Abstract:Although the resting-state networks of patients with disorders of consciousness (DOC) have been widely investigated, the underlying neural mechanisms remain unclear. Here we aimed to explore the static and dynamic alterations in the regional brain activity in patients with DOC and detect the diagnostic ability of each index. Nineteen patients in the vegetative state, 19 in the minimally conscious state (MCS), and 41 healthy controls were recruited for this study. The fractional amplitudes of the low-frequency fluctuation (fALFF) and dynamic fALFF (dfALFF) values were computed, and intergroup differences were detected by analysis of variance. Sub-frequency analysis (slow-4 band and slow-5 band) of fALFF was also performed. Machine learning classifiers were established based on these measures to explore the classification accuracies between patients and controls. The fALFF and dfALFF analyses showed significant intergroup differences in the medial prefrontal gyrus, precuneus, left angular gyrus, and right middle cingulate cortex (MCC), whereas only the dfALFF analysis revealed aberrations in the right inferior frontal gyrus (IFG), right angular gyrus, left supramarginal gyrus (SMG), and left middle occipital gyrus (MOG). Sub-frequency analysis suggested a potential frequency dependent alteration in the default mode network (DMN). The fALFF model exhibited a higher classification accuracy (ACC) (90.50%) than the dfALFF model (86.29%). The combination of fALFF and dfALFF did not improve classification performance. The strength and stability of regional brain activities were disrupted in patients with DOC. Our findings demonstrate that dynamic analysis may reveal more pathological regions and provide a better understanding of the pathophysiologic mechanisms of DOC.

Abstract:Resting-state functional magnetic resonance imaging (rs-fMRI) is widely used to research abnormal functional connectivity (FC) in patients with disorders of consciousness (DOC). However, most studies assumed steady spatial-temporal signal interactions between distinct brain regions during the scan period. The aim of this study was to explore abnormal dynamic functional connectivity (dFC) in DOC patients. After excluding 26 patients' data that failed to meet the requirements of imaging quality, we retained 19 DOC patients (12 with unresponsive wakefulness syndrome and 7 in a minimally conscious state, diagnosed with the Coma Recovery Scale-Revised [CRS-R]) for the dFC analysis. We used the sliding windows approach to construct dFC matrices. Then these matrices were clustered into distinct states using the k-means clustering algorithm. We found that the DOC patients showed decreased dFC in the sensory and somatomotor networks compared with the healthy controls. There were also significant differences in temporal properties, the mean dwell time (MDT) and the number of transitions (NT), between the DOC patients and the healthy controls. In addition, we also used a hidden Markov model (HMM) to test the robustness of the results. With the connectome-based predictive modeling (CPM) approach, we found that the properties of abnormal dynamic network can be used to predict the CRS-R scores of the patients after severe brain injury. These findings may contribute to a better understanding of the abnormal brain networks in DOC patients.

