Abstract:Background: To explore the relationship between sleep disturbances and falls in an elderly Chinese population. Methods: Data from 1726 individuals aged 70-87 years from the Rugao Longevity and Ageing Study were used. The Pittsburgh Sleep Quality Index (PSQI) was used to assess sleep variables. Outcomes were falls >= 1 time per year and falls >= 2 times per year. Results: A total of 22.7% of the participants experienced >= 1 fall, and 9.8% experienced >= 2 falls per year. Poor sleep quality was associated with >= 1 fall (OR 1.08, 95% CI 1.05-1.12; OR 1.27, 95% CI 1.14-1.41) and >= 2 falls (OR 1.08, 95% CI 1.03-1.14; OR 1.28, 95% CI 1.10-1.48), with an increase per PSQI score and SD PSQI score, respectively. In addition, sleep quality, sleep latency, sleep efficiency, and sleep disturbance subcomponents were associated with an increased risk of >= 1 fall with ORs of 1.44 (95% CI, 1.21-1.72), 1.23 (95% CI, 1.09-1.40), 1.12 (95% CI, 1.01-1.23) and 1.70 (95% CI, 1.35-2.14), respectively, and were associated with an increased risk of >= 2 falls with ORs 1.54 (95% CI, 1.22-1.96), 1.21(95% CI, 1.02-1.44), 1.17 (95% CI 1.02-1.33), and 1.78 (95% CI, 1.31-2.44), respectively. Further, participants slept <= 5 h per night had an increased risk of >= 1 fall (OR 2.34; 95% CI, 1.59-3.46) and >= 2 falls (OR 2.19; 95% CI, 1.30-3.69). Conclusions: Poor sleep quality and several subcomponent sleep symptoms were consistently associated with increased risk of falls >= 1 time and >= 2 times in Chinese elderly. The identification of sleep disturbances may help identify high-risk Chinese elders who may benefit from fall prevention education.

Abstract:Sleep disorders are a common syndrome and could affect the life quality of the older adults. This study aimed to investigate the association between nutritional status and sleep quality in the Chinese community-dwelling older adults. A total of 2,878 participants ≥ 65 years old from the Yiwu Elderly Cohort were included in the study. Nutritional status was assessed using the Mini Nutritional Assessment Short Form (MNA-SF). Participants were categorized as subjects with malnutrition, at risk of malnutrition or well-nourished based on their MNA-SF score. Abstract:Purposes This study aims to examine the moderating role of psychological resilience in the association between two common marital relationship stressors (poor spousal health and low marital satisfaction) and sleep (sleep quality and sleep duration) among older adults in China. Methods This study used four waves of data from the Chinese Longitudinal Healthy Longevity Survey (2008-2018) (N = 8,769) and mixed-effects logistic models were used. Results Poor spousal health and low marital satisfaction were independently associated with increased odds of poor sleep quality and short sleep duration, and decreased odds of long sleep duration. Greater psychological resilience was associated with increased odds of good sleep quality and decreased odds of short sleep duration, but had no significant effect on long sleep duration. In addition, psychological resilience moderated the odds of poor spousal health and low marital satisfaction on the risk of short sleep duration among older Chinese adults. Conclusion The findings provide novel evidence for the increased importance of marital stress and psychological resilience on sleep health in later life. Policy interventions are needed to support vulnerable older adults who experience poor spousal health and/or low marital satisfaction.

