Abstract:Sodium measurement during hemodialysis treatment is important to preserve the patient from clinical events related to hypo- or hyper-natremia Usually, sodium measurement is performed through laboratory equipment which is typically expensive, and requires manual intervention. We propose a new method, based on conductivity measurement after treatment of dialysate solution through ion-exchange resin. To test this method, we performed in vitro experiments. We prepared 40 ml sodium chloride (NaCl) samples at 280, 140, 70, 35, 17.5, 8.75, 4.375 mEq/l, and some "mixed samples", i.e., with added potassium chloride (KCl) at different concentrations (4.375-17.5 mEq/l), to simulate the confounding factors in a conductivity-based sodium measurement. We measured the conductivity of all samples. Afterwards, each sample was treated for 1 min with 1 g of Dowex G-26 resin, and conductivity was measured again. On average, the difference in the conductivity between mixed samples and corresponding pure NaCl samples (at the same NaCl concentration) was 20.9%. After treatment with the exchange resin, it was 14.7%, i.e., 42% lower. Similar experiments were performed with calcium chloride and magnesium chloride as confounding factors, with similar results. We also performed some experiments on actual dialysate solution during hemodialysis sessions in 15 patients, and found that the correlation between conductivity measures and sodium concentration improved after resin treatment (R=0.839 before treatment, R=0.924 after treatment, P<0.0001). We conclude that ion-exchange resin treatment coupled with conductivity measures may improve the measurement of sodium compared to conductivity measures alone, and may become a possible simple approach for continuous and automatic sodium measurement during hemodialysis.

Abstract:Rationale Trimethylamine-N-oxide (TMAO) is a potential indicator of cardiovascular disease and chronic kidney disorders. It is important to monitor the TMAO level in plasma or serum of hemodialysis patients. A simple liquid chromatography/differential ion mobility spectrometry tandem mass spectrometry (HPLC/DMS-MS/MS) method was established and validated for the determination of TMAO in the serum of hemodialysis patients. Methods Chromatographic separation was performed on a Waters Atlantis HILIC silica column (2.1 x 50 mm, 3 mu m). The gradient mobile phase consisted of 10 mM ammonium formate buffer and acetonitrile with 0.1% formic acid in both solvents. The serum sample was precipitated with acidic acetonitrile prior to HPLC/DMS-MS/MS analysis and TMAO-d(9) was used as the internal standard. Data acquisition was performed in positive ion mode with a DMS system before the electrospray ionization source. The selected reaction monitoring transitions were m/z 76.0 -> 58.0 and m/z 85.2 -> 66.1 for TMAO and the internal standard, respectively. Results Excellent linearity was observed over the calibration range 0.05-20 mu g/mL (r(2) > 0.995). The method was validated for good specificity and sensitivity. The inter-run and intra-run precision and accuracy were less than 13.6% and 10.7%, respectively. Conclusions We established a novel and robust HPLC/DMS-MS/MS method for the quantification of TMAO in human serum samples. The validated assay was simple, rapid, sensitive and reliable. The developed method could be applied to the assay of serum samples from patients with kidney disease who are undergoing hemodialysis.
Determination of Sodium, Potassium, Calcium and Magnesium in Urine, Using Microwave Plasma - Atomic Emission Spectrometry and Multi-Energy Calibration

