Abstract:Objectives: There is evidence of a negative association between diabetes and both abdominal aortic aneurysm and aortic diameter. However, little information is available on the relation between diabetes and aortic root diameter. Methods: We studied 109 patients with type 2 diabetes. Two-dimensional echocardiography was used to measure the aortic root at the aortic annulus, the sinus of Valsalva, the sinotubular junction and the proximal part of the ascending aorta. Measured mean values were compared with 218 age-and sex-matched patients without diabetes. A comparison of the prevalence of aortic regurgitation between the 2 groups was also performed. Results: In patients with diabetes, the mean aortic root dimensions were significantly smaller than in nondiabetic patients (p < 0.05). The prevalence of aortic root dilatation was significantly higher in nondiabetic subjects than in patients with diabetes (9.63 vs. 2.75%; p = 0.025). In the multivariable regression model, diabetes was a significant negative determinant of aortic root size at all measured sites. The prevalence of aortic regurgitation tended to be higher in nondiabetic subjects than in diabetic participants (11 vs. 18.8%); however, the difference did not achieve statistical significance (p = 0.071). Conclusions: In patients with diabetes, the aortic root dimension is significantly smaller than in patients without diabetes. Copyright (C) 2008 S. Karger AG, Basel

Abstract:Objective To assess the elasticity change of common carotid artery in early stage of atherosclerosis in patients with type 2 diabetes by velocity vector imaging(VVI).Methods Sixty patients with type 2 diabetes were divided into group A(32 cases)and group B(28case)according the vascular complications(including Macroangiopathy and Microangiopathy).Patients without vascular complications belong to group A,others belong to group B.Fifty-eight healthy volunteers were supplied as normal control group.Time to the peak of velocity(VTTP),maximum of velocity(Vmax),maximum of strain(Smax),and maximum of strain rate(SRmax)of six segment(anterolateral wall,anterior wall,anteromedial wall,posteromedial wall,posterior wall,posterolateral wall)were measured,which were supplied by VVI automatic analysis software.Results VTTP,Smax and SRmax had no significant differences in any segments intra-group A,B and control group(P＞0.05).Vmax had significant differences in some segments in control group,group A and group B,Vmax of anterior wall,anterolateral wall and posterolateral wall were higher than those of posterior wall,posteromedial wall and anteromedial wall(P＜0.05).VTTP,Vmax,Smax and SRmax of corresponding segments had significant differences among group A,group B and control group(P＜0.05),control group＞group A＞group B(P＜0.05).Conclusions The change of common carotid elasticity can be detected in early stage of atherosclerosis in patients with type 2 diabetes by VVI.
Effect of Intensive Multifactorial Treatment on the Intima-Media Thickness of Large Arteries in Patients with New-Onset Type 2 Diabetes Mellitus