Abstract:Abstract Background The present study is intended to examine the multiple mediating roles of sleep quality and depression in the relationship between psychological resilience and quality of life in middle-aged and older adults hospitalized with chronic diseases. Methods From October 2, 2021, to February 27, 2022, a questionnaire survey was conducted using a multistage stratified sampling method among 339 middle-aged and older adults (45 years and over) hospitalized with chronic diseases. These participants were recruited from a hospital in Zhejiang Province, China. The questionnaire included the Aged Cumulative Disease Rating Scale, the Psychological Resilience Scale, the Pittsburgh Sleep Quality Index Scale, the Depression Scale, and the Quality-of-Life Scale. A descriptive analysis was performed to characterize the sample. Linear regression was utilized to evaluate the relationship between psychological resilience and quality of life. Amos 24.0 was used to analyze the multiple mediated effects of sleep quality and depression. Results Psychological resilience exerted a remarkable direct effect on the quality of life in middle-aged and older adults hospitalized with chronic diseases (β = 0.239, 95% CI = 0.125–0.354), which represented 52.98% of the total effect. Through three significantly mediated pathways indirectly affect the quality of life: (1) through the sleep quality pathway (β = 0.115, 95% CI = 0.056–0.201), which represented 25.39% of the total effect; (2) through the depression pathway (β = 0. 060, 95% CI = 0.023–0.114), which represented 13.24% of the total effect; and (3) through both the sleep quality and depression pathway (β = 0. 038, 95% CI = 0.019–0.074), which represented 8.39% of the total effect. The total mediating effect was 47.02%. Conclusions Sleep quality and depression mediate the relationship between psychological resilience and quality of life in middle-aged and older adults hospitalized with chronic diseases. Therefore, healthcare professionals and stakeholders should be concerned about the sleep status and mental health of middle-aged and older adults hospitalized with chronic diseases, strengthen their attention to psychological resilience, and provide interventions and treatment measures for hospitalized patients who have sleep problems and depressive tendencies.

Age-friendliness of community and sleep quality: The role of mental health

Abstract:Background: The relationship between age-friendliness of community (AFC), mental health and sleep quality is still unclear. Method: Using a sample of Chinese older adults aged ≥60 years (N = 3099), this cross-sectional study aimed to investigate the serial mediating roles of resilience and depressive symptoms between AFC and sleep quality through structural equation modeling. Additionally, this study examined the association between each AFC dimension and sleep quality stratified by mental health, using the multivariable linear regression models. Mental health was categorized based on levels of resilience and depressive symptoms. Sleep quality was assessed using the Pittsburgh Sleep Quality Index (PSQI). The AFC dimensions included housing, transportation, built environment, social participation, as well as social inclusion and health services. Results: Resilience and depressive symptoms completely mediated the relationship between AFC and sleep quality through three paths: (1) the independent mediating role of resilience or (2) depressive symptoms, and (3) the serial mediating roles of resilience and depressive symptoms. Furthermore, higher AFC and higher age-friendliness in transportation dimension as well as social inclusion and health services dimension were associated with better sleep quality among older adults with high levels of mental health. Conclusions: Resilience and depressive symptoms played serial mediating roles in the relationship between AFC and sleep quality. For older adults with low or medium levels of mental health, a community-based approach may not be recommended. Limitations: This cross-sectional study used self-reported sleep measures and could not make cause inferences.
Social Engagement and Sleep Quality among Older Adults in Rural China: Is There a Sex Difference?

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Objectives</h3><p>Sleep quality plays a crucial role in maintaining the health of older people and has become a major public health concern throughout the world. Social engagement, as a core component of "Active Ageing", is one of the key determinants of health outcomes of older adults. This study estimated the relationship between social engagement and sleep quality among older adults living in rural China, and to determine whether there is a sex difference in these associations.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Design</h3><p>A cross-sectional design. Multilevel, multiple linear regression models were used to examine the independent associations between each type of social engagement and sleep quality. All standard errors were clustered at the community level.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Setting and Participants</h3><p>Data were from 3243 adults age 60 years or older from rural areas in China.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>Sleep quality was measured by the Pittsburgh Sleep Quality Index. Social engagement was measured by 3 types of social groups' activity in this study, including participation in hobby groups, community-related organization, and sports groups.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>Participating in hobby groups, community-related organization, and sports groups with high frequency are all associated with better sleep quality. In addition, the positive relation between participating in community-related organization and sleep quality is stronger for older male than female adults, and the positive association between participating in hobby groups/sports groups is stronger for older female than male adults.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions and Implications</h3><p>Older adults with high-frequency of social engagement may have better sleep quality. This study offers new insight for the sleep quality from the perspective of social engagement among older people, implies that encouraging targeted social engagement in older people with different sex may be effective in promoting sleep quality in rural China.</p>

