Abstract:Various behavioral and mental health issues have been reported by space crews for decades, with the overall number of mental health complications expected to be higher than is publicly known. The broad range of mental health complications encountered in space is expected to grow as people venture deeper into space. Issues with privacy, dual relationships, and delayed communications make rendering effective psychological therapy difficult in a spaceflight environment and nearly impossible in deep space. Automated psychotherapy offers a way to provide psychotherapy to astronauts both in deep space and low Earth orbit. Although automated psychotherapy is growing in popularity on Earth, little is known about its efficacy in space. This viewpoint serves to highlight the knowns and unknowns regarding this treatment modality for future deep space missions, and places an emphasis on the need for further research into the applicability and practicality of automated psychotherapy for the spaceflight environment, especially as it relates to long-duration, deep space missions.

Abstract:The psychological well-being of astronauts is becoming just as vital as their physical and technical readiness as space missions extend into deep space. Long-duration missions pose unique challenges, such as isolation, confinement, communication delays, and microgravity, which can significantly affect mental health and cognitive performance. This commentary discusses the need for innovative mental health support systems, including automated psychotherapy, as well as Earth-based training methods like mindfulness and relaxation techniques, to address the psychological demands of space travel. By integrating these approaches into pre-mission preparation and in-flight routines, astronauts can develop self-regulation strategies to manage stress, improve focus, and enhance emotional resilience. Automated psychotherapy available 24-7 provides real-time confidential support when communication with Earth is delayed. As space exploration moves forward, the success of missions will depend not only on technological advancements but also on the development of psychological countermeasures that prioritize mental health alongside physical well-being. This paper emphasizes the importance of continued research and collaboration to refine and test these tools in analog environments, ensuring astronauts are mentally and emotionally prepared for the challenges of space.
Autonomous, Computer-Based Behavioral Health Countermeasure Evaluation at HI-SEAS Mars Analog

Abstract:Introduction: Living in an isolated, confined environment (ICE) can induce conflict, stress, and depression. Computer-based behavioral health countermeasures are appealing for training and treatment in ICEs because they provide confidentiality and do not require communication with the outside environment. We evaluated the Virtual Space Station (VSS), a suite of interactive computer-delivered psychological training and treatment programs, at the Hawaii Space Exploration Analog and Simulation (HI-SEAS) III expedition. Methods: Six subjects (3 male, 3 female) spent 8 mo in group-isolation and used the Conflict, Stress, and Depression modules in the VSS. Survey evaluations, data collected within the program, and postdeployment interviews were collected. Results: This crew dealt with behavioral health issues common to ICEs. The VSS proved to be a valuable resource and was used both as intended, and in unanticipated ways, to help maintain behavioral health. The Conflict and Stress Modules were rated as highly acceptable (1.8 on a 7-point Likert scale). The crew identified a total of 13 stressors and worked on 9 problems through the VSS. Opinions about the modules were highly individualized. Crewmembers identified exercises in the VSS that were applicable and not applicable to their needs. Additional content to improve the program was identified. Discussion: Autonomous, confidential training and treatment for behavioral health issues will need to be a critical component of long duration spaceflight travel. Abstract:INTRODUCTION: Numerous issues in mental health benefit from technological innovation. An example involves the mental health challenges of long-duration spaceflight (such as a Mars mission), including prolonged confinement, microgravity, and different sunlight exposure lengths. Persisting on Earth are global mental health challenges stemming from disease burdens, limited interview-based diagnostic systems, trial-and-error treatment approaches, and suboptimal access. There is potential for cross-pollinating solutions between these seemingly disparate challenges using a range of emerging technologies such as sensors, omics, and big data. In this review, we highlight the bidirectional value of mental health technology transfer aimed to address issues both on Earth and in space.METHODS: We prepared a systematic review of studies pertaining to mental health technological innovation and space medicine.RESULTS: For Earth mental health technologies translatable to long-duration space missions, we cite several example technologies, including device-based psychotherapy and social support, conversational agents aka chatbots, and nutritional and physical activity focused mental health. Space technologies translatable to Earth mental health include remote sensing devices, global navigation satellite systems, satellite communications, chronotherapies, and nutritional advances.DISCUSSION: There is a rich history of space technologies informing Earth technological trends, including general health care on Earth, and vice versa. Abstract:Background: During future long-duration space missions, countermeasures need to be developed to deal with psychosocial issues that might impact negatively on crewmember performance and well-being. Methods: In our recently completed NASA-funded study of 5 U.S. astronauts, 8 Russian cosmonauts, and 42 U.S. and 16 Russian mission control personnel who participated in the Shuttle/Mir program, we evaluated a number of important psychosocial issues such as group tension, cohesion, leadership role, and the displacement of negative emotions from crewmembers to people in mission control and from mission control personnel to management. Results: Based on our findings, which are reviewed, a number of psychosocial countermeasures are suggested to help ameliorate the negative impact of potential psychosocial problems during future manned space missions. Conclusions: Crewmembers should be selected not only to rule out psychopathology but also to select-in for group compatibility and facility in a common language. Training should include briefings and team building related to a number of psychosocial issues and should involve both crewmembers and mission control personnel. During the mission, both experts on the ground and the crewmembers themselves should be alert to potential interpersonal problems, including the displacement of negative emotions from the crew to the ground. Supportive activities should consist of both individual and interpersonal strategies, including an awareness of changing leisure time needs. Finally, attention should be given to postmission readjustment and to supporting the families on Earth.

