Abstract:Purpose : The aim of this study was to evaluate the performance of healthy Chilean children aged 3–10 years in the 6-minute walking test (6MWT) and cardiometabolic risk variables and to determine sex- and age-specific reference values. Methods : This study involved 1165 healthy children (age = 6.36 [1.70] y old). The 6MWT was used to evaluate exercise performance. Furthermore, anthropometric measures were collected, like weight, height, body mass index, waist circumference, and skin folds. Resting heart rate and blood pressure (BP) were also evaluated. Results : The prevalence of overweight and obesity was 35.0% and 25.4% in preschoolers and 29.0% and 36.2% in school-age children, respectively, showing significant differences ( P < .05) between age groups. The distance walked (6-min walk distance) increased significantly year on year at ages from 3 to 10 years. According to the regression analysis, 6MWT performance was positively related to age, systolic BP, and height, whereas it was negatively related to ∑4 skinfold fat, resting heart rate, diastolic BP, and waist circumference. Conclusions : This study provides a reference equation and an age- and sex-adjusted percentile curve to assess the predicted 6MWT performance in a cohort of prepubertal Chilean children. The 6-minute walk distance depends mainly on age; however, other variables, such as resting heart rate, BP, skinfold fat, and waist circumference, add significant information and should be taken into account.

Abstract:The aim of this study is to examine the available scientific evidence on the reliability and criterion validity of 6-minute run walk field-based test when administered to children and adolescents. Systematic searches were performed in three electronic databases (MEDLINE/PubMed, SPORTDiscuss and Scopus) from their inception until February 2024, with an update on August 15, 2024. Studies were considered eligible if they provided information on the criterion-related validity and/or reliability of the 6-minute run walk test when conducted by children (aged 12 years and under) and/or adolescents (aged 13 to 17 years). To examine reliability and validity, two separate meta-analyses were implemented. In the meta-analysis focused on criterion-related validity, the five studies included indicated a fair validity ( r = 0.56 [95% CI: 0.44–0.66]) and high homogeneity (I 2 0%). In the meta-analysis focused on reliability, the pooled data from the five studies employing Pearson's correlation coefficient ( n = 585) revealed strong reliability values ( r = 0.82 [95% CI: 0.75–0.87]), albeit with a moderate level of heterogeneity (I 2 71% prediction interval 0.55–0.94). The evidence suggests that the 6-minute run walk field-based test demonstrated fair criterion-related validity and strong test-retest reliability when conducted with children and adolescent.

Abstract:Aims The 6-min walk test is an inexpensive, safe, and easy tool to assess functional capacity in patients with cardiopulmonary diseases including heart failure (HF). There is a lack of reference values, which are a prerequisite for the interpretation of test results in patients. Furthermore, determinants independent of the respective disease need to be considered when interpreting the 6-min walk distance (6MWD). Methods The prospective Characteristics and Course of Heart Failure Stages A-B and Determinants of Progression (STAAB) cohort study investigates a representative sample of residents of the City of Würzburg, Germany, aged 30 to 79 years, without a history of HF. Participants underwent detailed clinical and echocardiographic phenotyping as well as a standardized assessment of the 6MWD using a 15-m hallway. Results In a sample of 2762 participants (51% women, mean age 58 ± 11 years), we identified age and height, but not sex, as determinants of the 6MWD. While a worse metabolic profile showed a negative association with the 6MWD, a better systolic and diastolic function showed a positive association with 6MWD. From a subgroup of 681 individuals without any cardiovascular risk factors (60% women, mean age 52 ± 10 years), we computed age- and height-specific reference percentiles. Conclusion In a representative sample of the general population free from HF, we identified determinants of the 6MWD implying objective physical fitness associated with metabolic health as well as with cardiac structure and function. Furthermore, we derived reference percentiles applicable when using a 15-m hallway.

