Abstract:The study of the interaction of "zona-free" mouse, rat and hamster eggs with human spermatozoa showed clearly the species specificity of the plasma membrane in mice and rats where sperm penetration into the cytoplasm has never been observed. On the other hand, 66.1--77.0% of hamster eggs were penetrated and 21.9-57.5% of these eggs showed polyspermy. These results were not affected by any of the three proteolytic enzymes used for dissolution of the zona pellucida. Penetration activity of human spermatozoa expressed as percentage of penetrated zona-free hamster eggs decreased after 12 h of preincubation to 12.4% compared to 72.6% in spermatozoa preincubated for 6 h.
Experimental study of low-dose cervical CT scan and its clinical application

Abstract: Objective To access the optimal scan conditions of low- dose multi- slice CT on cervical region. Methods In the first step, 10 groups of low- dose scan conditions (140kV- 50mA, 140kV- 30mA, 120kV- 100mA, 120kV- 50mA, 120kV- 30mA, 90kV- 200mA, 90kV- 150mA, 90kV- 100mA, 90kV- 50mA , 90kV- 30mA) were selected according to the software prediction of CT radiation dose combined with the literature reports. In the second step, the necks of 3 goats were scanned with the conditions above. The quality of images was independently scored by two senior radiologists with double blind method and 9 groups of scan conditions (except 90kV- 30mA) met the requirement of diagnosis were selected. In the third step, 180 patients undergoing cervi-cal CT scan were randomly divided into 9 groups with 20 cases in each, patients were scanned with the conditions selected in animal experiments. And the quality of images was scored and anylyzed by SPSS 16.0 software. Results The CT radiation dose demonstrated a positive linear correlation with the change of tube voltage and current. The variety slope of CT radiation dose in-creased with the increasing of tube voltage or the current. The animal experiments demonstrated that 9 groups of conditions al met the requirement of diagnosis except 90kV- 30mA. The human cervical CT scan demonstrated that high radiation dose had a high score and bone scores were higher than soft tissue. 120kV- 100mA and 90kV- 200mA obtained good soft tissue images and excel ent bone images; 140kV- 50mA, 140kV- 30mA, 120kV- 50mA, 90kV- 150mA and 90kV- 100mA obtained good bone im-ages, and the diagnosis was not influenced by the soft tissue images. 120kV- 30mA and 90kV- 50mA obtained the soft tissue im-ages influencing the diagnosis and the bone images not influencing the diagnosis. Conclusion The images meeting the require-ment of diagnosis can be obtained by adopting low tube voltage and relative high tube current with lower radiation dose. In cervi-cal CT scan, 90kV- 100mA is the scan condition for soft tissue and 90KV- 50mA can be selected to observe the bone only.

Abstract:Background: Computed tomography (CT) is a non-invasive diagnostic imaging modality which can be used to study the anatomy and morphology of live or deceased animals in-situ. In cetaceans, existing CT anatomy studies mostly focused on the head and thoracic regions. Using postmortem CT (PMCT) scans of Indo-Pacific finless porpoises (Neophocaena phocaenoides), this study describes the cross-sectional imaging anatomy of the cetacean abdomino-pelvic organs for the first time. Methods: PMCT scans of finless porpoises stranded in Hong Kong waters were reviewed, of which two freshly dead cases, one male and one female, were selected for illustration. In addition, a contrast-enhanced PMCT scan was performed on the female subject as a trial for a PMCT-angiography study (PMCTA) in cetaceans. A total of 18 axial PMCT images were acquired at selected vertebral levels in the abdomen and supplemented with a series of corresponding labeled anatomical diagrams. Results: By applying different image rendering techniques, most osseous and soft tissue structures in the finless porpoise abdomen were successfully depicted and annotated on PMCT, including the male and female reproductive organs in the pelvic region. The application of contrast medium in PMCT created artificial radiodensity differences which improved the ability to visualize and differentiate soft organs and vasculature. The merits and limitations of CT compared to other imaging modalities, as well as the future directions of PMCT in stranding investigation, were discussed. Conclusions: The findings from this study significantly enhance the applications of CT in cetaceans by assisting researchers and veterinarians in the interpretation of cetacean abdomino-pelvic CT for morphological and pathological assessment during clinical or postmortem examination.
Post-mortem ultrasonographic and computed tomographic features of the anatomical variations and acquired pathological bony changes of the lumbosacroiliac region in a mixed population of horses

