Abstract:<h3>Purpose/Objective(s)</h3> To evaluate the feasibility and safety of creating radiation treatment plans based on a convolutional neural network auto-contouring (AC) model for patients receiving radiation for localized prostate cancer with a radio-opaque rectal spacer hydrogel. <h3>Materials/Methods</h3> The deep learning model produces ACs for target volumes (prostate, proximal seminal vesicle), OARs (rectum, bladder, penile bulb, femoral heads), and rectal hydrogel spacers. ACs were compared to manual contours (MC) by MDs for clinical use. Individual volumes and overall performance (composite score) were previously evaluated and reported, qualitatively using a 1 (minor discrepancy), 2 (moderate discrepancy), 3 (significant discrepancy), and 4 (rejected, gross error) scale and quantitatively using Dice Similarity Coefficient (DSC) and mean distance to agreement. VMAT treatment plans were created on unedited AC volumes from five composite 1 (C1) cases and five composite 2 (C2) cases and dose volume metrics were computed for the final MCs. Dosimetry comparison was performed to assess the safety of delivering a treatment plan created from unedited ACs. Cases were planned to 50.4 Gy in 28 fractions to the SVs, with a SIB of 70 Gy the prostate. PTV expansions were CTV+5 mm (CTV+4 mm posteriorly). <h3>Results</h3> Of 62 initial cases evaluated, 11 were scored as a C1, 28 were scored as a C2, and 23 received a C3. 10 cases were randomly selected for treatment planning. For prostate (CTV P) in selected C1 and C2 cases, mean score was 1 and 1.8 and mean DSC was 0.919 and 0.876, respectively. Clinical goals for planning and dosimetric data from treatment plans are shown in Table 1. The mean V70 Gy for prostate CTV/PTV was 99.83%/90.05% for C1 cases and 99.46%/89.15% for C2 cases. All clinical goals for OARs were met in both cohorts. <h3>Conclusion</h3> 63% of all cases evaluated received a C1 or C2 score. Our results indicate the treatment plans generated from unedited ACs on these cases respected all clinical goals for OARs. Although C1 plans produced better PTV coverage than C2, almost all plans had sub-optimal PTV coverage, indicating the need for MDs to edit target volumes before treatment planning.