Abstract:Objective To quantify the changes in blood glucose, blood lipids, blood pressure, and the intima-media thickness (IMT) of large arteries in patients with new-onset type 2 diabetes mellitus who received either intensive multifactorial treatment or conventional treatment. Methods Two-hundred and ten patients with new-onset type 2 diabetes mellitus were randomly assigned to two groups: an intensive treatment group ( n =110) and a conventional treatment group ( n =100). Fasting blood glucose (FBG), glycosylated hemoglobin A1c (HbA1c), blood pressure, blood lipids [total cholesterol (TC), triglyceride (TG), low-density lipoprotein C (LDL-C), and high-density lipoprotein C (HDL-C)], and IMTs of large arteries (carotid, iliac, and femoral arteries) were determined before and at one and two years after starting treatment. The patients in the conventional treatment group received routine diabetes management in our outpatient department. Targets were established for patients in the intensive treatment group. Their blood glucose, blood lipids, and blood pressure levels were regularly monitored and therapeutic regimens were adjusted for those whose measurements did not meet the target values until all the parameters met the established targets. Within-group and between-group differences were evaluated. Results A significantly greater percentage of patients in the intensive treatment group had LDL-C levels that reached the target value one year after starting treatment than those in the conventional treatment group (52.04% vs. 33.33%, P <0.05). No significant differences were found between groups for FBG, HbA1c, blood pressure, TG, TC, or HDL-C. The percentages of patients with TG (51.02% vs. 34.48%), TC (52.04% vs. 33.33%), and LDL-C (61.22% vs. 43.67%) who met the respective target values in the intensive treatment group were all significantly higher than the corresponding percentages in the conventional treatment group two years after starting treatment ( P <0.05). There were no significant differences in the percentages of patients with FBG, HbA1c, and blood pressure values meeting the respective targets between the groups at the two-year follow-up. One year after starting treatment, the LDL-C level, diastolic blood pressure (DBP), and the IMTs of the femoral and iliac arteries of the intensive treatment group were significantly lower compared to those of the conventional treatment group ( P <0.05), although there was no significant difference in other metabolic parameters. Two years after starting treatment, the TC, LDL-C, blood pressure [systolic blood pressure (SBP) and DBP], and the IMTs of the carotid and femoral arteries of the intensive treatment group were significantly lower than those of the conventional treatment group ( P <0.05). No significant differences in other metabolic parameters existed between the two groups two years after starting treatment. Conclusions Early comprehensive and intensive treatment of type 2 diabetes mellitus can delay or even reverse the increase in IMT of large arteries. Lowering blood pressure and blood lipid regulation in patients with type 2 diabetes mellitus have great significance in decreasing the risk of diabetes-related macrovascular lesions.
Changes of Macrovascular Endothelial Ultrastructure and Gene Expression of Endothelial Nitric Oxide Synthase in Diabetic Rats

Abstract:Background The most intimidatory pathological changes in patients with DM are cardiovascular illnesses, which are the major causes of death in diabetic patients and are far more prevalent than in nondiabetics because of accelerated atherosclerosis. In this study, we tried to clarify the changes in macrovascular endothelial ultrastructure and in the gene expression of endothelial nitric oxide synthase (eNOS) mRNA in diabetic rats. Methods The study was conducted on 52 of 10-week old Sprague Dawley (SD) rats with body weight of (320 +/- 42) g. SD rats were divided into: experimental group treated with a single intraperitoneal injection of streptozotocin (STZ, 60 mg/kg), (male, n = 20, diabetes mellitus (DMM)); female, n = 12, diabetes mellitus female (DMF)) and control group (male, n = 10, diabetes mellitus male control (DMMC); female, n = 10, diabetes mellitus female control (DMFC)). Four weeks after treatment, half of the rats were sacrificed; the remainders were sacrificed ten weeks after treatment. One part of the abdominal aortic sample was stored under glutaraldehyde (volume fraction Psi(B) = 2.5%). After the process of chemical fixation, chemical dehydration, drying and conductivity enhancement, all samples were observed and photographed using scanning electron microscopy (Leica-Stereoscan 260, England). The other part of the abdominal aortic sample was treated with liquid nitrogen and the expression of eNOSmRNA was assessed by semi-quantitative RTPCR. Results The aortic lumen of both experimental groups adsorbed much more debris than that of either control group. The endothelial surfaces of diabetic rats were coarse, wrinkled and protuberant like fingers or villi. The vascular endothelial lesions of diabetic male rats were very distinct after 4 weeks, and as obvious as those at 10 weeks. The vascular endothelial lesions of diabetic female rats were not severe at 4 weeks and only became marked after 10 weeks. In both males and females, the abdominal aortic eNOSmRNA content of 4 weeks and 10 weeks diabetic rats was very significantly lower (P < 0.01) than that of controls. Conclusions Aortic endothelial ultrastructure in DM rats is injured compared with controls. Abnormal changes of aortic endothelia in male DM rats are more obvious than those in females. Expression of abdominal aortic eNOSmRNA content of DM rats is significantly lower than that of controls.