Abstract:Background: Transrectal ultrasonography is the best technique for evaluating the ventral aspect of the lumbosacral and sacroiliac regions yet this diagnostic technique does not always lead to a final diagnosis of back pain in horses. Objectives: To describe anatomical variations and acquired pathological bony changes (APBCs) in the lumbosacral and sacroiliac regions detected by ultrasonography (US) and computed tomography (CT) examinations on specimens. We hypothesised that age, body mass, previous use and anatomical variations may be correlated with the presence and/or severity of APBCs. Study design: Descriptive cadaver study. Methods: Lumbosacroiliac specimens were obtained from 51 horses that died or were euthanised for reasons other than the study and underwent US and CT examinations post-mortem. Results: Forty-two specimens were analysed. The most prevalent lumbosacral disc morphology was type 2 (21/42), and protrusions were found in 15/42 specimens. Abnormal echogenicity of the L5-L6 and lumbosacral discs was detected in 11/42 and 30/42 specimens, respectively. Abnormalities in the size of the L5-L6 disc were found in 10/42 specimens and correlated with promontorium localisation (Cramér's V coefficient [V] = 0.42) and lumbosacral disc morphology (type 1: V = 0.41; type 5: V = 0.69). The most prevalent orientation of the L6 spinous process (SP) was convergent (24/42). The promontoria were mostly located between L6 and S1 (36/42). Lumbosacral spondylosis was detected in 24/42 specimens and spondylolisthesis in one. Age was associated with the severity of APBCs in the sacroiliac joints, lumbosacral intertransverse joints, articular process joints, spondylosis and L6 or S1 bone plate irregularities. Lumbosacral spondylosis or L6 extremitas caudalis irregularity was associated with lumbosacral angulation, variability in the size of the L5-L6 disc and localisation of the promontorium, L6 SP orientation and L6 extremitas caudalis irregularity with abnormal echogenicity of the LS disc. Main limitations: Lack of information on clinical signs. Conclusions: High variability and prevalence of the anatomical variations and APBCs in the lumbosacroiliac region were observed, and correlations between some anatomical variations and APBCs and between APBCs and age were reported.

Abstract:Background: Early diagnosis of metastatic epidural spinal cord compression (MESCC) is vital to expedite therapy and prevent paralysis. Staging CT is performed routinely in cancer patients and presents an opportunity for earlier diagnosis. Methods: This retrospective study included 123 CT scans from 101 patients who underwent spine MRI within 30 days, excluding 549 CT scans from 216 patients due to CT performed post-MRI, non-contrast CT, or a gap greater than 30 days between modalities. Reference standard MESCC gradings on CT were provided in consensus via two spine radiologists (11 and 7 years of experience) analyzing the MRI scans. CT scans were labeled using the original reports and by three radiologists (3, 13, and 14 years of experience) using dedicated CT windowing. Results: For normal/none versus low/high-grade MESCC per CT scan, all radiologists demonstrated almost perfect agreement with kappa values ranging from 0.866 (95% CI 0.787–0.945) to 0.947 (95% CI 0.899–0.995), compared to slight agreement for the reports (kappa = 0.095, 95%CI −0.098–0.287). Radiologists also showed high sensitivities ranging from 91.51 (95% CI 84.49–96.04) to 98.11 (95% CI 93.35–99.77), compared to 44.34 (95% CI 34.69–54.31) for the reports. Conclusion: Dedicated radiologist review for MESCC on CT showed high interobserver agreement and sensitivity compared to the current standard of care.

