Abstract:Treatment of progressive vitiligo aims to halt the spread of the disease and facilitate repigmentation of lesions. JAK inhibitor has been shown some therapeutic effects on vitiligo, but the data for the progressive vitiligo are limited. This retrospective study was performed to evaluate the efficacy and safety of oral tofacitinib in 25 refractory progressive vitiligo patients who experienced failure to previous steroid treatments. 16 (64%) of the 25 patients noted stopping disease progression, and nearly half of the 16 cases halted progression within one month. 10 (40%) patients had repigmentation in varying degrees. Combination with phototherapy was a key factor affecting the rate of repigmentation. Oral tofacitinib might be a potentially effective treatment for intractable progressive vitiligo. The limitations of the study include the retrospective, single-centre study with small sample size and lack of comparison with other systemic therapies.

Although various traditional treatments exist for vitiligo, their effectiveness varies considerably. This report presents a unique case of a vitiligo patient with concomitant systemic lupus erythematosus. Remarkably, after a 30-day course of treatment with tofacitinib, complete repigmentation of the white macular rash was achieved, and there were no adverse drug reactions. These findings provide compelling evidence for the efficacy and safety of oral JAK inhibitors, such as tofacitinib, in vitiligo treatment. Additionally, JAK inhibitors can yet be regarded as a promising new treatment option for vitiligo patients with concurrent autoimmune diseases. Keywords: vitiligo, systemic lupus erythematosus, tofacitinib, JAK inhibitors Vitiligo is a common skin depigmentation disorder that significantly impacts patients' appearance and quality of life. With a prevalence rate of 0.5–2%, 1 vitiligo is easy to diagnose and difficult to treat. Current understanding suggests that vitiligo is an autoimmune disease characterized by the dysregulation of the adaptive immune response. 2 Increasing studies have shown that CD8+ T cells and the cytokines they secrete, notably interferon-γ (IFN-γ), by activating the JAK-STAT pathway, appear to play a pivotal role in the autoimmunity of vitiligo. 3 Tofacitinib, an oral Janus kinase (JAK) inhibitor, demonstrates the ability to inhibit lymphocyte-mediated damage to melanocytes by blocking the JAK-STAT signaling pathway. 3 Based on this mechanism of action, we hypothesize that JAK inhibitors hold promise as targeted therapeutic agents for vitiligo. We report a case of vitiligo combined with SLE, who quickly re-pigmented after treatment with tofacitinib and experienced no adverse reactions. It is reported as follows. A 56-year-old female with stably controlled SLE was diagnosed 10 years prior and currently taking 5 mg/d oral prednisone. She followed up at the rheumatology clinic every six months. One month prior, a white macular rash appeared on the patient's forehead with a rapidly expanding area. Dermatological examination revealed a white patch on the forehead measuring approximately 2.5cm×1.5 cm with clear borders (Figure 1). Reflectance confocal microscopy (RCM) revealed complete depigmentation of the basal layer of the epidermis (Figure 2). The final diagnosis was vitiligo. Relevant imaging and laboratory tests were performed, including frontal and lateral chest radiographs, electrocardiography, complete blood count, liver function tests, kidney function tests, plasma lipid levels, D-dimer, erythrocyte sedimentation rate, rheumatoid factor, antinuclear antibodies, anti-dsDNA antibodies, complements, thyroid antibody, immunoglobulins, antibodies to various infectious diseases such as syphilis, hepatitis B, hepatitis C, HIV, tumor markers such as CEA, CA199, AFP, and PPD test; no significant abnormalities were observed. Considering that the patient is currently using glucocorticoids to treat SLE, and she refused to add the dose of glucocorticoid to treat vitiligo. After obtaining the patient's written informed consent, the patient was given 10 mg/d oral tofacitinib, without the combination of phototherapy and topical medication. After 30 days, the white macular rash was completely re-pigmented (Figure 3), the pigment granules in the basal layer of the epidermis returned to normal on RCM (Figure 4), and no significant abnormalities were found on imaging and laboratory examination, including frontal and lateral chest radiographs, complete blood count, liver function tests, kidney function tests, plasma lipid levels, D-dimer, hepatitis B, and PPD test. Abstract:Abstract Topical tacrolimus 0.1% is a widely accepted treatment for localized vitiligo. However, emerging Janus kinase inhibitors like tofacitinib show promise. Existing evidence for topical tofacitinib largely comes from open-label, uncontrolled studies and case reports, often in combination with other therapies. This prospective, 16-week, single-blinded, split-body study evaluated the efficacy and tolerability of topical tofacitinib 2% ointment vs. topical tacrolimus 0.1% ointment applied twice daily in a cohort of 25 patients with limited vitiligo. Participants aged ≥ 5 years with a diagnosis of nonsegmental vitiligo, limited to ≤ 10% body surface area, and at least two symmetrical vitiligo patches that had been active in last 12 months were included. After informed consent was obtained, vitiliginous patches on either side of the body were randomized to receive either topical tofacitinib or topical tacrolimus. The total duration of therapy was 16 weeks, and patients were followed up every 4 weeks. The primary outcome was Vitiligo Noticeability Scale (VNS) of two symmetrical target patches at week 16 as reported by patients, with a VNS of 4 (‘a lot less noticeable’) or 5 (‘no longer noticeable’) considered as treatment success. Secondary outcomes included clinician-reported percentage repigmentation, time to attainment of treatment success and incidence of adverse effects. The study cohort, comprising 25 patients analysed for 50 vitiligo patches, had a mean age of 22.4 years (range 5–59). The topical tofacitinib group achieved a VNS of 4 or 5 (considered treatment success) in 14 patients (56%), compared with 11 (44%) in the tacrolimus group (P = 0.40). Clinicians observed significant repigmentation exceeding 75% in 15 patients within the tofacitinib group and 13 patients within the tacrolimus group (P = 0.57). The mean (SD) time to attain treatment success was 12.4 (3.8) weeks for tacrolimus and 10.0 (4.4) weeks for tofacitinib. Strikingly, tofacitinib demonstrated treatment success as early as 4 weeks in three patients, whereas no such rapid success was observed in the tacrolimus group. Adverse effects, predominantly mild, included transient erythema (n = 1) and burning sensations (n = 5) with tacrolimus, while tofacitinib led to application-site pain and pruritus in one patient each (P = 0.12). Limitations of our study include the small sample size, short treatment duration and single-blinded study design. In summation, this study unveils the efficacy of both topical tofacitinib and tacrolimus in repigmenting vitiligo lesions. Tofacitinib exhibits a higher, though statistically nonsignificant, treatment success rate and a swifter onset of repigmentation, indicating its potential as a robust therapeutic option for limited vitiligo.

