Abstract:Rationale: Hereditary hemochromatosis (HH) is a hereditary disorder of iron metabolism. It is classified into 4 main types depending on the underlying genetic mutation: human hemochromatosis protein (HFE) (type 1), hemojuvelin (HJV) (type 2A), HAMP (type 2B), transferrin receptor-2 (TFER2) (type 3), and ferroportin (type 4). Type 4 HH is divided into 2 subtypes according to different mutations: type 4A (classical ferroportin disease) and type 4B (non-classical ferroportin disease). Type 4B HH is a rare autosomal dominant disease that results from mutations in the Solute Carrier Family 40 member 1 (SLC40A1) gene, which encodes the iron transport protein ferroportin. Patient concerns: Here we report 2 elderly Chinese Han men, who were brothers, presented with liver cirrhosis, diabetes mellitus, skin hyperpigmentation, hyperferritinaemia as well as high transferrin saturation. Diagnosis: Subsequent genetic analyses identified a heterozygous mutation (p. Cys326Tyr) in the SLC40A1 gene in both patients. Interventions: We treated the patient with iron chelator and followed up for 3 years. Outcomes: Iron chelator helped to reduce the serum ferritin and improve the condition of target organs, including skin, pancreas, liver as well as pituitary. Lessons: Type 4B HH is rare but usually tends to cause multiple organ dysfunction and even death. For those patients who have difficulty tolerating phlebotomy, iron chelator might be a good alternative.

Abstract:In March 2022, EASL released a new version of the clinical practice guidelines on haemochromatosis. Haemochromatosis is characterized by elevated transferrin saturation (TSAT) and progressive iron overload mainly involving the liver, and early diagnosis and venesection can prevent liver cirrhosis, hepatocellular carcinoma, diabetes, arthritis, and other complications. For patients with p.Cys282Tyr homozygous mutation of the hemochromatosis gene HFE, haemochromatosis can be diagnosed if serum iron parameters show TSAT ＞45% and ferritin ＞200 μg/L in female patients, or TSAT ＞50% and ferritin ＞300 μg/L in male patients and postmenopausal female patients. If a patient has elevated TSAT and ferritin and belongs to other HFE genotypes, magnetic resonance or liver biopsy is needed to confirm iron overload in the liver. Liver fibrosis stage and damage to other organs should be carefully assessed at the time of diagnosis, which will help to determine management strategies. Hepatocellular carcinoma should be screened for patients with progressive liver fibrosis. The goal of venesection is to achieve ferritin ＜50 μg/L during the induction stage and ferritin ＜100 μg/L during the maintenance stage.

SLC40A1-related hemochromatosis associated with a p.Y333H mutation in mainland China: a pedigree report and literature review

Abstract:Background: Haemochromatosis is a genetic disease characterized by the excessive deposition of iron in various tissues and organs, eventually results in organ damage including cirrhosis, diabetes, cardiomyopathy, etc. SLC40A1-related haemochromatosis is associated with gain-of-function mutations in the SLC40A1 gene, which encodes ferroportin. While sporadic reports of this condition exist in mainland China, the understanding of the phenotype and genetic pattern associated with the SLC40A1 p.Y333H mutation remains incomplete. Case presentation: We report a pedigree with heterozygous p.Y333H mutation in Chinese Han population. The proband is a 64-year-old man complaining of persistent abnormality of liver enzyme levels for 1 year, with a history of knee joint pain, diabetes and skin pigmentation. He displayed markedly elevated serum ferritin level and transferrin saturation. Magnetic resonance imaging showed iron deposition in the liver, spleen, and pancreas, along with cirrhosis and splenomegaly. Whole exome sequencing identified a heterozygous allelic variant c.997T > C (p.Y333H). Genetic screening of family members identified four first-degree relatives and three second-degree relatives having the same mutation. Additional cases with this mutation from two published studies were included. Among the probands and screened relatives, all eight males aged over 30 y had ferritin level > 1000 µg/L, transferrin saturation > 90%. Four patients with organ damage in the present study received therapeutic phlebotomy, alleviating clinical symptoms and improving in transferrin saturation and serum ferritin. Conclusions: This study reports the largest pedigree with heterozygous SLC40A1 p.Y333H mutation in the Chinese population to date. In Chinese families, males over 30 years old with hemochromatosis due to SLC40A1 p.Y333H mutation exhibit severe iron overload phenotypes.

