Abstract:Study objectives: The study aimed to investigate sex differences in the relationship between sleep quality (self-report and objective) and cognitive function across three domains (executive function, verbal memory, and attention) in older adults. Methods: We analyzed cross-sectional data from 207 participants with normal cognition (NC) or mild cognitive impairment (89 males and 118 females) aged over 60 years. The relationship between sleep quality and cognitive performance was estimated using generalized additive models. Objective sleep was measured with the GT9X Link ActiGraph, and self-reported sleep was measured with the Pittsburgh Sleep Quality Index. Results: We found that females exhibited lower executive function with increased objective total sleep time, with a steeper decline in performance after 400 minutes (p = .015). Additionally, longer objective sleep correlated with lower verbal memory linearly (p = .046). In males, a positive linear relationship emerged between objective sleep efficiency and executive function (p = .036). Self-reported sleep was not associated with cognitive performance in females and males with NC. However, in males with cognitive impairment, there was a nonlinear positive relationship between self-reported sleep and executive function (p < .001). Conclusions: Our findings suggest that the association between sleep parameters on cognition varies between older males and females, with executive function being most strongly associated with objective sleep for both sexes top of form. Interventions targeting sleep quality to mitigate cognitive decline in older adults may need to be tailored according to sex, with distinct approaches for males and females.
Prevalence of sleep disorders in geriatrics: an exploratory study using sleep questionnaires

Abstract:Objectives: This study aimed to investigate the prevalence of sleep problems in older subjects, considering sex and age differences. Methods: Subjects admitted to a geriatrics clinic underwent a medical visit and completed a battery of questionnaires assessing sleep quality, insomnia, sleep apnea risk, excessive daytime sleepiness (EDS), restless legs syndrome (RLS), chronotype, depression and global cognition. Results: Fifty-eight subjects (58.6 % women, mean age 77.36±6.07) were included. The most predominant sleep-related complaint was poor sleep quality (36.2 %), followed by sleep apnea risk (34.5 %), insomnia symptoms (25.9 %), EDS (15.5 %) and RLS (12.1 %). Older women reported more insomnia, poorer sleep quality and depressive symptoms than males. Patients aged ≥ 75 years old had more comorbidities and higher sleep apnea risk compared to those under 75 years old. Conclusions: Sleep problems are frequent in older adults, requiring their screening and treatment for possibly improving well-being and reduce the burden of neuropsychiatric and medical comorbidities.

Abstract:Abstract Introduction Sleep plays an important role in the prevention and development of several chronic diseases. However, there are significant differences between female and male sleep characteristics, some of which can be lifelong. Thus, the objective of this study was to analyze, according to sex, the polysomnographic findings of adults and older adults who participated in 2 editions (2007 and 2015) of EPISONO, an ongoing Brazilian, population-based sleep study. Methods The baseline randomized sample included 1,042 volunteers in 2007, of these 688 were reassessed in 2015. There were 574 women and 468 men in 2007, and 380 women and 308 men in 2015. All examinations and tests were undertaken using the same protocols at both times. All participants completed an institutional questionnaire to collect socioeconomic data, and a range of other questionnaires on health and sleep parameters, and underwent full-night polysomnography (PSG) and peripheral blood collection for biochemical and hematological measurements. In both editions (2007 and 2015) physical and anthropometric assessments were made. The exclusion criteria were: pregnant and nursing women, individuals with self-care limitations (physical or mental), and shift workers. Results We observed that adult (p=0.003) and older adult women(p=0.001) spent more time in N3 than men, with a greater number of awakenings (p=0.001), and an increased periodic leg movement index in older adult women (0.012), but only adult women in the follow-up presented greater sleep efficiency (p=0.047). Men (adults and older adults) remained longer in N1 and adult men (p=0.001) had a higher apnea-hypopnea index (p=0.001). Conclusion This study prospectively evaluated with full-night PSG the sleep of the general population and reported significant findings according to sex and age, suggesting that some aspects of sleep change differ with age and according to sex. Abstract:Summary Prior studies evaluating sleep quality have found that women often have better polysomnography‐defined sleep quality than men, but women subjectively report a greater frequency of sleep disturbances. Although these studies can be partially attributed to study design, it is currently unclear what may be causing these discrepancies. In this study, we aim to identify potential differences in objectively assessed sleep quality between men and women with further emphasis on subgroup analysis based on age. We hypothesize that women's sleep worsens after menopause. Sleep quality was assessed by comparing the Sleep Quality Index, Arousal Index, sleep efficiency and apnea–hypopnea index, as provided by the SleepImage Ring@ 2.3.0, between men and women, with a sub‐group analysis performed by age (18–40 years, 41–50 years, 51–60 years, 61–70 years, and > 70 years), run separately for women and men. In total, 1444 subjects (704 women and 740 men) with a mean age of 53.6 ± 14.71 years were enrolled in this analysis. In women, a significant drop in Sleep Quality Index was noted after age 51 years. Regression analysis demonstrated that age, Arousal Index, sleep efficiency and apnea–hypopnea index correlated significantly with Sleep Quality Index – with age, Arousal Index and apnea–hypopnea index negatively correlated, and sleep efficiency positively correlated. The highest correlation coefficient was obtained for Arousal Index in both women and men. In women, age older than 50 years was associated with a more rapid decrease of sleep quality than men, as defined by an increase in Arousal Index and apnea–hypopnea index with a concurrent decrease in Sleep Quality Index.

