Abstract:Rational: How to manage patients with prostate cancer (PCa) with biochemical recurrence (BCR) following primary curative treatment is a controversial issue. Multiple disciplinary team (MDT) mechanism may propose an appropriate treatment plan for patients and can effectively improve patient prognosis and survival, reduce patient diagnosis and treatment waiting time, and greatly improve patient satisfaction. Patient concerns: Here, we presented a case of a 77-year-old man with a persistently elevated serum level of prostate-specific antigen (PSA), who had a history of radical prostatectomy (RP) and of 9 years endocrine therapy. Diagnoses: Castration-resistant prostate cancer and locally recurrent prostate cancer. Interventions: Androgen-deprivation therapy was first utilized 2 months after RP, due to the consideration of BCR on May 5, 2007. And during the next 9 years, he was treated with different endocrine agents but failed to maintain serum levels of PSA stable. Finally, the MDT suggested patient to perform salvage radiation therapy (SRT). Under MDT mechanism, we avoid secondary surgery, so as to reduce the patients' mental suffering and cost of patient care. Outcomes: EPIC26 scale assessment revealed leak-free urine, good urine control, no defecation abnormalities or blood in the stool, no breast tenderness and breast enlargement significantly improved. The patient now has no adjuvant therapy, including endocrine therapy. The patient achieved good prognosis through local RT. Lessons: Pelvic SRT for patients with locally recurrent PCa may restore the same radical effect as RP. And more importantly, MDT mechanism plays an important role in making the most appropriate decisions for patients.
Penile Sparing Surgery for Penile Cancer: A Multicenter International Retrospective Cohort

Abstract:Purpose: We evaluated recurrence outcomes of penile sparing surgery in what is to our knowledge the largest multicenter cohort of patients to date. Materials and Methods: We retrospectively identified patients treated with penile sparing surgery from May 1990 to July 2016 at 5 tertiary referral institutions. Treatments consisted of circumcision, wide local excision, laser therapy with or without local excision, partial or total glansectomy and glans resurfacing. The study primary end point was local recurrence-free survival, defined from initial treatment to time of local recurrence and estimated with the Kaplan-Meier method. Results: After applying study exclusion criteria 1,188 patients were included in analysis. During the median followup of 43.0 months there were 252 local recurrences (21.2%), of which 99 (39.3%) developed in year 1. Median time to local recurrence was 16.3 months and the 5-year local recurrence-free survival incidence was 73.6%. When stratified by stage, the 5-year local recurrence-free survival rate was 75.0%, 71.4% and 75.9% in Ta/Tis, T1 and T2 cases, respectively (log rank p = 0.748). Of the recurrences 58.3% were treated with repeat organ sparing procedures and the secondary partial (total) penectomy rate was 19.0%. Only margin status was significantly associated with local recurrence on multivariate analysis (p = 0.001). Study limitations included the retrospective design and the heterogeneous clinical approach. Conclusions: Penile sparing surgery can provide excellent local control for superficial penile tumors as well as for appropriately selected invasive lesions. Strict followup in the early postoperative period is highly recommended.
Oncological and Functional Outcomes of Penile Shaft Sparing Surgery for Localised Penile Cancer: A Systematic Review

Abstract:Context Penile shaft sparing (PSS) surgery for localised penile cancer (PeCa) aims to balance oncological and functional outcomes. Objective To summarise the published evidence on different PSS approaches. Evidence acquisition We performed a systematic review adhering to Preferred Reporting Items for Systematic Reviews and Meta-analyses guidelines. The systematic search was performed on PubMed, EMBASE, and Scopus databases up to February 14, 2023. The inclusion criteria encompassed retrospective or prospective studies including patients ≥18 yr of age with localised PeCa treated with different PSS approaches, classified as laser ablation, circumcision, wide local excision, glansectomy with or without split skin graft, glans resurfacing, and mixed technique excision. The risk of bias was assessed using the Newcastle-Ottawa scale. A quantitative synthesis was not performed due to anticipated data heterogeneity and a lack of comparative studies. Evidence synthesis Out of 4343 articles identified, 47 met our inclusion criteria, including 10 847 patients. The year of publication ranged between 1983 and 2021. Nine studies were prospective case series, while the remaining studies were retrospective. No comparative studies were identified. Most of the cases included in these studies were Ta and T1. The oncological outcomes were good for all the approaches, with cancer-specific mortality ranging between 0% and 18%. Sexual and cosmetic outcomes, despite being under-reported, were good for all the approaches, with almost all patients being satisfied with their quality of life after surgery. The Newcastle and Ottawa scale revealed a high or severely high risk of bias in all the included studies. Conclusions PSS approaches were safe and had good functional outcomes, considering however the overall low quality of the studies on this issue. Patient summary The perioperative, function, and oncological outcomes of penile shaft sparing approaches are good. However, high-quality studies are needed to determine whether these approaches benefit patients with localised penile cancer.

