Towards the Cardiac Equivalent Source Models in Electrocardiography and Magnetocardiography: A Simulation Study

Abstract:The cardiac equivalent source model is very crucial in the electrocardiogram (ECG) and magnetocardiography (MCG) problems. In this study, we presented a thoroughly investigation of the three models: the dipole, equivalent double layer (EDL) and epicardial potential (EP) models in terms of the ECG/MCG forward problem within a unified framework. The cardiac electromagnetic field is calculated using boundary element method (BEM). The numerical performance and the properties of the three source models are studied in detail. Three different resolutions of dipole and two of EP and EDL models are studied. Besides, the effect of the volume conductor model is studied. The simulation results demonstrated that the resolution of the source models shouldn't be too small, otherwise some information about the cardiac activities will be lost as shown in the single dipole model situation. The effect of the resolution on the MCG is larger than that on BSP. The higher resolutions of the source, the larger effect of the volume conductor happened for each source. While for the EDL source, the lung has little effect on the MCG. The EDL source can generate the similar performance compared to the dipole source, and the largest difference happened at the time when the ventricle start or finish the activation. The presented study suggests that the EDL source is a good choice as source model for ECG/MCG problem

Abstract:Clinical translation of computational models of the heart has been hampered by the absence of complete and rigorous technical and clinical validation, as well as benchmarking of the developed tools. To address this issue, a dataset containing the cardiac anatomy and fibre orientations from magnetic resonance images (MRI), as well as epicardial transmembrane potentials from optical mapping acquired on ex-vivo porcine hearts, have previously been made available to the community. Image processing techniques were developed to integrate MRI images with electrical information. Different models were tested and compared with the integrated data1, including: i) a new methodology to customize and regularize heart shape and myocardial fibre orientation to predict activation waves in these personalized meshes with generic conduction parameters; ii) a statistical model-constrained framework to produce maximum a posteriori (MAP) estimation of the 3D distribution of transmembrane potential; iii) a personalized simplified reaction-diffusion 3D electrophysiological model; iv) a personalized fast conduction Purkinje system with a simple Eikonal-based electrophysiological model.
Fractional-order modeling of myocardium structure effects on atrial fibrillation electrograms

Abstract:Atrial fibrillation (AF) is the most common cardiac arrhythmia with mechanisms of initiation and sustaining that are not fully understood. The clinical procedure for AF contemplates the analysis of the atrial electrograms, whose morphology has been correlated with the underlying structure of the atrial myocardium. This study employs a mathematical model incorporating fractional calculus to simulate cardiac electrical conduction, accounting for tissue structural inhomogeneities using complex-valued orders. Simulations of different wavefront propagation patterns were performed, and virtual electrograms were analyzed using an asymmetry factor. Our results evinced that the shapes of the action potential and the propagating wavefront can be modulated through the fractional order under both healthy and AF conditions. Moreover, the asymmetry factor changes with variations in the fractional order. For a given propagation pattern under AF conditions, variation intervals for the asymmetry factor can be generated by forming sets of simulations with different configurations for the fractional order, representing diverse samples of atrial tissue with varying degrees of structural heterogeneity. This approach successfully reproduces the electrogram negative deflection predominance seen in AF patients, which standard integer-order models cannot predict. Our fractional–order conduction model aligns with the effects of atrial structure on the electrical dynamics observed in clinical AF. Therefore, it offers a valuable tool for studying cardiac electrophysiology, encompassing both electrical and structural interactions of the tissue within a unified model.

Abstract:There are many functions which are continuous everywhere but non-differentiable at some or all points such functions are termed as unreachable functions. Graphs representing such unreachable functions are called unreachable graphs. For example, ECG is such an unreachable graph. Classical calculus fails in their characterization as derivatives do not exist at the unreachable points. Such unreachable functions can be characterized by fractional calculus as fractional derivatives exist at those unreachable points where classical derivatives do not exist. Definition of fractional derivatives has been proposed by several mathematicians like Grunwald-Letinikov, Riemann-Liouville, Caputo, and Jumarie to develop the theory of fractional calculus. In this paper, we have used Jumarie type fractional derivative and consequently the phase transition (P.T.) which is the difference between left fractional derivative and right fractional derivatives to characterize those points. A comparative study has been done between normal ECG sample and problematic ECG sample (Right Ventricular Hypertrophy) by the help of the above mentioned mathematical tool.
Exploring a new frontier in cardiac diagnosis: ECG analysis enhanced by machine learning and parametric quartic spline modeling

