocular pressure (IOP) was >5 and ≤21/16 mm Hg without glaucoma medications and qualified with medications. Results: The mean age was 51.8±16.1 years. Twenty-one eyes underwent GATT and 9 eyes underwent phaco-GATT. Twenty-seven eyes had failed trabeculectomy and 3 eyes had failed glaucoma drainage device. Post-GATT, the IOP decreased from 27.1±7 to 16.9±6 mm Hg (P<0.001) at the end of 15 months, with a mean drop in AGM from 4.9±1.0 to 2±1.6. At postoperative 1 year, the probability of complete success was 20% (95% CI: 9-43) for an IOP criterion of both 21 and 16 mm Hg. The qualified success probability at 1 year was 82% (67-100) for an IOP criterion of 21 mm Hg and 57% (38-84) for an IOP criterion of 16 mm Hg. Risk factor for failure was older age [hazard ratio (HR): 1.03, 95% CI: 1.01-1.06]. The complications noted were hyphema in 14 eyes (46%), majority resolved within 1 week and all by 2 weeks. None needed any intervention. Conclusions: This study concludes that GATT can be an effective and safe surgical alternative for managing IOP in eyes with prior failed glaucoma surgeries, associated with minimal complications.

Abstract:Purpose: To compare outcomes of gonioscopy-assisted transluminal trabeculotomy (GATT) over a 12-month period with trabeculectomy in patients with advanced pigmentary glaucoma (PG). Methods: This was a pilot randomised controlled trial of patients with advanced PG (mean deviation worse than -12 dB), undergoing either GATT or a fornix-based trabeculectomy. Absolute success (criterion A) was defined as a postoperative intraocular pressure (IOP) between 6 and 18 mm Hg, with a drop of at least 30% from the treated preoperative value without need of any IOP-lowering medication. Success (criterion B) was also defined as per the target IOP, with an upper limit of 15 mm Hg for eyes with mean deviation (MD) between -12 and -24 dB, and 12 mm Hg or lower for MD values worse than -24 dB. Qualified success was a similar IOP standard on the same or fewer antiglaucoma medications. Results: For GATT (n=10), mean preoperative IOP and number of glaucoma medications were 28.2±11.2 mm Hg and 4±0.8 that reduced to 11.8±2.5 mm Hg and 0.7 at 12 months postoperatively, while in the trabeculectomy (n=12) group, they were 27.3±5.5 mm Hg and 3.6±0.7 that reduced to 11.5±2.2 mm Hg and 0.5±0.9, respectively. All eyes (100%) achieved qualified success. Absolute success was 60% and 67.7% by criterion A and 50% and 58.3% by criterion B for GATT and trabeculectomy, respectively. Two eyes in the trabeculectomy group developed hypotony while none of the GATT group had any sight-threatening complications (p=0.4). Conclusions: GATT alone demonstrated a significant reduction in IOP and number of glaucoma medications in patients with advanced PG.

Abstract:Glaucoma is a disease that leads to optic nerve damage and irreversible loss of the visual field. Gonioscopy-assisted transluminal trabeculotomy (GATT) is a minimally invasive glaucoma surgery technique, where the surgeon opens the trabecular network and Schlemm's canal to improve the outflow of the aqueous humor. This method has shown efficacy in reducing intraocular pressure in patients with primary and secondary glaucoma. GATT has gained popularity due to its minimally invasive nature, compatibility with cataract surgery and relatively low cost. Despite its advantages, the procedure can be associated with many complications such as hyphema, intraocular pressure spikes and corneal edema. In rare cases, it can lead to Descemet's membrane detachment or cystoid macular edema. The success of GATT procedure mostly depends on the surgeon's skill and experience, but also on the patient's risk factors, including age and preoperative intraocular pressure levels. Further studies are required to understand long outcomes and identify the patients, who may be at higher risk of complications.

