Abstract:Background: Chronic pelvic pain (CPP) is a highly prevalent pain condition in which pelvic floor myofascial pain syndrome (MPPS) is also frequently found. Optimal treatments for CPP and MPPS are unknown. The aims of this pilot study were to investigate the effect of pelvic floor magnetic stimulation (MS) in women with MPPS. Treatment effects were compared between patients receiving MS alone, myofascial release therapy (MRT) alone, and MS + MRT. Methods: Patients were divided into three groups: MS, MRT, and MS + MRT. Questionnaires including Short-form McGill Pain Questionnaire (SF-MPQ), Pelvic Pain and Urgency/Frequency questionnaire (PUF), Female Sexual Function Index (FSFI), Hamilton Anxiety Scale (HAMA), and clinical global impression scale (CGI) were used to assess changes in subjective symptoms before and after treatment. Pelvic floor muscle function was assessed by the Modified Oxford Scale and Surface electromyography (sEMG). Pain mapping was used to locate trigger points (TPs) and to score the intensity of pain. A Visual Analog Scale (VAS) was used to measure the intensity of pain on a scale of 0 to 10. Changes in the above evaluation indexes within each group and between groups were evaluated after 5 treatment sessions and 10 treatment sessions. Results: Nineteen patients completed the treatment between November 2020 and August 2021. The SF-MPQ and PUF scores decreased significantly (p < 0.01) after treatment. The VAS score for pelvic floor tenderness also decreased significantly after 5 and 10 treatment sessions (p < 0.01). At the end of 10 sessions, the HAMA score was significantly lower than prior to treatment (p < 0.01). Conclusions: This preliminary study shows that MS is effective for the treatment of MPPS. Musculoskeletal evaluation of the lower pelvic complex in women with endometriosis: A case-control study

Abstract:Objective Make a comparison about the presence of disorders of the pelvic loin complex between healthy women (without endometriosis) and women with endometriosis. Study Design Chronic pelvic pain (CPP) associated with endometriosis has diverse origins, including musculoskeletal factors. Musculoskeletal dysfunction in the pelvic region is theorized to result from sustained muscular contraction, triggered by altered visceral stimuli and adoption of antalgic postures, causing secondary damage to muscles, ligaments, and joints. CPP significantly impacts quality of life, relationships, sexuality, and mental health. However, limited data exists on musculoskeletal impacts of endometriosis and CPP. It was made a case-control study at Maternidade Escola Assis Chateaubriand from August 2017 to January 2021. Evaluated 71 women: 41 in endometriosis group (EG) and 30 in control group (CG). Data collection included sociodemographic questionnaires, musculoskeletal physiotherapeutic evaluations, pain mapping, pressure pain thresholds, kinesiophobia, and disability measurements. Statistical analysis was performed using Spearman's Rho test to determine correlations. Results Mean age of participants was 31 years. EG exhibited lower pain threshold variations in lumbopelvic trigger points than CG (P < .05). Significant muscle flexibility differences between groups were observed; EG had reduced flexibility (P < .05). Most common pain areas were hypogastrium in EG (48.78 %) and left lumbar in CG (30 %). EG had higher kinesiophobia values (P = .009). Weak kinesiophobia-pressure threshold association observed in CG's lumbar pelvic region. Conclusion Women with Endometriosis and CPP exhibit higher musculoskeletal disorder prevalence, lower pain thresholds, decreased lumbopelvic muscle range of motion, higher kinesiophobia scores, and increased disability indices with low back pain compared to healthy women.