Abstract:The heart's study holds paramount importance in human physiology, driving valuable research in cardiovascular health. However, assessing Electrocardiogram (ECG) analysis techniques poses challenges due to noise and artifacts in authentic recordings. The advent of machine learning systems for automated diagnosis has heightened the demand for extensive data, yet accessing medical data is hindered by privacy concerns. Consequently, generating artificial ECG signals faithful to real ones is a formidable task in biomedical signal processing. This paper introduces a method for ECG signal modeling using parametric quartic splines and generating a new dataset based on the modeled signals. Additionally, it explores ECG classification using three machine learning techniques facilitated by Orange software, addressing both normal and abnormal sinus rhythms. The classification enables early detection and prediction of heart-related ailments, facilitating timely clinical interventions and improving patient outcomes. The assessment of synthetic signal quality is conducted through power spectrum analysis and cross-correlation analysis, power spectrum analysis of both real and synthetic ECG waves provides a quantitative assessment of their frequency content, aiding in the validation and evaluation of synthetic ECG signal generation techniques. Cross-correlation analysis revealing a robust correlation coefficient of 0.974 and precise alignment with a negligible time lag of 0.000 s between the synthetic and real ECG signals. Overall, the adoption of quartic spline interpolation in ECG modeling enhances the precision, smoothness, and fidelity of signal representation, thereby improving the effectiveness of diagnostic and analytical tasks in cardiology. Three prominent machine learning algorithms, namely Decision Tree, Logistic Regression, and Gradient Boosting, effectively classify the modeled ECG signals with classification accuracies of 0.98620, 0.98965, and 0.99137, respectively. Notably, all models exhibit robust performance, characterized by high AUC values and classification accuracy. While Gradient Boosting and Logistic Regression demonstrate marginally superior performance compared to the Decision Tree model across most metrics, all models showcase commendable efficacy in ECG signal classification. The study underscores the significance of accurate ECG modeling in health sciences and biomedical technology, offering enhanced accuracy and flexibility for improved cardiovascular health understanding and diagnostic tools.
Sub-Nyquist Sampling of ECG Signals with Differentiated VPW Optimization Model

Abstract:In recent years, developments in wearable technologies and wireless communication have enabled wearable ECG monitoring. Long-term ECG monitoring result in large amounts of data. To sub-Nyquist sampling ECG signal, recently developed finite rate of innovation (FRI) technology is a practical option. However, existing FRI sampling schemes generally suffered from large model mismatch errors and insufficient reconstruction accuracy. In this paper, a sub-Nyquist sampling scheme based on the differentiated VPW optimization model is proposed for ECG signals sampling. The original ECG signal is modeled as a linear combination of several differentiated Variable Pulse Width (VPW) pulses and a model mismatch error signal. Then, a two-channel sampling framework is proposed to sample ECG signals with a sub-Nyquist rate. To improve the accuracy of the reconstruction, optimizing the reconstruction is considered. We formulate the objective function of the optimization to minimize the model mismatch error signal. A particle filter-based optimization method is proposed to solve the objective function with a high-dimensional variable. Block coordinate descent (BCD) technique is considered in the proposed optimization method. Finally, we conduct several simulations with real ECG signals from MIT-BIH Arrhythmia Database to test the performance of our proposed scheme. Simulation results have demonstrated the reconstruction accuracy of our proposed scheme outperforms other related schemes.