Abstract:OBJECTIVES: The association between excessive daytime sleepiness and health-related quality of life among older adults and at-risk individuals remains unclear. This study examined relationships between excessive daytime sleepiness and unfavorable health-related quality of life and explored the moderating effect of sex.STUDY DESIGN: This was a community-based study of adults aged 65 years or more. Excessive daytime sleepiness was defined as a score exceeding 10 on the Epworth Sleepiness Scale. Multiple logistic regression analyses were used to examine the relationships between excessive daytime sleepiness and health-related quality of life. The moderating effect of sex was examined by testing interaction terms.MAIN OUTCOME MEASURES: Health-related quality of life was measured using the Short Form 12 Health Survey, which includes a physical component summary and a mental component summary. Unfavorable health-related quality of life was defined as the lowest tertile of the scores for both components.RESULTS: In total, 3788 individuals participated. After controlling for covariates, older adults with excessive daytime sleepiness did not have an unfavorable physical component summary but were more likely to have an unfavorable mental component summary (odds ratio 1.96; 95 % confidence interval 1.47-2.61). When stratified by sex, excessive daytime sleepiness was associated with a poor physical component summary in men (odds ratio 1.77, 95 % confidence interval 1.00-3.13) but not in women.CONCLUSIONS: Excessive daytime sleepiness was associated with a poor mental component summary in both sexes; however, the association with a poor physical component summary was specific to men.

Abstract:Abstract Introduction Poor sleep health is a proposed risk factor for Alzheimer’s disease (AD). Despite known sex differences in AD prevalence (higher in women), whether sex impacts associations between objective sleep parameters and specific cognitive functions remains underexplored. Such investigations are warranted and will provide information regarding potential sex-specific targets for early intervention and/or markers of AD risk. This study tested whether sex moderates associations between polysomnographic assessed sleep and cognition in older adults. Methods Older adults (Mage=68.2±5.5; 44 women/29 men) completed overnight polysomnography (Sleep Profiler) and cognitive tasks [NIH Toolbox: Pattern Comparison (processing speed), Oral Symbol Digit Test (attention and processing speed), List Sorting (working memory), Auditory Verbal Memory Task (episodic memory), Flanker (inhibition), Dimensional Change Card Sort (cognitive flexibility)]. Multiple regressions examined whether sleep metrics (sleep onset latency, wake time after sleep onset, sleep efficiency, total sleep time), macro-architecture (%N1-N3, %REM) or micro-architecture (sleep spindle duration and index) interacted with biological sex in associations with cognition, controlling for age, education and apnea-hypopnea index. Results Sleep onset latency (R-squared=.06, p=.03) and sleep spindle duration (R-squared=.05, p=.05) interacted with sex in their associations with cognitive flexibility. Specifically, in women (not men), longer sleep onset latency (B=-.01, SE=.002, p=.03) and longer spindle duration (B=-.08, SE=.03, p = .005) were associated with worse cognitive flexibility. Conclusion The link between polysomnographic sleep and cognitive flexibility in older adults may be sex dependent. Sleep onset latency cognitive (rumination/worry) and neural (physiological arousal/pre-frontal activation) mechanisms may interact with female sex-steroid alterations (post-menopausal drops in circulating estrogen and progesterone) and exacerbate its negative impact on cognitive flexibility. We also speculate that longer sleep spindle duration in the presence of dysfunctional cognitive flexibility may reflect compensatory thalamo-prefrontal cortical regulation (greater need for nighttime cognitive/cortical arousal reduction) that is further influenced by prefrontal sex-steroid mechanisms. Longitudinal studies in larger samples of cognitively healthy and impaired older adults are needed to further understand whether polysomnographic sleep metrics represent sex-specific markers of future cognitive decline. Abstract:Abstract Introduction Sleep health disparity refers to variations in sleep health that negatively impact disadvantaged and underserved populations. Sleep resilience is the ability to function emotionally, cognitively, and physically in the presence of sleep disturbance. In this study, we aimed to investigate whether self-reported sleep resilience in the face of future sleep disturbance is associated with current sleep disturbance and whether this association differed based on demographic variables. Methods A sample of 454 participants ages 18-85 (M=45) completed an online survey through Qualtrics that included demographic variables and a novel measure of projected sleep resilience. Participants also completed the PROMIS-Sleep Disturbance and the PROMIS-Sleep-Related Impairment questionnaires. The demographic variables included age, sex (male, female, non-binary), income level, race (white/non-white), relationship status (married/not married), number of dependents, and education level. Mixture modeling was used to examine whether sleep disturbance and sleep-related impairment predicted projected sleep resilience. A follow-up class analysis was used to determine whether these associations differed across demographic variables. Results Greater sleep disturbance and greater sleep-related impairment predicted lower projected sleep resilience. A latent class analysis revealed 2 classes (traditionally underserved and privileged) that had significant differences in the association between sleep disturbance and projected resilience. The underserved group (Class 1) included higher numbers of women who were more likely to be non-white with lower income, less educated, unmarried, fewer dependents, and more sleep disturbance. The privileged group (Class 2) included higher numbers of men who were more likely to be white with a higher income, highly educated, married, and have more dependents and less sleep disturbance. The underserved group projected lower future sleep resilience holding current sleep disturbance constant than the privileged group. Conclusion Sleep resilience is an important dimension of sleep health that is often overlooked and understudied. Our findings suggest that sleep resilience is lower in traditionally underserved individuals, which may compound sleep health disparities. Having lower projected sleep resilience in the face of future sleep disturbances may increase current stress levels and negatively influence sleep health behaviors. In an effort to rectify sleep health disparities, targeting the factors that contribute to lower projected sleep resilience may be a fruitful avenue. Support (if any)