Abstract:PurposeTo detect the consecutive variations of the internetwork interactions over time, which helps to discover the underlying dysfunction of depressive disorders. Abnormal interactions of resting‐state functional networks have been reported in depression. However, little is known regarding the dynamics of how these crucial networks interact and the disease‐related dysfunction.Materials and MethodsFunctional magnetic resonance imaging data at 3.0T in the resting state were acquired from 20 depressed patients and 20 healthy controls. Twelve resting‐state networks were extracted by group‐independent component analysis, and their interactions were calculated through a sliding windowed Granger causality model analysis. The acquired effective connectivity matrices were used to construct multislice networks with modular structures that were detected via a multislice community detection method.ResultsNo significant differences were observed in the modularity and total module numbers between the depressed patients and the healthy controls. The P values were 0.133 with a confidence interval (–0.0001 0.0093) and 0.136 with a confidence interval (–0.30 0.90), respectively. However, the depressed patients exhibited decreased flexibility of the salience network (SN) compared with the controls (P = 0.048, corrected, with a confidence interval 0.0068 0.066).ConclusionSN was inclined to participate less in the multiple brain functional modules across the resting time in depression, and infrequently changed its modular allegiance. Abstract:Introduction Despite the significant scientific progress in the study of the mechanisms underlying mental disorders, stable biomarkers facilitating their diagnosis and differential diagnosis are lacking. Therefore, we attempted to explore possible functional connectivity (FC) differences across some of the more prevalent mental disorders, namely mood disorders. Objectives The current study aimed at investigating the alterations of the functional connectivity of major seeds of the Salience Network (SN) (the Anterior Cingulate Cortex (ACC), and Anterior Insula (AI) in patients with unipolar (Major Depressive Disorder-MDD) or bipolar (Bipolar Disorder-BD) depression as compared to healthy controls (HC). Methods For this study 103 adult subjects underwent resting-state Magnetic Resonance Imaging of whom 60 were patients with a depressive episode (MDD: n=35; BD: n=25), and 43 were healthy controls (HC). The individuals in both groups were matched for age and gender. Each participant has provided written informed consent (Ethics Committee: P186/22.01.2021). Seed-to-voxel analysis was performed via the CONN toolbox running on MATLAB. Random Field Theory parametric statistics was used with a cluster-level FWE correction p<0.05. Results Both the ACC and the right AI demonstrated a statistically significant increase in the FC to the somatosensory cortex and the motor cortex in patients as opposed to HC. In addition, there was hyperconnectivity between ACC and the right Superior Frontal Gyrus, Precuneus, and the Superior Parietal Lobule bilaterally in patients as well. A reduced FC between the ACC and the hippocampus and parahippocampus was observed in depression in comparison to HC. The analysis of the left AI seed yielded no statistically significant between-group differences. Conclusions Our results demonstrate aberrant connectivity between nodes of the SN, the Default Mode Network, and the Limbic Network which might provide clarification on the mechanisms of impaired balance between internally and externally oriented attention, affective and cognitive control in depression. In addition, the alterations of the FC between SN and the somatosensory and motor cortices may be suggested as a possible explanation of the disturbances in the psychomotor activity in mood disorders. Abstract:Variations in neural circuitry, inherited or acquired, may underlie important individual differences in thought, feeling, and action patterns. Here, we used task-free connectivity analyses to isolate and characterize two distinct networks typically coactivated during functional MRI tasks. We identified a "salience network," anchored by dorsal anterior cingulate (dACC) and orbital frontoinsular cortices with robust connectivity to subcortical and limbic structures, and an "executive-control network" that links dorsolateral frontal and parietal neocortices. These intrinsic connectivity networks showed dissociable correlations with functions measured outside the scanner. Prescan anxiety ratings correlated with intrinsic functional connectivity of the dACC node of the salience network, but with no region in the executive-control network, whereas executive task performance correlated with lateral parietal nodes of the executive-control network, but with no region in the salience network. Our findings suggest that task-free analysis of intrinsic connectivity networks may help elucidate the neural architectures that support fundamental aspects of human behavior.

Altered static and dynamic functional network connectivity in individuals with subthreshold depression: a large-scale resting-state fMRI study

Abstract:Dynamic functional network connectivity (dFNC) is an expansion of static FNC (sFNC) that reflects connectivity variations among brain networks. This study aimed to investigate changes in sFNC and dFNC strength and temporal properties in individuals with subthreshold depression (StD). Forty-two individuals with subthreshold depression and 38 healthy controls (HCs) were included in this study. Group independent component analysis (GICA) was used to determine target resting-state networks, namely, executive control network (ECN), default mode network (DMN), sensorimotor network (SMN) and dorsal attentional network (DAN). Sliding window and k-means clustering analyses were used to identify dFNC patterns and temporal properties in each subject. We compared sFNC and dFNC differences between the StD and HCs groups. Relationships between changes in FNC strength, temporal properties, and neurophysiological score were evaluated by Spearman's correlation analysis. The sFNC analysis revealed decreased FNC strength in StD individuals, including the DMN-CEN, DMN-SMN, SMN-CEN, and SMN-DAN. In the dFNC analysis, 4 reoccurring FNC patterns were identified. Compared to HCs, individuals with StD had increased mean dwell time and fraction time in a weakly connected state (state 4), which is associated with self-focused thinking status. In addition, the StD group demonstrated decreased dFNC strength between the DMN-DAN in state 2. sFNC strength (DMN-ECN) and temporal properties were correlated with HAMD-17 score in StD individuals (all p < 0.01). Our study provides new evidence on aberrant time-varying brain activity and large-scale network interaction disruptions in StD individuals, which may provide novel insight to better understand the underlying neuropathological mechanisms.