Abstract:Purpose of review: This review synthesizes recent advancements in understanding subchondral bone (SCB) biomechanics using computed tomography (CT) and micro-computed tomography (micro-CT) imaging in large animal models, particularly horses. Recent findings: Recent studies highlight the complexity of SCB biomechanics, revealing variability in density, microstructure, and biomechanical properties across the depth of SCB from the joint surface, as well as at different joint locations. Early SCB abnormalities have been identified as predictive markers for both osteoarthritis (OA) and stress fractures. The development of standing CT systems has improved the practicality and accuracy of live animal imaging, aiding early diagnosis of SCB pathologies. While imaging advancements have enhanced our understanding of SCB, further research is required to elucidate the underlying mechanisms of joint disease and articular surface failure. Combining imaging with mechanical testing, computational modelling, and artificial intelligence (AI) promises earlier detection and better management of joint disease. Future research should refine these modalities and integrate them into clinical practice to enhance joint health outcomes in veterinary and human medicine.
Accuracy of computed tomography in diagnosing temporomandibular joint osteoarthritis relative to histopathological findings - an ex-vivo study of 41 horses

Abstract:Computed tomography (CT) is commonly used for imaging the equine head, particularly when diseases of temporomandibular joint (TMJ), including osteoarthritis (OA), are suspected. However, certain CT findings have been proposed as 'anatomical variations', and their clinical significance remains unknown. The objectives of this prospective descriptive cross-sectional study were to describe the CT and histopathological findings of TMJ OA in relation to horse age, evaluate the accuracy of TMJ OA diagnosis with regard to 'CT anatomical variations', and explore the correlations between specific CT and histopathological findings. CT images and tissue samples from 41 heads (82 TMJs) were independently and simultaneously reviewed by two observers. Descriptive statistics on both findings, their age-related frequency, and accuracy of TMJ OA diagnosis were calculated. Based on histopathological assessment, TMJ OA was confirmed in 18% of joints in horses aged 1-4 years, 63% of joints in horses aged 5-15 years, and 83% of joints in horses over 15 years. Considering entire CT findings, TMJ OA was radiologically recognized in 76% of joints, with a sensitivity of 0.95 and a specificity of 0.37. When 'CT anatomical variations' were excluded, TMJ OA was recognized in 30% of joints, with a sensitivity of 0.51 and a specificity of 0.88. Findings suggest that the equine TMJ may undergo both age-related and disease-related remodeling, detectable through CT and histopathology, and that the inclusion or exclusion of certain 'CT anatomical variations' in the diagnostic protocol affects the sensitivity and specificity of CT-based TMJ OA recognition.

Abstract:The objective of this study was to describe the use of computed tomography (CT) for diagnosis of mechanical gastrointestinal (GIT) obstruction in canines and felines. Medical records of 130 canines and felines that underwent an abdominal CT scan between 2013 and 2015 at a specialty referral hospital for suspected gastrointestinal tract (GIT) obstruction were reviewed. Images were evaluated by a single board-certified radiologist for the presence of foreign material, evidence of obstruction, and location of foreign material present. Confirmation of CT findings was based on surgical exploration or medical management if surgery was not indicated. Of the 97 patients that met the inclusion criteria, 48 (49.48%) had evidence of foreign material present within the GIT and 49 (50.52%) did not. Forty-one patients had evidence of mechanical gastrointestinal obstruction. Thirty-nine of these patients had an obstruction due to foreign material; one had an intussusception with no foreign material, and another had obstruction secondary to mucosal thickening. Forty-five patients underwent exploratory laparotomy, and CT findings were confirmed in all patients. The presence of a GIT obstruction was confirmed intra-operatively in 37 patients and lack of obstruction was confirmed in the remaining eight. Non-surgical medical management was pursued for the remaining patients. Based on follow-up client interviews, clinical signs resolved in all of these patients. In conclusion, computed tomography appears to be useful for the diagnosis of GIT obstruction in canines and felines and is a helpful tool for guiding the recommendation for surgical intervention.
Morphometric Evaluation of the Thoracic and Lumbar Vertebral Canal and Spinal Cord Using Computed Tomography in Healthy Korean Shorthair Cats

