Abstract:PURPOSE:To evaluate the effect of ethanol-wet/hydrophobic resin bonding (EWB) in a simplified dehydration protocol with ultrasonic agitation (UA) of etch-and-rinse adhesives on the bonding quality in root canals.MATERIALS AND METHODS:56 extracted human maxillary central incisors were divided into 4 groups (n = 14) according to the procedures of bonding fiber posts in root canals: A: conventional etch-and-rinse adhesive as control (All-Bond 3, Bisco); B: EWB in a simplified dehydration protocol, ie, after 100% ethanol dehydration (30 s, 3 times), ethanol/hydrophobic resin (50% bis-GMA + TEG-DMA, 50% ethanol) was applied into the root canal as a primer, followed by applying pure co-monomer adhesive; C: EWB in a standard dehydration protocol, ie, ascending concentrations of ethanol (50%, 70%, 80%, 90%) for 30 s and 100% ethanol (30 s, 3 times) for dehydration, with the remaining procedure conducted as in group B; D: EWB in a simplified dehydration protocol under ultrasonic agitation (UA), ie, 100% ethanol under ultrasonic agitation for 30 s, ethanol/hydrophobic resin as a primer applied under UA for 60 s, followed by the application of pure co-monomer adhesive. All the specimens were cemented with Duo-Link (Bisco) and immersed in artificial saliva for 24 h or thermocycled for 10,000 times. Bond strength, failure mode, and nanoleakage were evaluated. Hybrid layer and resin tags were also observed with SEM. Data were analyzed with two-way ANOVA (bond strength) and Kruskal-Wallis analysis (nanoleakage, resin tag score) (α = 0.05).RESULTS:Significant differences existed in bond strength (p = 0.02) and nanoleakage (p = 0.02) among groups. After 24 h and after thermocycling, group D exhibited higher bond strengths than did group B (p = 0.02 after 24 h, p = 0.03 after aging) and group A (p = 0.06 after 24 h, p = 0.04 after aging). Bond strength and nanoleakage showed no significant difference after aging (p > 0.05). There was no significant difference in resin tag scores between any of the groups, but the apical third of the roots exhibited the lowest score (p = 0.001).CONCLUSION:The synergistic effects of the two pretreatment strategies (EWB in a simplified dehydration protocol and UA) improved the bonding quality. UA had a positive effect on the adhesion of the etch-and-rinse adhesives, when EWB in a simplified procedure was applied into the root canal.
Microtensile bond strength, 4-point bending and nanoleakage of resin-dentin interfaces: Effects of two matrix metalloproteinase inhibitors

Abstract:Chronic degradation of hybrid layer collagen by matrix metalloproteinases (MMPs) jeopardizes resin-dentin interfacial integrity and limits the durability of dental restorations. The 4-point bending strength (BS) is a valid but uncommon method of testing the mechanical behavior of resin-dentin interfaces. The present study aims to analyze the influence of two matrix metalloproteinase inhibitors on microtensile bond strength (µTBS), BS and nanoleakage. A total of 48M were divided into three groups according to bonding procedure. Teeth were horizontally sectioned to produce a flat dentin surface. In the control group, etch-and-rinse Prime&Bond One (Dentsply) bonding was used; in the self-etch group, methacryloyloxydodecylpyridinium bromide (MDPB)-containing Clearfil SE Protect (Kuraray) was used; and in the benzalkonium chloride (BAC)-etch group, BAC-etchant (Bisco) was used. A Ceram.X-One (Dentsply) composite was built as three successive layers and was light-cured. Samples were sectioned to produce microrods that were randomly divided into two groups for analysis at baseline and after 6 months of water immersion (n = 32), plus one slab for nanoleakage analysis (n = 8) via scanning electron microscopy (SEM) and digital image analysis (Fiji). Data were analyzed using the Weibull distribution and a mixed-model ANOVA with a post hoc Tukey test. All groups showed deterioration of the initial bonds. The self-etch group had a worse baseline µTBS than the control but had the best BS after aging. BAC-etch did not improve bond stability of etch-and-rinse adhesive. The µTBS and BS test results after aging were moderately correlated. Mixed fractures prevailed with regard to µTBS, whereas adhesive fractures dominated with regard to BS. Nanoleakage was not eliminated in any group and increased after aging. MDPB self-etch resisted bond degradation better than etch-and-rinse adhesives, even after BAC-etching. Integrating BS in studies of µTBS and nanoleakage might provide more clinically relevant outcomes for predicting the performance of dental adhesives.
Evaluation of Intra Root Canal Er,Cr:YSGG Laser Irradiation on Prosthetic Post Adherence