Abstract:Tests were designed to determine how well individuals can differentiate similar chest radiographs. These tests were given to 17 persons (radiologists, x-ray technologists, medical students, and secretaries). Each test contained a number of paired, dissimilar, and/or unique radiographs which were to be distinguished from one another or matched. The results suggest, that a radiologist does not have an innate advantage over untrained observers in pattern recognition. It is not known whether pattern-recognition tests are valuable in evaluating potential radiologists. A receiver operating characteristic curve is also described.

Abstract:Objective. To assess the possible association of aortic compliance and brachial endothelial function with cerebral small vessel disease in type 2 diabetes mellitus (DM2) patients by using 3.0 T high-resolution magnetic resonance imaging. Methods. Sixty-two clinically confirmed DM2 patients (25 women and 37 men; mean age: 56.8 +/- 7.5 years) were prospectively enrolled for noninvasive MR examinations of the aorta, brachial artery, and brain. Aortic arch pulse wave velocity (PWV), flow-mediated dilation (FMD) of brachial artery, lacunar brain infarcts, and periventricular and deep white matter hyperintensities (WMHs) were assessed. Pearson and Spearman correlation analysis were performed to analyze the association between PWV and FMD with clinical data and biochemical test results. Univariable logistic regression analyses were used to analyze the association between PWV and FMD with cerebral small vessel disease. Multiple logistic regression analyses were used to find out the independent predictive factors of cerebral small vessel disease. Results. Mean PWV was 6.73 +/- 2.00 m/s and FMD was 16.67 +/- 9.11%. After adjustment for compounding factors, PWV was found significantly associated with lacunar brain infarcts (OR = 2.00; 95% CI: 1.14-3.2;P < 0.05) and FMD was significantly associated with periventricular WMHs (OR = 0.82; 95% CI: 0.71-0.95;P < 0.05). Conclusions. Quantitative evaluation of aortic compliance and endothelial function by using high-resolution MRI may be potentially useful to stratify DM2 patients with risk of cerebral small vessel disease.
Analysis of Correlated Factors Influencing Macroangiopathy in Patients with New Onset Type 2 Diabetes Mellitus

Abstract:Objective To explore the correlated risk factors influencing macroangiopathy in patients with short duration type 2 diabetes mellitus(T2DM).Methods According to arterial intima-media thickness (IMT) measured by color Doppler ultrasonography,268 cases of T2DM diabetic patients(newly diagnosed or with the duration under one year) were divided into two groups: one group was 101 cases with atherosclerosis (AS group) and the other group was 167 cases without AS(non-AS group),compared various clinical data between the two groups and made correlation analysis.Results (1)The age, body mass index (BMI), smoking index (SI), the duration of hypertension, systolic blood pressure (SBP), diastolic blood pressure (DBP), total cholesterol (TC), low-density lipoprotein cholesterol (LDL-Ch) and C-reactive protein (CRP) were all significantly higher in the AS group than those in the non-AS group(P0.05). But the rate of diabetic family history and insulin sensitivity index (ISI) were lower in AS group than in non-AS group(P0.05).(2)The two groups were similar in sex , diabetes duration, fasting plasma glucose (FPG), post-OGTT 2 hour plasma glucose (2hPG), glycosylated hemoglobin (HbA 1c), fasting insulin (FINS), fasting triglyceride (TG) and high-density lipoprotein cholesterol (HDL-Ch).(3)By pearson analysis of correlation, the IMT mean showed a significant positive correlation with age (r=0.346, P0.001), BMI (r=0.229, P=0.001), SI ( r=0.349,P0.001), SBP (r=0.258, P0.001), DBP (r=0.207, P=0.002),LDL-Ch level (r=0.178, P=0.008).Conclusion The macroangiopathy of short duration type 2 diabetes mellitus is closely associated with age, smoking, overweight or obesity, hypertension and high LDL-Ch level, insulin resistance and CRP.
THE CORONARY ANGIOGRAPHY FEATURES AND CLINICAL CHARACTERISTICS OF PATIENTS WITH CORONARY HEART DISEASE COMPLICATED TYPE 2 DIABETES MELLITUS