Abstract:Background: Vitiligo is a chronic autoimmune condition characterized by depigmented patches on the skin, presenting significant treatment challenges. Recent studies have focused on the immunomodulatory effects of tofacitinib, a Janus kinase inhibitor, and the anti-inflammatory properties of low-dose oral steroids, yet comparative data on their efficacy and safety are scarce. Objective: The aim of this study was to compare the efficacy and safety of tofacitinib tablets with low-dose oral steroids in the treatment of vitiligo over a six-month period. Methods: This descriptive comparative study involved 42 patients with vitiligo, recruited from the Department of Dermatology at the Combined Military Hospital Abbottabad between November 15, 2022, and November 15, 2023. Participants were randomly assigned to receive either tofacitinib tablets (Group A) or low-dose oral steroids (Group B), with adherence monitored through routine follow-ups. The primary outcome measured was the proportion of repigmentation, assessed using standardized photography and Image software, alongside lesion size reduction and the incidence of adverse events. Statistical analyses were performed using SPSS version 25, with p-values ≤0.05 considered significant. Results: Both groups demonstrated comparable efficacy in terms of repigmentation and lesion size reduction, with no significant difference observed between the tofacitinib (55.8% ± 15.2 repigmentation; 100% lesion size reduction) and low-dose steroid groups (50.2% ± 13.5 repigmentation; 85.71% lesion size reduction) at the 6-month follow-up. The incidence of adverse events was similarly low and statistically non-significant between the two groups. Conclusion: Tofacitinib and low-dose oral steroids are both effective and safe for the treatment of vitiligo, offering valuable options for patient management. Further research with larger sample sizes and longer follow-up is necessary to confirm these findings and evaluate the long-term outcomes of these treatments.
A case of new-onset vitiligo in a patient on tofacitinib and brief review of paradoxical presentations with other novel targeted therapies

Abstract:With recent advancements in the understanding of vitiligo pathogenesis, Janus kinase (JAK) inhibitors have emerged as a promising new treatment modality, but their effects remain incompletely elucidated. Tofacitinib, an oral JAK 1/3 inhibitor approved for the treatment of rheumatoid arthritis, has previously been shown to induce significant re-pigmentation in vitiligo. However, as with other novel targeted therapies, cutaneous adverse effects have been observed. We report a 36-year-old woman with a history of rheumatoid arthritis, refractory to multiple pharmacotherapies, who was initiated on tofacitinib and subsequently developed progressive depigmented patches consistent with new-onset vitiligo. Although definitive causation cannot be established in this case without additional studies, it is important to note that many targeted therapies have the potential to induce paradoxical effects, that is, the occurrence or exacerbation of pathologic conditions that have been shown to respond to these medications. Paradoxical findings with other targeted therapies include the occurrence of melanoma during treatment with BRAF inhibitors, keratoacanthomas with PD-1 inhibitors, vitiligo and psoriasis with TNF-alpha inhibitors, and hidradenitis suppurativa with various biologic agents. Although JAK inhibitors hold therapeutic promise in the treatment of inflammatory skin disorders, further research is warranted to more fully comprehend their effects.