Abstract:BACKGROUND:Trabeculectomy is the most efficient surgical treatment. Prevention failure of bleb cicatrix would lead to unsatisfactory postoperative intraocular pressure (IOP) controlling and unsatisfactory success rate. The aim of this study was to evaluate the 5-year outcomes of trabeculectomy with a cross-linked sodium hyaluronate gel implantation for Chinese glaucoma patients. METHODS:This is a prospective, case-controlled study. Patients who were to be applied first-time trabeculectomy in the Department of Ophthalmology of Peking University Third Hospital between 2010 and 2012 were included in the study. Totally, 60 eyes were randomly assigned to the trabeculectomy group (TA group) or the trabeculectomy with cross-linked sodium hyaluronate gel implantation group (TH group). Follow-up was finished at 1 week, 1 month, 3 months, 1 year, 3 years, and 5 years after the operation. The statistical index of demographic data, IOP, bleb shape, and any complications or medications or surgical procedures were recorded and assessed by SPSS 19.0 software through independent t-test, one-way analysis of variance (ANOVA) and Pearson's Chi-square test, respectively. RESULTS:The baseline IOP was comparable between the two groups (t= -1.00, P= 0.32) while the postoperative IOP was significantly lower in the TH group at 1, 3 and 5 years' time points (P = 0.00, P= 0.01 and P = 0.01, respectively). According to the Indiana Bleb Appearance Grading Scale, the height and extent of bleb were better in the TH group at all follow-up time points (P < 0.05), however, the comparison of bleb vascularity showed no statistical difference (P > 0.05). TA group had a higher percentage of complications (13% vs. 3%) compared to TH group; however, there was no statistical difference in the comparison of each statistical item (P > 0.05, respectively). The complete success at 5 years was higher in the TH group than that in the TA group (78% vs. 54%, P = 0.03). CONCLUSION:Our results suggested that implantation of cross-linked sodium hyaluronate gel with trabeculectomy was more efficient and would improve the prognosis of glaucoma patients.

Abstract:Précis: Gonioscopy-assisted Transluminal Trabeculotomy may be an effective first line surgery for decreasing intraocular pressure and medication burden in patients with uveitis-related ocular hypertension or glaucoma. Purpose: The purpose of the study is to determine the efficacy of Gonioscopy-assisted transluminal trabeculotomy (GATT) to lower the intraocular pressure (IOP) in uveitis-related ocular hypertension (OHT) or glaucoma. Methods: Retrospective case series that included patients with uveitis-related OHT or glaucoma that underwent GATT with or without concomitant cataract extraction and intraocular lens implantation at two Canadian academic centres from July 2018 to May 2022. Primary outcomes were : complete (no medications) and qualified success (with medication), and failure defined as (1) IOP > 21mmHg with maximal medical therapy, (2) the need of additional glaucoma procedure, (3) loss of light perception secondary to glaucoma, (4) IOP < 6 mmHg for 3 months. Results: Twenty-one eyes from 18 patients were included with a mean preoperative IOP of 26.2±7.3 mmHg on 4.3±0.7 classes of glaucoma drops. Average follow-up was 29.2±17.6 months and 76% of eyes (n=16) had reached at least 12 months of follow-up. At the 12 month follow-up visit, there was a significant decrease in average IOP by 9.9±7.9 mmHg (38%, P =0.005) and a decrease of 1.9 in glaucoma medication classes ( P =0.002). Fourteen percent of eyes achieved complete success while 80% of eyes achieved qualified success. Six eyes failed (29%) and five patients (24%) required an additional glaucoma surgery. The most common postoperative complication was hyphema (n=9; 43%). Conclusion: This small case series suggests that GATT may be an effective first line surgery for decreasing IOP and medication burden in patients with uveitis-related OHT or glaucoma. Further studies with longer follow-up should be conducted to assess its long-term outcomes.