Abstract:Background and Objectives: Endometriosis is a chronic and recurrent disease defined as the presence and proliferation of endometrial glands and stroma outside the uterine cavity. It affects up to 6-10% of women of reproductive age and can be classified into superficial, ovarian, and deep infiltrating endometriosis (DIE). Deep infiltrating endometriosis can be associated with pain symptoms and pelvic floor muscle hypertone. Moreover, it may be responsible of bowel, urinary, and sexual dysfunctions with impairment of women's quality of life. Few studies have investigated the role of physiotherapy in women with DIE. Here, we aimed first to evaluate the effects of pelvic floor physiotherapy (PFP) on urinary, bowel, and sexual functions. Secondly, we aimed to evaluate the effects of ultrasound visual feedback during PFP on pelvic floor and subjective modifications in the frequency of sexual intercourse. Materials and Methods: This randomized controlled trial was conducted between June 2018 and December 2019 at our tertiary center. Nulliparous women with DIE and superficial dyspareunia were enrolled. At first examination, levator hiatal area (LHA) assessed with 3D/4D transperineal ultrasound, pain symptoms, urinary, bowel, and sexual functions were evaluated. Then, women were randomly assigned to no intervention (control group) or treatment with five individual sessions of PFP (experimental group), and after four months women underwent a second examination. Urinary, bowel, and sexual functions were assessed with validated questionnaires at first and second examinations. In particular, the Bristol Female Lower Urinary Tract Symptoms questionnaire was used to evaluate urinary symptoms, the Knowles-Eccersley-Scott-Symptom questionnaire to assess the presence of constipation, and the Female Sexual Function Index to investigate sexual function. Study outcomes were the comparisons among groups in terms of differences in actual changes in median of questionnaire scores between first and second examinations. Results: Thirty women (17 in the experimental group and 13 in the control group) completed the study. No significant differences were found between the two groups regarding urinary, bowel, and sexual functions, although women in the experimental group showed a tendency towards an improvement in constipation symptoms. Conclusion: In women with DIE, PFP does not appear to affect urinary, bowel, and sexual functions. Therefore, despite the improvement in superficial dyspareunia, chronic pelvic pain, and PFM relaxation with high treatment satisfaction, women should be informed about the unclear impact of PFP on urinary, bowel, and sexual functions. Larger studies are necessary to further investigate the impact of PFP on these functions.

Abstract:Introduction Sexual dysfunction is an alteration in one or more components of the sexual response cycle and dyspareunia is the dysfunction in which the woman experiences pain before, during or after vaginal penetration, which impairs quality of life and relationship with the couple. Physiotherapy uses several techniques to address this dysfunction, including pelvic floor muscle training (PFMT) and perineal massage (PM), but studies on the actual effects of these techniques and the resolution of dyspareunia are still scarce. Therefore, the objective of this study is to identify the evidence on the use of PFMT and MP techniques in the treatment of women with dyspareunia. Materials/Methods Research carried out in the PEDro, Pubmed and MEDLINE databases. Articles of the type were included: clinical trials, meta-analysis and systematic reviews, published in English and Portuguese; in the period from 2013 to 2023; who addressed TMAP and MP techniques in the treatment of dyspareunia. Those who did not meet the inclusion criteria were excluded. Results In total, 525 articles were identified, but only 9 studies were selected for this review as they met the inclusion criteria. Most of the results showed significant responses to treatment with the use of PFMT and MP, and even the studies in which they did not obtain significance did not rule out the possibility of these techniques, since they do not present side effects and have good potential. to improve. Many studies have low methodological quality. Conclusion It was observed in the literature that PFMT and MP have good responses in the treatment of dyspareunia, but there is not enough support in the literature to demonstrate their effectiveness. It is necessary to develop future studies with better methodological quality for more solid scientific evidence. Financing No conflict.

Abstract:Introduction The pelvic floor muscles (PFMs) have been suggested to play a key role in sexual function and response in women. However, syntheses of the evidence thus far have been limited to interventional studies in women with pelvic pain or pelvic floor disorders, and these studies have failed to fully capture the involvement of the PFMs in a broader population. Aim We sought to appraise the evidence regarding the role of the PFMs in sexual function/response in women without pelvic pain or pelvic floor disorders. More specifically, we examined the following: (1) effects of treatment modalities targeting the PFMs on sexual function/response, (2) associations between PFM function and sexual function/response, and (3) differences in PFM function between women with and those without sexual dysfunction. Methods We searched for all available studies in eight electronic databases. We included interventional studies evaluating the effects of PFM modalities on sexual outcomes, as well as observational studies investigating the association between PFM function and sexual outcomes or the differences in PFM function in women with and those without sexual dysfunction. The quality of each study was assessed using the Mixed Methods Appraisal Tool. Estimates were pooled using random-effects meta-analyses whenever possible, or a narrative synthesis of the results was provided. Main outcomes The main outcomes were sexual function (based on a questionnaire)/sexual response (based on physiological test), and PFM function (assessment of the PFM parameters such as strength and tone based on various methods). Results A total of 33 studies were selected, including 14 interventional and 19 observational studies, most of which (31/33) were deemed of moderate or high quality. Ten out of 14 interventional studies in women with and without sexual dysfunctions showed that PFM modalities had a significant effect on sexual function. Regarding the observational studies, a meta-analysis revealed a significant moderate association between PFM strength and sexual function ( r = 0.41; 95% CI, 0.08-066). Of the 7 observational studies performed to assess sexual response, all showed that the PFMs were involved in arousal or orgasm. Conflicting results were found in the 3 studies that evaluated differences in PFM function in women with and those without sexual dysfunction. Clinical implications Our results highlight the contribution of the PFMs in sexual function/response. Strengths and limitations One strength of this review is the inclusion of a broad range of study designs and outcomes, allowing a thorough synthesis of evidence. However, interpretations of these data should consider risk of bias in the studies, small sample sizes, and the absence of control/comparison groups. Conclusion The findings of this review support the involvement of the PFMs in sexual function/response in women without pelvic pain or pelvic dysfunction. Well-designed studies should be performed to further investigate PFM modalities as part of the management of sexual dysfunction.