Abstract:Electrocardiographic imaging (ECGI) can aid in identifying the driving sources that cause and sustain atrial fibrillation (AF). Traditional regularization strategies for addressing the ECGI inverse problem are not currently concerned about the multi-scale analysis of the inverse problem, and these techniques are not clinically reliable. We have previously investigated the solution based on uniform phase mode decomposition (UPEMD-based) to the ECGI inverse problem. Numerous other methods for the time-frequency analysis derived from empirical mode decomposition (EMD-based) have not been applied to the inverse problem in ECGI. By applying many EMD-based solutions to the ECGI inverse problem and evaluating the performance of these solutions, we hope to find a more efficient EMD-based solution to the ECGI inverse problem. In this study, five AF simulation datasets and two real datasets from AF patients derived from a clinical ablation procedure are employed to evaluate the operating efficiency of several EMD-based solutions. The Pearson's correlation coefficient (CC), the relative difference measurement star (RDMS) of the computed epicardial dominant frequency (DF) map and driver probability (DP) map, and the distance (Dis) between the estimated and referenced most probable driving sources are used to evaluate the application of various EMD-based solutions in ECGI. The results show that for DF maps on all simulation datasets, the CC of UPEMD-based and improved UPEMD (IUPEMD)-based techniques are both greater than 0.95 and the CC of the empirical wavelet transform (EWT)-based solution is greater than 0.889, and the RDMS of UPEMD-based and IUPEMD-based approaches is less than 0.3 overall and the RDMS of EWT-based method is less than 0.48, performing better than other EMD-based solutions; for DP maps, the CC of UPEMD-based and IUPEMD-based techniques are close to 0.5, the CC of EWT-based is 0.449, and the CC of the remaining EMD-based techniques on the SAF and CAF is all below 0.1; the RDMS of UPEMD-based and IUPEMD-based are 0.06∼0.9 less than that of other EMD-based methods for all the simulation datasets overall. On two authentic AF datasets, the Dis between the first 10 real and estimated maximum DF positions of UPEMD-based and EWT-based methods are 212∼1440 less than that of others, demonstrating these two EMD-based solutions are superior and are suggested for clinical application in solving the ECGI inverse problem. On all datasets, EWT-based algorithms deconstruct the signal in the shortest time (no more than 0.12s), followed by UPEMD-based solutions (less than 0.81s), showing that these two schemes are more efficient than others.
ECG Classification With Event-Driven Sampling

Abstract:Electrocardiogram (ECG) data's high dimensionality challenges real-time arrhythmia classification. Our approach employs functional approximation to condense ECG recordings into a compact feature set for simpler classification using Chebyshev polynomials. These polynomials, with 200 time points and 80 coefficients, accurately represent arrhythmias in an feature vector. We prove Chebyshev polynomials act as implicit low-pass filters on input signals. Using MIT-BIH Arrhythmia and MIT-BIH Supraventricular Arrhythmia datasets, we introduce classifiers that achieve significant accuracy. A three-layered Artificial Neural Network yields high F1-scores (0.99, 0.90, 0.93, and 0.76 for classes N, S, V, and F) with minimal parameters (20,964), surpassing existing models. Furthermore, our proposed ECG classification system exhibits minimal computational demands, requiring only 0.1 MIPS per beat. We also propose efficient signal reconstruction methods, with the iterative approach showcasing accurate reconstruction with negligible error. This approach accommodates various data sampling types and determines optimal Chebyshev coefficients for capturing signal bandwidth.

Abstract:To prevent lifestyle diseases, portable heartbeat detection systems by electrocardiograms (ECGs) for daily life monitoring have attracted attentions. For portable systems, power consumption is critical and it is thus beneficial to reduce the sampling frequency at the point of acquisition. Recent developed variable pulsewidth finite rate of innovation (VPW-FRI) framework provides an effective way for low-rate sampling and compression of ECG signals. Unfortunately, noise and model mismatch will induce inaccuracies to such scheme in the reconstruction process. In this article, a sub-Nyquist sampling scheme based on the extension of VPW model is proposed for ECG signals, which reduce the model dismatch error, and improve the reconstruction quality under model mismatch and noise scenario. In the proposed scheme, ECG signal is modeled as the differentiated VPW model to reduce the model mismatch error. Then a FRI sampling framework based on the differentiated VPW model is proposed to sample the ECG signal at a sub-Nyquist rate. After the ECG signal is sampled, some consecutive Fourier series coefficients are calculated according to the samples. Then a modified annihilating filter algorithm is proposed to estimate parameters of the ECG signal and reconstruct the ECG signal. Finally, simulation experiments with real ECG signals from Massachusetts Institute of Technology-Beth Israel Hospital (MIT-BIH) Arrhythmia Database have shown that the proposed scheme outperformed the traditional methods for model mismatch. The signal-to-residual ratio (SRR) of the reconstructed signal of the proposed scheme is higher than that of the VPW-FRI scheme.

