Abstract:Hip joint is a ball and socket type of joint and its anatomic features makes it very stable. However, Incidence of hip dislocation has increased recently due to high energy trauma like road traffic accident, fall from height etc. It is usually associated with fracture of femoral head, neck, shaft and acetabular wall. Association of ipsilateral intertrochantric fracture is exceedingly rare and only a few case reports are available in the literature. We are presenting a rare case report of neglected anterior hip dislocation with ipsilateral comminuted intertrochanteric fracture managed with hemiarthroplasty using long cemented distal press-fit revision stem. A 66-year-old male presented to our outpatient department with history of fall from a tree 22 days back with pain in right hip and inability to bear weight on right lower limb. Radiographs were suggestive of anterior hip dislocation with fracture of ipsilateral comminuted intertrochanteric fracture of right side. Anterior hip dislocation with ipsilateral intertrochanteric fracture is a very rare entity, and only a few case reports are available in the literature. Management includes early open reduction and fixation to achieve optimal outcomes, but doing primary arthroplasty is also a viable option especially in neglected cases as there is high chances of avascular necrosis and secondary osteoarthritis of the hip joint later on.
A rare concomitant ipsilateral hip and knee dislocations: A case report and review of the literature

Abstract:Ipsilateral hip and knee dislocation is a rare and complex injury that usually results from high-energy trauma. Only 14 cases were reported in the literature so far. We report the case of a 35-year-old man who presented with an ipsilateral right hip and knee dislocation after being involved in a road traffic accident. A contralateral floating knee was associated. The hip dislocation was associated to a posterior wall fracture of the acetabulum and a femoral head fracture. The knee dislocation was compound and associated with a patella fracture. The hip dislocation was reduced. The posterior wall and the femoral head fractures were treated conservatively. The knee dislocation was reduced and stabilized by external fixator. Early postoperative infection occurred and was treated surgically. At the last follow-up, the knee was ankylosed at 5° of flexion with a bony bridge between the femoral condyle and the tibial plateau. The patient described an occasional hip pain on exertion. He had full range of motion of the right hip. No avascular necrosis of the femoral head was seen. Outcomes of simultaneous hip and knee dislocation are very variable and remain unpredictable. Abstract:Case: This clinical case report presents a 40-year-old male who sustained an ipsilateral hip and knee dislocation with ipsilateral femoral head fracture and incomplete femoral neck fracture following a motorcycle collision. Conclusion: This report describes acute and later definitive orthopedic care and management, with focus on urgent interventions and timing of immediate treatments. Given the presented patient's favorable clinical outcomes, return to baseline activities, and absence of significant sequelae following injury, the considerations from the acute management and surgical planning of this patient's injuries can be used as a reference for treating the rare injury of ipsilateral knee and hip dislocations.

Abstract:Introduction: High-energy trauma has increased the incidence of traumatic hip dislocation and dislocation is often associated with fractures. However, a hip fracture-dislocation extending to the ipsilateral femoral trochanter is exceedingly rare and only a few cases have been described. We report a case of irreducible posterior dislocation of the hip with ipsilateral intertrochanteric fracture managed by open reduction and osteosynthesis. Case Report: A 50-year-old male presented with a history of fall from an electricity pole with pain in the left hip and an inability to bear weight on his left lower limb. Radiographs confirmed the presence of posterior dislocation of the left hip with comminuted intertrochanteric fracture on the same side. Conclusion: Posterior dislocation of the hip with same side inter trochanteric fracture is a rare traumatic entity, with only few case being reported. Literature does not provide any classification for such rare traumatic fracture dislocation. Management of such fracture requires early diagnosis and intervention which can result in good prognosis. Bilateral anterior shoulder dislocation: A case report