Abstract:Abstract Introduction Psychological resilience can be viewed as a personality trait, an ability to bounce back, or a positive outcome after life adversities. Although there have been increasing reports of the importance of sleep and circadian factors in contributing to an individual’s resilience, current studies were devoid of objective measurements and in-depth analysis. Whether psychological resilience is related to the slow wave sleep that facilitates recovery processes is of particular interest. Methods This study focused on the association between resilience and sleep in healthy young individuals free of physical and mental disorders. 160 young adults (61.7% female, mean age: 29.55±4.94) were included. Psychological resilience was defined as both a trait and a positive health outcome despite adversities. The trait resilience was measured by the Connor-Davidson Resilience Scale (CDRS-10), at which scores exceeding the 50th percentile of CDRS-10 were categorized as a “High resilient trait”. The outcome resilience was measured by the residual score of the linear regression model with psychological health level as the dependent variable and accumulative adversities as the independent variable. Those with a positive residual value were categorized as “Resilient outcome”. The accumulative adversities included childhood adversities (e.g., emotional and physical neglect, sexual abuse) and lifetime stressors (e.g., accident, death of families or close friends). Sleep architecture was assessed by polysomnography, and sleep microstructure was computed by spectrum power analysis and slow wave sleep autodetection. Results Resilient participants had more slow wave sleep (SWS; N3%: 15.69% vs 12.89%, p = 0.021) and higher delta power in N3 (Absolute: 750.36 uV2 vs 617.55 uV2, p = 0.059) than those non-resilient participants. Linear regression models showed that increased SWS was associated with higher resilient trait (SWS density: B=0.25, p=0.006; Absolute power of N3 delta: B=0.003, p=0.004) and resilient outcome (N3%: B=0.02, p=0.058; SWS density: B: 0.03, p=0.073; Delta power in N3: B=0.0004, p=0.020). Conclusion Psychological resilience was related to greater slow wave sleep. The findings suggested a critical role of sleep quality in maintaining psychological resilience. Longitudinal studies are need to examine the underlying mechanism of the link between resilience and slow wave sleep. On the origins of components separation.

