Abstract:OBJECTIVE:To explore radiographic results and clinical effects of posterior atlanto axial vertebra internal fixation in treating instability of occipitocervical.METHODS:The clinical data of 155 patients with instability of occipitocervical treated by posterior atlanto axial vertebra internal fixation were respectively analyzed from September 2005 to January 2011. There were 68 males and 87 females, ranging in age from 6 to 75 years old with an average of 45.6 years old. Of them, 53 cases were fresh odontoid fractures(Aderson type II C), 30 cases were os odontoideum, 20 cases were old odontoid fractures, 18 cases were unstable atlas fractures, 12 cases were atlanto axial rotatory dislocation, 11 cases were atlanto axial dislocation after rheumatoid arthritis, and 11 cases were basilar invagination. Radiographic results were evaluated in terms of atlas pedicle screw fixation, bone healing and bone graft fusion. Clinical effect evaluation included relief of pain in the occipital-cervical region by VAS score and JOA score.RESULTS:Totally 300 screws were set through atlas pedicle screw fixation in 150 patients. Five patients receivde hook fixation. Postoperative CT showed ideal nailing were 275 (91.7%),acceptable nailing were 14 (4.7%) and unacceptable nailing were 11 (3.6%). All patients were followed up, and the duration ranged from 16 to 40 months with an average of 25.4 months. The fresh fractures healed and 140 cases got bone graft fusion. Preoperative VAS and JOA score were respectively improved from (7.2 +/- 1.1), (7.3 +/- 2.4) to (3.2 +/- 1.1), (13.3 +/- 2.4) at the latest follow-up.CONCLUSION:Posterior atlanto axial vertebra internal fixation in treating instability of occipitocervical can effectively recover physiological curvature of cervical, provide mechanical stability, and obtain good clinical effect. For the young patients who require further activity, posterior fixation and non-fusion technology is a good choose, which can avoid bone graft.
The Treatment of Concomitant Odontoid Fracture and Lower Cervical Spine Injuries.

Abstract:STUDY DESIGN:A retrospective study.OBJECTIVE:To investigate the treatment of odontoid fractures combined with lower cervical spinal injury.SUMMARY OF BACKGROUND DATA:Odontoid fractures combined with a lower cervical spinal injury are rarely reported in the literature and their incidence and characteristics are not well known.METHODS:Seventy-six patients with type II shallow type III odontoid fractures were studied retrospectively. Nine odontoid fractures were found combined with lower cervical spinal injuries; the mean age of patients was 56.8 years. For lower cervical spinal injuries, dislocation was found in 4 patients and disruption of the disc and ligament was found in 5 patients. All patients received surgical management for the odontoid fracture and lower cervical spinal injury in the same operative session. Surgical priority was decided based on neurologic complications, segmental instability, and reducibility of the luxated lower cervical spine. For 3 quadriparetic patients, surgical priority was given to their spinal injuries. For the other 6 patients without neurologic deficits, surgical priority was given to the odontoid fractures in 5 patients because of successful reduction of the dislocation or nondislocation in the lower cervical spine; surgical priority was given to an irreducible lower cervical spine dislocation in the remaining patient.RESULTS:After an average follow-up period of 18 months, there were no complications and all patients showed fusion both in the odontoid fracture and the lower cervical spinal injury. Three patients with a neurologic deficit improved by 1 or 2 grades on the ASIA scale.CONCLUSION:In this series, 9 of 76 (12%) of patients with odontoid fractures also had a lower cervical spinal injury. Surgical stabilization was the choice of treatment as it facilitated early rehabilitation and reduced complications. We propose a new algorithm for treatment according to neurologic complications, segmental instability, and reducibility of the luxated lower cervical spine.
Deflective Placement of Anterior Odontoid Cannulated Screw for Treatment of Odontoid Fracture Combined with Atlantoaxial Posterior Dislocation

