Abstract:Objective Although rates of weight discrimination are on‐par with racial, ethnic, and gender discrimination, comparatively less work has examined impacts of weight‐based discrimination in youth, including on disordered eating. Knowing whether experiences of weight‐based discrimination, including in youth with multiply‐marginalized identities, are associated with disordered eating could identify vulnerable youth and inform intervention efforts. Method Youth (N = 11,875) ages 10–11 were recruited through the Adolescent Brain Cognitive Development (ABCD) study. Logistic regressions using cross‐sectional data examined discrimination experiences (weight, perceived sexual orientation, race/ethnicity, national origin) and disordered eating (binge‐eating, vomiting, weight‐gain fear, weight self‐worth). Models included race/ethnicity, age, sex, parental income, and degree of elevated weight. Raked poststratification weights were used. Results Rates of weight‐based discrimination (6.2%) were similar to rates of race/ethnicity (4.4%) and sexual orientation discrimination (4.5%). Weight‐based discrimination was associated with more disordered eating. Youth reporting multiple experiences of discrimination had significantly increased disordered eating compared to youth who did not report discrimination. Discussion Weight‐based discrimination is common in youth and associated with disordered eating. Youth with elevated weight are more likely to be multiply marginalized and experience disordered eating. These findings suggest discrimination, including weight‐based discrimination, is a critical intervention target to prevent and treat eating disorders. Public Significance Experiences of discrimination contribute to poorer health; however, weight‐based discrimination is relatively understudied. Data from a large sample of youth ages 10–11 showed that youth with elevated weight reported experiencing multiple types of discrimination, and multiply‐marginalized youth had increased odds of disordered eating with each additional type of discrimination. Together, this suggests that weight‐based discrimination is a critical target to prevent and treat eating disorders, especially in multiply‐marginalized youth.

Frequency and Correlates of Weight-Based Discrimination among Adolescents in China

Abstract:Background Unfair treatment on the basis of body weight is common in the West and associated with lower well-being, starting at least as early as adolescence. We examine whether the frequency and predictors and correlates of weight discrimination seen in the West extend to adolescents in China. Methods Participants ( N = 1539) were adolescents in China who took part in a longitudinal study of mental health and academic burnout in high school. At Wave 1, participants reported on their personality and well-being and completed a school health check that included measured height and weight. At Wave 2 approximately 6 months later, participants reported on experiences with unfair treatment and completed the same measures of well-being. Results Of 10 attributions for unfair treatment, weight was most common at 18%. Female gender and body mass index were associated with greater risk of reporting weight discrimination. Emotional stability, conscientiousness, and state happiness measured at Wave 1 were associated with lower risk of weight discrimination at Wave 2. Concurrently, weight discrimination was associated with lower happiness, less life satisfaction, and more distress. Conclusions Adolescents in China report weight discrimination, and the patterns of association between weight discrimination and psychological function are similar to what is seen in the West.

Abstract:Purpose This study aims to investigate the prospective associations between four types of perceived discrimination (country of origin, race and ethnicity, sexual orientation, and weight) and the development of manic symptoms in a diverse, nationwide sample of adolescents aged 9–14 years in the U.S. Methods We analyzed prospective cohort data from the Adolescent Brain Cognitive Development Study ( N = 7466; ages 9–14 years at Year 1 or 2 in 2017–2020; 48.5 % female; 39.4 % racial/ethnic minority). Multiple zero-inflated negative binomial analyses were conducted to examine the associations between Year 1 or 2 discrimination (by country of origin, race and ethnicity, sexual orientation, weight, sum score), and Year 3 manic symptoms (7 Up Mania scale), adjusting for covariates (age, sex, race and ethnicity, household income, parental education, sipping alcohol, puffing tobacco, anxiety symptoms, depressive symptoms, Year 1 manic symptoms, and study site). Results After adjusting for covariates, perceived discrimination based on country of origin (incidence rate ratio [IRR] = 1.46; 95 % confidence interval [CI] 1.15–1.86), sexual orientation (IRR = 1.36; 95 % CI 1.21–1.53), race and ethnicity (IRR = 1.28; 95 % CI 1.13–1.46), weight (IRR = 1.21 95 % CI 1.09–1.34), and sum scores (IRR = 1.18 5 % CI 1.12–1.24), were significantly associated with higher manic symptoms. Conclusion Perceived discrimination based on country of origin, race and ethnicity, sexual orientation, weight, and sum scores, are prospectively associated with greater manic symptoms in adolescents. These findings underscore the need for targeted interventions to address discrimination and associated psychological impacts. Efforts to reduce discrimination and to support affected adolescents are important components of comprehensive mental health care and public health strategies.

