Abstract:Background: Adverse childhood experiences (ACEs) have been associated with poorer health from childhood into adulthood. There has been limited prior research examining the associations between positive childhood experiences (PCEs) and health among children. Objective: The present study examines the association between PCES and child health, controlling for ACE counts, using a nationally representative sample. Participants and setting: : The data for this study came from the 2019-2020 National Survey of Children's Health and were limited to children six years of age or older with complete demographic information and information on ACEs, PCEs, and child health (n = 46,913). Methods: Bivariate analyses between PCEs, ACEs, child/adolescent characteristics, or caregiver's characteristics and child/adolescent health were examined using Pearson's Chi-square tests, weighted to produce nationally representative distributions. Multivariable regression models were used to examine the association between selected PCEs and good health, controlling for whether a child had two or more ACEs. Results: In adjusted analyses, children who experienced any of the following PCEs had a higher odds of good health, compared to children who did not experience each type of these PCEs: after school activities (aOR 1.85; 95% CI 1.11-3.09), resilient family (aOR 2.22; 95% CI 1.45-3.41), supportive neighborhood (aOR 1.56; 95% CI 1.01-2.41), and connected caregiver (aOR 1.84; 95% CI 1.22-2.77). Conclusions: Examining and understanding PCEs and how they are associated with child health is a unique opportunity to guide more targeted policies and intervention efforts. Efforts to provide PCEs in schools, homes, and communities may help to reduce health inequities early in childhood.
Examining the influence of positive childhood experiences on childhood overweight and obesity using a national sample

Abstract:Positive childhood experiences (PCEs) promote healthy social development, improve overall wellness, and help to moderate and prevent exposure to adverse childhood experiences. There has been limited research examining the association between positive childhood experiences and overweight or obesity status in children. The purpose of this study was to examine whether experiencing positive childhood experiences are associated with lower rates of overweight or obesity status in children between 10 and 17 years of age, using cross-sectional data from the 2018-2019 National Survey of Children's Health (n = 28,771), a nationally representative mail and online survey. Frequencies, proportions, and unadjusted associations for each variable were calculated using descriptive statistics and bivariate analyses. To examine the association between overweight or obesity and PCEs, multivariable regression models were used. Compared to children who were underweight or had a healthy weight, children who were overweight or obese were less likely to: participate after school activities (78.1%, p < 0.0001), volunteer in their community, school, or church (45.6%, p < 0.0001), have a mentor they feel comfortable going to for guidance (87.0%, p = 0.02), live in a safe neighborhood (61.3%, p < 0.0001), live in a supportive neighborhood (50.4%, p < 0.0001), and to live with a resilient family (78.3%; p = 0.0099). In adjusted analysis, among children exposed to two or more ACEs, children residing in a supportive neighborhood were less likely to be overweight or obese (aOR 0.87; 0.77-0.98). Our findings suggest that certain PCEs may mitigate overweight and obesity when children have experienced at least some childhood trauma.