Abstract:Objective To investigate deflective placement of anterior odontoid cannulated screw for treatment of odontoid fracture combined with atlantoaxial posterior dislocation.Method From August 2002 to December 2011,18 cases of fresh odontoid fracture combined with atlantoaxial posterior dislocation were treated with deflective placement of anterior odontoid cannulated screw in our hospital.They were 10 males and 8 females,aged from 20 to 50 years (mean,41 years).According to the classification of Anderson-D'Alonzon,12 cases were type Ⅱ and 6 type Ⅲ; according to the classification of American Spinal Injury Association (ASIA),3 cases were grade C,10 grade D and 5 grade E.All patients underwent skull traction pre-operation to reduce dislocation.During the operation,the head was bent close to the chest in order to maintain atlantoaxial restoration and odontoid reduction.Because in this posture the jawbone blocked Kirschner wire and made the placement challenging,the anterior odontoid cannulated screw was deflected in all cases.Results In this series,the operation time ranged from 1.0 to 1.5 hours and the blood loss 50 to 100 mL (mean,75 mL).The mean time of follow-up was 15 months (range,12 to 18 months).All cases got bony union 3 to 4 months postoperation without screw failure.According to the ASIA grading for neural function,one case improved from grade C to grade E,one from grade C to grade D,9 cases from grade D to grade E,and no improvement was noted in the other cases.The difference in atlantoaxial shift distance between preoperation and postoperation was statistically significant (P ＜ 0.05).Conclusion In the treatment of odontoid fracture combined with atlantoaxial posterior dislocation,deflective placement of anterior odontoid cannulated screw may preserve the rotation function of the atlas and axis and lead to positive outcomes.
Biomechanical and Clinical Study of Anterior Transarticular Screw Fixation for Treatment of Traumatic Instability of C1-C2 Joint

Abstract:Objective To evaluate the biomechanical stability of C1-C2 after anterior C1-C2 transarticular screw fixation and assess its curative effect clinically. Methods Eight human cadaveric occipitocervical spine specimens which have been harvested freshly were tested in three modes of loading,flexion-extension, lateral bending, and axial rotation, and four configurations including intact, after an odontoid fracture, and after anterior or posterior C1-C2 transarticular screw fixation. All tests were performed in load and torque control. Clinically, 20 patients who had C1 -C2 traumatic instability were instructed to be operated with anterior C1 -C2 transarticular screw fixation technique, planting cancellated granulate bone into the space between muscular tubercle of arias and odontoid bone. Results Both the anterior and posterior C1-C2 transarticular screw fixation techniques limited motion at the C1-C2 articulation at the tested directions and there were no significant differences between the two techniques. In the 20 patients, one with complete cervical cord injury died of pulmonary infection a month later, and the rest 19 were followed up about 7 months to 3 years (mean 18 months). They all had no trauma on vertebral artery and cervical cord during their cervical operations, and had aflantoaxial synostoses. Conclusion The anterior C1-C2 transarticular screw fixation not only is easy to manipulate and provides the same stability as posterior C1-C2 transarticular screw fixation, but has a less risk of injury on vertebral artery and cervical cord.
Surgical Options for Instability of Atlantoaxiai Complex Combined with Odontoid Fracture