Towards quantitative precision for ECG analysis: Leveraging state space models, self-supervision and patient metadata

Abstract:Deep learning has emerged as the preferred modeling approach for automatic ECG analysis. In this study, we investigate three elements aimed at improving the quantitative accuracy of such systems. These components consistently enhance performance beyond the existing state-of-the-art, which is predominantly based on convolutional models. Firstly, we explore more expressive architectures by exploiting structured state space models (SSMs). These models have shown promise in capturing long-term dependencies in time series data. By incorporating SSMs into our approach, we not only achieve better performance, but also gain insights into long-standing questions in the field. Specifically, for standard diagnostic tasks, we find no advantage in using higher sampling rates such as 500Hz compared to 100Hz. Similarly, extending the input size of the model beyond 3 seconds does not lead to significant improvements. Secondly, we demonstrate that self-supervised learning using contrastive predictive coding can further improve the performance of SSMs. By leveraging self-supervision, we enable the model to learn more robust and representative features, leading to improved analysis accuracy. Lastly, we depart from synthetic benchmarking scenarios and incorporate basic demographic metadata alongside the ECG signal as input. This inclusion of patient metadata departs from the conventional practice of relying solely on the signal itself. Remarkably, this addition consistently yields positive effects on predictive performance. We firmly believe that all three components should be considered when developing next-generation ECG analysis algorithms.

An Adaptive Particle Weighting Strategy for ECG Denoising Using Marginalized Particle Extended Kalman Filter: An Evaluation in Arrhythmia Contexts

Abstract:Model-based Bayesian frameworks have a common problem in processing electrocardiogram (ECG) signals with sudden morphological changes. This situation often happens in the case of arrhythmias where ECGs do not obey the predefined state models. To solve this problem, in this paper, a model-based Bayesian denoising framework is proposed using marginalized particle-extended Kalman filter (MP-EKF), variational mode decomposition, and a novel fuzzy-based adaptive particle weighting strategy. This strategy helps MP-EKF to perform well even when the morphology of signal does not comply with the predefined dynamic model. In addition, this strategy adapts MP-EKF's behavior to the acquired measurements in different input signal to noise ratios (SNRs). At low input SNRs, this strategy decreases the particles' trust level to the measurements while increasing their trust level to a synthetic ECG constructed with the feature parameters of ECG dynamic model. At high input SNRs, the particles' trust level to the measurements is increased and the trust level to synthetic ECG is decreased. The proposed method was evaluated on MIT-BIH normal sinus rhythm database and compared with EKF/EKS frameworks and previously proposed MP-EKF. It was also evaluated on ECG segments extracted from MIT-BIH arrhythmia database, which contained ventricular and atrial arrhythmia. The results showed that the proposed algorithm had a noticeable superiority over benchmark methods from both SNR improvement and multiscale entropy based weighted distortion (MSEWPRD) viewpoints at low input SNRs.

Abstract:Multi-lead electrocardiogram (ECG) is a fundamental and reliable diagnostic tool for the detection of heart arrhythmias. An increasing number of deep neural network models have been proposed for automatic arrhythmia detection using multi-lead ECGs. However, most of the existing models focus on intra-lead features while neglecting the inter-lead ones and fail to explain the reasons behind decisions due to their black-box nature. To address these challenges, we propose an interpretable multi-lead ECG detection framework that tackles these limitations in two key areas. First, a flexible double-kernel residual block (DKR-block) is proposed to effectively extract inter-lead and intra-lead features. In addition, a new two-dimensional ECG classification model is built based on the DKR-block for accurate arrhythmia detection. Second, a visualization method based on gradient-weighted class activation mapping is used to visualize the indicative features of the model's decisions. The proposed model achieves an AUCmacro of 0.929 on the PTB-XL data, a F1micro of 0.932 on the HFECGIC dataset, and an F1macro of 0.989 on the LUDB dataset. The results show that the proposed model outperforms several state-of-the-art methods, and the visualization features of its classification decision are consistent with the medical background knowledge.

