Abstract:To quantitatively and qualitatively evaluate a deep learning auto contouring model for prostate radiotherapy patients with pretreatment insertion of a hydrogel spacer (about water equivalent with no contrast) between prostate and rectum. The model employs convolutional neural networks (CNN) to learn features from input images that can be used to generate semantic segmentation. The study used 163 patients from three specialized GU radiation oncologists (referred to as A/B/C). The first 135 patients (A/B/C = 82/39/14) were used for training (125) and validation (10). The validation patients were randomly selected. The validated model was tested on 28 patients (A/B/C = 18/6/4) accrued during model development. There was no change of practice during the whole period. A simulation CT and MR were taken on the same day for each patient. In manual contouring, with MR fused to CT, spacer was contoured on T2 MR, prostate on CT with MR guidance, and other structures on CT only. The model was trained to auto contour prostate, proximal seminal vesicles (SV), bladder, rectum, penile bulb, femurs and spacer on CT without MR. Quantitatively, auto contours were evaluated against manual contours using the following metrics: sensitivity (% of voxels correctly drawn), false positive rate (FPR, % of voxels overdrawn), dice similarity coefficient (DSC), 95-percentile of Hausdorff distance (HD) and mean distances (dmean) between the two contours over all slices. The structures with high DSC were qualitatively evaluated by the original attending using a 1 (acceptable with minor editing), 2 (editable with efficiency gain over manual contouring) and 3 (rejected for no efficiency gain or gross error) scoring system. A gross error on rectum occurred for two patients (A/B = 1/1). These two points were excluded from quantitative analysis but counted as rejected in qualitative evaluation. On average, DSC was high for femurs (>0.95) and bladder (0.91), moderate for prostate (0.85) and rectum (0.81), but low for bulb (0.67), proximal SV (0.62) and spacer (0.52). For right femur/left femur/bladder/prostate/rectum, sensitivity = 0.93/0.92/0.88/0.86/0.81, FPR = 1.8%/1.5%/4.5%/15%/17%, 95% 95%-HD = 2.8/2.6/12.1/7.4/9.5 mm, and dmean = 0.9/1.0/2.6/2.5/2.4 mm. Qualitatively, femurs scored 1 in all cases. The average scores for bladder/prostate/rectum = 1.28/1.44/1.50, 1.83/2.17/1.67, 1.25/1.50/1.25 for physicians A, B, C, respectively, and 1.39/1.61/1.50 overall. Prostate and rectum both scored well below 2, despite their lower quantitative performance, as some errors caused by the inaccurate prediction of spacer without MR were deemed easily correctable by the physicians. The model produced clinically satisfactory results, both quantitatively and qualitatively, for femurs, bladder, prostate and rectum. The results for proximal SV and bulb were less ideal. The model drew the spacer in the correct location, but could not draw it accurately due to lack of contrast on CT.
Meralgia paresthetica: another complication of antipsychotic‐induced weight gain

