Abstract:Objective: Frontal comminuted depressed skull fractures need special attention due to complications and aesthetics. The optimal method of reconstruction and fixation of frontal bone fragments remains a matter of discussion. We explored the advantages of reconstruction of frontal bone with titanium clamps. Patients and Methods: From May 2007 to September 2011, we performed 18 craniotomies with titanium clamps to fix the cranial fragments. On the beginning of craniotomy, single-window craniotomy (n = 11) and 2-window craniotomy (n = 9) were designed. After dural closure and frontobasal reconstruction, these fragments were fixed with titanium clamps. A helical CT scan was obtained after operation and a 3-dimensional technique was performed to evaluate the postoperative results. Results: The CranioFix titanium clamp system was applied in 18 patients. No intraoperative and postoperative complications related to clamps were observed. The number of bone fragments ranged from 2 to 5 (average 3.3 pieces) while the number of clamps ranged from 3 to 8 (average 5.8). The time of refixation ranged from 5 to 17 minutes with an average of 9.4 minutes. Obvious statistical significance (P = 0.015) was found in postoperative CT scan results between single-window group and 2-window group, although it is not statistically different in intraoperative subjective valuation (P > 0.05). In all 18 cases, the alignment of fragments was precise, the fixation rigid, and the aesthetic result satisfying. Conclusions: Reconstruction of the forehead by refixation of bone fragments with titanium clamps in frontal depressed skull fractures is safe and suitable. This technique promises to be a good alternative in the repair of frontal comminuted depressed skull fractures. The simplicity, reliable fixation, and fast handling are its prominent features.

Abstract:Objective: To study the techniques of fractured cranial flap refixation (suture, wire and titanium clamps).Methods: Twenty-four cadaver craniotomy flaps, sawn as fractured flap models, were divided into three groups, and every eight fractured flaps were reattached with sutures, wires and titanium clamps, respectively. Bone flap refixation was timed and measured for offset. For every fixation technique, load-bearing tests were performed on craniotomied skulls by applying an external force until the refixation system failed. The results were compared.Results: The titanium clamp required significantly less time (142 +/- 16 s) to fix than either suture (631 +/- 47 s) or stainless steel wire (1104 +/- 48 s). The titanium clamp also showed significantly smaller offset (average offset: 0.35 +/- 0.07 mm) than either suture (average offset: 1.93 +/- 0.33 mm) or stainless steel wire (average offset: 1.80 +/- 0.42 mm). The titanium clamp (maximal force: 384.06 +/- 24.89 N) was stronger than suture (maximal force 89.43 +/- 13.76 N) and stainless steel wire (maximal force 285.51 +/- 10.46 N).Conclusion: The titanium clamp is a reasonable alternative method of fractured cranial flap refixation with respect to ease of use, time consumption, accuracy and strength. Abstract:Objective: To study the techniques of cranial flap fixation (suture, stainless steel wire, and rivetlike titanium clamp) on biomechanics. Methods: Twenty-four cadaver craniotomy flaps were reattached with either suture, stainless steel wire, or rivetlike titanium clamp. Cranial flap fixation was timed and measured for the offset between cranial flap with craniotomy skull for every fixation technique. Load-bearing tests were performed by applying an external force until the failure of the fixation system. Results: Rivetlike titanium clamp required significantly less time to fix (94 +/- 13 seconds) than either suture (172 +/- 14 seconds) or stainless steel wire (399 +/- 45 seconds). The offset of cranial flap after rivetlike titanium clamp fixation (0.20 +/- 0.12 mm) was significantly smaller than either suture (2.20 +/- 0.24 mm) or stainless steel wire (1.32 +/- 0.34 mm). Rivetlike titanium clamp was also stronger than suture and stainless steel wire in the load-bearing test. Conclusion: Rivetlike titanium clamp is easy to use, requires less time consumption, and shows accuracy and strength.