Abstract:Objective To explore the characteristics of clinical manifestation and artery lesion in coronary heart disease patients combined with type 2 diabetes mellitus(T2DM). Methods 313 cases of Coronary heart disease(CHD) confirmed by coronary angiography were divided into T2DM group and NDM group according to whether or not combined with T2DM.We analyzed and compared two group in the general condition,clinical data,serum markers and coronary angiographic findings.According to coronary lesion morphology classification standards and SYNTAX score,we can qualitatively and quantitatively evaluated coronary artery lesions. Results Compared with NDM group,the ratio of hypertension,hyperglycemia and acute coronary syndrome were much more in T2DM group;Serum triglyceride(TG) increased,high density lipoprotein-cholesterol(HDL-C) decreased in T2DM group(p0.05).The proportion of multiple vessel disease,diffuse change,complicated lesions and arteriostenosis,vessel lesions of type C and SYNTAX score was significantly higher in T2DM group.There were no significant differences in ages,history of smoking,total cholesterol(TC) and low density lipoprotein cholesterol(LDL-C) in both groups. Conclusion The T2DM patients complicating CHD have more chance accompanied with risk factors such as hypertension,hyperglycemia,the proportion of ACS in DM group were higher,and have more accompanied with multiple vessel disease,diffuse change,complicated lesions and arteriostenosis.
Alterations in Aortic Elasticity Indices among Type 2 Diabetes Patients in a Low and Middle Income Country Using M-Mode Echocardiography: A Cross-Sectional Comparative Study

Abstract:Background: Diabetes is one of the leading causes of noncommunicable diseases worldwide. It is known to induce cardiovascular remodeling, which can result in a variety of complications, including a considerable increase in aortic stiffness. While studies in Western populations have explored these effects, data on Asians, particularly Vietnamese, are limited. This study aimed to assess aortic elasticity in type 2 diabetes mellitus (T2DM) patients compared to healthy individuals. Methods: Aortic elasticity indices were measured using M-mode echocardiography. A comparison between the healthy group and the T2DM group revealed substantial differences in aortic elasticity indices. Results: The aortic stiffness index (ASI) was significantly greater in the T2DM group than in the control group, with median values of 6.10 (3.64-12.47) and 3.79 (2.40-8.50), respectively (p = 0.003). Aortic strain (AS) was substantially lower in the T2DM group than in the control group, with median values of 8.21 (4.24 - 13.07) and 10.66 (6.01 - 18.23), respectively (p = 0.039). Furthermore, the median aortic compliance (AC) and aortic distensibility (AD) in individuals with T2DM were 4.07 (2.28-7.44) and 3.08 (1.57-5.26), respectively, lower than those in the control group, with median values of 6.40 (3.08-10.75) and 5.33 (2.80-9.79), respectively. A longer diabetes duration was linked to a greater ASI (r = 0.43, p < 0.05), while the AS decreased (r = -0.37, p < 0.05). Conclusions: Substantial variations in aorta elasticity indices were found using M-mode echocardiography. More research is needed to investigate the consequences of these discrepancies and their significance for clinical purposes.
Evaluation of Morphological Changes in Retinal Vessels in Type 1 Diabetes Mellitus Patients with the Use of Adaptive Optics