Abstract:Abstract Introduction Poor sleep health is a proposed risk factor for Alzheimer’s disease (AD). Despite known sex differences in AD prevalence (higher in women), whether sex impacts associations between objective sleep parameters and specific cognitive functions remains underexplored. Such investigations are warranted and will provide information regarding potential sex-specific targets for early intervention and/or markers of AD risk. This study tested whether sex moderates associations between polysomnographic assessed sleep and cognition in older adults. Methods Older adults (Mage=68.2±5.5; 44 women/29 men) completed overnight polysomnography (Sleep Profiler) and cognitive tasks [NIH Toolbox: Pattern Comparison (processing speed), Oral Symbol Digit Test (attention and processing speed), List Sorting (working memory), Auditory Verbal Memory Task (episodic memory), Flanker (inhibition), Dimensional Change Card Sort (cognitive flexibility)]. Multiple regressions examined whether sleep metrics (sleep onset latency, wake time after sleep onset, sleep efficiency, total sleep time), macro-architecture (%N1-N3, %REM) or micro-architecture (sleep spindle duration and index) interacted with biological sex in associations with cognition, controlling for age, education and apnea-hypopnea index. Results Sleep onset latency (R-squared=.06, p=.03) and sleep spindle duration (R-squared=.05, p=.05) interacted with sex in their associations with cognitive flexibility. Specifically, in women (not men), longer sleep onset latency (B=-.01, SE=.002, p=.03) and longer spindle duration (B=-.08, SE=.03, p = .005) were associated with worse cognitive flexibility. Conclusion The link between polysomnographic sleep and cognitive flexibility in older adults may be sex dependent. Sleep onset latency cognitive (rumination/worry) and neural (physiological arousal/pre-frontal activation) mechanisms may interact with female sex-steroid alterations (post-menopausal drops in circulating estrogen and progesterone) and exacerbate its negative impact on cognitive flexibility. We also speculate that longer sleep spindle duration in the presence of dysfunctional cognitive flexibility may reflect compensatory thalamo-prefrontal cortical regulation (greater need for nighttime cognitive/cortical arousal reduction) that is further influenced by prefrontal sex-steroid mechanisms. Longitudinal studies in larger samples of cognitively healthy and impaired older adults are needed to further understand whether polysomnographic sleep metrics represent sex-specific markers of future cognitive decline. Abstract:Abstract Introduction Although individual sleep characteristics relate to mental and physical health among older adults, such sleep characteristics do not exist in isolation. Multidimensional sleep measures may better capture the comprehensive impact of sleep on health. However, few studies have examined multidimensional sleep health in relation to mental and physical health outcomes among retired older adults. Likewise, few studies have examined sex differences in these relationships. This study examined associations between multidimensional sleep and health outcomes (i.e., depressive symptoms, mental health, physical health, and overall perceived health) among retired older adults in models stratified by sex. Methods Older adults (n = 154; Mage = 68.4, 55.2 % assigned female at birth) reported depressive symptoms (CES-D), and perceived mental, physical, and overall health (RAND-12). The measurement of multidimensional sleep health involved utilizing wrist actigraphy to assess sleep efficiency, timing, duration, and regularity, alongside diary measures to evaluate alertness and satisfaction. Each component was dichotomized and summed to create a composite score (0-6); higher values indicated better sleep health. Hierarchical linear regressions were used to examine relationships between multidimensional sleep health and health outcomes. Covariates include race, age, years of education, subjective social status, and shiftwork exposure history. Results Compared to men, women had poorer multidimensional sleep health (t(135)=2.38, p=.028). In the full sample, poorer multidimensional sleep was associated with greater depressive symptoms (b = -.70, se = .31, p =.024, r2 = .39) as well as poorer physical (b = 1.03, se = .37, p = .006, r2 = .45), mental (b = 1.37, se = .40, p < .001, r2 = .44), and overall perceived health (b = 1.42, se = .35, p <.001, r2 = .52). Sex-stratified analyses showed that significant findings were exclusively evident in women and not in men. Conclusion Multidimensional sleep health influences the physical and mental health among older retired women. Future studies should identify the mechanisms underlying sex differences in multidimensional sleep and the links between multidimensional and health outcomes. Such information could provide valuable insights leading to tailored therapeutics that benefit each sex. Abstract:To determine if sleep quality and daytime sleepiness are associated with the risk of falls in older people. Among older patients with overall poor sleep quality (68.4%), regression models, considering the estimated risk of falling as dependent variable, showed OR: 2.04 for poor sleep quality, and OR: 2.56 for excessive daytime sleepiness (EDS). Poor sleep quality and EDS are significantly associated with increased estimated risk of falling in older adults. There is growing recognition of the importance of managing sleep disorders in the elderly, particularly given their complexity within a multidimensional framework. The aim of the present study is to investigate whether sleep quality and daytime sleepiness are associated with the risk of falls in older adults. This study included patients aged 75 years or older who were consecutively evaluated at the Geriatric Service, University Hospital of Monserrato, Cagliari, Italy. Participants underwent geriatric assessment, including sleep analysis using the Pittsburgh Sleep Quality Index (PSQI), and the Epworth Sleepiness Scale (ESS), as well as an assessment of risk of falls with Performance Oriented Mobility Assessment (POMA). The study included 206 subjects, of whom 70.4% were women, with a median age of 83 years. A total of 141 participants (68.4%) had poor sleep quality, and 43 (20.9%) had excessive daytime sleepiness (EDS). Among people at high risk of falls, 85 participants had bad sleep quality, and 24 a satisfying one (χ2: 8.23, p = 0.0029); additionally, 77 participants did not exhibit daytime sleepiness, while 32 did (χ2: 9.03, p = 0.0027). To further explore these results, we designed logistic regressions, considering dichotomized PSQI and dichotomized ESS as independent variables, and dichotomized POMA as the dependent variable, and finally adjusted them for confounders. The adjusted model for PSQI yielded OR: 2.04 (95%CI 1.02–4.10), and the adjusted model for ESS yielded OR: 2.56 (95%CI 1.17–5.91). Our study demonstrated that an increased estimated risk of falling is associated with poor sleep quality and EDS in older adults, independently of the confounders.