Abstract:Abstract Introduction Racism negatively affects children’s health, behavior, and relationships. Current literature focuses on associations between perceived discrimination and daytime behaviors in children and adolescents, with less known about associations with sleep behavior, an essential aspect on health during development. Racial/ethnic minorities, including Hispanic/Latinx youth, are at increased risk of sleep problems, along with mental health issues. Here, we examined the relationship between perceived discrimination and sleep and behavioral problems, and the potential role of familial cultural heritage connectedness as a mediator, in Hispanic/Latinx adolescents in the Adolescent Brain Cognitive Development (ABCD) Study®. Methods Data were analyzed from 1,213 Hispanic/Latinx adolescents (Mean age =11.89 years, range: 10-14 years, 48.22% female), collected as part of the ongoing ABCD Study® from the 2nd year follow up. Sleep was assessed with a question about having trouble falling or staying asleep in the past two weeks from the Kiddie Schedule for Affective Disorders and Schizophrenia (KSADS-5) (youth-report). Participants completed the Perceived Discrimination scale, an 11-item self-report about racial/ethnic perceived discrimination in the past year from teachers, peers, and other adults. Cultural heritage connectedness was gathered from the Vancouver Index of Acculturation (caregiver-report). Internalizing and externalizing behaviors were assessed using the Child Behavior Checklist (caregiver-report). Regression models examined associations between perceived discrimination and sleep behavior and externalizing/internalizing behavior, considering age, socio-demographic characteristics, parental education, and familial immigration status. Results Perceived discrimination correlated with more sleep problems (p< 0.01), with a stronger effect in female than male participants (p< 0.01). Perceived discrimination was also associated with internalizing and externalizing behaviors (p< 0.01). Connectedness to cultural heritage partially mediated the association between perceived discrimination and externalizing behaviors (p< 0.01), but not with sleep problems or internalizing behaviors. Conclusion Results show perceived discrimination is associated with sleep problems in Hispanic/Latinx adolescents, with a stronger relationship in female participants. Future investigation of links between perceived discrimination and health outcomes, including sleep and behaviors, is needed using longitudinal data and youth self-reports of the Vancouver Index of Acculturation, rather than caregiver reports to further investigate the potential role of cultural connectedness is these relationships. Abstract:In clinical research, there has been a call to move beyond individual psychosocial factors towards identifying cultural and social factors that inform mental health. Similar calls have been made in the eating disorders (ED) field underscoring the need to understand larger sociocultural influences on EDs. Discrimination is a social stressor that may influence mental health in similar ways to traumatic or adverse childhood experiences (ACEs). Given the high rates of EDs and discrimination among marginalized groups, it is vital to understand the role of discrimination and ACEs as predictors of ED symptoms in these populations. The aim of this study is to examine how perceived discrimination predicts ED pathology when statistically adjusting for gender, race, and ACEs.

Positive Childhood Experiences and Adult Mental and Relational Health in a Statewide Sample: Associations Across Adverse Childhood Experiences Levels

Abstract:Importance: Associations between adverse childhood experiences (ACEs) and risks for adult depression, poor mental health, and insufficient social and emotional support have been documented. Less is known about how positive childhood experiences (PCEs) co-occur with and may modulate the effect of ACEs on adult mental and relational health. Objective: To evaluate associations between adult-reported PCEs and (1) adult depression and/or poor mental health (D/PMH) and (2) adult-reported social and emotional support (ARSES) across ACEs exposure levels. Design, setting, and participants: Data were from the cross-sectional 2015 Wisconsin Behavioral Risk Factor Survey, a random digit-dial telephone survey of noninstitutionalized Wisconsin adults 18 years and older (n = 6188). Data were weighted to be representative of the entire population of Wisconsin adults in 2015. Data were analyzed between September 2016 and January 2019. Main outcomes and measures: The definition of D/PMH includes adults with a depression diagnosis (ever) and/or 14 or more poor mental health days in the past month. The definition of PCEs includes 7 positive interpersonal experiences with family, friends, and in school/the community. Standard Behavioral Risk Factor Survey ACEs and ARSES variables were used. Results: In the 2015 Wisconsin Behavioral Risk Factor Survey sample of adults (50.7% women; 84.9% white), the adjusted odds of D/PMH were 72% lower (OR, 0.28; 95% CI, 0.21-0.39) for adults reporting 6 to 7 vs 0 to 2 PCEs (12.6% vs 48.2%). Odds were 50% lower (OR, 0.50; 95% CI, 0.36-0.69) for those reporting 3 to 5 vs 0 to 2 PCEs (25.1% vs 48.2%). Associations were similar in magnitude for adults reporting 1, 2 to 3, or 4 to 8 ACEs. The adjusted odds that adults reported "always" on the ARSES variable were 3.53 times (95% CI, 2.60-4.80) greater for adults with 6 to 7 vs 0 to 2 PCEs. Associations for 3 to 5 PCEs were not significant. The PCE associations with D/PMH remained stable across each ACEs exposure level when controlling for ARSES. Conclusions and relevance: Positive childhood experiences show dose-response associations with D/PMH and ARSES after accounting for exposure to ACEs. The proactive promotion of PCEs for children may reduce risk for adult D/PMH and promote adult relational health. Joint assessment of PCEs and ACEs may better target needs and interventions and enable a focus on building strengths to promote well-being. Findings support prioritizing possibilities to foster safe, stable nurturing relationships for children that consider the health outcomes of positive experiences.