Abstract:BACKGROUND: Tofacitinib is an oral, small molecule Janus kinase inhibitor for the treatment of ulcerative colitis. We present final data from OCTAVE Open, an open-label, long-term extension study.AIMS: The primary objective of OCTAVE Open was to assess the safety and tolerability of long-term tofacitinib in patients with ulcerative colitis; evaluating efficacy was a secondary objective.METHODS: Eligible patients included OCTAVE Induction 1&2 non-responders and OCTAVE Sustain completers/treatment failures. Patients in remission at OCTAVE Open baseline received tofacitinib 5 mg b.d.; all others received 10 mg b.d. Incidence rates (unique patients with events/100 patient-years) for adverse events of special interest were calculated; ≤7.0 years of observation. Efficacy endpoints derived from Mayo score were reported ≤36 months (last scheduled endoscopy visit).RESULTS: In OCTAVE Open, 769 of 944 patients (81.5%) initially received tofacitinib 10 mg b.d. Among all patients (2440.8 patient-years of exposure), incidence rates (IRs; 95% confidence intervals) for deaths and adverse events of special interest were: deaths, 0.25 (0.09-0.54); serious infections, 1.61 (1.14-2.20); herpes zoster (non-serious and serious), 3.16 (2.47-3.97); opportunistic infections, 0.87 (0.54-1.33); major adverse cardiovascular events, 0.16 (0.04-0.42); malignancies (excluding non-melanoma skin cancer), 1.03 (0.67-1.52); non-melanoma skin cancer, 0.75 (0.45-1.19); deep vein thrombosis, 0.04 (0.00-0.23); pulmonary embolism, 0.21 (0.07-0.48). At Month 36, 66.9% and 40.3% showed clinical response, 64.6% and 37.1% had endoscopic improvement, and 58.9% and 33.7% maintained or achieved remission, with tofacitinib 5 and 10 mg b.d. respectively.CONCLUSION: Tofacitinib demonstrated consistent safety up to 7.0 years. Data collected up to Month 36 support long-term efficacy beyond the 52-week maintenance study.

Abstract:Abstract Background Tofacitinib is an oral non-selective Janus Kinase inhibitor approved for Ulcerative colitis (UC) after the failure of biological therapy. Tofacitinib as first line therapy in biologic naive patients has not been evaluated. Methods We conducted a prospective study to assess the safety and efficacy of Tofacitinib as first line therapy in biologic naive moderate to severe UC. Tofacitinib was given at a dose of 10mg BD for 8 weeks followed by de-escalation to 5mg BD maintenance in responders. Dose escalation was done for relapses. Demographics, disease characteristics, concomitant medication, adverse events, severity and time to relapse were recorded. Clinical response and remission (defined as partial Mayo score: PMS decrease ≥2 and PMS ≤1 respectively) were measured at 4 weeks, 8 weeks, 24 weeks and 52 weeks. Endoscopic response (UCEIS score decrease ≥2) and remission (UCEIS 0-1) were assessed at 1 year. Time to event analysis was done to evaluate the cumulative rate of clinical response. Results 176 patients (136 biologic naive, 58% male; median age 40y [IQR:31-48y]) were included. Median baseline PMS and UCEIS score were 5 (IQR: 5-7 and 4-6 respectively) (Table 1). Of the biologic naïve cohort, clinical response was achieved in 68.4% and 79.4% at 4 and 8 weeks respectively. Clinical remission was achieved in 45.6%(4-weeks) and 55% (8-weeks). Maintenance of remission was seen in 44.1% and 32.4% in 24 and 52 weeks respectively (Fig1A). There was a significant reduction in PMS from baseline to end of 8-weeks (p<0.001) (Fig 1B). Corticosteroid-free clinical remission was achieved in 64.8% at 24-weeks. 106 patients (77.9%) maintained response till last follow-up. 11 relapsed after dose de-escalation (median time of 5m [range 3-22m]. 7/11 responded to dose escalation to 10mg. Endoscopic response was noted in 62.5% and remission in 33.9% with significant reduction in UCEIS from baseline to 52-weeks(p<0.001) (Fig 1B). Severe and minor adverse events were noted in 4 and 3 patients respectively (Table 1). Overall 1/3 of patients discontinued therapy at 24-weeks (14%) and 52 weeks (10%). No significant difference in proportion of clinical or endoscopic remission was observed between the biologic naïve and those with history of prior biologic usage (n=31, median time from withdrawal of biologics to Tofacitnib initiation=9 months). Conclusion Tofacitinib was effective in induction of clinical remission in more than half of biologic naive, moderate to severe UC . Three-fourth of these patients continued to be in remission at one year with few serious adverse events. Tofacitinib can be considered as upfront oral therapy after failure of conventional management.