Abstract:Cholestasis refers to a pathological status of bile formation, secretion and excretion disorders with a variety of intrahepatic and extrahepatic causes; as a result, bile cannot flow normally into the duodenum and enter the bloodstream. The clinical manifestations of cholestasis include pruritus, fatigue, dark urine, and jaundice. At the early stage of the disease, patients are usually asymptomatic, and the only presentation is an increase in alkaline phosphatase (ALP) and gamma-glutamyl transferase (GGT) levels. After disease progression, patients can present with hyperbilirubinemia; when severe, it can cause liver failure or even death. Hepatobiliary diseases with the major presentation of cholestasis caused by a variety of reasons are collectively referred to cholestatic liver diseases. Cholestasis itself can further aggravate liver damage. According to the affected site, cholestatic liver diseases can be divided into intrahepatic and extrahepatic cholestasis. If cholestasis is persistent for over 6months, it is called chronic cholestasis. This consensus mainly describes intrahepatic cholestatic liver diseases.
Meta-analysis on Clinical Characteristics of Chinese Patients with Haemochromatosis in 1991-2010

Abstract:Objective To enhance the physicians to recognize the clinical features of in Chinese.Methods Chinese literature,published from 1991 to 2010,were searched with the key word haemochromatosis in the database of China Hospital Knowledge Database(CHKD).The study enrolled total 71 patients with haemochromatosis,whose liver biopsies and(or) liver magnetic resonance imaging(MRI) were possible.The patients were further divided into two groups of primary haemochromatosis(PHC) and secondary haemochromatosis(SHC),whose clinical characteristics were analyzed.Results With an average age of(38.1±15.2) years old,7 of 43 PHC patients were females and 12 of them had similar symptoms among the family members,and all the 28 patients with SHC were men and had an average age of(44.2±13.2) years old.Of 28 patients with SHC,9 cases had anemia and 19 cases had chronic liver diseases(CLDs).Though no significant difference existed,the main complains of patients with PHC and SHC were pigmentation of skin,fatigue,diabetes,impotence and actively short breath.And the main signs were frequently shown as pigmentation of skin,splenomegaly,hepatomegaly,hepatosplenomegaly,dilated cardiac dullness and multi-organ lesions usually appeared.With a percentage of 67.4%(29/43) and 67.9%(19/28),respectively,the most frequent incidence of multi-organ lesions in patients with PHC and SHC was the liver damage combined with pigmentation of skin.Furthermore,the percentages of complications such as pigmentation of skin,diabetes and diabetes combined with pigmentation of skin in PHC patients with liver cirrhosis were significantly higher than those without cirrhosis(P0.05).As for the laboratory tests,the percentage of serum ferritin(SF) rise in patients with PHC or SHC was 94.3% and 100.0%,respectively(P0.05).78.6% patients of PHC had an elevation of transferrin saturation(TS),compared to the elevation of both 2 cases of SHC patients detected for TS.Conclusion With multi-organ lesions,haemochromatosis is characterized by an iron-overloaded disease,whose diagnosis depended on liver biopsies and abdominal MRI.To avoid the diagnostic errors and missed diagnosis,the suspected patients with large amounts of blood transfusion or chronic liver diseases should be routinely detected by fast TS and SF,if elevated,hereditary genes should be further screened.Furthermore,an early diagnosis and some effective treatment means should be taken to improve the prognosis of patients with haemochromatosis.

Abstract:Hemochromatosis, also known as siderosis, is a disease caused by excessive iron deposition in human organs and tissues, resulting from iron metabolism disorders. It is clinically characterized by skin pigmentation (bronze color), liver cirrhosis, diabetes, weakness, and fatigue. Additional symptoms may include arthritis, hypothyroidism, heart failure, and sexual hypofunction. Clinical manifestations can vary from person to person, with a few patients showing no clinical manifestations, which makes the diagnosis difficult for clinicians. In this case report, we described hereditary hemochromatosis related to a mutation in the HAMP gene in Fuyang City, China, as a reference for clinicians. Hereditary hemochromatosis is rarely reported in China. Clinicians in China have relatively insufficient knowledge of this disease, which leads to frequent misdiagnosis. In this case report, we describe hereditary hemochromatosis related to HAMP gene mutation in Fuyang City, China, for the clinician’s reference.

Abstract:Hereditary hemochromatosis and β-thalassemia can both result in the inappropriately low production of the hormone hepcidin, which leads to an increase in intestinal absorption and excessive iron deposition in the parenchymal cells. To the best of our knowledge, there have been no reports on the coexistence of the two disorders in China. We herein report a case in a Chinese who presented with late-onset hepatic cirrhosis with hereditary hemochromatosis and β-thalassemia. We analyzed the pedigree of the two disorders and the iron status in his family members. Our case supports that a heterozygous H63D mutation can interact with β-thalassemia, leading to late-onset hemochromatosis.

