Abstract:This multimodal brain imaging study investigated functional MRI (fMRI) neural processing of cardiac interoceptive signals in 38 patients with functional neurological disorder (FND) compared to 38 healthy controls (HCs). Additionally, we characterized how brain fMRI responses during heartbeat counting (interoception) vs. tone counting (exteroception) or rest related to grey matter volume, interoceptive awareness, and psychopathology scores. For both interoception vs. rest and interoception vs. exteroception contrasts, principal component analyses showed that principal component 1 (PC1) as derived from all study participants was comprised primarily of salience, ventral attention and sensorimotor network co-activations, along with default mode and visual processing network co-deactivations. Compared to HCs, patients with FND showed reduced contribution to these PC1 co-activation/co-deactivations patterns in both interoception vs. exteroception and interoception vs. rest contrasts; only the interoception vs. exteroception between-group fMRI findings held adjusting for depression/anxiety scores, antidepressant use and FND subtype. For the interoception vs. rest contrast, increasingly negative PC1 contribution scores positively correlated with decreased cingulate gyrus volumes and increased psychopathology scores. This multimodal brain imaging study underscores a role for salience and default-mode networks in the pathophysiology of FND, and sets the stage for comprehensive research efforts further contextualizing the mechanistic importance of altered interoception in patients with FND.

Abstract:Altered interoception may be a pathophysiological mechanism in functional neurological disorder (FND). However, findings have been inconsistent across interoceptive dimensions in FND including functional motor symptoms (FMS) and seizures (FS). Here, individuals with FMS/FS (n = 17) and healthy controls (HC, n = 17) completed measures of interoceptive accuracy and insight (adapted heartbeat tracking task [HTT] with confidence ratings), a time estimation control task (TET) and the Multidimensional Assessment of Interoceptive Awareness–2 (MAIA-2) to assess interoceptive sensibility. The groups did not differ in interoceptive accuracy ( p = 1.00, g = 0.00) or confidence ( p = .99, g = 0.004), although the FMS/FS group displayed lower scores on the "Not-Distracting" ( p < .001, g = 1.42) and "Trusting" ( p = .005, g = 1.17) MAIA-2 subscales, relative to HCs. The groups did not differ in TET performance ( p = .82, g = 0.08). There was a positive relationship between HTT accuracy and confidence (insight) in HCs ( r = .61, p = .016) but not in FMS/FS ( r = 0.11, p = .69). HTT confidence was positively correlated with MAIA-2 "Self-Regulation" ( r = 0.77, p = .002) and negatively correlated with FND symptom severity ( r = −0.84, p < .001) and impact ( r = −0.86, p < .001) in FMS/FS. Impaired interoceptive accuracy may not be a core feature in FMS/FS, but reduced insight and altered sensibility may be relevant. Reduced certainty in self-evaluations of bodily experiences may contribute to the pathogenesis of FND symptoms.

Functional Connectivity of Anterior Insula Predicts Recovery of Patients with Disorders of Consciousness

Abstract:Background: We hypothesize that the anterior insula is important for maintenance of awareness. Here, we explored the functional connectivity alterations of the anterior insula with changes in the consciousness level or over time in patients with disorders of consciousness (DOC) and determined potential correlation with clinical outcomes. Methods: We examined 20 participants (9 patients with DOC and 11 healthy controls). Each patient underwent resting-state functional magnetic resonance imaging (rs-fMRI) and a standardized Coma Recovery Scale-Revised (CRS-R) assessment on the same day. We categorized the patients according to the prognosis: those who emerged from a minimally conscious state (recovery group, n = 4) and those who remained in the unconscious state (unrecovery group, n = 5). Two rs-fMRI scans were obtained from all patients, and the second scan of patients in the recovery group was obtained after they regained consciousness. We performed seed-based fMRI analysis and selected the left ventral agranular insula (vAI) and dorsal agranular insula (dAI) as the regions of interest. Correlations with CRS-R were determined with the Spearman's correlation coefficient. Results: Compared with healthy controls, the functional connectivity between dAI and gyrus rectus of patients who recovered was significantly increased (p < 0.001, cluster-wise family-wise error rate [FWER] < 0.05). The second rs-fMRI scan of patients who remained with DOC showed a significant decreased functional connectivity between the dAI to contralateral insula, pallidum, bilateral inferior parietal lobule (IPL), precentral gyrus, and middle cingulate cortex (p < 0.001, cluster-wise FWER < 0.05) as well as the functional connectivity between vAI to caudate and cingulum contrast to controls (p < 0.001, cluster-wise FWER < 0.05). Finally, the functional connectivity strength of dAI-temporal pole (Spearman r = 0.491, p < 0.05) and dAI-IPL (Spearman r = 0.579, p < 0.05) were positively correlated with CRS-R scores in all DOC patients. The connectivity of dAI-IPL was also positively correlated with clinical scores in the recovery group (Spearman r = 0.807, p < 0.05). Conclusions: Our findings indicate that the recovery of consciousness is associated with an increased connectivity of the dAI to IPL and temporal pole. This possibly highlights the role of the insula in human consciousness. Moreover, longitudinal variations in dAI-IPL and dAI-temporal pole connectivity may be potential hallmarks in the outcome prediction of DOC patients.
Corticolimbic fast-tracking: enhanced multimodal integration in functional neurological disorder