Abstract:Purpose: In clinical prosthetics procedures, including endodontics and post fixation, the presence of a smear layer can reduce the post bond strength. An Er,Cr:YSGG laser, which emits at 2780 nm, can promote a smear-layer-free surface due to the ablation process. Considering these aspects, the purpose of this work was to evaluate the influence of Er,Cr:YSGG laser irradiation using either a radial or an axial fiber tip on the bond strength of three resin cements to the fiber-reinforced composite posts. Materials and methods: Ninety recently extracted single rooted human teeth had their root canal instrumented and were randomly distributed into nine experimental groups, in which three resin cements (total-etching Variolink II, self-etching Panavia F, and self-adhesive RelyX Unicem Aplicap) and three root canal treatments (no treatment, laser irradiation using the radial fiber tip, laser irradiation using the axial fiber tip) were used. Specimens were then sectioned into three sections (cervical, middle, and apical thirds) with two slices on each section. A push-out test was performed on each slice, and the values were recorded as MPa. The push-out data were analyzed by a Ryan-Joiner normality test followed by a two-way ANOVA test and Tukey pairwise comparison. The statistical analysis was performed on each third section separately, with a 5% significance level. Results: Laser irradiation with axial fiber tip significantly increased the post bond strength of RelyX Unicem Aplicap on middle third of specimens (p < 0.001) when compared to other root canal treatments (unlased or irradiated with radial tip). Considering the Panavia resin cement, laser irradiation with either axial or radial tips promoted a significant increase on the post bond strength of middle third when compared to unlased specimens (p < 0.001); however, laser irradiation did not influence the post bond strength of Variolink resin cement. Conclusions: The use of the Er,Cr:YSGG laser for clinical prosthetics procedures enhances the post bond strength of Panavia and RelyX Unicem Aplicap resin cements, mainly at middle third of roots, and does not interfere with the bond strength of Variolink resin cement. For the RelyX Unicem Aplicap system, the use of axial tip is most advantageous and can be recommended for future clinical application.
Effect of Medications for Root Canal Treatment on Bonding to Root Canal Dentin

Abstract:Use of resin-based restorative materials recently has become widely accepted for treatment of endodontically treated teeth. However, some solutions routinely used during endodontic treatment procedures may have an effect on bond strengths of adhesive materials to root canal dentin. The purpose of this in vitro study was to evaluate the effect of various medications on microtensile bond strength to root canal dentin. Fourteen extracted human single-rooted teeth were used. The crowns and the pulp tissues were removed. The root canals were then instrumented and widened to the same size. The teeth were randomly divided into seven groups of two teeth each. The root canal dentin walls of the roots were treated with 5% sodium hypochloride (NaOCI), 3% hydrogen peroxide (H2O2), the combination of H2O2 and NaOCl, or 0.2% chlorhexidine gluconate for 60 s; or calcium hydroxide or formocresol for 24 h. The teeth in control group were irrigated with water. The root canals were obturated using C&B Metabond. After 24 h of storage in distilled water, serial 1-mm-thick cross-sections were cut, and approximately 12 samples were obtained from each group. Microtensile bond strengths to root canal dentin were then measured by using an Instron machine. The data were recorded and expressed as MPa. The results indicated that NaOCI, H2O2, or a combination of NaOCl and H2O2 treatment decreased bond strength to root canal dentin significantly (p < 0.05). The teeth treated with chlorhexidine solution showed the highest bond strength values (p < 0.05). In conclusion, chlorhexidine is an appropriate irrigant solution for root canal treatment before adhesive post core applications.

