Abstract:Introduction: This study aims to construct a mortality prediction model for patients with non-variceal upper gastrointestinal bleeding (NVUGIB) in the intensive care unit (ICU), employing advanced machine learning algorithms. The goal is to identify high-risk populations early, contributing to a deeper understanding of patients with NVUGIB in the ICU. Methods: We extracted NVUGIB data from the Medical Information Mart for Intensive Care IV (MIMIC-IV, v.2.2) database spanning from 2008 to 2019. Feature selection was conducted through LASSO regression, followed by training models using 11 machine learning methods. The best model was chosen based on the area under the curve (AUC). Subsequently, Shapley additive explanations (SHAP) was employed to elucidate how each factor influenced the model. Finally, a case was randomly selected, and the model was utilized to predict its mortality, demonstrating the practical application of the developed model. Results: In total, 2716 patients with NVUGIB were deemed eligible for participation. Following selection, 30 out of a total of 64 clinical parameters collected on day 1 after ICU admission remained associated with prognosis and were utilized for developing machine learning models. Among the 11 constructed models, the Gradient Boosting Decision Tree (GBDT) model demonstrated the best performance, achieving an AUC of 0.853 and an accuracy of 0.839 in the validation cohort. Feature importance analysis highlighted that shock, Glasgow Coma Scale (GCS), renal disease, age, albumin, and alanine aminotransferase (ALP) were the top six features of the GBDT model with the most significant impact. Furthermore, SHAP force analysis illustrated how the constructed model visualized the individualized prediction of death. Conclusions: Patient data from the MIMIC database were leveraged to develop a robust prognostic model for patients with NVUGIB in the ICU. The analysis using SHAP also assisted clinicians in gaining a deeper understanding of the disease.

Abstract:BACKGROUND:Acute pancreatitis (AP) is the most common pancreatic disease. Predicting the severity of AP is critical for making preventive decisions. However, the performance of existing scoring systems in predicting AP severity was not satisfactory. The purpose of this study was to develop predictive models for the severity of AP using machine learning (ML) algorithms and explore the important predictors that affected the prediction results.METHODS:The data of 441 patients in the Department of Gastroenterology in our hospital were analyzed retrospectively. The demographic data, blood routine and blood biochemical indexes, and the CTSI score were collected to develop five different ML predictive models to predict the severity of AP. The performance of the models was evaluated by the area under the receiver operating characteristic curve (AUC). The important predictors were determined by ranking the feature importance of the predictive factors.RESULTS:Compared to other ML models, the extreme gradient boosting model (XGBoost) showed better performance in predicting severe AP, with an AUC of 0.906, an accuracy of 0.902, a sensitivity of 0.700, a specificity of 0.961, and a F1 score of 0.764. Further analysis showed that the CTSI score, ALB, LDH, and NEUT were the important predictors of the severity of AP.CONCLUSION:The results showed that the XGBoost algorithm can accurately predict the severity of AP, which can provide an assistance for the clinicians to identify severe AP at an early stage.
Upper gastrointestinal haemorrhage patients' survival: A causal inference and prediction study

Abstract:This study, based on multicenter hospital data, aimed to develop models to predict in‐hospital and 30‐day survival of critically ill patients with upper gastrointestinal bleeding. The optimal model using the light GBM algorithm achieved high accuracy and generalizability. Significant factors for in‐hospital survival and significant factors for 30‐day survival were causally inferred, and coefficients of prognostic impact were presented. This model can assist clinicians in managing these patients to improve the discrimination of high‐risk patients. Background Upper gastrointestinal (GI) bleeding is a common medical emergency. This study aimed to develop models to predict critically ill patients with upper GI bleeding in‐hospital and 30‐day survival, identify the correlation factor and infer the causality. Methods A total of 2898 patients with upper GI bleeding were included from the Medical Information Mart for Intensive Care‐IV and eICU‐Collaborative Research Database, respectively. To identify the most critical factors contributing to the prognostic model, we used SHAP (SHapley Additive exPlanations) for machine learning interpretability. We performed causal inference using inverse probability weighting for survival‐associated prognostic factors. Results The optimal model using the light GBM (gradient boosting algorithm) algorithm achieved an AUC of .93 for in‐hospital survival, .81 for 30‐day survival in internal testing and .87 for in‐hospital survival in external testing. Important factors for in‐hospital survival, according to SHAP, were SOFA (Sequential organ failure assessment score), GCS (Glasgow coma scale) motor score and length of stay in ICU (Intensive critical care). In contrast, essential factors for 30‐day survival were SOFA, length of stay in ICU, total bilirubin and GCS verbal score. Our model showed improved performance compared to SOFA alone. Conclusions Our interpretable machine learning model for predicting in‐hospital and 30‐day mortality in critically ill patients with upper gastrointestinal bleeding showed excellent accuracy and high generalizability. This model can assist clinicians in managing these patients to improve the discrimination of high‐risk patients.