Abstract:Objectives: We aimed to assess the validity of the six-minute walk test (6MWT) to reflect the functional capacity of children with congenital heart disease (CHD), evaluate a possible correlation between the 6MWT distance with cardiopulmonary exercise test (CPET) variables, as well as to find a cutoff value to stratification the physical fitness in this population. Methods: We enrolled 459 children with CHD, 6–18 years old, who performed a complete CPET and 6MWT on the same day in a cross-sectional observational study. Correlations between variables of CPET and six-minute walking distance (6MWD) were analyzed and cutoff values of 6MWD were identified for the classification of the physical fitness in the population. Results: The mean distance ambulated during the 6MWT was 578 ± 65 m, 590 ± 65 m for boys, and 562 ± 62 m for girls ( p < 0.001). Both VO 2max and %predicted VO 2max showed a correlation with the 6MWT distance ( r = 0.35, p < 0.001 and r = 0.51, p < 0.001, respectively), and an inverse correlation was found between VE/VCO2 slope and the 6MWT distance ( r = −0.31; p < 0.001). There appeared to be a linear association between 6MWD and VO 2max up to a 6MWD of approximately 600 m. We divided the population into 4 subgroups (boys <130 cm; boys ≥130 cm; girls <130 cm; girls ≥130 cm), and get the cutoff values (554 m, 617 m, 549 m, 587 m) respectively equivalent to 80% of predicted VO 2max . The 6MWT distances of another 102 patients were applied for external verification of the cutoff values. Conclusions: Our study provided evidence on when a 6MWT should be considered as a convincing complementary test in the pediatric population with CHD and explored the classification of exercise tolerance using a 6MWD value. The cut-off values for 6MWD may be qualified as an intervention target for exercise rehabilitation.

Abstract:Objective: There is evidence that indicates that the Walked Distance (WD) in the 6-Minute Walk Test (6MWT) would be sensitive to the type of track and encouragement. The aim of study was compared the impact of track type and verbal encouragement provided in the 6MWT on WD, physiological cost, perceived exertion, and gait efficiency in healthy young adults unfamiliar with the test. Method: WD, heart rate, subjective sensation of dyspnea (SSD), and fatigue (SSF) were measured in four 6MWT protocols: i) 30 m linear track and protocolized encouragement (LT + PE), ii) 30 m linear track and constant encouragement (LT + CE), iii) 81 m elliptical track and protocolized encouragement (ET + PE), and iv) 81 m elliptical track and constant encouragement (ET + CE). In addition, the Gait Efficiency Index (GIE) associated with physiological cost, dyspnea and fatigue was calculated and compared between the different protocols. Results: The WD was significantly higher in the ET + CE protocol. The percentage of the heart rate reserve used (%HRRu) at minute 6 was higher in the ET + CE protocol. The SSD and SSD had difference in startup time between the protocols. The GEI was higher in %HRRu, SSD, and SSF for the ET + CE protocol. Conclusion: The ET + CE protocol showed a significant increase in WD during the 6MWT in healthy young adults. Although it obtained the highest physiological cost, it did not present perceptual differences when entering cardiopulmonary assessment windows relevant to a more efficient test for the participant. It is advisable to discuss, based on the findings, the fundamental objective of the 6MWT and national and international recommendations to achieve a result as close as possible to the real maximal effort.

Abstract:Physical activity plays a pivotal role in preventing obesity and cardiovascular risks. The six-minute walk test (6MWT) is a tool to assess functional capacity and predict cardiovascular events. The aim of this cross-sectional study was to compare the performance and haemodynamic parameters before and after a 6MWT between obese/overweight vs. normal-weight children (average age 8.7 ± 0.7 years) participating in a project involving four primary schools in South Verona (Italy). Validated questionnaires for physical activity and diet, as well as blood drops, were collected. Overweight or obese children (OW&OB n = 100) covered a shorter 6MWT distance compared to normal-weight children (NW, n = 194). At the test's conclusion, the OW&OB group exhibited a higher Rate Pulse Product (RPP = Systolic Blood Pressure × Heart Rate) as compared to the NW. Body Mass Index, waist-to-height ratio, fat mass by electrical impedance, and trans fatty acids showed direct correlations with pre and post-test haemodynamic parameters, such as RPP, and inverse correlations with oxygen saturation. OW&OB children demonstrated lower performance in this low-intensity exercise test, along with an elevated haemodynamic response. Excess fat in childhood can be considered a risk factor for haemodynamic stress, with potential deleterious consequences later in life. Efforts should be initiated early to break this cycle.