Abstract:Carry‐over effects on brain states have been reported following emotional and cognitive events, persisting even during subsequent rest. Here, we investigated such effects by identifying recurring co‐activation patterns (CAPs) in neural networks at rest with functional magnetic resonance imaging (fMRI). We compared carry‐over effects on brain‐wide CAPs at rest and their modulation after both affective and cognitive challenges. Healthy participants underwent fMRI scanning during emotional induction with negative valence and performed cognitive control tasks, each followed by resting periods. Several CAPs, overlapping with the default‐mode (DMN), salience, dorsal attention, and social cognition networks were impacted by both the preceding events (movie or task) and the emotional valence of the experimental contexts (neutral or negative), with differential dynamic fluctuations over time. Temporal metrics of DMN‐related CAPs were altered after exposure to negative emotional content (compared to neutral) and predicted changes in subjective affect on self‐reported scores. In parallel, duration rates of another attention‐related CAP increased with greater task difficulty during the preceding cognitive control condition, specifically in the negative context. These findings provide new insights on the anatomical organization and temporal inertia of functional brain networks, whose expression is differentially shaped by emotional states, presumably mediating adaptive homeostatic processes subsequent to behaviorally challenging events.

Abstract:Learning from nature has inspired the fabrication of novel artificial materials that enable researchers to understand and to imitate biology. Bio-inspired research, in particular, owes much of its current development to advances in materials science and creative "smart" system design. The development and application of bio-inspired nanochannels is a burgeoning area in this field of research. Bio-inspired nanochannels enable many potential approaches to study various biomolecules in confined spaces and in real-time by current measurements. In this Perspective, we describe how these bio-inspired systems can be used to build novel, smart nanodevices with precisely controlled functions. Applications for these systems range from simulating the process of ion transport in living organisms by using biomimetic nanochannels to applying artificial nanochannel systems to investigate the chemistry, structure, size, and conformational states of biomolecules.

Abstract:Functional connectivity (FC) as measured by correlation between fMRI BOLD time courses of distinct brain regions has revealed meaningful organization of spontaneous fluctuations in the resting brain. However, an increasing amount of evidence points to non-stationarity of FC; i.e., FC dynamically changes over time reflecting additional and rich information about brain organization, but representing new challenges for analysis and interpretation. Here, we propose a data-driven approach based on principal component analysis (PCA) to reveal hidden patterns of coherent FC dynamics across multiple subjects. We demonstrate the feasibility and relevance of this new approach by examining the differences in dynamic FC between 13 healthy control subjects and 15 minimally disabled relapse-remitting multiple sclerosis patients. We estimated whole-brain dynamic FC of regionally-averaged BOLD activity using sliding time windows. We then used PCA to identify FC patterns, termed "eigenconnectivities", that reflect meaningful patterns in FC fluctuations. We then assessed the contributions of these patterns to the dynamic FC at any given time point and identified a network of connections centered on the default-mode network with altered contribution in patients. Our results complement traditional stationary analyses, and reveal novel insights into brain connectivity dynamics and their modulation in a neurodegenerative disease.

BOLD signal variability as potential new biomarker of functional neurological disorders

Abstract:Background: Functional neurological disorder (FND) is a common neuropsychiatric condition with established diagnostic criteria and effective treatments but for which the underlying neuropathophysiological mechanisms remain incompletely understood. Recent neuroimaging studies have revealed FND as a multi-network brain disorder, unveiling alterations across limbic, self-agency, attentional/salience, and sensorimotor networks. However, the relationship between identified brain alterations and disease progression or improvement is less explored. Methods: This study included resting-state functional magnetic resonance imaging (fMRI) data from 79 patients with FND and 74 age and sex-matched healthy controls (HC). First, voxel-wise BOLD signal variability was computed for each participant and the group-wise difference was calculated. Second, we investigated the potential of BOLD signal variability to serve as a prognostic biomarker for clinical outcome in 47 patients who attended a follow-up measurement after eight months. Results: The results demonstrated higher BOLD signal variability in key networks, including the somatomotor, salience, limbic, and dorsal attention networks, in patients compared to controls. Longitudinal analysis revealed an increase in BOLD signal variability in the supplementary motor area (SMA) in FND patients who had an improved clinical outcome, suggesting SMA variability as a potential state biomarker. Additionally, higher BOLD signal variability in the left insula at baseline predicted a worse clinical outcome. Conclusion: This study contributes to the understanding of FND pathophysiology, emphasizing the dynamic nature of neural activity and highlighting the potential of BOLD signal variability as a valuable research tool. The insula and SMA emerge as promising regions for further investigation as prognostic and state markers.
Functional and structural brain connectivity in disorders of consciousness