Machine Learning Models for Predicting Mortality in Patients with Cirrhosis and Acute Upper Gastrointestinal Bleeding at an Emergency Department: A Retrospective Cohort Study

Abstract:Background: Cirrhosis is a major global cause of mortality, and upper gastrointestinal (GI) bleeding significantly increases the mortality risk in these patients. Although scoring systems such as the Child-Pugh score and the Model for End-stage Liver Disease evaluate the severity of cirrhosis, none of these systems specifically target the risk of mortality in patients with upper GI bleeding. In this study, we constructed machine learning (ML) models for predicting mortality in patients with cirrhosis and upper GI bleeding, particularly in emergency settings, to achieve early intervention and improve outcomes. Methods: In this retrospective study, we analyzed the electronic health records of adult patients with cirrhosis who presented at an emergency department (ED) with GI bleeding between 2001 and 2019. Data were divided into training and testing sets at a ratio of 90:10. The ability of three ML models-a linear regression model, an XGBoost (XGB) model, and a three-layer neural network model-to predict mortality in the patients was evaluated. Results: A total of 16,025 patients with cirrhosis and 32,826 ED visits for upper GI bleeding were included in the study. The in-hospital and ED mortality rates were 11.2% and 2.2%, respectively. The XGB model exhibited the highest performance in predicting both in-hospital and ED mortality (area under the receiver operating characteristic curve: 0.866 and 0.861, respectively). International normalized ratio, renal function, red blood cell distribution width, age, and white blood cell count were the strongest predictors in all the ML models. The median ED length of stay for the ED mortality group was 17.54 h (7.16-40.01 h). Conclusions: ML models can be used to predict mortality in patients with cirrhosis and upper GI bleeding. Of the three models, the XGB model exhibits the highest performance. Further research is required to determine the actual efficacy of our ML models in clinical settings.

Abstract:Objectives: Postoperative major bleeding is a common problem in patients undergoing cardiac surgery and is associated with poor outcomes. We evaluated the performance of machine learning (ML) methods to predict postoperative major bleeding. Methods: A total of 1,045 patients who underwent isolated coronary artery bypass graft surgery (CABG) were enrolled. Their datasets were assigned randomly to training (70%) or a testing set (30%). The primary outcome was major bleeding defined as the universal definition of perioperative bleeding (UDPB) classes 3–4. We constructed a reference logistic regression (LR) model using known predictors. We also developed several modern ML algorithms. In the test set, we compared the area under the receiver operating characteristic curves (AUCs) of these ML algorithms with the reference LR model results, and the TRUST and WILL-BLEED risk score. Calibration analysis was undertaken using the calibration belt method. Results: The prevalence of postoperative major bleeding was 7.1% (74/1,045). For major bleeds, the conditional inference random forest (CIRF) model showed the highest AUC [0.831 (0.732–0.930)], and the stochastic gradient boosting (SGBT) and random forest models demonstrated the next best results [0.820 (0.742–0.899) and 0.810 (0.719–0.902)]. The AUCs of all ML models were higher than [0.629 (0.517–0.641) and 0.557 (0.449–0.665)], as achieved by TRUST and WILL-BLEED, respectively. Conclusion: ML methods successfully predicted major bleeding after cardiac surgery, with greater performance compared with previous scoring models. Modern ML models may enhance the identification of high-risk major bleeding subpopulations.