Abstract:Treatment of penile cancer (PC) focuses on organ preservation, employing various surgical and non-surgical approaches. These interventions may lead to disfigurement, impacting patients' functional outcomes and psychosocial well-being. We reviewed studies related to penile health and PC up to February 2024, limited to studies published in English. Studies employing health-related quality of life (HRQoL) assessments have identified a detrimental association between aggressive treatment and overall health status, physical functioning, and relationships. In contrast, organ-sparing demonstrates improved measures related to HRQoL and sexual function. Assessment through validated questionnaires reveals diverse voiding outcomes, and varying impacts on QoL and sexual activity, emphasizing the necessity for multidisciplinary personalized care. Studies highlight substantial variations in sexual function, with patients reporting adaptations, reduced satisfaction, and concerns about body image and sexual well-being. Furthermore, unmet needs include challenges in patient–clinician communication, obtaining information, and accessing psychosocial support. Patient experiences underscore the importance of timely diagnosis, treatment access, and addressing psychological consequences. Organ-sparing approaches have higher QoL preservation and sexual function. Individualized support, including sexual therapy, support groups, and family counseling, is essential for post-treatment rehabilitation. Timely diagnosis and comprehensive care are paramount in addressing the multifaceted impact of PC on patients and families.

Abstract:Context: Penile cancer is a rare disease but has a significant impact on quality of life. Its incidence is increasing, so it is important to include new and relevant evidence in clinical practice guidelines. Objective: To provide a collaborative guideline that offers worldwide physician and patient guidance for the management of penile cancer. Evidence acquisition: Comprehensive literature searches were performed for each section topic. In addition, three systematic reviews were conducted. Levels of evidence were assessed, and a strength rating for each recommendation was assigned according to the GRADE (Grading of Recommendations, Assessment, Development, and Evaluation) methodology. Evidence synthesis: Penile cancer is a rare disease but its global incidence is increasing. Human papillomavirus (HPV) is the main risk factor for penile cancer and pathology should include an assessment of HPV status. The main aim of primary tumour treatment is complete tumour eradication, which has to be balanced against optimal organ preservation without compromising oncological control. Early detection and treatment of lymph node (LN) metastasis is the main determinant of survival. Surgical LN staging with sentinel node biopsy is recommended for patients with a high-risk (≥pT1b) tumour with cN0 status. While (inguinal) LN dissection remains the standard for node-positive disease, multimodal treatment is needed in patients with advanced disease. Owing to a lack of controlled trials and large series, the levels of evidence and grades of recommendation are low in comparison to those for more common diseases. Conclusions: This collaborative penile cancer guideline provides updated information on the diagnosis and treatment of penile cancer for use in clinical practice. Organ-preserving surgery should be offered for treatment of the primary tumour when feasible. Adequate and timely LN management remains a challenge, especially in advanced disease stages. Referral to centres of expertise is recommended. Patient summary: Penile cancer is a rare disease that significantly impacts quality of life. While the disease can be cured in most cases without lymph node involvement, management of advanced disease remains challenging. Many unmet needs and unanswered questions remain, underlining the importance of research collaborations and centralisation of penile cancer services.
Surgical Management of Penile Carcinoma in Situ: Results from an International Collaborative Study and Review of the Literature