Abstract:Aim: This study evaluated the effect of four intracanal medications commonly used in regenerative endodontic procedures on the bond strength of four calcium silicate-based materials, in which two are powder-to-liquid products (MTA and MTA-HP) and the other are ready-to-use materials (EndoSequence Root Repair Material Fast Set Putty [ERRM] and Bioceramic Repair [BIO-C Repair]).Methods: Ten bovine central incisors were selected and 4 slices (1.0 ± 0.1 mm) were prepared from each root. Next, four 0.8-mm wide holes were drilled in each slice and specimens were filled with one of the following intracanal medications: triple antibiotic paste, double antibiotic paste, calcium hydroxide with distilled water, and calcium hydroxide with 2% chlorhexidine gel. After 21 days, holes were filled with one of the materials: MTA, MTA-HP, ERRM, or BIO-C Repair. After storage, push-out test and failure analysis were performed. Data were submitted to analysis of variance in a 4 × 4 factorial scheme. Tukey's test was used for multiple comparisons.Results: The use of different interappointment dressings did not influence the results (P > 0.05). ERRM and BIO-C Repair presented significantly higher values than MTA and MTA-HP (P < 0.0001). Specimens showed a 100% occurrence of adhesive failures.Conclusion: The use of different intracanal medications presented similar impact on bond strength of calcium silicate-based materials. Ready-to-use ERRM and BIO-C Repair materials presented the best push-out values to dentine, whereas powder-to-liquid MTA and MTA-HP cements showed the lowest results.
Effects of pretreatment in post space on the bonding strength of self-adhesive resin cement to fiber posts

Abstract:BACKGROUND:The bonding interface of resin cements with dentin is unstables, therefore, improvement on the bonding strength is of great significance. OBJECTIVE:To evaluate the effects of different pretreatments to post space on bonding strength of the self-adhesive resin cement EmbraceCoreTM Resin Cement to fiber posts after phosphoric acid etching or self-etching. METHODS:Twenty-five human mandible canines or premolars with single-rooted canal were selected. After conventional root canal treatment and post space preparation, they were randomly divided into five groups (n=5 per group):group I (control group):0.9%normal saline irrigation;group II:etching with 35%phosphoric acid for 15 seconds;group III:self-etching with CLEARFILTM SE BOND binder for 20 seconds;group IV:etching with 35%phosphoric acid for 15 seconds and bonding with single bong 2;group V:self-etching with CLEARFILTM SE BOND binder for 20 seconds and bonding with CLEARFILTM SE BOND binder. Fiber posts were edusing self-adhesive resin cement EmbraceCoreTM Resin Cement. The thin-slice push-out test was performed to record the bonding strengths, and the failure modes of fiber posts were observed by stereomicroscope. RESULTS AND CONCLUSION:The bond strength of groups I-V were as fol ows:(8.66±2.43), (7.05±3.0), (7.37±2.83), (9.88±2.9), and (10.04±2.86) MPa. Groups II, IV and V showed significantly different bond strength from the group I (P<0.05), but there were no significant differences between groups III and I (P>0.05). The main failure models in the groups I, II and III were A1 and A2, and less B1 and B2 appeared. The failure modes in the groups IV and V also focused on the A1 and A2, but B1 and B2 increased gradual y. These results indicate that neither the pretreatments by phosphoric acid etching nor by self-etching in root canal dentin can improve the bonding strength of EmbraceCoreTM Resin Cement to fiber posts, but the use of binder relative to phosphoric acid etching or self-ctching is helpful.
Influence of the Dentin Smear Layer Trait on the Bond Strength of the Self-Adhesive Resin Cements

Abstract:OBJECTIVE:This work aimed to evaluate the influence of smear layer on the bonding effectiveness and durability of the self-adhesive resin cements to dentin. METHODS:A total of 48 fresh caries-free third molars with exposed dentin surface were divided into two groups. The dentin surfaces were treated using a standard grit diamond bur (group A) or further polished using a fine grit diamond bur (group B) and then bonded with either of the two self-adhesive resin cements, namely, Clearfil SA Cement (CSA, Kuraray) and Multilink Speed (MS, Ivoclar Vivadent). After 24 h or 2-year water storage, a microtensile bond strength test was performed. RESULTS:In group A, the dentin surface was rough, the smear layer was thick, and the dentin tubule orifice detritus showed low embolism value. In group B, the dentin surface roughness decreased, the embolism proportion increased, and the smear layer became thin. The initial bonding strengths of CSA and MS in group A were significantly lower than those in group B (P<0.05). The bonding strengths of CSA and MS significantly decreased after two years of water storage (P<0.05). For CSA, the bond strength of group B was significantly lower than that of group A (P<0.05). For MS, the differences in the bonding strength between groups A and B were insignificant (P>0.05). CONCLUSIONS:The properties of the smear layer and the types of self-adhesive resin cement used affected the bond strength and durability.
Effects of Etching Time and Ethanol Wet Bonding on Bond Strength and Metalloproteinase Activity in Radicular Dentin