Abstract:Bilateral anterior shoulder dislocation (BASD) is a rare condition typically associated with a history of trauma. This case report presents a patient with BASD resulting from trauma and discusses treatment options. A 51-year-old male patient was admitted to the emergency department following a fall from a height. Radiographic examination revealed BASD and a comminuted intra-articular fracture of the distal end of the left radius. The patient did not have any other additional pathologies. After a preoperative evaluation, the patient was urgently taken to the operating room, where both shoulders underwent reduction using the Hippocratic maneuver under sedation anesthesia. Immediately after this procedure, the left wrist was reduced closed, and a penning-type external fixator was inserted under fluoroscopic guidance. Following the reductions, both shoulders were wrapped with a Velpeau bandage and immobilized. An elastic bandage was wrapped around the wrist. By the end of the third week, shoulder mobility exercises were initiated, with a gradual increase in the intensity of the exercises. The wrist fixator was removed in the fifth week after radiographic evaluations, followed by the initiation of passive and then active exercises. Given the rarity of BASD, it is crucial to obtain a detailed clinical history, conduct a comprehensive clinical examination, and perform detailed imaging studies-radiography, computed tomography, and magnetic resonance imaging to avoid overlooking such pathologies in emergency situations. Bilateral anterior shoulder dislocation is a pathology that results from major trauma. It is important to remember that this particular type of pathology may be accompanied by various other pathologies, such as fractures (of the tuberculum majus), rotator cuff injuries, and neurovascular injuries.

Abstract:During our research programme on implantation failures after in-vitro fertilization (IVF), we investigated the expression of leukaemia inhibitory factor (LIF) by endometrial cells in women who have experienced several implantation failures after embryo transfer. Their embryos appeared to be morphologically normal and were either fresh or cryopreserved. Our intention was to investigate the role of LIF in regulating human preimplantation embryo development. In animals, the addition of 1000 IU/ml of recombinant human LIF (rhLIF) to culture medium, in comparison with control medium, significantly decreased the degeneration of ovine embryos (9 versus 27%), enhanced embryo hatching (64 versus 16%), and improved rates of implantation (50 versus 16%) (Fry et at., 1992). In the same way, co-culture cells that express LIF (including Vero cells and human embryonic fibroblasts) seemed to be superior to cells that did not express LIF (including human placental villous core mesenchymal cells) for the support of the early preimplantation development of mouse embryos (83-86% blastocysts versus 71 %) (Kauma and Matt, 1995). In our study, we first assessed LIF expression in endometrial cells by taking three uterine biopsies in one natural cycle in a group of women. Biopsies were taken in the luteal phase, i.e. early (2-4 days after ovulation), mid (6-7 days) and late (10-l2 days). LIF expression was checked by immunocytochemistry using double labelling with both anti-cytokeratin antibodies (fluorescein) and anti-LIF antibodies (rhodamine). Three observations were made: (i) endometrial cells (epithelial and stromal) contained LIF immunoreactivity in the luteal phase (3-11 days after ovulation); (ii) labelling was more intense for epithelial than stromal cells, indicating that differences existed in their LIF secretion; (iii) labelling was stable during the luteal phase in epithelial cells whereas it increased in stromal cells during the luteal phase. In a second experiment, embryo co-culture was performed on endometrial cell monolayers. Abstract:Introduction: Central hip dislocations are rare orthopedic injuries, and their concomitant occurrence with ipsilateral knee dislocations is an even rarer phenomenon. We present a unique case of central hip dislocation along with ipsilateral knee dislocation and additional fractures involving the lateral condyle of the left tibia and patella. This complex injury pattern resulted from a severe road traffic accident, necessitating operative management to address the multiple musculoskeletal injuries. Ultimately, femoral head avascular necrosis (AVN) developed, leading to the need for total hip replacement (THR). Case report: A 28-year-old male was involved in a high-impact road traffic accident, leading to central hip dislocation, ipsilateral knee dislocation, and fractures of the lateral condyle of the left tibia and patella. The patient was promptly assessed, and operative intervention was initiated. Closed reduction and internal fixation with K-wires were performed for the fractures of the lateral condyle of the left tibia and patella. For the central hip dislocation, open reduction was achieved using a 9-hole RECON plate. Following the procedures, a long leg slab was applied for 6 weeks to facilitate optimal healing.Postoperatively, at the 6-week mark, K-wires were removed, and the patient commenced physiotherapy with partial weight-bearing permitted. However, during the rehabilitation phase, the patient reported instability in his left knee. Magnetic resonance imaging revealed an avulsion fracture of the posterior cruciate ligament (PCL) from its tibial attachment site and a complete anterior cruciate ligament (ACL) tear. It was observed that the femoral head had developed AVN. This complication necessitated further intervention, leading to the performance of a THR. Discussion: This case underscores the challenges and complexities associated with managing central hip dislocation and ipsilateral knee dislocation with multiple fractures. The post-operative instability of the knee, diagnosed as an avulsion fracture of the PCL and complete ACL tear, necessitated additional intervention. The patient subsequently underwent PCL reconstruction using a semitendinosus graft, highlighting the importance of a comprehensive approach to address the diverse musculoskeletal injuries resulting from high-impact trauma. Conclusion: The successful management of central hip dislocation and ipsilateral knee dislocation with associated fractures requires a multidisciplinary approach, incorporating timely surgical intervention, rigorous rehabilitation, and vigilant post-operative monitoring to address potential complications. This case emphasizes the need for ongoing clinical assessment and imaging studies to identify and manage secondary injuries that may manifest during the recovery phase. In addition, it highlights the development of femoral head AVN, ultimately leading to the necessity for THR.

