Abstract:Purpose We aimed to perform the commissioning and clinical evaluation of myQA SRS detector array for patient‐specific quality assurance (PSQA) of stereotactic radiosurgery (SRS)/ stereotactic body radiotherapy (SBRT) plans. Methods To perform the commissioning of myQA SRS, its dose linearity, dose‐rate dependence, angular dependence, and field‐size dependence were investigated. Ten SBRT plans were selected for clinical evaluation: 1) Common clinical deviations based on the original SBRT plan (Plan0), including multileaf collimator (MLC) positioning deviation and treatment positioning deviation were introduced. 2) Compared the performance of the myQA SRS and a high‐resolution EPID dosimetry system in PSQA measurement for the SBRT plans. Evaluation parameters include gamma passing rate (GPR) and distance‐to‐agreement (DTA) pass rate (DPR). Results The dose linearity, angle dependence, and field‐size dependence of myQA SRS system exhibit excellent performance. The myQA SRS is highly sensitive in the detection of MLC deviations. The GPR of (3%/1 mm) decreases from 90.4% of the original plan to 72.7%/62.9% with an MLC outward/inward deviation of 3 mm. Additionally, when the setup error deviates by 1 mm in the X, Y, and Z directions with the GPR of (3%/1 mm) decreasing by an average of −20.9%, −25.7%, and −24.7%, respectively, and DPR (1 mm) decreasing by an average of −33.7%, −32.9%, and −29.8%. Additionally, the myQA SRS has a slightly higher GPR than EPID for PSQA, However, the difference is not statistically significant with the GPR of (3%/1 mm) of (average 90.4%% vs. 90.1%, p = 0.414). Conclusion Dosimetry characteristics of the myQA SRS device meets the accuracy and sensitivity requirement of PSQA for SRS/SBRT treatment. The dose rate dependence should be adequately calibrated before its application and a more stringent GPR (3%/1 mm) evaluation criterion is suggested when it is used for SRS/SBRT QA.

Dosimetric Systems in Pre-Treatment QA for Stereotactic Treatments: Correlation Agreements and Target Volume Dependency

Abstract:Aim: This study comprehensively investigated pre-treatment quality assurance (QA) for 100 cancer patients undergoing stereotactic treatments (SRS/SRT) using various detectors. Methods: The study conducted QA for SRS/SRT treatments planned with a 6MV SRS beam at a dose rate of 1,000 MU/min, utilizing Eclipse v13.6 Treatment Planning System (TPS). Point dose measurements employed 0.01cm3 and 0.13cm3 cylindrical ionization chambers, while planar dose verification utilized Gafchromic EBT-XD Film and Portal Imager (aS1000). Plans were categorized by target volume, and a thorough analysis compared point dose agreements, planar dose gamma pass rates, and their correlations with chamber volume mean dose, detector type, and point dose agreement. Additionally, the consistency between different ionization chambers was assessed. Results: Point dose agreement generally improved with increasing target volume, except for volumes over 10cm3 with 0.01cm3 chambers, showing a contrary trend. Significant differences (p<0.05) were observed between TPS and measured doses for both chambers. Gamma pass rate improved with increasing target volume in EBT XD and aS1000 analyses, except for the >10cm3 group in EBT XD. EBT XD demonstrated better agreement with TPS for target volumes up to 10cm3 compared to aS1000, with a statistically significant difference (p<0.05) between the detectors. Strong correlations were found between chamber point dose and chamber volume mean dose agreement, as well as between the two gamma criteria analyses of the same detector type in the planar dose correlation analysis. However, weak correlations were discovered for other analyses. Conclusion: This study found weak correlation between different detector types in pre-treatment QA for point dose and planar dose evaluation. However, within a specific detector type, strong correlation was observed for different point dose evaluation methods and gamma criteria. This highlights the importance of cautious interpretation of QA results, particularly for SRS QA, due to the lack of correlation between detector types.
Evaluation of a complementary metal oxide semiconductor detector as a tool for stereotactic body radiotherapy plan quality assurance