Methods Patients receiving tofacitinib 5 or 10 mg twice daily [BID] from completed phase [P]2/3 placebo-controlled studies, an open-label, long-term extension study [final data cut-off: August 24, 2020], and interim analysis of a P3b/4 study (interim data cut-off: February 20, 2020; Overall plus P3b/4 [2020] Cohort) were included. Proportions with adverse events [AEs] and serious AEs, and incidence rates [IRs; unique patients with events/100 patient-years] for deaths and AEs of special interest [AESI] were evaluated. Opportunistic infections, malignancies, major adverse cardiovascular events [MACE] and gastrointestinal perforations were adjudicated. Results In total, 1157 patients received one or more dose of tofacitinib (mean duration: 946.9 days); 955/1157 [83%] received a predominant dose of 10 mg BID; 412/1157 [35.6%] received tofacitinib for >4 years; 992/1157 [85.7%] had AEs, 244/1157 [21.1%] had serious AEs and 134/1157 (11.6%) discontinued use due to AEs. IRs [95% confidence intervals] for all tofacitinib doses were: deaths, 0.23 [0.09–0.46]; serious infections, 1.69 [1.26–2.21]; herpes zoster [non-serious and serious], 3.30 [2.67–4.04]; opportunistic infections, 1.03 [0.70–1.46]; malignancies (excluding non-melanoma skin cancer [NMSC]), 0.84 [0.55–1.24]; NMSC, 0.73 [0.45–1.10]; MACE, 0.29 [0.13–0.55]; deep vein thrombosis, 0.03 [0.00–0.18]; pulmonary embolism, 0.19 [0.07–0.42]; gastrointestinal perforations, 0.10 [0.02–0.28]. Conclusions AESI IRs were stable to 7.8 years and generally <2.0 in the Overall plus P3b/4 [2020] Cohort, with the exception of herpes zoster [a known risk of tofacitinib treatment]. Temperature Gradients Between Arterial Blood and Brain in the Monkey.∗

Abstract:Summary Temperature measurements were made simultaneously in the arterial blood and various brain structures in 5 chronically prepared rhesus monkeys at rest in a lighted, sound-attenuated, thermoregulated chamber. The level of alertness of the animals was also noted by observing changes in overt behavior together with EEG and EMG recordings. Spontaneous and induced shifts in the level of arousal were followed in seconds by warming or cooling of the arterial blood and seconds to minutes later by parallel shifts in the temperature of the majority of the brain sites. Arterial blood temperature was thus found to yield a sensitive index of the behavioral state of the monkey in relation to sleep and arousal. Changes in the mean temperature of the arterial blood going to the brain appeared primarily responsible for temperature fluctuations in the brain. A number of steadily maintained but differing thermal gradients were found to exist between regional brain sites and the arterial blood.