Abstract:(1) Background: The objective of this in vitro study was to evaluate the impact of different etching times and ethanol pre-treatments on the immediate bond strength of a hydrophilic multi-mode universal adhesive (Clearfil Universal Bond Quick, Kuraray, UBQ) and on the consequent gelatinolytic activity of metalloproteinases (MMPs) on radicular dentin. (2) Methods: Sixty single-root teeth were selected and divided into four groups according to the adhesive protocol applied for fiber post cementation: (G1) 15 s H3PO4 application + UBQ; (G2) 30 s H3PO4 application + UBQ; (G3) 15 s H3PO4 application + ethanol pre-treatment + UBQ; (G4) 30 s H3PO4 + ethanol pre-treatment + UBQ. After adhesive procedures, fiber posts were luted into the post space with a dual-curing cement (DC Core, Kuraray) and light-cured for 40 s. To perform the push-out test and nanoleakage analyses for both coronal end apical areas, 1 mm slices were prepared, following a 24 h storage period in artificial saliva. Additionally, an in situ zymographic assay was conducted to explore endogenous MMP activity within the radicular layer. Results were statistically analyzed with ANOVA and Tukey post hoc tests. Statistical significance was set at p < 0.05. (3) Result: ANOVA revealed a statistically significant difference in push-out bond strength related to the pre-treatment variable but did not highlight any significance of etching time. Specimens pre-treated with ethanol wet bond application showed higher bond strength (p < 0.01). In situ zymography quantification analyses revealed that all tested groups, independently of etching time end ethanol pre-treatment, activated MMP gelatinolytic activity. A significant increase in MMP activity was detected for the 30 s etching time. However, ETOH pre-treatment significantly reduced MMP activity within the adhesive interface (p < 0.01). (4) Conclusions: The tested adhesive showed similar results regardless of the etching time protocol. The gelatinolytic activity of MMPs was observed in all the groups. Further investigations and extended follow-ups are required to validate the results of the present study in vivo.

Abstract:The aim of this study is to analyze the effects of different endodontic irrigants and adhesive systems on the resin bond strength of fiber post cementation. In total, 144 single-rooted, unrestored human teeth were endodontically treated and randomly divided into 12 groups according to four endodontic irrigants (distilled water as control; EDTA 17%; NaOCl 5%; chlorhexidine digluconate 2%) and three different adhesive/resin cement systems (etch-and-rinse: orthophosphoric acid, Parabond® A+B/Paracore®; self-etch: ParaBond® Non-Rinse Conditioner, Parabond® A+B/Paracore®; Universal: ClearfilTM Universal Bond/ClearfilTM DC Core Plus). Forty-eight hours after post cementation, ten teeth from each group were cross-sectioned into three discs (cervical, middle and apical regions). Thirty specimens of each group (n = 30) were submitted to a push-out test at a crosshead speed of 1 mm/min. The remaining two teeth of each group were sectioned in the same manner, and the resin–dentin interface was evaluated using scanning electron microscopy (SEM). The results were statistically analyzed with the ANOVA test and Tukey's test (p 0.01). The combination of NaOCl 5% and ClearfilTM Universal Bond reduced the adhesive strength (p < 0.01). The NaOCl 5%, in relation to other irrigants, significantly decreased the push-out bond strength.

Comparing Various Resin Luting Cement Systems in Different Etching Modes Through Bond Durability and Morphological Features

Abstract:SUMMARY Objectives This study aimed to investigate bond performance of various resin luting cement (RLC) systems on enamel and dentin in different etching modes and to compare the RLC-tooth interface morphology using scanning electron microscopy (SEM). Methods and Materials The self-adhesive RLC systems used in combination with universal adhesives were as follows: Scotchbond Universal Adhesive Plus + RelyX Universal (3M Oral Care) and Clearfil Universal Bond Quick ER + SA Luting Multi (Kuraray Noritake Dental). These RLC systems were also used alone as self-adhesive RLC systems without universal adhesives (self-adhesive mode). The conventional RLC systems for comparison materials were as follows: BeautiBond Xtream + ResiCem EX (Shofu) and Tooth Primer + Panavia V5 (Kuraray Noritake Dental). Twelve specimens were prepared for each group to measure shear bond strength (SBS) in different etching modes. A stainless-steel rod was used as a substitute for indirect restorations. Bonded specimens were allocated to baseline (stored for 24 hours) and artificially aged (thermocycling [TC] for 10,000 cycles) groups. Representative treated tooth surfaces and RLC-tooth interfaces were observed using SEM. Results Three-way analysis of variance (ANOVA) revealed that all the factors (etching mode, storage period, and RLC system) had a significant influence on the enamel SBS values (p<0.05). Enamel SBS was significantly higher in etch-&-rinse (ER) mode than in self-etch (SE) mode, regardless of RLC system or storage period. Three-way ANOVA showed that all the factors had a significant influence on the dentin SBS values (p<0.001). Most RLC systems showed significantly higher dentin SBS in SE mode than in ER mode, regardless of storage period. However, the combination with Scotchbond Universal Adhesive Plus and RelyX Universal showed no significant difference in SBS values between etching modes at the baseline and showed a significantly higher SBS value in ER mode than in SE mode after TC. Conclusions The self-adhesive RLC systems with universal adhesives tended to show higher enamel and dentin bond performance than the self-adhesive RLC systems alone. The morphology of treated tooth surfaces and resin cement-tooth interfaces were dependent on the RLC system and etching mode used. The RLC systems with primer application showed a thin, high-density layer above the intact dentin in both etching modes, suggesting chemical interaction.