Abstract:Background and purpose: A sub-mm resolution Complementary Metal Oxide Semiconductor sensor has been developed for stereotactic radiotherapy quality assurance. Herein we evaluate its basic dosimetric performance and its application for linac C-arm stereotactic body radiotherapy (SBRT) plan quality assurance. Materials and methods: The detector was integrated into its accompanying phantom or in Water Equivalent Plastic (WEP). The measurement reproducibility, stability, dose linearity and dependence on angularity, dose rate and field size were investigated. Clinical plan measurements were compared to our radiotherapy treatment planning system and radiochromic film. Sensitivity to introduced Multi Leaf Collimator (MLC) offsets was evaluated by simulating single MLC offsets in SBRT plans and comparing measurements to expected doses. Results: Signal reproducibility was within ± 0.1 % and output calibration was stable over a 6 month period. Detector showed good linearity with dose (r2 = 1). Signal decreased by 5 % when dose rate was decreased from 1300 MU/min to 300 MU/min. Output factors agreed within 0.5 % of chamber measurements for 1x1 cm field sizes or greater. Angularity measurements showed good agreement with reference. For measurement of planned clinical doses, gamma pass-rates were 98.5 % ± 2.3 % (treatment planning system reference, 2 %/2mm) and 99.2 % ± 1.0 % (film reference, 2 %,2mm). The detector also showed sensitivity to errors of 1 mm offsets in MLC positioning. Conclusion: The detector performed well when used for pre-treatment SBRT plan quality assurance, offering a good alternative to radiochromic film.
Assessment of stereotactic high-resolution detectors for stereotactic body radiotherapy: comparative analysis between myQA® SRS and Gafchromic EBT-XD films

Abstract:The study aimed to evaluate dosimetry systems used for stereotactic body radiotherapy (SBRT), specifically 2D array dosimetry and film dosimetry systems, for exploring their characteristics and clinical suitability. For this, high-resolution myQA SRS detectors and Gafchromic EBT-XD films were employed. Film analysis included net optical density (OD) values depending on energy, dose rate, scanner orientation, scanning side, and post-exposure growth. For myQA SRS, signal values were evaluated in terms of dose rate (400–1400 MU/min) and angular dependence (0–180° at 30° intervals) along with couch angles of 0°, 45°, and 90°. Pre-treatment verification included 32 SBRT patients for whom myQA SRS results were compared with those obtained with Gafchromic EBT-XD films. Analysis revealed less than 1% deviation in net OD for energy and dose rate dependence. Scanner orientation caused 2.5% net OD variation, with minimal differences between film front and back scan orientations (variance < 1.0%). A rapid OD rise occurred within six hours post-exposure, followed by gradual increase. The myQA SRS detector showed − 3.7% dose rate dependence (400 MU/min), while the angular dependence at 90° was − 26.7%. A correction factor effectively reduced these differences to < 1%. For myQA SRS, gamma passing rates were—93.6% (2%/1 mm), while those for EBT-XD films were—92.8%. Improved rates were observed with 3%/1 mm: for myQA SRS—97.9%, and for EBT-XD film—98.16%. In contrast, for 2%/2 mm with 10% threshold, for myQA SRS—97.7% and for EBT-XD film—98.97% were obtained. It is concluded that both myQA SRS detectors and EBT-XD films are suitable for SBRT pre-treatment verification, ensuring accuracy and reliability. However, myQA SRS detectors are preferred over EBT-XD film due to the fact that they offer real-time measurements and user-friendly features.

Abstract:Purpose To characterize detector array spacing and gamma index for quality assurance (QA) of stereotactic radiosurgery (SRS) deliveries. Use the Nyquist theorem to determine the required detector spacing in SRS fields, and find optimal gamma indices to detect MLC errors using the SRS MapCHECK, ArcCHECK, and a portal imaging device (EPID). Methods The required detector spacing was determined via Fourier analysis of small radiation fields and profiles of typical SRS treatment plans. The clinical impact of MLC errors of 0.5, 1, and 2 mm was evaluated. Global gamma (low‐dose threshold 10%) was evaluated for the three detector systems using various combinations of the distance to agreement and the dose difference. Results While MLC errors only slightly affected mean dose to PTV and a 2 mm thick surrounding structure (PTV_2 mm), significant PTV underdose incurred with increase in maximum dose to PTV_2 mm. Gamma indices with highest sensitivity to the introduced errors at 95% tolerance level for plans on target volumes of 3.2 cm3 (plan 3 cc) and 35.02 cm3 (plan 35 cc) were 2%/1 mm for the SRS MapCHECK and 2%/3 mm for the ArcCHECK, with 3%/1 mm (plan 3cc) and 2%/1 mm (plan 35cc) for the EPID. Drops in passing rates for a 2 mm MLC error were (46.2%, 41.6%) for the SRS MapCHECK and (12.2%, 4.2%) for the ArcCHECK for plan 3cc and plan 35cc, respectively. For Portal Dose, values were 4.5% (plan 3cc) and 7% (plan 35cc). The Nyquist frequency of two SRS dose distributions lie between 0.26 and 0.1 mm−1, corresponding to detector spacings of 1.9 and 5 mm. Evaluation of SRS MapCHECK data with doubled detector density indicates that increased detector density may reduce the system's sensitivity to errors, necessitating a tighter gamma index. Conclusions The present results give insight on the performance of detector arrays and gamma indices for the investigated detectors during SRS QA.