Abstract:Objective To investigate 3 operative treatments and their clinical outcomes for instability of atlantoaxial complex combined with odontoid fracture.Methods From January 2002 to December 2011,54 cases of instability of atlantoaxial complex combined with odontoid fracture were treated surgically in our department.They were 39 males and 15 females,with a mean age of 36 years (range,18 to 55 years).Of them,5 fractures were obsolete and 19 fresh.According to the classification of Anderson-D'Alonzon,30 cases were type Ⅱ and 24 type Ⅲ,12 cases had atlas anterior dislocation,12 atlas posterior dislocation and 30 no obvious dislocation.The preoperative neural function by the American Spinal Injury Association (ASIA) system:grade B in 2,grade C in 5,grade D in 16 and grade E in 31 cases.According to the fracture type and individual status,30 patients underwent internal fixation with anterior odontoid cannulated screws (group A),8 patients had internal fixation with anterior C1-C2 transarticular screws (group B)and 16 patients received internal fixation with posterior C1-C2 screw-rods (group C).Results No intraoperative injury to the vertebral artery or the spinal cord occurred.The patients with atlas dislocation obtained fine reduction post-operation.The mean time of follow-up was 16 months (range,6 to 24 months).All the patients got bony union 3 to 4 months after operation except for the 2 cases in group A who showed nonunion 6 months after operation without clinical symptoms.No instrument failure was found at the final follow-up.According to the ASIA grading 6 months after operation,2 cases were rated as grade C,6 as grade D and 46 as grade E.Conclusions The above 3 surgical treatments are all effective for instability of atlantoaxial complex combined with odontoid fracture.The internal fixation with anterior odontoid cannulated screws should be the first option.Only when it is inappropriate,fixation with anterior C1-C2 transarticular screws and fixation with posterior C1-C2 screw-rods can be taken into consideration.
Anterior Internal Fixation with Hollow Screws for Dontoid Process Fractures

Abstract:Objective To study the curative effect of anterior internal fixtion with hollow screws on the odontoid process fractures. Methods Twenty-four patients with odontoid process fractures were treated by anterior internal fixation with hollow screws under the monitor of X-ray. Results Following-up from 6 to 40 months shows that the fractures in all the patient were healed. No complications occurred in all the patients. Conclusion The curative effect of the hollow screws internal fixation, which can reserve the normal motion function between atlas and epistropheus and has low complication rate,on odontiod process fracture is good.
Anterior Minimally Invasive Surgery for Type Ⅱ Odontoid Fracture Complicated with Burst Fracture of Vertebral Body of Axis

Abstract:Objective To explore the surgical feasibility and clinical outcome of anterior minimally invasive surgery for type Ⅱ odontoid fracture complicated with burst fracture of vertebral body of axis.Methods Twenty cases of type Ⅱodontoid fracture complicated with burst fracture of vertebral body of axis received X-ray in anterioposterior,lateral and month-open of cervical vertebra,spiral CT 3D reconstruction,MRI and skull traction before operation.All cases underwent reposition and anterior fascia dentata screw combined with bone plate set under general anesthesia.Results No severe complications were observed such as intraoperative injuries of vertebral artery,nerve root and spinal cord or postoperative deterioration of spinal cord injury.All cases were followed up for 6 to 24 months.The clinical symptom relieved significantly postoperatively.The healing rate of fracture was 100% and all screws were in the proper position,and no loosening or nail broken of instrument occurred.Conclusion The anterior fascia dentata screw combined with bone plate set is safe and effective for odontoid fracture complicated with burst fracture of vertebral body of axis.
Posterior Arthrodesis of C1-C3 for the Stabilization of Multiple Unstable Upper Cervical Fractures with Spinal Cord Compromise

Abstract:Background: Multiple fractures of the atlas and axis are rare. The management of multiple fragment axis fractures and unstable atlas fractures is still challenging for the spinal surgeon. There are no published reports of similar fractures with 3-part fracture of axis associated with an unstable atlas fracture.Case summary: We present a patient with concurrent axis and atlas fractures, which have not been reported. The patient suffered hyperextension injury with neck pain and numbness of the bilateral upper extremity associated with weakness after a 2-m fall. The axis fractures included an odontoid type IIA fracture and traumatic spondylolisthesis of C2-C3. The atlas fracture was unstable. The neurological examination manifested as central canal syndrome, which was due to the hyperextension injury of cervical spine and spondylolisthesis of C2-C3. The patient was diagnosed as multiple unstable upper cervical fractures with spinal cord compromise. We performed posterior arthrodesis of C1-C3. Postoperatively, the patient showed neurological improvement, and C1-C3 had fused at the 3-month follow-up.Conclusion: Posterior arthrodesis of C1-C3 could provide a stable fixation for the 3 parts of axis (an odontoid type IIA fracture and traumatic spondylolisthesis of C2-C3) combined an unstable atlas fracture. Both the patient and the doctor were satisfied with the results of the treatment. So posterior arthrodesis of C1-C3 is a suitable treatment option for the treatment of a concurrent unstable atlas fracture and multiple fractures of the axis.
Motion Preservation in Type II Odontoid Fractures Using Temporary Pedicle Screw Fixation: a Preliminary Study

