Abstract:A 61-year-old man had experienced an irritating cough for 1 month. He received a diagnosis of lung adenocarcinoma by bronchoscopy. Computed tomography showed a mass in the left hilum and mediastinal lymph node enlargement. After two cycles of neoadjuvant chemotherapy, the patient underwent a robotic assisted atypical sleeve lobectomy (left lower lobe + S4+5). The patient's postoperative course was uneventful, and he was discharged on the tenth postoperative day. This is the first description of the feasibility of robotic extended sleeve lobectomy for a lung cancer patient receiving neoadjuvant chemotherapy.

An assessment of national needs for community physician manpower in Nigeria: staff training needs and programmes.

Abstract:This second part of a national survey was designed to identify the training needs and programmes considered most appropriate by the most senior health management staff of the state governments for the field practice of community medicine in Nigeria. The operation of the current primary health care programme and the field practice of community medicine although constitutionally a local government responsibility, effectively rests on the state governments at present because of economic and political constraints at the LG level. The main needs identified in the study are for a certifiable and appropriate generalist community physician training programme as well as the relative levels of emphasis needed in the different sub-specialty areas. While holders of the current fellowship diploma in public health of the National Postgraduate Medical College of Nigeria are welcome to practice in the field if they are available, many state governments and officials consider the programme ill-suited to the needs of field practice community physicians. Thirteen (68.4% of the) respondents, consider it suitable only for jobs at state employment levels and higher up. Twelve of the 17 physician respondents considered clinical practice essential for all community physicians. Eighteen of the 21 state governments would be ready to sponsor their physician candidates to local training programmes in public health as identified in this study, if different from the current postgraduate fellowship. Maternal and child health was the area of sub-specialty training considered most needed in the states. This was followed by epidemiology, environmental health, health education, health and biostatistics, health management, and occupational health in that order.(ABSTRACT TRUNCATED AT 250 WORDS)
Robotic Sleeve Resection of the Airway: Outcomes and Technical Conduct Using Video Vignettes

Abstract:Background: Our objectives are to report our outcomes and to demonstrate our evolving technique for robotic sleeve resection of the airway, with or without lobectomy, using video vignettes. Methods: We retrospectively reviewed a single-surgeon prospective database from October 2010 to October 2019. Results: Over 9 years, of 5573 operations 1951 were planned for a robotic approach. There were 755 robotic lobectomies and 306 robotic segmentectomies, and 23 consecutive patients were scheduled for elective completely portal, robotic sleeve resection. Sleeve lobectomy was performed in 18 patients: 10 right upper lobe, 6 left upper lobe, and 2 right lower lobe. Two patients had mainstem bronchus resections and 2 underwent right bronchus intermedius resections that preserved the entire lung. One patient had a robotic pneumonectomy. One operation was converted to open thoracotomy because of concern for anastomotic tension in a patient who received neoadjuvant therapy. All patients had an R0 resection. In the last 10 operations we modified our airway anastomosis, using a running self-locking absorbable suture. The median length of hospital stay was 3 days (range, 1-11), with no 30- or 90-day mortalities. Within a median follow-up of 18 months, there were no anastomotic strictures and no recurrent cancers. Conclusions: Our early and midterm results show that a completely portal robotic sleeve resection is safe and oncologically effective. Trhe technical aspects of a robotic sleeve resection of the airway are demonstrated using video vignettes.
Robot-assisted Bronchial Sleeve Left Upper Lobectomy for Locally Advanced Bronchopulmonary Squamous Cell Carcinoma after Neoadjuvant Chemotherapy: a Case Report

Abstract:For patients with central bronchopulmonary malignancies, sleeve lobectomy serves as an alternative to pneumonectomy. In recent years, robotic-assisted technique has been innovatively developed and applied in sleeve lobectomy. Here, we introduce a patient treated with chemotherapy who received robotic bronchial sleeve lobectomy without pulmonary vessel anastomosis in our department. The patient was a 53-year-old male smoker, who had suffered irritable cough for 6 months. During surgery we found that the tumor was tightly adhered to the pulmonary artery. Robotic scissors were used to separate the tumor from the surface of the pulmonary artery. Finally, the tumor was removed along with the relevant bronchus, and the pulmonary artery remained intact. The tumor margins were verified as negative by fast frozen pathology during the procedure. This experience in our department demonstrates the advantages of robotassisted bronchial lobectomy in preserving pulmonary vessels in patients with pulmonary arterial adventitia involvement.