Abstract:Hemochromatosis is the most common genetic disease in northern European populations. Body iron stores progressively increase in most patients, which can lead to cirrhosis of the liver, hepatocellular carcinoma, heart failure, arthritis, and pigmentation. Simple blood tests such as the serum ferritin and transferrin saturation are useful to suggest the diagnosis which can be confirmed in most cases with a simple genetic test for the C282Y mutation of the HFE gene. However, these blood tests are often misinterpreted and there are rare patients with iron overload without HFE mutations. A diagnostic approach is presented based on a large referral practice and a population-based study (HEIRS) which screened for iron overload in 101,168 participants.

Abstract:Myocardial involvement was indicated by isolated findings (especially on the ECG) without corresponding functional disorders in one third of cases of dermatomyositis. Rapidly fatal heart failure occurred rarely in this disease, and was noteworthy for a QS complex in the central precordial leads, and for the presence of intracardiac thromboses which could give rise to emboli. Terminal cardiac failure was the rule in cases of periarteritis nodosa, which is a much rarer disease; this occurrence was secondary to hypertension. The most characteristic lesions were nodular coronary arteritis and disseminated myocarditis due to the arteriolar lesions.

Abstract:With China's outstanding achievements in the prevention and treatment of hepatitis, hereditary cholestasis caused by genetic variants has gradually become an important cause of death or transplantation in children with liver disease. The continuous identification of new pathogenic genes expands the disease spectrum and clinician's understanding of disease. The disease characteristics and clinical manifestations of hereditary cholestasis caused by different gene variants vary, and the severity of diseases caused by the same gene variants and the response to treatment are also significantly different. Therefore, early genetic diagnosis is of great value for improving the clinical management of patients. In terms of treatment, in addition to traditional drugs and surgery, targeted therapy and gene therapy are also gradually moving towards clinical application. Advances in metabolomics, gene editing technology, and structural biology have made it possible to provide personalized and precise treatment of children with hereditary cholestasis in the future; however, this which will put forward higher requirements for on relevant practitioners.

Abstract:Iron overload disorders lead to excess iron deposition in the body, which can occur as a result of genetic or secondary causes. Genetic iron overload, referred to as hereditary hemochromatosis, may present as a common autosomal recessive mutation or as one of several uncommon mutations. Secondary iron overload may result from frequent blood transfusions, exogenous iron intake, or certain hematological diseases such as dyserythropoietic syndrome or chronic hemolytic anemia. Iron overload may be asymptomatic, or may present with significant diseases of the liver, heart, endocrine glands, joints, or other organs. If treated appropriately prior to end-organ damage, life expectancy has been shown to be similar compared to matched populations. Alongside clinical assessment, diagnostic studies involve blood tests, imaging, and in some cases liver biopsy. The mainstay of therapy is periodic phlebotomy, although oral chelation is an option for selected patients.

Abstract:Hemochromatosis (HC) is the main genetic disorder of iron overload and is regarded as metal-related human toxicosis. HC may result from HFE and rare non-HFE gene mutations, causing hepcidin deficiency or, sporadically, hepcidin resistance. This review focuses on HFE-related HC. The illness presents a strong biochemical penetrance, but its prevalence is low. Unfortunately, the majority of patients with HC remain undiagnosed at their disease-curable stage. The main aim of HC management is to prevent iron overload in its early phase and remove excess iron from the body by phlebotomy in its late stage. Raising global awareness of HC among health staff, teaching them how not to overlook early HC manifestations, and paying attention to careful patient monitoring remain critical management strategies for preventing treatment delays, upgrading its efficacy, and improving patient prognosis.

A farewell to phlebotomy-use of placenta-derived drugs Laennec and Porcine for improving hereditary hemochromatosis without phlebotomy: a case report

Abstract:Background: Human hepcidin, produced by hepatocytes, regulates intestinal iron absorption, iron recycling by macrophages, and iron release from hepatic storage. Recent studies indicate that hepcidin deficiency is the underlying cause of the most known form of hereditary hemochromatosis. Case presentation: A 44-year-old Asian man who developed type 2 diabetes mellitus had elevated serum ferritin levels (10,191 ng/mL). Liver biopsy revealed remarkable iron deposition in the hepatocytes and relatively advanced fibrosis (F3). Chromosomal analysis confirmed the presence of transferrin receptor type 2 mutations (c.1100T>G, c.2008_9delAC, hereditary hemochromatosis type 3 analyzed by Kawabata). The patient received intravenous infusions of Laennec (672 mg/day, three times/week) or oral administration with Porcine (3.87 g/day) for 84 months as an alternative to repeated phlebotomy. At the end of the treatment period, serum ferritin level decreased to 428.4 ng/mL (below the baseline level of 536.8 ng/mL). Hemoglobin A1c levels also improved after treatment with the same or lower dose of insulin (8.8% before versus 6.8% after). Plural liver biopsies revealed remarkable improvements in the grade of iron deposition and fibrosis (F3 before versus F1 after) of the liver tissue. Conclusion: The discovery of hepcidin and its role in iron metabolism could lead to novel therapies for hereditary hemochromatosis. Laennec (parenteral) and Porcine (oral), which act as hepcidin inducers, actually improved iron overload in this hereditary hemochromatosis patient, without utilizing sequential phlebotomy. This suggests the possibility of not only improving the prognosis of hereditary hemochromatosis (types 1, 2, and 3) but also ameliorating complications, such as type 2 diabetes, liver fibrosis, and hypogonadism. Laennec and Porcine can completely replace continuous venesection in patients with venesection and may improve other iron-overloading disorders caused by hepcidin deficiency.