Abstract:Purpose To assess the feasibility of temporary pedicle screw fixation for motion preservation of type II odontoid fractures unsuitable for anterior screw. Methods Between 2012 and 2013, temporary pedicle screw fixation was performed in 13 patients with type II odontoid fractures unsuitable for anterior screw. The patients were 10 men and 3 women with an average age of 40. The implant was removed after fracture union. Dynamic CT was performed ≥1 month after implant removal to evaluate atlantoaxial rotation and axial neck rotation. Literature data on axial neck rotation were obtained as a historical control. Results Fifty-two pedicle screws were placed successfully with satisfactory fracture reduction achieved in all patients. The average follow-up period was 14.2 months. At a mean of 9 months after the initial operation, fracture union was confirmed and implants were removed. The average total axial neck rotation was 116.5° ± 25.8° (78.2 ± 14.8 % of the historical controls). The average total atlantoaxial rotation was 34.2° ± 22.0° (27.3 ± 13.8 % of total axial neck rotation). Conclusions Temporary pedicle screw fixation is a feasible technique for motion preservation of type II odontoid fractures unsuitable for anterior screw.
Treatment of Atlantoaxial Instability with C1-C2 Posterior Transarticular Screw Fixation

Abstract:OBJECTIVE:To evaluate the technique of C1-C2 transarticular screw fixation for atlantoaxial instability or dislocation.METHODS:Among 14 patients with atlantoaxial instability, 10 were male and 4 patients were female, with an average age of 38.6 years ranging from 17 to 62 years. All patients were treated by internal fixation with transarticular screws.RESULTS:There were 28 screws applied in 14 patients. All patients were followed up. The average follow-up period was 16 months (range, 9 to 35 months). The postoperative JOA score was ranging from 13.8 to 15.8 with the average score of (14.50 +/- 0.66) and the improved rate of (76.12 +/- 4.94)%. No spinal injury and vertical artery injury was found and osseous fusion was completed in all patients.CONCLUSION:The technique of C1-C2 transarticular screw fixation is one of the best treatments for atlantoaxial instability. Without the help of structural bone graft and aided internal fixation, morselized cancellous bone graft can acquire effective osseous fusion.
Posterior Atlantoaxial Transpedicular Screw Fixation of Unstable Atlas Fractures Combined with Rupture of Transverse Ligament

Abstract:Objective To determine the outcome of unstable atlas fractures combined with rupture of transverse ligament treated by posterior atlantoaxial transpedicular screw fixation.Methods A retrospective case series study was made on 17 patients with unstable atlas fractures combined with rupture of transverse ligament treated by posterior atlantoaxial transpedicular screw fixation and fusion from January 2008 to December 2015.There were 13 males and 4 females,with age range of 34-69 years (mean,47.8 years).All atlas fractures were Jefferson fractures (Levine-Edwards type Ⅲ).Classification of transverse ligament rupture was type Ⅰ in 12 patients and type Ⅱ in 5 patients.No patients had neurologic deficit [American spinal injury association (ASIA) classification grade E].Operation time,blood loss,implant failure,bone fusion and visual analogue scale (VAS) were recorded after operation.Results Operation time was 85-120 min (mean,102 min).Blood loss was 90-150 ml (mean,115 ml).All patients were followed up for 10-20 months (mean,17.8 months).At the final follow-up,all patients achieved bone union,with no implant loosening or breakage happened.VAS was improved from preoperative (5.5 ± 1.8) points to (2.4 ± 1.5) points at tbe final follow-up (P ＜ 0.05).ASIA Grade E remained in all patients.Conclusion Posterior atlantoaxial transpedicular screw fixation of unstable atlas fractures combined with rupture of transverse ligament is a safe and effective surgical procedure that is able to restore the atlanto-axial vertebral stability and relieve pain.
Combined anterior odontoid screw fixation and C1-C2 Jefferson fracture anterior fixation: a step-by-step technical note