Abstract:Cumulative exposure to ACEs is associated with internalizing disorders throughout life. Efforts to move beyond the cumulative risk framework call for assessment of various aspects of adversity (e.g., timing) to improve identification of mechanistic outcomes and accuracy in predicting individual health risk following ACEs. Here, we test whether a cumulative ACE score, an average score of perceived negative effect, or their interaction, are associated with internalizing symptoms among young adults. Six hundred undergraduates ( M age = 19.05; 75% female; 69% Black) completed questionnaires related to ACEs, anxiety, and depressive symptoms. The perceived negative effect score was independently associated with anxiety and depressive symptoms, whereas a cumulative ACE score was only associated with depressive symptoms. No interactions were detected. The findings highlight the value in assessing perception as one aspect of exposure to ACEs. Future research should consider other contextual factors, and how they may interact with perception to influence health outcomes.

Negative Pressure Wound Therapy for a Giant Wound Secondary to Malignancy-induced Necrotizing Fasciitis: Case Report and Review of the Literature.

Abstract:BACKGROUND Necrotizing fasciitis (NF) is a life-threatening condition in which rapid diagnosis, debridement of nonviable tissue, and broad-spectrum antibiotics are critical to effective treatment. The debridement required can be extensive, resulting in large wounds that can sometimes be covered with split-thickness skin grafts (STSGs) with the help of negative pressure wound therapy (NPWT), or vacuum-assisted closure, to decrease the wound size. CASE REPORT The authors report a rare case of NF due to malignancy-associated bowel perforation with a giant lower extremity wound secondary to debridement that involved 20% of the total body surface area (TBSA) in a 64-year-old, previously healthy, nonsmoking man. The wound was surgically debrided twice and packed before NPWT was applied. Based on the authors' literature search, this case is 1 of the single largest wounds successfully managed with a STSG and NPWT. CONCLUSIONS Rapid diagnosis of NF is critical to guide surgical management and administration of antibiotics. It is important to be mindful of the origin of certain necrotizing infections, and clinicians should have a greater index of suspicion for NF when assessing skin infections in unwell patients with concomitant bowel perforation secondary to gastrointestinal malignancy.

Abstract:Purpose This study aims to investigate the prospective associations between four types of perceived discrimination (country of origin, race and ethnicity, sexual orientation, and weight) and the development of manic symptoms in a diverse, nationwide sample of adolescents aged 9–14 years in the U.S. Methods We analyzed prospective cohort data from the Adolescent Brain Cognitive Development Study ( N = 7466; ages 9–14 years at Year 1 or 2 in 2017–2020; 48.5 % female; 39.4 % racial/ethnic minority). Multiple zero-inflated negative binomial analyses were conducted to examine the associations between Year 1 or 2 discrimination (by country of origin, race and ethnicity, sexual orientation, weight, sum score), and Year 3 manic symptoms (7 Up Mania scale), adjusting for covariates (age, sex, race and ethnicity, household income, parental education, sipping alcohol, puffing tobacco, anxiety symptoms, depressive symptoms, Year 1 manic symptoms, and study site). Results After adjusting for covariates, perceived discrimination based on country of origin (incidence rate ratio [IRR] = 1.46; 95 % confidence interval [CI] 1.15–1.86), sexual orientation (IRR = 1.36; 95 % CI 1.21–1.53), race and ethnicity (IRR = 1.28; 95 % CI 1.13–1.46), weight (IRR = 1.21 95 % CI 1.09–1.34), and sum scores (IRR = 1.18 5 % CI 1.12–1.24), were significantly associated with higher manic symptoms. Conclusion Perceived discrimination based on country of origin, race and ethnicity, sexual orientation, weight, and sum scores, are prospectively associated with greater manic symptoms in adolescents. These findings underscore the need for targeted interventions to address discrimination and associated psychological impacts. Efforts to reduce discrimination and to support affected adolescents are important components of comprehensive mental health care and public health strategies.