Abstract:The aim of this study was to assess age- and sex-related differences in multiple sleep latency test (MSLT) results and in the performance of the Epworth Sleepiness Scale (ESS) at classifying objective hypersomnia (mean sleep latency (MSL) ≤ 8 min). We studied 480 consecutive adults (39.3 ± 15.3 years old [18–93], 67.7% female) who underwent hypersomnia evaluation. We fit linear regression models to investigate associations between age and sex and sleep latencies (mean and for every nap), after adjusting for total sleep time and sleep efficiency (on polysomnography), and REM-suppressing antidepressant effect. A logistic regression was performed to assess whether age and sex were associated with sleep-onset REM period (SOREMP) occurrence. ROC analysis assessed the diagnostic performance of ESS scores to identify a MSL ≤ 8 min in different age/sex groups. For every 10 years of age, there was 0.41 (95% CI 0.11–0.72, p = 0.008) min reduction in MSL. Objectively (MSL ≤ 8 min) sleepy patients had shortening of latencies in naps 4 and 5 with aging. Female sex was associated with a higher MSL only in patients with MSL > 8 min. A 2.4% reduction in the odds of SOREMP occurrence was observed for every year of age in objectively sleepy patients ( p = 0.045). ESS scores had a better diagnostic performance in older (≥ 50 years old) men than younger (< 50 years old) women ( p < 0.05). Older patients with objectively confirmed hypersomnia may be sleepier in later naps, possibly due to less restorative naps and/or circadian rhythm factors. Self-reported sleepiness is more predictive of objective sleepiness in older men than younger women.