Abstract:Dear Editor, Weight gain is a well known side effect of many antipsychotic and antidepressant medications. The incidence of substantial or significant weight gain with olanzapine is reported to be up to 7% (1). Obesity is known to increase the risk for metabolic side effects. In addition, obesity associated with an increased abdominal girth can double the risk of developing meralgia paresthetica (MP). MP is often unrecognized or misdiagnosed, resulting in chronic pain and disability (2). In our clinical practice, we became suspicious of MP in two patients whose increased abdominal girth with protrusion over their tight belts and pants lead to their complaints of severe, lateral thigh pain. We attributed the neuropathy to the adverse effect of olanzapine – induced weight gain. Meralgia paresthetica, or Bernhardt–Roth Syndrome, was first described in the late 1890s and named in the 1900s. This syndrome of burning pain, tingling, numbness or ‘dysthesia’ is due to compression of the lateral femoral cutaneous nerve at the level of the anterior iliac spine. The syndrome was once most commonly diagnosed in soldiers who continually wore wide military belts that fit tightly around the waist. There are case reports of solders in the Iraq war who wore military armour and developed MP (3). The belt and the tight pants resulted in the compression of the nerve. An abnormal increase in weight and abdominal girth itself, or its resultant pressure on clothing waistbands may create a similar pressure-related nerve compression injury. The lateral femoral cutaneous nerve is derived from the posterior L2 and L3 nerve roots, and normally runs medially to the anterior superior iliac spine of the hip and under the inferior portion of the inguinal ligament. (Fig. 1) The nerve then forms an angle as it leaves the pelvis. This angle is increased when the leg is extended and decreased when the leg is flexed. The nerve then divides beneath the thigh fascia and becomes superficial. The lateral femoral cutaneous nerve is vulnerable to entrapment or compression when the nerve passes through the fibrous canal near the end of the inguinal nerve, and comes through the fascia to become superficial. The primary cause of the syndrome remains obscure. However, localized trauma, surgery and scarring, pregnancy, obesity, ascites, seat belts and restrictive clothing such as tight or low cut pants and belts are highly associated with the onset (4). Toxins (lead) and metabolic factors (alcoholism and diabetes) can contribute to paraesthesias (5). Unfortunately, it is not a rare condition and a misdiagnosis or delay in the diagnosis can cause permanent nerve damage. Diagnostic tools include electromygraphy (EMG) neurography, somatosensory evoked potential, imaging studies and/or a diagnostic nerve block (6,7). In MP, the EMG of the thigh muscles is usually normal, but is useful to rule out myleopathy and often includes a nerve conduction study. The initial treatment includes correction of mechanical problem, such as weight loss and removal of restrictive clothing and belts. Nonsteroidal anti-inflammatory drugs, nerve blocks and steroid injections are used following treatment if needed. Decompressive surgery may be necessary (6–11). If treatment is withheld too long, the condition may not be reversible.
Evaluation of Deep Learning‐based Auto‐segmentation Algorithms for Delineating Clinical Target Volume and Organs at Risk Involving Data for 125 Cervical Cancer Patients

Abstract:Objective To evaluate the accuracy of a deep learning-based auto-segmentation mode to that of manual contouring by one medical resident, where both entities tried to mimic the delineation "habits" of the same clinical senior physician. Methods This study included 125 cervical cancer patients whose clinical target volumes (CTVs) and organs at risk (OARs) were delineated by the same senior physician. Of these 125 cases, 100 were used for model training and the remaining 25 for model testing. In addition, the medical resident instructed by the senior physician for approximately 8 months delineated the CTVs and OARs for the testing cases. The dice similarity coefficient (DSC) and the Hausdorff Distance (HD) were used to evaluate the delineation accuracy for CTV, bladder, rectum, small intestine, femoral-head-left, and femoral-head-right. Results The DSC values of the auto-segmentation model and manual contouring by the resident were, respectively, 0.86 and 0.83 for the CTV (P < 0.05), 0.91 and 0.91 for the bladder (P > 0.05), 0.88 and 0.84 for the femoral-head-right (P < 0.05), 0.88 and 0.84 for the femoral-head-left (P < 0.05), 0.86 and 0.81 for the small intestine (P < 0.05), and 0.81 and 0.84 for the rectum (P > 0.05). The HD (mm) values were, respectively, 14.84 and 18.37 for the CTV (P < 0.05), 7.82 and 7.63 for the bladder (P > 0.05), 6.18 and 6.75 for the femoral-head-right (P > 0.05), 6.17 and 6.31 for the femoral-head-left (P > 0.05), 22.21 and 26.70 for the small intestine (P > 0.05), and 7.04 and 6.13 for the rectum (P > 0.05). The auto-segmentation model took approximately 2 min to delineate the CTV and OARs while the resident took approximately 90 min to complete the same task. Conclusion The auto-segmentation model was as accurate as the medical resident but with much better efficiency in this study. Furthermore, the auto-segmentation approach offers additional perceivable advantages of being consistent and ever improving when compared with manual approaches.