Abstract:Objectives To evaluate recurrence after penile-sparing surgery (PSS) in the management of carcinoma in situ (CIS) of the penis in a large multicentre cohort of patients. Patients and Methods We identified consecutive patients from five major academic centres, treated between June 1986 and November 2014, who underwent PSS for pathologically proven penile CIS. The primary outcome was local recurrence-free survival (RFS), which was estimated using the Kaplan-Meier method. Results A total of 205 patients were identified. Treatment methods included circumcision, glansectomy, wide local excision, laser therapy and total glans resurfacing. Over a median (interquartile range [IQR]) follow-up of 40 (26-65.6) months, there were 48 local recurrences, with 45.8% occurring in the first year and 81.3% occurring by year 5. The majority of recurrences were observed in the laser group (58.3%). The median (IQR) time to local recurrence was 15.9 (5.66-26.14) months. The 1-2- and 5-year RFS rates were 88.4, 85.6 and 75%, respectively, and the median (IQR) RFS time was 106.5 (80.2-132.2) months. Conclusions Among patients with penile CIS selected for surgical management, durable responses at intermediate-to long-term follow-up were noted. For those with glandular CIS, glans resurfacing offered the best outcomes.
Local Therapy and Reconstruction in Penile Cancer: A Review

Abstract:Local therapy for penile cancer provides robust survival and can preserve the penis functionally and cosmetically. Interventions must target the appropriate clinical stage. We reviewed studies regarding the primary therapy in penile cancer, from topical therapy to radical penectomy, and reconstructive techniques. Topical therapy (5-FU or Imiquimod) provides a robust oncologic response in patients with Ta or Tis disease. Multiple laser therapies are available for localized patients and those with low-grade T1 disease. There is a non-trivial risk of progression and nodal metastases in poorly selected patients. Wide local excision provides an oncologically sound option in patient with up to T1 disease; less evidence exists for Mohs microsurgery in the setting of penile cancer. Increasingly aggressive approaches include glansectomy and partial/radical penectomy, which provide 5- and 10-year cancer-specific survival rates of over 80%. Meticulous reconstruction is necessary for the durable function of the remaining penis. Preservation of voiding and sexual function occurs via penile skin grafting, glans resurfacing, creation of a functional penile stump, and phalloplasty with a penile implant. Perineal urethrostomy provides an alternative in pathology demanding extensive partial or radical penectomy, and a durable option for seated voiding. Clinical suspicion and timely diagnosis are paramount in terms of management as less-invasive options for earlier-stage disease develop.

Abstract:Rare tumours such as penile carcinoma have been largely neglected by the urology scientific community in favour of more common — and, therefore, more easily fundable — diseases. Nevertheless, penile cancer represents a rising burden for health-care systems around the world, because a lack of widespread expertise, ineffective centralization of care and absence of research funds have hampered our ability to improve the global care of these patients. Moreover, a dichotomy has arisen in the field of penile cancer, further impeding care: the countries that are mainly supporting research on this topic through the development of epidemiological studies and design of clinical trials are not the countries that have the highest prevalence of the disease. This situation means that randomized controlled trials in developed countries often do not meet the minimum accrual and are intended to close before reaching their end points, whereas trials are almost completely absent in those areas with the highest disease prevalence and probability of successful recruitment, such as Africa, South America and South Asia. The scientific and organizational inaction that arises owing to this mismatch translates into a burdensome cost for our patients. A global effort to gather experts and pull together scientific data from around the world may be the best way to boost clinical research, to change clinical practice and, ultimately, to improve care for patients and their families.

Abstract:Introduction: Penile squamous cell carcinoma (PSCC) is a rare tumor with an aggressive behavior. The Meet-URO 23/I-RARE registry includes rare genitourinary malignancies. We extracted patients with PSCC to conduct a retrospective study aimed at assessing clinical outcomes and prognostic factors. Patients and Methods: Primary endpoints were overall survival and progression-free survival. Prognostic factors for OS and PFS were analyzed using univariate and multivariate analysis. From the Meet-URO 23/I-RARE database, we extracted 128 patients with diagnosis of PSCC. 48% of patients underwent first-line of therapy. Results: In the overall population, median OS from diagnosis was 34.6 months. Significant differences in median OS were observed according to ECOG PS at diagnosis (57.3 months vs 8.3 months; p < 0.001), and median age (≤ 77y 88.8 months vs > 77y 26 months; p = 0.013). At multivariate analysis, ECOG PS 2-4 at diagnosis (HR 3.04) and lymph node metastases (HR 2.49) were independently associated with a higher risk of death. Among patients undergoing first-line therapy (n = 61), median OS was 12.3 months, and a statistically significant difference was found according to type of response to first-line (DCR 24.4 months vs PD 7.1 months; p 77 years was associated with a worse OS (HR 2.16). A statistically significant difference in PFS was found according to platinum plus 5-fluorouracil vs platinum plus taxane (4.9 vs 3.4 months; p = 0.036) and regimens with two vs three drugs (3.4 vs 8.6 months; p = 0.019). At the multivariate analysis only regimens with platinum plus taxane were associated with worse PFS (HR 2.83). Conclusion: In our registry study, PSCC is confirmed to be an aggressive disease. Poor ECOG PS, presence of lymph node metastases, and higher age at diagnosis appear to be associated with worse survival outcomes.