Abstract:Background: The aims of this cross-sectional study were to (a) assess actigraphy-based sleep parameters (total sample and gender differences), (b) assess differences in morphological parameters and physical function between short- versus normal-sleepers and poor- versus good-sleepers, and (c) assess the possible correlations between sleep variables and morphological and physical function parameters in older subjects. Methods: This study enrolled 42 healthy older participants (60-80 years). Participants completed the following clinical evaluations: (1) whole-body dual-energy X-ray absorptiometry to assess the appendicular skeletal muscle mass index; (2) magnetic resonance imaging acquisition to determine the cross-sectional muscle area of thigh muscles and intermuscular adipose tissue; (3) risk of fall assessment through the mini-Balance Evaluation Systems Test; (4) strength assessment: (a) chair stand test and (b) handgrip strength test; (5) sleep monitoring by actigraphy to assess total sleep time, sleep efficiency, wake after sleep onset, sleep latency, fragmentation index, mobile time, and subjective sleep quality. Results: 31.0% of subjects were short-sleepers (total sleep time < 6 hr), 19.1% were poor-sleepers (sleep efficiency < 85%), and gender differences were detected in mobile time (males: 15.8 ± 6.0 and females: 13.4 ± 6.8; p < .001) and fragmentation index (males: 35.3 ± 14.3 and females: 29.6 ± 14.6; p < .001); no significant differences were observed between groups (short- vs. normal-sleepers and poor- vs. good-sleepers) in morphological and physical function variables; correlation analysis showed that sleep latency negatively correlated with Mini-Balance Evaluation Systems Test (r = -.352; p = .022) and a positive correlation was detected between cross-sectional muscle area and mobile time (r = .349, p = .023). Conclusion: No differences were observed in morphological and function parameters between good- versus poor-sleepers, those subjects with worse sleep onset latency (i.e., longer time to fall asleep) registered higher for risk of fall. The potential role of sleep in the physiological mechanisms of muscular aging must be explored through cross-sectional cohort studies with a larger population.
Sex differences in Parkinson's disease-related non motor symptoms: a focus on sleep problems

Abstract:Parkinson's disease (PD) symptomatology differs between females and males, yet the contribution of sex on sleep problems needs further analysis. Here, we aimed to investigate sex-specific patterns in the relationship between sleep problems, assessed using the Parkinson's disease sleep scale (PDSS-2), non motor symptoms (NMS), measured by the NMS scale (NMSS), and health-related quality of life (HR-QoL), evaluated by the Parkinson's disease questionnaire (PDQ-39), in a large cohort of PD patients. One-hundred-fifty-four PD patients were included in the study. Female PD patients (n = 62) exhibited a higher prevalence of sleep problems than males (n = 92), with nocturnal motor-related sleep issues being the most frequent. Sleep disturbances differently correlated with a range of NMS between the two sexes. In females, sleep problems mostly correlated with pain; on the other hand, sleep disturbances were linked to a frailer phenotype characterized by global dysautonomia, perception disturbances, and impaired cognitive function in males. Whether female PD patients experienced a lower HR-QoL than males, sleep disturbances were associated with a worse HR-QoL in both sexes. In conclusion, sleep problems in PD differently burden the two sexes, suggesting possible different etiopathogenesis, diagnostic investigations, and possibly tailored approaches.