Abstract:Background Acute kidney injury (AKI) is a common and important complication in patients with gastrointestinal bleeding who are admitted to the intensive care unit. The present study proposes an artificial intelligence solution for acute kidney injury prediction in patients with gastrointestinal bleeding admitted to the intensive care unit. Methods Data were collected from the eICU Collaborative Research Database (eICU-CRD) and Medical Information Mart for Intensive Care-IV (MIMIC-IV) database. The prediction model was developed using the extreme gradient boosting (XGBoost) model. The area under the receiver operating characteristic curve, accuracy, precision, area under the precision–recall curve (AUC-PR), and F1 score were used to evaluate the predictive performance of each model. Results Logistic regression, XGBoost, and XGBoost with severity scores were used to predict acute kidney injury risk using all features. The XGBoost-based acute kidney injury predictive models including XGBoost and XGBoost+severity scores model showed greater accuracy, recall, precision AUC, AUC-PR, and F1 score compared to logistic regression. Conclusion The XGBoost model obtained better risk prediction for acute kidney injury in patients with gastrointestinal bleeding admitted to the intensive care unit than the traditional logistic regression model, suggesting that machine learning (ML) techniques have the potential to improve the development and validation of predictive models in patients with gastrointestinal bleeding admitted to the intensive care unit.

Abstract:Acute upper gastrointestinal bleeding (UGIB) is a common life-threatening clinical emergency with a poor prognosis. The aim of this study was to develop a risk prediction model for in-hospital mortality in patients with UGIB. We performed a post hoc analysis of a publicly available retrospective clinical data. A total of 360 patients with UGIB were included in this study. The least absolute shrinkage and selection operator regression was used to screen predictors and a restricted cubic spline function was used to investigate the assumption of linear relationships between continuous predictors and the risk of in-hospital mortality. Backward stepwise selection with the Akaike information criterion was used to identify variables for the best prediction model. A nomogram was developed based on the results of the best prediction model. The receiver operating characteristic curve, GiViTI calibration plot, and decision curve analysis were used to evaluate the performance of the nomogram. The optimal prediction model consisting of 4 predictors: red cell distribution width (odds ratio [OR] = 8.44; 95% confidence interval [CI]: 1.77-89.10), platelet count (OR = 0.99; 95% CI: 0.99-1.00), pulse rate (OR = 1.03; 95% CI: 1.01-1.05), and SpO2 (OR = 0.92; 95% CI: 0.86-0.96). The nomogram model had good discrimination (area under the curve = 0.86, 95% CI: 0.78-0.95), calibration, and clinical usefulness. In this study, we developed a nomogram model for predicting death during hospitalization in patients with UGIB based on blood biomarkers and baseline vital signs at the time of admission. The model has good performance, allowing rapid risk stratification of patients with UGIB.

Abstract:Background: Acute upper gastrointestinal bleeding (AUGIB) is a major cause of morbidity and mortality. This presentation however is not universally high risk as only 20–30% of bleeds require urgent haemostatic therapy. Nevertheless, the current standard of care is for all patients admitted to an inpatient bed to undergo endoscopy within 24 h for risk stratification which is invasive, costly and difficult to achieve in routine clinical practice. Objectives: To develop novel non-endoscopic machine learning models for AUGIB to predict the need for haemostatic therapy by endoscopic, radiological or surgical intervention. Design: A retrospective cohort study Method: We analysed data from patients admitted with AUGIB to hospitals from 2015 to 2020 ( n = 970). Machine learning models were internally validated to predict the need for haemostatic therapy. The performance of the models was compared to the Glasgow-Blatchford score (GBS) using the area under receiver operating characteristic (AUROC) curves. Results: The random forest classifier [AUROC 0.84 (0.80–0.87)] had the best performance and was superior to the GBS [AUROC 0.75 (0.72–0.78), p < 0.001] in predicting the need for haemostatic therapy in patients with AUGIB. A GBS cut-off of ⩾12 was associated with an accuracy, sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) of 0.74, 0.49, 0.81, 0.41 and 0.85, respectively. The Random Forrest model performed better with an accuracy, sensitivity, specificity, PPV and NPV of 0.82, 0.54, 0.90, 0.60 and 0.88, respectively. Conclusion: We developed and validated a machine learning algorithm with high accuracy and specificity in predicting the need for haemostatic therapy in AUGIB. This could be used to risk stratify high-risk patients to urgent endoscopy.