Abstract:Abstract Background TOFAST is an observational, prospective, multicenter cohort study aiming to describe the real-life clinical benefit of tofacitinib in moderate-to-severe Ulcerative Colitis (UC). Methods We report the results of the interim analysis after 6 months of follow-up, describing patient's characteristics, effectiveness, including change in daily PRO2 scores [RB and stool frequency (SF)] during the first 14 days and the median time to relief, and safety. Clinical response was defined as a decrease in partial Mayo score (PMS) (≥ 3 points and ≥30%) from inclusion with a concomitant decrease in rectal bleeding (RB) subscore ≥ 1 point (absolute subscore of 0 or 1). Clinical remission was defined as PMS ≤2 with no subscore >1. Symptomatic relief was reported by patients as the time in days from initiation of tofacitinib to improvement of their symptoms. Safety has been been reported as the % of Adverse Events (AE), serious AE and AE of special interest. Results 88 patients were included in the safety analysis (one patient was excluded from the effectiveness analysis as he did not meet the eligibility criteria) with a median exposure duration of 11.7 [Q1; Q3: 6.0; 22.1] months. Of these patients, 67 completed a 6-month follow-up visit by 15/07/2023 and were included in the effectiveness analysis. At inclusion 59.8% of patients were male with a mean (± SD) age of 37.5 ±13.2 years, a median disease duration of 5.9 years [Q1; Q3: 2.7; 13.1]. 46.5% of patients had pancolitis. The mean total Mayo Score was 8.1 ±2.4. 94.3% of patients had previously received at least one anti-TNF agent, 67.8% vedolizumab and 35.6% ustekinumab. Tofacitinib was initiated at 10 mg b.i.d, in combination with corticosteroids for 35.6% of patients. At 6 months, 53 (77.9%) patients were still treated with tofacitinib, 58.5% at 10 mg BID dose. Corticosteroids were ongoing in 22.9% of patients. Clinical response and remission were observed in 64.2% and 61.2% of patients, respectively. For the 68 (78.2%), patients who reported improvement the median time to relief was 7 days [Q1; Q3: 3.5 ;14.5]. PRO2 scores normalization during the first 14 days is shown in the figure. 84.1% of patients reported at least one adverse event (AE) and 21.6% reported at least one serious AE. Seventeen (19.3%) infections and 3 (3.4%) cases of herpes zoster were observed as well as one non-invasive low-grade urothelial carcinoma. No major cardiovascular or venous thromboembolic event, nor malignancy or death were observed (Table). Conclusion In this real-life cohort of patients with refractory UC, tofacitinib induced a clinical response and remission in over 60% of patients, with symptoms relief observed as early as the first two weeks. AEs were consistent with the known tofacitinib safety profile.

Abstract:Abstract Background Up to 10% of ulcerative colitis patients and an ileal-pouch anal anastomosis will develop chronic or antibiotic refractory pouchitis. Several case reports and -series investigated the effect of tofacitinib for refractory pouchitis, but reported conflicting results(1-5). In this study, we aimed to prospectively evaluate the efficacy of tofacitinib in chronic or antibiotic refractory pouchitis using clinical and objective outcome measures. Methods We conducted a prospective single centre pilot study in which thirteen patients with chronic or antibiotic refractory pouchitis (Pouchitis Disease Activity Index (PDAI) ≥ 7, ≥2 episodes in the preceding year for which antibiotic therapy was started, or ≥4 weeks symptoms requiring maintenance therapy) were treated with tofacitinib 10mg BID for eight weeks. Clinical, biochemical, endoscopic and histologic disease activity was assessed at baseline and week 8. The primary endpoint was the proportion of patients reaching clinical remission, defined as a PDAI < 7 and a reduction of ≥ 3 points from baseline at week 8. Pouch endoscopy videos were scored by two gastroenterologists and pouch biopsies were assessed by an IBD pathologist, who were blinded to the time points and clinical outcomes. Medians and interquartile ranges were calculated, and Wilcoxon signed rank test was used to analyse changes from baseline. Results Thirteen patients (61% male, median age 34 years) were included(table 1). Three patients did not complete eight weeks of treatment due to adverse events (pyelonephritis, primo EBV infection, worsening of pouchitis), two of whom underwent an early termination endoscopy. In total, 31% achieved clinical remission (4/13) and 62% showed clinical response (8/13). Both the total PDAI score (11 (IQR 9 – 12.75) vs 8 (4.5 – 9.75), p = 0.033), and the clinical PDAI subscore (4 (3 – 4) vs 2 (0.25 – 3.75), p = 0.014) decreased significantly from baseline compared to week 8 or early withdrawal, respectively (figure 1). We did not observe a significant change in endoscopic PDAI subscore (4.5 (2 – 5.75) vs 3 (1 – 4.75), p = 0.120) or histologic subscore (3 (2 – 4) vs 2 (1 – 4), p = 0.214). Biochemical parameters did not decrease significantly compared to baseline (faecal calprotectin: 389 mg/kg (38 – 1222) vs 116 mg/kg (31 – 466), p = 0.530 (figure 2) and CRP: 1,2 mg/L (0.75 – 15.2) vs 0.95 mg/L (0.58 – 17.7), p = 0.307, (figure 3)). Conclusion In this pilot study, a clinical remission rate of 31% was achieved in patients with chronic pouchitis after eight weeks of treatment with tofacitinib. However, treatment duration of eight weeks is presumably too short to achieve biochemical, endoscopic or histologic response in these refractory patients.