Abstract:Background As cardiovascular disease (CVD) is the leading cause of death worldwide, more research is needed on the psychosocial factors that can reduce CVD risk across development. One cross-sectional study demonstrated that retrospectively-reported positive childhood experiences (PCEs) are associated with lower CVD risk in adulthood (Slopen et al., 2017). The current study extends this research by examining prospective associations between PCEs assessed during adolescence and CVD risk in young adulthood. Method The current analyses used data from the National Longitudinal Study of Adolescent to Adult Health, a nationally representative sample of adolescents followed into adulthood. The analytic sample (N= 13,567; 53% female) included participants who completed in-home interviews at Wave 1 (M age= 16.07, SD= 1.69) and Wave 4 (M age = 28.95, SD= 1.72). PCEs were assessed via 14 items in Wave 1, as adapted from the Benevolent Childhood Experiences scale-Revised (Narayan et al., 2018; e.g., whether individuals feel safe in their neighborhood or feel socially accepted). A 30-year CVD risk score was calculated in Wave 4 using a Framingham-based prediction model (Pencina et al., 2009). Covariates included gender, race/ethnicity, age at outcome, and childhood maltreatment (measured via retrospective reports of physical, emotional, and sexual abuse). Results Linear regression analyses indicate that higher PCEs predicted lower CVD risk, even after controlling for covariates (β= -.047, p <.001). Conclusion Findings from these analyses highlight the importance of assessing and screening for PCEs. Future prevention efforts may focus on increasing PCEs to reduce adult CVD risk.

Abstract:Objective Although rates of weight discrimination are on‐par with racial, ethnic, and gender discrimination, comparatively less work has examined impacts of weight‐based discrimination in youth, including on disordered eating. Knowing whether experiences of weight‐based discrimination, including in youth with multiply‐marginalized identities, are associated with disordered eating could identify vulnerable youth and inform intervention efforts. Method Youth (N = 11,875) ages 10–11 were recruited through the Adolescent Brain Cognitive Development (ABCD) study. Logistic regressions using cross‐sectional data examined discrimination experiences (weight, perceived sexual orientation, race/ethnicity, national origin) and disordered eating (binge‐eating, vomiting, weight‐gain fear, weight self‐worth). Models included race/ethnicity, age, sex, parental income, and degree of elevated weight. Raked poststratification weights were used. Results Rates of weight‐based discrimination (6.2%) were similar to rates of race/ethnicity (4.4%) and sexual orientation discrimination (4.5%). Weight‐based discrimination was associated with more disordered eating. Youth reporting multiple experiences of discrimination had significantly increased disordered eating compared to youth who did not report discrimination. Discussion Weight‐based discrimination is common in youth and associated with disordered eating. Youth with elevated weight are more likely to be multiply marginalized and experience disordered eating. These findings suggest discrimination, including weight‐based discrimination, is a critical intervention target to prevent and treat eating disorders. Public Significance Experiences of discrimination contribute to poorer health; however, weight‐based discrimination is relatively understudied. Data from a large sample of youth ages 10–11 showed that youth with elevated weight reported experiencing multiple types of discrimination, and multiply‐marginalized youth had increased odds of disordered eating with each additional type of discrimination. Together, this suggests that weight‐based discrimination is a critical target to prevent and treat eating disorders, especially in multiply‐marginalized youth.

Abstract:Little is known about how discrimination contributes to health behaviors in childhood. We examined the association between children's exposure to discrimination and their snacking behavior in a sample of youth of color (N = 164, Mage = 11.5 years, 49% female, 60% Black, 40% Hispanic/Latinx). We also explored whether children's body mass index (BMI) or sleepiness moderated the association between discrimination and calorie consumption. The significant link between discrimination and calorie consumption was moderated by children's BMI, such that discrimination was associated with calorie consumption for children with BMI percentiles above 79%. Children's sleepiness did not serve as an additional moderator. Efforts to promote health should consider children's broader socio‐contextual experiences, including discrimination, as factors that may shape eating patterns.

