Abstract:The design of indoor airflow environments can significantly reduce the risk of respiratory epidemic infections indoors. Some studies have successfully developed theoretical models for calculating the effect of airflow fields on infection rates. However, up until now, studies have primarily focused on simulating and calculating the distribution of viral infection rates in current building scenarios. Due to the lack of a direct influence model for the design parameters and infection rate calculation, the present studies lack a quantitative analysis of the design parameters. This paper investigates the building openings design approach in a medium-sized kindergarten in Germany, intending to explore passive-based design solutions to improve the building's ability to prevent the virus' spread. We calculate the infection rate distribution in space by CFD combined with the Wells-Riley model. And then, use the Grasshopper platform to build an optimization model with the design parameters of building openings and infection rate values to discuss the relationship between geometric parameters and infection rate variation. The results show that the building openings' design parameters in transition spaces significantly affect the indoor infection rate under the condition that the input wind speed at the building openings is stable. We can see that optimizing building openings significantly reduces the average infection rate in space. The infection rate in the area with the largest decrease can be reduced by 18.41%. The distribution of infection rate in space is much more uniform, and the excess area is significantly reduced. This study has implications for future research and practice in designing public buildings under the influence of long-standing and cyclical outbreaks of epidemics.
Ventilation Strategies for Mitigation of Infection Disease Transmission in an Indoor Environment: A Case Study in Office

Abstract:During the normalization phase of the COVID-19 epidemic, society has gradually reverted to using building space, especially for public buildings, e.g., offices. Prevention of airborne pollutants has emerged as a major challenge. Ventilation strategies can contribute to mitigating the spread of airborne disease in an indoor environment, including increasing supply air rate, modifying ventilation mode, etc. The larger ventilation rate can inevitably lead to high energy consumption, which may be also ineffective in reducing infection risk. As a critical factor affecting the spread of viral contaminant, the potential of ventilation modes for control of COVID-19 should be explored. This study compared several ventilation strategies in the office, including mixing ventilation (MV), zone ventilation (ZV), stratum ventilation (SV) and displacement ventilation (DV), through analyzing ventilation performance and infection risk for the optimal one. By using ANSYS Fluent, the distributions of airflow and pollutant were simulated under various ventilation modes and infected occupants. The SV showed greater performance in mitigating infection disease spread than MV, ZV and DV, with an air distribution performance index (ADPI) of 90.5% and minimum infection risk of 13%. This work can provide a reference for development of ventilation strategies in public space oriented the prevention of COVID-19.

Numerical modelling of ventilation strategies for mitigating cough particles transmission and infection risk in hospital isolation rooms

Abstract:Indoor and Built Environment, Ahead of Print. This study used numerical modelling to analyze air velocity, cough particle distribution and infection risks in an isolation room. It investigated air change rates, inlet/outlet vent positioning and assessed various ventilation rates and outlet configurations for reducing infection risks. Quantitative assessments revealed different particle escape timings. In Case 1, smaller particles (2–4 μm) took 8.2 s to escape, while in Case 2, this time extended to 22.7 s. At 48 ACH, there were significant improvements in removing particles of various sizes, particularly those sized 2–4 μm, 16–24 μm and 40–50 μm, reducing the infection risk. The use of the Wells-Riley model highlighted considerable reductions in infection probabilities with higher ACH. Specifically, infection risks were reduced to 5% in Case 1 and 17% in Case 2, underscoring the marked advantage of Case 1 in reducing infection probabilities, particularly for smaller particles. Furthermore, escalated ACH values consistently correlated with decreased infection probabilities across all particle sizes, highlighting the pivotal role of ventilation rates in mitigating infection risks. The study comprehensively investigated the distribution of air velocity, dynamics of cough particles and infection risk associated with different ventilation strategies in isolation rooms.