Abstract:Introduction. Diabetes mellitus contributes to the development of microvascular complications in the eye. Moreover, it affects multiple end organs, including brain damage, leading to premature death. The use of adaptive optics technique allows to perform non-invasive in vivo assessment of retinal vessels and to identify changes in arterioles about 100 μm in diameter. The retinal vasculature may be a model of the cerebral vessels both morphologically and functionally. Aim. To evaluate morphological parameters of retinal arterioles in patients with type 1 diabetes mellitus (DM1). Material and methods. The study included 22 DM1 patients (13 females) aged 43.00 ± 9.45 years with a mean diabetes duration of 22.55 ± 10.05 years, and 23 healthy volunteers (10 females) aged 41.09 ± 10.99 years. Blood pressure, BMI, waist circumference, and metabolic control markers of diabetes were measured in both groups. Vascular examinations were performed using an rtx1 adaptive optics retinal camera (Imagine Eyes, Orsay, France); the vessel wall thickness (WT), lumen diameter (LD), wall-to-lumen ratio (WLR), and vascular wall cross-sectional area (WCSA) were assessed. Statistical analysis was performed with the application of IMB SPSS version 23 software. Results. The DM1 group did not differ significantly in age, BMI, waist circumference, blood pressure, or axial length of the eye compared to the control group. Intraocular pressure (IOP) in both groups was normal, but in the DM1 group it was significantly higher. The DM1 group had significantly higher WT, WLR, and WCSA. These parameters correlated significantly with the duration of diabetes, but not with IOP. Conclusions. The presented study demonstrates the presence of significant morphological changes in retinal vessels in DM1 patients without previously diagnosed diabetic retinopathy. Similar changes may occur in the brain and may be early indicators of cardiovascular risk, but further investigation is required to confirm that.

Sex differences and determinants of coronary microvascular function in asymptomatic adults with type 2 diabetes

Abstract:Background: Coronary microvascular dysfunction (CMD) is a significant complication in type 2 diabetes (T2D) and may be more common in women. We aimed to evaluate the sex differences and sex-specific clinical determinants of CMD in adults with T2D without prevalent cardiovascular disease. Methods: Single center pooled analysis of four prospective studies comparing asymptomatic people with T2D and controls. All subjects underwent comprehensive cardiovascular phenotyping with myocardial perfusion reserve (MPR) quantified with perfusion MRI. Participants with silent coronary disease were excluded. Multivariable linear regression was performed to identify determinants of MPR with an interaction term for sex. Results: Four-hundred and seventy-nine T2D (age 57 ± 11y, 42% women) were compared with 116 controls (age 53 ± 11y, 41% women). Men with T2D, but not women, demonstrated worse systolic function and higher extra-cellular volume fraction than controls. MPR was significantly lower in T2D than controls (women, 2.6 ± 0.9 vs. 3.3 ± 1.0, p <0.001; men, 3.1 ± 0.9 vs. 3.5 ± 1.0, p = 0.004), and lower in women than men with T2D (p <0.001). More women than men with T2D had MPR <2.5 (46% vs. 26%, p <0.001). There was a significant interaction between sex and body mass index (BMI) for MPR (p-interaction <0.001). Following adjustment for clinical risk factors, inverse association with MPR were body mass index (BMI) in women (β = -0.17, p = 0.045) and systolic blood pressure in men (β = -0.14, p = 0.049). Conclusions: Among asymptomatic adults with T2D, women had a greater prevalence of CMD than men. Risk factors modestly but significantly associated with coronary microvascular dysfunction in asymptomatic people with T2D were BMI among women and systolic blood pressure among men.
A retrospective study on the role of diabetes mellitus in longitudinal extension of abdominal aortic aneursym