Abstract:Microwave plasma - atomic emission spectrometry with multi-energy calibration (MP-AES-MEC) was used for the determination of four major elements in urine. In the family of atomic emission spectrometry, the distinctive features of MP-AES are: (i) nitrogen-based toroidal-shape plasma; (ii) good plasma tolerance to total solids; (iii) exceptionally low operating cost. On the other hand, due to relatively low plasma temperature, this technique is susceptible to spectral interferences and sample-to-sample fluctuating baseline is typical limitation, if previous acid digestion is not performed. MEC is a non-conventional quantification method, not requiring baseline correction and it has been selected in this work to achieve reliable determination of Na, K, Ca and Mg in simply diluted urine. The principle underlying MEC is the proportionality between signal intensity and analyte concentration, occurring at any emission line for given element. Accordingly, for each sample, only two solutions were prepared likewise in the one-point standard addition and two analytical runs were performed, yet four experimental points were generated for calibration according with the number of emission lines utilized. Based on the results obtained by analyzing urines from different subjects, the sample dilution fold was selected to adjust the analyte concentration below half of the calibration range (150 for K, 200 for Na, 50 for Ca and 25 for Mg), while the addition of standard was done roughly doubling natural concentration in the diluted sample. The evaluated instrumental limits of detection were: 0.009 ± 0.005 mg L-1 for K, 0.131 ± 0.011 mg L-1 for Na, 0.050 ± 0.014 mg L-1 for Ca and 0.059 ± 0.010 mg L-1 for Mg (five replicates in different days). Each analysis was performed in triplicate yielding percentage relative standard deviation £ 11 %. The percentage recoveries calculated taking the results obtained in acid-digested samples by external calibration as reference values were in the range: 83.3-102 % for K, 88.4-110 % for Na, 82.9-113 % for Ca and 85.8-108 % for Mg. The capability of the proposed MP-AES-MEC procedure for monitoring four elements in different clinical conditions was demonstrated by analyzing ten urines from diabetic patients and ten from non-diabetic control subjects; statistical differences between these two groups was found for Na and K. Resumen. La espectrometría de emisión atómica con excitación en plasma de microondas y con el método de calibración multi-energéa (MP-AES-MEC) fueron empleados para la determinación de cuatro elementos mayoritarios en orina. Dentro de la familia de técnicas de espectrometría de emisión atómica, las características distintivas de MP-AES son las siguientes: (i) uso de plasma de nitrógeno con geometría toroidal; (ii) buena tolerancia del plasma a sólidos totales; (iii) excepcionalmente bajo costo de operación. Por otra parte, debido a la relativamente baja temperatura del plasma, una típica limitación de esta técnica es que es susceptible a interferencias espectrales y la fluctuación de línea base entre muestras si estas no son digeridas previamente. El método MEC es un método de cuantificación no-convencional, el cual no requiere corrección de linea base y fue seleccionado en este trabajo para lograr la determinación confiable de Na, K, Ca and Mg después de una simple dilución de orina. MEC se sustenta en la proporcionalidad entre intensidad de la señal y la concentración del analito existente en cualquier linea de emisión de un elemento dado. Para cada una de las muestras, se prepararon dos soluciones, igual que en el método de un punto de adición de estándar y se realizaron dos corridas analíticas, pero se generaron cuatro puntos experimentales para la calibración, correspondientes a cuatro líneas de emisión seleccionadas por elemento. Con base en los resultados obtenidos analizando orinas de diferentes sujetos, el factor de dilución de la muestra fue seleccionado para ajustar la concentración del analito por debajo de la mitad del rango de calibración (150 para K, 200 para Na, 50 para Ca, 25 para Mg), mientras que la adición de estándar se realizó subiendo aproximadamente al doble la concentración natural en la muestra diluida. Los límites de detección instrumentales fueron: 0.009 ± 0.005 mg L-1 para K, 0.131 ± 0.011 mg L-1 para Na, 0.050 ± 0.014 mg L-1 para Ca, 0.059 ± 0.010 mg L-1 para Mg (con base en cinco réplicas realizadas en diferentes días). Cada análisis se realizó por triplicado, obteniéndose valores de desviación estándar relativa £ 11 %. Los porcentajes de recuperación calculados considerando los resultados obtenidos en muestras digeridas con ácido mediante calibración externa convencional como valores de referencia fueron los siguientes: 83.3-102 % para K, 88.4-110 % para Na, 82.9-113 % para Ca, 85.8-108 % para Mg. La capacidad del procedimiento MP-AES-MEC para monitorear cuatro elementos en diferentes condiciones clínicas se demostró analizando orinas de pacientes diabéticos y orinas de sujetos en un grupo control, encontrándose diferencias estadísticamente significativas para Na y K.