Abstract:Lichen planus (LP) is an inflammatory disease that affects the skin, hair, nails and mucous membranes. Erosive LP is a chronic and difficult-to-treat subtype of lichen planus, characterized by lesions on mucosal surfaces, particularly in the oral and genital areas. The prevalence of erosive LP has not been determined. To date, treatment has consisted of surgical intervention, photodynamic therapy, laser therapy, and systemic or topical drugs, including steroids and immunomodulatory agents. LP usually need longer periods of treatment and are known as precancerous lesions with a 0.4% to 12% conversion rate. In addition, nearly 25% of patients who develop erosive LP of the vulva are resistant to topical corticosteroids, which are the first choice of treatment. This study reports 6 cases with a mean age of 3.33 years, who were diagnosed with erosive LP lesions and previously failed in treatment with local, intralesional, and systemic steroids, and hydroxychloroquine. These patients were then treated with 10 mg of tofacitinib per day. Interestingly, with the new treatment, the patients' mean overall satisfaction score was 9.16 out of 10 (range: 8-10), the mean pain relief score was 9.16 out of 10 (range: 9-10) and patients' symptom improvement also began an average of 1.33 months after starting treatment (range: 1-2.5 months).

Abstract:Abstract Background Tofacitinib is a rapidly acting Janus kinase (JAK) inhibitor with several case series reporting effectiveness in patients with acute severe ulcerative colitis (ASUC). However, there remains a paucity of prospective data evaluating its efficacy and rapidity of onset in ASUC. Methods The TRIUMPH study is a phase 4 prospective interventional trial of tofacitinib in ASUC conducted in five hospitals across Canada (Clinicaltrials.gov: NCT04925973). Patients with ASUC (biologic naïve and experienced) refractory to three days of intravenous corticosteroids (Modified Truelove-Witts Severity Index [MTWSI] > 10 despite steroids) were eligible for enrollment. Patients were treated with tofacitinib 10mg bid and assessed daily while in hospital. The primary outcome was day 7 clinical response (MTWSI reduction of > 3 from baseline and < 10). Secondary outcomes included rapidity of clinical and biomarker improvements over the first 7 days, colectomy over the course of one year, and corticosteroid-free clinical remission (total Mayo score < 2), endoscopic improvement (endoscopic Mayo score 0 or 1), and adverse events at 3, 6, and 12 months. Results This is an interim analysis as the study has been fully recruited with six-month follow-up available for all participants. 24 patients with ASUC were recruited and received tofacitinib 10mg bid. The mean total baseline Mayo score was 10.1 (SD 1.4) and all patients had a baseline Mayo endoscopic subscore of 2 (25%, 6/24) or 3 (75%, 18/24). One third of the patients (8/24) had previous anti-TNF failure. Day 7 clinical response was achieved in 58.3% (14/24) patients. The mean number of days to achieve clinical response was 2.4 days (SD 1.3). Marked reduction in C-reactive protein was observed in responders as soon as one day after tofacitinib initiation compared to non-responders (Figure 1). Colectomy occurred in 16.7% (4/24) patients by day 7 and 25% (6/24) by six months. At six months, 45.8% (11/24) patients remained on tofacitinib (including 78.6% (11/14) of day 7 responders), with 33.3% (8/24) having achieved endoscopic improvement and corticosteroid-free clinical remission. A total of six patients reported adverse events, one of which was considered severe (stroke at day 3 after initiation of tofacitinib). Conclusion Tofacitinib may be an effective induction strategy in patients hospitalized with steroid-refractory ASUC. Randomized controlled trials are needed to compare JAK inhibitors with infliximab for steroid refractory ASUC.