Abstract:Background Combined triple C1-C2 fixation has been described in previous literature as a safe, effective, and minimally invasive procedure for complex atlas and odontoid fractures that allows for a greater range of motion compared to posterior approaches and atlanto-occipital fusion but is performed scarcely due to the occipital-cervical diastasis resulting from often-fractured C1 joint masses. There is no evidence-based consensus regarding treating complex atlantoaxial fractures, and surgical strategies are only based on clinical experience. Methods We report the combined triple C1-C2 fixation technique with manual reduction of the joint masses during patient positioning on the operating table, thus allowing effective stabilization in a single surgical time. We describe our experience in the management of a 75-year-old patient presenting with an acute complex type II fracture of C1, also involving one lateral mass combined with a type II odontoid fracture and occipital-cervical diastasis. Results We provide a step-by-step guide to combined triple C1-C2 anterior fixation with manual fracture reduction and describe the clinical case of an acute complex type II fracture of C1 also involving one lateral mass, combined with a type II odontoid fracture and occipital-cervical diastasis. Conclusion The combined triple C1-C2 fixation represents a safe and efficient minimally invasive anterior approach for complex type II fractures of C1 with type II odontoid fractures. Manual reduction of the joint masses during patient positioning allows effective stabilization in a single surgical time.

Diagnosis and Treatment of Odontoid Fracture in Children

Abstract:Objective To explore the clinical characteristics and treatment strategies of odontoid fractures in children.Methods A total of 7 pediatric patients with odontoid fractures were analyzed retrospectively.There were 5 males and 2 females with an average age of 5.6 (3-12) years.The causes were traffic accident (n =2) and fall (n =5).All of them belonged to type Ⅱ fracture according to the Anderson-DAlonzo classification.And the factures were synchondrosis (n =5) and real (n =2).Two patients had neurologic involvements of unilateral extremity paresthesia (n =1) and high paraplegia (n =1).Five patients underwent closed reduction and Minerva cast immobilization.One senior boy had atlantoaxial fixation and fusion while another patient with unilateral extremity paresthesia temporary atlantoaxial fixation.Results The patient with high paraplegia achieved fair reduction after Minerva cast immobilization,but died 1 month later from severe pulmonary infection.The reminder was followed up for a median period of 34(7-84) months.Complete fracture union was achieved.The median healing time was 2.6 (2-4) months.Four patients with Minerva cast immobilization regained excellent cervical motion.The cervical rotation was merely 30 for patients of atlantoaxial fixation and fusion and 65 for those of temporary atlantoaxial fixation.The patient with unilateral extremity paresthesia achieved complete neurologic recovery at 3 months post-operation.Conclusions Most odontoid fractures in children are synchondrosis type.Closed reduction and Minerva cast immobilization is an effective first option for pediatric odontoid fractures.Open reduction and atlantoaxial fixation results in cervical rotation limitation.And it may be used for some select patients.Internal fixation should removed after fracture union.
Efficacy And Safety Of Different Treatments For Grauer Type Ii Odontoid Fractures: A Preliminary Study