Abstract:<span><br/>Over the past 60 years, our ability to live and work in space has evolved. From short sojourns in small spacecraft to landing on the moon and residing in an orbiting international space station, we have learned to adapt to an extreme environment and safely return home. Human missions to the Moon, Mars, and exploration of deep space are different. This paper summarizes the challenges of providing medical care, specifically mental health care during long-duration flights. Considerable information about challenges that crews bound for Mars will face is available. Literature regarding this issue is summarized. This manuscript provides a short historical summary of long-duration spaceflight to date; the challenges including limited communication with mission controllers on Earth; and, a summary of the behavioral impacts space flight has had on humans. A look at how the future autonomous systems might support physical and mental health when definitive care is millions of miles away, is also provided. Human spaceflight to Mars or other distant sites will require new approaches to mission preparedness and inflight medical support systems. Exploration class missions will be more autonomous than anything deployed until now. The concepts of telemedicine that have aptly supported crews from the 1960s to the present will no longer be in real-time. While communication between Earth and Mars is possible, it will be characterized by significant time delays. Mars-based crews will need to have systems onboard and on Mars to support all health and performance issues.<br/></span>

Psychiatric issues affecting long duration space missions

Abstract:Background: As we move into the next Millennium, increasing numbers of people will travel into space. Psychological screening methods will be relaxed on near-Earth missions, such as might occur on a space station or a lunar colony. Crewmembers on interplanetary missions such as a trip to Mars will have to deal with psychiatric problems themselves with no possibility of evacuating an affected individual. For these reasons, it is important for support personnel on Earth and crewmembers in space to be knowledgeable about psychiatric difficulties that might occur and their appropriate treatments. Methods: Anecdotal reports from long duration manned space missions and studies from space analog environments on Earth were reviewed for information concerning these issues. Results: Psychiatric problems pertinent to the space environment include adjustment and psychosomatic reactions, asthenia, mood and thought disorders, and post-mission personality changes and family problems. Counter-measures to ameliorate these difficulties involve pre-launch selection and training; monitoring, support, and counseling/psychotherapy during the mission; and post-return debriefings with crew members and their families. Psychoactive medications have been useful during space missions, although unclear pharmacokinetic issues related to microgravity need to be taken into account. Conclusion: Although much is known about psychiatric issues related to long duration manned space travel, more empirical work needs to be done during actual space missions.

Abstract:Reports an error in "Integrating between-session homework in psychotherapy: A systematic review of immediate in-session and intermediate outcomes" by Truls Ryum, Mia Bennion and Nikolaos Kazantzis ( Psychotherapy , Advanced Online Publication, Apr 27, 2023, np). In the article (https://doi.org/10.1037/ pst0000488), the number of included studies in Figure 2 omitted two studies that were added to the literature synthesis. Therefore, the following has been added to the Reports excluded box in Figure 2: A further two studies were added to the research synthesis by author knowledge. All versions of this article have been corrected. (The following abstract of the original article appeared in record 2023-66181-001.) This article reviews the evidence for between-session homework (BSH) in individual psychotherapy. Whereas previous reviews have demonstrated a positive association between-client compliance with BSH and distal treatment outcomes; here, we pay particular attention to therapist behaviors that may promote client engagement with BSH assessed as immediate (in-session) and intermediate (session-to-session) outcomes, and moderators of these effects. For our systematic review, we identified 25 studies with 1,304 clients and 118 therapists, mostly on cognitive behavioral therapy such as exposure-based treatments with depression and anxiety disorders. A box score approach was utilized to summarize findings. Results for immediate outcomes were mixed but neutral. Results for intermediate outcomes were positive. Presenting a convincing rationale, being flexible in collaboratively designing, planning, and reviewing homework tasks in accordance with the clients' goals, aligning BSH with the clients' takeaways from the session and providing a written summary of homework and rationale, are some therapist behaviors that can promote client engagement with BSH. Abstract:Anecdotal reports from space and results from space analogue experiments on Earth have suggested a number of interpersonal issues that may negatively affect crewmember performance and well-being. We examined some of these issues in a questionnaire survey of 54 astronauts and cosmonauts who had flown in space and in a 135-d Mir Space Station simulation study in Moscow. We also conducted a NASA-funded study involving missions to the Mir Space Station, where 5 U.S. astronauts, 8 Russian cosmonauts, and 42 U.S. and 16 Russian mission control subjects completed weekly mood and group climate questionnaires. There were few findings that supported hypothesized changes in tension and group behavior in terms of time on-orbit. Crewmembers reported decreasing leader support in the second half of their mission, and U.S. astronauts gave evidence for a novelty effect in the first few weeks. There was strong support for our hypothesized displacement of tension and negative emotions from crewmembers to mission control personnel and from mission control personnel to management. There were several significant differences in response between Americans vs. Russians and crewmembers vs. mission control personnel. These findings have training countermeasure implications for future on-orbit space missions. During expeditionary type space missions, such as a trip to Mars, additional interpersonal stressors will need to be dealt with. These include increased crew autonomy, more dependence on onboard technical resources, communication delays with the Earth, increased isolation and monotony, and the Earth-out-of-view phenomenon.
Psychosocial issues in space: results from Shuttle/Mir