Abstract:AIM: To investigate the biomechanical properties and practical application of absorbable materials in orbital fracture repair. METHODS: The three-dimensional (3D) model of orbital blowout fractures was reconstructed using Mimics21.0 software. The repair guide plate model for inferior orbital wall fracture was designed using 3-matic13.0 and Geomagic wrap 21.0 software. The finite element model of orbital blowout fracture and absorbable repair plate was established using 3-matic13.0 and ANSYS Workbench 21.0 software. The mechanical response of absorbable plates, with thicknesses of 0.6 and 1.2 mm, was modeled after their placement in the orbit. Two patients with inferior orbital wall fractures volunteered to receive single-layer and double-layer absorbable plates combined with 3D printing technology to facilitate surgical treatment of orbital wall fractures. RESULTS: The finite element models of orbital blowout fracture and absorbable plate were successfully established. Finite element analysis (FEA) showed that when the Young's modulus of the absorbable plate decreases to 3.15 MPa, the repair material with a thickness of 0.6 mm was influenced by the gravitational forces of the orbital contents, resulting in a maximum total deformation of approximately 3.3 mm. Conversely, when the absorbable plate was 1.2 mm thick, the overall maximum total deformation was around 0.4 mm. The half-year follow-up results of the clinical cases confirmed that the absorbable plate with a thickness of 1.2 mm had smaller maximum total deformation and better clinical efficacy. CONCLUSION: The biomechanical analysis observations in this study are largely consistent with the clinical situation. The use of double-layer absorbable plates in conjunction with 3D printing technology is recommended to support surgical treatment of infraorbital wall blowout fractures.

Abstract:The aim of this study was to investigate the clinical outcomes of orbital blowout fracture repair by using the three-dimensional (3D) printing-assisted fabrication of individual titanium mesh. Clinical and radiologic data were analyzed for 12 patients with orbital floor and/or medial wall fractures. Lower eyelid incision was used to expose the fractures. Preoperative computed tomographic data were input into an imaging software to rebuild a 3D orbit and mirror the unaffected side into the affected side to replace the demolished orbit. A resin model of the reshaped orbit was generated and used to develop an individual titanium mesh for repairing the fractured orbital. The surgical results were assessed by value of enophthalmos and a comparison of preoperative and postoperative orbital volume difference. All patients had a successful treatment outcome without any complications. Clinical significant enophthalmos were not observed after treatment, and diplopia were solved within 2 weeks postoperative. No extraocular muscle limitation was observed. Postoperative computed tomography scans demonstrated appropriate positioning of titanium mesh and there was no implant displacement. The postoperative orbital volume and enophthalmos difference between the 2 eyes decreased significantly than preoperative (P< 0.001). Three-dimensional printing-assisted fabrication of individual titanium mesh is appropriate for use in orbital blowout fracture.

Abstract:Introduction: In this study, we attempted to demonstrate the actual process of orbital floor fracture visually and computationally in anatomically reconstructed structures and to investigate them using finite element analysis. Methods: A finite element model of the skull and cervical vertebrae was reconstructed from computed tomography data, and an eyeball surrounded by extraocular adipose was modeled in the orbital cavity. Three-dimensional volume mesh was generated using 173,894 of the 4-node hexahedral solid elements. Results: For the cases where the impactor hit the infraorbital foramen, buckling occurred at the orbital bone as a result of the compressive force, and the von Mises stress exceeded 150 MPa. The range of stress components included inferior orbital rim and orbital floor. For the cases where the impactor hit the eyeball first, the orbital bone experienced less stress and the range of stress components limited in orbital floor. The critical speeds for blowout fracture were 4 m/s and 6 m/s for buckling and hydraulic mechanism. Conclusion: Each mechanism has its own fracture inducing energy and its transmission process, type of force causing the fracture, and fracture pattern. It is possible to determine the mechanism of the fracture based on whether an orbital rim fracture is present.

Abstract:Introduction: Harvesting autogenous calvarial bone grafts can potentially weaken the donor site and make it more susceptible to trauma. Conversely, restoring the integrity of the cranial vault structure can result in better dissipation of traumatic energy. This study proposes to mathematically model traumatic situations to evaluate the effect of cranioplasty on biomechanical properties of the craniofacial skeleton. Methods: Preoperative and 6-month postoperative computed tomography-generated DICOM data were used to extract a tetrahedral volumetric representation of the craniofacial skeleton. These data were then used in finite element solver-simulating traumatic events. Results: Deformational stresses accumulate around defect edges and can serve as seed points for calvarial fractures at much lower energy levels in the preoperative models when compared to the postoperative models. Accumulation of stresses in the orbital roof/medial orbital wall areas was observed, similar to fracture patterns observed in the younger pediatric population Conclusions: Restoring architectural integrity of the craniofacial skeleton results in significant increased resistance to deformational stresses despite structural weakening caused by harvesting cranial bone.

