Abstract:Background: Endometritis is an inflammatory disease of the endometrium that is closely associated with endometrial dysfunction, a higher incidence of recurrent pregnancy loss, and an increased probability of infertility. The disordered communication induced by infections, stress, or surgical injuries may destruct the healthy endometrial milieu, leading to endometritis. SCM-198, an alkaloid active component of Leonurus japonicus Houtt, possesses various biological activities, especially anti-inflammatory effects. However, the molecular mechanism of SCM-198 in preventing endometritis has not been completely elucidated. Methods: In vitro, we evaluated the inflammatory level of LPS-stimulated human endometrial stromal cells (hESCs), macrophages or their coculture after SCM-198 treatment, via RT-PCR, ELISA, Western blot and Flow cytometry. In vivo, endometritis C57BL/6J mice stimulated by LPS were vaginally or intraperitoneally injected with SCM-198 and the uterus tissues were studied compared to untreated mice. Findings: We found that SCM-198 could prevent lipopolysaccharide (LPS)-induced endometrial inflammation in hESCs and a mouse model. SCM-198 directly attenuated LPS-induced inflammation via TLR-4/NF-κB signaling in hESCs. SCM-198 also efficiently decreased CCL2 production by endometrial stromal cells (ESCs), which inhibited the endometrial infiltration of monocytes and promoted local macrophage switching to M2 via a CCL2/CCR2 interaction. Polarized M2 macrophages protected ESCs against inflammatory damage, thereby reinforcing the therapeutic effect of SCM-198. Interpretation: Our study is the first to propose the concept that dysregulated communication between ESCs and macrophages via CCL2/CCR2 axis boosting local macrophage enrichment and proinflammation amplification is a critical mechanism of the occurrence of endometritis. SCM-198 can inhibit CCL2 production in ESCs and interrupt the CCL2-mediated ESC-macrophage interaction loop. It also provided a new entry point for the prevention and treatment of endometritis in clinical practice.