Abstract:Brain connectivity, allowing information to be shared between distinct cortical areas and thus to be processed in an integrated way, has long been considered critical for consciousness. However, the relationship between functional intercortical interactions and the structural connections thought to underlie them is poorly understood. In the present work, we explore both functional (with an EEG-based metric: the median weighted symbolic mutual information in the theta band) and structural (with a brain MRI-based metric: fractional anisotropy) connectivities in a cohort of 78 patients with disorders of consciousness. Both metrics could distinguish patients in a vegetative state from patients in minimally conscious state. Crucially, we discovered a significant positive correlation between functional and structural connectivities. Furthermore, we showed that this structure-function relationship is more specifically observed when considering structural connectivity within the intra- and inter-hemispheric long-distance cortico-cortical bundles involved in the Global Neuronal Workspace (GNW) theory of consciousness, thus supporting predictions of this model. Altogether, these results support the interest of multimodal assessments of brain connectivity in refining the diagnostic evaluation of patients with disorders of consciousness.
Fronto-parietal network dynamics to understand deficits in attention performance in multiple sclerosis

Abstract:Introduction Impaired attention performance is a significant burden to people with multiple sclerosis (MS). Brain connectivity fluctuates with transitions between cognitive states, so measurement of network dynamics during these conditions may help to understand MS-related attention impairment. Methods In people with MS and healthy controls, attention was measured using the Attention Network Test. 3T MRI was used to measure structural connectivity and both static and dynamic functional connectivity in the attention-related fronto-parietal network (FPN) at rest and during an attentionally-demanding task. Groups were compared on connectivity of the FPN during rest and task performance. Relationships between network connectivity and attention performance were tested using linear regression. Results The sample comprised 37 people with MS and 23 matched controls. At rest, people with MS had significantly lower structural connectivity (R2=0.13, p=0.004), lower static functional connectivity (R2=0.07, p=0.032) and higher dynamic functional connectivity (R2=0.08, p=0.026) of the FPN. Higher dynamic connectivity was significantly associated with poorer attention performance in people with MS (R2=0.20, p=0.008). During attention-task performance, static functional connectivity was greater in people with MS than controls (R2=0.10, p=0.008). The task-induced reduction in static connectivity (relative to rest) was directly related to attention performance (R2=0.23, p<0.001). Conclusion Increased dynamic functional connectivity of the FPN at rest may be a useful indicator of deficits in sustained attention in people with MS. The transition from rest to active-attentive state is accompanied by an increase in dynamic connectivity, and decrease in static connectivity which may be helpful in understanding aetiology and treatment of attention impairment.

Abstract:In the last two decades, functional magnetic resonance imaging (fMRI) has emerged as one of the most effective technologies in clinical research of the human brain. fMRI allows researchers to study healthy and pathological brains while they perform various neuropsychological functions. Beyond task-related activations, the human brain has some intrinsic activity at a task-negative (resting) state that surprisingly consumes a lot of energy to support communication among neurons. Recent neuroimaging research has also seen an increase in modeling and analyzing brain activity in terms of a graph or network. Since graph models facilitate a systems-theoretic explanation of the brain, they have become increasingly relevant with advances in network science and the popularization of complex systems theory. The purpose of this study is to look into the abnormalities in resting brain functions in adults with Attention Deficit Hyperactivity Disorder (ADHD). The primary goal is to investigate resting-state functional connectivity (FC), which can be construed as a significant temporal coincidence in blood-oxygen-level dependent (BOLD) signals between functionally related brain regions in the absence of any stimulus or task. When compared to healthy controls, ADHD patients have lower average connectivity in the Supramarginal Gyrus and Superior Parietal Lobule, but higher connectivity in the Lateral Occipital Cortex and Inferior Temporal Gyrus. We also hypothesize that the network organization of default mode and dorsal attention regions is abnormal in ADHD patients.