Surgical Treatment of Primary Disease for Penile Squamous Cell Carcinoma: A Surveillance, Epidemiology, and End Results Database Analysis.

Abstract:Current guidelines recommend penile sparing surgery (PSS) for selected penile cancer cases. The present study described the use of PSS in a population-based cohort, and also examined the role of PSS on penile cancer-specific mortality (PCSM). Data from the Surveillance, Epidemiology, and End Results (SEER) database were used to identify individuals that were diagnosed with penile squamous cell carcinoma between 1998 and 2009 and treated with surgery. Patients were sorted into two groups: Local tumor excision (LTE) and partial/total penectomy (PE). Factors associated with the receipt of LTE and PCSM following LTE were examined. In addition, PCSM was compared between LTE and PE following propensity score matching. Of the 1,292 eligible patients, 24.2% underwent LTE. For stage T1 disease, the rates of LTE increased moderately from 29 to 40% over the last decade. Following multivariate analyses, young age, African descent, a tumor size of <3 cm and stage T1 disease were identified to positively influence the receipt of LTE. With a median follow-up period of 55 months, the four-year PCSM rate was 9.8% in patients treated with LTE. Older age, a tumor size of 3-4 cm and regional/distant disease (SEER stage) were significant predictors of PCSM. Furthermore, in matched cohorts with stage T1 disease, the four-year PCSM rates were 8.9 and 10.0% for patients that received LTE or PE, respectively (P=0.93). In conclusion, underuse of PSS is pronounced in the general community with significant age and ethnicity disparities. The current population-based study provides evidence supporting the oncological safety of PSS compared with PE in early-stage disease.
The impact of diagnosis and treatment of penile cancer on intimacy: a qualitative assessment

Abstract:Penile cancer is a rare malignancy (0.5–0.93/100,000 in Western countries) with significant psychosocial and sexual repercussions. This qualitative study explored the impact of penile cancer diagnosis and treatment on intimacy. A convenience sample was identified of 20 potential candidates who were at least 5 months post penile cancer surgery at a hospital centralizing penile cancer care. Participants were recruited by telephone and admitted until data saturation was reached, resulting in a sample of nine men (44–74 years old), none withdrew from participation. All interviews were performed by the same female researcher with no prior relationship to the men. The one-time interviews (35–61 min) followed a semi-structured interview guide, were audio-recorded and transcribed verbatim. Three researchers analysed the data independently using descriptive phenomenological analysis, resulting in a gradually drawn up coding tree mapping out the patient's journey. The central themes that emerged were: (1) Intimate area led to diagnostic delays, intensified diagnosis and induced secrecy; (2) Impact on sexuality prior to surgery; (3) The voyage of sexual re-discovery; (4) A partnered voyage of sexual discovery; (5) Care needs related to intimate area. This study highlights the need for comprehensive and personalized care, including pre-surgical information provision and post-surgical psychosexual support. Addressing the current unmet needs of men with penile cancer requires guidelines for psychosexual interventions and proactive efforts to reduce stigma and to raise awareness.