Abstract:Background As radiotherapy techniques advance, so do planning methods for multi‐target intracranial SRS cases. Multi‐target‐single‐isocenter (MTSI) planning offers high‐precision beam delivery with shortened duration. However, accommodating all targets in a single Patient‐Specific‐Quality‐Assurance (PSQA) with QA devices like SRS MapCHECK (SRS MC) is generally impractical. Purpose Consequently, we conducted PSQA, using a custom script, by relocating each Target or Neighboring‐Target‐Group (T‐NTG) relative to the beam isocenter on the PSQA device, ensuring each target's dose coverage at high precision. Methods SRS treatment plans use 6MV–FFF beams, consisting of four Volumetric Modulated ARC Therapy (VMAT) arcs, including one full‐arc and three half arcs with couch‐kicks. A custom script calculated T‐NTG coordinates relative to the beam isocenter. QA verification plans were created for each T‐NTG, redefining the beam isocenter for precise alignment with the center of the SRS MC. CBCT images were acquired during PSQA for SRS MC alignment, and gamma‐index analysis (GIA) was performed. A single‐tail paired t‐test assessed the passing rate (PR) for 75 QA verification plans. Results GIA with l.0 mm/2.0% criteria for each QA plan yielded a PR > 95.5%, with an average of 98.9%. Plans achieving PR > 99.0% and > 97.0% constituted 63% and 92% of studied plans, respectively. Statistical significance was observed in a t‐test with an ideal PR value of 100%, while insignificance was found with a PR value of 99%, suggesting that PSQA for individual targets consistently approaches 99% PR. In MTSI cases using 6MV‐FFF beams, targets within the lateral dose‐fall‐off region require careful verification for acceptability. Our clinical study on individual T‐NTG relocation demonstrates that the presented PSQA methods are generally acceptable, supported by a statistically insignificant PR against a 99% PR value. Conclusions Presented statistical analysis results indicate that the proposed PSQA approach can serve as a reliable tool in clinical settings.

Implementation of novel measurement-based patient-specific QA for pencil beam scanning proton FLASH radiotherapy

Abstract:Background: Several studies have shown pencil beam scanning (PBS) proton therapy is a feasible and safe modality to deliver conformal and ultra-high dose rate (UHDR) FLASH radiation therapy. However, it would be challenging and burdensome to conduct the quality assurance (QA) of the dose rate along with conventional patient-specific QA (psQA). Purpose: To demonstrate a novel measurement-based psQA program for UHDR PBS proton transmission FLASH radiotherapy (FLASH-RT) using a high spatiotemporal resolution 2D strip ionization chamber array (SICA). Methods: The SICA is a newly designed open-air strip-segmented parallel plate ionization chamber, which is capable of measuring spot position and profile through 2 mm-spacing-strip electrodes at a 20 kHz sampling rate (50 μs per event) and has been characterized to exhibit excellent dose and dose rate linearity under UHDR conditions. A SICA-based delivery log was collected for each irradiation containing the measured position, size, dwell time, and delivered MU for each planned spot. Such spot-level information was compared with the corresponding quantities in the treatment planning system (TPS). The dose and dose rate distributions were reconstructed on patient CT using the measured SICA log and compared to the planned values in volume histograms and 3D gamma analysis. Furthermore, the 2D dose and dose rate measurements were compared with the TPS calculations of the same depth. In addition, simulations using different machine-delivery uncertainties were performed, and QA tolerances were deduced. Results: A transmission proton plan of 250 MeV for a lung lesion was planned and measured in a dedicated ProBeam research beamline (Varian Medical System) with a nozzle beam current between 100 to 215 nA. The worst gamma passing rates for dose and dose rate of the 2D SICA measurements (four fields) compared to TPS prediction (3%/3 mm criterion) were 96.6% and 98.8%, respectively, whereas the SICA-log reconstructed 3D dose distribution achieved a gamma passing rate of 99.1% (2%/2 mm criterion) compared to TPS. The deviations between SICA measured log, and TPS were within 0.3 ms for spot dwell time with a mean difference of 0.069 ± 0.11 s, within 0.2 mm for spot position with a mean difference of -0.016 ± 0.03 mm in the x-direction, and -0.036 ± 0.059 mm in the y-direction, and within 3% for delivered spot MUs. Volume histogram metric of dose (D95) and dose rate (V40Gy/s ) showed minimal differences, within less than 1%. Conclusions: This work is the first to describe and validate an all-in-one measurement-based psQA framework that can fulfill the goals of validating the dose rate accuracy in addition to dosimetric accuracy for proton PBS transmission FLASH-RT. The successful implementation of this novel QA program can provide future clinical practice with more confidence in the FLASH application.
Comprehensive stereotactic radiosurgery platform characterization: A novel end‐to‐end approach with anthropomorphic 3D dosimetry