Abstract:BACKGROUND: The interest in using fractional calculus operators has grown in the field of image processing. Image enhancement is one of image processing tools that aims to improve the details of an image. The enhancement of medical images is a challenging task due to the unforeseeable variation in the quality of the captured images.METHODS: In this study, we present a mathematical model based on the class of fractional partial differential equations (FPDEs). The class is formulated by the proportional-Caputo hybrid operator (PCHO). Moreover, some properties of the geometric functions in the unit disk are applied to determine the upper bound solutions for this class of FPDEs. The upper bound solution is indicated in the relations of the general hypergeometric functions. The main advantage of FPDE lies in its capability to enhance the low contrast intensities through the proposed fractional enhanced operator.RESULTS: The proposed image enhancement algorithm is tested against brain and lungs computed tomography (CT) scans datasets of different qualities to show that it is robust and can withstand dramatic variations in quality. The quantitative results of Brisque, Piqe, SSEQ, and SAMGVG were 40.93%, 41.13%, 66.09%, and 31.04%, respectively for brain magnetic resonance imaging (MRI) images and 39.07, 41.33, 30.97, and 159.24 respectively for the CT lungs images. The comparative results show that the proposed image enhancement model achieves the best image quality assessments.CONCLUSIONS: Overall, this model significantly improves the details of the given datasets, and could potentially help the medical staff during the diagnosis process.

Abstract:This study examines the application of fractional calculus in the analysis and modeling of electrical circuits of fractional order, highlighting its potential to explain the behaviour of complex electrical circuits accurately. In the domain of electrical circuits, fractional differential equations are employed in the analysis and simulation of systems that consist of resistors, capacitors and inductors. In the present paper, a novel approach utilizing fractional order modified Taylor wavelets is implemented to solve the fractional model of RL, LC, RC and RLC electrical circuits under generalized Caputo fractional derivative which offers precise and flexible modeling of non-locality and hereditary characteristics in complex systems. Furthermore, an additional parameter (time scale parameter) is incorporated in fractional circuit dynamics to maintain the physical dimensionality. The considered wavelets with the collocation technique offer an efficient solution by converting the fractional model of electrical circuits into a set of algebraic equations which are further solved by using the Newton iteration method. Moreover, this study discusses the significance of Ulam-Hyers stability, emphasizing its role in ensuring stable and reliable circuit performance. The impact of fractional order on the dynamics of the electric circuit model is presented by tables and graphs. The approximate solutions obtained by the proposed method are well comparable with exact solutions and some other existing wavelet-based techniques. The residual errors are also evaluated under various model parameters for fractional orders. Furthermore, the graphs illustrate that the error progressively decreases as the number of wavelets basis increases.

Abstract:Fractional calculus (integral and differential operations of noninteger order) is not often used to model biological systems. Although the basic mathematical ideas were developed long ago by the mathematicians Leibniz (1695), Liouville (1834), Riemann (1892), and others and brought to the attention of the engineering world by Oliver Heaviside in the 1890s, it was not until 1974 that the first book on the topic was published by Oldham and Spanier. Recent monographs and symposia proceedings have highlighted the application of fractional calculus in physics, continuum mechanics, signal processing, and electromagnetics, but with few examples of applications in bioengineering. This is surprising because the methods of fractional calculus, when defined as a Laplace or Fourier convolution product, are suitable for solving many problems in biomedical research. For example, early studies by Cole (1933) and Hodgkin (1946) of the electrical properties of nerve cell membranes and the propagation of electrical signals are well characterized by differential equations of fractional order. The solution involves a generalization of the exponential function to the Mittag-Leffler function, which provides a better fit to the observed cell membrane data. A parallel application of fractional derivatives to viscoelastic materials establishes, in a natural way, hereditary integrals and the power law (Nutting/Scott Blair) stress-strain relationship for modeling biomaterials. In this review, I will introduce the idea of fractional operations by following the original approach of Heaviside, demonstrate the basic operations of fractional calculus on well-behaved functions (step, ramp, pulse, sinusoid) of engineering interest, and give specific examples from electrochemistry, physics, bioengineering, and biophysics. The fractional derivative accurately describes natural phenomena that occur in such common engineering problems as heat transfer, electrode/electrolyte behavior, and sub-threshold nerve propagation. By expanding the range of mathematical operations to include fractional calculus, we can develop new and potentially useful functional relationships for modeling complex biological systems in a direct and rigorous manner.