Abstract:Purpose: To quantify interfractional anatomical variations and their dosimetric impact during the course of fractionated proton therapy (PT) of prostate cancer and to assess the robustness of the current treatment planning techniques.Methods: Simulation and daily in-room CT scans from ten prostate carcinoma patients were analyzed. PT treatment plans (78 Gy in 39 fractions of 2 Gy) were created on the simulation CT, delivering 25 fractions to PTV1 (expanded from prostate and seminal vesicles), followed by 14 boost fractions to PTV2 (expanded from prostate). Plans were subsequently applied to daily CT, with beams aligned to the prostate center in the sagittal plane. For five patients having a sufficiently large daily imaging volume, structure contours were manually drawn, and plans were evaluated for all CT sets. For the other five patients, the plans were evaluated for six selected fractions. The daily CT was matched to the simulation CT through deformable registration. The registration accuracy was validated for each fraction, and the three patients with a large number of accurately registered fractions were used for dose accumulation.Results: In individual fractions, the coverage of the prostate, seminal vesicles, and PTV1 was generally maintained at the corresponding prescription dose. For PTV2, the volume covered by the fractional prescription dose of 2 Gy (i.e., V2) was, on average, reduced by less than 3% compared to the simulation plan. Among the 225 (39 x 5 + 6 x 5) fractions examined, 15 showed a V2 reduction larger than 5%, of which ten were caused by a large variation in rectal gas, and five were due to a prostate shift in the craniocaudal direction. The fractional dose to the anterior rectal wall was found to increase for one patient who had large rectal gas volume in 25 of the 39 fractions, and another who experienced significant prostate volume reduction during the treatment. The fractional bladder dose generally increased with decreasing fullness. In the total accumulated dose for the three patients after excluding a few fractions with inaccurate registration due to a large amount of rectal gas (a condition inconsistent with RTOG protocol), 98.5%, 96.6%, and 98.2% of the PTV2 received the prescription dose of 78 Gy. The V75 and V70 of the anterior rectal wall and bladder both remained within tolerance.Conclusions: The results confirm that the PT planning techniques and dose constraints used at our institution ensure that target coverage to the prescription dose is maintained in the presence of interfractional anatomical variations. Dose coverage in individual fractions can be compromised, and normal tissue dose increased, due to deviations in the bladder and rectal volume compared to the simulation plans or progressive changes in the prostate volume during the treatment. Abstract:Purpose: This study evaluated the first clinical implementation of daily iterative cone beam computed tomography (iCBCT)-guided online adaptive radiation therapy (oART) in the postoperative treatment of endometrial and cervical cancer. Methods and materials: Seventeen consecutive patients treated with daily iCBCT-guided oART were enrolled in this prospective study, with a reduced uniform 3-dimensional PTV margin of 5 mm. Treatment plans were designed to deliver 45 or 50.4 Gy in 1.8 Gy daily fractions to PTV. Pre- and posttreatment ultrasound and iCBCT scans were performed to record intrafractional bladder and rectal volume changes. The accuracy of contouring, oART procedure time, dosimetric outcomes, and acute toxicity were evaluated. Results: The average time from first iCBCT acquisition to completion of treatment was 22 minutes and 26 seconds. During this period, bladder volume increased by 44 cm3 using iCBCT contouring, whereas rectal volume remained stable (62.9 cm3 pretreatment vs 61.9 cm3 posttreatment). A total of 91.6% of influencers and 88.1% of CTVs required no or minor edits. The adapted plan was selected in all (434) fractions and significantly improved the dosimetry coverage for CTV and PTV, especially the vaginal PTV coverage by nearly 7% (P < .05). The adapted bladder Dmean was 104.61 cGy, and the rectum Dmean was 123.67 cGy, significantly lower than the scheduled plan of 108.24 and 128.19 cGy, respectively. The bone marrow and femur head left and right dosimetry were also improved with adaptation. Grade 2 acute gastrointestinal and genitourinary toxicities were 24% and 0, respectively. There was a grade 3 acute toxicity of decreased white blood cell count in 1 patient. Conclusions: Daily oART was associated with favorable dosimetry improvement and low acute toxicity, supporting its safety and efficacy for postoperative treatment of endometrial and cervical cancer. These results need to be validated in a larger prospective randomized controlled cohort.

Abstract:Importance: Patients with newly diagnosed locally advanced cervical carcinomas or recurrences after surgery undergoing radiochemotherapy whose tumor is unsuited for a brachytherapy boost need high-dose percutaneous radiotherapy with small margins to compensate for clinical target volume deformations and set-up errors. Cone-beam computed tomography-based online adaptive radiotherapy (ART) has the potential to reduce planning target volume (PTV) margins below 5 mm for these tumors. Objective: To compare online ART technologies with image-guided radiotherapy (IGRT) for gynecologic tumors. Design, setting, and participants: This comparative effectiveness study comprised all 7 consecutive patients with gynecologic tumors who were treated with ART with artificial intelligence segmentation from January to May 2022 at the West German Cancer Center. All adapted treatment plans were reviewed for the new scenario of organs at risk and target volume. Dose distributions of adapted and scheduled plans optimized on the initial planning computed tomography scan were compared. Exposure: Online ART for gynecologic tumors. Main outcomes and measures: Target dose coverage with ART compared with IGRT for PTV margins of 5 mm or less in terms of the generalized equivalent uniform dose (gEUD) without increasing the gEUD for the organs at risk (bladder and rectum). Results: The first 10 treatment series among 7 patients (mean [SD] age, 65.7 [16.5] years) with gynecologic tumors from a prospective observational trial performed with ART were compared with IGRT. For a clinical PTV margin of 5 mm, IGRT was associated with a median gEUD decrease in the interfractional clinical target volume of -1.5% (90% CI, -31.8% to 2.9%) for all fractions in comparison with the planned dose distribution. Online ART was associated with a decrease of -0.02% (90% CI, -3.2% to 1.5%), which was less than the decrease with IGRT (P < .001). This was not associated with an increase in the gEUD for the bladder or rectum. For a PTV margin of 0 mm, the median gEUD deviation with IGRT was -13.1% (90% CI, -47.9% to 1.6%) compared with 0.1% (90% CI, -2.3% to 6.6%) with ART (P < .001). The benefit associated with ART was larger for a PTV margin of 0 mm than of 5 mm (P = .004) due to spreading of the cold spot at the clinical target volume margin from fraction to fraction with a median SD of 2.4 cm (90% CI, 1.9-3.4 cm) for all patients. Conclusions and relevance: This study suggests that ART is associated with an improvement in the percentage deviation of gEUD for the interfractional clinical target volume compared with IGRT. As the gain of ART depends on fractionation and PTV margin, a strategy is proposed here to switch from IGRT to ART, if the delivered gEUD distribution becomes unfavorable in comparison with the expected distribution during the course of treatment.
Forecasting patient-specific dosimetric benefit from daily online adaptive radiotherapy for cervical cancer