Abstract:In a virus pandemic context, buildings ventilation has been recognized as a solution for preventing transmission of the virus in aerosolized form. The impact of the widespread recommendation of window opening and sealing door on ventilation circuits needs to be considered with a multizone approach. We modeled the airflow distribution in a building where people are isolating in a pandemic context, including one infected person. We analyzed the impact of opening the window and sealing the door in the quarantine room on exposures and probability of infection for occupants of the flat and of adjacent flats. In order to study the sensitivity of the results, we tested three ventilation systems: balanced, exhaust‐only, and humidity‐based demand‐controlled, and several window‐ and door‐opening strategies. When the door of the quarantine room is sealed, we observe that opening the window in the quarantine room always results in increased exposure and probability of infection for at least one other occupant, including in neighbors' apartments. When all internal doors are opened, we observe moderate impacts, with rather an increase of exposure of the occupants of the same apartments and of their probability of infection, and a decrease for the occupants located in other apartments. Based on the analysis on the airflows distribution in this case study, we conclude that sealing the internal door has more influence than opening the window of the quarantine room, whatever the ventilation system. We observe that this widespread recommendation to open the window of a quarantine room and to seal the door is based on the consideration of a single zone model. We illustrate the importance of moving from such a single zone approach to a multizone approach for quantifying ventilation and airing impacts in multizone buildings as residences in order to prevent epidemics of viruses such as SARS‐CoV‐2. It highlights the need of air leakage databases.

Ventilation reconstruction in bathrooms for restraining hazardous plume: Mitigate COVID-19 and beyond

Abstract:Converging evidence reports that the probability of vertical transmission patterns via shared drainage systems, may be responsible for the huge contactless community outbreak in high-rise buildings. Publications indicate that a faulty bathroom exhaust fan system is ineffective in removing lifted hazardous virus-laden aerosols from the toilet bowl space. Common strategies (boosting ventilation capability and applying disinfection tablets) seem unsustainable and remain to date untested. Using combined simulation and experimental approaches, we compared three ventilation schemes in a family bathroom including the traditional ceiling fan, floor fan, and side-wall fan. We found that the traditional ceiling fan was barely functional whereby aerosol particles were not being adequately removed. Conversely, a side-wall fan could function efficiently and an enhanced ventilation capability can have increased performance whereby nearly 80.9% of the lifted aerosol particles were removed. There exists a common, and easily-overlooked mistake in the layout of the bathroom, exposing occupants to a contactless vertical pathogen aerosol transmission route. Corrections and dissemination are thus imperative for the reconstruction of these types of family bathrooms. Our findings provide evidence for the bathroom and smart ventilation system upgrade, promoting indoor public health and human hygiene.
The Risk of Aerosol Transmission of Respiratory Infectious Diseases in the Isolation Ward

Abstract:Nosocomial infections have caused significant health and socio-economic losses to patients,healthcare workers,and the nation.Aerosols can carry various pathogens and serve as important media for the transmission of nosocomial infections.Airborne transmission is a crucial pathway for nosocomial infections.During routine diagnosis,treatment,and epidemic prevention and control,healthcare workers often need to have close contact with patients and are exposed to aerosols exhaled by patients.Calculating the concentration of pathogens in hospitals and assessing the infection risk is of great significance in reducing the occurrence of nosocomial infections and responding to newly emerging infectious diseases.Currently,most studies on infection risk assessment are based solely on simulations of spatial and temporal distributions of aerosols exhaled by patients,making it difficult to accurately calculate the distribution of viral concentration and infection risk. In this study,the computational fluid dynamics(CFD)method is employed to simulate the dispersion and deposition of aerosols exhaled by infected patients in the isolation ward at Wuhan Huoshenshan Hospital.A quantitative microbial risk analysis(QMRA)model is developed to evaluate the risk of both inhalation and surface contact infections within the mentioned ward,taking into consideration uncertainties regarding viral loads,virus half-life,deposition on respiratory tracts,ventilation,protective measures,and personnel activity.Subsequently,the risk distribution within the isolation ward and the probability of infection among medical staff under various conditions are calculated.The findings of this study reveal significant spatial heterogeneity in the risk distribution within the negative isolation ward.The transport and deposition of viral aerosols are strongly influenced by airflow dynamics and ventilation systems,resulting in the formation of high-risk areas within the ward.Moreover,it is observed that the risk of inhalation infection is closely associated with the intensity of individual activity and the depth of breathing.At high activity levels,the risk of infection can be approximately twice as high as that at moderate activity levels.Furthermore,the calculations demonstrate that the utilization of N95 masks can substantially reduce the risk of inhalation infection by 60％-90％.It is worth noting that certain mutations of SARS-CoV-2 may result in an elevated viral load and subsequently an increased risk of infection. Based on these findings,this study concludes that the utilization of appropriate personal protective equipment such as N95 masks,face masks,and disposable gloves can effectively mitigate the risk of infection among medical and nursing staff.Furthermore,rearranging office areas to avoid high-risk zones,adjusting disinfection intervals within wards to prevent the build-up of high viral concentrations on object surfaces,implementing strict control over the working hours of medical and nursing staff,and enhancing protection measures for patients with mutant strains of the infection are all effective measures in reducing the risk of infection.Special attention should also be given to the influence of individual variations in infection.The outcomes of this study can serve as a theoretical reference and offer technical support for the prevention and reduction of nosocomial infections.
Impact of ventilation and avoidance measures on SARS-CoV-2 risk of infection in public indoor environments