Abstract:SUMMARY The method of abdominal hernia repair known as components separation, described by Ramirez et al. in 1990, has enjoyed widespread popularity because of the sound anatomical principles it uses and most importantly the clinical success of the procedure. Although the authors who described this technique made no proprietary claims and did not discuss the history of abdominal hernia repair, they have certainly been credited with the development of this procedure. With this article, the author hopes to place components separation in historical perspective, and give credit to Donald Herron Young, who published the concept of external oblique relaxing incisions for repair of epigastric hernias in 1961. Born in Canada, Young studied and worked as a surgeon in England for most of his life. A biographical sketch is included.
Prevalence and associated factors of poor sleep quality among Chinese older adults living in a rural area: a population-based study

Abstract:ObjectiveTo investigate the prevalence and associated factors of poor sleep quality among community-dwelling elderly population in a rural area of Northern China.MethodsWe conducted a cross-sectional survey in August–December 2014 and recruited 2195 participants who were aged 65 years or older and living in Yanlou Town of Yanggu County in western Shandong Province, China. Data on demographics, health-related behaviors, and clinical conditions were collected through structured interviews. The Pittsburgh Sleep Quality Index (PSQI) was used to assess the sleep quality and patterns. Poor sleep quality was defined as a PSQI score > 7. We employed multiple logistic models to relate poor sleep quality to various factors.ResultsThe overall prevalence rates of poor sleep quality were 33.8% in the total sample, 39.2% in women and 26.3% in men (P < 0.01). The most common abnormal sleep domains were prolonged sleep latency (39.7%), decreased sleep duration (31.0%), and reduced habitual sleep efficiency (28.8%). Multiple logistic regression analyses revealed that poor sleep quality was significantly associated with female sex (OR = 1.76, 95% CI 1.46–2.12) and clinical comorbidities such as hypertension (OR = 1.28, 95% CI 1.06–1.54), coronary heart disease (OR = 1.60, 95% CI 1.27–2.00), and chronic obstructive pulmonary disease (OR = 1.82, 95% CI 1.34–2.49).ConclusionsThe sleep disorders were highly prevalent among the elderly in rural China. Modifiable risk factors such as cardiometabolic risk factors and disorders were associated with poor sleep quality, which might be potential targets for interventions to improve sleep quality in elderly population.