Abstract:Objective: Some individuals with functional neurological disorder (FND) exhibit motor and affective disturbances, along with limbic hyper-reactivity and enhanced motor-limbic connectivity. Given that the multimodal integration network (insula, dorsal cingulate, temporoparietal junction (TPJ)) is implicated in convergent sensorimotor, affective and interoceptive processing, we hypothesised that patients with FND would exhibit altered motor and amygdalar resting-state propagation to this network. Patient-reported symptom severity and clinical outcome were also hypothesised to map onto multimodal integration areas. Methods: Between-group differences in primary motor and amygdalar nuclei (laterobasal, centromedial) were examined using graph-theory stepwise functional connectivity (SFC) in 30 patients with motor FND compared with 30 healthy controls. Within-group analyses correlated functional propagation profiles with symptom severity and prospectively collected 6-month outcomes as measured by the Screening for Somatoform Symptoms Conversion Disorder subscale and Patient Health Questionnaire-15 composite score. Findings were clusterwise corrected for multiple comparisons. Results: Compared with controls, patients with FND exhibited increased SFC from motor regions to the bilateral posterior insula, TPJ, middle cingulate cortex and putamen. From the right laterobasal amygdala, the FND cohort showed enhanced connectivity to the left anterior insula, periaqueductal grey and hypothalamus among other areas. In within-group analyses, symptom severity correlated with enhanced SFC from the left anterior insula to the right anterior insula and TPJ; increased SFC from the left centromedial amygdala to the right anterior insula correlated with clinical improvement. Within-group associations held controlling for depression, anxiety and antidepressant use. Conclusions: These neuroimaging findings suggest potential candidate neurocircuit pathways in the pathophysiology of FND.

Abstract:INTRODUCTION: Simultaneous interoceptive, emotional, and social cognition deficits are observed across neurodegenerative diseases. Indirect evidence suggests shared neurobiological bases underlying these impairments, termed the allostatic-interoceptive network (AIN). However, no study has yet explored the convergence of these deficits in neurodegenerative diseases or examined how structural and functional changes contribute to cross-domain impairments.METHODS: A PRISMA Activated Likelihood Estimate (ALE) metanalyses encompassed studies meeting inclusion criteria: interoception, emotion, or social cognition tasks; neurodegenerative diseases (behavioral variant frontotemporal dementia (bvFTD), primary progressive aphasias (PPAs) Alzheimer's disease (AD), Parkinson's Disease (PD), multiple sclerosis (MS)); and neuroimaging (structural: MRI voxel-based morphometry; functional: fMRI and FDG-PET).RESULTS: From 20,593 studies, 170 met inclusion criteria (58 interoception, 65 emotion, and 47 social cognition) involving 7032 participants (4963 patients and 2069 healthy controls). In all participants combined, conjunction analyses revealed AIN involvement of the insula, amygdala, orbitofrontal cortex, anterior cingulate, striatum, thalamus, and hippocampus across domains. In bvFTD this conjunction was replicated across domains, with further involvement of the temporal pole, temporal fusiform cortex, and angular gyrus. A convergence of interoception and emotion in the striatum, thalamus, and hippocampus in PD and the posterior insula in PPAs was also observed. In AD and MS, disruptions in the AIN were observed during interoception, but no convergence with emotion was identified.INTERPRETATION: Neurodegeneration induces dysfunctional AIN across atrophy, connectivity, and metabolism, more accentuated in bvFTD. Findings bolster the predictive coding theories of large-scale AIN, calling for more synergistic approaches to understanding interoception, emotion, and social cognition impairments in neurodegeneration.