Abstract:Background Stereotactic radiosurgery (SRS) is a widely employed strategy for intracranial metastases, utilizing linear accelerators and volumetric modulated arc therapy (VMAT). Ensuring precise linear accelerator performance is crucial, given the small planning target volume (PTV) margins. Rapid dose falloff is vital to minimize brain radiation necrosis. Despite advances in SRS planning, tools for end‐to‐end testing of SRS treatments are lacking, hindering confidence in the procedure. Purpose This study introduces a novel end‐to‐end three‐dimensional (3D) anthropomorphic dosimetry system for characterization of a radiosurgery platform, aiming to measure planning metrics, dose gradient index (DGI), brain volumes receiving at least 10 and 12 Gy (V10, V12), as well as assess delivery uncertainties in multitarget treatments. The study also compares metrics from benchmark plans to enhance understanding and confidence in SRS treatments. Methods The developed anthropomorphic 3D dosimetry system includes a modified Stereotactic End‐to‐End Verification (STEEV) phantom with a customized insert integrating 3D dosimeters and a fiber optic CT scanner. Labview and MATLAB programs handle optical scanning, image preprocessing, and dosimetric analysis. SlicerRT is used for 3D dose visualization and analysis. A film stack insert was used to validate the 3D dosimeter measurements at specific slices. Benchmark plans were developed and measured to investigate off‐axis errors, dose spillage, small field dosimetry, and multi‐target delivery. Results The accuracy of the developed 3D dosimetry system was rigorously assessed using radiochromic films. Two two‐dimensional (2D) dose planes, extracted from the 3D dose distribution, were compared with film measurements, resulting in high passing rates of 99.9% and 99.6% in gamma tests. The mean relative dose difference between film and 3D dosimeter measurements was −1%, with a standard deviation of 2.2%, well within dosimeter uncertainties. In the first module, evaluating single‐isocenter multitarget treatments, a 1.5 mm dose distribution shift was observed when targets were 7 cm off‐axis. This shift was attributed to machine mechanical errors and image‐guided system uncertainties, indicating potential limitations in conventional gamma tests. The second module investigated discrepancies in intermediate‐to‐low dose spillage, revealing higher measured doses in the connecting region between closely positioned targets. This discrepancy was linked to uncertainties in treatment planning system (TPS) modeling of out‐of‐field dose and multileaf collimator (MLC) characteristics, resulting in lower DGI values and higher V10 and V12 values compared to TPS calculations. In the third module, irradiating multiple targets showed consistent V10 and V12 values within 1 cm3 agreement with dose calculations. However, lower DGI values from measurements compared to calculations suggested intricacies in the treatment process. Conducting vital end‐to‐end testing demonstrated the anthropomorphic 3D dosimetry system's capacity to assess overall treatment uncertainty, offering a valuable tool for enhancing treatment accuracy in radiosurgery platforms. Conclusions The study introduces a novel anthropomorphic 3D dosimetry system for end‐to‐end testing of a radiosurgery platform. The system effectively measures plan quality metrics, captures mechanical errors, and visualizes dose discrepancies in 3D space. The comprehensive evaluation capability enhances confidence in the commissioning and verification process, ensuring patient safety. The system is recommended for commissioning new radiosurgery platforms and remote auditing of existing programs.

A multi-institutional trial evaluating the use of an integrated quality assurance phantom for frameless single-isocenter multitarget stereotactic radiosurgery