Abstract:Abstract Introduction Although individual sleep characteristics relate to mental and physical health among older adults, such sleep characteristics do not exist in isolation. Multidimensional sleep measures may better capture the comprehensive impact of sleep on health. However, few studies have examined multidimensional sleep health in relation to mental and physical health outcomes among retired older adults. Likewise, few studies have examined sex differences in these relationships. This study examined associations between multidimensional sleep and health outcomes (i.e., depressive symptoms, mental health, physical health, and overall perceived health) among retired older adults in models stratified by sex. Methods Older adults (n = 154; Mage = 68.4, 55.2 % assigned female at birth) reported depressive symptoms (CES-D), and perceived mental, physical, and overall health (RAND-12). The measurement of multidimensional sleep health involved utilizing wrist actigraphy to assess sleep efficiency, timing, duration, and regularity, alongside diary measures to evaluate alertness and satisfaction. Each component was dichotomized and summed to create a composite score (0-6); higher values indicated better sleep health. Hierarchical linear regressions were used to examine relationships between multidimensional sleep health and health outcomes. Covariates include race, age, years of education, subjective social status, and shiftwork exposure history. Results Compared to men, women had poorer multidimensional sleep health (t(135)=2.38, p=.028). In the full sample, poorer multidimensional sleep was associated with greater depressive symptoms (b = -.70, se = .31, p =.024, r2 = .39) as well as poorer physical (b = 1.03, se = .37, p = .006, r2 = .45), mental (b = 1.37, se = .40, p < .001, r2 = .44), and overall perceived health (b = 1.42, se = .35, p <.001, r2 = .52). Sex-stratified analyses showed that significant findings were exclusively evident in women and not in men. Conclusion Multidimensional sleep health influences the physical and mental health among older retired women. Future studies should identify the mechanisms underlying sex differences in multidimensional sleep and the links between multidimensional and health outcomes. Such information could provide valuable insights leading to tailored therapeutics that benefit each sex. Abstract:Objective To examine associations of sleep duration and quality with cognitive impairment in older adults and the moderating role of gender and age in these associations. Methods This community-based cross-sectional study included 4,837 participants aged 60 years and above. Cognitive function was assessed using the Chinese version of the Mini-Mental State Examination (MMSE), and the participants were grouped based on the presence of cognitive impairment. The duration and quality of sleep were assessed using the Pittsburgh Sleep Quality Index (PSQI). Multivariate logistic regression models were used to analyze associations of sleep duration and quality with cognitive impairment. The role of age and gender in these associations have also been explored. Results The age (mean ± SD) of the participants was 71.13 ± 5.50 years. Of all older adults, 1,811 (37.44%) were detected as cognitive impairment, and 1755 (36.8%) had poor sleep quality. Among those with cognitive impairment, 51.09% were female. The proportion of the participants with cognitive impairment is significantly higher in those with symptoms of depression (49.73%, 273/549) ( χ 2 = 41.275, p < 0.001) than in those without depressive symptoms. After adjustment for multiple confounding factors and the crucial covariate (depressive symptoms), the odds ratios (OR) (95% confidence interval [CI]) of cognitive impairment (with 7–7.9 h regarded as the reference group) for individuals with a sleep duration of <6, 6–6.9, 8–8.9, and ≥ 9 h were 1.280 (1.053–1.557), 1.425 (1.175–1.728), 1.294 (1.068–1.566), and 1.360 (1.109–1.668), respectively. Subgroup analysis showed a V-shaped association between night sleep duration and cognitive impairment in males ( p ≤ 0.05), and the association was stronger for individuals aged 60–80 years. With regard to sleep quality, the fully adjusted OR (95%CI) of cognitive impairment were 1.263 (1.108–1.440). According to scores of subscales in the PSQI, daytime dysfunction was associated with an increased risk of cognitive impairment (OR: 1.128, 95%CI: 1.055–1.207). Subgroup analysis also revealed a statistically significant correlation between poor sleep quality (including daytime dysfunction) and cognitive impairment in different gender and age groups, with the association being stronger in females (OR: 1.287, 95%CI: 1.080–1.534) and those aged 81–97 years (OR: 2.128, 95%CI: 1.152–3.934). For cognitive impairment, the group aged 81–97 years with daytime dysfunction was associated with a higher odds ratio than other age groups. Conclusion The present study showed that inadequate or excessive sleep was associated with cognitive impairment, especially in males, who exhibited a V-shaped association. Cognitive impairment was also associated with poor sleep quality as well as daytime dysfunction, with females and individuals aged 81–97 years exhibiting the strongest association.

Abstract:Objectives Sleep quality plays a vital role in maintaining health in older adults. Sedentary behavior may be a risk factor for poor sleep quality in older adults. This study aimed to explore the relationship between sedentary behavior and sleep quality among older adults in rural China and determine whether there is a sex difference in this association. Design A longitudinal design. The data used in this study were obtained from the Shandong Rural Elderly Health Cohort (wave 1: 2019, wave 2: 2020). Setting and Participants Data were from 2731 individuals aged ≥60 years from rural areas in China. Methods Sleep quality was assessed using the Pittsburgh Sleep Quality Index. Sedentary behavior and control variables were measured using self-reported questions. Multivariable logistic regression and generalized estimating equations were used to assess the associations. Results After full adjustment, the association between sedentary behavior and poor sleep quality was statistically significant (odds ratio [OR] 1.49, 95% CI 1.20-1.85). Specifically, a longer sedentary time was associated with worse subjective sleep quality, less sleep latency, and lower habitual sleep efficiency (OR 1.39-1.58). A significant association was observed in women but not men. Conclusions and Implications Older adults who spend more time engaging in sedentary activities have poorer sleep quality and more sleep problems. Prolonged sedentary time is more detrimentally associated with poor sleep quality in women than men. There is a need for tailored exercise prescriptions and guidelines to stimulate older adults of different sexes to change their sedentary behavior, which may improve sleep quality in older adults.