Abstract:Patients with persistent somatoform pain disorder (PSPD) usually experience various functional impairments in pain, emotion, and cognition, which cannot be fully explained by a physiological process or a physical disorder. However, it is still not clear for the mechanism underlying the pathogenesis of PSPD. The present study aimed to explore the intra- and inter-network functional connectivity (FC) differences between PSPD patients and healthy controls (HCs). Functional magnetic resonance imaging (fMRI) was performed in 13 PSPD patients and 23 age- and gender-matched HCs. We used independent component analysis on resting-state fMRI data to calculate intra- and inter-network FCs, and we used the two-sample t-test to detect the FC differences between groups. Spearman correlation analysis was employed to evaluate the correlations between FCs and clinical assessments. As compared to HCs, PSPD patients showed decreased coactivations in the right superior temporal gyrus within the anterior default-mode network and the anterior cingulate cortex within the salience network, and increased coactivations in the bilateral supplementary motor areas within the sensorimotor network and both the left posterior cingulate cortex and the medial prefrontal cortex within the anterior default-mode network. In addition, we found that the PSPD patients showed decreased FNCs between sensorimotor network and audio network as well as visual network, between default-mode network and executive control network as well as audio network and between salience network and executive control network as well as right frontoparietal network, and increased FNCs between sensorimotor network and left frontoparietal network, salience network as well as cerebellum network, which were negatively correlated with the clinical assessments in PSPD patients. Our findings suggest that PSPD patients experience large-scale reorganization at the level of the functional networks, which suggests a possible mechanism underlying the pathogenesis of PSPD.

Resting-state fMRI data were collected from 15 patients with FSM (mean age=42.3±9.4, female=80%) and 52 age-matched healthy controls (HC, mean age=42.1±8.6, female=73%). Each patient underwent a resting-state functional MRI scan for spatial dFNC evaluation and transcranial magnetic stimulation protocols for indirect assessment of GABAergic and glutamatergic transmission. We considered three dynamic brain networks, i.e., the somatomotor network (SMN), the default mode network (DMN) and the salience network (SN), each summarized into four distinct recurring spatial configurations. Compared to HC, patients with FSM showed significant decreased dwell time, e.g. the time each individual spends in each spatial state of each network, in state 2 of the SMN (HC vs. FSM, 13.5±27.1 vs. 1.9±4.1, p=0.034). Conversely, as compared to HC, FSM spent more time in state 1 of the DMN (10.8±14.9 vs. 27.3±38.9, p=0.033) and in state 3 of the SN (23.1±23.0 vs. 38.8±38.2, p=0.001). We found a significant correlation between the dwell time of impaired functional state of the SMN and measures of GABAergic neurotransmission (r=0.581, p=0.037). Specifically, longer impaired dwell time was associated with greater GABAergic inhibition. These findings demonstrate that FSM present altered functional brain network dynamics, which correlate with measures of GABAergic neurotransmission. Both dFNC and GABAergic neurotransmission may serve as potential targets for future intervention strategies.
Interoceptive disruption in functional neurological disorder: a multimodal brain imaging study

Abstract:This multimodal brain imaging study investigated functional MRI (fMRI) neural processing of cardiac interoceptive signals in 38 patients with functional neurological disorder (FND) compared to 38 healthy controls (HCs). Additionally, we characterized how brain fMRI responses during heartbeat counting (interoception) vs. tone counting (exteroception) or rest related to grey matter volume, interoceptive awareness, and psychopathology scores. For both interoception vs. rest and interoception vs. exteroception contrasts, principal component analyses showed that principal component 1 (PC1) as derived from all study participants was comprised primarily of salience, ventral attention and sensorimotor network co-activations, along with default mode and visual processing network co-deactivations. Compared to HCs, patients with FND showed reduced contribution to these PC1 co-activation/co-deactivations patterns in both interoception vs. exteroception and interoception vs. rest contrasts; only the interoception vs. exteroception between-group fMRI findings held adjusting for depression/anxiety scores, antidepressant use and FND subtype. For the interoception vs. rest contrast, increasingly negative PC1 contribution scores positively correlated with decreased cingulate gyrus volumes and increased psychopathology scores. This multimodal brain imaging study underscores a role for salience and default-mode networks in the pathophysiology of FND, and sets the stage for comprehensive research efforts further contextualizing the mechanistic importance of altered interoception in patients with FND.