Abstract:Objectives: To evaluate the long-term structural changes of the anterior chamber (AC) angle following gonioscopy-assisted transluminal trabeculotomy (GATT). Methods: The AC angle of 10 eyes that underwent GATT at least 6 years previously was assessed for structural changes. A detailed gonioscopy was performed to determine the state of the cleft and the position of the trabecular flap. An anterior segment optical coherence tomography (AS-OCT) examination was performed on the corresponding areas on gonioscopy. Results: The typical finding of the angle following GATT was an open cleft with a visible trabecular flap. However, the gonioscopy of our patients revealed three different cleft appearances: open, closed, and segmentally open cleft. In the long-term, the trabecular flap re-approximated the incision site in some areas resulting in the appearance of a closed cleft on gonioscopy. On AS-OCT the cleft was identified when the lumen of Schlemm's canal was connected to the AC, while the position of the flap differed. The cleft was observed as open in median 4.0 (IQR: 2.8-6.0) clock hours. The cleft was found open mostly in the superior quadrants of the angle (nine eyes). No correlation was found between the extent of open cleft and the percentage of IOP reduction. Conclusion: AS-OCT, when used in conjunction with gonioscopy, was found helpful to evaluate the structural changes following GATT. As observed in the study, the cleft tended to close in some areas. It was found preserved mostly in the superior half of the angle in the long term.
Safety and Efficiency of Trabectome-mediated Trabecular Meshwork Ablation for Chinese Glaucoma Patients: A Two-year, Retrospective, Multicentre Study.

Abstract:Background: The aim of the study was to evaluate the long-term safety and efficacy of the Trabectome for Chinese glaucoma patients. Methods: This was a multicenter, retrospective, observational study. Glaucoma patients, except those with neovascular glaucoma, with/without a visually significant cataract were enrolled. The patients received Trabectome or a combined surgery with phacoemulsification and intraocular lens implantation. The primary outcome evaluation was a reduction in intraocular pressure (IOP), and the secondary outcomes were a reduction in glaucoma medication, the 2-year success percentages, and complications. Success was defined as an IOP <21 mmHg and at least a 20% IOP reduction from baseline after 3 months for any two consecutive visits, without additional glaucoma surgery. The data were processed using the R Stats Package version 3.0.0. The Wilcoxon test was used to compare the postoperative IOP and the number of glaucoma medications with baselines. The Kaplan–Meier test was used to calculate the 2-year success percentage. The risk factors related to Trabectome failure were determined by logistic regression. Results: A total of 120 glaucoma patients were enrolled. The Trabectome efficiently reduced the IOP from a baseline of 22.8 ± 1.34 mmHg to 17.6 ± 0.96 mmHg, and the use of glaucoma medications from 2.2 ± 0.17 mmHg to 1.4 ± 0.21 in a 2-year follow-up (both, P < 0.01). The overall success percentage was 80%. No risk factor related to Trabectome failure was identified. No vision-threatening complication was observed. Ten patients, who required secondary glaucoma surgery, all reached the target IOP. Conclusions: In a 2-year follow-up, Trabectome was an efficient and safe procedure for Chinese glaucoma patients.
[Trabeculotomy on Primary Congenital Glaucoma: a Retrospective Study of 164 Cases (257 Eyes)].

Abstract:OBJECTIVE:To study the relationship of long-term effect of trabeculotomy on primary congenital glaucoma and the related risk factors.METHOD:164 consecutive patients with primary congenital glaucoma (257 eyes), underwent initial surgery of trabeculotomy between 1996 and 2007. Follow-up was conducted for 30.9 (8.6 - 58.3) months, with a follow-up rate of 89.02%. Multivariate analysis by Logistic regression was conducted to analyze the relation of the factors including age of onset, time between onset and operation, preoperative intraocular pressure, clarity of cornea, and corneal diameter to the failure of surgery. Cox proportional hazards regression modeling was used to analyze the factors related to success of surgery.RESULT:Multivariate logistic regression showed that the preoperative intraocular pressure (IOP) and clarity of cornea were independent risk factors for final outcome (OR(IOP) = 1.408, P = 0.047, and OR(CLA) = 1.691, P = 0.019). Cox regression showed that clarity of cornea was the factor related to the surgery success time (OR(CLA) = 1.632, P = 0.008).CONCLUSION:Clarity of cornea reflexes the condition of primary congenital glaucoma more stably than IOP. It is possible to prognosticate the surgical outcome to combine the clarity of cornea with the IOP value before operation.