Abstract:Objective: The purpose of this study was to establish the constructive of craniofacial suture in the three-dimensional finite element model of craniofacial complex in child, to analyse the respose precedure of the zygomatic impact, to explore the biomechanics characteristic of the children craniofacial trauma. Methods: A 7-year-old female was adopted for study The complex cranial geometry was measured from a series of two-dimensional CT images. The multy-lay spiral CT scans were transformed with a self-developed preprocessor into a finite element mesh. The craniofacial sutures were constructed through the MSC Patran program. Identical impact and boundary conditions were used for the zygomatic impact simulations. Results: It has been shown that the finite element model(FEM) exhibited fine morphological and mechanical comparability. The higher stresswas showed in the zygomatic regions and atlas occipital articular in 3d FEM. The maximum von Mises stress was found at the zygomatic regions and atlas occipital articular. Conclusion: The concentrations of shear stress and tension stress in the suture and articular would increase the risk of injury in this area. But the conduction of the stress might be weakening in the suture of child skull.

Abstract:The clinical finite element analysis (FEA) application in maxillofacial surgery for mandibular fracture is limited due to the lack of a validated FEA model. Therefore, this study aims to develop a validated FEA model for mandibular fracture treatment, by assessing non-comminuted mandibular fracture fixation. FEA models were created for mandibles with single simple symphysis, parasymphysis, and angle fractures; fixated with 2.0 mm 4-hole titanium miniplates located at three different configurations with clinically known differences in stability, namely: superior border, inferior border, and two plate combinations. The FEA models were validated with series of Synbone polymeric mandible mechanical testing (PMMT) using a mechanical test bench with an identical test set-up. The first outcome was that the current understanding of stable simple mandibular fracture fixation was reproducible in both the FEA and PMMT. Optimal fracture stability was achieved with the two plate combination, followed by superior border, and then inferior border plating. Second, the FEA and the PMMT findings were consistent and comparable (a total displacement difference of 1.13 mm). In conclusion, the FEA and the PMMT outcomes were similar, and hence suitable for simple mandibular fracture treatment analyses. The FEA model can possibly be applied for non-routine complex mandibular fracture management.

Abstract:Finite element analysis (FEA) could efficiently simplify the mechanical behaviors of complex loaded constructions. Condylar base fracture related FEAs have been always published, but it is absent for the therapeutic effects of diverse implanting devices’ locations and materials on osteosynthesis. The current FEA aimed to elucidate the biomechanical effects of unilateral mandibular condylar base fractures with open reduction and internal fixation (ORIF) of different locations and materials. The implanting positions included posterolateral border of ramus (A), anterolateral border along sigmoid notch (B), and lateral border in the middle area (C). Two different materials were presented, titanium and u-HA/PLLA (Poly-L-lactic acid combined with uncalcined and unsintered hydroxyapatite). For titanium, A + B presented lowest values of maximum von Mises stress in plates (198.66 MPa) and screws (237.39 MPa), and lowest value of peak displacement in the whole mandible (260.35 μm), while A + C presented lowest value of maximum von Mises stress in cortical bone surrounding screws (65.96 MPa). For u-HA/PLLA, A + B presented lowest value of maximum von Mises stress in screws (134.15 MPa), and lowest value of peak displacement in the whole mandible (374.43 μm), while A + C presented highest value of maximum von Mises stress in cortical bone surrounding screws (166.41 MPa). The findings suggest that both posterolateral border of mandibular ramus and anterolateral border along sigmoid notch are crucial for ORIF of unilateral mandibular condylar base fractures, whatever titanium or u-HA/PLLA devices. In addition, we speculate that titanium devices of position B could be adjusted backward and downward appropriately, but not for u-HA/PLLA device.

Abstract:Abstract Background The objective of the study was to validate biomechanical characteristics of a 3D-printed, novel-designated fixation plate for treating mandibular angle fracture, and compare it with two commonly used fixation plates by finite element (FE) simulations and experimental testing. Methods A 3D virtual mandible was created from a patient’s CT images as the master model. A custom-designed plate and two commonly used fixation plates were reconstructed onto the master model for FE simulations. Modeling of angle fracture, simulation of muscles of mastication, and defining of boundary conditions were integrated into the theoretical model. Strain levels during different loading conditions were analyzed using a finite element method (FEM). For mechanical test design, samples of the virtual mandible with angle fracture and the custom-designed fixation plates were printed using selective laser sintering (SLS) and selective laser melting (SLM) printing methods. Experimental data were collected from a testing platform with attached strain gauges to the mandible and the plates at different 10 locations during mechanical tests. Simulation of muscle forces and temporomandibular joint conditions were built into the physical models to improve the accuracy of clinical conditions. The experimental vs the theoretical data collected at the 10 locations were compared, and the correlation coefficient was calculated. Results The results show that use of the novel-designated fixation plate has significant mechanical advantages compared to the two commonly used fixation plates. The results of measured strains at each location show a very high correlation between the physical model and the virtual mandible of their biomechanical behaviors under simulated occlusal loading conditions when treating angle fracture of the mandible. Conclusions Based on the results from our study, we validate the accuracy of our computational model which allows us to use it for future clinical applications under more sophisticated biomechanical simulations and testing.