Abstract:BackgroundThe literature includes many studies which individually assess the efficacy of protective measures against the spread of the SARS-CoV-2 virus. This study considers the high infection risk in public buildings and models the quality of the indoor environment, related safety measures, and their efficacy in preventing the spread of the SARS-CoV-2 virus.MethodsSimulations are created that consider protective factors such as hand hygiene, face covering and engagement with Covid-19 vaccination programs in reducing the risk of infection in a university foyer. Furthermore, a computational fluid dynamics model is developed to simulate and analyse the university foyer under three ventilation regimes. The probability of transmission was measured across different scenarios.FindingsEstimates suggest that the Delta variant requires the air change rate to be increased >1000 times compared to the original strain, which is practically not feasible. Consequently, appropriate hygiene practices, such as wearing masks, are essential to reducing secondary infections. A comparison of different protective factors in simulations found the overall burden of infections resulting from indoor contact depends on (i) face mask adherence, (ii) quality of the ventilation system, and (iii) other hygiene practices.InterpretationRelying on ventilation, whether natural, mechanical, or mixed, is not sufficient alone to mitigate the risk of aerosol infections. This is due to the internal configuration of the indoor space in terms of (i) size and number of windows, their location and opening frequency, as well as the position of the air extraction and supply inlets, which often induce hotspots with stagnating air, (ii) the excessive required air change rate. Hence, strict reliance on proper hygiene practices, namely adherence to face coverings and hand sanitising, are essential. Consequently, face mask adherence should be emphasized and promoted by policymakers for public health applications. Similar research may need to be conducted using a similar approach on the Omicron (B.1.1.529) variant.

Abstract:The outbreak of COVID-19 has caused increasing public attention to laboratory-acquired infections (LAIs), especially for a mobile Bio-Safety Level 4 Lab (BSL-4) with high potential of exposure. In this paper, the distribution and removal mechanism of bioaerosols in the biosafety laboratory were studied. A simulation model of airflow distribution in the opening and closing state of air-tight door was established and verified. The results showed that the airflow entrainment velocity during the opening of the door was approximately 0.12 m/s. It increased the probability of vortex generation in the laboratory. The deposition rate of particles was doubled when the air-tight door opening is compared with air-tight door closing. Besides, nearly 80% of the particles deposited on the surface of the wall and ceiling, increasing the possibility of LAIs. The findings of this paper could provide new scientific methods for high-level biosafety laboratories to avoid cross-infection. Moreover, future work regarding air-tight door rotation speed regulation and control should be emphasized.