[Tone recognition ability for post-lingually deafened cochlear implant users using pitch information].

Abstract:OBJECTIVE The present study was aimed to assess the ability of tone recognition performance for postlingually deafed cochlear implanted adults using no-duration tone test materials. METHOD Sixteen cochlear implanted users have used their devices at least six months. Ten monosyllabic syllables were selected, and combined with four lexical tones which were made up of 40 tone tokens. Then, these original words were recorded using one adult male and one adult female native Beijing Mandarin speaker. The speakers were asked to record these 40 monosyllabic words multiple times, and the 80 tokens in which the durations of four tones in each monosyllabic word were within 5 ms precision were chosen as the original tone tokens. The subjects were asked to perform a four alternative forced-choice study and select the tone they had heard. RESULT The mean score of lexical tone perception tasks for 16 cochlear implanted users is (70.7 ± 22.0)%, and Tone 4 was the easiest to perceive, while Tone 2 was the hardest for cochlear implanted subjects to perceive. Tone 1 was more often misperceived as Tone 2 relative to other tones. Tone 2 was more often misperceived as Tone 1 and 3. Tone 3 was more often misperceived as Tone 2. The tone perception performance was positively correlated with duration of hearing aid use. CONCLUSION Duration cues may impact on the ability of tone recognition performance for cochlear implant subjects, especially for tone 3. Due to the limited ability for cochlear implant subjects on pitch perception, tone l and tone 2, and tone 2 and tone 3 are most often confused with each other without duration cues. The experience of hearing aid use may benefit for postlingual deafness cochlear implant recipients on tone perception performance.
In vitro remineralization of hybrid layers using biomimetic analogs

Abstract:Resin-dentin bond degradation is a major cause of restoration failures. The major aim of the current study was to evaluate the impact of a remineralization medium on collagen matrices of hybrid layers of three different adhesive resins using nanotechnology methods. Coronal dentin surfaces were prepared from freshly extracted premolars and bonded to composite resin using three adhesive resins (FluoroBond II, Xeno-III-Bond, and iBond). From each tooth, two central slabs were selected for the study. The slabs used as controls were immersed in a simulated body fluid (SBF). The experimental slabs were immersed in a Portland cement-based remineralization medium that contained two biomimetic analogs (biomineralization medium (BRM)). Eight slabs per group were retrieved after 1, 2, 3, and 4 months, respectively and immersed in Rhodamine B for 24 h. Confocal laser scanning microscopy was used to evaluate the permeability of hybrid layers to Rhodamine B. Data were analyzed by analysis of variance (ANOVA) and Tukey's honest significant difference (HSD) tests. After four months, all BRM specimens exhibited a significantly smaller fluorescent area than SBF specimens, indicating a remineralization of the hybrid layer (P≤0.05). A clinically applicable biomimetic remineralization delivery system could potentially slow down bond degradation.
Evaluation of Sealer Remnants Using a Bioceramic Sealer Single-cone Technique after Post Space Preparation and Its Influence on the Adhesion of Fibre Posts in Vitro.