Abstract:The purpose of this study was to investigate the clinical efficacy of different treatment methods for Grauer type II odontoid fractures (OF). Forty-two cases of type II OF that received treatment in our hospital from 2006 to 2013 were retrospectively analyzed. There were six cases of type II A, 26 of type II B, and 10 of type II C. The six cases of type II A OF first underwent skull traction, followed by the application of a Halo vest; 19 cases of type II B received anterior odontoid stabilization (group A); the other seven cases of type II B patients with obvious fracture displacement and 10 cases of type II C patients underwent posterior atlantoaxial stabilization (group B). The 42 patients were followed up for 12-24 months, and all patients' fractures healed. The screw used in the atlantoaxial complex was loose after six months in one type II C patient in group B, who received a revision to re-set the screw with iliac bone grafting. In that case, union was achieved in the one-year follow-up, with satisfactory clinical efficacy. Type II A OF should be treated with conservative therapy, type II B OF should be treated with anterior odontoid stabilization, and type II C OF and the II B OF with apparent fracture displacement should be treated with posterior atlantoaxial stabilization. Appropriate treatment plans should be applied according to the different types of OF to obtain satisfactory results.

Abstract:This study aims to investigate the feasibility and efficacy of anterior atlantoaxial motion preservation fixation (AMPF) in treating axis complex fractures involving the odontoid process fracture and Hangman's fractures with C2/3 instability. A retrospective study was conducted on eight patients who underwent AMPF for axis complex fractures at the General Hospital of Central Theater Command from February 2004 to October 2021. The types of axis injuries, reasons for injuries, surgery time, intraoperative blood loss, spinal cord injury classification (American Spinal Injury Association, ASIA), as well as complications and technical notes, were documented. This study included eight cases of type II Hangman's fracture, five cases of type II and three cases of type III odontoid process fracture. Five patients experienced traffic accidents, while three patients experienced falling injuries. All patients underwent AMPF surgery with an average intraoperative blood loss of 288.75 mL and a duration of 174.5 min. Two patients experienced dysphagia 1 month after surgery. The patients were followed up for an average of 15.63 months. One case improved from C to E in terms of neurological condition, three cases improved from D to E, and four cases remained at E. Bony fusion and Atlantoaxial Motion Preservation were successfully achieved for all eight patients. AMPF is a feasible and effective way for simultaneous odontoid process fracture and Hangman's fractures with C2/3 instability, while preserving atlantoaxial movement.

Abstract:Objective:To summarize the treatment results of new odontoid fractures and evaluate three treatment options for new odontoid fractures according to different fracture types.Method:All 54 cases with new odontoid fractures were admitted to our hospital from Jan 2000 to Dec 2007.According to Grauer modified Anderson-D′Alonzo′s typing:7 cases with type ⅡA,23 cases with type ⅡB,8 cases with type ⅡC,16 cases with type Ⅲ.4 cases with cervical cord injuries(3 ⅡB,1 ⅡC) .The type ⅡA cases,2 type ⅡB cases incorporated with adjacent vertebrae fracture,6 type ⅡC cases and type Ⅲ cases were managed with Halo-vest.16 type ⅡB cases were treated with odontiod screw fixation.5 type ⅡB cases with severe fracture displacement and 2 type ⅡC cases were managed with posterior atlantoaxial screw-plate fusion.All cases were followed up over 24 weeks,the lateral and open mouth radiograph and CT scan of cervical vertebra were performed for viewing the union of the fractures.Result:Of 31 cases with Halo-vast treatment,30 cases showed union of fracture at 12 weeks,but 4 cases with primitive anterior displacement of fractures had malunion because of re-displacement after installation of the fixtor.1 cases with type ⅡC underwent posterior atlantoaxial screwplate fusion after 2 weeks because the fracture displaced anteriorly.Among 16 type ⅡB cases treated with anterior odontiod screw fixation,there were 3 cases developed nonunion at 24 weeks.7 cases with posterior atlantoaxial screw-plate fusion obtained bony fusion at 12 weeks.4 cases with spinal cord injuries had recovery of neurological function to some extent.Conclusion:Halo-vest is suitable to type ⅡA,non-displaced ⅡC and Ⅲ odontoid fractures,but the strength is not enough for fixation of the fractures with primitive anterior displacement.Reducible type ⅡB fracture can be treated successfully by anterior odontoid screw fixation.However,posterior atlantoaxial screw-plate fusion is better choice for type ⅡB comminuted fractures with severe displacement.
Efficacy of anterior screw fixation for typeⅡodontoid fracture assisted by intraoperative 3-dimension computer navigation system