Numerical investigation of airborne transmission in low-ceiling rooms under displacement ventilation

Abstract:This study employs computational fluid dynamics (CFD) simulations to evaluate the risk of airborne transmission of COVID-19 in low-ceiling rooms, such as elevator cabins, under mechanical displacement ventilation. The simulations take into account the effects of the human body's thermal environment and respiratory jet dynamics on the transmission of pathogens. The results of the study are used to propose a potential mitigation strategy based on ventilation thermal control to reduce the risk of airborne transmission in these types of enclosed indoor spaces. Our findings demonstrate that as the ventilation rate (Qv) increases, the efficiency of removing airborne particles (Ep) initially increases rapidly, reaches a plateau (Ep,c) at a critical ventilation rate (Qc), and subsequently increases at a slower rate beyond Qc. The Qc for low-ceiling rooms is lower compared to high-ceiling rooms due to the increased interaction between the thermal plume generated by the occupants or infectors and the ventilation. Further analysis of the flow and temperature fields reveals that Ep is closely linked to the thermal stratification fields, as characterized by the thermal interface height, the height of the temperature isosurface, and temperature gradient. The simulations also indicate that the location of infector relative to ventilation inlet/outlet affects Qc and Ep,c with higher Qc and lower Ep,c observed when infector is in a corner due to potential formation of a local hot spot of high infection risk when infector is near the ventilation inlet. In conclusion, based on the simulations, we propose a ventilation thermal control strategy, by increasing the ventilation temperature, to reduce the risk of airborne transmission in low-ceiling rooms. Our findings indicate that the thermal environment plays a critical role in the transmission of airborne diseases confined spaces.

Abstract:Viable aerosols in the airflow may increase the risk of occupants contracting diseases. Natural ventilation is common in buildings and is accompanied by re-entry airflow during the ventilation process. If the re-entry airflow contains toxic or infectious species, it may cause potential harm to residents. One of the Covid-19 outbreaks occurred in a public residential building at Luk Chuen House (LC-House) in Hong Kong. It is highly suspected that the outbreak of the disease is related to the re-entry airflow. The study attempts to explain and discuss possible causes of the outbreak. In order to understand the impact of airflow on the outbreak, a public residential building similar to LC-House was used in the study. Two measurements M − I and M − II with the same settings were conducted for a sampling unit in the corridor under low and strong wind conditions respectively. The sampling unit and the tracer gas carbon dioxide (CO2) were used to simulate the index unit and infectious contaminated airflow respectively. The CO2 concentrations of the unit and corridor were measured simultaneously. Two models of Traditional Single-zone model (TSZ-model) and New Dual-zone model (NDZ-model) were used in the analysis. By comparing the ACH values obtained from the two models, it is indicated that the re-entry airflow of the unit is related to the corridor wind speeds and this provides a reasonable explanation for the outbreak in LC-House, and believes that the results can help understand the recent frequent cluster outbreaks in other residential buildings.

Exploring high-rise preventive ventilation: Experimental investigation of inter-zone air pressurization with tracer gas analysis

Abstract:Over the past four years, five major variants of the SARS-CoV-2 virus have circulated globally, causing seven million deaths. Meanwhile, specific preventative ventilation measures were recommended to minimize exposure to the aerosol infection risk, such as opening doors, and disabling ventilation systems. Given more attention to high-rise buildings, where quite a few outbreaks have been reported, there is a notable lack of reported studies assessing preventive ventilation measures. Therefore, this study focused on practicing several ventilation measures in a 16-story high-rise building located in Montreal, Canada. A variety of inter-zonal tracer measurements, including room-to-floor and inter-floor tests, were carried out considering controlled airflow/pressure across zones. An automated data acquisition system was developed for real-time monitoring of tracer/CO 2 spatial concentrations. Findings show that enabled ventilation with opened doors reduces CO 2 concentrations by up to 82 % in nearby source locations and positively pressurized zones; however, negatively pressurized zones experience a 40 % increase in tracer exposure. With ventilation active, a corridor source leads to a higher risk of tracer exposure (up to 86 %) than the in-room source. Disabled ventilation with closed doors is a recommended ventilation measure, for corridors with infection sources, to minimize tracer transport across zones. Zones with infection sources follow the latter ventilation measure; however, ventilation with open doors offers a conditional ventilation measure if careful airflow is designed particularly at return and exhaust points within the ventilation system. This study has implications for practicing preventative ventilation measures for post-COVID-19 building practices and design considerations.