Variations in Penile Cancer Management: Results from the Global Society of Rare Genitourinary Tumors Survey

Abstract:Given the rarity of penile cancer, the Global Society of Rare Genitourinary Tumors (GSRGT) conducted a survey to identify the global variations in disease presentation and various aspects of its management. The results of this survey help identify research areas for multinational collaborative efforts which is a key mission of the Introduction: There is paucity of evidence and consensus on various aspects of management of penile cancer (PeCa), which is intuitive considering the rarity of this disease. We present here the details of an online survey conducted by the Global Society of Rare Genito-urinary Tumors (GSRGT) with the aim of capturing the variations in PeCa care across different regions of the world. Materials and Methods: An online questionnaire was developed by experts within the GSRGT and then circulated via email in English and Spanish versions to clinicians dealing with PeCa. Respondents were allowed 8 weeks to reply. Results: We received 102 responses; the majority of them were from South America (37.2%) followed by North America and Asia (17.6% each). Only 11.7% of the respondents treated more than 25 patients with PeCa annually. Total penectomy is performed by 21.5% of the respondents in > 50% of their patients. Less than a fifth of the experts (19.6%) responded that > 50% of their patients are clinically node-negative (cN0) at presentation. For intermediate-risk cN0 patients (T1 G2 cancer), about a third of the experts chose surveillance. For invasive inguinal staging, the options of Dynamic Sentinel Lymph Node Biopsy (DSNB), Modified Inguinal Lymph Node Dissection (MILD), Superficial Inguinal Lymph Node Dissection (SILD), and Video-Endoscopic Inguinal Lymphadenectomy (VEIL) were chosen by 28.4%, 26.4%, 31.3%, and 13.7% of the respondents respectively. Considerable variation was seen in the worldwide use of these techniques. For clinically node-positive patients, respondents were in favor of giving adjuvant chemotherapy instead of neoadjuvant chemotherapy, except for cN3 patients. Conclusion: The results of this questionnaire objectified the variations in global practices in the management of PeCa. This serves as the baseline information which can help pr ior itize research areas for multinational collaborative efforts, a key mission of the GSRGT.

In this present study we analyzed the outcomes of patients treated with circumcision, partial/total glansectomy, laser ablation, or wide local excision (WLE) for primary PC. METHODS We included 1260 consecutive patients who underwent PSS at 5 academic institutions from July 2000 to June 2015. Tumor and patient characteristics such as age, tumor size (cm), tumor grade and pathological stage were recorded and analyzed. Kaplan-Meier analysis was performed to evaluate 1 year, 2 year and 5 year recurrence free survival (RFS) among the groups. Multivariable Cox proportional hazards regression was used to assess prognostic factors for RFS and overall survival (OS). RESULTS Glansectomy patients had higher size, grade and pathologic stage tumors at time of surgery (p < 0.05). At a median follow up of 39.9 months, 37.1 % of patients treated with laser ablation had a local recurrence vs. those who underwent glansectomy or WLE alone (7.6 and 19.5%, respectively; p < 0.001). Patients who had laser procedures also had worse 1-yr, 2-yr and 5-yr RFS (74.4, 65.1 and 51%) vs. glansectomy (81.2, 72.4 and 68.8%, respectively) or WLE (80.3, 66.7 and 55.4%, respectively) (p = 0.056). Mutlivariable analysis found tumor grade (HR = 1.5, p <0.001) and corporal invasion (HR = 1.4, p = 0.05) to be significant, independent predictors for disease recurrence. Age was a significant prognosticator for OS (HR = 1.1, p < 0.001). CONCLUSIONS Tumor grade and corporal invasion were found as independent predictors of disease recurrence after penile sparing treatments. Patients treated with laser ablative techniques had worse recurrence outcomes. Radiotherapy Versus Partial Penectomy for T1 Squamous Cell Carcinoma of the Penis

Abstract:Background: Radiotherapy (RT) represents an alternative treatment option for patients with T1 squamous cell carcinoma of the penis (SCCP), with proven feasibility and tolerability. However, it has never been directly compared with partial penectomy (PP) using cancer-specific mortality (CSM) as an end point. Methods: In the Surveillance, Epidemiology, and End Results database (2000-2020), T1N0M0 SCCP patients treated with RT or PP were identified. This study relied on 1:4 propensity score-matching (PSM) for age at diagnosis, tumor stage, and tumor grade. Subsequently, cumulative incidence plots as well as multivariable competing risks regression (CRR) models addressed CSM. Additionally, the study accounted for the confounding effect of other-cause mortality (OCM). Results: Of 895 patients with T1N0M0 SCCP, 55 (6.1%) underwent RT and 840 (93.9%) underwent PP. The RT and PP patients had a similar age distribution (median age, 70 vs 70 years) and more frequently harbored grade I or II tumors (67.3% vs 75.8%) as well as T1a-stage disease (67.3% vs 74.3%). After 1:4 PSM, 55 (100%) of the 55 RT patients versus 220 (26.2%) of the 840 PP patients were included in the study. The 10-year CSM derived from the cumulative incidence plots was 25.4% for RT and 14.4% for PP. In the multivariable CRR models, RT independently predicted a higher CSM than PP (hazard ratio, 1.99; 95% confidence interval, 1.05-3.80; p = 0.04). Conclusion: For the T1N0M0 SCCP patients treated in the community, RT was associated with nearly a twofold higher CSM than PP. Ideally, a validation study based on tertiary care institution data should be conducted to test whether this CSM disadvantage is operational only in the community or not.