Abstract:This study aimed to evaluate the spinal morphometry of the thoracic and lumbar regions in normal Korean Shorthair cats using computed tomography (CT) and to investigate the relationship with variables such as sex, age and body weight. Fifteen clinically healthy Korean Shorthair cats (eight males, seven females) from Seoul National University Veterinary Medical Teaching Hospital were included in this retrospective study. Measurements of the height, width and area of the vertebral canal and spinal cord on CT images were taken at the cranial, middle and caudal points of the thoracic and lumbar vertebrae by three observers, and the ratios of the spinal cord area to the vertebral canal area were calculated. The significance of the differences in measurements between sexes and correlations with age and body weight were analysed. The mean age of the cats was 7 years (range: 2-12 years), with a mean weight of 5.27 kg (range: 2.6-8.3 kg). The height, width and area of the vertebral canal and spinal cord were significantly greater in males than in females (p < 0.05). The ratios of the spinal cord area to the vertebral canal area showed no significant difference between sexes (p > 0.05), and no significant correlations were found between the ratios of the spinal cord area to the vertebral canal area and age or body weight. This study provides useful reference intervals for spinal morphometry in the thoracic and lumbar regions of healthy Korean Shorthair cats and investigate the relationship with variables such as sex, age and body weight. This anatomical information may assist in the diagnosis and prognosis of thoracic, lumbar vertebral and spinal cord diseases using CT.
Pediatric Primary Care to Help Prevent Child Maltreatment: The Safe Environment for Every Kid (SEEK) Model

Abstract:CONTEXT. Effective strategies for preventing child maltreatment are needed. Few primary care–based programs have been developed, and most have not been well evaluated. OBJECTIVE. Our goal was to evaluate the efficacy of the Safe Environment for Every Kid model of pediatric primary care in reducing the occurrence of child maltreatment. METHODS. A randomized trial was conducted from June 2002 to November 2005 in a university-based resident continuity clinic in Baltimore, Maryland. The study population consisted of English-speaking parents of children (0–5 years) brought in for child health supervision. Of the 1118 participants approached, 729 agreed to participate, and 558 of them completed the study protocol. Resident continuity clinics were cluster randomized by day of the week to the model (intervention) or standard care (control) groups. Model care consisted of (1) residents who received special training, (2) the Parent Screening Questionnaire, and (3) a social worker. Risk factors for child maltreatment were identified and addressed by the resident physician and/or social worker. Standard care involved routine pediatric primary care. A subset of the clinic population was sampled for the evaluation. Child maltreatment was measured in 3 ways: (1) child protective services reports using state agency data; (2) medical chart documentation of possible abuse or neglect; and (3) parental report of harsh punishment via the Parent-Child Conflict Tactics scale. RESULTS. Model care resulted in significantly lower rates of child maltreatment in all the outcome measures: fewer child protective services reports, fewer instances of possible medical neglect documented as treatment nonadherence, fewer children with delayed immunizations, and less harsh punishment reported by parents. One-tailed testing was conducted in accordance with the study hypothesis. CONCLUSIONS. The Safe Environment for Every Kid (SEEK) model of pediatric primary care seems promising as a practical strategy for helping prevent child maltreatment. Replication and additional evaluation of the model are recommended.
Imaging findings of a haemangiosarcoma in a cervical vertebra of a horse