Abstract:Objective Our study focuses on evaluating the effect of diabetes mellitus on longitudinal extension in AAA progression.Methods We studied 460 male and female patients with AAA,aged 41 to 92 years,who have been admitted at least twice into our hospital from January 2000 to April 2010.All of them accepted abdominal vascular ultrasound examination for scanning maximal length,transverse diameter and anteroposterior diameter of abdominal aortic aneurysm during each admission period.Results Our results indicated that length changes were positively associated with diabetes mellitus,but not to transverse expansion in AAA progression.Length increases of AAA were observed in patients with diabetes when compared with those without diabetes.Glycosylated hemoglobin wasn't significantly different between diabetes patients and non-diabetes patients.Bivariate correlation(r=0.295,P<0.05)and partial correlation analysis(r=0.343,P<0.05)indicated that diabetes mellitus was associated with length extension of AAA.Conclusions Diabetes mellitus is related to the extension of the length growth of AAA and receiving better glucose controled doesn't affect extension of aneurysm progression in diabetes patients.
Impact of type 2 diabetes on serial changes in tissue characteristics of coronary plaques: an integrated backscatter intravascular ultrasound analysis

Abstract:Aims: Several studies have demonstrated that type 2 diabetes mellitus (T2DM) is associated with accelerated atherosclerosis, which results in an increased risk of coronary vascular events. However, serial changes in plaque characteristics have not been reported in vivo. We evaluated the progression of coronary atherosclerosis in patients with T2DM using an integrated backscatter intravascular ultrasound (IB-IVUS) examination. Methods and results: Forty-two T2DM and 48 non-diabetic patients who underwent percutaneous coronary intervention were enrolled in the study. Non-culprit 20-mm length coronary lesions with mild-to-moderate stenosis were measured using a 40-MHz (motorized pullback of 0.5 mm/s) IVUS catheter. IVUS examinations were performed on one target lesion in each patient. Six months later, a follow-up IVUS examination was repeated in the same coronary segment imaged at the baseline examination. T2DM patients demonstrated a greater total plaque volume (TPV; 139 ± 53 vs. 114 ± 45 mm(3), P = 0.02) and total lipid volume (TLV; 67 ± 26 vs. 55 ± 30 mm(3), P = 0.039) at the baseline examination. The progression of TPV (8.6 ± 15.4 vs. -2.2 ± 16.0%, P < 0.01) and TLV (10.8 ± 28.8 vs. -2.5 ± 20.0%, P < 0.05) from the baseline was observed in T2DM patients, but not in non-diabetic patients. The increase in TLV was blunted in T2DM patients who achieved HbA1c levels of <6.5%. Conclusion: Accelerated plaque progression with an increase in the lipid-rich component of non-culprit plaques was observed in T2DM, despite the use of standard medical treatment. Better glycaemic control ameliorated the worsening of plaque characteristics in T2DM.
Coronary artery calcification, carotid intima-media thickness and cardiac dysfunction in young adults with type 2 diabetes mellitus

Abstract:Aims: Cardiovascular diseases (CVD) are the major causes of mortality in patients with type 2 diabetes mellitus (T2DM). There is paucity of information on prevalence of subclinical atherosclerosis and cardiac dysfunction in young adults with T2DM. This study aimed to assess the prevalence of subclinical atherosclerosis and cardiac dysfunction in young adults with T2DM, asymptomatic for CVD. Methods: Sixty-two patients with T2DM, age between 30 and 50 years were evaluated for coronary artery calcium (CAC) score, carotid intima-media thickness (CIMT) and flow-mediated dilatation (FMD) at the brachial artery. All were subjected to 2D-color Doppler echocardiography, electrocardiography and testing for serum N-terminal pro-brain natriuretic peptide (NT-proBNP) and high-sensitivity C-reactive protein (hsCRP). The results were compared with those in 60 age, sex and BMI-matched healthy controls. Results: Prevalence of a positive CAC score was comparable among subjects with and without T2DM (14.5% vs 11.7%). Patients with T2DM had a significantly higher CIMT (0.54 ± 0.15 vs 0.49 ± 0.10 mm, p = 0.01), left ventricular (LV) mass (170 ± 36 vs 147 ± 23 g, p < 0.001), heart rate (83 ± 13 vs 74 ± 11, p < 0.001) and QTc interval (402 ± 20 vs 382 ± 21 ms, p < 0.001) compared to controls. FMD was lower in patients with T2DM compared to controls (9.1 ± 4.4% vs 10.7 ± 3.9%, p = 0.04). There was a higher prevalence of LV hypertrophy (37% vs 7%, p < 0.001) and diastolic dysfunction (7% vs 0) in patients with T2DM compared to controls. None of the participants had systolic dysfunction. Hypertension (42 vs 7%, p < 0.001) and metabolic syndrome (76 vs 35%, p < 0.001) were more prevalent in the patient group. In the multivariate analysis, age was the lone predictor of CIMT and FMD; while T2DM and male gender were the independent predictors of LV mass. Conclusions: Young adults with T2DM, asymptomatic for CVD had a higher prevalence of CVD risk factors, LV hypertrophy and diastolic dysfunction. A higher CIMT and LV mass, and a lower FMD were noted in patients with T2DM. CAC score was comparable between the groups and thus may not be a useful tool for assessment of subclinical atherosclerosis in this cohort, where CIMT and FMD may be more appropriate.
Effects of diabetes mellitus and glycemic traits on cardiovascular morpho-functional phenotypes