Abstract:ObjectiveAutomated methods for prostate segmentation on MRI are typically developed under ideal scanning and anatomical conditions. This study evaluates three different prostate segmentation AI algorithms in a challenging population of patients with prior treatments, variable anatomic characteristics, complex clinical history, or atypical MRI acquisition parameters.Materials and methodsA single institution retrospective database was queried for the following conditions at prostate MRI: prior prostate-specific oncologic treatment, transurethral resection of the prostate (TURP), abdominal perineal resection (APR), hip prosthesis (HP), diversity of prostate volumes (large ≥ 150 cc, small ≤ 25 cc), whole gland tumor burden, magnet strength, noted poor quality, and various scanners (outside/vendors). Final inclusion criteria required availability of axial T2-weighted (T2W) sequence and corresponding prostate organ segmentation from an expert radiologist. Three previously developed algorithms were evaluated: (1) deep learning (DL)-based model, (2) commercially available shape-based model, and (3) federated DL-based model. Dice Similarity Coefficient (DSC) was calculated compared to expert. DSC by model and scan factors were evaluated with Wilcox signed-rank test and linear mixed effects (LMER) model.Results683 scans (651 patients) met inclusion criteria (mean prostate volume 60.1 cc [9.05–329 cc]). Overall DSC scores for models 1, 2, and 3 were 0.916 (0.707–0.971), 0.873 (0–0.997), and 0.894 (0.025–0.961), respectively, with DL-based models demonstrating significantly higher performance (p < 0.01). In sub-group analysis by factors, Model 1 outperformed Model 2 (all p < 0.05) and Model 3 (all p < 0.001). Performance of all models was negatively impacted by prostate volume and poor signal quality (p < 0.01). Shape-based factors influenced DL models (p < 0.001) while signal factors influenced all (p < 0.001).ConclusionFactors affecting anatomical and signal conditions of the prostate gland can adversely impact both DL and non-deep learning-based segmentation models.

Abstract:Inter-and intra-observer variation in delineating regions of interest (ROIs) occurs because of differences in expertise level and preferences of the radiation oncologists. We evaluated the accuracy of a segmentation model using the U-Net structure to delineate the prostate, bladder, and rectum in male pelvic CT images. The dataset used for training and testing the model consisted of raw CT scan images of 85 prostate cancer patients. We designed a 2D U-Net model to directly learn a mapping function that converts a 2D CT grayscale image to its corresponding 2D OAR segmented image. Our network contains blocks of convolution 2D layers with variable kernel sizes, channel number, and activation functions. On the left side of the U-Net model, we used three 3x3 convolutions, each followed by a rectified linear unit (ReLu) (activation function), and one max pooling operation. On the right side of the U-Net model, we used a 2x2 transposed convolution and two 3x3 convolution networks followed by a ReLu activation function. The automatic segmentation using the U-Net generated an average dice similarity coefficient (DC) and standard deviation (SD) of the following: DC +- SD (0.88 +- 0.12), (0.95 +- 0.04), and (0.92 +- 0.06) for the prostate, bladder, and rectum, respectively. Furthermore, the mean of average surface Hausdorff distance (ASHD) and SD were 1.2 +- 0.9 mm, 1.08 +- 0.8 mm, and 0.8 +- 0.6 mm for the prostate, bladder, and rectum, respectively. Our proposed method, which employs the U-Net structure, is highly accurate and reproducible for automated ROI segmentation. This provides a foundation to improve automatic delineation of the boundaries between the target and surrounding normal soft tissues on a standard radiation therapy planning CT scan.