Abstract:To investigate the differences in pelvic floor muscle (PFM) electromyography (EMG) parameters between women with or without sexual dysfunction (FSD) and their correlations. Women who voluntarily participated in a questionnaire-based survey on sexual function and underwent PFM EMG in Weifang People's Hospital during the period from March 2021 to December 2021 were retrospectively enrolled. The female sexual (dys)function was measured using the Female Sexual Function Index. Glazer PFM EMG was performed using a Melander instrument (MLD A2 Deluxe). The differences in PFM EMG parameters between women with or without FSD were compared, and the relationships between PFM EMG parameters and FSD were analysed using multiple linear regression models. A total of 305 women were enrolled, with 163 in the FSD group and 142 in the non-FSD group. Comparisons of PFM EMG parameters between these two groups revealed that the FSD group had significantly higher peak EMG amplitude during the phasic (flick) contractions and shorter recovery latency during the tonic contractions than the non-FSD group (both P < 0.05). Multivariate linear regression suggested that the peak EMG amplitude during the phasic (flick) contractions was 5.39 μV higher in the FSD group than in the non-FSD group, whereas the recovery latency during the tonic contractions was 0.29 s shorter (both P < 0.05). The results of the pelvic floor EMG in this study suggest that the pelvic floor muscles of women with FSD may be more susceptible to fatigue, and may have poorer coordination of their pelvic floor muscles.

Muscle mechanical properties of pelvic floor and paravertebral muscles in women with and without urge urinary incontinence: a case-control study

Abstract:Background: Muscle mechanical properties (MMPs) are relevant in the pathophysiology of lumbopelvic disorders. However, they have not been described in the pelvic floor muscles (PFM) and lumbar paravertebral muscles (LPM) of women with urge urinary incontinence (UUI). Objective: To identify differences between MMPs of PFM and LPM in patients with UUI and healthy controls. Secondarily also aimed to observe the relationship between sociodemographic and clinical variables with the PFM and LPM MMPs. Methods: The participants of this case-control study comprised 34 women with UUI (UUI group) and 34 continent women (control group). Sociodemographic variables were obtained together with data on the clinical status of the pelvic floor. The MMPs, i.e., frequency (tone), stiffness, decrement (inverse of elasticity), and viscoelastic properties (VP), such as relaxation time and creep, of PFM and LPM were assessed with a hand-held tonometer. Between-group differences and intra-group correlations were identified. Results: The UUI group presented higher frequency and stiffness, as well as lower relaxation time in PFM, whereas the LPM had lower tone and stiffness, and higher VP, compared to the control group (p < 0.05). The UUI group showed a pattern of moderate correlations (|0.403|<r<|0.600|) among all MMPs of PFM and tone, stiffness, and VP of LPM, which did not appear in the control group. Conclusion: The presence of UUI may influence MMPs at PFM and LPM levels, increasing the tone and stiffness of PFM, whereas these properties are reduced in LPM. These findings emphasize the clinical interest of the lumbopelvic determination of MMPs, obtained through externally applied hand-held instruments, in the pathophysiology of UUI.