Abstract:Sexual orientation discrimination increases the risks of negative health outcomes for sexual minorities. Previous studies have found increased rates of eating disorder symptoms in sexual minority individuals, which is attributable to minority stress and discrimination that they experience. Emerging research suggests relationships between sexual orientation discrimination and eating disorder symptoms. However, there is a lack of studies focusing on early adolescents. Prevalence of Weight Stigma in Medical Settings Among Reproductive-Aged Women in the All of Us Study

Abstract:OBJECTIVE: To analyze health care treatment experiences among a diverse cohort of reproductive-aged women from the All of Us study, focusing on variations due to body mass index (BMI). METHODS: We conducted a cross-sectional study that used data from the All of Us Research Program. We analyzed weight bias among reproductive-aged women aged 18–44 years. Weight bias was assessed using a proxy 7-item survey that evaluated discrimination in health care experiences, including courtesy, respect, service quality, being treated as competent, displaying fear, being treated as inferior, and being listened to during health care visits. RESULTS: Overall, 16,791 reproductive-aged women (mean [SD] age, 35.8 [6.3]) were included in the study. Of 11,592 patients with data available on BMI, 582 were Asian (5.0%), 1,984 (17.1%) were Hispanic or Latinx, 1,007 (8.7%) were non-Hispanic Black, and 7,370 (63.6%) were non-Hispanic White. Approximately 24.2% of participants (n=2,878) had obesity, and 11.1% of participants (n=1,292) had severe obesity. In general, participants in higher BMI categories had negative experiences in the health care setting. Women with severe obesity were 1.5 to 2 times more likely to report poor experiences with their health care clinicians compared with women with healthy weight. CONCLUSION: Results show significant disparities in the health care experiences among reproductive-aged women. As participants' BMI category increased, so did their negative experiences in the health care setting. These findings underscore the urgent need for targeted interventions to address these inequities. Health care systems must prioritize strategies to ensure that all individuals, regardless of weight or BMI, receive equitable and respectful care.

Abstract:Background Disordered eating in early adolescence impacts development, with long‐term health implications. Minoritised adolescents might be at higher risk of disordered eating due to minority stress, but most research has focused on White, heterosexual, cisgender individuals; less is known about disordered eating among minoritised adolescents. We examined sexual, gender, racial, and ethnic identities in relation to disordered eating in early adolescence. Method Using 2‐year follow‐up data from adolescents ages 10–14 in the Adolescent Brain Cognitive Development Study (N = 9385), we examined associations between sexual, gender, racial, and ethnic identities and past‐2 week disordered eating (preoccupation with weight, weight control behaviors, and binge eating). Results Compared to heterosexual peers, gay/bisexual adolescents had higher odds of all three outcomes (AOR 1.90–3.32); those "questioning" their sexual identity had higher odds of preoccupation with weight (AOR 1.82) and binge eating (AOR 2.53). Compared to cisgender adolescents, transgender adolescents had higher odds of binge eating (AOR 2.62); those "questioning" their gender identity had higher odds of preoccupation with weight (AOR 2.45). Adolescents whose racial identity was categorised as "Another" had higher odds of preoccupation with weight (AOR 1.46) and weight control behaviors (AOR 1.58) compared to White adolescents. Finally, Hispanic adolescents had higher odds of all disordered eating outcomes than non‐Hispanic adolescents (AOR 1.25–1.59). Discussion This study is among the first to reveal disparities in disordered eating among minoritised early adolescents. Further examination of these disparities can inform future interventions. Healthcare providers are encouraged to screen for disordered eating, recognising that minoritised early adolescents may be at risk.