Abstract:The analysis of toxic and essential elements in human matrices is used in clinical diagnostics and for biomonitoring of different populations to study related health outcomes. This work aimed to develop fast and reliable methods for the analysis of a broad range of elements in liquid human matrices, such as whole blood, serum, and urine, with a similar setup for the three matrices and different analysis needs. An easy and fast‐forward dilute‐and‐shoot method for 33 elements (i.e., Ag, Al, As, B, Ba, Be, Bi, Cd, Ce, Co, Cr, Cu, Hg, I, Li, Mn, Mo, Ni, Pb, Pd, Pt, Sb, Se, Sn, Sr, Te, Th, Tl, U, V, W, Zn, and Zr) was developed. 200 μL of either sample material was diluted with an alkaline reagent to a volume of 4 mL in total. Sample dilution and preparation of matrix‐matched calibration standards were performed in 48‐well plates by an automated liquid handler. Diluted samples were analyzed by inductively coupled plasma mass spectrometry on a Perkin Elmer NexIon 300D ICP‐MS instrument equipped with an ESI‐FAST SC2DX autosampler in kinetic energy discrimination mode with helium as cell gas at either 4.8 mL or 5.7 mL and 1600 W RF generator power. The method validation results showed good accuracy for fresh human samples from an external quality assessment scheme with measured concentrations within the assigned concentration ranges. Good precision and reproducibility for most elements were demonstrated with variation coefficients below or far below 8% and 15% for whole blood, 8% and 10% for serum, and 10% and 10% for urine, respectively. The developed reagent and instrumental setup were applicable to all three matrices. This minimizes the risk of human errors when switching between analyses of the different sample matrices and allows a rapid and easy analysis of whole blood, serum, and urine within one day if needed. The method demonstrated robustness over time, withstanding minor changes in the preparation of working solutions and samples, instrumental analysis, and setup. Analysis of human real samples showed the method's applicability for 33 toxic and essential elements in whole blood, serum, and urine and at concentrations relevant to clinical diagnostics as well as biomonitoring.

Abstract:BackgroundOur established method for measuring plasma catecholamines utilized a Shimadzu HPLC coupled to an ESA Coulochem III electrochemical detector. Chromatographic run times were long, resulting in slow turnaround times. Instrument maintenance was labor intensive, and the assay was prone to interferences. To overcome these limitations, we changed the method to liquid chromatography-tandem mass spectrometry (LC-MS/MS) measurements.MethodsAfter the addition of a labeled internal standard for each analyte, samples are loaded onto 50 mg of activated alumina at pH 8.6, utilizing a Tris-HCl, sodium carbonate and EDTA buffer. After incubation the alumina is then washed with 50% acetonitrile followed by water. Elution off the alumina is performed with 2% acetic acid. Derivatization is then performed on the eluate utilizing acetaldehyde and a cyanoborohydride buffer. Following derivatization, the samples are analyzed for epinephrine, norepinephrine, and dopamine by LC-MS/MS using a Sciex 7500 coupled to a ThermoFisher Scientific TLX-4 equipped with Vanquish pumps. Chromatographic separation was obtained using a Phenomenex Kinetex 2.6 um BiPhenyl 50x3 mm analytical column.ResultsRuntimes were significantly shorter than with the previous method, and no interferences were observed. Assay validation consisted of: intra/inter assay precision, accuracy, recovery, linearity, carryover, limit of detection (LOD), and interferences. Three levels were assessed for intra assay precision and each of the analytes monitored yielded %CVs of less than 6%. Four levels were assessed for inter assay precision and each of the analytes monitored yielded %CVs of less than 10%. When using linear regression analysis to compare results with the previous clinical assay, the slope, y-intercept and R 2 were the following; dopamine: 1.06, 2.74, 0.978; epinephrine: 0.905, 2.20, 0.989; norepinephrine: 0.919, 9.75, 0.953. Spike and recovery from five samples yielded a mean average recovery of 93.4% for dopamine, 100% for epinephrine and 98.1% for norepinephrine. For linearity 4 samples were diluted and the mean % difference from expected was -1.66% for dopamine, -0.25% for epinephrine and 1.78% for norepinephrine. Blanks were injected following the high standard and carryover was negligible. LOD was determined by running a low sample pool along with a blank over 20 assays. The LOD was determined to be less than the LLOQ. Sixty-two common drugs/vitamins along with varying levels of hemolysis, lipemia, and bilirubin were also tested to ensure no interferences were seen.ConclusionsThe LC-MS/MS method exceeded the performance of the historical Shimdazu HPLC system with ESA Coluochem III electrochemical detector method. We anticipate that the better analytical performance will improve the testing quality of our patients.