Optimizing indoor air quality: CFD simulation and novel air cleaning methods for effective aerosol particle inhibition in public spaces

Abstract:In contemporary building ventilation, displacement and mixing ventilation demand high air volumes for rapid virus elimination, resulting in elevated energy consumption. To minimize the spread of viruses and decrease energy consumption for ventilation, this study employed CFD to explore the efficacy of a downward uniform flow field in impeding the transmission of aerosol particles in a high-traffic public facility, like a supermarket. The findings indicate that the downward uniform flow field proves insufficient when individuals remain static for extended periods. A wind speed of 0.1 m/s or higher becomes essential to overpower the stationary thermal plume, which disrupts this flow field. In areas with human presence, however, this technique is found to be particularly efficient since mobile heat sources do not generate a fixed thermal plume. A 0.05 m/s downward uniform flow field can settle 90% of particles within just 22 s. This flow pattern contributes to the swift settling of aerosol particles and effectively diminishes their dispersion. Employing this flow pattern in public places with increased foot traffic, like supermarkets, can lower the risk of contracting novel coronavirus without augmenting energy consumption. In order to implement the flow field in a part of the domain, a new air purification device is proposed in this study. The device combined with shelves can optimize the flow field uniformity through the MLA (PSO-SVR) algorithm and alteration of the air distribution structure. The uniformity of the final flow field increased to 0.925. The combination of data-driven MLA with CFD showed good performance in predicting the flow field uniformity. These findings offer valuable insights and practical applications for the prevention and control of respiratory diseases, particularly in post-epidemic scenarios.

Abstract:In recent years, a large number of respiratory infectious diseases (especially COVID-19) have broken out worldwide. Respiratory infectious viruses may be released in the air, resulting in cross-infection between patients and medical workers. Indoor ventilation systems can be adjusted to affect fine particles containing viruses. This study was aimed at performing a series of experiments to evaluate the ventilation performance and assess the exposure of healthcare workers (HW) to virus-laden particles released by patients in a confined experimental chamber. In a typical ward setting, four categories (top supply and exhaust, side supply and exhaust) were evaluated, encompassing 16 different air distribution patterns. The maximum reduction in the cumulative exposure level for HW was 70.8% in ventilation strategy D (upper diffusers on the sidewall supply and lower diffusers on the same sidewall return). The minimum value of the cumulative exposure level for a patient close to the source of the contamination pertained to Strategy E (upper diffusers on the sidewall supply and lower diffusers on the opposite sidewall return). Lateral ventilation strategies can provide significant guidance for ward operation to minimizing the airborne virus contamination. This study can provide a reference for sustainable buildings to construct a healthy indoor environment.

Effects of ventilation rate and social distancing on risk of transmission of disease: a numerical study using Eulerian-Lagrangian method