Abstract:Background: The effects of diabetes on the cardiac and aortic structure and function remain unclear. Detecting and intervening these variations early is crucial for the prevention and management of complications. Cardiovascular magnetic resonance imaging-derived traits are established endophenotypes and serve as precise, early-detection, noninvasive clinical risk biomarkers. We conducted a Mendelian randomization (MR) study to examine the association between two types of diabetes, four glycemic traits, and preclinical endophenotypes of cardiac and aortic structure and function. Methods: Independent genetic variants significantly associated with type 1 diabetes, type 2 diabetes, fasting insulin (FIns), fasting glucose (FGlu), 2 h-glucose post-challenge (2hGlu), and glycated hemoglobin (HbA1c) were selected as instrumental variables. The 96 cardiovascular magnetic resonance imaging traits came from six independent genome-wide association studies. These traits serve as preclinical endophenotypes and offer an early indication of the structure and function of the four cardiac chambers and two aortic sections. The primary analysis was performed using MR with the inverse-variance weighted method. Confirmation was achieved through Steiger filtering and testing to determine the causal direction. Sensitivity analyses were conducted using the weighted median, MR-Egger, and MR-PRESSO methods. Additionally, multivariable MR was used to adjust for potential effects associated with body mass index. Results: Genetic susceptibility to type 1 diabetes was associated with increased ascending aortic distensibility. Conversely, type 2 diabetes showed a correlation with a reduced diameter and areas of the ascending aorta, as well as decreased distensibility of the descending aorta. Genetically predicted higher levels of FGlu and HbA1c were correlated with a decrease in diameter and areas of the ascending aorta. Furthermore, higher 2hGlu levels predominantly showed association with a reduced diameter of both the ascending and descending aorta. Higher FIns levels corresponded to increased regional myocardial-wall thicknesses at end-diastole, global myocardial-wall thickness at end-diastole, and regional peak circumferential strain of the left ventricle. Conclusions: This study provides evidence that diabetes and glycemic traits have a causal relationship with cardiac and aortic structural and functional remodeling, highlighting the importance of intensive glucose-lowering for primary prevention of cardiovascular diseases.
An Analysis of Risk Factors of Atherosclerosis in Patients with T2DM