Abstract:Background Endometriosis is a condition of the female reproductive system associated with pelvic pain. Chronic pain can affect physical performance by limiting the functional activities, thus, it is hypothesized that women with endometriosis may also present decreased functional capacity, decreased strength, and mobility. The objective of this study is to compare physical performance in women with and without endometriosis. Methods This is a cross-sectional study composed of 115 women equally divided into two groups: the endometriosis group (EG), composed of women with a confirmed diagnosis of the disease by magnetic resonance imaging, and the comparator group (CG), consisting of women without suspicion of the disease. Physical performance (dependent variable) was assessed using hand dynamometry, the 6-min walk test (6MWT), gait speed, and the chair stands test. CG participants performed the tests during the luteal phase of the menstrual cycle. Descriptive statistics, unpaired t-tests, and chi-square tests were used to describe and compare the groups. Multiple linear regression tested the associations adjusted for covariates (age, income, education, age at menarche, and body mass index). Results The EG had worse gait speed (mean difference: −0.11; 95% CI: [−0.18 to −0.04]), weaker grip strength (mean difference: −3.32; 95% CI: [−5.30 to −1.33]), shorter distance covered in the 6MWT (mean difference: −83.46; 95% CI: [−121.38 to −45.53]), and a lower number of repetitions in the chair stands test (mean difference: −8.44; 95% CI: [−10.64 to −6.25]) than the CG, even after adjusting for covariates. Conclusion Grip strength, lower limb strength, mobility, and functional capacity were worse in women diagnosed with endometriosis. Women with endometriosis should be encouraged to engage in physical exercise, adopt healthy lifestyle habits, and participate in rehabilitation activities to control pain, with the aim of reducing functional impairments.

Abstract:Objectives: This study aims to evaluate the presence of myofascial trigger points (TrPs), widespread pressure pain sensitivity, and mechanosensitivity of neural tissue in women with chronic pelvic pain.Design: Case-control study.Setting: Faculty of Health Sciences.Subjects: Forty women with chronic pelvic pain between age 18 and 60 years and 40 matched healthy controls were included in the study.Methods: TrPs were bilaterally explored in gluteus maximus, gluteus medius, gluteus minimus, quadratus lumborum, and adductor magnus muscles. The referred pain reproduced lumbopelvic symptoms. Pressure pain thresholds (PPTs) were also bilaterally assessed over the Pfannenstiel incision point on the abdominal, C5-C6 zygapophyseal joint, second metacarpal, and tibialis anterior muscle. Mechanosensitivity of neural tissue was assessed with the neurodynamics tests of slump and the straight-leg raising.Results: Significant between-group differences were found in TrP presence in patients with chronic pelvic pain (P < 0.001) compared with those included in the control group. Widespread pressure pain hyperalgesia was also found, with PPTs significantly reduced in the points assessed. Neurodynamics show a significantly decreased value in women with CPP.Conclusions: Patients with chronic pelvic pain presented a high percentage of TrPs that reproduce their symptoms. Patients also showed a widespread pressure pain hyperalgesia and more mechanosensitive neural tissue due to a decrease on the range of motion related to neurodynamics.

Pelvic myofascial pain is associated with restriction of sexual activity in women with pelvic pain: a cross-sectional study

Abstract:Dyspareunia and restriction of sexual activity are often reported by patients with chronic pelvic pain, but less is known about which conditions or factors contribute most to these symptoms. This is a cross-sectional study of consecutive new patients who presented to a chronic pelvic pain and endometriosis referral clinic between January 2013 and December 2015. Patients completed a detailed questionnaire prior to their first visit and underwent a comprehensive standardised pelvic examination at their first visit. Patients were grouped according to description of sexual activity as 'normal activity without pain', 'normal activity but painful', 'severely restricted by pain', or 'absent due to pain.' Primary outcome was restriction of sexual activity due to pain. Multinomial logistic regression was performed with 'normal activity without pain' group as reference category and adjusted for age, depression, history of endometriosis, and pelvic myofascial pain. Over half of the 187 patients with chronic pelvic pain described severely restricted ( n = 75, 40.1%) or absent ( n = 21, 11.2%) sexual activity due to pain. Pelvic myofascial pain was highly prevalent in this population and pelvic myofascial tenderness score was associated with higher odds of sexual activity that was severely restricted by pain (OR 1.05, 95% CI 1.01–1.09, P = 0.006) or absent due to pain (OR 1.09, 95% CI 1.04–1.13, P < 0.001). History of endometriosis and depression were not independently associated with restriction of sexual activity due to pain. Dyspareunia and sexual dysfunction are highly prevalent among patients with chronic pelvic pain, and pelvic myofascial pain is strongly associated with restriction of sexual activity due to pain.