Discrimination and excessive weight gain during pregnancy among Black and Latina young women

Abstract:RATIONALE: Excessive weight gain during pregnancy is a major determinant of later life obesity among both Black and Latina women and their offspring. However, psychosocial determinants of this risk, including everyday discrimination, and potential moderators of such effects remain unexplored.OBJECTIVE: We examined the influence of discrimination, a culturally relevant stressor, on odds of gaining weight beyond Institute of Medicine recommendations during pregnancy. Whether the effect was moderated by race/ethnicity, age, or depressive symptoms was also examined.METHOD: Participants were 413 Black and Latina pregnant young women, ages 14-21 years. Experience with discrimination and all moderators were assessed in the second trimester. Last weight recorded in the third trimester was abstracted from medical records and used to determine excessive weight gain.RESULTS: Ever experiencing discrimination was associated with a 71% increase in the odds of excessive weight gain. The effect of discrimination was primarily present among women who attributed this treatment to membership in a historically oppressed group (e.g., ethnic minority, female) or to membership in other stigmatized groups (e.g., overweight). The effect of ever experiencing discrimination was not moderated by race/ethnicity or age but was moderated by depressive symptoms. Supporting the perspective of the environmental affordances model, discrimination strongly predicted excessive weight gain when women were low in depressive symptoms but had no effect when women were high in depressive symptoms. The moderating role of depressive symptoms was equivalent for Black and Latina women.CONCLUSION: Results highlight the role of discrimination in perpetuating weight-related health disparities and suggest opportunities for improving health outcomes among young pregnant women.

Abstract:While the impact of interpersonal discrimination on mental health is well established, its effects on physical health outcomes have not been fully elucidated. This study systematically reviewed the literature on the prospective association between interpersonal discrimination and markers of adiposity. Medline, Web of Science, Scopus, PsycInfo, SciELO, LILACS, Google Scholar, Capes/Brazil and ProQuest databases were used to retrieve relevant information in November 2016. The results from the 10 studies that met the inclusion criteria support an association between interpersonal self-reported discrimination and the outcomes. In general, the most consistent findings were for weight and body mass index (BMI) among women, i.e. high levels of self-reported discrimination were related to increased weight and BMI. Waist circumference (WC) showed a similar pattern of association with discrimination, in a positive direction, but an inverted U-shaped association was also found. Despite a few inverse associations between discrimination and markers of adiposity, none of the associations were statistically significant. Overall, markers of adiposity were consistently associated with discrimination, mainly through direct and nonlinear associations. This review provides evidence that self-reported discrimination can play an important role in weight, BMI and WC changes.

Bullying Victimization Profiles of School-Aged Adolescents and Associations With Weight Statuses: A Latent Class Analysis

Abstract:Bullying and childhood obesity are public health concerns in the United States. While obesity has been found to be associated with bullying victimization, it is less clear how it is differentially associated with biological sex and grade level across varying patterns of bullying victimization. Based on the subset of U.S. students in the Health Behavior in School-Aged Children study, patterns of co-occurring bullying victimization and sex and grade level differences in the association between the patterns of bullying victimization and weight status were examined among 9,124 students in Grades 5 to 10. The latent class analysis revealed three patterns of bullying victimization: verbal-relational victimization (Class 1), all-type victimization (Class 2), and noninvolved pattern (Class 3). The results found that females had a higher likelihood of being in Class 1 than males. Moreover, fifth to sixth and seventh to eighth-graders were more likely than ninth to tenth graders to be in both Class 1 and Class 2. Obese males and overweight females were more likely to be in Class 1 and Class 2 than in Class 3. In addition, obesity and overweight status were positively associated with bullying victimization among fifth to sixth and seven to eighth-graders. However, no significant association was found between obesity and overweight status and bullying victimization among ninth to tenth graders. Findings from the study suggest that school-based strategies, especially sex- and age-specific programs, need to take into consideration the bullying involvement of students who are overweight or obese.
Current status of weight bias and stigma in pediatrics and the need for greater focus on populations at risk

Abstract:Purpose of review: Obesity is one of the most common pediatric chronic conditions in the United States, affecting approximately 20% of American youth and is more common amongst Black, Latino, and Indigenous and low socioeconomic populations. The condition places children and adolescents at increased risk of physical and mental health conditions partly mediated by the weight bias and stigmatization experienced during the potentially vulnerable periods of childhood and adolescence. Recent findings: Weight bias and the resulting stigma are pervasive in society. Children have been shown to internalize this bias and its devaluation, which have been shown to contribute to worsening metabolic and mental health outcomes independently. Studies suggest weight stigmatization more adversely affects Black, Latino, and Indigenous children, suggesting the potential for adverse synergistic effects of these historical biases on such youth. Summary: Addressing childhood obesity successfully across all racial, ethnic, and socioeconomic lines requires addressing weight bias and stigma. Steps toward this end include collaborative efforts to promote cross-cultural competence and upstander bias education and training for those who care for children, person-centered communication, and a culture of inclusivity across governmental, healthcare, educational, entertainment, and advertising sectors.
Body shaming and internalized weight bias as potential precursors of eating disorders in adolescents