Abstract:Th e University of Padua along with the Italian Society for Vocational Guidance, the University of Bologna, the Italian Association of Psychology (AIP) as well as the Laboratory of Research and Intervention in Vocational Guidance (La.R.I.O.S) organized an international conference on career counselling. Th e subject of the conference centred on the issues implied by its theme, concerning the challenges and perspectives in the support of professional development faced by counselling science and practice. Th e purpose of the conference was to search for an answer to the question of how consultancy and career counselling can be practiced in conditions of common uncertainty, deep social and economic crisis as well as the threat of unemployment and marginalization, which are accompanied by an increasing demand for mobility and fl exibility in both private and professional life. Th erefore, Laura Nota and Salvatore Soresi, the organizers and hosts of the meeting, made questions regarding the future of counselling and the science dealing with its issues, the centre of the conference’s attention. Th e three-day discussion focused on searching for the answer to the question of counsellors’ readiness and competence to give their support in the current economic and social situation in the western world, as well as the choice and promotion of a theoretical model of career counselling, which would be adequate for existing and future challenges. Th e organizers did not try to conceal their ambition to make a breakthrough in the practice of European counselling, which is bolstering up professional development through promulgating the idea of life designing, which can be identifi ed with the concept of biographical counselling formulated in our country. Th e image of Galileo, which was promoted at the Italian conference, was the symbol of this paradigmatic breakthrough. Th e choice of the Renaissance scholar, who rejected scholasticism as a method of scientifi c approach, which was commonly used in his time, in favour of empiricism, does not seem to be accidental. Th e initiation of diverse changes in science has always been noticeable throughout the history of the Paduan University. Abstract:Interactions between top-down attention and bottom-up visceral inputs are assumed to produce conscious perceptions of interoceptive states, and while each process has been independently associated with aberrant interoceptive symptomatology in psychiatric disorders, the neural substrates of this interface are unknown. We conducted a preregistered functional neuroimaging study of 46 individuals with anxiety, depression, and/or eating disorders (ADE) and 46 propensity-matched healthy comparisons (HC), comparing their neural activity across two interoceptive tasks differentially recruiting top-down or bottom-up processing within the same scan session. During an interoceptive attention task, top-down attention was voluntarily directed towards cardiorespiratory or visual signals, whereas during an interoceptive perturbation task, intravenous infusions of isoproterenol (a peripherally-acting beta-adrenergic receptor agonist) were administered in a double-blinded and placebo-controlled fashion to drive bottom-up cardiorespiratory sensations. Across both tasks, neural activation converged upon the insular cortex, localizing within the granular and ventral dysgranular subregions bilaterally. However, contrasting hemispheric differences emerged, with the ADE group exhibiting (relative to HCs) an asymmetric pattern of overlap in the left insula, with increased or decreased proportions of co-activated voxels within the left or right dysgranular insula, respectively. The ADE group also showed less agranular anterior insula activation during periods of bodily uncertainty (i.e., when anticipating possible isoproterenol-induced changes that never arrived). Finally, post-task changes in insula functional connectivity were associated with anxiety and depression severity. These findings confirm the dysgranular mid-insula as a key cortical interface where attention and prediction meet real-time bodily inputs, especially during heightened awareness of interoceptive states. Further, the dysgranular mid-insula may indeed be a "locus of disruption" for psychiatric disorders.

Interoceptive training impacts the neural circuit of the anterior insula cortex

Abstract:Interoception is the perception of afferent information that arises from anywhere and everywhere within the body. Recently, interoceptive accuracy could be enhanced by cognitive training. Given that the anterior insula cortex (AIC) is a key node of interoception, we hypothesized that resting functional connectivity (RSFC) from AIC was involved in an effect of interoceptive training. To address this issue, we conducted a longitudinal intervention study using interoceptive training and obtained RSFC using fMRI before and after the intervention. A heartbeat perception task evaluated interoceptive accuracy. Twenty-two healthy volunteers (15 females, age 19.9 ± 2.0 years) participated. After the intervention, interoceptive accuracy was enhanced, and anxiety levels and somatic symptoms were reduced. Also, RSFC from AIC to the dorsolateral prefrontal cortex (DLPFC), superior marginal gyrus (SMG), anterior cingulate cortex (ACC), and brain stem, including nucleus tractus solitarius (NTS) were enhanced, and those from AIC to the visual cortex (VC) were decreased according to enhanced interoceptive accuracy. The neural circuit of AIC, ACC, and NTS is involved in the bottom-up process of interoception. The neural circuit of AIC, DLPFC, and SMG is involved in the top-down process of interoception, which was thought to represent the cognitive control of emotion. The findings provided a better understanding of neural underpinnings of the effect of interoceptive training on somatic symptoms and anxiety levels by enhancing both bottom-up and top-down processes of interoception, which has a potential contribution to the structure of psychotherapies based on the neural mechanism of psychosomatics.