Abstract:This study aimed to develop a machine learning (ML)-based tool for early and accurate prediction of in-hospital mortality risk in patients with spontaneous intracerebral hemorrhage (sICH) in the intensive care unit (ICU). We did a retrospective study in our study and identified cases of sICH from the MIMIC IV (n = 1486) and Zhejiang Hospital databases (n = 110). The model was constructed using features selected through LASSO regression. Among five well-known models, the selection of the best model was based on the area under the curve (AUC) in the validation cohort. We further analyzed calibration and decision curves to assess prediction results and visualized the impact of each variable on the model through SHapley Additive exPlanations. To facilitate accessibility, we also created a visual online calculation page for the model. The XGBoost exhibited high accuracy in both internal validation (AUC = 0.907) and external validation (AUC = 0.787) sets. Calibration curve and decision curve analyses showed that the model had no significant bias as well as being useful for supporting clinical decisions. XGBoost is an effective algorithm for predicting in-hospital mortality in patients with sICH, indicating its potential significance in the development of early warning systems.

Abstract:Hemorrhage is a prevalent and critical condition in the intensive care unit (ICU), marked by high incidence, elevated mortality rates, and substantial therapeutic challenges. Accurate prediction of mortality in patients with hemorrhage is essential for the development of personalized prevention and treatment strategies. Nevertheless, the implementation of effective predictive models in clinical practice remains limited, largely due to the current gap in robust and interpretable prediction tools. This study aimed to develop an interpretable model for predicting mortality risk in critically ill patients with hemorrhage in intensive care units (ICUs). The SHapley Additive exPlanation (SHAP) method was applied to interpret the extreme gradient boosting (XGBoost) model, allowing for the exploration of key prognostic factors in this patient population. In this retrospective cohort study, we developed and evaluated the performance of a predictive model using data from the eICU Collaborative Research Database (eICU-CRD). Data from the first 24 hours of each ICU admission were extracted, with the dataset randomly split into a training set (80%) and a validation set (20%). The predictive performance of the XGBoost model was compared to four other machine learning models using the area under the curve (AUC) as the metric. The SHapley Additive exPlanation (SHAP) method was employed to interpret the XGBoost model. Following initial validation, external validation was performed using data from a Chinese retrospective cohort, Refrain, which focuses on hemorrhage and coagulopathy in critically ill patients. A total of 10306 eligible patients with hemorrhage were included in the final cohort for this study. The observed in-hospital mortality of patients with hemorrhage was 11.5%. Comparatively, the XGBoost model had the highest predictive performance among five models with an area under the curve (AUC=0.81) , whereas LR had the poorest generalization ability (AUC=0.726). The decision curve showed that the net benefit of the XGBoost model surpassed those of other machine learning models at 10%~30% threshold probabilities. The SHAP method reveals the top 15 predictors of hemorrhage according to the importance ranking, and the bilirubin level was recognized as the most important predictor variable. Additionally, in the external validation using the REFRAIN cohort, the XGBoost model demonstrated robust predictive performance with an AUC of 0.776. The interpretable predictive model enhances the accuracy of mortality risk prediction in ICU patients with hemorrhage, enabling clinicians to devise more effective treatment plans and optimize resource allocation. Moreover, the interpretability framework increases model transparency, thereby facilitating clinicians' understanding and trust in the reliability of the predictive model.
Mortality Prediction in Cerebral Hemorrhage Patients Using Machine Learning Algorithms in Intensive Care Units

Abstract:Objectives: This study aims to investigate whether the machine learning algorithms could provide an optimal early mortality prediction method compared with other scoring systems for patients with cerebral hemorrhage in intensive care units in clinical practice. Methods: Between 2008 and 2012, from Intensive Care III (MIMIC-III) database, all cerebral hemorrhage patients monitored with the MetaVision system and admitted to intensive care units were enrolled in this study. The calibration, discrimination, and risk classification of predicted hospital mortality based on machine learning algorithms were assessed. The primary outcome was hospital mortality. Model performance was assessed with accuracy and receiver operating characteristic curve analysis. Results: Of 760 cerebral hemorrhage patients enrolled from MIMIC database [mean age, 68.2 years (SD, ±15.5)], 383 (50.4%) patients died in hospital, and 377 (49.6%) patients survived. The area under the receiver operating characteristic curve (AUC) of six machine learning algorithms was 0.600 (nearest neighbors), 0.617 (decision tree), 0.655 (neural net), 0.671(AdaBoost), 0.819 (random forest), and 0.725 (gcForest). The AUC was 0.423 for Acute Physiology and Chronic Health Evaluation II score. The random forest had the highest specificity and accuracy, as well as the greatest AUC, showing the best ability to predict in-hospital mortality. Conclusions: Compared with conventional scoring system and the other five machine learning algorithms in this study, random forest algorithm had better performance in predicting in-hospital mortality for cerebral hemorrhage patients in intensive care units, and thus further research should be conducted on random forest algorithm.