Abstract:Objective Objective:To evaluate the safety and efficacy of Trabectome in the treatment of open angle glaucoma.Methods A total of 42 cases were recruited in the study including primary or secondary open angle glaucoma.All cases had undergone Trabectome,and 34 cases had undergone Trabectome combined with phacoemulsification cataract extraction.Major outcomes include intraocular pressure (IOP),number of glaucoma medications and complications,if any.Success was defined as IOP ≤ 21mmHg,at least 20％ IOP reduction in any two consecutive visits after 3 months (with or without medication) and no additional glaucoma surgery needed.Results The longest follow-up period was 51 months,and 13 cases (31％) have been followed up at least 24 months and 30 cases (71％) have been followed up at least 12 months.IOP was reduced 24％ and 20％,at 12 and 24 months after operation,respectively.The number of glaucoma medications was reduced from 2.1±0.9 to 1.0±0.7 at 12 months and to 1.5±1.0 at 24 months with statistical significance (P ＜ 0.05).All surgeries were successful with blood reflux during the surgery.Mild hyphema was seen in 2 cases,which was absorbed within 2 days.The survival rate was 78％ at 12 months and 64％ at 24 months.In the group of combined phacoemulsification,the survival rate was 96％ at 12 months and 90％ at 24 months.Three cases required additional glaucoma surgery.Conclusions Trabectome is a safe and effective minimally invasive glaucoma surgery,with less complications.Combining with phacoemulsification,Trabectome showed higher successful rate in the treatment of open angle glaucoma.

The Chinese patients were enrolled from three glaucoma centers and a group of comparable Japanese POAG patients was analyzed from our international Trabectome database. The patients received Trabectome or a combined surgery with phacoemulsification and intraocular lens implantation. The primary outcome was intraocular pressure (IOP) reduction. Secondary outcomes included reduction of glaucoma medications, surgical complications, and success at 2y. Success was defined as: 1) IOP <= 21 mm Hg and at least 20% IOP reduction from baseline after 3mo at any two consecutive visits; 2) no additional glaucoma surgery required.Results: A total of 42 Chinese POAG patients from three glaucoma centers were enrolled. Twelve patients underwent Trabectome surgery combined with phacoemulsification and intraocular lens implantation while the remainder underwent Trabectome surgery alone. Thirteen patients had a history of failed glaucoma surgery and were considered as complicated cases. In China data, the mean preoperative IOP was 21.4 +/- 1.23 mm Hg. The Trabectome lowered IOP to 17.9 +/- 1.8 mm Hg at 2y (P=0.05). The number of glaucoma medications also decreased significantly from a baseline of 2.0 +/- 0.9 to 1.1 +/- 0.8 at 2y post-surgery (P=0.04). The overall 2-year success rate was 78%, with patients undergoing combined surgery having a higher success rate compared with those undergoing Trabectome surgery alone (100% vs 76%). In Japan data, the mean preoperative IOP was 20.8 +/- 7.7 mm Hg. The Trabectome lowered IOP to 12.20 +/- 2.0 mm Hg at 2y. The number of glaucoma medications also decreased significantly from a baseline of 2.1 +/- 0.9 to 3.4 +/- 0.6 at 2y post-surgery. In all patients, no major complications were seen.Conclusion: Surgery with the Trabectome appears to be an efficient and safe procedure in Chinese POAG patients in the long-term.
[Long-term effect of trabeculotomy on primary congenital glaucoma].