Intrinsic Functional Brain Connectivity Patterns Underlying Enhanced Interoceptive Sensibility.

Abstract:BACKGROUND:Cumulative evidence has suggested that brain regions including the bilateral insula and the anterior cingulate cortex play critical roles in the processing of interoceptive information. However, the brain functional connectivity patterns underlying interoceptive sensibility (IS) and their role in the relationship between IS and self-reported bodily symptoms remain unknown. We aimed to investigate the intrinsic functional connectivity patterns associated with IS and how this modulates the relationship between IS and self-reported bodily symptoms.METHODS:Resting-state functional magnetic resonance imaging was used to test the intrinsic large-scale functional connectivity in 459 healthy subjects. IS and self-reported bodily symptoms were assessed by questionnaires.RESULTS:Individuals with greater IS had a stronger tendency to report bodily symptoms. Higher IS was correlated with decreased ventral anterior insula-superior temporal gyrus, dorsal anterior cingulate cortex-middle frontal cortex and amygdala-medioventral occipital cortex connectivity. The large-scale functional connectivity of cingulo-opercular task control network (CON)-default mode network, CON-subcortical network and CON-dorsal attention network moderated the association between IS and bodily symptoms.LIMITATIONS:The Body Perception Questionnaire mainly reflects the self-perceived tendency to focus on negatively-valenced interoceptive sensations. Future research should distinguish neutral and negative IS in order to make the definition of IS clearer.CONCLUSIONS:This study suggests that IS may be related to impaired intrinsic functional connectivity between brain areas related to multisensory integration and cognitive-affective control, resulting in increased vigilance-attention to bodily signals. These findings offer new empirical evidence for a better understanding of the intricate relationships between interoception and symptom reporting.

Abstract:Biocompatible polysuccinimide (PSI) derivatives conjugated with diethylenetriaminepentaacetic acid gadolinium (DTPA-Gd) were prepared as magnetic resonance imaging (MRI) contrast agents. In this study, we synthesized PSI derivatives incorporating methoxy-poly(ethylene glycol) (mPEG) as hydrophilic ligand, hexadecylamine as hydrophobic ligand, and DTPA-Gd as contrast agent. PSI was synthesized by the polycondensation polymerization of aspartic acid. All the synthesized materials were characterized by proton nuclear magnetic resonance (1H NMR). Critical micellization concentrations were determined using fluorescent probes (pyrene). Micelle size and shape were measured by electro-photometer light scattering (ELS) and atomic force microscopy (AFM). The formed micelle size ranged from 100 to 300 nm. The T1-weighted MR images of the phantom prepared with PSI-mPEG-C16-(DTPA-Gd) were obtained in a 3.0 T clinical MR imager, and the conjugates showed a great potential as MRI contrast agents.

BOLD signal variability as potential new biomarker of functional neurological disorders

Abstract:Background: Functional neurological disorder (FND) is a common neuropsychiatric condition with established diagnostic criteria and effective treatments but for which the underlying neuropathophysiological mechanisms remain incompletely understood. Recent neuroimaging studies have revealed FND as a multi-network brain disorder, unveiling alterations across limbic, self-agency, attentional/salience, and sensorimotor networks. However, the relationship between identified brain alterations and disease progression or improvement is less explored. Methods: This study included resting-state functional magnetic resonance imaging (fMRI) data from 79 patients with FND and 74 age and sex-matched healthy controls (HC). First, voxel-wise BOLD signal variability was computed for each participant and the group-wise difference was calculated. Second, we investigated the potential of BOLD signal variability to serve as a prognostic biomarker for clinical outcome in 47 patients who attended a follow-up measurement after eight months. Results: The results demonstrated higher BOLD signal variability in key networks, including the somatomotor, salience, limbic, and dorsal attention networks, in patients compared to controls. Longitudinal analysis revealed an increase in BOLD signal variability in the supplementary motor area (SMA) in FND patients who had an improved clinical outcome, suggesting SMA variability as a potential state biomarker. Additionally, higher BOLD signal variability in the left insula at baseline predicted a worse clinical outcome. Conclusion: This study contributes to the understanding of FND pathophysiology, emphasizing the dynamic nature of neural activity and highlighting the potential of BOLD signal variability as a valuable research tool. The insula and SMA emerge as promising regions for further investigation as prognostic and state markers.