Abstract:Summary An 18‐year‐old Warmblood gelding was presented with a recent, abrupt onset of ataxia. Transcranial magnetic stimulation (TMS) was performed and revealed a delayed response in both thoracic and pelvic limbs. Radiographic examination of the cervical region demonstrated osteolytic lesions in the vertebral body and arch of the third cervical vertebra (C3). A cervical myelogram revealed spinal cord swelling in this region on the lateral projection. Computed tomographic (CT) myelography was performed, which showed osteolytic lesions and circumferential thinning of the contrast column due to extra‐dural compression at the cranial cervical region. These findings were compatible with an aggressive bone lesion compressing the spinal cord. Primary or secondary neoplasia and osteomyelitis were considered in the differential diagnosis. Because of the severity of the findings and poor prognosis, the horse underwent euthanasia. Post mortem histopathological diagnosis was a haemangiosarcoma involving the cranial cervical vertebrae and epidural space. This report demonstrates the additional value of CT myelography on the extent and exact location of cervical vertebral compressive lesions in the horse. This information can be useful for prognosis, biopsy and/or planned surgery of these lesions. See also Clinical Commentary by Southwood

Abstract:Horses underwent either cervical epidural space (CES) catheterization or subarachnoid space (SAS) catheterization while restrained in stocks, under deep sedation (detomidine and morphine) and local anesthesia (mepivacaine 2%) block. Catheters were placed under ultrasound guidance with visualization of the dura, SAS, and spinal cord between the first (C1) and second (C2) cervical vertebrae. Following sedation and sterile skin preparation, operator 1 placed under ultrasound guidance, a 6- or 8-inch Tuohy needle with the bevel oriented caudally. For CES, a 6-inch Touhy needle was used with the hanging drop technique to detect negative pressure, and operator 2 then passed the epidural catheter into the CES. For SAS, following puncture of the dura, cerebrospinal fluid (CSF) was aspirated prior to placement of the epidural catheter. Placement into either CES or SAS was confirmed with plain and contrast radiography. Catheters were wrapped for the duration of the study. CSF cytology was assessed up to every 24 h for the study period. Horses were assessed daily for signs of discomfort, neck pain, catheter insertion site swelling, or changes in behavior. A complete postmortem assessment of the spinal tissues was performed at the end of the study period (72 h). Two horses had CES catheters and five horses had SAS catheters placed successfully. All horses tolerated the catheter well for the duration of the study with no signs of discomfort. Ultrasound was essential to assist placement, and radiography confirmed the anatomical location of the catheters. CSF parameters did not change over the study period (P > 0.9). There was evidence of mild meningeal acute inflammation in one horse and hemorrhage in another consistent with mechanical trauma. Placement of an indwelling CES or SAS catheter appears to be safe, technically simple, and well tolerated in standing sedated normal horses.

Abstract:A plane spiral orbital angular momentum (OAM) waves-based $4 \times 4$ multiple-in-multiple-out (MIMO) communication system for line-of-sight channel is proposed and experimentally demonstrated in comparison with conventional MIMO. The transmitting OAM waves are propagating along the transverse direction; hence there is no singularity or divergence problem. Compared with the conventional MIMO, this system could achieve lower spatial correlations and a higher capacity gain while the OAMantennas as the elements of the transmitting array are sorted in an ordered sequence. The measured bit-error-rate performance clearly demonstrates the feasibility of the proposed system, and the causation of the improved performance is also detailedly discussed. This technology can be applied in near-field communications or short-range MIMO systems to realize higher capacity communications.
Computed tomographic imaging and surgical management of distal insertional avulsion fragments of the caudal cruciate ligament in four horses