Abstract:Background: Research shows that adverse childhood experiences (ACEs) are problematic and may impact delinquency and arrest in adolescence. However, resiliency theories suggest the association between ACEs and delinquency/arrest may be mitigated by positive childhood experiences (PCEs). Objective: This study tests the hypothesis that an accumulation of PCEs ameliorates the relationship between ACEs and delinquency and arrest during adolescence. Participants and setting: Prospective data were collected from approximately 800 participants in the Longitudinal Studies of Child Abuse and Neglect (LONGSCAN). Methods: Self-report data from 16-year-old participants were used to measure the two dependent variables: the number of delinquent acts committed and having at least one arrest in the past year. Count measures reflected the number of ACEs and PCEs, with the former based on reports from child protective services, caregivers, and youth collected from ages 4-12, and the latter based on self-reports at age 12. Negative binomial models analyzed delinquency outcomes, logistic regression models examined arrest, and interaction terms assessed moderating effects. Results: ACEs were associated with significantly more delinquent acts but not arrest, and PCEs were associated with significantly fewer delinquent acts but not arrest. PCEs significantly reduced the relationship between ACEs and delinquency but only for youth with moderate levels of ACEs. Conclusions: Findings indicate a need for additional research measuring the moderating effects of a variety of PCEs and to the need to enhance positive experiences for youth to prevent delinquency.

Abstract:The study has evaluated in vivo, ex vivo and in vitro ontogenesis and functional significance of the arcuate nucleus neurons expressing either individual enzymes of dopamine synthesis, tyrosine hydroxylase or aromatic L-amino acid decarboxylase as well as both of them (dopaminergic neurons) in rats from the 17th embryonic day to adulthood. Monoenzymatic tyrosine hydroxylase-containing neurons were initially observed on the 18th embryonic day. On the 20-21 day, the monoenzymatic tyrosine hydroxylase- or aromatic L-amino acid decarboxylase-expressing neurons comprised more than 99% of the whole neuron population expressing the dopamine-synthesizing enzymes. The dopamine production in the fetus arcuate nucleus was sufficient to provide an inhibitory control of prolactin secretion like in adults. The data suggest a possibility of the dopamine synthesis in the fetus arcuate nucleus by the monoenzymatic neurons containing either tyrosine hydroxylase or aromatic L-amino acid decarboxylase-expressing neurons in co-operation.
Peer Discrimination, Deviant Peer Affiliation, and Latino/a Adolescent Internalizing and Externalizing Symptoms: A Prospective Study

Abstract:Objective: U.S. Latino/a adolescents experience high levels of ethnic discrimination, particularly in new immigrant destinations. Due to the salience of peers during adolescence, this study examined how peer discrimination related directly and indirectly, through deviant peer affiliation, to changes in Latino/a adolescents' internalizing and externalizing symptoms. Culture-specific moderators hypothesized to buffer discrimination impacts on adolescent symptomology included Spanish language enculturation and adolescents' social ties to relatives in the family's country-of-origin. Method: The sample of 547 Latino/a adolescent participants from the Caminos al Bienestar study (55.4% female; age M = 12.8, range = 11-16) was selected at random from middle schools in a large, suburban school district in Atlanta, Georgia. Three time points of survey data spaced roughly 6 months apart were collected during 2018 and 2019. Results: Results from longitudinal structural equation models revealed that peer discrimination was associated indirectly with increased externalizing symptoms, through increases in affiliation with deviant peers (β = 0.05; SE = 0.02; B = 0.02; 95% CI = 0.01, 0.09). We did not observe direct or indirect effects of peer discrimination on changes in internalizing symptoms, and we found no significant protective effects of either Spanish language enculturation or social ties with the country-of-origin. Conclusions: Ethnic discrimination by peers may lead to deviant peer affiliation and, in turn, increased externalizing behaviors. Future research identifying protective factors that buffer discrimination impacts on deviant peer affiliation is needed to inform the development of interventions that can prevent Latino/a adolescents' externalizing symptoms.
Longitudinal and Geographic Trends in Perceived Racial Discrimination Among Adolescents in the U.S.: The Adolescent Brain Cognitive Development (ABCD) Study