Exploring the relationship of sleep duration on cognitive function among the elderly: a combined NHANES 2011-2014 and mendelian randomization analysis

Abstract:Background: As one of the key features of sleep, sleep duration (SD) has been confirmed to be associated with multiple health outcomes. However, the link between SD and cognitive function (CF) is still not well understood. Methods: We employed a combined approach utilizing data from the National Health and Nutrition Examination Survey (NHANES 2011-2014) and Mendelian Randomization (MR) methods to investigate the relationship between SD and CF. In the NHANES cross-sectional analysis, the association between these variables was primarily examined through multivariate linear regression to explore direct correlations and utilized smoothing curve fitting to assess potential nonlinear relationships. To ensure the robustness of our findings, subgroup analyses were also conducted. MR analysis was used to assess the causal relationship between SD and sleeplessness on CF. After excluding confounding factors, univariate and multivariate MR were performed using inverse variance weighting (IVW) as the main analysis method, and sensitivity analysis was performed. Results: The results of our cross-sectional study indicate a notable negative association between SD and CF, forming an inverted U-shaped curve with the inflection point occurring at SD = 6 h. This relationship remains consistent and robust across subgroup analyses differentiated by variables such as age, levels of physical activity, and frequency of alcohol intake. In MR analysis, IVW analysis showed no causal relationship between SD and sleeplessness on CF (Both P > 0.05). Conclusion: Cross-sectional studies suggest the existence of an inverted U-shaped correlation between SD and CF among the elderly. However, MR analysis did not reveal a causal relationship between SD and CF, which the lack of nonlinear MR analysis may limit. These findings provide evidence from a sleep perspective for optimizing cognitive strategies in older adults.
Factors involved in sleep efficiency: a population-based study of community-dwelling elderly persons

Abstract:STUDY OBJECTIVES: Research indicates that sleep efficiency below 80% substantially increases mortality risk in elderly persons. The aim of this study was to identify factors that would best predict poor sleep efficiency in the elderly, and to determine whether associations between these factors and sleep efficiency were similar for men and women and for younger and older elderly persons.METHODS: A total of 2468 individuals aged 65-96 years (40.7% men) participated. They were recruited via random generation of telephone numbers according to a geographic sampling strategy. The participants agreed to have health professionals visit their home and to answer structured interview questions. Sleep efficiency was calculated based on interview responses. Descriptive statistics and logistic regressions were conducted.RESULTS: The factors most strongly associated with sleep efficiency below 80% were pain, nocturia, sleep medication use, and awakening from bad dreams. Some factors varied by sex: women aged 75 years and older or who had an anxiety disorder were more likely to have sleep efficiency below 80%, whereas being single or having painful illness raised the likelihood for men only. Except for sex, all the factors that showed associations with sleep efficiency affected younger and older elderly persons similarly.CONCLUSIONS: Poor sleep efficiency is prevalent among elderly persons. The results shed new light on factors associated with poor sleep efficiency, highlighting the presence of sex differences and that certain factors make no significant contribution, such as typically proscribed sleep hygiene behaviors, mood disorders, and illness in general.

Longitudinal Associations Between Sleep and Cognitive Function in a Cohort of Older Puerto Rican Adults: Sex and Age Interactions.

Abstract:Background Evidence on sleep duration or quality and cognitive function in diverse older adults is limited. We examined prospective associations between subjective sleep measures and cognitive function, with modifying effects of sex and age (<65 vs & GE;65 years). Methods Data are from the longitudinal Boston Puerto Rican Health Study, Waves 2 (n = 943) and 4 (n = 444), with mean follow-up of 10.5 years (range 7.2-12.8). Subjective measures of sleep duration (short <7, ref. 7, or long & GE;8 hours) and insomnia symptoms (sum of difficulty falling asleep, waking up at night, and early morning awakening), were assessed at Wave 2. Linear regression models were used to assess changes in global cognition, executive function, memory, and Mini-Mental State Examination, and tested for modifying roles of sex and age. Results Significant 3-way interaction (sex x age x cognition) in fully adjusted models showed greater decline in global cognitive function in older men with short (& beta; [95% confidence interval]: -0.67 [-1.24, -0.10]) or long sleep duration (-0.92 [-1.55, -0.30]), compared to women, younger men, and older men with 7 hours of sleep. Insomnia symptoms were associated with a greater decline in memory (-0.54, [-0.85, -0.22]) among older men, compared to women and younger men. Conclusion Sleep duration showed a U-shaped association with cognitive decline, and insomnia symptoms were associated with memory decline in fully adjusted models. Older men, versus women and younger men, were at relatively greater risk for cognitive decline associated with sleep factors. These findings are important for personalizing sleep interventions to support cognitive health.