Abstract:Objective: Adaptive Radiotherapy (ART) is an emerging technique for treating cancer patients which facilitates higher delivery accuracy and has the potential to reduce toxicity. However, ART is also resource-intensive, requiring extra human and machine time compared to standard treatment methods. In this analysis, we sought to predict the subset of node-negative cervical cancer patients with the greatest benefit from ART, so resources might be properly allocated to the highest-yield patients. Approach: CT images, initial plan data, and on-treatment Cone-Beam CT (CBCT) images for 20 retrospective cervical cancer patients were used to simulate doses from daily non-adaptive and adaptive techniques. We evaluated the correlation (R 2 ) between dose and volume metrics from initial treatment plans and the dosimetric benefits to the Bowel V 40Gy , Bowel V 45Gy , Bladder D mean , and Rectum D mean from adaptive radiotherapy using reduced 3mm or 5mm CTV-to-PTV margins. The LASSO technique was used to identify the most predictive metrics for Bowel V 40Gy . The three highest performing metrics were used to build multivariate models with leave-one-out validation for Bowel V 40Gy . Main Results: Patients with higher initial bowel doses were correlated with the largest decreases in Bowel V 40Gy from daily adaptation (linear best fit R 2 =0.77 for a 3mm PTV margin and R 2 =0.8 for a 5mm PTV margin). Other metrics had intermediate or no correlation. Selected covariates for the multivariate model were differences in the initial Bowel V 40Gy and Bladder D mean using standard versus reduced margins and the initial bladder volume. Leave-one-out validation had an R 2 of 0.66 between predicted and true adaptive Bowel V 40Gy benefits for both margins. Significance: The resulting models could be used to prospectively triage cervical cancer patients on or off daily adaptation to optimally manage clinical resources. Additionally, this work presents a critical foundation for predicting benefits from daily adaptation that can be extended to other patient cohorts.

Abstract:Introduction: This study aimed to evaluate the effect of image-guided radiation therapy (IGRT) on the margin between the clinical target volume (CTV) and planning target volume (PTV) in lung cancer. Methods: The CTV and PTV margin were determined in three dimensions by four radiation oncologists using a standard method in 10 lung cancer patients, and compared to consensus values. Transfer error was measured using a rigid phantom containing gold markers. Systematic error (Sigma(setup)) and random error (sigma(setup)) set up errors were calculated in three dimensions from pre-treatment and post-treatment cone beam CT scans. Finally, the margin between the CTV and PTV was corrected for set up error and calculated. Results: The margins between the CTV and PTV with IGRT (and without IGRT) were 0.88 cm (0.96 cm), 0.99 cm (1.08 cm) and 1.28 cm (1.82 cm) in the anterior and posterior (AP), left and right (LR) and superior and inferior (SI) directions, respectively. Images from two other patients verified the validity of the corrected margin. The target delineation errors of the radiation oncologists are considered to be the largest compared with the set up errors. The application of IGRT reduced the set up errors and the margins between CTV and PTV. Conclusions: The delineation errors of radiation oncologists are the most important factor to consider for the margin between CTV and PTV for lung cancer. IGRT can reduce the margins by reducing the set up errors, especially in the SI direction. Further research is required to assess whether the reduction in the margin is solely based on set up errors.