Prevalence and Risk Factors of Poor Sleep Quality among Chinese Elderly in an Urban Community: Results from the Shanghai Aging Study

Abstract:BACKGROUND: Sleep disorders causes a significant negative effect on mental and physical health, particularly among the elderly. The disease burden and risk factors of poor sleep quality of the elderly need to be verified using a validated form of measurement in urban mainland China.METHODS: This study included 1086 community residents aged ≥ 60 years who completed the Chinese version of the Pittsburgh Sleep Quality Index (CPSQI). Poor sleeper was defined by a CPSQI global score of >5. Subjects also accepted the neurological and neuropsychological assessments, including the Mini-Mental State Examination, Center for Epidemiological Studies Depression Scale, and Zung Self-Rating Anxiety Scale (ZSAS). A history of chronic diseases was confirmed by the medical records of each participant.RESULTS: The prevalence of poor sleep quality in this population was 41.5% (95% confidence interval (CI) = 38.6-44.5%), with a higher rate observed in elderly females (45.8% [95% CI = 41.9-49.7%]) than that in elderly males (35.8% [95% CI = 31.4-40.1%]). The prevalence rate increased with age, from 32.1% (95% CI = 27.8-36.4%) in those aged 60-69 years to 52.5% (95% CI = 45.9-59.1%) in those aged ≥ 80 years (p value for trend<0.001). Multivariate logistic regression analysis indicated that age (OR = 1.03[95% CI = 1.01-1.05], p<0.001), less education duration (OR = 1.04 [95% CI = 1.01-1.08, p = 0.014), living alone (OR = 1.62 [95% CI = 1.02-2.58], p = 0.04), anxiety (ZSAS score: OR = 1.09 [95% CI = 1.05-1.12], p<0.001), number of chronic disease (OR = 1.18 [95% CI = 1.07-1.30], p = 0.14) and arthritis (OR = 1.45[95% CI = 1.05-2.01], p = 0.025) were risk factors of poor sleep quality.CONCLUSIONS: Poor sleep quality is highly prevalent among elderly Chinese residents in urban Shanghai. Growing attention and comprehensive countermeasures involving psycho-social and personal activities might alleviate the sleep problem in the elderly.

Abstract:Although the association between poor sleep quality and frailty has been previously reported, the relationship between sleep health and intrinsic capacity (IC) remains largely unknown. We aimed to examine the association between sleep health and IC among older adults. This was a cross-sectional study, and 1268 eligible participants completed a questionnaire collecting information on demographic, socioeconomic, lifestyle, sleep health, and IC. Sleep health was measured by the RU-SATED V2.0 scale. High, moderate, and low levels of IC were defined using the Integrated Care for Older People Screening Tool for Taiwanese. The ordinal logistic regression model estimated the odds ratio and corresponding 95% confidence interval. Low IC was significantly associated with age of 80 years or above, female, currently unmarried, uneducated, currently not working, financially dependent, and having emotional disorders. A one-point increase in sleep health was significantly associated with a 9% reduction in the odds of poor IC. An increase in daytime alertness was related to the greatest reduction in poor IC (aOR, 0.64; 95% CI, 0.52–0.79). In addition, the subitems sleep regularity (aOR, 0.77; 95% CI, 0.60–0.99), sleep timing (aOR, 0.80; 95% CI, 0.65–0.99), and sleep duration (aOR, 0.77; 95% CI, 0.61–0.96) were associated with a reduced OR of poor IC but with marginal statistical significance. Our findings showed that sleep health across multiple dimensions is related to IC, particularly daytime alertness in older adults. We suggest developing interventions to improve sleep health and prevent IC decline, which is crucial in causing poor health outcomes.