Abstract:Objective: To compare calcium silicate-based endodontic sealer and epoxy resin-based sealer remnants on root canal walls after post space preparation and their influence on the bond strength offibre posts fixed with a dual-cured resin cement. Methods: Thirty-six extracted single-root mandibular premolars were instrumented and divided randomly into two equal groups with different endodontic sealers. iRoot SP (Innovative BioCeramix, Vancouver, Canada) was employed in the experimental group and All Plus observed under an endodontic microscope after root canal therapy and post space preparation. Fibre posts were fixed with dual-cured resin cement. Specimens were taken at each third of the post space. The push-out bond strength was measured using a universal testing machine and fracture modes were assessed. Statistical analysis was performed using an independent samples t test and one-way analysis of variance. Results: There was no statistically significant difference in bond strength offibre posts between the control and experimental group (P > 0.05); however, sealer remnants were observed in 38.9% of the samples treated with iRoot SP and none of the samples treated with AH Plus. The major fracture mode in samples treated with iRoot SP was adhesive failure between the resin cement and the post, and no adhesive failure between the resin cement and dentine occurred at the site of the sealer remnant. The presence of iRoot SP remnants on the root canal walls after post space preparation did not interfere with bonding. Conclusion: iRoot SP is a viable option for root canal obturation before fibre post cementation.
Influence of Initial Root Canal Treatment on Intratubular Penetrability and Bond Strength of Nonsurgical Retreatment: An InVitro Study

Abstract:Introduction: To assess the effect of combinations of two different endodontic sealers used in initial and endodontic retreatment on the bond strength of the secondary obturation and the penetrability of the sealers. Methods: Forty-eight mandibular premolars were used, receiving standardized endodontic access and biomechanical preparation. Twenty-four teeth received AH Plus sealer (AHP) in primary obturation and the others received Bio-C Sealer (BCS). Retreatment protocol was performed with an R50 instrument. The samples were further subdivided into four groups (n = 12) based on the combination of primary/secondary obturation sealers: AHP/AHP; AHP/BCS; BCS/AHP; and BCS/BCS. Four samples from each subgroup received the addition of fluorophores to the sealer for penetrability analysis using laser scanning confocal fluorescence microscopy. The root portion on the 8 push-out samples was sectioned into 6 slices of 1.0 mm. Bond strength (BS) was assessed using a universal testing machine until displacement of the filling mass. Failure pattern was evaluated under a stereomicroscope (20× magnification). BS data were analyzed using two-way analysis of variance followed by Tukey's test (P < .05), and the association between the failure pattern and BS value was assessed using the chi-square test (P < .05). Penetrability was qualitatively evaluated. Results: The highest BS values were observed in the AHP/AHP (4.54 ± 1.5 MPa) and BCS/AHP (5.00 ± 1.0 MPa) groups (P < .05), with a higher percentage of adhesive failures to the filling material for all groups. Laser scanning confocal fluorescence microscopy images indicated greater penetrability of AHP compared to BCS, both in initial treatment and retreatment. Conclusion: AHP sealer exhibited higher BS and greater penetrability compared to BCS sealer.
A Comparative Study of the Repair Bond Strength of New Self-Adhesive Restorative Materials With a Resin Composite Material

Abstract:Aim: The aim of the present study is to compare the repair bond strengths (RBSs) of Cention-N (light-cure and self-cure modes), Equia Forte HT Fil and a nanohybrid resin composite. Materials and methods: Equia Forte HT Fil (GC, Tokyo, Japan), Cention-N (Ivoclar Vivadent, Schaan, Liechtenstein) and Filtek Z550 (3M ESPE, St Paul, MN, USA) were used in this study. Equia Forte HT Fil (EQF), Cention-N self-cure (CSC), Cention-N light-cure (CLC) and Filtek Z550 (Z550) groups were formed. A total of 40 samples were prepared; 10 samples in each group (n = 10). After the polymerization was completed according to the manufacturer's instructions, the samples were polished with OptiDisc (Kerr Corporation, Orange, USA) for 5 s each, from extra-course to extra-fine. After all samples were stored in 37°C water for 24 h, 10,000 cycles of brushing simulator and thermal cycles were applied to the samples. The samples were prepared in accordance with the selected repair protocol, and microshear bond strength (µSBS) test was performed. Fracture analysis on debonded surfaces was visualized by scanning electron microscopy. The conformity of the data to normal distribution was analyzed by the Shapiro-Wilk test. Multiple comparisons were performed using Dunn's test. Results: Z550 showed significantly higher µSBS as compared to the other three groups. There is no difference between CSC, CLC and EQF. Conclusion: The use of Cention-N's self-cure or light-cure mode did not affect the RBS values. The RBS values of Cention-N and Equia Forte HT Fil materials are lower than those of the composite resin material.
Effect of dentin biomodification techniques on the stability of the bonded interface