Abstract:Surgery is the cornerstone of treatment for penile squamous cell carcinoma. Following surgical excision, reconstructive surgery is beneficial to restore aesthetics, functionality, and overall quality of life of these patients. In this mini-review, we discuss the use of skin grafts, perineal urethrostomy, phalloplasty, and vascularised flaps as reconstructive options following penile cancer treatment. Illustrated by videos, we highlight the surgical approach, indications, complications, and outcomes of these reconstructive strategies. Patient summary Reconstructive surgery is important to restore appearance, urinary function, and sexual function in patients who have been treated for penile cancer. We discuss the benefits and potential complications of various surgical reconstructive options, which are illustrated with videos.

Abstract:Penile cancer (PC) mainly affects lower socioeconomic groups and those with less education. Patients who develop PC often experience symptoms for months before consult a urologist. The delay of the treatment affects the severity of the prognosis. Penectomy is still the best treatment available. This study aims to understand the meaning of the experience of patients who had PC and underwent partial or total penectomy treatment. Patients were approached and invited to participate. Upon acceptance, the nature of the research, objectives and all ethical precautions were explained. All patients were seen by the same psychologist. The technique used was the open interview. We interviewed 7 patients who had and treated penile cancer with partial or total penectomy. Data were collected through recording. Data processing followed the following steps: literal transcription of the interviews; description of interviews in formal language; first reduction seeking the meaning of the reported experiences; listing of the units of meaning of each of the interviews collected. The next step was to specify what is common in the interviewees stories in relation to their lived experience. It is possible to see that CP is a debilitating experience for those who live it. QoL is affected, as is the experience of masculinity itself. It is a very uncomfortable experience, both before and after the treatment. The patient with CP lives with the symptoms for a long time before seeking a solution. It is concluded that the PC has multiple variables, and the number of reports that talk about how difficult it was to reach at the diagnosis, not only due to the patient's own denial of symptoms, but also due to the inefficient care received in countless consultations, brings the reflection on the quality of knowledge of the professionals who provide assistance to this population in relation to PC. Abstract:When feasible from an oncologic standpoint, partial penectomy (PP) is often preferred to total penectomy (TP) for penile cancer treatment, for the preservation of functional urinary outcomes. However, to date, there has not been a direct comparison of perioperative outcomes between PP and TP. Comparing treatments for penile cancer has proven difficult due to the rarity of penile cancer in the United States. We aimed to report differences in pre-operative risk factors, intra-operative outcomes, and postoperative outcomes between TP and PP for penile cancer. Using the National Surgical Quality Improvement Program database, we conducted a retrospective cohort review of penile cancer patients enlisted in the database between the years 2006–2016 using the International Classification of Diseases clinical modification 9th revision codes. A total of 260 patients, 67 TP and 193 PP patients, were included. PP patients were less likely to be transferred patients ( p = 0.002), diabetic ( p = 0.026), and were more likely to have preoperative laboratory values within normal limits. PP patients also had shorter lengths of stay in the hospital ( p < 0.001) and operating time ( p < 0.001). Significant differences were also found for inpatient stay ( p < 0.001), 30-day post-surgery complications ( p < 0.001), deep incisional surgical site infection (SSI) ( p = 0.017), wound disruption ( p = 0.017), intraoperative or postoperative transfusion ( p = 0.029), and sepsis ( p < 0.005). Finally, PP patients required fewer concurrent surgical procedures ( p < 0.001). Demographic differences between PP and TP patients may reflect patients presenting with more advanced oncologic disease. PP is associated with fewer postoperative complications, shorter surgeries, shorter hospital stays, fewer concurrent surgical procedures, and comorbid conditions compared to TP. A gap remains in the reported data pertaining to postoperative sexual function and erectile outcomes for PP at a national level.