Abstract:Objectives To establish age-specific and sex-specific reference values and equations for the 1-minute sit-to-stand (1MSTS) and 5 times sit-to-stand (5TSTS) tests for Portuguese adults. Design Cross-sectional study. Descriptive statistics were explored to compute reference values and reference equations were established with a forward stepwise multiple regression. Setting Community. Participants In total, 546 adult volunteers without disabilities [age range 18 to 95 years; 58% female] were recruited. Main outcome measures Data on age, sex, height, weight, body mass index (BMI), smoking status and physical activity were collected using a structured questionnaire developed specifically for this study. Participants performed three repetitions of the 1MSTS and 5TSTS and the best repetition was used for analysis. Results Normative values were provided by sex for each age decade. Reference equations were: 1MSTS= 61.53 – (0.34 x age) – (3.57 x sex) – (0.33 x BMI), r 2 =26%; and 5TSTS=3.89 + (0.10 x age) - (0.96 x physical activity), r 2 =27%. Conclusions The proposed reference values and equations will help to interpret the results of functional capacity obtained from healthy or diseased adult populations.

Abstract:Background Submaximal field walking tests are easy to apply and low cost, but it is necessary to standardize their application, especially in the pediatric population. The feasibility and its use in patients with congenital heart disease (CHD) have been studied. Objectives To verify the submaximal field walking tests applied in the cardiopulmonary evaluation of children and adolescents with CHD. Methods Literature systematic review, the search for scientific articles was carried out in the electronic databases Medical Literature Analysis and Retrievel System Online (MEDLINE via PubMed), Latin American and Caribbean Literature in Health Sciences (LILACS), Cochrane-Library, Physiotherapy Evidence Database (PEDro), Scientific Electronic Library Online (SciELO) and ScienceDirect, structured in PICO format, without date restrictions. For the search strategy, words from the Medical Subject Heading Terms (MeSH) dictionary were used with the following descriptors: [(“Congenital Heart Defects” OR “Congenital Heart Defect” OR “Malformation of Heart” OR “Heart Abnormality” OR “Congenital Disorders” OR “Neonatal Diseases and Abnormalities” OR “Tetralogy of Fallot” OR “Tricuspid Atresia” OR “Ebstein Anomaly” OR “Ebstein's Malformation” OR “Birth Defects” OR “Congenital Abnormalities”) AND (“Walk Test” OR “6 -min Walk Test” OR “6-minute Walk Test” OR “Six-minute Walk Test” OR “Endurance Shuttle Walk Test”)], which were later adapted to the other bases that were used in this review. Looking for studies that used submaximal field walking tests in children and adolescents with congenital heart disease aged 5 to 18 years. Methodological quality, effectiveness and safety and risk of bias were assessed. Results Five studies met the eligibility criteria with a sample of 160 individuals with CHD, and all used the six-minute walk test (https://www.physio-pedia.com/Six_Minute_Walk_Test_/_6_Minute_Walk_Test6MWT). Note that different methodologies and modifications are used. The only clinical trial showed good methodological quality. Four studies had a low risk of bias, and one had a moderate risk. Conclusion In this review, the 6MWT proved to be the first-choice method for assessing exercise capacity in children and adolescents with CHD, however, the lack of standardization in the application of the test became evident, which made it difficult to compare the results. Implications Reducing the limitations and heterogeneity in the application of the test will enable more concrete outcomes and facilitate their reproduction in clinical practice.

Heart Rate Response Predicts 6-Minutes Walking Distance in Pulmonary Arterial Hypertension