Abstract:AIM: The aim of this study is to evaluate the effect of different bonding techniques ethanol wet bonding and dimethyl sulfoxide (DMSO) wet bonding and a novel collagen cross-linker Quercetin application on the durability of resin-dentin bond and observe the bonded interface under the scanning electron microscope (SEM).MATERIALS AND METHODS: For shear bond strength testing, flat coronal dentin surfaces were prepared on 110 extracted human molars. Teeth were randomly divided into five experimental groups according to different surface pretreatments techniques. Group A was control group without any surface pretreatment. In Group B, ethanol wet bonding pretreatment was done before the application of adhesive. In Group C, DMSO wet bonding was done before the application of adhesive and in Groups D and E, Quercetin along with ethanol and Quercetin along with DMSO pretreatment, respectively, were done before adhesive application. Composite restorations were placed in all the samples. Twenty samples from each group were subjected to immediate and delayed (9 months) shear bond strength evaluation. In addition, two samples per group were subjected to the scanning electron microscopic analysis for the observation of resin-dentin interface.STATISTICAL ANALYSIS: Data collected were subjected to the statistical analysis using the one-way analysis of variance and post hoc Tukey's test at a significance level of P < 0.05.RESULTS: Dentin pretreatment with all the techniques resulted in significantly higher resin-dentin bond strength after 9 months storage with DMSO group showing the highest bond strength values.CONCLUSION: It can be concluded that these biomodification techniques can improve the durability of the resin-dentin bond.

Abstract:Objectives : The aim of this study was to compare the dentin adhesion of bulk-fill composites in high C-factor class I-cavities before and after thermocycling to a control group using incremental layering technique. Methods : A standardized class I-cavity was prepared into 195 human molars, then different universal adhesives were applied either in self-etch or etch & rinse mode, and the cavity was filled according to each materials application protocol. The material combinations used were a conventional layered composite as control, the respective bulk-fill product, two other bulk-fill composites made by different manufacturers, with one of them being tested using two different polymerization times. Furthermore, one thermoviscous bulk-fill composite and one self-adhesive restorative were examinend of which the latter can only be applied in self-etch mode. In each group the dentin adhesion to the cavity bottom was measured using microtensile bond strength test initially (24h water storage) and after thermocycling. All results were statistically analyzed using STATA 17.0. Results : The statistical analysis showed significant differences between the control and the experimental groups (p < 0.001). The highest mean bond strength before (14.8 ± 10.7 MPa) and after aging (14.2 ± 11.5 MPa) was measured for the etch & rinse-control group. Among the bulk-fill groups, the etch & rinse technique consistently showed higher bond strengths. Bond strength of groups with shortened polymerization did not exceed 2.1 MPa. The bond strength of the self-adhesive restoration material was low before and after thermocycling (2.7 MPa/ 0.0 MPa). Groups with low bond strength values showed a high number of pre-testing-failures. Conclusions : Bulk-fill materials used in high C-factor class I-cavities showed lower bond strength during self-etch application. The same applies for a shortened polymerization regime, which cannot be recommended for high C-factor cavities. Clinical significance : Today, a large variety of materials and application techniques can be used when placing an adhesive restoration. Whether new instead of established procedures should be applied in high C-factor cavities has to be critically assessed, as they are a demanding scenario for adhesive restorations.

Abstract:Objectives of the study were to investigate biomechanical properties of severely compromised premolars restored with composite restorations using finite element analysis (FEA), and in vitro fracture resistance test. A 3-D model of an endodontically treated premolar was created in Solidworks. Different composite restorations were modelled (direct restoration-DR; endo-crown-EC; post, core, and crown-C) with two different supporting tissues: periodontal ligament/alveolar bone (B), and polymethyl methacrylate (PMMA). Models were two-point axially loaded occlusally (850 N). Von Mises stresses and strains were calculated. The same groups were further tested for static fracture resistance in vitro (n = 5, 6.0 mm-diameter ball indenter, vertical load). Fracture resistance data were statistically analyzed ( p < 0.050). The highest stresses and strains in all FEA models were observed on occlusal and vestibular cervical surfaces, corresponding to fracture propagation demonstrated in vitro. C showed the lowest stress in dentin, while EC showed lower stresses and strains in crown cement. B models demonstrated larger high stress areas in the root than PMMA models. No significant differences in fracture resistance (N) were observed between groups (DR: 747.7 ± 164.0, EC: 867.3 ± 108.1, C: 866.9 ± 126.3; p = 0.307). More conservative restorations seem a feasible alternative for endodontically treated premolars to conventional post-core-crown.