Abstract:We aimed to explore the relationship between sleep quality and frailty, and depression as a mediator and its interaction with sleep quality on frailty. This was a cross-sectional study among 936 Chinese community-dwelling adults aged≥60 years. Sleep quality, frailty and depression were measured by the Pittsburgh Sleep Quality Index (PSQI), the Frailty Phenotype and the 5-item Geriatric Depression Scale (GDS-5), respectively. We found that depression mediated the association between poor sleep quality and physical frailty, attenuating the association between poor sleep and physical frailty by 51.9%. Older adults with both poor sleep quality and depression had higher risk of frailty than those with poor sleep quality or depression alone. These results implicate multidisciplinary care for frail older adults with poor sleep quality.

Abstract:Study Objectives: To examine factors associated with self-reported sleep quality and duration among very old adults in China.Design: Cross-sectional analysis of the 2005 wave of the Chinese Longitudinal Healthy Longevity Survey (CLHLS).Setting: In-home interview with older adults in 22 provinces in mainland China.Participants: A total of 15,638 individuals aged 65 and older (5,047 aged 65-79, 3,870 aged 80-89, 3,927 aged 90-99, and 2,794 aged 100 and older, including 6,688 men and 8,950 women).Interventions: N/AMeasurements and Results: Two self-reported sleep questions together with numerous sociodemographic and health status measures were used in this study. Sixty-five per cent of Chinese elders reported good quality of sleep. The average number of self-reported hours of sleep was 7.5 (SD 1.9), with 13.1%, 16.2%, 18.0%, 28.0%, 9.2%, and 15.5% reporting <= 5 5, 6, 7, 8, 9, and >= 10 hours, respectively (weighted). Multivariate analyses showed that male gender, rural residence, Han ethnicity, higher socioeconomic status, and good health conditions were positively associated with good quality of sleep. All other factors being equal, octogenarians, nonagenarians, and centenarians were more likely to have good sleep quality than the young elders aged 65-79. Elders with poorer health status or older age were more likely to have either relatively shorter (<= 6 h) or longer (>= 10 h) sleep duration. Married elders were more likely to have an average duration between these two values. Except for some geographic variations, associations between all other factors and sleep duration were weak compared to the effects of health.Conclusions: Age and health conditions are the two most important factors associated with self-reported sleep quality and duration. Good quality of sleep among long-lived old adults may have some implications for achieving healthy longevity.

Abstract:Abstract Prior research links poor sleep and higher systemic inflammation. Gender differences in both sleep and in inflammatory responses have been observed. We investigated gender differences in the association between levels as well as variability of objective sleep health and inflammatory markers (including stimulated inflammatory markers) among a diverse sample of older adults. Participants free of dementia (N=215, Mage=77.5 years, 64% women; 46% White, 39% Black, 15% Hispanic/other) were from The Einstein Aging Study. Actigraphy (2 weeks) yielded wake after sleep onset (WASO), total night sleep time (TST), and midpoint night sleep. Inflammatory markers included circulating C-reactive protein (CRP) and circulating and lipopolysaccharide (LPS)-stimulated levels of interleukin (IL)-1β, IL-4, IL-6, IL-8, IL-10, and tumor necrosis factor (TNF)-α. Bayesian variability models evaluated associations of inflammatory markers with mean and variability of sleep measures, adjusted for BMI, gender, age, education, race/ethnicity. Circulating inflammatory markers were not associated with sleep measures, except higher TST variability with higher circulating IL-6 among women. For stimulated cytokines, among men, higher mean WASO, TST, and later midpoint were associated with higher stimulated IL-8 and TNF-α. All stimulated cytokines except IL-1β were associated with greater variability in WASO, TST, and sleep timing among men. Among women, the only significant association with stimulated cytokines was between higher mean TST and higher stimulated IL-8. We found gender differences in the associations between sleep and inflammation in older adults, primarily in men and for stimulated (not circulating) cytokines. Interestingly, associations were also more frequently observed for sleep variability than mean levels.