Abstract:This paper explores the Fourier decomposition method to approximate the decomposition of electrocardiogram (ECG) signals into their component waveforms, such as the QRS-complex and T-wave. We compute expansion coefficients using the Fourier transform and the traditional Fourier transform. Numerical examples are presented, and the analysis focuses on ECG signals as a real-world application, comparing the performance of the and Fourier transforms. Our results demonstrate that the Fourier transform significantly enhances the separation of ECG signal components, such as the QRS-complex and T-wave. This improvement is attributed to a notable reduction in the Gibbs phenomenon introduced by the Fourier-series expansion when using the Fourier transform, as opposed to the traditional Fourier transform.

Abstract:Models in cardiac electrophysiology are coupled systems of reaction diffusion PDE and of ODE. The ODE system displays a very stiff behavior. It is non linear and its upgrade at each time step is a preponderant load in the computational cost. The issue is to develop high order explicit and stable methods to cope with this <a class="link-external link-http" href="http://situation.In" rel="external noopener nofollow">this http URL</a> this article, is is analyzed the resort to exponential Adams Bashforth (EAB) integrators in cardiac electrophysiology. The method is presented in the framework of a general and varying stabilizer, that is well suited in this context. Stability under perturbation (or 0-stability) is proven. It provides a new approach for the convergence analysis of the method. The Dahlquist stability properties of the method is performed. It is presented in a new framework that incorporates the discrepancy between the stabilizer and the system Jacobian matrix. Provided this discrepancy is small enough, the method is shown to be A(alpha)-stable. This result is interesting for an explicit time-stepping method. Numerical experiments are presented for two classes of stiff models in cardiac electrophysiology. They include performances comparisons with several classical methods. The EAB method is observed to be as stable as implicit solvers and cheaper at equal level of accuracy.

Abstract:THIS subject has been little discussed in the recent literature, and now that otologists have had the opportunity of reviewing statistical reports of the end results of fenestration operations, it becomes apparent that they are definitely in need of such a discussion. The more experienced surgeons are agreed that when cases are well selected a large majority of the patients will have their hearing improved to the practical level in consequence of the fenestration operation. The improvement occurs usually within three to six weeks. The percentage of cases in which hearing is so improved varies, according to the reports, from 70 to as high as 90 per cent. These figures are strikingly impressive. Whatever the actual percentage of early good results may be, ultimate failures are of such frequency that the question of revision becomes one of major importance. Abstract:In the realm of artificial intelligence, the generation of realistic training data for supervised learning tasks presents a significant challenge. This is particularly true in the synthesis of electrocardiograms (ECGs), where the objective is to develop a synthetic 12-lead ECG model. The primary complexity of this task stems from accurately modeling the intricate biological and physiological interactions among different ECG leads. Although mathematical process simulators have shed light on these dynamics, effectively incorporating this understanding into generative models is not straightforward. In this work, we introduce an innovative method that employs ordinary differential equations (ODEs) to enhance the fidelity of generating 12-lead ECG data. This approach integrates a system of ODEs that represent cardiac dynamics directly into the generative model's optimization process, allowing for the production of biologically plausible ECG training data that authentically reflects real-world variability and inter-lead dependencies. We conducted an empirical analysis of thousands of ECGs and found that incorporating cardiac simulation insights into the data generation process significantly improves the accuracy of heart abnormality classifiers trained on this synthetic 12-lead ECG data.