Abstract:The success of dental restorations depends mainly on the ability to bond to other filling materials and tooth substances, in order to resist the multitude of forces acting on the bond within the oral cavity. Although the shortcomings of composite resins have been significantly reduced over the past three decades, microleakage due to shrinkage under masticatory loads is unavoidable. In order to overcome such problems, two materials laminated with matched properties can be used to achieve optimum results. The sandwich technique is an approach in which dentine is replaced by glass ionomer cement (GIC), and enamel is replaced by composite resin. In the past, numerous materials have been proposed with adequate properties to be used in this manner, but the results are conflicting in terms of bonding to the various forms of GIC, and the appearance of microcracks or gap formation during functional loading. This study aimed to evaluate the shear bond strength (SBS) and mode of failure between the following core materials: composite resins (CR) (Methacrylate Z350TM, Ceram XTM, and SpectrumTM) with a base material of glass ionomer cement (GIC, Ketac MolarTM). Eight samples were made with the help of polytetrafluoroethylene sheets (TEFLON, Wilmington, DE, USA). Each sheet consisted of holes which were 4 mm in diameter and 2 mm in thickness. The combination of materials was sandwiched. The samples were stored in distilled water and then placed in an incubator for 24 h in order to ensure complete polymerization. The samples were thermocycled for 500 cycles between 5–55 °C/ 30 s. Following thermocycling, SBS testing was performed using a universal testing machine. Additionally, scanning electron microscopy (SEM) was performed on representative samples for the bond failure analysis between GIC and the composite resins. The Ceram-XTM nanocomposite showed significantly higher bond strength than Methacrylate Z350TM or SpectrumTM (p = 0.002). The Methacrylate Z350TM and the SpectrumTM composite specimens demonstrated a similar SBS (p = 0.281). The SBS of the Ceram XTM to GIC was the highest compared to Methacrylate Z350TM and SpectrumTM. Therefore Ceram XTM may produce a better bond with GIC, and may protect teeth against recurrent caries and failure of the restoration. Methacrylate Z350TM is comparable to SpectrumTM CR and can be used as an alternative. A combination of adhesive and mixed failure was observed in Methacrylate Z350TM CR and GIC, while adhesive failure was predominantly found in both Ceram XTM and SpectrumTM with GIC restorations.

Abstract:Background Hybrid ceramics bonding with the dentinal substrate is crucial for clinical success and longevity. To enhance adhesion, the surface of ceramic restorations is modified through various conditioning techniques Aim Effect of Different Surface Conditioners Erbium-doped yttrium aluminum garnet (ER: YAG) laser and Low-level laser therapy (LLLT) activated Methylene blue (MB) on the surface roughness (Ra) and shear bond strength (SBS) of Polymer infiltrating ceramic network (PICN) discs bonded using different viscosity resin cement before and after thermal aging. Material and Methods One hundred and fifty human central incisors and one hundred and fifty-three PICN discs were prepared. PICN discs were randomly allocated into three groups based on the surface conditioning(n=51) Group 1:10% HF acid-S, Group 2: LLLT (MB), and Group 3: Er: YAG laser. Following conditioning Ra scores of ten samples were performed. Surface topography of samples was performed using a scanning electron microscope (SEM). Bonding of forty PICN discs from each conditioning group was performed with high (A) or low viscosity (B) dual-curing resin cement (n= 20 each). Each subgroup was divided into two cohorts and subjected to varying storage conditions. The SBS test and failure mode analysis were performed using a universal testing machine and stereomicroscope. Descriptive statistics of SBS and Ra for each group were analyzed using ANOVA and Tukey post hoc tests. HF acid-S attained the highest Ra scores. Results Group 1A (HF-S+HIGH) samples achieved the highest SBS values at baseline. Group 3A, on the other hand, displayed the lowest bond score (Er: YAG laser+HIGH) after thermal aging Intergroup comparison analysis at baseline unveiled that Group 1A and Group 1B (HF(S) + LOW) displayed no significant difference in their bond strength scores (p>0.05). Following artificial aging, it was observed that Group 2A (LLLT (MB) + HIGH) and Group 3A (Er:YAG laser + HIGH) ) presented comparable SBS(p>0.05). Thermal aging decreased SBS significantly in all groups. PICN conditioned with HF-S results in high surface roughness. Conclusion PICN conditioned with HF acid with silane and bonded with low-viscosity resin cement to the dentinal substrate is preferable. Aging influences SBS.