Abstract:Abstract Introduction/Background Vitiligo is a chronic autoimmune disease characterized by melanocyte destruction, leading to skin depigmentation. Limited data are available regarding the efficacy of long-term topical vitiligo treatment. In 2 randomized, double-blinded, vehicle-controlled phase 3 studies in adults and adolescents (aged ≥12 y) with nonsegmental vitiligo (TRuE-V1 [NCT04052425]; TRuE-V2 [NCT04057573]), ruxolitinib cream application resulted in statistically superior improvements in repigmentation versus vehicle in the primary and all key secondary endpoints at Week 24 and was well tolerated. In the open-label period of TRuE-V1/TRuE-V2 (Weeks 24–52) and the TRuE-V long-term extension (LTE; Weeks 52–104) study (NCT04530344), further improvements in facial and body repigmentation (as assessed by Vitiligo Area Scoring Index [VASI] responses) were observed through Week 104 among patients who continued to apply ruxolitinib cream. Objectives In this pooled analysis, we evaluated shifts in facial and total body VASI (F-VASI/T-VASI) responses among patients with vitiligo who had limited or no repigmentation at Week 24 and who continued to apply ruxolitinib cream for an additional 80 weeks. Methods In TRuE-V1/TRuE-V2, patients were randomized 2:1 to apply twice-daily 1.5% ruxolitinib cream or vehicle for 24 weeks, after which all patients could apply 1.5% ruxolitinib cream through Week 52. Patients who completed TRuE-V1/TRuE-V2 were eligible to enroll in the TRuE-V LTE. Patients who did not achieve ≥90% improvement in F-VASI at Week 52 continued to apply open-label 1.5% ruxolitinib cream until Week 104. Patients initially randomized to apply ruxolitinib cream who had <25% improvement from baseline in F-VASI or T-VASI at Week 24 and had non-missing VASI assessments at the evaluated time points were included in this analysis. Shifts in F-VASI and T-VASI were assessed among patients with no facial/body repigmentation or worsening depigmentation (≤0% improvement in F-VASI/T-VASI) and patients with limited facial/body repigmentation (>0%–<25% improvement in F-VASI/T-VASI) at Week 24. Results Among patients with no facial repigmentation at Week 24, improvements in F-VASI at Weeks 52 and 104 were observed in 77.8% (49/63) and 97.1% (34/35) of patients, respectively. Among patients with limited facial repigmentation at Week 24, F-VASI improvements at Weeks 52 and 104 were observed in 64.0% (32/50) and 83.3% (30/36) of patients, respectively. Across both groups, 54.9% (39/71) of patients achieved ≥75% improvement from baseline in F-VASI (F-VASI75) at Week 104. Among those with no body repigmentation at Week 24, T-VASI improvements at Weeks 52 and 104 were observed in 79.6% (39/49) and 93.3% (28/30) of patients, respectively. For those with limited body repigmentation at Week 24, T-VASI improvements at Weeks 52 and 104 were observed in 64.5% (80/124) and 81.6% (62/76) of patients, respectively. Across both groups who had <25% improvement in total body repigmentation at Week 24, 50.0% (53/106) of patients achieved ≥50% improvement in T-VASI (T-VASI50) at Week 104. Conclusions In the TRuE-V studies, ruxolitinib cream application for an additional 80 weeks resulted in improved repigmentation among patients with nonsegmental vitiligo who had limited or no repigmentation at Week 24. These 2-year TRuE-V results highlight the importance of prolonged treatment in patients with vitiligo, even when minimal or no repigmentation is achieved after 6 months of treatment.

Abstract:Abstract Introduction/objective Tofacitinib is an oral Janus kinase inhibitor for the treatment of rheumatoid arthritis (RA). This post-hoc analysis evaluated the effect of temporary discontinuation and reinitiation of tofacitinib on disease control in patients with RA in the vaccine sub-study of the long-term extension (LTE) study ORAL Sequel (NCT00413699). Methods The sub-study of ORAL Sequel was a randomized, parallel-group, open-label study. Patients who received tofacitinib 10 mg twice daily for ≥ 3 months in ORAL Sequel were randomized to receive continuous (tofacitinib monotherapy/with methotrexate) or interrupted (tofacitinib withdrawn for 2 weeks post-randomization then reinitiated as monotherapy/with methotrexate) treatment. Efficacy assessments included ACR20/50/70 response rates, change from baseline (∆) in C-reactive protein (CRP), Health Assessment Questionnaire-Disability Index (HAQ-DI), Disease Activity Score in 28 joints, erythrocyte sedimentation rate (DAS28-4 [ESR]), Clinical Disease Activity Index (CDAI), Patient Global Assessment of arthritis (PtGA), Pain (Visual Analog Scale [VAS]), and Physician Global Assessment of arthritis (PGA). Safety was assessed throughout. Results The sub-study included 99 patients each in the continuous and interrupted treatment groups. ACR20/50 response rates, ∆CRP, ∆HAQ-DI (day 15), ∆DAS28-4 (ESR), ∆CDAI, ∆PtGA, ∆Pain (VAS), and ∆PGA were significantly worse in interrupted vs continuous patients during dose interruption, but were generally similar to pre-interruption/continuous treatment levels 28 days post-reinitiation. A numerically higher proportion of interrupted patients reported adverse events (49.5%) vs continuous patients (35.4%). Conclusions Tofacitinib efficacy can be re-established after temporary withdrawal and reinitiation. The safety profile of patients who temporarily discontinued tofacitinib in the sub-study was consistent with previous tofacitinib LTE studies over 9 years. Abstract:Abstract Background To assess the rates of clinical response at week 8, biochemical response, steroid-free clinical remission, endoscopic healing and safety data at week 26 of tofacitinib in biologic naive patients with moderate-severe ulcerative colitis (UC). Methods This was a retrospective analysis of prospectively collected data extracted from the notes of biologic naïve patients with moderate-severe UC who were treated with tofacitinib in the last 3 years in 5 Greek IBD centers and who had a follow up of at least 26 weeks after initiation of treatment. Tofacitinib was administered at a dose of 10 mg bid for 8 or 16 weeks followed by a maintenance dose of 5mg bid thereafter. Week-8 or -16 clinical response was defined as the reduction in the partial Mayo score of at least 2 points, without concomitant use of any steroid preparation [budesonide, prednis(ol)one, or methylprednisolone]. Steroid free clinical remission at week 26 was defined as a partial Mayo score of ≤1 points without concomitant use of any steroid preparation. Biochemical response at week 26 was defined as any reduction in fecal calprotectin or plasma C-reactive protein concentrations as compared to their baseline levels.Mucosal healing at week 26 was defined as Mayo endoscopic sub-score of ≤1. A safety list of all reported adverse events was completed. Treatment discontinuation due to adverse events was considered as treatment failure, whereas patients needing escalation of the maintenance dose were also considered as non-responders. Results Overall, 41 patients (26 males), aged (mean, SD) 35.2 (12.7) years and disease duration of mean (SD) 6.5 (4.8) years were included. UC was extensive in 22, left-sided in 17, and proctitis in 2 patients. Clinical response was seen in 31 (75.6%) and 34 (82.9%) patients at week 8 and 12 respectively.Biochemical response and steroid free clinical remission at week 26 were seen in 35 (85.4%) patients. Endoscopic healing was seen in 29 (70.7%) patients. Adverse events,which did not lead to treatment discontinuation, occurred in 5 patients (hyperlipidemia n=3, elevated liver enzymesn=1, herpes zoster n=1). Conclusion In this retrospective real-world ongoing cohort study, tofacitinib demonstrated clinical response at week 8 or 16 and steroid free clinical remission at week 26 in more than 75% of biologic naïve patients with moderate-severe UC with a good safety profile.