Development of environmentally friendly voltammetric methods for the individual and simultaneous determination of antihypertensives: Validation and application

Abstract:In this study, voltammetric methods were developed and validated for the individual determination of lisinopril (LSNP) and its association with hydrochlorothiazide (HCTZ) employing a cathodically pretreated boron-doped diamond (CP-BDD) electrode. The studies of the experimental and instrumental parameters that affect the performance of both methods were carefully carried out by optimizing each parameter. The methods were validated by evaluating the figures of merits recommended by the validation guidelines. The method for the individual determination of LSNP presented a linear working range between 9.99 and 196.01 nmol L −1 , with a limit of detection of 3.05 nmol L −1 by differential pulse voltammetry in BR buffer solution (pH 2.0). Simultaneous analytical curves over the concentration ranges of 0.40–3.99 and 0.050–1.045 μmol L −1 for HCTZ and LSNP, respectively, were obtained by square-wave voltammetry in Britton-Robinson buffer solution (pH 3.0). The limit of detection values calculated were 0.052 μmol L −1 and 0.020 μmol L −1 , respectively for HCTZ and LSNP. The sensitivity observed for individual analytical curves of HCTZ and LSNP was statistically equal to that obtained for the simultaneous analytical curves at the 95 % confidence level. The developed methods were applied to determine these drugs in environmental, biological, and pharmaceutical samples, with satisfactory results. The method's greenness was assessed demonstrating that both validated methods constitute a greener alternative to conventional analytical methods reported in the literature.

Abstract:A series of well defined calibration solutions is proposed to eliminate some of the discrepancies in the measurement of sodium, potassium and calcium in blood plasma by ion-selective electrode potentiometry. The calibration matrix has been chosen to resemble the electrolyte composition of blood plasma in terms of ionic composition and ionic strength. This reduces liquid-junction and activity coefficient bias. A flow-through test rig was constructed from sodium, potassium and calcium ion-selective electrode modules from commercial instruments and a single ceramic plug KCl reference electrode. These modules were calibrated with the proposed solutions and used in blood plasma. Preliminary experiments were performed to evaluate residual liquid-junction potentials and the effects of heparin and carbon dioxide.

Abstract:Few studies were reported on trace elements' alterations in uremic patients undergoing long-term (>24 months) hemodialysis (HD), especially by using the whole blood as the biological fluid for the measuring purpose. Our objective was to develop an improved micro-sampling inductively coupled plasma-mass spectrometry (ICP-MS) method to determine the levels of Ca, Mg, Cu, Zn, Fe, Mn, Se and Pb in uremic patients receiving long-term HD. A ICP-MS method with a modified whole blood sample preparation procedure with small volumes was established and applied for the simultaneous quantification of the various elements in uremic patients undergoing long-term HD. 124 eligible uremic patients receiving long-term HD (75 males and 49 females) and 77 healthy subjects (54 males and 23 females) were recruited and Ca, Mg, Cu, Zn, Fe, Mn, Se and Pb levels were further determined. Our results revealed that uremic patients with HD had significantly higher blood levels of Ca, Mg, Zn and Pb and lower Cu, Fe, Se and Mn concentrations than healthy controls. In conclusion, a reproducible and reliable ICP-MS method using minimal whole blood sample volume (50 μL) with a simple dilution-based preparation procedure was successfully improved, validated and applied. Uremic patients undergoing long-term HD might be at increased risk of some essential trace elements deficiency (especially for Cu, Fe and Se) or toxic trace element excess (Pb) in respect to healthy subjects. Monitoring of blood levels and supplementation of some trace elements may be indicated in uremic patients undergoing long-term HD.