Abstract:Based on reviews of the literature and experts' consensus, the Chinese Society of Hepatology developed guidelines for the diagnosis and treatment of liver cirrhosis, in order to improve clinical practice. In addition to what has been covered in previously published guidelines on the management of cirrhosis complications, these guidelines add new sections and provide updates. The guidelines emphasize the early diagnosis of the cause and assessment of complications. Comprehensive treatments including etiological treatment and complication management should be initiated immediately. In addition, regular monitoring, especially surveillance of hepatocellular carcinoma, is crucial for managing patients.

Abstract:BACKGROUND:Primary liver cancer, around 90% are hepatocellular carcinoma in China, is the fourth most common malignancy and the second leading cause of tumor-related death, thereby posing a significant threat to the life and health of the Chinese people.SUMMARY:Since the publication of Guidelines for Diagnosis and Treatment of Primary Liver Cancer (2017 Edition) in 2018, additional high-quality evidence has emerged with relevance to the diagnosis, staging, and treatment of liver cancer in and outside China that requires the guidelines to be updated. The new edition (2019 Edition) was written by more than 70 experts in the field of liver cancer in China. They reflect the real-world situation in China regarding diagnosing and treating liver cancer in recent years.KEY MESSAGES:Most importantly, the new guidelines were endorsed and promulgated by the Bureau of Medical Administration of the National Health Commission of the People's Republic of China in December 2019.

Abstract:The reaction νd → µ − ∆ ++ n is studied in the region of low q 2 to investigate the effect of deuteron structure and width of the ∆ resonance on the differential cross section. The results are used to extract the axial vector N − ∆ coupling C A 5 from the experimental data on this reaction. The possibility to determine this coupling from electroweak interaction experiments with high intensity electron accelerators is discussed.

Abstract:Hämochromatose ist eine der häufigsten genetischen Erkrankungen in Europa, gekennzeichnet durch eine fortschreitende Eisenüberladung. Sie resultiert häufig aus Mutationen im HFE -Gen, besonders der p.C282Y-Variante. Diese Krankheit kann zu schweren Organmanifestationen führen, wenn sie unbehandelt bleibt. Dieser Artikel bietet einen Überblick über Diagnose und Therapie der Hämochromatose. Hemochromatosis is a disorder of genetic origin which affects iron hemostasis, resulting in an increased transferrin saturation, hyperferritinemia and parenchymal iron overload. Recently, a new system for the classification of hemochromatosis has been proposed, wherein patients are separated into 4 groups, based on the disease affected iron regulatory genes. Excess iron and increased transferrin saturation results in the formation of non-transferrin bound iron which leads to tissue damage. Hemochromatosis is a common genetic disease, but screening of the general population is not routinely recommended. In order to provide ideal care for hemochromatosis patients, it is crucial to delineate hemochromatosis from other causes of hyperferritinemia, which is a common finding in patients with metabolic disorders. This article summarizes the diagnostic algorithm for hemochromatosis. Furthermore, recommendations for optimal care – including targets for phlebotomy – are discussed. Die Hämochromatose ist eine Erkrankung genetischen Ursprungs, welche durch die Manifestation einer Eisenüberladung definiert wird. Die Serum-Eisenparameter (Ferritin, Transferrin, Transferrinsättigung) sind die Grundlage jeder Abklärung und identifizieren jene Patienten, welche eine weitere diagnostische Abklärung benötigen. Eine isolierte Hyperferritinämie tritt häufig bei Patienten mit metabolischen Erkrankungen (Diabetes mellitus, steatotische Lebererkrankung) auf und stellt keine Indikation zur Aderlasstherapie dar. Eine p.C282Y-Homozygotie im HFE -Gen bestätigt bei gleichzeitigem Vorliegen einer erhöhten Transferrinsättigung und Ferritinkonzentration die Diagnose der Hämochromatose und sollte mittels Aderlässen behandelt werden. Aderlässe stellen den Gold-Standard der Therapie dar, sind aber für manche Patient:innen nicht geeignet. Alternative Therapiemöglichkeiten existieren, die Indikation dafür sollte jedoch in einem Zentrum geprüft werden. Article published online: 09 October 2024 © 2024. Thieme. All rights reserved.