Abstract:Introduction: Body shaming (BS) is a growing phenomenon within the school context, especially among adolescents. Recently, it has been described as an unrepeated act in which a person expresses unsolicited, mostly negative comments about an individual's body. The targeted person perceives these comments as negative, offensive or body shame-inducing. Empirical evidence also suggests that body weight is the most common reason that youths are teased and bullied. Indeed, weight stigma, described as bias or discriminatory behaviors, attitudes, feelings, and thinking about individuals, because of their weight, can lead to weight-based discrimination and victimization. Preliminary evidence suggests that BS and weight stigma have negative effects on psychological health both in the short and long term. In the delicate stage of adolescence development and pubertal maturation, BS experiences can be highly prevalent and it can lead to adverse outcomes such as eating disorders (ED). However, prevalence data in the Italian context are still lacking. Methods: The study aims to estimate weight-related BS perceived by different sources (i.e., peers and family members) and their associations with public and internalized weight bias, body mass index (BMI), body dissatisfaction, and ED symptoms. A sample of 919 high school students ( M age = 15.97, SD = 1.58; 57.1% boys) completed a battery of self-report questionnaires assessing these variables. Results: One in four students reported experiences of weight-related BS by peers or family members. A total of 37% reported having at least one BS experience in a lifetime. Higher scores of ED symptoms, body dissatisfaction, and weight bias were reported by adolescents who experienced BS, especially females. Among overweight participants, results showed that internalized weight bias partially mediated the relationship between BS by family members and ED symptoms and fully mediated the relationship between BS by peers and ED symptoms, after controlling for age, sex and BMI. Discussion: These findings, despite their cross-sectional nature, add an important contribution to the creation of quantitative empirical evidence on the phenomenon of BS. Its role in explaining eating disorders, both alone and with the mediation of internalized weight stigma has been first proved and needs to be confirmed by longitudinal results.

Abstract:Summary Background Negative familial weight talk may contribute to higher weight bias internalization in pre‐ and early adolescents (hereafter referred to as children) and may differ by gender, weight status, and race and ethnicity. Objective Examine the relationship between negative familial weight talk and weight bias internalization and examine differences by gender, weight status, and race and ethnicity. Methods We cross‐sectionally analysed 5th–7th graders (10–15 years old) living in Massachusetts (n = 375, 52.3% girls, 21.3% BMI ≥85th percentile, 54.8% non‐Hispanic White). Negative familial weight talk frequency during the past 3 months was self‐reported and discretized as 'never,' 'occasionally' (1–9 times) and 'often' (>9 times); the Modified Weight Bias Internalization Scale assessed weight bias internalization. Generalized linear models estimated the relationship between negative familial weight talk and weight bias internalization and sub‐analyses estimated the relationship across gender, weight status, and race and ethnicity. Weight-teasing among adolescents: correlations with weight status and disordered eating behaviors

Abstract:OBJECTIVES: This study aimed to assess the prevalence of perceived weight-teasing and associations with unhealthy weight-control behaviors and binge eating in a population-based sample of youth. Particular focus was placed on overweight youth, who may be most vulnerable to weight-teasing.METHODS: The study population included 4746 adolescents from St Paul/Minneapolis public schools who completed surveys and anthropometric measurements as part of Project EAT, a population-based study of eating patterns and weight concerns among teens.RESULTS: There were statistically significant associations between perceived weight-teasing and weight status; both overweight and underweight youth reported higher levels of teasing than average weight youth. Very overweight youth (body mass index (BMI) ≥95th percentile) were most likely to be teased about their weight; 63% of very overweight girls, and 58% of very overweight boys reported being teased by their peers, while weight-teasing by family members was reported by 47% of these girls and 34% of these boys. Youth who were teased about their weight, particularly overweight girls, reported that it bothered them. Perceived weight-teasing was significantly associated with disordered eating behaviors among overweight and non-overweight girls and boys. For example, among overweight youth, 29% of girls and 18% of boys who experienced frequent weight-teasing reported binge-eating as compared to 16% of girls and 7% of boys who were not teased.CONCLUSIONS: Many adolescents, in particular those who are overweight, report being teased about their weight and being bothered by the teasing. Weight-teasing is associated with disordered eating behaviors that may place overweight youth at increased risk for weight gain. Educational interventions and policies are needed to curtail weight-related mistreatment among youth.