Abstract:The study aimed to evaluate the relation between depression, anxiety stress symptoms, duration and pain intensity in women with chronic pelvic pain (CPP). This study consisted of a sample of women diagnosed with CPP from April 2021 to August 2023, including women aged 18 years or older who agreed in writing to participate in the study with the Free Informed Consent Form, who could understand the study's objectives and participate. Women between 31 and 37 years old, married or in a stable relationship, with a higher education degree predominated. Were observed outside the normal range scale 58.8 percent of depression, 66.7 percent of anxiety and 59.8 percent of stress. The level of alteration most often observed in patients was extremely severe in 24 percent of with depression and 33.3 percent of anxiety, and the level was mild in 19.6 percent of stress symptoms. No statistical association was found between duration of pain and emotional states of anxiety, depression or stress. Regarding pain intensity levels, there was no significant association with the presence or absence of symptoms of depression outside the normal range or its levels. It's was significantly associated with the presence or absence of anxiety symptoms outside the normal range (p = .003) and with their levels (p = .005). Also significantly associated with the presence or absence of stress symptoms outside the normal range (p = .007), as was its levels (p = .023). The symptoms of depression, anxiety and stress occur frequently in women with CPP and that the intensity of pain experienced is significantly associated with the presence of stress and anxiety but not with depression.

Abstract:Background The pelvic floor musculature (PFM) plays a role that influences bladder, intestinal and sexual functions and when this musculature is affected, it can develop several symptoms with high prevalence in the female population. For women of reproductive age, every month passed by menstrual periods could have dysmenorrhea, a source of chronic pelvic pain originating from a difficult menstrual flow. There are few data on the influence of dysmenorrhea on other PFM disorders. Objective To correlate dysmenorrhea with PFM disorders in young nulliparous women. Methods A descriptive, observational, cross-sectional study with a quantitative approach was carried out. Enrollment was optional, with women aged between 18 and 30 years old, nulliparous, who had never been pregnant, had already experienced their first sexual intercourse and who did not have their menstruation on the day of the assessment. The evaluation was carried out through the application of tests (socio-clinical, International Consultation on Incontinence Questionnaire-Short Form, Pelvic Floor Distress Inventory, Visual Analog Scale (VAS) and, later, through the physical examination of the strength of the PFM through the Perfect scheme. Data were analyzed according to the sample's normality distribution, comparing the groups with and without pelvic floor dysfunction according to the presence of dysmenorrhea using the t-test for independent samples. The Statistical Program for Social Science program (version 23) was used, considering a significance level of 5%. Results The sample was fixed by 45 women (median age 21 years). The prevalence of dysmenorrhea was 77.7% with pain intensity 5 (2 - 6.50) on the VAS scale. The main symptoms of premenstrual tension were those of an emotional nature, such as irritability (84.4%), anxiety (73.3%), desire to cry (71.1%) and sadness (71.1%). The sample showed muscle weakness with a median of 3 (2 – 3) in the Perfect scheme, and 24.4% of the sample reported some type of urinary incontinence, and 17.7% had constipation. The group with dysmenorrhea had worse vaginal (p=0.04) and intestinal (p=0.03) dysfunctions. There was interference between dysmenorrhea, vaginal (R=0.81) and intestinal (R=0.57) disorders. Conclusion Dysmenorrhea is prevalent in young nulliparous women with an association between vaginal and intestinal dysfunctions, this population presents pelvic floor muscle weakness and symptoms of premenstrual tension predominantly of an emotional nature. Implications In scientific terms, this study demonstrated that PFM dysfunctions are a reality among young nulliparous women and that women with dysmenorrhea every month may be more subject to vaginal and intestinal dysfunctions with possible chronic implications for their health. In clinical terms, it is important to consider the complaint of dysmenorrhea in the evaluation and to question and physically assess the bladder, vaginal, sexual, and intestinal functions that may be associated.