Abstract:This study utilized the Eulerian-Lagrangian method to investigate the risk of transmission of disease by tracking particles generated through coughing. The effects of ventilation rates (0.5, 5.0, and 10 ACH) and social distancing (0.9 and 1.8 m) were examined in a small office room using a displacement ventilation system. Numerical simulations considered different particle sizes (1, 5, 10, 20, 40, and 80 μm) to understand particle behavior and transmission routes. The airflow resulting from human coughing was validated with experimental data. Results showed that at a social distance of 0.9 m, increasing the ventilation rate led to a higher fraction of particles directly inhaled by susceptible individuals, potentially causing droplet and airborne transmission. However, maintaining a social distance of 1.8 m and a ventilation rate of 10 ACH significantly reduced the fraction of inhaled particles. Larger particles tended to deposit on floors and surfaces, while smaller particles remained suspended in the air. Higher ventilation rates increased particle deposition on the body surface of susceptible individuals, whereas increasing social distance reduced particle deposition. These findings highlight the importance of appropriate ventilation rates and social distancing in reducing the risk of infection transmission. Maintaining a social distance of 1.8 m combined with increased ventilation effectively reduced the fraction of inhaled particles. Larger particles were more likely to deposit on surfaces, emphasizing the need for regular disinfection. Understanding the dynamics of infectious particles and implementing effective ventilation and distancing measures can help mitigate the spread of infectious diseases in indoor environments. Abstract:The Covid-19 virus can spread rapidly in the air and cause the epidemic to rage around the world. This study takes a typical office as the research object and proposes four different partition settings to suppress the spread of the virus in the air based on displacement ventilation. And these strategies are verified by DPM model simulations of two daily situations of indexed person for coughing and talking. This study then employed a modified Wells-Riley model to evaluate the effectiveness of these strategies in mitigating the risk of viral infection. As expected, in the absence of a partition, a susceptible person at the other end of the seat greatly increased the risk of infection. Partitions can significantly reduce the risk of infection (100% → 20%), while partitions with a full-wrapped structure can reduce the risk of infection to less than 1%. For the instantaneous release scenario of coughing, the barrier can capture a large particle diameter (>50 μm) particles, and the smaller particle diameter can be discharged out of the room with the thermal plume. The strategies provided in this study can provide recommendations for indoor epidemic prevention and control in the context of the Covid-19 pandemic.

Abstract:Indoor and Built Environment, Ahead of Print. The high concentration of viral bioaerosols within the negative pressure isolation wards could pose a challenge to preventing potential cross-infection amongst healthcare workers (HCWs) and patients. Using the Euler–Lagrange methodology, this study numerically simulated the spatial and temporal distribution characteristics of bioaerosols in a typical negative pressure isolation ward as well as determined the interaction of ventilation mode and patient posture on ward ventilation performance. The removal effect of particle groups produced by two respiratory behaviours (breathing and coughing) was quantitatively analyzed, and the effect of exhaust air ratio and air exchange rate on particle distribution was discussed. The results showed that the migration characteristics of bioaerosol particles were sensitive to both the ventilation pattern and patient posture, which showed significant interactions. On this basis, the ventilation pattern with the best ventilation performance was evaluated, showing a particle removal effect of 70–85%. Due to the initial momentum difference, the diffusion behaviour of cough and breath particles was not consistent, but optimizing the airflow distribution near the exhaust outlet could improve their removal efficiency in the meantime. Further studies found that equal exhaust air velocity ratio facilitated the removal of aerosol particles, and an appropriate increase in the air exchange rate could also reduce the particle content.

The airborne transmission of infection between flats in high-rise residential buildings: Tracer gas simulation

Abstract:Airborne transmission of infectious respiratory diseases in indoor environments has drawn our attention for decades, and this issue is revitalized with the outbreak of severe acute respiratory syndrome (SARS). One of the concerns is that there may be multiple transmission routes across households in high-rise residential buildings, one of which is the natural ventilative airflow through open windows between flats, caused by buoyancy effects. Our early on-site measurement using tracer gases confirmed qualitatively and quantitatively that the re-entry of the exhaust-polluted air from the window of the lower floor into the adjacent upper floor is a fact. This study presents the modeling of this cascade effect using computational fluid dynamics (CFD) technique. It is found that the presence of the pollutants generated in the lower floor is generally lower in the immediate upper floor by two orders of magnitude, but the risk of infection calculated by the Wells-Riley equation is only around one order of magnitude lower. It is found that, with single-side open-window conditions, wind blowing perpendicularly to the building may either reinforce or suppress the upward transport, depending on the wind speed. High-speed winds can restrain the convective transfer of heat and mass between flats, functioning like an air curtain. Despite the complexities of the air flow involved, it is clear that this transmission route should be taken into account in infection control.