Abstract:Because the 6-minute walking test (6MWT) is a self-paced submaximal test, the 6-minute walking distance (6MWD) is substantially influenced by individual effort level and physical condition, which is difficult to quantify. We aimed to explore the optimal indicator reflecting the perceived effort level during 6MWT. We prospectively enrolled 76 patients with pulmonary arterial hypertension and 152 healthy participants; they performed 2 6MWTs at 2 different speeds: (1) at leisurely speed, as performed in daily life without extra effort (leisure 6MWT) and (2) an increased walking speed, walking as the guideline indicated (standard 6MWT). The factors associated with 6MWD during standard 6MWT were investigated using a multiple linear regression analysis. The heart rate (HR) and Borg score increased and oxygen saturation (S p O 2 ) decreased after walking in 2 6MWTs in both groups (all p <0.001). The ratio of difference in HR before and after each test (ΔHR) to HR before walking (HR at rest ) and the difference in SpO 2 (ΔSpO 2 ) and Borg (ΔBorg) before and after each test were all significantly higher in both groups after standard 6MWT than after leisure 6MWT (all p <0.001). Multiple linear regression analysis revealed that ΔHR/HR at rest was an independent predictor of 6MWD during standard 6MWT in both groups (both p <0.001, adjusted R 2 = 0.737 and 0.49, respectively). 6MWD and ΔHR/HR at rest were significantly lower in patients than in healthy participants (both p <0.001) and in patients with cardiac functional class III than in patients with class I/II (both p <0.001). In conclusion, ΔHR/HR at rest is a good reflector of combined physical and effort factors. HR response should be incorporated into 6MWD to better assess a participant's exercise capacity.

Abstract:The 6-min walking distance (6MWD) test is used in clinical practice and research into patients with chronic obstructive pulmonary disease (COPD). However, little is known about natural long-term change in this parameter. The 6MWD was measured at baseline and then annually for 5 yrs in 294 patients with COPD and its annual rate of decline was determined. Forced expiratory volume in one second (FEV1) was also measured and the relationship between changes in both markers was explored. At baseline, the median 6MWD was 380 m (range 160-600 m). It declined by 19% (16 m.yr(-1)) over the 5 yrs compared with baseline in patients with American Thoracic Society/European Respiratory Society stage III COPD (FEV1 30-50% predicted) and by 26% (15 m.yr(-1)) in patients with stage IV COPD (FEV1 <30% pred). Over the 5-yr follow-up, the proportion of patients with a minimal clinically significant decline of 54 m increased with the severity of the disease. It was 24% in stage II, 45% in stage III, and 63% in stage IV disease. In contrast, the rate of decline of FEV1 was greater in patients with milder airflow obstruction and lesser in patients with lower absolute FEV1 values. In conclusion, the 6-min walking distance test provides increasingly useful information as the severity of chronic obstructive pulmonary disease increases.

Abstract:Aims The aim was to evaluate whether standardised exercise performance during the incremental shuttle walk test (ISWT) can be used to assess disease severity in children and young people (CYP) with chronic conditions, through (1) identifying the most appropriate paediatric normative reference equation for the ISWT, (2) assessing how well CYP with haemophilia and cystic fibrosis (CF) perform against the values predicted by the best fit reference equation and (3) evaluating the association between standardised ISWT performance and disease severity. Methods A cross-sectional analysis was carried out using existing data from two independent studies (2018–2019) at paediatric hospitals in London,UK. CYP with haemophilia (n=35) and CF (n=134) aged 5–18 years were included. Published reference equations for standardising ISWT were evaluated through a comparison of populations, and Bland-Altman analysis was used to assess the level of agreement between distances predicted by each equation. Associations between ISWT and disease severity were assessed with linear regression. Results Three relevant reference equations were identified for the ISWT that standardised performance based on age, sex and body mass index (Vardhan, Lanza, Pinho). A systematic proportional bias of standardised ISWT was observed in all equations, most pronounced with Vardhan and Lanza; the male Pinho equation was identified as most appropriate. On average, CYP with CF and haemophilia performed worse than predicted by the Pihno equation, although the range was wide. Standardised ISWT, and not ISWT distance alone, was significantly associated with forced expiratory volume in 1 s in CYP with CF. Standardised ISWT in CYP with haemophilia was slightly associated with haemophilia joint health score, but this was not significant. Conclusions ISWT performance may be useful in a clinic to identify those with worsening disease, but only when performance is standardised against a healthy reference population. The development of validated global reference equations is necessary for more robust assessment.