Abstract:Background: In the ICU, patients with acute myocardial infarction and cardiogenic shock (AMI-CS) often face high mortality rates, making timely and precise mortality risk prediction crucial for clinical decision-making. Despite existing models, machine learning algorithms hold the potential for improved predictive accuracy. Methods: In this study, a predictive model was developed using the MIMIC-IV database, with external validation performed on the eICU-CRD database. We included ICU patients diagnosed with AMI-CS. Feature selection was conducted using the Boruta algorithm, followed by the construction and comparison of four machine learning models: Logistic Regression (LR), eXtreme Gradient Boosting (XGBoost), Adaptive Boosting (AdaBoost), and Gaussian Naive Bayes (GNB). Model performance was evaluated based on metrics such as AUC (Area Under the Curve), accuracy, sensitivity, specificity, and so on. The SHAP method was employed to visualize and interpret the importance of model features. Finally, we constructed an online prediction model and conducted external validation in the eICU-CRD database. Results: In this study, a total of 570 and 391 patients with AMI-CS were included from the MIMIC-IV and eICU-CRD databases, respectively. Among all machine learning algorithms evaluated, LR exhibited the best performance with a validation set AUC of 0.841(XGBoost: 0.835, AdaBoost: 0.839, GNB: 0.826). The model incorporated five variables: prothrombin time, blood urea nitrogen, age, beta-blockers and Angiotensin-Converting Enzyme Inhibitors or Angiotensin II Receptor Blockers. SHAP plots are employed to visualize the importance of model features and to interpret the results. An online prediction tool was developed, externally validated with the eICU-CRD database, achieving an AUC of 0.755. Conclusion: Employing the LR algorithm, we developed a predictive model for assessing the mortality risk among AMI-CS patients in the ICU setting. Through model predictions, this facilitates early detection of high-risk individuals, ensures judicious allocation of healthcare resources.

A nomogram to predict in-hospital mortality of gastrointestinal bleeding patients in the intensive care unit

Abstract:Background Gastrointestinal bleeding (GIB) is a common condition in clinical practice, and predictive models for patients with GIB have been developed. However, assessments of in-hospital mortality due to GIB in the intensive care unit (ICU), especially in critically ill patients, are still lacking. This study was designed to screen out independent predictive factors affecting in-hospital mortality and thus establish a predictive model for clinical use. Methods This retrospective study included 1,442 patients with GIB who had been admitted to the ICU. They were selected from the Medical Information Mart for Intensive Care IV (MIMIC-IV) 1.0 database and divided into a training group and a validation group in a ratio of 7:3. The main outcome measure was in-hospital mortality. Least absolute shrinkage and section operator (LASSO) regression was used to screen out independent predictors and create a nomogram. Results LASSO regression picked out nine independent predictors: heart rate (HR), activated partial thromboplastin time (aPTT), acute physiology score III (APSIII), sequential organ failure assessment (SOFA), cerebrovascular disease, acute kidney injury (AKI), norepinephrine, vasopressin, and dopamine. Our model proved to have excellent predictive value with regard to in-hospital mortality (the area under the receiver operating characteristic curve was 0.906 and 0.881 in the training and validation groups, respectively), as well as a good outcome on a decision curve analysis to assess net benefit. Conclusion Our model effectively predicts in-hospital mortality in patients with GIB, indicating that it may prove to be a valuable tool in future clinical practice.