Abstract:Interoception is the ability to consciously perceive internal bodily states. Neuroimaging suggests that the insula (IC) and anterior cingulate cortex (ACC) mediate interoception, while studies involving patients/animals with brain lesions suggest the medial temporal lobe (MTL) is particularly important. One reason for these contrasting conclusions may lie in the types of interoceptive task used by these different approaches. Some tasks probably require integration of current physiological state with mnemonic information (e.g., how much one last ate), and these may be especially reliant upon MTL processing. We compared one task that probably requires integration - a water load task - with one that likely does not - a heart-rate tracking task - in two individuals with selective MTL damage (and with intact IC and ACC). A group of matched healthy individuals served as controls. The main finding was that individuals with MTL damage, relative to controls, were equally and significantly impaired on both types of interoception task. This suggests that MTL structures are involved in mediating interoception even when using a task (heart rate tracking) that does not seemingly require memory and that in neuroimaging studies activates the IC and ACC. The reasons for this apparent inconsistency with neuroimaging findings and the functional role of the MTL in interoception are discussed.

We designed a task to engage interoceptive and exteroceptive attention by orienting healthy participants to their heartbeats or auditory stimuli and investigated whether brain dynamics (Kolmogorov complexity – KC, permutation entropy – PE, weighted symbolic mutual information – wSMI, power spectrum density – PSD) and the heartbeat-evoked potential (HEP) distinguished interoceptive from exteroceptive covert attention. Exteroceptive attention yielded an overall flattening of the PSD, whereas during interoceptive attention there was a decrease in complexity, an increase in frontal connectivity and oscillations in the theta range, and a modulation of the HEP. Subject-level classifiers based on HEP features classified the attentional state of 17/20 participants. KC, PE, and wSMI showed comparable accuracy in classifying exteroceptive-interoceptive attention and exhibited a synergic behavior with the HEP features. PSD features demonstrated exceptional performance (20/20). Command-following was assessed in 5 brain-injured patients with a modified version of the task. An Unresponsive Wakefulness Syndrome/Vegetative State patient and a locked-in syndrome patient demonstrated a willful modulation of the HEP and the patient-level classifiers suggest that patients were complying with task instructions. Our findings show that directing attention to bodily rhythms or external stimuli elicits distinct neural responses that can be employed to track covert attention at the individual level. Importantly, the brain markers studied in this work provide multiple layers to explore information processing in disorders of conscious patients.

Abstract:Background Functional constipation (FCon), is a symptom-based functional gastrointestinal disorder without an organic etiology and altering brain structure and function. However, previous studies mainly focused on isolated brain regions involved in brain plasticity. Therefore, little is known about the altered large-scale interaction of brain networks in FCon. Methods For this study, we recruited 20 patients with FCon and 20 healthy controls. We used group independent component analysis to identify resting-state networks (RSNs) and documented intra- and inter-network alterations in the RSNs of the patients with FCon. Results We found 14 independent RSNs. Differences in the intra-networks included decreased activities in the bilateral caudate of RSN 3 (strongly related to emotional and autonomic processes) and decreased activities in the left precuneus of RSN 10 (default mode network). Notably, the patients with FCon exhibited significantly decreased interactive connectivity between RSNs, mostly involving the connections to the visual perception network (RSN 7–9). Conclusion Compared with healthy controls, patients with FCon had extensive brain plastic changes within and across related RSNs. Furthermore, the macroscopic brain alterations in FCon were associated with interoceptive abilities, emotion processing, and sensorimotor control. These insights could therefore lead to the development of new treatment strategies for FCon.