Abstract:Objective: To establish reference values for the 6-minute walk test (6MWT) and 2-minute walk test (2MWT) distances, to investigate the correlation between these 2 tests, and to establish prediction equations for these distances in healthy populations of Belgium and Vietnam. Design: Cross-sectional study. Subjects and methods: The 6MWT and 2MWT were administered to a convenience sample of 239 Belgian and 303 Vietnamese participants between the ages of 18 and 80 years. Results: The mean (standard deviation; SD) 2MWT distances were 215 (32.8) m for Belgian participants and 156 (25.5) m for Vietnamese participants. The mean (SD) 6MWT distances were 625 (90.7) m for Belgian participants and 449 (70.4) m for Vietnamese participants. The Pearson correlation coefficient between the 2 tests was 0.901 (p < 0.001) for Belgian participants and 0.871 (p < 0.001) for Vietnamese participants. Age and sex were the 2 most important predictors of walking distance, followed by body mass index for Belgium and height for Vietnam. The adjusted R2 ranged from 0.31 to 0.49 across 4 predictive equations. Conclusion: These results can be used to determine the presence of walking performance deficits and to guide future studies. The 2MWT is suggested as a useful and convenient alternative to the 6MWT for assessing walking performance in clinical practice.

Abstract:Background: Nordic walking (NW) is a physical sports activity that has been sufficiently studied from the point of view of health, but physiological and performance analyses have not been so much. Objectives: With this study, we intend to analyse the physical work areas, according to ventilatory thresholds, that occur during a NW competition. Methods: Four participants of different characteristics anthropometrics (weight 57.6-85.6 kg; height 165.8-178 cm; and fat percentage 14.5-21.5%) gender (3 males and 1 female) and age (15-57 years) who participated in the NW regional championship have been chosen, and their electrocardiographic tracing was recorded using a NUUBO® device throughout the race, obtaining average and maximum heart rates (HR) in eight sections of the circuit. Previously, in the laboratory, a maximal stress test was performed to determine the maximum oxygen consumption (VO2max), the first (VT1) and second (VT2) ventilatory threshold (VT). With these data, four work areas were obtained. Results: Most of the sections of the circuit were conducted with average HRs in zone 2a (above average between VT1 and VT2 but below VT2) and peak HRs in zone 3 (between VT2 and VO2max). Conclusions: We conclude that, with the data collected on HR, VO2max, and VT, the training zones obtained can be related to the heart rates in the different sections of the circuit. This can be used to improve the sports performance of the walkers.

Abstract:The giant panda (Ailuropoda melanoleuca David, 1869) is an iconic species for global conservation, yet field research has only recently advanced to the point where adaptive management is possible. Here, we review recent developments in giant panda conservation science and propose a strategic plan for moving panda conservation forward. Because of scientific, funding, political, and logistical hurdles, few endangered species management programs have embraced adaptive management, wherein management decisions are shaped iteratively by targeted scientific research. Specific threats, such as habitat destruction, anthropogenic disturbance and fragmented nonviable populations, need to be addressed simultaneously by researchers, managers and policy-makers working in concert to understand and overcome these obstacles to species recovery. With the backing of the Chinese Government and the conservation community, the giant panda can become a high-profile test species for this much touted, but rarely implemented, approach to conservation management.

Abstract:Background: Cardiorespiratory fitness (CRF) is an important component of overall physical fitness and is associated with numerous health benefits, including a reduced risk of heart disease, diabetes, and obesity. However, direct measurement of CRF is time-consuming and therefore not feasible for screening purposes. Methods: A maximal treadmill exercise test with the Bruce protocol was performed to estimate VO2max in 1047 Spanish men and women aged 17 to 62 years. Weight, height, and heart rate were measured. Leisure-time physical activity (LTPA) was recorded using the Minnesota Leisure Time Physical Activity Questionnaire. A multiple linear regression model was developed to predict exercise-based VO2max. The validity of the model was examined by correlation, concordance, Bland–Altman analysis, cross-validation, and construct validity analysis. Results: There was no significant difference between VO2max obtained by the Bruce protocol (43.56 mL/kg/min) or predicted by the equation (43.59 mL/kg/min), with R2 of 0.57, and a standard error of the estimate of 7.59 mL/kg/min. Pearson's product–moment correlation and Lin's concordance correlation between measured and predicted CRF values were 0.75 and 0.72, respectively. Bland–Altman analysis revealed a significant proportional bias of non-exercise eCRF, overestimating unfit and underestimating highly fit individuals. However, 64.3% of participants were correctly classified into CRF tertile categories, with an important 69.9% in the unfit category. Conclusions: The eCRF equation was associated with several cardiovascular risk factors in the anticipated directions, indicating good construct validity. In conclusion, the non-exercise eCRF showed a reasonable validity to estimate true VO2max, and it may be a useful tool for screening CRF.