Abstract:Purpose This study simulates a novel prostate SBRT intra-fraction re-optimization workflow in MRIgART to account for prostate intra-fraction motion and evaluates the dosimetric benefit of reducing PTV margins. Materials and methods VMAT prostate SBRT treatment plans were created for 10 patients using two different PTV margins, one with a 5 mm margin except 3 mm posteriorly (standard) and another using uniform 2 mm margins (reduced). All plans were prescribed to 36.25 Gy in 5 fractions and adapted onto each daily MRI dataset. An intra-fraction adaptive workflow was simulated for the reduced margin group by synchronizing the radiation delivery with target position from cine MRI imaging. Intra-fraction delivered dose was reconstructed and prostate DVH metrics were evaluated under three conditions for the reduced margin plans: Without motion compensation (no-adapt), with a single adapt prior to treatment (ATP), and lastly for intra-fraction re-optimization during delivery (intra). Bladder and rectum DVH metrics were compared between the standard and reduced margin plans. Results As expected, rectum V18 Gy was reduced by 4.4 ± 3.9%, D1cc was reduced by 12.2 ± 6.8% (3.4 ± 2.3 Gy), while bladder reductions were 7.8 ± 5.6% for V18 Gy, and 9.6 ± 7.3% (3.4 ± 2.5 Gy) for D1cc for the reduced margin reference plans compared to the standard PTV margin. For the intrafraction replanning approach, average intra-fraction optimization times were 40.0 ± 2.9 seconds, less than the time to deliver one of the four VMAT arcs (104.4 ± 9.3 seconds) used for treatment delivery. When accounting for intra-fraction motion, prostate V36.25 Gy was on average 96.5 ± 4.0%, 99.1 ± 1.3%, and 99.6 ± 0.4 for the non-adapt, ATP, and intra-adapt groups, respectively. The minimum dose received by the prostate was less than 95% of the prescription dose in 84%, 36%, and 10% of fractions, for the non-adapt, ATP, and intra-adapt groups, respectively. Conclusions Intra-fraction re-optimization improves prostate coverage, specifically the minimum dose to the prostate, and enables PTV margin reduction and subsequent OAR sparing. Fast re-optimizations enable uninterrupted treatment delivery.

Abstract:Charing Cross Hospital provides facilities for the study of handicapped children in a normal nursery which forms part of its Child Development Centre. The period of assessment varies from one day to three weeks; the advantages of prolonged assessment are discussed. Parental involvement and support, and the teaching of medical students are emphasized. A study of the Centre's first 385 patients is presented.

Patients' daily CT scans and portal images were collected to generate reconstructed 3D dose distributions. Contours of targets and OARs were recontoured on these daily CT scans by a clinician or an auto-segmentation algorithm, then dose-volume indices were calculated, and the percent deviation of these indices to their original plans were determined. This deviation was regarded as the metric for clinically relevant patient QA. The tolerance level was obtained using a 95% confidence interval of the QA metric distribution. These deviations could be further divided into anatomically relevant or delivery relevant indicators for error source analysis. Finally, our QA solution was validated on an additional six clinical patients.Main results.In rectal cancer, the 95% confidence intervals of the QA metric for PTV ΔD95(%) were [-3.11%, 2.35%], and for PTV ΔD2(%) were [-0.78%, 3.23%]. In validation, 68% for PTV ΔD95(%), and 79% for PTV ΔD2(%) of the 28 fractions are within tolerances of the QA metrics. one patient's dosimetric impact of anatomical variations during treatment were observed through the source of error analysis.Significance.The online patient QA solution using daily CT scans and EPID-basedin vivodosimetry is clinically feasible. Source of error analysis has the potential for distinguishing sources of error and guiding ART for future treatments.