Real-Time Dialysis Dose: Ionic Dialysis Versus Classical Urea Kinetic Modeling Indices

Abstract:Introduction Worldwide, over 850 million people need renal replacement therapy, and over 80% of them are on hemodialysis. The term "dialysis adequacy" is most often associated with the achievement of minimally acceptable indices - single pool Kt/V (spKt/V) and urea reduction ratio (URR%) and largely does not take into account other clinical indicators in patients. In addition, it should be taken into account that in the conditions of clinical practice, their measurement is carried out according to standards controlled by the regulatory structures and is carried out with a frequency between one and three months, and not during each dialysis procedure. Accordingly, based on the obtained value, we assume that it refers to the urea clearance of the entire dialysis prescription. A new methodology for measuring dialysis adequacy through ionic dialysis allows dose estimation in real time without the need for additional blood tests by recording the difference in sodium ion conductivity at the dialyzer inlet and outlet. The latter corresponds to the dialysis dose delivered and can be measured at each dialysis session, enabling timely therapeutic intervention at minimal cost to the community. The aim of this original article is to present ion dialysis to the community as a reliable and inexpensive alternative to classical urea kinetic modeling (UKM), evaluating the comparability of the two methods and the possible sources of error in the result. Material and methods This was a retrospective study of 32 patients undergoing hemodialysis at the Clinic of Nephrology and Dialysis at the University Hospital St. Marina (Varna, Bulgaria) for the period between January and December 2020. For each of the patients, four measurements (every three months) of the delivered dialysis dose were performed by ion dialysis and classical urea kinetic modeling in order to assess the comparability and reliability of the two methods. Results The analysis of the results proved a high correlation between the validated indicators of dialysis adequacy (spKt/V; URR) and those registered with online monitoring (online clearance monitor) by ionic dialysis - online Kt/V (onKt/V) while at the same time reporting a significant difference in the two methods - primarily based on the anthropometric formulas used to estimate the volume. The established regression models confirmed the high predictive value of ionic dialysis in relation to the actually delivered dose. Discussion Despite the high bond strength, our recorded values for onKt/V are 8% lower compared to results using UKM. Therefore, onKt/V as an assessment metric has the ability to underestimate the dialysis dose received. Various factors as a possible source of error and its cause have been reported by some authors. Several studies have reported differences of 2-5% in instantaneous conductance measurements, which they associate mainly with differences in the diffusion coefficients of urea and sodium, as well as different effects of dialysis membrane charge or inadequate ultrafiltration correction. Conclusions Our study confirms that online clearance monitoring (OCM) is a practical non-invasive tool for daily use that complements the classic performance of OCM by helping to deliver an adequate dialysis dose with increased patient benefit and minimal cost of financial resources.

Abstract:The aim of this study was to develop a high‐performance liquid chromatography–tandem mass spectrometry method for the determination of 6‐cyanodopamine, 6‐nitrodopamine, 6‐nitrodopa, 6‐nitroadrenaline and 6‐bromodopamine in human plasma samples. Strata‐X 33 μm solid‐phase extraction cartridges were used for the extraction of the catecholamines from human plasma samples. The catecholamines were separated in a 150 × 3 mm Shim‐pack GIST C18‐AQ column with 3 μm particle size, placed in an oven at 40°C and perfused with 82% mobile phase A (acetonitrile–H2O; 90:10, v/v) + 0.4% acetic acid and 18% mobile phase B (deionized H2O) + 0.2% formic acid at a flow rate of 340 μl/min in isocratic mode. The injected volume was 4 μl and the run lasted 4 min. The method was linear from 0.1 to 20 ng/ml and the lower limit of quantification was 0.1 ng/ml for all analytes. The method was applied to evaluate the plasma levels of catecholamines in plasma of patients with chronic kidney disease and allowed the detection for the first time of circulating levels of the novel catecholamines 6‐bromodopamine and 6‐cyanodopamine.

Abstract:To establish and validate a candidate reference method for determination of serum magnesium based on flame atomic absorption spectrometry(FAAS),the samples of serum magnesium diluted by 100 folds at the presence of HCl and La2O3 were measured by AA6800 atomic absorption spectrophotometer.The results showed that the intra-and inter-assay coefficients of variation were 0.41%(0.17%～0.89%) and 0.79%(0.63%～0.90%) respectively,and the total imprecision was 0.83%(0.69%～0.92%).The Bias of assay was lower than 1.5%.The recovery rates of low and high level were 100.31% and 99.70% respectively,the sensitivity of assay was 1.87×10-3 mmol/L and the limit of detection was 0.0023mmol/L.The applicable measurement range was 0.5～2.0mmol/L,and the expected uncertainty was 0.9859%.The method for determination of serum magnesium based on FAAS is very accurate and precise;it might be regarded as a candidate reference method for the determination of total magnesium in serum.