Abstract:Important psychosocial issues involving tension, cohesion, leader support, and displacement of negative emotions were evaluated in a 4 1/2-year study involving five U.S. and four Russian Shuttle/Mir space missions. Weekly mood and group climate questionnaires were completed by five U.S. astronauts, eight Russian cosmonauts, and 42 U.S. and 16 Russian mission control subjects. There were few findings that supported our hypothesized changes in tension, cohesion, and leader support in crew and ground subjects using various time models, although crewmembers reported decreasing leader support in the 2nd half of the missions, and astronauts showed some evidence of a novelty effect in the first few weeks. There was no evidence suggesting a 3rd quarter effect among crewmembers on any of the 21 subscales evaluated. In contrast, there was strong evidence to support the hypothesized displacement of tension and negative emotions from crewmembers to mission control personnel and from mission control personnel to management. There were several significant differences in response between Americans vs. Russians, crewmembers vs. mission control personnel, and subjects in this study vs. people in comparable groups on Earth. Subject responses before, during, and after the missions were similar, and we did not find evidence for asthenia in space. Critical incidents that were reported generally dealt with events on-board the Mir and interpersonal conflicts, although most of the responses were from a relatively small number of subjects. Our findings have implications for future training and lead to a number of countermeasures.

Abstract:Society today is experiencing a golden age of robotic space exploration and interest in human spaceflight has regained popularity as entities like NASA and the burgeoning private space industry refocus attention on sending humans back to the Moon and into orbit. This is a critical turning point for society, as some look to our future as a possibly spacefaring civilization while others wonder who will be enabled to participate in that space exploration. Historically, Disabled individuals and other minoritized groups have been excluded from space science and technology fields, as well as from participation in astronaut programs. However, human space exploration can be made more inclusive with research and innovation in the area of accessible design. Universal accessible design brings advantages to all individuals operating in an environment, and the ability of Disabled individuals to adapt to environments not suited for them can be leveraged as a strength in spaceflight. In this work, disabled and mixed ability crews performed research on parabolic zero-gravity flights which produce weightlessness, exploring tools and technologies which may mitigate challenges for disabled individuals operating in space-like environments. Here we discuss the experiments performed on our flights, highlighting what types of technologies offer promising solutions for accessible design of space habitats, suits, and tools, and accommodations which can enable future disabled astronauts to operate safely in space. We will highlight universal design solutions that not only provide access to previously excluded researchers but have the potential to improve safety and efficiency for all astronauts, regardless of disability. We will also discuss operational strategies which can be incorporated into training and procedure to leverage the strengths of mixed ability crews in ensuring everyone operates effectively together.

Abstract:The International Space Station (ISS) has continued to evolve from an operational perspective and multiple studies have monitored both stress and the immune system of ISS astronauts. Alterations were ascribed to a potentially synergistic array of factors, including microgravity, radiation, psychological stress, and circadian misalignment. Comparing similar data across 12 years of ISS construction and operations, we report that immunity, stress, and the reactivation of latent herpesviruses have all improved in ISS astronauts. Major physiological improvements seem to have initiated approximately 2012, a period coinciding with improvements onboard ISS including cargo delivery and resupply frequency, personal communication, exercise equipment and protocols, food quality and variety, nutritional supplementation, and schedule management. We conclude that spaceflight associated immune dysregulation has been positively influenced by operational improvements and biomedical countermeasures onboard ISS. Although an operational challenge, agencies should therefore incorporate, within vehicle design limitations, these dietary, operational, and stress-relieving countermeasures into deep space mission planning. Specific countermeasures that have benefited astronauts could serve as a therapy augment for terrestrial acquired immunodeficiency patients.