Abstract:Objective To explore the risk factors of atherosclerosis in patients with type 2 diabetes mellitus(T2DM).Methods 118 cases of patients with T2DM were divided into two groups according to disease,group A of 60 cases,with macrovascular disease;group B of 58 cases,without macrovascular disease.The two groups had the determination of biochemical and radioimmunoassay indicators,liver and carotid artery intima-media thickness(IMT) were examed with ultrasound.The differences of the risk factors between two groups were analyzed.Results Compared with group B,IMT of group A increased more significantly(1.01±0.22 vs 0.84±0.16,P0.05);The morbidity of nonalcoholic fatty liver disease(NAFLD) was higher(67.5% vs 47.5%,P0.05);There were significant differences between the mean of two groups' ALT,TC,LDL-C,HDL-C,BMI and HOMA-IR(P0.05);There is no significant difference between the means of two groups' TG(P0.05).Multiple regression analysis indicated that two groups' carotid artery IMT were related to the level of ALT,TC,LDL-C,HOMA-IR and BMI.By patial correlations analysis,carotid artery IMT was correlated with level of ALT independently.Conclusion Abnormal lipid metabolism,insulin resistance,obesity and NAFLD were related to the occurrence and development of atherosclerosis in patients with T2DM.

Abstract:Individuals with type 2 diabetes have a three- to fivefold increased risk of developing heart failure. Diabetic cardiomyopathy is typified by left ventricular (LV) concentric remodeling, which is a recognized predictor of adverse cardiovascular events. Although the mechanisms underlying LV remodeling in type 2 diabetes are unclear, progressive aortic stiffening may be a key determinant. The aim of this study was to assess the relationship between aortic stiffness and LV geometry in younger adults with type 2 diabetes, using multiparametric cardiovascular MRI. We prospectively recruited 80 adults (aged 18–65 years) with type 2 diabetes and no cardiovascular disease and 20 age- and sex-matched healthy control subjects. All subjects underwent comprehensive bio-anthropometric assessment and cardiac MRI, including measurement of aortic stiffness by aortic distensibility (AD). Type 2 diabetes was associated with increased LV mass, concentric LV remodeling, and lower AD compared with control subjects. On multivariable linear regression, AD was independently associated with concentric LV remodeling in type 2 diabetes. Aortic stiffness may therefore be a potential therapeutic target to prevent the development of heart failure in type 2 diabetes.

Abstract:Abstract Background Coronary artery disease (CAD) confers considerable morbidity and mortality in diabetes. However, the role of CAD in additive effect of left ventricular (LV) function has rarely been explored in type 2 diabetes mellitus (T2DM) patients. This study aimed to investigate how CAD affect LV systolic and diastolic function in T2DM patients. Materials and methods A total of 282 T2DM patients {104 patients with CAD [T2DM (CAD +)] and 178 without [T2DM (CAD −)]} and 83 sex- and age- matched healthy controls underwent cardiac magnetic resonance scanning. LV structure, function, global strains [including systolic peak strain (PS), peak systolic (PSSR) and diastolic strain rate (PDSR) in radial, circumferential and longitudinal directions] and late gadolinium enhancement (LGE) parameters were measured. T2DM (CAD +) patients were divided into two subgroups based on the median of Gensini score (60) which was calculated to assess the severity of CAD. Multivariable linear regression analyses were constructed to investigate the determinants of reduced LV function. Results Compared with normal controls, T2DM (CAD −) patients exhibited increased LV end-diastolic and end-systolic volume index and decreased LV global strains, while T2DM(CAD +) patients showed more marked increase and decrease than T2DM(CAD-) and healthy controls, except for longitudinal PDSR (PDSR-L) (all P 60 group, except for PDSR-L (all P < 0.017). CAD was an independent predictor of reduced LV global circumferential PS (GCPS, β = 0.22, p < 0.001), PSSR (PSSR-C, β = 0.17, p = 0.005), PDSR (PDSR-C, β = 0.22, p < 0.001), global radial PS (GRPS, β = 0.19, p = 0.001), and global longitudinal PS (GLPS, β = 0.18, p = 0.003) in T2DM. The Gensini score was associated with decreased GCPS, PSSR-C, PDSR-C, GRPS, and GLPS in T2DM (CAD +) (all p < 0.05). Conclusion CAD has an additive deleterious effect on LV systolic and diastolic function in T2DM patients. Among T2DM (CAD +) patients, the Gensini score is associated with reduced LV contractile and diastolic function. Trial registration Retrospectively registered