Abstract:A method for As determination in sulfur-containing active pharmaceutical ingredients (SC-APIs) by direct solid sampling graphite furnace atomic absorption (DSS-GF AAS) was developed. The proposed method was successfully applied to three SC-APIs (hydrochlorothiazide, furosemide and sulfadiazine). Palladium was used as chemical modifier as well as hydrogen during the pyrolysis allowing the direct determination of As in the SC-APIs without interferences caused by gaseous sulfur species. Sample masses (hydrochlorothiazide) from 0.4 to 3 mg were used and calibration with aqueous standard solutions was feasible. The limit of quantification was 0.033 mg g-1 and the calibration ranged from 0.1 to 1.6 ng As. Recoveries for As solutions added directly to the solid samples were between 95 and 103%, showing a good accuracy. The method validation highlighted its robustness, since variation in pyrolysis and atomization temperatures, as well as in Pd and sample masses, did not change significantly the results. Additional experiments showed that this method can be applied to other SC-APIs (as e.g., furosemide and sulfadiazine). Arsenic concentration in hydrochlorothiazide samples ranged from 0.13 to 0.48 mg g-1, while in furosemide and sulfadiazine samples it was from 0.49 and 0.54 mg g-1, respectively. The use of DSS-GF AAS does not require previous sample digestion and As could be directly determined in the solid samples providing some advantages, as lower risks of contamination and analyte losses, good accuracy and limits of quantification.

Abstract:Aderamastat (FP-025) is a small molecule, selective matrix metalloproteinase (MMP)-12 inhibitor, under development for respiratory conditions which may include chronic inflammatory airway diseases and pulmonary fibrosis. To support evaluation of the pharmacokinetic parameters of Aderamastat in humans, we developed and validated a high-performance liquid chromatography tandem mass spectrometry (LC-MS/MS) analytical method for the quantification of Aderamastat in human plasma. This assay was validated in compliance with the Food and Drug Administration (FDA) Good Laboratory Practice Regulations (GLP) and European Medicines Agency (EMA) guidelines. K2EDTA human plasma samples were spiked with internal standard, processed by liquid-liquid extraction, and analyzed using reversed-phase HPLC with Turbo Ion Spray® MS/MS detection. Separation was done using a chromatographic gradient on 5 µm C6-Phenyl 110 Å, 50*2 mm analytical column at a temperature of 35 °C. The LC-MS/MS bioanalytical method, developed by QPS Taiwan to determine the concentration of Aderamastat in K2EDTA human plasma, was successfully validated with respect to linearity, sensitivity, accuracy, precision, dilution, selectivity, hemolyzed plasma, lipemic plasma, batch size, recovery, matrix effect, and carry-over. These data indicate that the method for determination of Aderamastat concentrations in human K2EDTA plasma can be used in pharmacokinetics studies and subsequent clinical trials with Aderamastat. Sampling from Extracorporeal Circuit: A Step Forward for Dose Monitoring in Continuous Renal Replacement Therapy