Abstract:There is evidence that sexual minority populations have a potentially heightened risk of poor health outcomes due in part to the discrimination they may face. In the present study, we examined whether overweightness and obesity vary by sexual minority subgroup using a large, nationally representative sample. Data were drawn from 2014–2017 Behavioral Risk Factor Surveillance System (BRFSS) surveys (n = 716,609). We grouped participants according to sexual identity (straight, lesbian or gay, bisexual, and other/don’t know/not sure). The propensity score matching technique was used to address covariate imbalance among sexual identity groups. In addition, subgroup analyses were performed for both males and females. Compared to straight adults, lesbian females had significantly higher odds of being overweight (OR (odds ratio) 1.33; 95% CI (confidence interval) 1.17–1.53), whereas gay males had significantly lower odds (OR 0.66; 95% CI 0.59–0.73). Similarly, lesbians were more likely to be obese (OR 1.49; 95% CI 1.31–1.70), whereas gay men had significantly lower odds of obesity (OR 0.77; 95% CI 0.69–0.86) when compared to straight adults. Bisexual females had significantly higher odds of being overweight (OR 1.21; 95% CI 1.10–1.34) and obese (OR 1.43; 95% CI 1.29–1.59), whereas bisexual males showed no significant difference. Our results strengthen previous findings and further highlight the need for research by sexual minority subgroup.

Abstract:Introduction The present study examined the between‐ and within‐person associations among negative weight‐related experiences, weight bias internalization, and body shame, embarrassment, and pride in adolescents. Methods Participants were 93 Canadian students (M age = 15.54, 59.10% girls, 40.86% white) who completed a 5‐day daily diary study in 2021. Multilevel models were estimated to examine the between‐ and within‐person associations, as well as the cross‐level interactions. Results Fifty‐nine negative weight‐related experiences were reported from 22 participants (23.66%) over the 5‐day study period. Adolescents with higher average negative weight‐related experiences (OR = 19.60, 95% CI = 1.90–202.67) and weight bias internalization (OR = 3.66, CI = 2.07–6.46) had greater odds of reporting shame. Similarly, higher average negative weight‐related experiences (OR = 16.29, CI = 3.65–72.75) and weight bias internalization (OR = 2.08, CI = 1.53–2.82) was associated with greater odds of embarrassment. No within‐person effects were noted, such that reporting more negative weight‐related experiences or weight bias internalization than one's own average was not related to body emotions. Conclusions This distinction underscores that the persistent, rather than episodic, aspects of negative weight‐related experiences and weight bias internalization are most impactful on adolescents' body image. These findings have implications for recruitment and screening for individual‐level interventions for internalized weight bias and body image, and highlight the need for system‐level policies and changes that prohibit negative weight‐related experiences and messages to reduce likelihood of internalizing weight bias among adolescents.

Abstract:The negative impacts of perceived discrimination on health have been documented, but how perceptions of discrimination vary by sex among people with obesity (PWO) is not well understood. This study assessed sex differences in perceived discrimination among PWO. This cross-sectional study analysed self-reported data from racially/ethnically diverse PWO attending an academic obesity program. The primary outcome was perceived discrimination, assessed by the Major Experiences of Discrimination (MED) and Everyday Discrimination Scale (EDS). Sex differences were evaluated by univariate and multivariable regression analysis adjusted for key descriptive variables. The final analytical sample included 60 PWO (62% female, 61% non-Hispanic white (NHW), 12% Hispanic, 39% non-Hispanic black (NHB), mean age 53 years, mean BMI 34 kg/m2). Women experienced everyday discrimination 59% less than men but this was not significant (OR 0.41; 95% CI 0.08-2.16; p = .29). Participant highest BMI was correlated with acute discrimination (total MED score) (r = 0.39; p = .0024) and chronic discrimination (total EDS score) (r = 0.28; p = .03). Most participants reported more chronic discrimination compared to acute discrimination (85% vs. 48%). Mean MED (β -0.18; 95% CI -0.93 to +0.56; p = .62) and EDS scores (β -0.25; 95% CI -3.37 to +2.87; p = .87) were not significantly different between women and men. Perceived discrimination was common and experienced equally between men and women with obesity. Incorporating discrimination experiences into obesity care models may be an important step to improving quality of care and outcomes.