Abstract:cDNA clones coding for a catalytic subunit of acetylcholinesterase were isolated from cDNA libraries constructed from Torpedo marmorata electric organ. The nucleotide sequence of the cloned cDNAs codes for a 599‐amino acid precursor containing a 24‐amino acid signal peptide. This primary structure has been compared with the sequences of Torpedo californica and Drosophila melanogasta acetylcholinesterases, and with that of human butyrylcholinesterase. Genomic blot experiments carried out with cDNA restriction fragments used as hybridization probes are in agreement with the existence of a single gene coding for the different catalytic subunits of Torpedo acetylcholinesterase. Unexpectedly, we observed multiple 5′‐untranslated regions, which may contain several initiation codons.

Abstract:<span><br/>Sacroiliac joint dislocation is an uncommon and potentially life-threatening injury secondary to high-energy trauma. Given that bilateral sacroiliac joint dislocation has rarely been reported, its management constitutes a clinical challenge. The aim of this study was to review the literature available about sacroiliac joint dislocation management and to show an effective and reliable method to treat this kind of injury. Case report. We present a 15-year-old female patient who suffered a complete bilateral sacroiliac joint dislocation. The definitive treatment consisted of bilateral L4-iliac assembly plus right sacroiliac screw assisted by an O-arm navigation system. Surgical instrumentation allowed mobilization and full-weight-bearing a few days after surgery. At the 2-year follow-up, the patient remained asymptomatic. The triangle fixation technique stabilizes the sacroiliac joint and binds the lumbar spine to the pelvis. This method could represent a good option that allows early mobilization and weight-bearing.<br/></span>

Abstract:HIP International, Ahead of Print. Introduction:Posterior native hip dislocations (NHD) are high-energy injuries. Thompson-Epstein Type I dislocations describe those without significant associated femoral or acetabular fracture. The aim of this study was to compare the clinical and radiological outcomes of patients with Type I NHDs. We also evaluate the association between radiological indicators of femoroacetabular impingement (FAI) and NHD.Patients and methods:A retrospective study from January 2012 to May 2021 compared skeletally mature patients (⩾16 years) with Type I posterior NHD to age and gender-matched controls with Type II–V posterior NHD. Patient demographics, mechanism of injury, complications and patient-reported outcome measures (PROMs) are presented. Post reduction radiographs and computed tomography were used to assess for FAI. Univariate analyses were performed to evaluate radiological outcomes.Results:13 patients (77% male) with Type I posterior NHD were compared to a control group of 40 patients (80% male) with Type II–V posterior NHD. 11 patients in the study group and 14 in the control group experienced isolated injuries (p = 0.01). Post-reduction complications were similar. The study group had significantly lower post-injury osteoarthritis incidence (n = 0) compared to controls (n = 18, p = 0.0083). Patients reported a mean Oxford Hip Score of 43.5 ± 2.2 and EQ-5D-VAS score of 87.1 ± 7.4, with 6 patients indicating minimal symptoms across all EQ-5D-5L domains. Radiological femoroacetabular impingement (FAI) was prevalent in both groups, especially among males.Conclusions:Patients who underwent emergent closed reduction of Type I NHD demonstrated good short to medium term outcomes. Our radiological findings suggest a high prevalence of FAI. Future work should aim to quantify longer term outcomes following this injury. We call for further comparative studies of patients who suffer NHD with and without fractures to aid our understanding of risk factors. Given the rarity of this injury, multicentre efforts will be required to capture large numbers of patients.