Topical tacrolimus therapy for vitiligo

Abstract:Objective: To observe the safety and efficacy of topical application of tacrolimus. Methods: A series of 33 patients with vitiligo were treated with a topical immunomodulator, tacrolimus ointment 0.1%, twice daily for 1 to 4 months. They were devoid of any topical or systemic therapy for 2 months prior to inclusion. Results: Forty - nine of 81 (60.49%) patches experienced some repigmentation. Eight patches showed complete repigmentation. A slight pruritus was experienced in 2 patients, and inflammatory papules observed in other 2 patients after application of the ointment. Conclusions: Tacrolimus is proved effective to restore skin color in lesions of vitiligo. Because it does not produce atrophy or other adverse effects, tacrolimus may be very useful for sensitive areas of the skin.

Abstract:Abstract Background Tofacitinib was licensed for use in patients with ulcerative colitis in August 2018. Because it is administered orally, has a rapid onset of action and is relatively inexpensive, it was quickly adopted in real-world practice. Safety warnings linking high-dose tofacitinib use to venous thromboembolic events (VTE) and death followed. Further warnings were issued linking its use to an increased risk of malignancy and adverse cardiovascular events in patients: over 65 years, with a history of diabetes, current or past smokers, and those with a history of coronary vascular disease. The most recent Medicines and Healthcare products Regulatory Agency (MHRA) statement advised against the use of all commercially available JAKi in this high-risk cohort unless a suitable alternative is unavailable (MHRA 2023). We sought to define the number of patients who had, or were, being treated with tofacitinib who were in this high-risk group and the proportion who opted to switch therapy once we had outlined their risk-status and the MHRA advice to them. Methods We conducted a retrospective observational cohort study of 131 patients with UC and IBD-U (61% male [80/131]; median age 43 years [range 16-85]; 46% [60/131] patients had previously had at least one biologic agent) treated with tofacitinib from October 2018 to December 2022 in our centre with at least 3 months follow-up. Patients were categorised as high-risk according to the MHRA statement if they had at least one of the criteria above. Results Overall, about one-third, 31.3% [41/131] patients that we had treated with tofacitinib were classified in the MHRA high-risk group. Rates of treatment discontinuation were similar in the high-risk versus no risk group. In the high-risk group ten patients (24.4% [10/41]) had discontinued treatment due to lack of response (4) or an adverse event (6). The most common adverse events were herpes zoster infections (14.6%), malignancy (3.3%) and infections requiring hospitalisation (3.3%). Two patients died while on treatment (one due to complications following cervical spinal surgery and the other due to ischaemic heart disease). There were no incidences of VTE. We clinically reviewed 29 high risk patients who were being treated with tofacitinib after a median of 2.4 years (range 0.6-4.56) of treatment. 25/29 high-risk patients decided on continuing their treatment in spite of the health care warnings and alternative therapies being available. Conclusion About one-third of our tofacitinib-treated patients were classified as at high-risk. The majority of high-risk patients opted to continue therapy even though alternative therapies were available.