Abstract:Objective: To describe cases with caudal cruciate ligament (CdCL) avulsion fragments diagnosed based on computed tomography (CT) examination and report on arthroscopic fragment removal. Animals: Four Warmblood horses with hindlimb lameness and osseous fragments located in the caudal medial femorotibial joint (mFTJ). Study design: Short case series. Methods: CT and arthroscopic evaluation of the caudal mFTJ were performed. The caudal mFTJ and the insertion of the CdCL on the tibia were assessed and removal of the avulsion fragments was attempted in three horses using a cranial intercondylar approach. Results: The fragment was not accessible via caudomedial approaches in one horse. A cranial intercondylar approach was used in three horses, allowing removal of the intra-articular fragment in two horses, and removal of two-thirds of the proximal fragment in the last horse. Acute, profuse, arterial bleeding occurred in this horse during surgery with transient postoperative soft tissue swelling. Comorbidities included medial femoral condyle cartilage defects (3), cranial cruciate ligament lesions (2), and medial collateral ligament lesions (2). Horses were followed up for 16 months (median, range 11-28 months), at which point all were back in ridden exercise; owners' satisfaction was good. Conclusion: CT examination confirmed the diagnosis and allowed evaluation of the stifle joint for comorbidities. A cranial intercondylar arthroscopic approach facilitated the removal of CdCL insertional avulsion fragments, although not always complete. Clinical significance: A cranial intercondylar approach can allow access to CdCL avulsion fragments, but complications and incomplete removal remain possible.

Abstract:The sacrum and sacroiliac joints pose a long-standing challenge for adequate imaging because of their complex anatomical form, oblique orientation, and posterior location in the pelvis, making them subject to superimposition. The sacrum and sacroiliac joints are composed of multiple diverse tissues, further complicating their imaging. Varying imaging techniques are suited to evaluate the sacrum, each with its specific clinical indications, benefits, and drawbacks. New techniques continue to be developed and validated, such as dual-energy computed tomography (CT) and new magnetic resonance imaging (MRI) sequences, for example susceptibility-weighted imaging. Ongoing development of artificial intelligence, such as algorithms allowing reconstruction of MRI-based synthetic CT images, promises even more clinical imaging options. Osseous pathologic changes in the thoracic region of the equine vertebral column: A descriptive post-mortem study in three breeds

Abstract:Background: Data on equine thoracic pathology hardly exist in breeds other than Thoroughbreds. Objectives: To describe pathological changes of the osseous thoracic vertebral column in Warmblood horses, Shetland ponies and Konik horses, and to compare prevalence and severity. Study design: Descriptive post-mortem study. Methods: Computed tomography was used to examine the thoracic vertebral column of 34 Warmblood horses, 28 Shetland ponies, and 18 Konik horses. Osteoarthritis (OA), periarticular osteolysis, osseous cyst-like lesions and fragments of articular processes (APJs), costovertebral and costotransverse joints, soft tissue mineralisation surrounding these joints, intervertebral disc (IVD) mineralisation, impingement of spinous processes (SPs) and spondylosis were scored. Results: APJ aplasia (98%; 58/59) occurred in 14 Shetland ponies. OA predominated in Warmblood horses across all joints (45% vs. 29 and 32%, p < 0.001). OA and periarticular osteolysis were found in particular in the costotransverse joints (p < 0.001) across all breeds. Warmblood and Konik horses had more and severe soft tissue mineralisation than Shetland ponies (2.3%, score 1.5 and 3.2%, score 1.3, respectively, vs. 0.3%, score 0.9, p < 0.01). Mid location was most often affected with highest severity score (4.2%, 1.5, p < 0.001). Konik horses displayed the highest IVD mineralisation (20%, p < 0.001), which notably increased with age (p < 0.001) compared with Warmblood horses (4.5%) and Shetland ponies (1.1%). SPs impingement was absent in Shetland ponies, and most prevalent in Warmblood horses (11.2%, p < 0.001) compared with Konik horses (2.2%). Osseous cyst-like lesions, fragments and spondylosis were minimal in all breeds and nearly absent in Shetland ponies. Main limitations: Clinical histories and macroscopic and histologic examinations were unavailable. Conclusions: Distinct breed differences exist in prevalence and severity of thoracic vertebral column osseous pathologies. Costotransverse joint pathology is more common than APJ pathology in all breeds. Warmblood horses are prone to OA, spondylosis and SPs impingement; Konik horses exhibit significant IVD mineralisation, and Shetland ponies demonstrate a high prevalence of APJ aplasia. Periarticular osteolysis, osseous cyst-like lesions, fragments and soft tissue mineralisation are notably low in Shetland ponies.