Abstract:Open dislocation of the ankle without associated fracture is extremely rare. Epstein's superior type of hip dislocation is the rarest type of hip dislocation. We reported a case of 50 years Nepalese gentleman who presented with a combination of these two rare orthopedic emergencies with the newly reported mechanism of injury by a garden tractor. Early extensive debridement of the wound with broad spectrum antibiotics coverage, reduction with delta frame external fixator, and postoperative rehabilitation helps in saving limb along with its function.
Clinical Outcome of Floating Hip with Irreducible Hip Dislocation and Distal Third Femur Fracture with Intra-Articular Extension: A Rare Case Report

Abstract:Introduction: Floating hip with hip dislocation is a very high-energy, devastating, and rare injury whose treatment is very challenging, and the outcome is usually poor. Case report: A 35-year-old man presented posterior wall fracture acetabulum and dislocation of the hip with ipsilateral distal third shaft femur fracture with intra-articular extension fracture and un-displaced patella fracture. We achieved a reduction of hip dislocation by a knee-spanning external fixator followed by open reduction and internal fixation with anatomical locking plate for distal third femur fracture with intra-articular extension followed by open reduction and internal fixation for posterior wall of acetabulum with recon plate in Kocher-Langenbeck approach in stages. The patient was able to partial weight bear after 12 weeks of the injury and mobilized independently without any support after 5 months. Conclusion: Floating hip with hip dislocation is difficult to manage but reducing the hip dislocation with knee spanning external fixator and management in stages will reduce the complications and better outcome.
Dual mobility hip arthroplasty: iatrogenic intraprosthetic dislocation of a 22 mm head

Abstract:We describe the first intraprosthetic dislocations of a 22 mm head in a dual mobility hip replacement in the literature. This case emphasises the importance of information gathering and planning when dealing with arthroplasty complications. Furthermore, it highlights the necessity of adequate muscle relaxation and analgesia when dealing with hip dislocations. A 78-year-old patient presented to the emergency department (ED) with a spontaneous dislocation of her left dual-mobility total hip replacement 3 months after first-stage revision surgery for infection. Reduction via manipulation under analgesia was then attempted in the ED but was ultimately unsuccessful; an iatrogenic intraprosthetic dislocation of the polyethylene liner was sustained. Several factors may have contributed to liner dislocation: a failure to appreciate the implant type, multiple manipulations in ED were attempted under analgesia, but no sedation or muscle relaxation was administered to the patient. The patient subsequently underwent revision surgery.

Abstract:Abstract Background Bilateral acetabular fracture is a very rare presentation among the trauma patients, as the pattern and the degree of the forces required to fracture both acetabula is very unique. The primary purpose of this study is to report a series of adult patients presenting with post-traumatic bilateral acetabular fracture without any history of pathological or metabolic bone disease. Patients and methods In this retrospective study, 18 cases of traumatic bilateral acetabular fracture were included. There was predominance of both column (four patients on left and six on right) followed by anterior column (two patients left and four on right) and posterior wall (three patients left and right). They were treated surgically through open reduction and internal fixation. All cases were followed up for at least 13 months. Matta’s criteria were used for radiological evaluation on plain radiographs. Functional outcome was evaluated using the Merle d’Aubigne and postel score at final follow-up. Results No patients were lost during the follow-up period; there was one case of surgical site infection. There were three cases of postoperative osteoarthritis, one case of heterotrophic ossification, one case of persistent sciatic nerve palsy and one case of lateral femoral cutaneous nerve palsy. The radiological evaluation according to Matta’s criteria revealed anatomic reduction in 12 patients, imperfect reduction in three patients while other three patients had poor reduction. According to modified Merle d’Aubigne and Postel score, 10 cases were rated as excellent, five cases as good and three cases presented fair (one case) to poor (two cases) results. Conclusion We report an unusual case series of bilateral acetabular fracture successfully managed surgically with good clinical outcome. With the increasing incidence of route traffic accidents, such cases would probably be recurrent in the upcoming years.