Investigation of the effect of physiological factors on resting-state and task-based functional connectivity

Abstract:Understanding the brain's functional network through functional connectivity (FC) is crucial for gaining deeper insights into brain functional mechanism and identifying a potential biomarker for diagnosing neurological disorders. Despite the development of various FC measures, their reliability under different conditions remains under-explored. Moreover, physiological noise can obscure true neural activity, and accordingly, introduce errors into FC patterns. This issue necessitates further investigation. In this study, we evaluate and compare the performance of various methods using Local Field Potential and Blood-Oxygen-Level-Dependent signals across different conditions. We also examine the impact of physiological artifacts on BOLD-FC results. Our comprehensive assessment covers multiple modalities of brain signals, diverse task paradigms, and varying noise levels. Our findings reveal that while Granger Causality-based methods exhibit significant limitations, particularly with BOLD data, multivariate techniques (e.g. partial correlation) demonstrate greater robustness in distinguishing between different types of connections within the network. Notably, our results indicate that physiological artifacts substantially affect FC values, leading to erroneous connectivity estimates, especially with bivariate methods. This research offers a foundational analysis of the effects of physiological artifacts on FC results and provides valuable insights for future studies.

Abstract:Introduction: Neuroimaging studies suggest that the human brain consists of intrinsically organized, large-scale neural networks. Among these networks, the interplay among default-mode, salience, and central-executive networks has been widely employed to understand the functional interaction patterns in health and disease. This triple network model suggests that salience network causally controls over default-mode and central-executive networks in healthy individuals. This interaction is often referred to as salience network's dynamic regulating mechanism. However, such interactions are not well understood in individuals with schizophrenia. Methods: In this study, we leveraged resting-state functional magnetic resonance imaging (fMRI) data from schizophrenia (n = 67) and healthy controls (n = 81) and evaluated the directional functional interactions among default-mode, salience, and central-executive networks using stochastic dynamical causal modeling methodology. Results: In healthy controls, our analyses replicated previous findings that salience network regulates default-mode network and central-executive network activities (Mann-Whitney U test; p < 10-8). In schizophrenia, however, our analyses revealed a disrupted salience network-based controlling mechanism over default-mode and central-executive networks (Mann-Whitney U test; p < 10-16). Conclusions: These results indicate that the disrupted controlling mechanism of salience network over the other two neural networks may be a candidate neuroimaging phenotype in schizophrenia.

Abstract:Objective: Cushing's syndrome is associated with long-term cognitive deficits and affective symptoms such as depression and anxiety. The alterations in brain function underlying these deficits after Cushing's syndrome are unclear and therefore we aimed to explore alterations in resting-state functional connectivity in patients with Cushing's syndrome in remission. Design: Cross-sectional case-control study Methods: Nineteen women with Cushing's syndrome in remission for a median time of 7 years (IQR: 6-10) and a mean age of 45 years were included at three University clinics. These patients and 38 age-matched female controls underwent brain imaging at a single center. The main outcome measure was functional connectivity at rest, measured with functional magnetic resonance imaging. Results: The medial temporal lobe (MTL) and prefrontal cortex networks, exhibited elevated functional connectivity among patients compared to controls. The degree of elevated functional connectivity in the MTL was negatively associated with time in remission. Conclusions: Resting-state functional connectivity within glucocorticoid receptor-rich regions, particularly the MTL and medial prefrontal cortex, was increased in patients. These differences in connectivity may provide a neural basis for the cognitive deficits and affective symptoms commonly experienced by patients with Cushing's syndrome in remission.