Abstract:Objectives To investigate the efficacy of pelvic floor muscle training (PFMT) in treating female sexual dysfunction. Data Sources A systematic review of databases, including PubMed, Ovid Medline, CINAHL, Embase, BVSalud, Scopus, and Cochrane Library, was performed in July 2021 and updated in May 2023. Study eligibility criteria Full-text articles of randomized controlled trials (RCTs) comparing PFMT and no intervention or another conservative treatment in at least one arm to improve women's sexual function or treat sexual dysfunction were included. Study appraisal and synthesis methods The data for this review was extracted and analyzed by two independent reviewers. Data on the characteristics of each intervention was extracted using the Consensus on Exercise Reporting Template (CERT). The risk of bias and certainty of evidence was assessed using the Physiotherapy Evidence Database (PEDro) scale and the Grading of Recommendations, Assessment, Development, and Evaluations (GRADE) criteria, respectively. A meta-analysis was conducted considering the post-treatment mean score difference of female sexual function index (FSFI) between the control and treatment groups. Results Twenty-one RCTs were included in this review. The CERT revealed a varying quality of the PFMT protocols. Four studies were included in the meta-analysis showing that PFMT improved arousal (1.49; 95% confidence interval [CI]: 0.13 to 2.85), orgasm (1.55; 95%CI: 0.13 to 2.96), satisfaction (1.46; 95%CI: 0.14 to 2.77), pain (0.74; 95%CI: 0.11 to 1.37), and the FSFI overall score (7.67; 95%CI: 0.77 to 14.57). The GRADE showed a very low certainty of evidence due to the data's high clinical and statistical heterogeneity. No side effects of the interventions were reported. Conclusion This systematic review with meta-analysis showed that PFMT improved female FSFI total score and several subscales; however, the certainty of the evidence is low.

Abstract:Introduction and hypothesisWe performed a review of the literature reporting on the effects of pelvic floor muscle training (PFMT) on female sexual function (SF).MethodsPubmed (from 1946 to December 2014), Ovid Medline (from 1946 to December 2014), CINAHL (from 1937 to December 2014), PsycINFO (from 1805 to December 2014), Scopus and Cochrane Central Register of Controlled Trials were searched by two independent reviewers. Randomised controlled trials (RCTs) investigating the impact of PFMT on women’s SF published in English were included. Methodological quality was scored using the PEDro scale. Data were analysed qualitatively and interpreted.ResultsA total of 1341 women were included in the eight RCTs covered by this review. The studies were published between 1997 and 2014. Methodological scores were between 4 and 7. The sample included derived from heterogeneous populations of women. In only one study was SF the primary outcome measure. Pelvic floor dysfunction was an inclusion criterion in the majority of studies. Most studies reported a significant improvement in SF score after PFMT between control and intervention groups.ConclusionsAlthough most studies indicated an improvement of at least one sexual variable in women with pelvic floor dysfunction, and one study demonstrated an improvement in SF in postpartum women selected independently of their continence status, the results need to be interpreted with caution. High-quality RCTs specifically designed to investigate the impact of PFMT on women’s SF are required.

Abstract:Decision making in orthopaedic surgery is facilitated by outcome data, not only from the literature, but also from one's own practice. Between 1971 and 1996, 2602 primary and 587 revision total hip replacements were performed. All had prospective collection of data, which altered surgical practice. A posterior approach in 147 primary total hip replacements resulted in 10 dislocations (6.8%) leading to the use of the anterolateral approach in 1788 subsequent hips with only eight dislocations (0.45%). The initial 298 hips were done in a standard operating room with restricted access and were covered with perioperative antibiotics. No infections occurred permitting ongoing use of inexpensive infection prevention. From 1971 to 1975, 168 classic polished Charnley stems were implanted with only seven stems requiring revision to date. In contrast, use of the matte finished Müller prosthesis in the early 1970s in 58 hips has resulted in nine revised stems. From 1980 to 1990, 789 second generation design matte finished Iowa stems were implanted. Prospective data collected disclosed that a small number (1.5%) of early loosenings occurred and all were associated with extensive bone lysis which made revision difficult. Only polished stems are now cemented by the author. The author firmly believes that his practice patterns have been promptly and appropriately altered by the availability of a prospectively collected hip data file.

Abstract:Abstract Introduction: Pelvic floor muscle training (PFMT) and neuromuscular electrical stimulation (NMES) are physiotherapeutic conservative treatments to prevent and to treat pelvic floor dysfunctions. Objective: To investigate the immediate effect of one session of PFMT versus NMES associated to pelvic floor muscle (PFM) contraction on the PFM function in nulliparous women. Methods: This is a cross-sectional experimental study. Twenty women were randomized into the “PFMT Group” and “NMES Group”. PFM function evaluation was performed by vaginal palpation and manometry before and after a single session. PFMT was composed by one series of eight sustained contractions of 6 seconds and one series of four fast contractions, in four different positions. NMES parameters were: biphasic pulsed current; frequency: 50 Hz; pulse duration: 0.7 ms; cycle on:off 4:8s; rise/decay: 2/2s, time: 20 minutes; and intensity: participant' sensibility. Data was analyzed by the ANOVA two-way for repeated measures to verify the difference between groups, within group and the interactions for PFM function. A 5% probability was considered in all tests. Results: There were no significant differences between groups. At intra-group analysis, there was a significant decrease in the maximal voluntary contraction (p = 0.01), by manometry, between pre- and post-session for both groups. Conclusion: The immediate effects of a single session of PFMT and NMES associated with voluntary PFM contraction are similar on PFM function, that is, no difference was found between groups.