Abstract:Objective: The accurate evaluation of outcomes at a personalized level in patients with intracerebral hemorrhage (ICH) is critical clinical implications. This study aims to evaluate how machine learning integrates with routine laboratory tests and electronic health records (EHRs) data to predict inpatient mortality after ICH. Methods: In this machine learning-based prognostic study, we included 1,835 consecutive patients with acute ICH between October 2010 and December 2018. The model building process incorporated five pre-implant ICH score variables (clinical features) and 13 out of 59 available routine laboratory parameters. We assessed model performance according to a range of learning metrics, such as the mean area under the receiver operating characteristic curve [AUROC]. We also used the Shapley additive explanation algorithm to explain the prediction model. Results: Machine learning models using laboratory data achieved AUROCs of 0.71–0.82 in a split-by-year development/testing scheme. The non-linear eXtreme Gradient Boosting model yielded the highest prediction accuracy. In the held-out validation set of development cohort, the predictive model using comprehensive clinical and laboratory parameters outperformed those using clinical alone in predicting in-hospital mortality (AUROC [95% bootstrap confidence interval], 0.899 [0.897–0.901] vs. 0.875 [0.872–0.877]; P <0.001), with over 81% accuracy, sensitivity, and specificity. We observed similar performance in the testing set. Conclusions: Machine learning integrated with routine laboratory tests and EHRs could significantly promote the accuracy of inpatient ICH mortality prediction. This multidimensional composite prediction strategy might become an intelligent assistive prediction for ICH risk reclassification and offer an example for precision medicine.

Abstract:Through analysis of thermally stimulated polarization and depolarization currents (TSPC and TSDC), dielectric properties of polyethylene terephthalate (PET) and polyethylene naphthalate (PEN) are compared. Since the two polymers have very similar chemical structures, their dielectric properties are also similar. However, the temperature at which TSPC or TSDC starts to increase rapidly is about 20 °C higher in PEN than in PET. This difference is mostly due to the fact that the glass transition temperature Tg is higher in PEN. This means that thermal properties including the glass transition affect significantly the dielectric behavior of PET and PEN. At temperatures about 30 °C lower than Tg, the two polymers show a hump in their first-run TSPC spectra, probably originating from impurity or moisture. It is also clearly demonstrated that both PET and PEN can be reliable electrical insulating materials, especially at temperatures below their Tg's.
Development and Validation of an ICU-Venous Thromboembolism Prediction Model Using Machine Learning Approaches: A Multicenter Study

Abstract:Purpose: The purpose of this study was to establish and validate machine learning-based models for predicting the risk of venous thromboembolism (VTE) in intensive care unit (ICU) patients. Patients and methods: The clinical data of 1494 ICU patients who underwent Doppler ultrasonography or venography between December 2020 and March 2023 were extracted from three tertiary hospitals. The Boruta algorithm was used to screen the essential variables associated with VTE. Five machine learning algorithms were employed: Random Forest (RF), eXtreme Gradient Boosting (XGBoost), Support Vector Machine (SVM), Gradient Boosting Decision Tree (GBDT), and Logistic Regression (LR). Hyperparameter optimization was conducted on the predictive model of the training dataset. The performance in the validation dataset was measured using indicators, including the area under curve (AUC) of the receiver operating characteristic (ROC) curve, specificity, and F1 score. Finally, the optimal model was interpreted using the SHapley Additive exPlanation (SHAP) package. Results: The incidence of VTE among the ICU patients in this study was 26.04%. We screened 19 crucial features for the risk prediction model development. Among the five models, the RF model performed best, with an AUC of 0.788 (95% CI: 0.738-0.838), an accuracy of 0.759 (95% CI: 0.709-0.809), a sensitivity of 0.633, and a Brier score of 0.166. Conclusion: A machine learning-based model for prediction of VTE in ICU patients were successfully developed, which could assist clinical medical staff in identifying high-risk populations for VTE in the early stages so that prevention measures can be implemented to reduce the burden on the ICU patients.