Abstract:In real-life scenarios, occupants typically alternate between sitting and walking in a room. The wake induced by human movement affects the local airflow, enhances local mixing effects, and increases airborne propagation laterally and across long distances. This study aimed to provide a comprehensive comparison of the impact of human movement on the dispersion of coughed droplets in an office under four ventilation strategies (i.e. Mixed Ventilation (MV), Displacement Ventilation (DV), Stratum Ventilation (SV) and Stratum Displacement Ventilation (SDV)). The improved Wells-Riley model was used to calculate the risk of infection for susceptible individuals in both static and dynamic indoor environments under different ventilation strategies. The results showed that different ventilation strategies, as well as the presence or absence of human movement, significantly affected the diffusion of droplets. When there was no human movement or human movement occurred close to the air supply, the risk of infection for susceptible individuals was lowest under SDV, 5.03 × 10 −6 and 21.44 × 10 −6 , respectively. The risk of infection for susceptible individuals during the infection stage was almost linearly related to exposure time when human beings moved away from the air supply, and all four ventilation methods presented a high risk of infection. The SDV has the best performance, followed by DV, SV, and finally MV for reducing the risk of indoor cross infection. These results can provide a reference for epidemic control in both static and dynamic indoor environments.

Abstract:To prevent respiratory infections between patients and medical workers, the transmission risk of airborne pollutants in hospital wards must be mitigated. The ventilation modes, which are regarded as an important strategy to minimize the infection risk, is challenging to systematically design. Studies have considered the effect of ventilation openings (inlets/outlets) or infected source locations on the airflow distribution, pollutant removal, and infection risk mitigation. However, the relationship (such as relative distance) between ventilation openings and infected sources is critical because it affects the direct exhaust of exhaled pollutants, which has not been thoroughly studied. To explore pollutant removal and infection prevention in wards, different ventilation modes (with varying ventilation openings) and infected patient locations must be jointly considered. This study investigated displacement ventilation (DV), downward ventilation (DWV), and stratum ventilation (SV) with 4, 6, and 10 scenarios of ventilation openings, respectively. The optimal ventilation mode and relative distance between outlets and infected patients were analyzed based on the simulated pollutant concentration fields and the evaluated infection risk. The pollutant removal effect and infection risk mitigation of SV in the ward were largely improved by 67% and 59% compared with DV and DWV, respectively. The average infection risk was reduced below 7% when a non-dimensional relative distance (a ratio of the actual distance to the cubic root of the ward volume) was less than 0.25 between outlets and infected patient. This study can serve as a guide for the systematic ventilation system design in hospitals during the epidemic.

Abstract:Full-scale experiments and CFD simulations were performed to study potential inter-cubicle airborne transmissions through a shared anteroom due to the hinged door opening. When doors are closed, current negative pressure designs are effective for the containment of airborne pathogens in the 'dirty' cubicle with an index patient. When the 'dirty' cubicle door is open, airborne agents can move into the other 'clean' cubicle via the shared anteroom. As the door being opened or closed, the door sweeping effect is the main source of the two-way airflow and contaminant exchange through the doorway. When the dirty cubicle door remains fully open, temperature difference and concentration gradient across the doorway induce the two-way buoyancy-driven flow and transport of airborne agents across the doorway. The longer the dirty cubicle door remains fully open (10 s, 30s or 60 s) or the smaller the air change rate (34-8.5 ACH for each cubicle), the more airborne pathogens are being transported into the 'clean' cubicle and the longer time it takes to remove them after the door is closed. Keeping the door completely open is potentially responsible for the majority of inter-cubicle transmissions if its duration is much longer than the duration of door motion (only 3 s). Our analyses suggest a potential inter-cubicle infection risk if the shared anteroom is used for multiple isolation cubicles. Decreasing the duration of door opening, raising air change rate or using a curtain at the doorway are recommended to reduce inter-cubicle exposure hazards. (C) 2015 Elsevier Ltd.