In validation, the 68% for PTV {\Delta}D95 (%) and the 79% for PTV {\Delta}D2 ({%)of the 28 fractions are within tolerances of the QA metrics. By using four or more out-of-tolerance QA metrics as an action level, there were 5 fractions (18%) have four or more out-of-tolerance QA metrics in validation patient dataset. The online patient QA solution using daily CT scans and EPID-based in vivo dosimetry is clinically feasible. Source of error analysis has the potential for distinguishing sources of error and guiding ART for future treatments.

Abstract:Introduction Radiotherapy plays a key role in the multidisciplinary management of oesophageal cancers across neoadjuvant, definitive and palliative settings. Improved precision in radiotherapy planning and delivery techniques have allowed treating disease with tighter margins reducing toxicity. In this study, we examine the appropriateness of current practice in defining the planning target volume (PTV). Methods This is a single institutional retrospective study of patients who received radiotherapy for oesophageal cancers using volumetric modulated arc therapy (VMAT) during 2020. All cone‐beam computerised tomography (CBCT) scans were reviewed to assess whether PTV expansions appropriately accounted for tumour motion and interfractional variation. Results Of the 27 patients, 2 (7%), 5 (19%) and 20 (74%) had cervical, thoracic and distal/gastro‐oesophageal junction (GOJ) disease, respectively. 16 (59%) had adenocarcinoma and 9 (33%) had squamous cell carcinoma. 9 of 20 distal/GOJ patients were planned and treated according to the institutional stomach filling protocol. 521/528 (98.7%) CBCTs demonstrated adequate target coverage. Cervical, thoracic and GOJ regions demonstrated adequate target coverage in 57/58, 96/97 and 368/373 CBCTs with median PTV expansions of 5, 7 and 7 mm, respectively. In four patients with GOJ disease, CBCT review identified five episodes where the target volume was insufficiently covered during the treatment course. Conclusion In this single institutional retrospective study, for the vast majority of patients, our institutional practice of defining PTV margins achieved satisfactory target treatment. The interfractional variations observed in patients with GOJ tumours due to target motion and variable gastric volume, highlights the role of further refinements to motion management techniques in this cohort.

Abstract:Purpose : To use our CBCT shift results to determine how much of the CTV to PTV margin is necessary to compensate for the daily setup uncertainty inherent in the treatment of head and neck cancer patients. Method and Materials : To accommodate the daily setup uncertainly, a 3–6 mm uniform margin is used for head and neck IMRT patients to expand CTV to PTV. In order to verify patient setup accuracy, CBCT is taken prior to the treatment. Our protocol calls for CBCT for the first five consecutive fractions, followed by two CBCT every week. Each CBCT is registered to the planning CT to determine the corresponding translational shift. The registration is based on the region‐of‐interest—the rigid bones near the tumor. On the days no CBCT is taken, the average of the previous three calculated translational shifts is used instead. No rotational correction is considered. Results : A total of 22 head and neck IMRT patients were studied with a Siemens Artiste linear accelerator using an MVision CBCT scanner. Each patient had approximately 14 sets of CBCT images taken over the entire course of treatment. The average interfraction shift was 0.6±1.6, 1.4±1.5, 0.1±1.6 mm in the anterior‐posterior, left‐right, and superior‐inferior dimensions, respectively. These data indicate that in the absence of CBCT image guided radiotherapy, a CTV to PTV margin of 2.5, 4.7 and 1.3 mm is needed in the anterior‐posterior, left‐right, and superior‐inferior dimensions, respectively. Conclusion : Our results have shown that asymmetric margins of 2.5, 4.7 and 1.3 mm can be used for CTV to PTV expansion for head and neck cancer radiotherapy if no IGRT presented. Our CBCT protocol will allow a reduction in the CTV to PTV margin. Further investigation needs to be done to find out the specific CTV to PTV margin with different CBCT frequency.