Abstract:Objective: The aim of this study was to investigate the association between sleep duration and muscle quality index (MQI) in middle-aged and older age groups, as limited evidence exists on this topic. Methods: In order to assess the relationship between sleep duration and MQI, a cross-sectional study was undertaken, utilizing data from the National Health and Nutrition Examination Survey (NHANES) acquired during the period from 2011 to 2014. The study comprised a total of 4598 participants aged 20 years and above. To examine the association between sleep duration and MQI, sophisticated weighted multivariate linear regression models were employed. Additionally, smooth curve fitting techniques were applied to examine the possibility of any non-linear relationship between the two variables. Results: The average age of the adults who were enrolled in the study was 38.48±11.69 years, and 46.75% of them were female. The results of the multivariable linear regression models showed that sleep duration had a positive correlation with MQI. However, when subgroup analysis was conducted, it was found that this positive correlation only existed among women (β = 0.09, 95% CI: 0.014 to 0.167). To further confirm the differences between sexes in the relationship between sleep duration and MQI, a weighted generalized additive model (GAM) was used. Conclusions: This research study provides evidence that there is a positive correlation between the duration of sleep and MQI specifically in females, while no such association was observed in males. These findings shed light on the existence of gender disparities in the connection between sleep duration and MQI.

Abstract:Summary Gender and age are well‐established determinants of health and sleep health that influence overall health, which also often varies by gender and age. Sleep architecture is an important component of sleep health. The goal of this analysis was to examine whether associations between age and sleep stages differ by gender in the absence of moderate–severe obstructive sleep apnea (OSA) in a rural setting in Brazil. This study conducted polysomnography recordings in the Baependi Heart Study, a cohort of Brazilian adults. Our sample included 584 women and 309 men whose apnea–hypopnea index was ≤15 events/h. We used splines to distinguish non‐linear associations between age, total sleep time, wake after sleep onset (WASO), N2, N3, and rapid‐eye‐movement sleep. The mean (standard deviation; range) age was 47 (14; 18–89) years. All sleep outcomes were associated with age. Compared to men, women had more N3 sleep and less WASO after adjusting for age. Model‐based comparisons between genders at specific ages showed statistically higher mean WASO for men at ages 60 (+13.6 min) and 70 years (+19.5 min) and less N3 for men at ages 50 (−13.2 min), 60 (−19.0 min), and 70 years (−19.5 min) but no differences at 20, 30, 40 or 80 years. The other sleep measures did not differ by gender at any age. Thus, even in the absence of moderate–severe OSA, sleep architecture was associated with age across adulthood, and there were gender differences in WASO and N3 at older ages in this rural community.

Abstract:Males and females can respond differentially to the same environmental stimuli and experimental conditions. Chronic sleep loss is a frequent and growing problem in many modern societies and has a broad variety of negative outcomes for health and well-being. While much has been done to explore the deleterious effects of sleep deprivation (SD) on cognition in both human and animal studies over the last few decades, very little attention has been paid to the part played by sex differences and gonadal steroids in respect of changes in cognitive functions caused by sleep loss. The effects of gonadal hormones on sleep regulation and cognitive performances are well established. Reduced gonadal function in menopausal women and elderly men is associated with sleep disturbances and cognitive decline as well as dementia, which suggests that sex steroids play a key role in modulating these conditions. Finding out whether there are sex differences in respect of the effect of insufficient sleep on cognition, and how neuroendocrine mediators influence cognitive impairment induced by SD could provide valuable insights into the best therapies for each sex. In this review, we aim to highlight the involvement of sex differences and gonadal hormone status on the severity of cognitive deficits induced by sleep deficiency in both human and animal studies.