Abstract:The ability to attend to and consciously process interoceptive signals is deemed critical for the development of minimal self, adaptive self-regulation and affective experience, and optimal expression of both instrumental and executive cognitive functions. Yet, notwithstanding the richness of theoretical proposals concerning inferential accounts of interoception, empirical evidence is still scarce. Building on such premises, this study was designed to investigate the electrophysiological signature of cognitive processes leading to aware coding of interoceptive signals via EEG source localization. Thirty-six healthy participants completed an interoceptive accuracy task, i.e., the Heartbeat Counting Task (HCT), while we collected task-related and resting state electrophysiological activity. eLORETA modelling and statistical nonparametric mapping were used to estimate intracortical current density and link such estimates to participants' performance at the task. Source analysis highlighted higher current density estimates for alpha frequencies during HCT with respect to rest, with the primary cortical generator in the right parahippocampal gyrus. Also, a set of medial cortical structures—primarily represented by the cingulate gyrus—showed significant relation between task-related changes in current density estimates for beta oscillations and HCT scores. Findings suggest the informativity of EEG task-related measures of neural activation when used to assess interoceptive skills, as well as of the potential of metrics and analysis based on source localization in the quest to improve our understanding of interoceptive accuracy and neurofunctional correlates of related active inferences.

Abstract:Patients with persistent somatoform pain disorder (PSPD) usually experience various functional impairments in pain, emotion, and cognition, which cannot be fully explained by a physiological process or a physical disorder. However, it is still not clear for the mechanism underlying the pathogenesis of PSPD. The present study aimed to explore the intra- and inter-network functional connectivity (FC) differences between PSPD patients and healthy controls (HCs). Functional magnetic resonance imaging (fMRI) was performed in 13 PSPD patients and 23 age- and gender-matched HCs. We used independent component analysis on resting-state fMRI data to calculate intra- and inter-network FCs, and we used the two-sample t-test to detect the FC differences between groups. Spearman correlation analysis was employed to evaluate the correlations between FCs and clinical assessments. As compared to HCs, PSPD patients showed decreased coactivations in the right superior temporal gyrus within the anterior default-mode network and the anterior cingulate cortex within the salience network, and increased coactivations in the bilateral supplementary motor areas within the sensorimotor network and both the left posterior cingulate cortex and the medial prefrontal cortex within the anterior default-mode network. In addition, we found that the PSPD patients showed decreased FNCs between sensorimotor network and audio network as well as visual network, between default-mode network and executive control network as well as audio network and between salience network and executive control network as well as right frontoparietal network, and increased FNCs between sensorimotor network and left frontoparietal network, salience network as well as cerebellum network, which were negatively correlated with the clinical assessments in PSPD patients. Our findings suggest that PSPD patients experience large-scale reorganization at the level of the functional networks, which suggests a possible mechanism underlying the pathogenesis of PSPD.
Decreased Functional Connectivity and Disrupted Neural Network in the Prefrontal Cortex of Affective Disorders: A Resting-State Fnirs Study.

Abstract:Background: Affective disorders (AD) have been conceptualized as neural network-level diseases. In this study, we utilized functional near infrared spectroscopy (fNIRS) to investigate the spontaneous hemodynamic activities in the prefrontal cortex (PFC) of the AD patients with or without medications.Methods: 42 optical channels were applied to cover the superior frontal gyrus (SFG), middle frontal gyrus (MFG), and inferior frontal gyrus (IFG), which constitute one of the most important affective networks of the brain. We performed resting-state measurements on 28 patients who were diagnosed as having AD and 30 healthy controls (HC). Raw fNIRS data were preprocessed with independent component analysis (ICA) and a band-pass filter to remove artifacts and physiological noise.Results: By systematically analyzing the intra-regional, intrahemispheric, and interhemispheric connectivities based on the spontaneous oscillations of.[HbO], our results indicated that patients with AD exhibited significantly reduced intra-regional and symmetrically interhemispheric connectivities in the PFC when compared to HC. More specifically, relative to HC, AD patients showed significantly lower locally functional connectivity in the right IFG, and poor long-distance connectivity between bilateral IFG. In addition, AD patients without medication presented more disrupted cortical organizations in the PFC, and the severity of self-reported symptoms of depression was negatively correlated with the strength of intra-regional and symmetrically interhemispheric connectivity in the PFC.Limitations: Regarding the measuring technique, fNIRS has restricted measurement depth and spatial resolution. During the study, the subgroups of AD, such as major depressive disorder, bipolar, comorbidity, or non-comorbidity, dosage of psychotropic drugs, as well as different types of pharmacological responses were not distinguished and systematically compared. Furthermore, due to the limitation of the research design, it was still not very clear how pharmacological treatment affected the resting state cortical organization of the prefrontal lobe, and the degree of the effect in patients with AD.Conclusion: These results strongly supported that RSFC measured by fNIRS could be a useful and powerful way of delineating the neuropathology of AD.