Abstract:Purpose The aim of this study was to evaluate generalization ability of segmentation accuracy for limited FOV CBCT in the male pelvic region using a full‐image CNN. Auto‐segmentation accuracy was evaluated using various datasets with different intensity distributions and FOV sizes. Methods A total of 171 CBCT datasets from patients with prostate cancer were enrolled. There were 151, 10, and 10 CBCT datasets acquired from Vero4DRT, TrueBeam STx, and Clinac‐iX, respectively. The FOV for Vero4DRT, TrueBeam STx, and Clinac‐iX was 20, 26, and 25 cm, respectively. The ROIs, including the bladder, prostate, rectum, and seminal vesicles, were manually delineated. The U2‐Net CNN network architecture was used to train the segmentation model. A total of 131 limited FOV CBCT datasets from Vero4DRT were used for training (104 datasets) and validation (27 datasets); thereafter the rest were for testing. The training routine was set to save the best weight values when the DSC in the validation set was maximized. Segmentation accuracy was qualitatively and quantitatively evaluated between the ground truth and predicted ROIs in the different testing datasets. Results The mean scores ± standard deviation of visual evaluation for bladder, prostate, rectum, and seminal vesicle in all treatment machines were 1.0 ± 0.7, 1.5 ± 0.6, 1.4 ± 0.6, and 2.1 ± 0.8 points, respectively. The median DSC values for all imaging devices were ≥0.94 for the bladder, 0.84–0.87 for the prostate and rectum, and 0.48–0.69 for the seminal vesicles. Although the DSC values for the bladder and seminal vesicles were significantly different among the three imaging devices, the DSC value of the bladder changed by less than 1% point. The median MSD values for all imaging devices were ≤1.2 mm for the bladder and 1.4–2.2 mm for the prostate, rectum, and seminal vesicles. The MSD values for the seminal vesicles were significantly different between the three imaging devices. Conclusion The proposed method is effective for testing datasets with different intensity distributions and FOV from training datasets.

Abstract:Prostate cancer represents a major threat to health. Early detection is vital in reducing the mortality rate among prostate cancer patients. One approach involves using multi-modality (CT, MRI, US, etc.) computer-aided diagnosis (CAD) systems for the prostate region. However, prostate segmentation is challenging due to imperfections in the images and the prostate's complex tissue structure. The advent of precision medicine and a significant increase in clinical capacity have spurred the need for various data-driven tasks in the field of medical imaging. Recently, numerous machine learning and data mining tools have been integrated into various medical areas, including image segmentation. This article proposes a new classification method that differentiates supervision types, either in number or kind, during the training phase. Subsequently, we conducted a survey on artificial intelligence (AI)-based automatic prostate segmentation methods, examining the advantages and limitations of each. Additionally, we introduce variants of evaluation metrics for the verification and performance assessment of the segmentation method and summarize the current challenges. Finally, future research directions and development trends are discussed, reflecting the outcomes of our literature survey, suggesting high-precision detection and treatment of prostate cancer as a promising avenue.

Abstract:Purpose: Manual contouring of gross tumor volumes (GTV) is a crucial and time-consuming process in rectum cancer radiotherapy. This study aims to develop a simple deep learning-based autosegmentation algorithm to segment rectal tumors on T2-weighted MR images. Material and methods: MRI scans (3T, T2-weighted) of 93 patients with locally advanced (cT3-4 and/or cN1-2) rectal cancer treated with neoadjuvant chemoradiotherapy followed by surgery were enrolled in this study. A 2D U-net similar network was established as a training model. The model was trained in two phases to increase efficiency. These phases were tumor recognition and tumor segmentation. An opening (erosion and dilation) process was implemented to smooth contours after segmentation. Data were randomly separated into training (90%) and validation (10%) datasets for a 10-folder cross-validation. Additionally, 20 patients were double contoured for performance evaluation. Four indices were calculated to evaluate the similarity of automated and manual segmentation, including Hausdorff distance (HD), average surface distance (ASD), Dice index (DSC), and Jaccard index (JSC). Results: The DSC, JSC, HD, and ASD (mean +/- SD) were 0.74 +/- 0.14, 0.60 +/- 0.16, 20.44 +/- 13.35, and 3.25 +/- 1.69 mm for validation dataset; and these indices were 0.71 +/- 0.13, 0.57 +/- 0.15, 14.91 +/- 7.62, and 2.67 +/- 1.46 mm between two human radiation oncologists, respectively. No significant difference has been observed between automated segmentation and manual segmentation considering DSC (P = 0.42), JSC (P = 0.35), HD (P = 0.079), and ASD (P = 0.16). However, significant difference was found for HD (P = 0.0027) without opening process. Conclusion: This study showed that a simple deep learning neural network can perform segmentation for rectum cancer based on MRI T2 images with results comparable to a human. Abstract:Magnetic resonance imaging (MRI) has a growing role in the clinical workup of prostate cancer. However, manual three-dimensional (3D) segmentation of the prostate is a laborious and time-consuming task. In this scenario, the use of automated algorithms for prostate segmentation allows us to bypass the huge workload of physicians. In this work, we propose a fully automated hybrid approach for prostate gland segmentation in MR images using an initial segmentation of prostate volumes using a custom-made 3D deep network (VNet-T2), followed by refinement using an Active Shape Model (ASM). While the deep network focuses on three-dimensional spatial coherence of the shape, the ASM relies on local image information and this joint effort allows for improved segmentation of the organ contours. Our method is developed and tested on a dataset composed of T2-weighted (T2w) MRI prostatic volumes of 60 male patients. In the test set, the proposed method shows excellent segmentation performance, achieving a mean dice score and Hausdorff distance of 0.851 and 7.55 mm, respectively. In the future, this algorithm could serve as an enabling technology for the development of computer-aided systems for prostate cancer characterization in MR imaging.