Abstract:Background: Brain radiosurgery treatments require multiple quality-assurance (QA) procedures to ensure accurate and precise treatment delivery of ablative doses. As single-isocenter multitarget radiosurgery treatments become more popular for treating patients with multiple brain metastases, quantifying off-axis accuracy of linear accelerators is crucial. In this study, we developed a novel brain radiosurgery integrated phantom and validated this phantom at multiple institutions to enable radiosurgery QA with a single phantom to facilitate implementation of a frameless single-isocenter, multitarget radiosurgery program. The phantom combines multiple independent verification system tests including the Winston-Lutz test, off-axis accuracy evaluation (i.e., off-axis Winston-Lutz), as well as dosimetric measurements utilizing both point dose and film measurement. Methods and materials: A novel 3D-printed phantom, coined OneIso , was designed with a movable insert which can switch between Winston-Lutz test targets and dose measurement without moving the phantom itself. In total, four phantoms were printed, and eight institutions participated in this study, which included both Varian TrueBeam (n=6) and Elekta Versa (n=2) linear accelerators. For off-axis Winston-Lutz measurements, a row of off-axis ball-bearings (BBs) was integrated into the OneIso. To quantify the spatial accuracy versus distance from isocenter, two-dimensional displacements were calculated between the planned and delivered BB locations relative to their respective MLC-defined field borders. For dose verification, brain radiosurgery clinical treatment plans previously treated were delivered at multiple cancer centers (six of eight centers). Radiochromic film and pinpoint ion chamber comparison measurements were obtained with OneIso. Results: Dose verification performed using the OneIso phantom across the different centers were all within on average 3% agreement, for both film and point-dose measurements. OneIso identified a reduction in spatial accuracy further away from isocenter for all eight radiosurgery machines. Differences increased as distance from isocenter increased, exceeding recommended radiosurgery accuracy tolerances (<1mm) at different distances for each machine (2-7cm), indicating that the tolerance is machine-dependent. Conclusion: OneIso provides a streamlined, single-setup workflow for single-isocenter multitarget frameless linac-based radiosurgery QA that can be easily translated to multiple institutions. Additionally, quantifying off-axis spatial discrepancies allows for determination of the maximum distance between targets and iso that meet single-isocenter multitarget radiosurgery program recommendations.

Abstract:Purpose Patient‐specific quality assurance (PSQA) for vertebra stereotactic body radiation therapy (SBRT) presents challenges due to highly modulated small fields with high‐dose gradients between the target and spinal cord. This study aims to explore the use of the SRS MapCHECK® (SRSMC) for vertebra SBRT PSQA. Methods Twenty vertebra SBRT treatment plans including prescriptions 20 Gy/1 fraction and 24 Gy/2 fractions were selected for each of Millennium (M)‐Multileaf Collimator (MLC), and high‐definition (HD)‐MLC. All 40 plans were measured using Gafchromic EBT3 film (film) and SRSMC, using the StereoPHAN phantom. Plan complexity was assessed using modulation complexity score (MCS), edge metric (EM) (mm−1), modulation factor (MU/cGy), and average leaf pair opening (ALPO) (mm) and its correlation with gamma‐pass rate was investigated. The high dose gradient between the target and the spinal cord was analyzed for film and SRSMC and compared against the treatment planning system (TPS). Applying the methodology proposed by AAPM TG‐218, action and tolerance values specific to the SRSMC for vertebra SBRT were determined for β values ranging from 5 to 8. Results Film and SRSMC gamma‐pass rates showed no correlation (p > 0.05). A moderate negative correlation (R = ‐0.57, p = 0.01) is present between EM and SRSMC 3%/1 mm gamma‐pass rate for HD‐MLC plans. Both film and SRSMC accurately measured high dose gradients between the target and the spinal cord (R2 > 0.86, p ≤ 0.05). Notably, dose‐gradient of HD‐MLC plans is 22% steeper and has a smaller standard deviation to M‐MLC plans (p ≤ 0.05). Applying TG‐218, the film tolerance limit was 96% with action limit 95% for 5%/1 mm (β = 6) and for the SRSMC tolerance limit was 97% with an action limit of 96% for 4%/1 mm (β = 6). Conclusion Our findings suggest that universal TG‐218 limits may not be suitable for vertebra SBRT PSQA. This study demonstrates that SRSMC is a viable tool for vertebra SBRT PSQA, supported by TG‐218 implementation of process‐based tolerance and action limits.

Independent Monte Carlo dose calculation identifies single isocenter multi‐target radiosurgery targets most likely to fail pre‐treatment measurement