Abstract:OBJECTIVE:To evaluate the long-term effect of trabeculotomy on primary congenital glaucoma.\nMETHODS:This retrospective study included primary congenital glaucoma patients who underwent trabeculotomy as an initial surgery between 1988 and 1999 with follow-up at least one year. Definitions of successful operation were determined as (1) intraocular pressure (IOP) </= 21 mm Hg with clear cornea, (2) no antiglaucoma medication or only local medication of topical antiglaucoma drops, and (3) no adverse progression in cup-disc ratio (C/D) and cornea diameter, or cup-disc ratio (C/D) and cornea diameter reduced compared to pre-operation.\nRESULTS:The study included 33 eyes with 72.7% patients who were followed-up for over three years. The mean follow-up time was (54.2 +/- 33.2) months. After trabeculotomy, the average IOP was significantly lowered to (16.1 +/- 6.2) mm Hg from (29.9 +/- 11.7) mm Hg (P < 0.001); and cornea diameter was significantly reduced to (12.9 +/- 1.0) mm from (13.7 +/- 1.4) mm (P = 0.002). Although there was no significant difference in the average C/D after operation (P = 0.148), the change in C/D was significantly different in patients in the success group [(0.7 +/- 0.2) mm, pre-operation, (0.6 +/- 0.3) mm, post-operation, P = 0.007]. Based on the criteria for successful operation, the success rate of trabeculotomy was 97.0%, 93.2, 74.5% at one, three and five years by Kaplan-Meier analysis respectively. For 20 eyes which received only one operation, the success rate was 70.0%. However, in the rest of 13 eyes (39.6%) that had received two or more operations, the success rate was reduced slightly to 69.2%. In patients with successful operations, 41.7% of patients had correct visual acuity >/= 0.4, 29.1% between 0.1 and 0.3, and 29.2% </= 0.1. By linear regression analysis, it was found that C/D > 0.8 (P = 0.013) was risk factor responsible for poor vision. Among 18 eyes that were examined for visual field, glaucomatous visual field loss was found in 14 eyes (77.7%). Early-stage defect was seen in 42.8%, intermediate-stage in 28.6%, and late-stage in 28.6% of eyes. Hyphema was found in 14 eyes (42.4%). However, hyphema in 10 out of 14 eyes (71.4%) was absorbed in just one day. Therefore, hyphema should not be considered as a high risk for surgical failure (P = 0.795).\nCONCLUSIONS:Trabeculotomy had a high success rate in treatment of primary congenital glaucoma with fewer complications during the follow-up time. Observation from repeated operations also showed satisfactory results. Therefore, the optimal goal in preserving useful visual acuity and visual field is to obtain better control of glaucoma with trabeculotomy and treatment of amblyopia.
XEN gel implant versus gonioscopy-assisted transluminal trabeculotomy for the treatment of open-angle glaucoma

Abstract:<h3 class="Heading">Objectives</h3><p class="Para">To compare successes and complications of XEN GEL Stent implantation and gonioscopy-assisted transluminal trabeculotomy (GATT) surgery in patients with open-angle glaucoma (OAG).</p><h3 class="Heading">Methods</h3><p class="Para">The multicentre, non-randomized, comparative, interventional, retrospective chart review study included 114 eyes undergoing XEN implantation and 107 eyes undergoing GATT for treatment of OAG. A comprehensive ophthalmic examination consisting of best-corrected LogMAR visual acuity (BCVA), Goldmann Applanation Tonometry, biomicroscopy, fundoscopy and gonioscopy was performed, and then, clinical findings, complications and number of antiglaucomatous medications were noted at the 3-, 6-, 12-, 18- and 24-month postoperative visits. Surgical success was defined as IOP ≤ 21 mmHg and 20% ≥ IOP reduction from baseline with (qualified success) or without (complete success) further medication, and without any further IOP-lowering surgery.</p><h3 class="Heading">Results</h3><p class="Para">The mean ages were 65.8 ± 10.6 and 59.1 ± 14.3 in XEN and GATT groups, respectively (<em class="EmphasisTypeItalic">p</em> = 0.001). In XEN group, the percentage of IOP reduction and need of medication were significantly more improved than those in GATT group. The complete surgical success rates were 34.2% and 50.5% in XEN and GATT groups, respectively (<em class="EmphasisTypeItalic">p</em> = 0.039), with a success rate of 41.1% in total. Qualified surgical success rates were 97.4% and 89.7% in XEN and GATT groups, respectively (<em class="EmphasisTypeItalic">p</em> = 0.025), with a success rate of 93.7% in total. Transient hyphema, the most observed postoperative complication in both groups, cleared in a few days.</p><h3 class="Heading">Conclusions</h3><p class="Para">Both MIGS have good efficacy and safety outcomes in lowering IOP and need of medication in patients with OAG. XEN Gel Stent implantation may be preferred in patients with lower IOP values targeted.</p>