Abstract:The first-line treatment of penile cancer confined to the glans (Tis-T2) is based on organ-sparing approaches. Our aim is to report functional outcomes of total glans resurfacing (TGR), wide local excision (WLE) and glansectomy. A retrospective analysis was conducted from January 2013 to October 2022. Ninety-nine patients were enrolled (22 TGR, 29 WLE, and 48 glansectomy). Sexual and urinary outcomes were explored using ad hoc and validated questionnaires (IIEF-15 and IPSS). The mean follow-up was 25.28 ± 24.87 months [95% CI: 20.38; 30.18]. 44 patients (12 TGR, 10 WLE, and 22 glansectomy) were assessed for functional outcomes. Overall, 86.36% of patients were satisfied with the surgery. The mean IIEF-15 score pre-operation was 54.91 ± 21.38 [95% CI: 48.41–61.41], and at 12 months post-operation, it was 44.39 ± 23.01 [95% CI: 37.39–51.39], with the change being statistically significant (mean difference: –10.52, (–19.15), p < 0.001). During the 0–12-month interval, IIEF-15 scores decreased across all techniques. Glansectomy and WLE showed significant decreases (Glansectomy: –12.955, –24.14%, [95% CI: –21.52, –4.38], p = 0.002; WLE: –14.1, –22.92%, [95% CI: –26.8, –1.39], p = 0.025 respectively), whereas TGR experienced a non-significant decrease (–3.083, –5.97%, CI: [–14.68, 8.51], p = 1.0). Concerning urinary function, only 18.18% of overall patients reported a negative impact of surgery. At 12-months, patients returned almost to pre-intervention IPSS values. Organ-sparing surgery guarantees a decent preservation of both erectile and voiding functions. TGR seems to provide better sexual outcomes when compared to other organ sparing approaches.

Abstract:Penile cancer is a rare malignancy in the western world, and is increasing in incidence [1,2,3]. Most primary and secondary care clinicians rarely manage this condition except in countries where such conditions and their management are centralised to subspecialist centres e.g. in the UK. The article by Vasqualez et al. [4] outlining the 30 day complications of Total Penectomy (TP) and Partial Penectomy (PP) is welcome. The authors utilise retrospective and historic data over a 10 year period from the NSQIP (National Surgical Quality Improvement Programme) database with data from various centres and surgeons' outcomes. To-date there hasn't been any data on direct comparison of these two surgeries and expected postoperative outcomes. Vasqualez et al. showed that TP is a more invasive procedure and has a greater risk of complications within 30 days of surgery as compared to PP [4]. Like most surgery involving malignancies, goals of oncological outcomes usually are paramount to those of functional outcomes. Survival is dictated largely by stage, with early detection and management, especially of recurrent disease or metastatic groyne disease being key to better survival.

Abstract:OBJECTIVE To evaluate the complications and outcomes of organ-sparing surgery in penile cancer. METHODS Thirty-two penile cancer patients had undergone organ-sparing surgery between 2006 and 2010 in Fudan University Shanghai Cancer Center. Applied surgical techniques included radical circumcision and/or wide local excision. Intraoperative frozen section analyses of circumference and deep margins were performed to guarantee complete removal of cancerous tissue. Complications and oncological and functional outcomes were recorded prospectively. Bilateral inguinal lymphadenectomy was routinely performed in patients with invasive penile cancer. RESULTS Of 32 patients, 8 underwent radical circumcision, 18 were treated with wide local excision, and 6 received wide local excision and circumcision. Postoperatively, only 3 (9.4%) patients had minor complications. With a median follow-up of 26.5 months, local control was achieved in 29 (90.6%) patients, and 3 patients (9.4%) with positive lymph nodes died of disseminated disease. Of 29 patients who completed follow-up surveys of functional outcome, only 1 (4.5%) claimed decreased sexual function and all reported satisfied urination. CONCLUSION Organ-sparing surgery, such as radical circumcision and wide local excision is an appropriate treatment option for selected penile cancer patients.