Comparative Clinical Evaluation of Deep-Learning-Based Algorithms in Auto-Segmentation of Organs-At-Risk for Head and Neck Cancers

Abstract:This IRB-approved study evaluated the quality of contours auto-generated by two deep learning (DL) contouring algorithms for organs-at-risk (OAR) volumes in head and neck cancers. Eleven consecutive head and neck (HandN) patients treated by Tomotherapy were selected for evaluation. Dose prescriptions ranged from 60-70Gy in 30-35 fractions. Each patient had three sets of OAR volumes generated, one clinically used and drawn by humans (physician and dosimetrist) and two auto-generated with DL contouring solutions, trained using convolutional neural network algorithms in large external datasets. The two DL models used for comparison were a HandN model (DLCExpert, Mirada Medical, Oxford, UK) and a Ua-Net model (DeepVoxel Inc, Irvine, CA). Using human-generated volumes as the ground truth, we evaluated the performance of these two models using 3 spatial overlap based metrics (Dice coefficient, Jaccard index(JAC) and True positive rate sensitivity(TPR)), 2 surface distance metrics (95% Hausdorff distance(HD) and average distance(AD)), and 1 volume matrix (volume similarity index(VS)). Seventeen common OAR structures were evaluated including brachial plexus, brainstem, esophagus, eyes, larynx, lenses, mandible, optical nerves and chiasm, oral cavity, parotids, pharyngeal constrictors (PC), submandibular glands (SMGs), spinal cord and trachea. Both DL models offered a feasible solution to delineate structures from CT images. The Mirada model had only 10 common organs for comparison. As shown in Table 1, both models produced comparable results while the DeepVoxel matched human contour better in most OARs. Different image segmentation metrics showed consistent results. DL contours were most similar to human generated contours for brainstem, esophagus, eyes, larynx, lens, mandible, parotids, SMGs, spinal cord and trachea where Dice, JAC, TPR, HD, AD, VS in DeepVoxel model were 0.80(range 0.68-0.91), 0.68(0.53-0.84), 0.78(0.67-0.94), 5.1(2.1-11.4), 1.9mm(1.0-3.9) and 0.88(0.76-0.97) respectively. Brachial plexus, optical nerves and chiasm, oral cavity and PC still needed improvement, partly due to the differences in organ definition. For example, teeth were included in DeepVoxel's oral cavity but not in Mirada and human-generated contours. Those discrepancies will be corrected in our next DL model. DL auto-generated contours from two different models showed high similarity to human generated ones for a variety of OARs in the head and neck, with potential to be adopted in routine clinical practice. In contrast with atlas-based or active shape model approaches, DL models are capable of producing contours with a high level of clinical acceptance and show promise to be indistinguishable from human generated ones. Automatic Segmentation of the Prostate on CT Images Using Deep Neural Networks (DNN).