Abstract:Abstract Background Perihilar cholangiocarcinoma is a challenging technique to be performed by minimally invasive approach being the type III among the most complex procedure. Nowadays, the robotic approach is gaining increasing interest among the surgical community, and more and more series describing robotic liver resection have been reported. However, few cases of minimally invasive Bismuth type IIIA cholangiocarcinoma have been reported. Robotic approach allows for a better dissection and suture thanks to the flexible and precise instruments movements, overcoming some of the limitations of the laparoscopic technique. Therefore, robotic technique can facilitate some of the critical steps of a technically demanding procedure, such as the extended right hepatectomy for perihilar cholangiocarcinoma Bismuth IIIA type. Methods In this multimedia video we describe, for the first time in the literature, a full robotic surgical step-by-step technique with some tips and tricks for treating a perihilar cholangiocarcinoma Bismuth IIIA type, performing a radical extended right hemihepatectomy, including segment I combined with regional lymphadenectomy anf left bile duct reconstruction. A 55-year-old woman with obstructive jaundice (10 mg/dl) was referred to our center. The endobiliary brushing confirmed adenocarcinoma, and MRI/CT showed a focal perihilar lesion of 2 cm, including the main biliary duct bifurcation and extending up to the right duct (Bismuth Type IIIA hilar cholangiocarcinoma). After endoscopic biliary stents placement and 6 weeks after right portal vein embolization, the future liver remnant, including segments II and III, reached an enough hypertrophy volume with a ratio of 30%. A right hemihepatectomy with caudate lobe, including standard standard lymphadenectomy and left biliary duct reconstruction was performed. Results The operation lasted 670 min with an estimated blood loss of 350 ml. Postoperative pathological examination revealed a moderately differentiated adenocarcinoma pT1N0 with 15 retrieved nodes and free margins. The patient experienced a type A biliary fistula and was discharged on the 21st postoperative day without abdominal drainage. Conclusions Through the tips and tricks presented in this multimedia article, we show the advantages of the robotic approach for performing correctly one of the most complex surgeries. 1-7

Early Outcomes of Robotic Versus Thoracoscopic Segmentectomy for Early-Stage Lung Cancer: A Multi-Institutional Propensity Score-Matched Analysis

Abstract:Objectives: Anatomical segmentectomy via robotic thoracic surgery and video-assisted thoracic surgery (VATS) are minimally invasive surgical approaches for treatment of early-stage non-small cell lung cancer (NSCLC). However, few research studies have compared early outcomes. Methods: A retrospective analysis was made of 774 patients, 298 who received robotic and 476 who received VATS, who underwent minimally invasive segmentectomy for early-stage NSCLC at 3 academic institutions between June 2015 and August 2019. Perioperative outcomes were compared after propensity score-matching on the basis of age, gender, body mass index, percent forced expiratory volume in I second, smoking status, American Society of Anesthesiologists score, type of segmentectomy, tumor size, and institution. Results: There were 257 patients in each group after propensity score-matching. The baseline characteristics and type of segmentectomy were comparable. Three conversions to thoracotomy occurred in the VATS group, and 1 in the robotic group (P = .624). There was no significant difference in operative time (147.91 +/- 52.42 vs 149.23 +/- 49.66 minutes; P = .773), blood loss (50 mL [interquartile range (IQR), 50-100 mL] vs 100 mL [IQR, 30-100 mL]; P = .177), rates of overall complications (17.9 vs 14.8%; P = .340), and length of stay (4 days [IQR, 3-5 days] vs 4 days [IQR, 3-5 days]; P = .417) between the robotic and VATS groups, respectively. Robotic segmentectomy was more costly ($12,019.30 +/- 1678.30 vs $7834.80 +/- 1291.20; P < .001) because of the amortization and consumables of the robotic system. There were a greater number of N1 lymph nodes and N1 stations in the robotic group. Conclusions: Segmentectomy with robotic and VATS are safe and feasible for early-stage NSCLC treatment. A robotic approach might lead to a better N1 lymph node dissection.
Comparison of Robotic and Video-Assisted Thoracic Surgery for Lung Cancer: a Propensity-Matched Analysis.