Abstract:Zygomaticomaxillary complex (ZMC) fracture repair is one of the most common surgical procedures performed in craniomaxillofacial trauma management. Miniplates and screws are used to stabilize the fractured bone using small local incisions, however, these procedures are not infrequently associated with hardware-related post-operative complications. The amount of fixation hardware utilized varies depending on the fracture pattern and surgical judgment, with three-point fixation being the conventionally accepted treatment. However, limited experimental testing and clinical studies have suggested that ZMC stabilization may be achieved with less than three-point fixation. In this study, we utilized a previously developed finite element modeling approach that allows for detailed bone and muscle representation to study the mechanical behavior of the fractured craniomaxillofacial skeleton (CMFS) under one, two, or three-point fixation of the ZMC. Results suggest that using a miniplate along the infraorbital rim in three-point fixation increases the amount of strain and load transfer to this region, rather than offloading the bone. Two-point (zygomaticomaxillary and zygomaticofrontal) fixation yielded strain patterns most similar to the intact CMFS. One-point (zygomaticofrontal) fixation resulted in higher tensile and compressive strains in the zygomaticofrontal region and the zygomatic arch, along with a higher tensile strain on the zygomatic body. These modeling results provide biomechanical evidence for the concept of over-engineering in the stabilization of facial fractures. Furthermore, they support previous suggestions that less than three-point fixation of ZMC fractures may be adequate to achieve uneventful healing.

Abstract:BACKGROUND:Thoracolumbar burst fracture is a common injury in clinic. Inner fixation surgery through anterior and posterior approach is the commonly used method to rebuild the stability. The stress distribution has not been reported in literature. OBJECTIVE:To investigate the stress distribution of internal fixation materials in titanium mesh reconstruction screw-rods system fixation and posterior approach screw-rods system fixation for the treatment of L1 segment burst fracture by three-dimensional finite element analysis, and to compare the stability of these two methods. METHODS:One healthy male volunteer was selected to establish the T12-L2 segment three-dimensional finite element model through CT scan and finite element software. The L1 segment burst fracture was simulated on the effective finite element model to establish the anterior approach L1 segment titanium mesh reconstruction combine screw-rods system fixation (model A) and posterior approach short segment screw-rods system fixation (model B), the load was applied on the models in order to observe the stress distribution and compare the stability. RESULTS AND CONCLUSION:A T12-L2 segment three-dimensional finite element model was established. In the loading experiment, the stress of model A was mainly in the titanium mesh area and the stress of model B was mainly concentrated in the screw-rod junction. Both model A and model B could gain valid stability in reconstruction of L1 segment burst fracture, and the former was better than later. Mean stress in T12 plane of model A was less than that of model B, and the difference was statistical y significant (P<0.01). Titanium mesh deposition after anterior approach and screw-rod fracture after posterior approach may relate with local stress concentration. The anterior approach titanium mesh reconstruction combine screw-rods system fixation has the better stability than posterior approach screw-rods system fixation.

Abstract:In the literature, many researchers investigated static loading effects on an implant. However, dynamic loading under impact loading has not been investigated formally using numerical methods. This study aims to evaluate, with 3D finite element analysis (3D FEA), the stress transferred (maximum peak and variation in time) from a dynamic impact force applied to a single implant-supported prosthesis made from different materials. A 3D implant-supported prosthesis model was created on a digital model of a mandible section using CAD and reverse engineering. By setting different mechanical properties, six implant-supported prostheses made from different materials were simulated: metal (MET), metal-ceramic (MCER), metal-composite (MCOM), carbon fiber-composite (FCOM), PEEK-composite (PKCOM), and carbon fiber-ceramic (FCCER). Three-dimensional FEA was conducted to simulate the collision of 8.62 g implant-supported prosthesis models with a rigid plate at a speed of 1 m/s after a displacement of 0.01 mm. The stress peak transferred to the crown, titanium abutment, and cortical bone, and the stress variation in time, were assessed.

Abstract:Data from three surveys of California sheltered care residents and facilities were used to determine which of three models best describes the use of sheltered care by residents and the provision of services to residents between 1973 and 1983. The continuum-of-care model emphasizes a person's movement along a treatment continuum to greater independence. In the residual model, the focus is on providing habitation for a chronic population from the era before deinstitutionalization. The developmental model emphasizes providing long-term care to persons who seek supported living arrangements at a particular point in their life cycle. Results indicated that the developmental model best describes the way residents used sheltered care, while service provision was based on the continuum-of-care model. In light of these findings, it should be determined whether service provision reflects the actual needs of this population.