Abstract:Purpose: Recent advances in deep neural networks (DNNs) have unlocked opportunities for their application for automatic image segmentation. We have evaluated a DNN-based algorithm for automatic segmentation of the prostate gland on a large cohort of patient images. Methods and Materials: Planning-CT data sets for 1114 patients with prostate cancer were retrospectively selected and divided into 2 groups. Group A contained 1104 data sets, with 1 physician-generated prostate gland contour for each data set. Among these image sets, 771 were used for training, 193 for validation, and 140 for testing. Group B contained 10 data sets, each including prostate contours delineated by 5 independent physicians and a consensus contour generated using the STAPLE method in the CERR software package. All images were resampled to a spatial resolution of 1 x 1 x 1.5 mm. A region (128 x 128 x 64 voxels) containing the prostate was selected to train a DNN. The best-performing model on the validation data sets was used to segment the prostate on all testing images. Results were compared between DNN and physician-generated contours using the Dice similarity coefficient, Hausdorff distances, regional contour distances, and center-of-mass distances. Results: The mean Dice similarity coefficients between DNN-based prostate segmentation and physician-generated contours for test data in Group A, Group B, and Group B-consensus were 0.85 +/- 0.06 (range, 0.65-0.93), 0.85 +/- 0.04 (range, 0.80-0.91), and 0.88 +/- 0.03 (range, 0.82-0.92), respectively. The Hausdorff distance was 7.0 +/- 3.5 mm, 7.3 +/- 2.0 mm, and 6.3 +/- 2.0 mm for Group A, Group B, and Group B-consensus, respectively. The mean center-of-mass distances for all 3 data set groups were within 5 mm. Conclusions: A DNN-based algorithm was used to automatically segment the prostate for a large cohort of patients with prostate cancer. DNN-based prostate segmentations were compared to the consensus contour for a smaller group of patients; the agreement between DNN segmentations and consensus contour was similar to the agreement reported in a previous study. Clinical use of DNNs is promising, but further investigation is warranted. (C) 2019 Elsevier Inc. All rights reserved.

Abstract:Abstract Objectives To determine the value of a deep learning masked (DLM) auto-fixed volume of interest (VOI) segmentation method as an alternative to manual segmentation for radiomics-based diagnosis of clinically significant (CS) prostate cancer (PCa) on biparametric magnetic resonance imaging (bpMRI). Materials and methods This study included a retrospective multi-center dataset of 524 PCa lesions (of which 204 are CS PCa) on bpMRI. All lesions were both semi-automatically segmented with a DLM auto-fixed VOI method (averaging < 10 s per lesion) and manually segmented by an expert uroradiologist (averaging 5 min per lesion). The DLM auto-fixed VOI method uses a spherical VOI (with its center at the location of the lowest apparent diffusion coefficient of the prostate lesion as indicated with a single mouse click) from which non-prostate voxels are removed using a deep learning–based prostate segmentation algorithm. Thirteen different DLM auto-fixed VOI diameters (ranging from 6 to 30 mm) were explored. Extracted radiomics data were split into training and test sets (4:1 ratio). Performance was assessed with receiver operating characteristic (ROC) analysis. Results In the test set, the area under the ROC curve (AUCs) of the DLM auto-fixed VOI method with a VOI diameter of 18 mm (0.76 [95% CI: 0.66–0.85]) was significantly higher ( p = 0.0198) than that of the manual segmentation method (0.62 [95% CI: 0.52–0.73]). Conclusions A DLM auto-fixed VOI segmentation can provide a potentially more accurate radiomics diagnosis of CS PCa than expert manual segmentation while also reducing expert time investment by more than 97%. Key Points • Compared to traditional expert-based segmentation, a deep learning mask (DLM) auto-fixed VOI placement is more accurate at detecting CS PCa. • Compared to traditional expert-based segmentation, a DLM auto-fixed VOI placement is faster and can result in a 97% time reduction. • Applying deep learning to an auto-fixed VOI radiomics approach can be valuable.