Abstract:BACKGROUND:Reports of comparison between robotic and thoracoscopic surgery for lung cancer are limited, we aimed to compare the perioperative outcomes of robotic and thoracoscopic anatomic pulmonary resection for lung cancer.METHODS:A total of 184 patients with lung cancer underwent anatomic pulmonary resection by robotics or thoracoscopy. A propensity-matched analysis with incorporated preoperative variables was used to compare the perioperative outcomes between the two procedures.RESULTS:Overall, 71 patients underwent robotic pulmonary resection, including 64 lobectomies and 7 segmentectomies, while 113 patients underwent thoracoscopic lobectomy and segmentectomy. Propensity match produced 69 pairs. The mean length of postoperative stay (7.6±4.6 vs. 6.4±2.6 d, P=0.078), chest tube duration (5.3±3.7 vs. 4.4±1.7 d, P=0.056), number of lymph nodes retrieved (17.9±6.9 vs. 17.4±7.0, P=0.660), stations of lymph nodes resected (7.4±1.6 vs. 7.6±1.7, P=0.563), operative blood loss (53.9±29.3 vs. 50.3±37.9 mL, P=0.531), morbidity rates (42.0% vs. 30.4%, P=0.157) were similar between the robotics and thoracoscopy. However, robotics was associated with higher cost ($12,067±1,610 vs. $8,328±1,004, P<0.001), and longer operative time (136±40 vs. 111±28 min, P<0.001).CONCLUSIONS:Robotics seems to have higher hospital costs and longer operative time, without superior advantages in morbidity rates and oncologic efficiency. Further prospective randomized clinical trials were needed to validate both of its short- and long-term oncologic efficiency.
A robotic fissureless right upper lobectomy using a posterior approach

Abstract:The fissureless technique in a lobectomy is considered useful to avoid postoperative prolonged air leak when a fissure is fused because it is not dissected. In particular, this technique has been used most frequently in right upper lobectomies because the dense fissure was most frequently found between the right upper and middle lobes. We believe that the surgical steps in this technique should be modified depending on the surgical approach, although the concept that the hilar structures, including the pulmonary vessels and bronchi, are each transected prior to division of a dense fissure is the same. We demonstrate a robotic right upper lobectomy with an explanation of the nuances of its performance. The operating time was 135 minutes with a blood loss of 50 ml. The patient's postoperative course was uneventful. We removed the chest tube on postoperative day 1, and the patient was discharged on postoperative day 3. The final pathology report was pT1bN0M0, stage 1A2, squamous cell carcinoma. These good perioperative results indicate the feasibility of this technique.

Abstract:OBJECTIVES:We summarize our experiences of video-assisted thoracoscopic surgery (VATS) sleeve lobectomy with bronchoplasty for non-small-cell lung cancer and discuss the indications and technical details of the operation.METHODS:From September 2011 to December 2012, 15 patients underwent VATS sleeve lobectomy with bronchoplasty at our institution (right upper lobe 10, right middle and lower lobes 1, left lower lobe 2 and left upper lobe 2), with mediastinal lymphadenectomy. Three incisions were utilized. The utility incision was made at the fourth intercostal space, anterior axillary line. Simple continuous and simple interrupted suturing of the membranous and cartilaginous portions of the bronchus were performed for the anastomosis.RESULTS:All procedures were uneventful, with a median operative time of 165 min, a median bronchial anastomosis time of 44 min and a median blood loss of 150 ml. There were no conversions to thoracotomy. There were 14 squamous carcinomas and 1 adenocarcinoma. All patients recovered well, and 1 experienced a minor complication. The median duration of chest tube drainage was 5.4 days, and the median length of hospital stay was 7 days. All patients were followed postoperatively for a range of 1-16 months without tumour recurrence.CONCLUSIONS:VATS sleeve lobectomy with bronchoplasty is safe and effective. The utility incision placed at the fourth intercostal space, anterior axillary line, is convenient for the anastomosis, and the suturing technique is expeditious and secure. Preserving the azygos vein does not compromise exposure for the anastomosis. This technique is very suitable for centrally located lung cancers <3 cm in diameter, particularly when the cancers are located within the brachial lumen.

Abstract:Over the last 5 years, there has been a tremendous increase in the interest in and use of robotics in thoracic surgery. The focus of this review is on the use of robotics for pulmonary lobectomy, which is being approached with 3 or 4 arm techniques. Early experiences suggest that the learning curve is approximately 20 cases for most surgeons but could be shortened with previous advanced thoracoscopic skills. Robotic lobectomy is feasible and safe in limited reports from experienced centers. Operative and clinical outcomes favor robotic lobectomy over open and appear to be similar to VATS. Limited data on oncologic effectiveness and survival suggest that robotics is similar to VATS. Widespread adoption and integration into practice will require future research efforts to prove oncologic and survival benefits in concert with cost effectiveness evaluation.