Abstract:Introduction: Continuous renal replacement therapies (CRRTs) require constant monitoring and periodic treatment readjustments, being applied to highly complex patients, with rapidly changing clinical needs. To promote precision medicine in the field of renal replacement therapy and encourage dynamic prescription, the Acute Dialysis Quality Initiative (ADQI) recommends periodically measuring the solutes extracorporeal clearance with the aim of assessing the current treatment delivery and the gap from the therapeutic prescription (often intended as effluent dose). To perform this procedure, it is therefore necessary to obtain blood and effluent samples from the extracorporeal circuit to measure the concentrations of a target solute (usually represented by urea) in prefilter, postfilter, and effluent lines. However, samples must be collected simultaneously from the extracorporeal circuit ports, with the same suction flow at an unknown rate. Methods: The proposed study takes the first step toward identifying the technical factors that should be considered in determining the optimal suction rate to collect samples from the extracorporeal circuit to measure the extracorporeal clearance for a specific solute. Results: The results obtained identify the low suction rate (i.e., 1 mL/min) as an ideal parameter for an adequate sampling method. Low velocities do not perturb the external circulation system and ensure stability prevailing pressures in the circuit. Higher velocities can be performed only with blood flows above 120 mL/min preferably in conditions of appropriate filtration fraction. Discussion/conclusions: The specific value of aspiration flow rate must be proportioned to the prescription of CRRT treatments set by the clinician.
Zero Diffusive Sodium Balance in Hemodialysis Provided by an Algorithm‐Based Electrolyte Balancing Controller: A Proof of Principle Clinical Study

Abstract:Restoring and controlling fluid volume homeostasis is still a challenge in contemporary end-stage kidney disease patients treated by intermittent hemodialysis (HD) or hemodiafiltration (HDF). This primary target is achieved by ultrafiltration (dry weight probing) and control of intradialytic sodium transfer (dialysate-plasma Na gradient). The latter task is mostly ignored in clinical practice by applying a dialysate sodium prescription uniform for all patients of the dialysis center but unaligned to individual plasma sodium levels. Depending on the patient's natremia, a positive gradient gives rise to intradialytic diffusive sodium load and postdialytic thirst. On the contrary, a negative gradient may cause unwanted diffusive sodium removal and intradialytic symptoms. To overcome these challenges, a new conductivity-based electrolyte balancing algorithm embedded in a hemodialysis machine with the aim to achieve "zero diffusive sodium balance" in HD and online HDF treatments was tested in the form of a prospective clinical trial. The study comprised two phases: a first phase with a conventional fixed-sodium dialysate (standard care phase), followed by a phase with the electrolyte balancing control (EBC) module activated (controlled care phase). The results show a reduction in the variability of the intradialytic plasma sodium concentration shift, but it is overlain by a small but statistically significant increase in the mean plasma sodium levels. However, no clinical manifestations were observed. This sodium load can be explained by the design of the algorithm based on dialysate conductivity instead of sodium concentration. Furthermore, the increase in plasma sodium can be corrected by taking into account the potassium shift during the treatment. This study showed that the EBC module incorporated in the HD machine is able to automatically individualize the dialysate sodium to the patient's plasma sodium without measuring or calculating predialytic plasma levels from previous laboratory tests. This tool has the potential to facilitate fluid management, to control diffusive sodium flux, and to improve intradialytic tolerance in daily clinical practice.

Abstract:Digital microfluidics (DMF) is a platform that enables the automated manipulation of individual droplets of sizes ranging from nano- to microliter and can be coupled with numerous techniques, including colorimetry. However, although DMF electrode architecture is highly versatile, its integration with different analytical methods often requires either changes in sample access, top plate design, or the integration of supplementary equipment into the system. As an alternative to overcome these challenges, this study proposes a simple integration between paper-based analytical devices (PADs) and DMF for automated and eco-friendly sample processing aiming at the colorimetric detection of creatinine (an important biomarker for kidney disease) in artificial urine. Optimized and selective Jaffé reaction was performed on the device, and reaction products were delivered to the PAD, which was subsequently analyzed with a bench scanner. The optimal operational parameters on DMF were 45 s reaction time with circular mixing and capture of the image after 5 min. Under optimized conditions, a linear behavior was obtained for creatinine concentrations ranging from 2 to 32 mg dL-1, with limits of detection and quantitation equal to 1.4 mg dL-1 and 3.1 mg dL-1, respectively. For the concentration range tested, the relative standard deviation varied from 2.5 to 11.0%, considering four measurements per concentration. Creatinine-spiked synthetic urine samples were subjected to analysis via DMF-PAD and the spectrophotometric reference method. The concentrations of CR determined using both analytical techniques were close to the theoretical values, with the resultant standard deviations of 2-9% and 1-4% for DMF-PAD and spectrophotometry, respectively. Furthermore, the recovery values were within the acceptable range, with DMF-PAD yielding 96-108% and spectrophotometry producing 95-102%. Finally, the greenness levels of DMF-PAD and spectrophotometric methods were evaluated using the Analytical Greenness Metric software, in which 0.71 and 0.51 scores were obtained, respectively. This indicates that the proposed method presents a higher greenness level, mainly due to its miniaturized characteristic using a smaller volume of reagent and sample and the possibility of automation, thus reducing user exposure to potentially toxic substances. Therefore, the DMF-PADs demonstrated great potential for application in the clinical analysis of creatinine, aiding in routine tests by introducing an automated, simple, and environmentally friendly process.