Abstract:This study examines longitudinal and geographic trends in perceived racial discrimination among U.S. adolescents using data from the Adolescent Brain Cognitive Development (ABCD) Study. A diverse sample of 11,868 children aged 9-10 at baseline from 22 sites across the U.S. was analyzed, assessing perceived discrimination at ages 10-11, 11-12, and 13-14 using items adapted from the Perceived Discrimination Scale. Binomial logistic regression models were used to evaluate longitudinal trends and geographic variation, adjusting for demographic factors such as race/ethnicity, parental education, and income. Results show that perceived racial discrimination increased significantly from ages 10-11 to 13-14, particularly among Black and Asian adolescents. By age 13-14, nearly half of Black adolescents and over a quarter of Asian adolescents reported discrimination. Geographic analysis revealed that Black adolescents in the Western U.S. and predominantly White affluent neighborhoods had the highest odds of perceived discrimination. Higher state-level anti-Black bias was associated with lower discrimination rates among Black adolescents but higher rates for Asian adolescents. These findings highlight the evolving nature of racial discrimination during adolescence and underscore the need for targeted interventions that address racisms mental health impacts on adolescents, particularly in high-risk geographic and socio-economic contexts.
Impact of adverse childhood experiences on risk for internalizing psychiatric disorders in youth at clinical high-risk for psychosis.

Abstract:Introduction Research has established that adverse childhood experiences (ACEs) confer risk for psychiatric diagnoses, and that protective factors moderate this association. Investigation into the effect of protective factors in the relationship between ACEs and internalizing disorders (e.g., depression, anxiety) is limited in high-risk groups. The present study investigated the relationship between ACEs and risk for internalizing disorders in youth at clinical high risk for psychosis (CHR-P) and tests the hypothesis that protective factors moderate this relationship. Methods 688 participants aged 12-30 ( M = 18; SD =4.05) meeting criteria for CHR-P were administered measures of child adversity, protective factors (SAVRY), and diagnostic assessment (SCID- 5). Logistic regression tested whether ACEs predicted internalizing disorders. Moderation regression analyses determined whether these associations were weaker in the presence of protective factors. Results & Conclusions Higher levels of ACEs predicted history of depressive disorder (β= .26(1.30), p <.001), self-harm/suicide attempts (β= .34(1.40), p <.001), and substance use (β= .14(1.15), p =.04). Childhood sexual abuse (β= .77(2.15), p=.001), emotional neglect (β= .38(1.46), p =.05), and psychological abuse (β= .42(1.52), p =.04), predicted self- harm/suicide attempts. Sexual abuse (β= 1.00 (2.72), p=.001), and emotional neglect (β= .53(1.71), p=.011), were also linked to depressive disorder. There was no association between ACEs and anxiety disorder, and no moderation effect of protective factors in the relationship between ACEs and psychiatric outcomes. These findings add nuance to a growing literature linking ACEs to psychopathology and highlight the importance of investigation into the mechanisms that may buffer this relationship.

Abstract:This study is based on the hopelessness theory of depression and previous research on perceived everyday discrimination (PED) and both depressive symptoms and Interleukin‐6 (an inflammatory cytokine; IL‐6) in adolescents. The purpose of this study is to examine the negative attribution, self, and consequence cognitive styles (CSs) proposed in the hopelessness theory as a possible mechanism underlying the association between PED and inflammation in adolescents and expand our understanding of the comorbidities between depressive symptoms and systemic inflammation (IL‐6). This cross‐sectional study featured a sample of 102 adolescents aged 13–16 (M = 14.10, SD = 0.52) who identified as White (47.5%), Black (41.4%), Mixed Race (7.1%), Latino (2%), and other (2%). Data analysis was conducted using PROCESS to compute regressions and effects between PED, negative CSs, depressive symptoms, and Interleukin‐6. Results showed that negative attribution CS is the only negative CS associated with PED, depressive symptoms, and IL‐6. Negative attribution CS is also the only negative CS of the three negative CSs that mediates both the association between PED and depressive symptoms and PED and IL‐6 in our adolescent sample. Overall, these results indicate that individual negative CSs proposed in the hopelessness theory impact adolescents' physical and mental outcomes differently, which can inform targeted treatments. Nurses should provide cognitive‐based interventions and promote societal‐level change to reduce the experience and impact of PED on the mental and physical health of their adolescent patients.