Abstract:Purpose For individual targets of single isocenter multi‐target (SIMT) Stereotactic radiosurgery (SRS), we assess dose difference between the treatment planning system (TPS) and independent Monte Carlo (MC), and demonstrate persistence into the pre‐treatment Quality Assurance (QA) measurement. Methods Treatment plans from 31 SIMT SRS patients were recalculated in a series of scenarios designed to investigate sources of discrepancy between TPS and independent MC. Targets with > 5% discrepancy in DMean[Gy] after progressing through all scenarios were measured with SRS MapCHECK. A matched pair analysis was performed comparing SRS MapCHECK results for these targets with matched targets having similar characteristics (volume & distance from isocenter) but no such MC dose discrepancy. Results Of 217 targets analyzed, individual target mean dose (DMean[Gy]) fell outside a 5% threshold for 28 and 24 targets before and after removing tissue heterogeneity effects, respectively, while only 5 exceeded the threshold after removing effect of patient geometry (via calculation on StereoPHAN geometry). Significant factors affecting agreement between the TPS and MC included target distance from isocenter (0.83% decrease in DMean[Gy] per 2 cm), volume (0.15% increase per cc), and degree of plan modulation (0.37% increase per 0.01 increase in modulation complexity score). SRS MapCHECK measurement had better agreement with MC than with TPS (2%/1 mm / 10% threshold gamma pass rate (GPR) = 99.4 ± 1.9% vs. 93.1 ± 13.9%, respectively). In the matched pair analysis, targets exceeding 5% for MC versus TPS also had larger discrepancies between TPS and measurement with no GPR (2%/1 mm / 10% threshold) exceeding 90% (71.5% ± 16.1%); whereas GPR was high for matched targets with no such MC versus TPS difference (96.5% ± 3.3%, p = 0.01). Conclusions Independent MC complements pre‐treatment QA measurement for SIMT SRS by identifying problematic individual targets prior to pre‐treatment measurement, thus enabling plan modifications earlier in the planning process and guiding selection of targets for pre‐treatment QA measurement.

Abstract:Theoretically, long-latency somatosensory evoked potentials (SEPs) provide information on the function of somatosensory associative cortical structures. Their potential role in clinical studies and research has been hampered by the lack of standardized methodology in the use of these SEPs. Other factors, such as drugs, simultaneous stimuli, and state of consciousness, also have far-reaching influences on the various parameters of long-latency SEPs. The knowledge of the origin of most SEP components is at best fragmentary; studies on clinical–electrophysiological correlations seem to be hopeful in this respect. As yet, clinical applications of long-latency SEPs are limited; for future research, studies of disturbances of SEPs are most promising, mainly with regard to diseases of the gray matter, the influence of drugs on the cerebral function, and psychopathology.
Evaluation of a novel patient-specific quality assurance phantom for robotic single-isocentre, multiple-target stereotactic radiosurgery, and stereotactic radiotherapy

Abstract:Abstract Objectives To evaluate patient-specific quality assurance (PSQA) of 3 targets in a single delivery using a novel film-based phantom. Methods The phantom was designed to rotate freely as a sphere and could measure 3 targets with film in a single delivery. After identifying the coordinates of 3 targets in the skull, the rotation angles about the equator and meridian were computed for optimal phantom setup, ensuring the film plane intersected the 3 targets. The plans were delivered on the CyberKnife system using fiducial tracking. The irradiated films were scanned and processed. All films were analysed using 3 gamma criteria. Results Fifteen CyberKnife test plans with 3 different modalities were delivered on the phantom. Both automatic and marker-based registration methods were applied when registering the irradiated film and dose plane. Gamma analysis was performed using a 3%/1 mm, 2%/1 mm, and 1%/1 mm criteria with a 10% threshold. For the automatic registration method, the passing rates were 98.2% ± 1.9%, 94.2% ± 3.7%, and 80.9% ± 6.3%, respectively. For the marker-based registration approach, the passing rates were 96.4% ± 2.7%, 91.7% ± 4.3%, and 78.4% ± 6.2%, respectively. Conclusions A novel spherical phantom was evaluated for the CyberKnife system and achieved acceptable PSQA passing rates using TG218 recommendations. The phantom can measure true-composite dose and offers high-resolution results for PSQA, making it a valuable device for robotic radiosurgery. Advances in knowledge This is the first study on PSQA of 3 targets concurrently on the CyberKnife system.

Investigation of a 3D dosimetry system utilizing radio-chromic gel and telecentric optical-CT readout