Abstract:Predictive equations are widely employed for estimating the volume of oxygen consumption (VO2) while walking, which is ultimately employed to determine energy expenditure and tailor exercise prescription. This study aimed to test the agreement between the measured VO2 and estimated VO2 during a walking protocol on a treadmill at 3.5, 4.5, 5.5, and 6.5 km/h. Thirty‐eight young adults (50% women) participated in this cross‐sectional study. The Omnical (Maastricht Instruments, Maastricht, The Netherlands) and K5 (Cosmed, Rome, Italy) metabolic systems were used to measure VO2. To determine the predictive equations, a comprehensive literature search was conducted using the MEDLINE database from May 2022 to July 2023. Seven predictive equations were found and included for estimating VO2 values. We calculated the mean bias (mean difference between measured VO2 and estimated VO2) obtained at each speed using one‐sample t‐tests. We compared the VO2 measured and estimated values using repeated measures analysis of variance and the Bland–Altman method. One‐sample t‐tests showed that all score errors were different from zero (ranging from 1.1 to 5.4 mL/kg/min). Thus, no predictive equation estimated similar VO2 values in comparison with the Omnical and K5 metabolic systems at all intensities. However, the Weyand equation showed the lowest bias across all intensities (score error of 1.1 mL/kg/min). This study showed a lack of agreement between the Omnical and K5 systems compared to diverse predictive equations specially designed to estimate VO2 during walking. Nevertheless, based on our results, the Weyand equation should be the preferred option.

Abstract:Introduction: Exercise testing and interpretation is important for determining exercise capacity and identifying factors which might limit the exercise performance. Impaired cardiorespiratory fitness and sedentary lifestyle are hallmarks of obesity and thus assessing the VO2 peak in such a population is important. While both the SMST and 6MWT are commonly used functional tests, limited research directly compares their effectiveness in Class 1 obese adults. Addressing this gap in the literature can provide valuable insights into the strengths and limitations of each test, guiding researchers and clinicians in selecting the most appropriate assessment tool for this specific population. Identifying the test that poses lower risks and discomfort for obese individuals is essential to ensure their safety during exercise testing. This observational study aimed to compare the Six Minute Step Test (6MST) and the Six Minute Walk Test (6MWT) in measuring VO2 peak in obese Class 1 adults. Method: This Observational study, with convenient sampling and sample size 66, included adults between age 18- 25 within the category of Obese Class 1 (BMI 30-34.9 kg/m2). The exclusion criteria was any Cardio-vascular and respiratory conditions, recent fractures or diagnosed case of any neurological condition. The participants performed both the 6 Minute Walk Test and 6 Minute Step Test in random order and the heart rate and SpO2 were recorded pre and post-test, and VO2 peak was calculated by the indirect method. Result: Results were calculated using paired t-test and significant statistical difference was found between the 6 Minute Walk test and 6 Minute Step Test. Specifically, both VO2 peak and heart rate were significantly different between the tests, with 6MST yielding higher values. Conclusion: Consequently, this study suggests that 6MST cannot substitute for 6MWT in assessing aerobic capacity in Class 1 obese adults. The implications of these findings underscore the importance of considering specific physiological demands and practical constraints when selecting assessment tools for this population. Step by Step: Evaluation of Cardiorespiratory Fitness in Healthy Children, Young Adults, and Patients with Congenital Heart Disease Using a Simple Standardized Stair Climbing Test