Abstract:Minimally invasive thoracic surgery, including video-assisted thoracoscopic surgery and robot-assisted thoracoscopic surgery, has been proven to have an advantage over open thoracotomy with less pain, fewer postoperative complications, faster discharge, and better tolerance among elderly patients. We introduce a uniportal robot-assisted thoracoscopic double-sleeve lobectomy performed on a patient following neoadjuvant immunotherapy. Specialized instruments like customized trocars with a reduced diameter, bulldog clamps, and double-needle sutures were utilized to facilitate the maneuverability through the single incision. This technique integrates the merits of multiport robot-assisted thoracic surgery with uniportal video-assisted thoracoscopic surgery.

Abstract:Objective: Stage III non-small cell lung cancer (NSCLC) is a heterogeneous group of diseases. For this subset of patients, clinical management is still under debate and prognosis remains poor so far. In the present study, we aimed to evaluate the feasibility and safety of robotic-assisted thoracic surgery after neoadjuvant chemoimmunotherapy in stage III NSCLC. Methods: A real-world prospective cohort study was performed in a single-center setting from April 2021 to May 2022. Patients who were diagnosed with resectable or potentially resectable stage IIIA–B NSCLC and received neoadjuvant chemoimmunotherapy followed by robotic-assisted thoracic surgery were enrolled. Pathological response to neoadjuvant chemoimmunotherapy, treatment-related adverse events, and surgical outcomes of these patients were evaluated. Results: A total of 44 patients who underwent robotic-assisted thoracic surgery after three doses of neoadjuvant chemoimmunotherapy were included in this study. Of these, 36 of 44 (81.8%) patients had a major pathological response, and 26 (59.1%) had a pathological complete response based on pathological examination of surgical specimen. Eight patients (18.2%) suffered grade 3 treatment-related adverse events, including neutropenia (n = 4), increased aminotransferases (n = 3), anemia (n = 1), and cutaneous capillary endothelial proliferation (n = 1). Robotic-assisted thoracic surgery was performed subsequently, and R0 resection was achieved in all patients. Only two (4.5%) patients required conversion to thoracotomy. Surgical complications occurred in five (11.4%) patients, including air leak (n = 3), chylothorax (n = 2), and surgical site infection (n = 1). There was no re-surgery or postoperative mortality within 90 days. Conclusion: Robotic-assisted thoracic surgery following neoadjuvant chemoimmunotherapy showed good feasibility and safety in stage III NSCLC. It was not associated with unexpected perioperative morbidity or mortality and may be a promising therapeutic option in stage III NSCLC. These results need further confirmation by more large-scale clinical trials.

Abstract:Background: Rib tumors are typically curable through rib resection, associated with an excellent prognosis. Although transthoracic robotic first rib resection for thoracic outlet syndrome (TOS) has been previously documented, this paper presents our experience and technique in conducting robotic-assisted wire saw resections for high-position rib tumors.Methods: From January 2019 to May 2022, five patients diagnosed with high-position rib tumors underwent robotic-assisted wire saw resections. For our entire portal approach, we employed two 8-mm working ports, a 12-mm camera port, and a 12-mm assistant port. Data regarding the short-term and clinical long-term treatment effects were collected.Results: The median operation time was 124.2 minutes (range, 87-185 minutes), with no observed complications. The average intraoperative blood loss was 185 mL (range, 85-410 mL). Chest tubes were typically removed between 1 and 3 days post-operation. The average hospital stay post-surgery was 2.8 days, with a range of 2-5 days. We observed no relevant intraoperative or postoperative complications. No recurrence was reported during routine follow-ups 12 months post-surgery.Conclusions: Our findings indicate that the technique of robotic-assisted wire saw resection for high -position rib tumors is both feasible and reliable. This provides valuable insights for surgeons to consider robotic-assisted resection for high-position rib tumors.

Abstract:Economical procedures for mapping large numbers of cosmids and YACs, using panels of somatic cell hybrids are of practical importance in genome mapping. We developed a method based on direct PCR of individual cosmid colonies with primers complementary to the cosmid vector and 2 primers complementary to the extreme 5 prime and 3 prime parts of the Alu consensus sequence (1). We also applied this method on samples of YAC DNA, either liquid or in agarose blocks. The cosmid vector is p2CpG, a derivative of pCpG and pKNUNl (2, Dauwerse, unpublished), its primers: ML and RL. The YAC vector is YAC4, its primers: YL5 and YL3. The primers for the ALU repeat ALU5 and ALU3. The primers PDJ33 and PDJ34 suggested by Dr P. de Jong (personal communication) are also used for YACs.