Abstract:Background: Sophisticated modern radiation therapy treatments require comprehensive validation in 3D. Purpose: Investigation and characterization of a novel 3D dosimetry system consisting of ClearView radiochromic gel dosimeters (commercially available from Modus Inc) and an in-house telecentric optical CT scanner DLOS (the Duke Large Field of View Optical-CT Scanner). Methods: Spectrophotometry measurements were made on small volumes of ClearView gel irradiated with 6X photon doses up to 40 Gy to determine linearity and temporal stability of dose response. Clinical evaluation of Clearview/DLOS system was conducted in two phases. Phase one involved simple photon and electron benchmark irradiations, delivered to 15 and 10 cm diameter dosimeters, at various energies and doses. Phase 2 investigated application to the verification of two single isocenter multi-target (SIMT) stereotactic radiosurgery (SRS) deliveries. These were patient treatments for two and five brain lesions, respectively, and delivered to 15 cm diameter dosimeters. SIMT treatments were delivered by Varian TrueBeam 6X with doses of 40 Gy. For dose read-out, dosimeters were optically scanned in the DLOS both pre- and post- irradiation (within 24 h). 3D reconstructions (1 mm3 resolution) of the change in linear-optical- attenuation (proportional to dose) was obtained using in-house software and 3D Slicer. Measured and predicted (Eclipse TPS) doses were compared through percent depth-dose (PDD), cross plane and in-plane profiles, and relative 3D gamma analysis (performed at a range of 7%/4 mm down to 2%/2 mm). Regions of known artifacts were excluded from analysis (jar base, neck, and wall). The SIMT SRS deliveries were additionally compared to SciMoca, an independent Monte Carlo second check software. Results: Linearity of dose response was confirmed with R2 ≥ 0.9986 at both 520 and 630 nm wavelengths and at three post-irradiation time points: 21 h, 6 and 10 days. Dose profiles of all benchmark irradiations, in both 15 and 10 cm dosimeters, show good agreement in useable areas of the gel compared to Eclipse dose calculations, with root mean square errors (RMSE) ≤ 0.0054, and R2 ≥ 0.9808. Gamma pass rates for the 15 cm dosimeter benchmark irradiations were ≥ 94% at 2%/2 mm (central axis), ≥ 90% at 3%/3 mm (left lateral), ≥ 90% at 2%/2 mm (electron), and ≥ 94% at 3%/2 mm (stacking field). Similar high passing rates were observed for benchmark irradiations to the smaller 10 cm diameter dosimeters. Very high Gamma pass rates were found for SIMT SRS deliveries, with 99.82% and 97.80% at 3%/2 mm, for the two and five target plans, respectively. Conclusion: This work presents the first investigation of ClearView dosimeters in combination with a telecentric optical-CT scanner (DLOS). Simple benchmark irradiations demonstrate ClearView/DLOS can accurately recreate and measure relative 3D dose within non-artifact regions (i.e., > 1 cm away from walls). Application to SIMT SRS deliveries demonstrated the viability of the system as a means for comprehensive 3D verification of complex treatment deliveries as well as confirming the treatment planning system dose distribution. The results indicate that DLOS/ClearView system is a highly effective 3D verification tool for SIMT irradiations and can be applied with 3%/2 mm gamma criteria where passing rates of > 95% are to be expected.
Predicting the PSQA results of volumetric modulated arc therapy based on dosiomics features: a multi-center study

Abstract:Backgroud and objectives: The implementation of patient-specific quality assurance (PSQA) has become a crucial aspect of the radiation therapy process. Machine learning models have demonstrated their potential as virtual QA tools, accurately predicting the gamma passing rate (GPR) of volumetric modulated arc therapy (VMAT)plans, thereby ensuring safe and efficient treatment for patients. However, there is limited multi-center research dedicated to predicting the GPR. In this study, a dosiomics-based machine learning approach was employed to construct a prediction model for classifying GPR in multiple radiotherapy institutions. Additionally, the model's performance was compared by evaluating the impact of two distinct feature selection methods. Methods: A retrospective data collection was conducted on 572 VMAT patients across three radiotherapy institutions. Utilizing a three-dimensional dose verification technique grounded in real-time measurements, γ analysis was conducted according to the criteria of 3%/2 mm and 2%/2 mm, employing a dose threshold of 10% along with absolute dose and global normalization mode. Dosiomics features were extracted from the dose files, and distinct subsets of features were selected as inputs for the model using the random forest (RF) and RF combined with SHapley Additive exPlanations (SHAP) methods. The data underwent training using the extreme gradient boosting (XGBoost) algorithm, and the model's classification performance was assessed through F1-score and area under the curve (AUC) values. Results: The model exhibited optimal performance under the 3%/2 mm criteria, utilizing a subset of 20 features and attaining an AUC value of 0.88 and an F1-score of 0.89. Similarly, under the 2%/2 mm criteria, the model demonstrated superior performance with a subset of 10 features, resulting in an AUC value of 0.91 and an F1-score of 0.89. The feature selection methods of RF and RF + SHAP have achieved good model performance by selecting as few features as possible. Conclusion: Based on the multi-center PSQA results, it is possible to utilize dosiomics features extracted from dose files to construct a machine learning predictive model. This model demonstrates excellent discriminative abilities, thus promoting the progress of gamma passing rate prognostic models in clinical application and implementation. Furthermore, it holds potential in providing patients with secure and efficient personalized QA management, while also reducing the workload of medical physicists.