Abstract:(1) Background: Cardiorespiratory fitness (CRF) is known to be a prognostic factor regarding long-term morbidity and mortality. This study aimed to develop a standardized Stair Climbing Test (SCT) with a reliable correlation to spiroergometry and the 6MWT which can be used in healthy children as well as patients with congenital heart disease (CHD) and a restricted exercise capacity. (2) Methods: A total of 28 healthy participants aged 10–18 years were included. We tested the individuals’ CRF by cardiopulmonary exercise testing (CPET) on a treadmill, the 6MWT, and a newly developed Stair Climbing Test (SCT). For the SCT, we defined a standardized SCT protocol with a total height of 13.14 m to achieve maximal exercise effects while recording time and vital parameters. To compare the SCT, the 6 Min Walking Test, and CPET, we introduced an SCT-Index that included patient data (weight, height) and time. To assess the SCT’s feasibility for clinical practice, we also tested our protocol with five adolescents with complex congenital heart disease (i.e., Fontan circulation). (3) Results: A strong correlation was observed between SCT-Index and O2 pulse (r = 0.921; p < 0.001). In addition, when comparing the time achieved during SCT (tSCT) with VO2max (mL/min/kg) and VO2max (mL/min), strong correlations were found (r = −0.672; p < 0.001 and r = −0.764; p < 0.001). Finally, we determined a very strong correlation between SCT-Index and VO2max (mL/min) (r = 0.927; p = <0.001). When comparing the 6MWD to tSCT, there was a moderate correlation (r = −0.544; p = 0.003). It appears to be feasible in patients with Fontan circulation. (4) Conclusions: We were able to demonstrate that there is a significant correlation between our standardized SCT and treadmill CPET. Therefore, we can say that the SCT can be used as an easy supplement to CPET and in certain contexts, it can also be used as a screening tool when CPET is not available. The advantages would be that the SCT is a simple, quick, cost-effective, and reliable standardized (sub)maximal exercise test to evaluate CRF in healthy children on a routine basis. We can even assume that it can be used in patients with congenital heart disease.

Abstract:Background Submaximal field walking tests are easy to apply and low cost, but it is necessary to standardize their application, especially in the pediatric population. The feasibility and its use in patients with congenital heart disease (CHD) have been studied. Objectives To verify the submaximal field walking tests applied in the cardiopulmonary evaluation of children and adolescents with CHD. Methods Literature systematic review, the search for scientific articles was carried out in the electronic databases Medical Literature Analysis and Retrievel System Online (MEDLINE via PubMed), Latin American and Caribbean Literature in Health Sciences (LILACS), Cochrane-Library, Physiotherapy Evidence Database (PEDro), Scientific Electronic Library Online (SciELO) and ScienceDirect, structured in PICO format, without date restrictions. For the search strategy, words from the Medical Subject Heading Terms (MeSH) dictionary were used with the following descriptors: [(“Congenital Heart Defects” OR “Congenital Heart Defect” OR “Malformation of Heart” OR “Heart Abnormality” OR “Congenital Disorders” OR “Neonatal Diseases and Abnormalities” OR “Tetralogy of Fallot” OR “Tricuspid Atresia” OR “Ebstein Anomaly” OR “Ebstein's Malformation” OR “Birth Defects” OR “Congenital Abnormalities”) AND (“Walk Test” OR “6 -min Walk Test” OR “6-minute Walk Test” OR “Six-minute Walk Test” OR “Endurance Shuttle Walk Test”)], which were later adapted to the other bases that were used in this review. Looking for studies that used submaximal field walking tests in children and adolescents with congenital heart disease aged 5 to 18 years. Methodological quality, effectiveness and safety, and risk of bias were assessed. Results Five studies met the eligibility criteria with a sample of 160 individuals with CHD, and all used the six-minute walk test (https://www.physio-pedia.com/Six_Minute_Walk_Test_/_6_Minute_Walk_Test6MWT). Note that different methodologies and modifications are used. The only clinical trial showed good methodological quality. Four studies had a low risk of bias, and one study had a moderate risk. Conclusion In this review, the 6MWT proved to be the first-choice method for assessing exercise capacity in children and adolescents with CHD, however, the lack of standardization in the application of the test became evident, which made it difficult to compare the results. Implications Reducing the limitations and heterogeneity in the application of the test will enable more concrete outcomes and facilitate their reproduction in clinical practice.