Abstract:An antiserum specific for the major benzo(a)pyrene (BP) adduct formed with deoxyguanosine in vivo has been used by enzyme-linked immunosorbent assay to monitor the formation and removal of DNA-bound products in BALB/c mouse epidermis exposed topically to initiating doses of BP and in BALB/c mouse keratinocytes exposed in vitro to BP or its activated derivatives. In mouse epidermal DNA, formation of antibody-recognizable products increased proportionally between doses of 50 and 250 nmol of BP, giving 2.3 to 6.0 fmol/micrograms of DNA, respectively, and reached a plateau of 10 to 11 fmol/micrograms of DNA at doses between 1000 and 1500 nmol. Antibody-recognizable adducts comprised roughly one-half of the total BP-DNA binding, since a 250-nmol dose of [3H]BP yielded 6 fmol/micrograms of DNA by enzyme-linked immunosorbent assay and 12.9 fmol/micrograms of DNA by radiolabeling. Removal of trans-(7R)-N2-(10-[7 beta, 8 alpha, 9 alpha-trihydroxy-7,8,9,10-tetrahydrobenzo(a)pyrene]-yl)-deoxyguanosine adducts was monitored in epidermal DNA of mice exposed to 500 nmol of BP and, although no correction was approximated for DNA turnover in the skin, about one-half of the adducts formed by 24 hr were removed 3 days later, and only 10% remained at the end of a week. BP-DNA binding and removal were also studied in cultured mouse keratinocytes, where proliferating basal cells and terminally differentiating cells can be selectively studied by modulating the Ca2+ concentration of the medium. BP dose-response studies showed that, in cells of different maturation states, BP-DNA adduct levels were similar. Adduct formation greater than 10 to 11 fmol/micrograms (the highest obtained in vivo) was associated with extensive cytotoxicity and cell death. The kinetics of adduct removal was followed in culture under conditions in which dilution by DNA synthesis or cell loss could be monitored. Results of these experiments suggested that initial removal of BP-DNA adducts was more rapid in the differentiating population although, in both populations, 50% of the adduct was removed by 24 hr. The formation of foci resistant to Ca2+-induced terminal differentiation has been associated previously with carcinogen treatment in cultured keratinocytes. Exposure to BP or the antidiol-epoxide, at concentrations producing low cytotoxicity, yielded frequencies of differentiation-altered foci proportional to the dose of the compound used and to the number of DNA adducts formed.

Abstract:Introduction: The manual single lumen alternating micro-batch hemodiafiltration (mSLAMB) system is a closed-loop dialysis system designed to provide kidney support in emergency situations (e.g., fluid overload, hyperkalemia, acidemia). If done repeatedly in small batches and at high flow rates, this system was found to achieve clearance levels comparable to traditional renal replacement therapy (RRT). Methods: Using a porcine model, uremic toxins and exogenous fluorescent tracer concentrations were successfully lowered within just 1 hour of treatment. Results: With a maximal dialysate flow, mSLAMB can achieve decreases in serum potassium concentration of > 0.5 mmol/L/ hr. With the mSLAMB hemodiafiltration system, micro-batch processing was also successful in removing up to 250mL of ultrafiltrate in 8 cycles. Conclusion: This process could create a better fluid balance allowing for administering therapeutic fluids such as sodium bicarbonate in the clinic. Electrolyte imbalance and volume overload remain severe life-threatening emergencies in low resource settings, therefore mSLAMB should be explored further due to its modest vascular access requirements, low cost, and ability to be performed without electricity or batteries.