Abstract:Purpose To design a patient specific quality assurance (PSQA) process for the CyberKnife Synchrony system and quantify its dosimetric accuracy using a motion platform driven by patient tumor traces with rotation. Methods The CyberKnife Synchrony system was evaluated using a motion platform (MODUSQA) and a SRS MapCHECK phantom. The platform was programed to move in the superior‐inferior (SI) direction based on tumor traces. The detector array housed by the StereoPhan was placed on the platform. Extra rotational angles in pitch (head down, 4.0° ± 0.15° or 1.2° ± 0.1°) were added to the moving phantom to examine robot capability of angle correction during delivery. A total of 15 Synchrony patients were performed SBRT PSQA on the moving phantom. All the results were benchmarked by the PSQA results based on static phantom. Results For smaller pitch angles, the mean gamma passing rates were 99.75% ± 0.87%, 98.63% ± 2.05%, and 93.11% ± 5.52%, for 3%/1 mm, 2%/1 mm, and 1%/1 mm, respectively. Large discrepancy in the passing rates was observed for different pitch angles due to limited angle correction by the robot. For larger pitch angles, the corresponding mean passing rates were dropped to 93.00% ± 10.91%, 88.05% ± 14.93%, and 80.38% ± 17.40%. When comparing with the static phantom, no significant statistic difference was observed for smaller pitch angles (p = 0.1 for 3%/1 mm), whereas a larger statistic difference was observed for larger pitch angles (p

Abstract:Purpose Experimental patient‐specific QA (PSQA) is a time and resource‐intensive process, with a poor sensitivity in detecting errors. Radiation therapy facilities aim to substitute it by means of independent dose calculation (IDC) in combination with a comprehensive beam delivery QA program. This paper reports on the commissioning of the IDC software tool myQA iON (IBA Dosimetry) for proton therapy and its clinical implementation at the MedAustron Ion Therapy Center. Methods The IDC commissioning work included the validation of the beam model, the implementation and validation of clinical CT protocols, and the evaluation of patient treatment data. Dose difference maps, gamma index distributions, and pass rates (GPR) have been reviewed. The performance of the IDC tool has been assessed and clinical workflows, simulation settings, and GPR tolerances have been defined. Results Beam model validation showed agreement of ranges within ± 0.2 mm, Bragg‐Peak widths within ± 0.1 mm, and spot sizes at various air gaps within ± 5% compared to physical measurements. Simulated dose in 2D reference fields deviated by ‐0.3% ± 0.5%, while 3D dose distributions differed by 1.8% on average to measurements. Validation of the CT calibration resulted in systematic differences of 2.0% between IDC and experimental data for tissue like samples. GPRs of 99.4 ± 0.6% were found for head, head and neck, and pediatric CT protocols on a 2%/2 mm gamma criterion. GPRs for the adult abdomen protocol were at 98.9% on average with 3%/3 mm. Root causes of GPR outliers, for example, implants were identified and evaluated. Conclusion IDC has been successfully commissioned and integrated into the MedAustron clinical workflow for protons in 2021. IDC has been stepwise and safely substituting experimental PSQA since February 2021. The initial reduction of proton experimental PSQA was about 25% and reached up to 90% after 1 year.

Abstract:Stereotactic radiosurgery (SRS) is a radiation treatment method that irradiates a large amount of radiation at once, and spatial accuracy is important, so quality assurance (QA) using a phantom is important. Although metal balls used as markers in QA phantoms are clearly visible in megavoltage images, the position of the isocenter is difficult to determine in cone-beam computed tomography (CBCT) owing to the presence of metal artifacts. Thus, we fabricated five types of acrylic hexahedron blocks, including concentric metal balls with a diameter of 1 mm and dummy blocks without metal balls. We produced a cube phantom for SRS that can insert an acrylic hexahedron block into a desired location. A linear accelerator was used to deliver a 6 MV photon beam to the phantom. We evaluated the accuracy of mechanical isocenter through the position of metal spheres in EBT3 films and megavoltage images. The average and deviation of the isocenters of the film and megavoltage images of our linear accelerator system were 0.82 ± 0.19 mm and 0.9 ± 0.17 mm, respectively, and the isocenter difference between the film and megavoltage images was 0.08 ± 0.07 mm. The accuracy of the isocenter and off-axis center positioning for the SRS was evaluated by acquiring megavoltage and kilovoltage images using the block phantom. The block phantom for the SRS is accurate and easy to use; therefore, it will be useful for SRS QA.