Abstract:Accurate and robust prostate segmentation in transrectal ultrasound (TRUS) images is of great interest for ultrasound-guided brachytherapy for prostate cancer. However, the current practice of manual segmentation is difficult, time-consuming, and prone to errors. To overcome these challenges, we developed an accurate prostate segmentation framework (A-ProSeg) for TRUS images. The proposed segmentation method includes three innovation steps: (1) acquiring the sequence of vertices by using an improved polygonal segment-based method with a small number of radiologist-defined seed points as prior points; (2) establishing an optimal machine learning-based method by using the improved evolutionary neural network; and (3) obtaining smooth contours of the prostate region of interest using the optimized machine learning-based method. The proposed method was evaluated on 266 patients who underwent prostate cancer brachytherapy. The proposed method achieved a high performance against ground truth with a Dice similarity coefficient (DSC) of 96.2% ± 2.4%, a Jaccard similarity coefficient (Ω) of 94.4% ± 3.3%, and an accuracy (ACC) of 95.7% ± 2.7%, which are all higher than the state-of-the-art methods. Sensitivity evaluation on different noise levels demonstrated that our method achieved high robustness against changes in image quality. Meanwhile, an ablation study was performed, and the significance of all the key components of the proposed method was demonstrated.

Abstract:Introduction: Evaluation of artificial intelligence (AI) algorithms for prostate segmentation is challenging because ground truth is lacking. We aimed to (1) create a reference standard dataset with precise prostate contours by expert consensus and (2) evaluate various AI tools against this standard. Materials and methods: We obtained prostate MRI cases from six institutions from the Quantitative Prostate Imaging Consortium. A panel of four experts (two genitourinary radiologists, two prostate radiation oncologists) meticulously developed consensus prostate segmentations on axial T2-weighted series. We evaluated the performance of six AI tools (three commercially available, three academic) using Dice scores, distance from reference contour, and volume error. Results: The panel achieved consensus prostate segmentation on each slice of all 68 patient cases included in the reference dataset. We present two patient examples to serve as contouring guides. Depending on the AI tool, median Dice scores (across patients) ranged from 0.80 to 0.94 for whole prostate segmentation. For a typical (median) patient, AI tools had a mean error over the prostate surface ranging from 1.3 to 2.4 mm. They maximally deviated 3.0 to 9.4 mm outside the prostate and 3.0 to 8.5 mm inside the prostate for a typical patient. Error in prostate volume measurement for a typical patient ranged from 4.3% to 31.4%. Discussion: We established an expert consensus benchmark for prostate segmentation. The best-performing AI tools have typical accuracy greater than that reported for radiation oncologists using CT scans (most common clinical approach for radiotherapy planning). Physician review remains essential to detect occasional major errors.

Abstract:Abstract Three-dimensional (3D) transrectal ultrasound (TRUS) is utilized in prostate cancer diagnosis and treatment, necessitating time-consuming manual prostate segmentation. We have previously developed an automatic 3D prostate segmentation algorithm involving deep learning prediction on radially sampled 2D images followed by 3D reconstruction, trained on a large, clinically diverse dataset with variable image quality. As large clinical datasets are rare, widespread adoption of automatic segmentation could be facilitated with efficient 2D-based approaches and the development of an image quality grading method. The complete training dataset of 6761 2D images, resliced from 206 3D TRUS volumes acquired using end-fire and side-fire acquisition methods, was split to train two separate networks using either end-fire or side-fire images. Split datasets were reduced to 1000, 500, 250, and 100 2D images. For deep learning prediction, modified U-Net and U-Net++ architectures were implemented and compared using an unseen test dataset of 40 3D TRUS volumes. A 3D TRUS image quality grading scale with three factors (acquisition quality, artifact severity, and boundary visibility) was developed to assess the impact on segmentation performance. For the complete training dataset, U-Net and U-Net++ networks demonstrated equivalent performance, but when trained using split end-fire/side-fire datasets, U-Net++ significantly outperformed the U-Net. Compared to the complete training datasets, U-Net++ trained using reduced-size end-fire and side-fire datasets demonstrated equivalent performance down to 500 training images. For this dataset, image quality had no impact on segmentation performance for end-fire images but did have a significant effect for side-fire images, with boundary visibility having the largest impact. Our algorithm provided fast (<1.5 s) and accurate 3D segmentations across clinically diverse images, demonstrating generalizability and efficiency when employed on smaller datasets, supporting the potential for widespread use, even when data is scarce. The development of an image quality grading scale provides a quantitative tool for assessing segmentation performance.