Abstract:Objectives Sleep quality plays a vital role in maintaining health in older adults. Sedentary behavior may be a risk factor for poor sleep quality in older adults. This study aimed to explore the relationship between sedentary behavior and sleep quality among older adults in rural China and determine whether there is a sex difference in this association. Design A longitudinal design. The data used in this study were obtained from the Shandong Rural Elderly Health Cohort (wave 1: 2019, wave 2: 2020). Setting and Participants Data were from 2731 individuals aged ≥60 years from rural areas in China. Methods Sleep quality was assessed using the Pittsburgh Sleep Quality Index. Sedentary behavior and control variables were measured using self-reported questions. Multivariable logistic regression and generalized estimating equations were used to assess the associations. Results After full adjustment, the association between sedentary behavior and poor sleep quality was statistically significant (odds ratio [OR] 1.49, 95% CI 1.20-1.85). Specifically, a longer sedentary time was associated with worse subjective sleep quality, less sleep latency, and lower habitual sleep efficiency (OR 1.39-1.58). A significant association was observed in women but not men. Conclusions and Implications Older adults who spend more time engaging in sedentary activities have poorer sleep quality and more sleep problems. Prolonged sedentary time is more detrimentally associated with poor sleep quality in women than men. There is a need for tailored exercise prescriptions and guidelines to stimulate older adults of different sexes to change their sedentary behavior, which may improve sleep quality in older adults.

Abstract:Context: Sparse published data are available from India regarding sleep disorders in elderly, sleep quality, and daytime sleepiness.Aims: To study sleep disturbances in the elderly (>60 years) subjects.Settings and Design: Hospital-based cross-sectional study.Methods and Material: All the subjects underwent a thorough clinical evaluation which included detailed history and a thorough physical examination. The daytime sleepiness was assessed using the Epworth Sleepiness Scale (ESS). The sleep quality (SQ) was evaluated with the Pittsburgh Sleep Quality Index (PSQI).Statistical Analysis Used: Association between categorical variables was studied by Chi-square (χ2) test with continuity correction. All tests were two-tailed; a P value <0.05 was considered as significant.Results: During the period November 2019 to July 2020, 122 elderly subjects were studied; women (n = 70; 57.4%) outnumbered men. Only six (4.9%) patients presented with sleep-related complaints. Seven (5.7%) patients had an ESS score >10 suggestive of increased day time sleepiness. Sixty-four (52.5%) were labelled "bad sleepers" (PSQI >5). Compared with those with ≤3 comorbid conditions, a higher proportion of elderly with >3 comorbid conditions had significantly higher occurrence of poor sleep quality (PSQI >5) and daytime sleepiness (ESS >10). Increased daytime sleepiness (ESS >10) was significantly high in patients with osteoarthritis and cerebrovascular disease.Conclusions: Our observations suggest that a high proportion of elderly subjects who did not complain of sleep-related symptoms were found to have poor sleep quality. Therefore, incorporating SQ assessment as a part of routine geriatric assessment screening would be beneficial in early detection of this condition.

Abstract:OBJECTIVES: The association between excessive daytime sleepiness and health-related quality of life among older adults and at-risk individuals remains unclear. This study examined relationships between excessive daytime sleepiness and unfavorable health-related quality of life and explored the moderating effect of sex.STUDY DESIGN: This was a community-based study of adults aged 65 years or more. Excessive daytime sleepiness was defined as a score exceeding 10 on the Epworth Sleepiness Scale. Multiple logistic regression analyses were used to examine the relationships between excessive daytime sleepiness and health-related quality of life. The moderating effect of sex was examined by testing interaction terms.MAIN OUTCOME MEASURES: Health-related quality of life was measured using the Short Form 12 Health Survey, which includes a physical component summary and a mental component summary. Unfavorable health-related quality of life was defined as the lowest tertile of the scores for both components.RESULTS: In total, 3788 individuals participated. After controlling for covariates, older adults with excessive daytime sleepiness did not have an unfavorable physical component summary but were more likely to have an unfavorable mental component summary (odds ratio 1.96; 95 % confidence interval 1.47-2.61). When stratified by sex, excessive daytime sleepiness was associated with a poor physical component summary in men (odds ratio 1.77, 95 % confidence interval 1.00-3.13) but not in women.CONCLUSIONS: Excessive daytime sleepiness was associated with a poor mental component summary in both sexes; however, the association with a poor physical component summary was specific to men.