Abstract:The objective of this study was to evaluate the efficacy and factors of myofascial release therapy combined with electrical and magnetic stimulation in the treatment of chronic pelvic pain syndrome (CPPS). A total of 79 female patients diagnosed with CPPS from January 2021 to December 2022 were prospectively analyzed. Every patient received 3 weeks of treatment which included myofascial release therapy combined with electrical and magnetic stimulation. The visual analog score (VAS) of pelvic floor muscle (PFM) trigger points (TrPs) and the changes in pelvic floor surface electromyography before and after treatment were compared. Multiple linear regression was used to analyze the influencing factors of each outcome index. There were significant differences in VASs of muscle TrPs before and after treatment ( P < 0.05). For the surface electromyography of PFMs, the differences in pre-baseline rest, post-baseline rest, isometric contractions for muscle endurance evaluation, and coefficient of variation were statistically significant ( P < 0.05). Linear regression analysis showed that disease course ( X 1 ), dyspareunia ( X 5 ), and urinary incontinence ( X 6 ) were influencing factors for the decline of pre-baseline rest ( r 5 = 1.067, R 2 = 0.089), post-baseline rest ( r 1 = 0.055, r 5 = 0.99, R 2 = 0.119), VASs of ischial spine ( r 5 = 0.916, R 2 = 0.102), obturator internus ( r 5 = 0.796, r 6 = −0.703, R 2 = 0.245), and pubococcygeus ( r 5 = 0.885, R 2 = 0.149) after treatment in the CPPS group. This study confirmed that individualized myofascial release therapy combined with electrical and magnetic stimulation has significant efficacy for patients with CPPS. At the same time, it is more effective for CPPS patients with longer course of disease, dyspareunia, and without urinary incontinence.

Observations on the Therapeutic Efficacy of Biomimetic Physiotherapy Combined with Manipulation Therapy in Managing Myofascial Pelvic Pain Syndrome in Women

Abstract:Objective: This study aims to investigate the clinical efficacy of biomimetic physiotherapy combined with manipulation therapy in the management of female myofascial pelvic pain syndrome (MPPS). Methods: A total of 120 patients diagnosed with MPPS at our hospital from June 2018 to June 2021 were included. All patients had a history of sexual activity, met the diagnostic criteria for female chronic pelvic pain, and exhibited pelvic floor muscle and myofascial trigger points in gynecological examinations. Based on treatment methods, patients were categorized into a control group (n=64, treated with biomimetic physiotherapy) and an experimental group (n=56, treated with biomimetic physiotherapy plus manipulation therapy). Pre- and post-treatment assessments in both groups included pelvic floor muscle surface electromyogram, Visual Analogue Scale (VAS) score, pelvic floor muscle tenderness score, and pelvic floor muscle strength. Results: After treatment, in the control group, the mean values of pre-resting potential and post-resting potential declined significantly, from (9.58±2.22) to (4.06±0.77) and from (8.18±1.78) to (3.56±0.61), respectively. In the experimental group, these values decreased from (9.61±2.77) to (3.15±0.58), and from (8.16±1.78) to (2.79±0.59). The VAS score exhibited a noteworthy decrease from (6.18±1.00) to (3.15±0.56) in the control group and from (6.20±1.13) to (2.04±0.68) in the experimental group. The pelvic floor muscle tenderness score decreased from (8.14±0.86) to (3.78±0.77) in the control group and from (7.91±1.03) to (1.93±0.80) in the experimental group. Furthermore, the percentage of patients whose pelvic floor muscle strength increased from <grade III to ≥grade III, to 69.39% in the control group and 72.73% in the experimental group. Conclusions: Biomimetic physiotherapy plus manipulation therapy demonstrated enhanced pelvic floor muscle contractility, reduced subjective pain and pelvic floor myofascial tenderness, improved synergic movement, increased muscle fatigue resistance, and alleviated muscle spasms. This combined approach proved to be effective in the treatment of female MPPS.