Abstract:Stability and change in patterns of health service use over a 3-year period were determined for a sample of elderly people in an urban area who claimed one of five types of health service provider as a primary source of health care--a hospital, a private physician, a network model health maintenance organization (HMO), a hospital-based group practice program (G-HMO), or a preferred provider organization (PPO). Despite certain differences in use rates for individual services, the total volume of ambulatory service use was equivalent for all five groups as was the relative rank order of use of specific ambulatory services for four of the five groups. People who claimed a hospital as their primary care source had the most unique use patterns over a full range of health care services, characterized by extremely low rates of physician visits and the highest rates of visits to hospital outpatient clinics across three time periods. G-HMO members used health-related services more frequently than did all others. PPO members, at baseline, had lower rates of total and mean hospital days than other source group members except hospital users. People who changed principal source of care during the study period were most likely to report a hospital as their care source initially. Although there is much consistency in hospital and ambulatory use across groups, the persistence of certain use patterns for members of some groups suggests that health care systems can leave an imprint on the health service use of people for whom they provide regular care.
First implementation and results of online adaptive radiotherapy for cervical cancer based on CT-Linac combination

Abstract:Purpose: To assess the dosimetric effectiveness of image-guided radiation treatment (IGRT) and online adaptive radiation therapy (oART) for cervical cancer. As well as survival follow-up was conducted to validated the safety and efficacy of oART. Methods: A total of 15 cervical cancer patients were enrolled. oART was performed on a CT-integrated linear accelerator. The initial plan was revised to include the distribution of IGRT dose using daily fan-beam CT (FBCT) images, after which the distinctions between ART and IGRT in terms of target coverage and organs at risk (OARs) sparing were analyzed. Survival follow-up was conducted to validated the safety and efficacy of oART in this group. Results: PTV Dmax value decreased by 1.23 Gy in the ART plan when compared to that in the IGRT plan; PTV D95 increased by 1.34 Gy; PTV V50 coverage increased by 4.86%; CTV coverage increased by 3.02%; PTV D2cc of the colon, rectum, and small intestine decreased by 1.24 Gy, 1.29 Gy, and 1.12 Gy, respectively. The V10 and V30 of the pelvis increased by 1.27% and 0.56%, respectively, while the V30 of the left and right femoral heads dropped by 2.82% and 3.41%, respectively. Except for the pelvic changes, all other differences were statistically significant (p < 0.01). The average time for the ART procedure was 21.22 min (range: 18.72–24.90 min). The median follow-up time is 28.0 months. Median event-free survival and overall survival were not reached. EFS rate and OS rate at 3 years were 79.4% and 92.9%. Conclusion: Online ART for cervical cancer can minimize the dose of OARs and enhance the target volume coverage significantly when compared to IGRT with satisfied survival time.