Abstract:Background: The odontoid fracture is the most common cervical fracture in the elderly population. At present, there is still controverse regarding the treatment of odontoid fractures, especially type Ⅱ odontoid fractures. The healing rate and complications of this fracture are quite different. Objective: To summarize the clinical results of anterior cannulated odontoid screw fixation for the type Ⅱ odontoid fracture assisted by navigation system. Methods: From January 2010 to Au-gust 2017, clinical data of 32 patients with type Ⅱ odontoid fracture treated surgically in our hospital were reviewed. Among them there were 21 males and 11 females, aged from 21 to 56 years old, mean age (39.1±12.4) years old. All patients com-plained of neck pain and limited cervical movement due to traffic accident (18 patients), falling (10 patients) and hit (4 pa-tients). All patients received the cannulated odontoid screw fixation assisted by the real-time navigation system. Results: No intraoperative vertebral artery injury or spinal cord injury were noted. All 32 patients followed up for an average of (9.6± 3.2) months (range, 3-16 months). 29 patients got bony union and 3 patients obtained fibrous healing. Cervical movement of patients recovered to normal function. Conclusions: The application of computer assisted navigation in anterior odontoid screw fixation improves the accuracy and safety of the operation, with good bone union and clinical effect.
A Technique for the In Vivo Study of Three-dimensional Cervical Segmental Motion Characteristics After Anterior Screw Fixation for Odontoid Process Fractures

Abstract:Study design: Kinematics of the cervical spine was investigated using cone beam computed tomography (CBCT) images combined with three dimensions to three dimensions (3D-3D) registration technology in patients after anterior odontoid screw fixation (AOSF) surgery. Objective: The aim of this study was to investigate in vivo 3D cervical motion characteristics of patients who had undergone AOSF surgeries. Summary of background data: AOSF surgery is a classic surgical method for the treatment of odontoid fracture, but there are few studies that investigated its effect on in vivo biomechanics of the cervical spine. Postoperative biomechanical characters of the atlantoaxial joint (C1-C2) and the caudal adjacent segment (C2-C3) have yet to be clarified. Methods: The study involved 14 patients subjected to a procedure of AOSF with lag screw. Subjects were matched with 14 healthy controls. All subjects underwent CBCT scanning of the cervical spine under seven functional positions. A 3D-3D registration was performed for each vertebra at each functional position to calculate the segmental motion characteristics. The ranges of motion (ROMs) of the C1-C2, C2-C3, and the overall cervical spine (C1-C7) for each of the functional positions were determined. Results: The ROMs of the AOSF group were significantly (P < 0.05) smaller than the control group in flexion-extension positions for the C1-C2 (7.0°vs.11.0°), C2-C3 (3.7°vs.6.7°) and C1-C7 (43.3°vs.54.4°). The twisting ROM of the C1-C2 was 39.3° in the AOSF group and 65.7° in the control (P < 0.05), the bending ROM of the C2-C3 was 2.8° in the AOSF group and 8.9° in the control (P < 0.05). The twisting ROM of C1-C7 segment was 63.2° for the AOSF and 98.1° for the healthy control groups (P < 0.05). Conclusion: Although AOSF surgery reduced the flexion-extension ROMs of all investigated spinal segments, additionally, it reduced twisting ROMs of C1-C2 and C1-C7, but only lateral bending ROM of C2-C3, when compared with the control group. The data implied that the AOSF surgery would result in different biomechanics changes in the atlantoaxial segment and caudal adjacent segment. Longer-term follow-up studies of larger patient cohorts are necessary to evaluate the clinical outcomes of patients after the AOSF surgery.Level of Evidence: 3.
C1-2 Segmental Fixation Without Anterior Screw for the Motion-Preservative Treatment of Type II Dens Injuries