Computational models have been developed and verified for understanding mandibular fracture fixation, but most of the investigation has been limited to a simplified model, single force and evaluation of plate-only designs. Using finite elements, we analysed the fracture stability with five materials - Nitinol, Magnesium alloys, two types of Titanium alloys (Ti-6Al-4V and Ti-29Nb-13Ta-4.6Zr) and Stainless Steel 316L, for four types of fixation plate designs. The soft tissues such as periodontal ligament, were included and molar clenching tasks were considered for accurately mimicking the physiological mastication cycle. The maximum principal tensile strain in the mandible was found to be decreasing with an increase in stiffness for most of the cases, except for the trapezoid and strut plates. We attribute the differences between trapezoid/strut and double mini/lambda to the presence of an additional screw near the sigmoid notch in the proximal segment in case of double mini and lambda plates. As the mandible reconstructed with double mini plates had the least interfragmentary gap, the double mini plate was deemed as the most suitable design. The results such as a lower von Mises stress in plates (compared to yield strength) indicated that the anchorage provided by titanium alloys (Ti-6Al-4V and TNTZ) is sufficient for load bearing.

Mechanical behaviour of reconstructed defected skull with custom PEEK implant and Titanium fixture plates under dynamic loading conditions using FEM

Abstract:Skull reconstruction using cranial implants is often required for repairing skull defects caused due to trauma, diseases, or malignancy to protect intracranial structures. For relieving Intracranial Pressure (ICP) surgeons restore cranial defects either using natural bones or fabricated custom cranial implants. With the increase in Traumatic Brain Injuries (TBI) and challenges faced by TBI patients to regain normalcy, it is imperative to analyse the mechanical behaviour of skull-implant assemblies under some Head Injury Criteria (HIC). Medical grade materials including Titanium Alloys (Ti-6Al-4V) and Polyether-ether-ketone (PEEK) are used by fabricating Patient-Specific Implants (PSI) manufactured using 3D imaging, modelling and printing techniques. 3D technologies are preferred over conventional manufacturing methods, as they enable fabrication of custom shapes, sizes and properties for these PSI. For an effective attachment of PSI with a defective skull, a stable joint and plate arrangement as fixture plates is necessary at their interface. These fixtures can have variable numbers, design shapes, materials and location arrangements. This paper presents the Finite Element Method/Analysis (FEM/FEA) study of PSI attached to a defected skull for reconstruction, with linear shaped fixture configuration, when subjected to an external dynamic loading at 5 m/s, strain rate of 10s-1 to 243s-1 and ICP of 15mm Hg from three sides of the skull faces. Three different materials as Neoprene (soft), Concrete (medium rigid) and E-Glass (highly rigid) have been used, in the form of a rectangular thin cuboidal wall structure, at an angle of 45° with the skull face. Four linear shaped fixture plates which were simplest to design, were used to attach the PSI-skull assembly, to ensure that weight of the PSI-fixation assembly on the patient remains minimal, overall assembly has symmetrical fixations and efforts required by a surgeon for fitment of these plates remain minimal. Placement of these fixture plates has been optimized to encompass the complete PSI-skull interface section, due to which the stresses within all the assembly components (PSI, fixture plate and skull) reduced by nearly 2.5 times than the initial design and remained within yielding limits, thereby, averting any failure under heavy external dynamic loading.
Finite Element Analysis of Two- and Three-Dimensional Fixation in Treating Mandibular Symphyseal Fracture Combined with Bilateral Condylar Intracapsular Fractures.

Abstract:The aim of this study was to compare through finite element analysis two- and three-dimensional (2D and 3D) fixation in the treatment of mandibular symphyseal fracture combined with bilateral condylar intracapsular fractures. The authors created 2 fixation models for the above fracture, and analyzed the stress and displacement in the mandible and fixation materials under 3 loading conditions. The von Mises stress of the mandible and plates peaked during lateral occlusion, and was lowest during central occlusion. In all conditions, stresses in the fixation materials did not exceed the yield stress of titanium. The inferior border of the symphyseal fracture segments showed opposing displacements, and the mandible tended to widen in the 2D fixation model. However, the fracture displacement did not exceed 150 mu m for either fixation method. The results suggested that after well reduction and fixation of condylar intracapsular fractures, either 2D or 3D fixation for symphyseal fracture can provide adequately strong fixation. Compared with 2D fixation, 3D fixation has more advantages in controlling the mandibular width and preventing the fixation materials from enduring excessive stress.