Abstract:Introduction: Atraumatic hip dislocation after short femoral nail (SFN) fixation for an intertrochanteric fracture is extremely rare. Case Report: An 84-year-old woman presented with an atraumatic posterior hip dislocation that occurred 8 years after SFN fixation for an intertrochanteric hip fracture. She experienced an acute-onset left hip pain when standing up from a sitting position while bathing at a day care facility. We performed total hip arthroplasty (THA) with a dual mobility system. During the post-operative THA evaluation, the combined anteversion angle was within the optimum range of 57. However, the dislocation recurred after the THA. During the revision THA, a dual mobility system was used to moderately extend the stem neck. Measures were adopted to strain the posterior soft tissues of the hip joint. We speculated that the posterior hip joint capsule ruptured because the support of the posterior hip joint was weak after the intertrochanteric hip fracture. At the final follow-up visit at 6 months after the operation, the hip joint pain had disappeared, and her activities of daily living recovered to almost the same level as her preinjury activities. Conclusions: We should consider the lack of support of posterior soft tissues as a cause of this atraumatic posterior hip dislocation following intertrochanteric fracture fixation. Abstract:Objective It is challenging to resect the pelvic malignancy involving the acetabulum and reconstruct it with hemipelvic endoprosthesis. The postoperative hip dislocation rate is relatively high. The aim of this study is to outline key technical considerations in dealing with early-stage hip dislocation after pelvic tumor resection and hemipelvic endoprosthesis reconstruction. Methods 169 cases who had pelvic tumor resection and hemipelvic endoprosthesis reconstruction between July 2003 and December 2011 were reviewed. After tumor resection, artificial total hip with anti-dislocation mechanism was used, and periacetabular muscular tension was carefully rebuilt. Postoperative rehabilitation was supervised by a specialized nursing team. Early-stage dislocation was defined as the dislocation occurred within 6 months postoperatively. Open reduction was considered only when closed reduction under subarachnoid anesthesia failed. A normal acetabulum abduction angle was defined between 30 and 55 degrees. The chi-square test was used to detect group differences ( dislocation group and non-group dislocation ) of acetabulum abduction angle. Results 14 patients had early-stage dislocation. The midterm time between dislocation and operation was 16 days ( range;0-94 days ). Successful close reduction was accomplished in 5 patients and the other 9 patients had to undergo open reduction. No hemipelvic prosthesis loosening or sciatic nerve injury was observed during reduction. Second hip dislocation happened in 2 cases after close reduction. No case had an acetabulum abduction angel less than 30 degree. 5 of the dislocated hip had an acetabulum abduction angle more than 55 degrees, while 50 of the non-dislocation group had that an acetabulum abduction angle ( Fisher’s Exact Test, P=0.773 ). Conclusions Despite all the techniques we applied, hip dislocation rate is high after periacetabular tumor resection and hemipelvic prosthesis reconstruction. This is partly due to the inadequate muscular tension around the hip joint and improper postural changes. In most cases, open reduction is necessary.

Abstract:The hemodynamic alterations occurring in portal hypertension are com plex and difficult to evaluate. Investigation and understanding of the vas cular pathophysiology assume clinical importance because operative treat ment very often alters splanchnic blood flow suddenly and dramatically. I t is generally conceded that hepatic function is a major determinant in the degree of well being of patients with portal hypertension, especially after complications of portal hypertension are treated by portosystemic shunt. The concept that hepatic function is not the sole determining factor is em phasized by a number of clinical studies. Callow et al. (I), Conn & Linden muth (2) and Jackson et al. (3) have analyzed the results of shunt therapy in controlled, randomized studies. Survival rates in the surgical groups are not superior to those in the groups treated medically. In a review of the subject, Grace et al. (4) came to the same conclusion. Although survival rates were similar, modes of death differed. In the medically treated control group, hemorrhage from esophageal varices was the major cause while in patients undergoing portacaval shunt, liver failure was the most common cause of death. Johnson et al. (S) reported the results of nonshunting procedures in the control of esophageal varices. None of the long-term survivors were troubled with hepatic encephalopathy or liver failure. In bilharzial cirrhosis hepato cellular function usually remains good but Hassab (6) found that 20.6 per cent of such patients undergoing portacaval shunt developed encephalopathy. After nonshunting operations on 3SS patients with the same disease only one patient developed encephalopathy in the absence of bleeding. Mikkelsen et al. (7) reported on 36 patients, in 17 of whom the portal vein was patent, liver function was excellent and there was no evidence of liver disease on gross examination. Following portacaval shunts, 10 developed signs of encephalopathy and five died of liver failure. In those patients with spon taneously occluded portal veins these complications were infrequent. The foregoing clinical observations suggest that the sudden complete diversion of portal blood flow, as occurs following an end-to-side or side-to side (8, 9) portacaval shunt, may be detrimental to the liver under certain circumstances.