Abstract:Background: Brachycephalic dogs with pulmonary stenosis are known to have a higher incidence of concurrent coronary artery abnormalities than non-brachycephalic breeds, which increases risk when performing balloon valvuloplasty. The use of ECG-gated CT angiography has been reported for the evaluation of coronary arteries in normal dogs and dogs with pulmonary stenosis. The purpose of this study was to report findings of coronary artery origination and morphology of main branches using ECG-gated CT angiography in brachycephalic dogs with pulmonary stenosis. Methods: An ECG-gated CT angiographic protocol was used to image coronary artery anatomy in nine brachycephalic dogs with pulmonary stenosis. Images were assessed for quality as well as coronary artery morphology by one veterinary radiologist, one veterinary cardiologist and one veterinary radiology resident. Results: All nine dogs had good to excellent image quality. Coronary artery anomalies were identified in three of nine dogs: one R2A anomaly, one L2A anomaly and one L2C anomaly. Two dogs were assessed to be poor balloon valvuloplasty candidates based on CT angiographic images. Limitation: Coronary artery morphology was not confirmed via postmortem examination in all patients. Conclusion: ECG-gated CT angiography is a minimally invasive imaging modality capable of diagnosing various coronary artery anomalies in brachycephalic dogs with pulmonary stenosis and aiding in the determination of patient candidacy for balloon valvuloplasty.
Floppy eyelid syndrome: a case report and clinical review.

Abstract:Floppy eyelid syndrome is a clinical entity associated with a chronic papillary conjunctivitis which is resistant to topical therapeutic agents. The disorder is usually found in older, obese males with upper eyelids which are loose, rubbery, and easily everted. This paper is a review of the clinical characteristics and treatment of the syndrome and includes a case of a 16-year-old male with floppy eyelid syndrome, one of the youngest reported.
Temporal distributions of seizure occurrence from various epileptogenic regions

Abstract:Objective: The aim of this study was to determine whether seizure occurrence in partial epilepsy is under the influence of circadian rhythms and rhythmic exogenous factors, and how this influence varies according to cortical brain region. For these ends, we determined and analyzed detailed temporal distributions of seizures arising from the frontal, parietal, occipital, neocortical temporal, and mesial temporal lobes. Methods: We retrospectively analyzed intracranial EEG recordings from 131 consecutive adult subjects whose partial epilepsy was sufficiently localized for surgical resection. In all, 669 seizures were analyzed: 132 frontal, 77 parietal, 83 occipital, 217 mesial temporal, and 160 neocortical temporal. Results: Seizure distribution was dependent on brain region (p < 10−9). Nonuniform seizure distributions were observed in the parietal (p < 10−4), occipital (p < 10−7), mesial temporal (p < 0.02), and neocortical temporal lobes (p < 0.04). Occipital and parietal seizures occurred in strong gaussian-like distributions, 180° out of phase relative to each other; occipital seizure occurrence peaked between 16:00 and 19:00, whereas parietal seizures peaked between 4:00 and 7:00. Frontal lobe seizures followed a unimodal distribution, peaking between 4:00 and 7:00. Seizures from the mesial temporal lobe were distributed bimodally, with the primary peak in the late afternoon between 16:00 and 19:00 and secondary peak in the morning between 7:00 and 10:00. Neocortical temporal seizures peaked slightly before the primary peak observed in the mesial temporal lobe; however, these distributions did not differ significantly. Conclusions: Seizure occurrence in partial epilepsy is not random. Endogenous circadian rhythms and rhythmic exogenous factors likely play substantial roles in seizure occurrence. These roles vary considerably according to brain region. Frontal and parietal lobe seizures seem most likely to occur nocturnally, whereas occipital and temporal lobe seizures seem to have strong afternoon preferences.