Abstract:Background: Timely and accurate outcome prediction plays a critical role in guiding clinical decisions for hypertensive ischemic or hemorrhagic stroke patients admitted to the ICU. However, interpreting and translating the predictive models into clinical applications are as important as the prediction itself. This study aimed to develop an interpretable machine learning (IML) model that accurately predicts 28-day all-cause mortality in hypertensive ischemic or hemorrhagic stroke patients. Methods: A total of 4,274 hypertensive ischemic or hemorrhagic stroke patients admitted to the ICU in the USA from multicenter cohorts were included in this study to develop and validate the IML model. Five machine learning (ML) models were developed, including artificial neural network (ANN), gradient boosting machine (GBM), eXtreme Gradient Boosting (XGBoost), logistic regression (LR), and support vector machine (SVM), to predict mortality using the MIMIC-IV and eICU-CRD database in the USA. Feature selection was performed using the Least Absolute Shrinkage and Selection Operator (LASSO) algorithm. Model performance was evaluated based on the area under the curve (AUC), accuracy, positive predictive value (PPV), and negative predictive value (NPV). The ML model with the best predictive performance was selected for interpretability analysis. Finally, the SHapley Additive exPlanations (SHAP) method was employed to evaluate the risk of all-cause in-hospital mortality among hypertensive ischemic or hemorrhagic stroke patients admitted to the ICU. Results: The XGBoost model demonstrated the best predictive performance, with the AUC values of 0.822, 0.739, and 0.700 in the training, test, and external cohorts, respectively. The analysis of feature importance revealed that age, ethnicity, white blood cell (WBC), hyperlipidemia, mean corpuscular volume (MCV), glucose, pulse oximeter oxygen saturation (SpO 2 ), serum calcium, red blood cell distribution width (RDW), blood urea nitrogen (BUN), and bicarbonate were the 11 most important features. The SHAP plots were employed to interpret the XGBoost model. Conclusions: The XGBoost model accurately predicted 28-day all-cause in-hospital mortality among hypertensive ischemic or hemorrhagic stroke patients admitted to the ICU. The SHAP method can provide explicit explanations of personalized risk prediction, which can aid physicians in understanding the model.

BackgroundPulmonary embolism (PE) patients combined with heart failure (HF) have been reported to have a high short-term mortality. However, few studies have developed predictive tools of 30-day mortality for these patients in intensive care unit (ICU). This study aimed to construct and validate a machine learning (ML) model to predict 30-day mortality for PE patients combined with HF in ICU.MethodsWe enrolled patients with PE combined with HF in the Medical Information Mart for Intensive Care Database (MIMIC) and developed six ML models after feature selection. Further, eICU Collaborative Research Database (eICU-CRD) was utilized for external vali- dation. The area under curves (AUC), calibration curves, decision curve analysis (DCA), net reclassification improvement (NRI), and integrated discrimination improvement (IDI) were performed to evaluate the prediction performance. Shapley additive explanation (SHAP) was performed to enhance the interpretability of our models.ResultsA total of 472 PE patients combined with HF were included. We developed six ML models by the 13 selected features. After internal validation, the Support Vector Ma- chine (SVM) model performed best with an AUC of 0.835, a superior calibration degree, and a wider risk threshold (from 0% to 90%) for obtaining clinical benefit, which also outperformed traditional mortality risk evaluation systems,as evaluated by NRI and IDI. The SVM model was still reliable after external validation. SHAP was performed to explain the model. Moreover, an online application was developed for further clinical use.ConclusionThis study developed a potential tool for identify short-term mortality risk to guide clinical decision making for PE patients combined with HF in the ICU. The SHAP method also helped clinicians to better understand the model.

Abstract:To examine whether machine learning (ML) approach can be used to predict hematoma expansion in acute intracerebral hemorrhage (ICH) with accuracy and widespread applicability, we applied ML algorithms to multicenter clinical data and CT findings on admission. Patients with acute ICH from three hospitals (n = 351) and those from another hospital (n = 71) were retrospectively assigned to the development and validation cohorts, respectively. To develop ML predictive models, the k-nearest neighbors (k-NN) algorithm, logistic regression, support vector machines (SVMs), random forests, and XGBoost were applied to the patient data in the development cohort. The models were evaluated for their performance on the patient data in the validation cohort, which was compared with previous scoring methods, the BAT, BRAIN, and 9-point scores. The k-NN algorithm achieved the highest area under the receiver operating characteristic curve (AUC) of 0.790 among all ML models, and the sensitivity, specificity, and accuracy were 0.846, 0.733, and 0.775, respectively. The BRAIN score achieved the highest AUC of 0.676 among all previous scoring methods, which was lower than the k-NN algorithm (p = 0.016). We developed and validated ML predictive models of hematoma expansion in acute ICH. The models demonstrated good predictive ability, showing better performance than the previous scoring methods.