Abstract:Objective To observe the preliminary clinical results of temporary atlantoaxial segmental fixation for motion preservation of type Ⅱ dens fractures not amenable to anterior cancellous screw fixation.Methods Between 2012 and 2013,thirteen patients with type Ⅱ dens fractures not amenable to anterior screw fixation were enrolled and treated using temporary C1-2 pedicle screw fixation without fusion,including 10 males and 3 females,aging 18-62 years old (mean,40 years).The internal fixations were removed after the consolidation of the fractures was radiologically confirmed.Functional CT scan was carried out to evaluate residual atlantoaxial rotation and axial neck rotation at least one month after the implant removal.Results All of 52 pedicle screws were successfully placed and satisfactory fracture reduction was achieved and maintained by internal fixation in all of the 13 patients.No major complication was noted.The average follow-up time was 14.2 months (7 to 20 months).The fusion time after the surgery was 6 to 15 months,with an average of 9.0±2.8 months.At 9 months after initial surgery,the consolidation of the fractures was confirmed with no failure of fixation,and then the internal fixations were removed.The neck pain was released significantly,from preoperative VAS of 6 to 9 points (average,7.5± 1.0) to postoperative 1 to 3 points with an average of 2.3±0.6.The mean axial neck rotation was 59.7° to the left and 56.8° to the right.The mean atlantoaxial rotation was 17.0° to the left and 17.2° to the right.Total axial neck rotation was a mean of 116.5° and total atlantoaxial rotation was 34.2° on average.Atlantoaxial rotation took up about 27.3％ of total axial neck rotation.In comparison to age and gender matched normal individuals,total axial neck rotation was reduced to about 78.2％.Conclusion As an alternative to fusion,posterior reduction and temporary segmental fixation could be a feasible technique for the motion-preservative treatment of type Ⅱ dens fractures not amenable to anterior screw.
Posterior Atlantoaxial Fixation of Osteoporotic Odontoid Fracture: Biomechanical Analysis of the Magerl versus Harms Techniques in a Cadaver Model.

Abstract:Background context Odontoid fractures are among the most common cervical spine fractures in the elderly and are associated with increased morbidity and mortality. Clinical evidence suggests improved survival and quality of life after operative intervention compared to nonoperative treatment. Purpose This study seeks to examine the stability of an osteoporotic Type II odontoid fracture following posterior atlantoaxial fixation with either the Magerl transarticular fixation technique or the Harms C1 lateral mass screws C2 pedicle screw rod fixation. Study design Biomechanical cadaveric study Methods Eighteen cadaveric specimens extending from the cephalus to C7 were used in this study. Reflective marker arrays were attached to C1 and C2 and a single marker on the dens to measure movement of each during loading with C2-C3 and occiput-C1 being allowed to move freely. A biomechanical testing protocol imparted moments in flexion-extension, axial rotation, and lateral bending while a motion capture system recorded the motions of C1, C2 and the dens. The spines were instrumented with either the Harms fixation (n=9) or Magerl fixation (n=9) techniques, and a simulated Type II odontoid fracture was created. Motions of each instrumented spine were recorded for all moments, and then again after the instrumentations was removed to model the injured, non-instrumented state. Results Both Harms and Magerl posterior C1-C2 fixation allowed for C1, C2, and the dens to move as a relative unit. Without fixation the dens motion was coupled with C1. No significant differences were found in X, Y, Z translation motion of the dens, C1 or C2 during neutral zone motions between the Magerl and Harms fixation techniques. There were no significant differences found in Euler angle motion between the two techniques in either flexion-extension, axial rotation, or lateral bending motion. Conclusions Our findings suggest that both Harms and Magerl fixation can significantly reduce dens motion in Type II odontoid fractures in an osteoporotic cadaveric bone model. Clinical significance Both Harms and Magerl posterior atlantoaxial fixation techniques allowed for C1, C2, and the dens to move as a relative unit following odontoid fracture, establishing more anatomic stability to the upper cervical spine.