Abstract:Objective We explore the potential dosimetric benefits of reducing treatment volumes through daily adaptive radiation therapy for head and neck cancer (HNC) patients using the Ethos system/Intelligent Optimizer Engine (IOE). We hypothesize reducing treatment volumes afforded by daily adaption will significantly reduce the dose to adjacent organs at risk. We also explore the capability of the Ethos IOE to accommodate this highly conformal approach in HNC radiation therapy. Methods Ten HNC patients from a phase II trial were chosen, and their cone-beam CT (CBCT) scans were uploaded to the adaptive RT (ART) emulator. A new initial reference plan was generated using both a 1 mm and 5 mm planning target volume (PTV) expansion. Daily adaptive ART plans (1 mm) were simulated from the clinical CBCT taken every fifth fraction. Additionally, using physician-modified ART contours the larger 5 mm plan was recalculated on this recontoured on daily anatomy. Changes in target and OAR contours were measured using Dice coefficients as a surrogate of clinician effort. PTV coverage and organ-at-risk (OAR) doses were statistically compared, and the robustness of each ART plan was evaluated at fractions 5 and 35 to observe if OAR doses were within 3 Gy of pre-plan. Results This study involved six patients with oropharynx and four with larynx cancer, totaling 70 adaptive fractions. The primary and nodal gross tumor volumes (GTV) required the most adjustments, with median Dice scores of 0.88 (range: 0.80–0.93) and 0.83 (range: 0.66–0.91), respectively. For the 5th and 35th fraction plans, 80 % of structures met robustness criteria (quartile 1–3: 67–100 % and 70–90 %). Adaptive planning improved median PTV V100% coverage for doses of 70 Gy (96 % vs. 95.6 %), 66.5 Gy (98.5 % vs. 76.5 %), and 63 Gy (98.9 % vs. 74.9 %) ( p < 0.03). Implementing ART with total volume reduction yielded median dose reductions of 7–12 Gy to key organs-at-risk (OARs) like submandibular glands, parotids, oral cavity, and constrictors ( p < 0.05). Conclusions The IOE enables feasible daily ART treatments with reduced margins while enhancing target coverage and reducing OAR doses for HNC patients. A phase II trial recently finished accrual and forthcoming analysis will determine if these dosimetric improvements correlate with improved patient-reported outcomes.

Abstract:Stereotactic radiation therapy (SBRT) has emerged as a promising treatment modality for locally advanced pancreatic cancer. The aim of this study is to assess the dosimetric efficacy of online adaptive radiotherapy (ART) in comparison to image-guided radiation therapy (IGRT) for pancreatic cancer. We conducted a retrospective analysis involving 8 patients diagnosed with locally advanced pancreatic cancer. The gross tumor volume (GTV) delineates the visible extent of the tumor on imaging, while the planning tumor volume (PTV) was generated by expanding 5 mm from the GTV and ensuring a 3 mm distance from the small intestine, duodenum, and stomach simultaneously. Treatment planning was executed using the United Imaging Healthcare Treatment Planning System workstation. The control group underwent evaluation based on daily validated fan-beam CT (FBCT) scans, assessing both the dose delivered to actual organs at risk (OARs) and the target volume. Radiotherapy plans were developed utilizing simulation CT, and conventional radiotherapy with daily image-guided radiation therapy (IGRT) was administered using FBCT-Linac. Conversely, patients in the study group received daily validated FBCT-guided adaptive radiotherapy plans, with a focus on mean dose assessment of both the target volume and OARs. Subsequently, we compared the average outcomes of each treatment fraction between IGRT and online adaptive radiotherapy (ART). Comparison between ART and IGRT treatment plans revealed significant differences in various dosimetric parameters: For PTV: V98%: ART (96.28%) vs IGRT (89.73%), p = 0.000, V95%: ART (96.28%) vs IGRT (89.73%), p = 0.031, V90%: ART (98.58%) vs IGRT (93.65%), p = 0.000, Dmean: ART (4912.91) vs IGRT (4804.11), p = 0.000. For the Dmean of the left kidney and right kidney: Left kidney: ART (248.28) vs IGRT (239.65), p = 0.100. Right kidney: ART (314.55) vs IGRT (307.17), p = 0.345. These results suggest significant improvements in PTV coverage and sparing of OARs with ART compared to IGRT, indicating the potential of ART in optimizing treatment outcomes for pancreatic cancer patients. Compared to conventional IGRT-guided SBRT programs, ART-based SBRT for pancreatic cancer not only enhances the dose distribution to the target volume but also mitigates the radiation exposure to critical organs-at-risk (OARs) such as the duodenum, small intestine, and stomach. This approach may offer a more favorable safety profile while concurrently enhancing treatment efficacy.