Abstract:In post-operative radiotherapy for prostate cancer, the cancerous prostate gland has been surgically removed, so the clinical target volume (CTV) to be irradiated encompasses the microscopic spread of tumor cells, which cannot be visualized in typical clinical images such as computed tomography or magnetic resonance imaging. In current clinical practice, physicians segment CTVs manually based on their relationship with nearby organs and other clinical information, per clinical guidelines. Automating post-operative prostate CTV segmentation with traditional image segmentation methods has been a major challenge. Here, we propose a deep learning model to overcome this problem by segmenting nearby organs first, then using their relationship with the CTV to assist CTV segmentation. The model proposed is trained using labels clinically approved and used for patient treatment, which are subject to relatively large inter-physician variations due to the absence of a visual ground truth. The model achieves an average Dice similarity coefficient (DSC) of 0.87 on a holdout dataset of 50 patients, much better than established methods, such as atlas-based methods (DSC<0.7). The uncertainties associated with automatically segmented CTV contours are also estimated to help physicians inspect and revise the contours, especially in areas with large inter-physician variations. We also use a 4-point grading system to show that the clinical quality of the automatically segmented CTV contours is equal to that of approved clinical contours manually drawn by physicians.

Abstract:Accurate segmentation of prostate cancer histopathology images is crucial for diagnosis and treatment planning. This study presents a comparative analysis of three deep learning-based methods, Mamba, SAM, and YOLO, for segmenting prostate cancer histopathology images. We evaluated the performance of these models on two comprehensive datasets, Gleason 2019 and SICAPv2, using Dice score, precision, and recall metrics. Our results show that the High-order Vision Mamba UNet (H-vmunet) model outperforms the other two models, achieving the highest scores across all metrics on both datasets. The H-vmunet model's advanced architecture, which integrates high-order visual state spaces and 2D-selective-scan operations, enables efficient and sensitive lesion detection across different scales. Our study demonstrates the potential of the H-vmunet model for clinical applications and highlights the importance of robust validation and comparison of deep learning-based methods for medical image analysis. The findings of this study contribute to the development of accurate and reliable computer-aided diagnosis systems for prostate cancer. Abstract:Background and purpose: Radiation-induced erectile dysfunction (RiED) is commonly seen in prostate cancer patients. Clinical trials have been developed in multiple institutions to investigate whether dose-sparing to the internal-pudendal-arteries (IPA) will improve retention of sexual potency. The IPA is usually not considered a conventional organ-at-risk (OAR) due to segmentation difficulty. In this work, we propose a deep learning (DL)-based auto-segmentation model for the IPA that utilizes CT and MRI or CT alone as the input image modality to accommodate variation in clinical practice. Materials and methods: 86 patients with CT and MRI images and noisy IPA labels were recruited in this study. We split the data into 42/14/30 for model training, testing, and a clinical observer study, respectively. There were three major innovations in this model: 1) we designed an architecture with squeeze-and-excite blocks and modality attention for effective feature extraction and production of accurate segmentation, 2) a novel loss function was used for training the model effectively with noisy labels, and 3) modality dropout strategy was used for making the model capable of segmentation in the absence of MRI. Results: The DSC, ASD, and HD95 values for the test dataset were 62.2%, 2.54mm, and 7mm, respectively. AI segmented contours were dosimetrically equivalent to the expert physician's contours. The observer study showed that expert physicians' scored AI contours (mean=3.7) higher than inexperienced physicians' contours (mean=3.1). When inexperienced physicians started with AI contours, the score improved to 3.7. Conclusion: The proposed model achieved good quality IPA contours to improve uniformity of segmentation and to facilitate introduction of standardized IPA segmentation into clinical trials and practice.
